Étude du suivi conjoint par un médecin spécialiste chez les adultes avec maladies chroniques suivis en première ligne

Contexte : Les médecins spécialistes peuvent participer aux soins ambulatoires des personnes atteintes de maladies chroniques (MCs) et comorbidité comme co-gestionnaire ou consultant selon qu’ils sont responsables ou non du suivi du patient. Il y a un manque d’évidences sur les déterminants et l’impact du type d’implication du médecin spécialiste, ainsi que sur la façon optimale de mesurer la comorbidité pour recueillir ces évidences. Objectifs : 1) déterminer chez les patients atteints de MCs les facteurs associés à la cogestion en spécialité, dont les caractéristiques des organisations de première ligne et la comorbidité; 2) évaluer si le type d’implication du spécialiste influence le recours à l’urgence; 3) identifier et critiquer les méthodes de sélection d’un indice de comorbidité pour la recherche sur l’implication des spécialistes dans le suivi des patients. Méthodologie : 709 adultes (65 +/- 11 ans) atteints de diabète, d’arthrite, de maladie pulmonaire obstructive chronique ou d’insuffisance cardiaque furent recrutés dans 33 cliniques de première ligne. Des enquêtes standardisées ont permis de mesurer les caractéristiques des patients (sociodémographiques, comorbidité et qualité de vie) et des cliniques (modèle, ressources). L’utilisation des services de spécialistes et de l’urgence fut mesurée avec une base de données médico-administratives. Des régressions logistiques multivariées furent utilisées pour modéliser les variables associées à la cogestion et comparer le recours à l’urgence selon le type d’implication du spécialiste. Une revue systématique des études sur l’utilisation des services de spécialistes, ainsi que des revues sur les indices de comorbidité fut réalisée pour identifier les méthodes de sélection d’un indice de comorbidité utilisées et recommandées. Résultats : Le tiers des sujets a utilisé les services de spécialistes, dont 62% pour de la cogestion. La cogestion était associée avec une augmentation de la gravité de la maladie, du niveau d’éducation et du revenu. La cogestion diminuait avec l’âge et la réception de soins dans les cliniques avec infirmière ayant un rôle innovateur. Le recours à l’urgence n’était pas influencé par l’implication du spécialiste, en tant que co-gestionnaire (OR ajusté = 1.06, 95%CI = 0.61-1.85) ou consultant (OR ajusté = 0.97, 95%CI = 0.63-1.50). Le nombre de comorbidités n’était pas associé avec la cogestion, ni l’impact du spécialiste sur le recours à l’urgence. Les revues systématiques ont révélé qu’il n’y avait pas standardisation des procédures recommandées pour sélectionner un indice de comorbidité, mais que 10 critères concernant principalement la justesse et l’applicabilité des instruments de mesure pouvaient être utilisés. Les études sur l’utilisation des services de spécialistes utilisent majoritairement l’indice de Charlson, mais n’en expliquent pas les raisons. Conclusion : L’implication du spécialiste dans le suivi des patients atteints de MCs et de comorbidité pourrait se faire essentiellement à titre de consultant plutôt que de co-gestionnaire. Les organisations avec infirmières ayant un rôle innovateur pourraient réduire le besoin pour la cogestion en spécialité. Une méthode structurée, basée sur des critères standardisés devrait être utilisée pour sélectionner l’indice de comorbidité le plus approprié en recherche sur les services de spécialistes. Evidences concerning determinants and impact of type of specialist involvement are currently lacking, in particular the influence of comorbidity and how best to measure this factor. Objectives: The objectives were 1) to determine clinical, patient and PHC organizational characteristics associated with type of specialist involvement in patients with CDs; 2) to assess whether type of specialist involvement is associated with emergency department (ED) use and; 3) to identify methods for selecting a comorbidity index for specialist services research. Methods: 709 adults (65 +/- 11 years) with diabetes, heart failure, arthritis, or chronic obstructive pulmonary disease were recruited in 33 PHC practices. Standardized surveys were used to measure patient (gender, age, education, income, comorbidity, quality of life) and practice characteristics (model, remuneration mode, resources, role of nurse). Information on specialist services and ED use was procured from the Quebec physician claims database. We used multivariate logistic regression to 1) model variables associated with being comanaged and 2) compare ED use among persons with different types of specialist involvement. We conducted two systematic reviews: 1) review articles on comorbidity indices to identify proposed selection procedures and 2) studies on specialist services utilization to identify selection processes actually used. Results: One third of our sample saw a specialist; the majority (62%) was as a comanager. Comanagement was associated with higher disease severity, younger age, higher education level and income and primary care management in practices without a nurse in advanced practice role. There was no difference in rates of ED use over one year between patients with or without specialist involvement, either as a comanager (adjusted OR = 1.06, 95%CI = 0.61-1.85) or as a consultant (adjusted OR = 0.97, 95%CI = 0.63-1.50). Quantity of comorbidity was not associated with either comanagement or impact of specialist involvement on ED use. Our systematic review revealed no standardized selection process of a comorbidity index. However, 10 distinct criteria related to accuracy and applicability of a measurement scale or validity of reported studies were compiled. Studies on specialist services utilization mostly used the Charlson comorbidity index, but none justified their choice. Conclusion: Specialist support in the management of patients with CDs and comorbidity should be provided on a consultant basis. The PHC practice model with a nurse in an advanced practice role may reduce the need for specialist comanagement. When adjusting for comorbidity, researchers should use a structured process to select the appropriate index based on standard criteria such as validity and applicability. The influence of primary care clinic characteristics on the quality of care for depression in patients with different comorbidity profiles

Dans les services de première ligne, la majorité des personnes atteintes de dépression souffrent également d’autres maladies chroniques comorbides. Offrir des soins de haute qualité à ces patients représente un défi important pour les intervenants en première ligne ainsi que pour le système de santé. Il y a des raisons de croire que les contextes organisationnels dans lesquels les intervenants pratiquent ont une influence importante sur les soins. Cependant, peu d’études ont examiné directement la façon dont les caractéristiques des cliniques facilitent ou entravent les soins offerts aux patients atteints de dépression et de différents types de maladies chroniques comorbides. L’objectif général de ce projet de recherche était donc de mieux comprendre comment différentes caractéristiques des cliniques de première ligne influencent la qualité des soins pour la dépression chez des patients ayant différents profils de comorbidité. La thèse comporte deux études. Tout d'abord, nous avons effectué une revue systématique examinant les relations entre la comorbidité physique chronique et la qualité des soins pour la dépression dans les services de première ligne afin de clarifier la nature de ces relations et d’identifier les facteurs qui pourraient influer sur ces relations. Ensuite, nous avons effectué une étude aux méthodes mixtes ayant deux volets : (a) un volet quantitatif examinant les relations entre la qualité des soins pour la dépression, les profils de comorbidité des patients, et les caractéristiques des cliniques de première ligne par le biais d’analyses de régression multiniveaux de données issues de deux enquêtes, et (b) un volet qualitatif basé sur une étude de cas explorant les perceptions des professionnels des services de première ligne sur les facteurs organisationnels pouvant influencer la qualité des soins offerts aux patients souffrant de dépression et d’autres maladies chroniques comorbides. Les résultats de ces études ont montré que, bien que la qualité des soins de la dépression en soins primaires soit sous-optimale, certains sous-groupes de patients dépressifs sont plus à risque que d’autres de recevoir des soins pour la dépression inadéquats, notamment des patients ayant uniquement des comorbidités chroniques physiques. Cependant, plusieurs caractéristiques des cliniques de première ligne semblent faciliter l’offre de soins de qualité aux patients atteints de dépression et de maladies chroniques comorbides : les normes et les valeurs liées au travail d'équipe et le soutien mutuel, l'accès au soutien des professionnels ayant une expertise en santé mentale, l’utilisation des algorithmes de traitement et d’autres outils d’aide à la décision pour la dépression, et l’absence d’obstacles liés aux modèles de rémunération inadéquats. Une des façons dont ces caractéristiques favorisent la qualité est en facilitant la circulation des connaissances dans les cliniques de première ligne. Nos résultats suggèrent que des interventions organisationnelles ciblées sont nécessaires pour améliorer la qualité des soins pour la dépression que reçoivent les patients ayant des maladies chroniques comorbides. Ces interventions peuvent viser différents domaines organisationnels (ex : caractéristiques structurelles/stratégiques, sociales, technologies et épistémiques) mais doivent aussi prendre en compte comment les éléments de chaque domaine interagissent et comment ils pourraient influencer les soins pour des patients ayant des profils de comorbidité différents. / In primary care settings, most patients presenting with major depression also suffer from comorbid chronic mental and/or physical conditions. Delivering high quality depression care to these patients represents an important challenge for primary care providers and for health care systems. There are reasons to believe that the organizational contexts in which providers practice have an important influence on the care they provide to patients with depression and comorbid chronic conditions. However, few studies have directly examined how primary care organizational characteristics facilitate or impede care for these patients. The main objective of this thesis was to better understand how different primary care organizational characteristics influence the quality of depression care provided to patients with different profiles of comorbid chronic conditions. The thesis has two components. First, we conducted a systematic review examining relationships between chronic physical comorbidity and the quality of depression recognition and treatment in primary care in order to clarify the nature of these relationships and identify factors associated with observed differences in these relationships across studies. Next, we carried out a mixed-methods study conducted in two sequential phases: (a) a quantitative phase examining relationships between depression care quality, patients’ comorbidity profiles, and primary care clinic characteristics using multilevel regression analyses of data collected from two inter-related surveys, and (b) a qualitative phase relying on a case study approach that examines primary care professionals’ perceptions of the organizational factors that influence the quality of care they provide to patients living with depression and other comorbid chronic diseases. Results of these studies revealed that while the quality of depression care in primary care is suboptimal, some patient subgroups are more at risk of receiving low quality of care than others. Specifically, depressed patients with chronic physical comorbidity only were more at risk of receiving suboptimal care than patients presenting only with psychiatric comorbidity. However, several characteristics of primary care organizations facilitated the delivery of high quality care to patients with depression and comorbid chronic conditions. These included norms and values related to teamwork and mutual support, access to supports from professionals with expertise in mental health, use of decision support tools for depression treatment, and a lack of barriers due to inadequate payment models. One of the ways these characteristics support quality is by facilitating the flow of knowledge in primary care organizations. Our findings suggest that targeted organizational interventions are needed to improve the quality of depression care delivered to patients with depression and comorbid chronic conditions. Interventions could target multiple organizational domains (e.g. structural/strategic, social, informational and epistemic characteristics) and should consider how elements in each domain interact and how they might impact patients with different comorbidity profiles.

Prevalência do transtorno dismórfico corporal em pacientes dermatológicos e avaliação da crítica sobre os sintomas nessa população / Prevalence of body dysmorphic disorder among dermatological patients and level of insight about their symptoms

São cada vez mais freqüentes as queixas cosméticas na sociedade contemporânea objetivando a perfeição das formas do corpo e da pele. Os dermatologistas e cirurgiões plásticos são frequentemente consultados para avaliar e tratar essas queixas. Sendo assim é importante conhecer o Transtorno Dismórfico Corporal, inicialmente chamado de dismorfofobia que foi pouco estudado até recentemente. Esse transtorno é relativamente comum, por vezes incapacitante, e envolve uma percepção distorcida da imagem corporal caracterizada pela preocupação exagerada com um defeito imaginário na aparência ou com um mínimo defeito corporal presente. A maioria dos pacientes apresenta algum grau de prejuízo no funcionamento social e ocupacional e como resultado de suas queixas obsessivas com a aparência podem desenvolver comportamentos compulsivos, em casos mais graves há risco de suicídio. O nível de juízo crítico é prejudicado, não reconhecem que seu defeito é mínimo ou inexistente e freqüentemente procuram tratamentos cosméticos para um transtorno psíquico. A prevalência do transtorno na população geral é de 1 a 2% e em pacientes dermatológicos e de cirurgia cosmética de 2,9 a 16%. Neste estudo investigou-se a prevalência do Transtorno Dismórfico Corporal em pacientes dermatológicos. Entrevistadores treinados avaliaram com questionários e entrevistas semi-estruturadas (SCID) pacientes que procuravam tratamentos cosméticos clínicos e cirúrgicos (grupo Cosmiatria, n=150), que procuravam a dermatologia em geral (grupo Geral, n=150) e grupo controle de 50 pacientes. Três psiquiatras independentes fizeram a melhor estimativa diagnóstica (best estimate diagnosis). Foram diagnosticados 32 pacientes (Cosmiatria 14%; Geral 6,7%; Controle 2%). As diferenças entre as prevalências nos três grupos foram significativas, bem como entre o grupo da Cosmiatria e o Controle. A regressão logística mostrou maior prevalência na Cosmiatria do que no grupo Geral e Controle, em indivíduos solteiros e com menor índice de massa corpórea. A gravidade foi moderada (em escala validada) e as obsessões foram mais significativas no grupo da Cosmiatria do que no Geral. A aplicação de escala de avaliação de crenças mostrou que o nível de juízo crítico estava mais prejudicado nos pacientes do grupo da Cosmiatria. Nenhum paciente havia sido diagnosticado previamente. As comorbidades psiquiátricas foram freqüentes, principalmente o Transtorno Depressivo Maior e o Transtorno Obsessivo Compulsivo. A comparação de subgrupos de pacientes que tinham o Transtorno Dismórfico Corporal ou este associado ao Transtorno Obsessivo-Compulsivo não mostrou diferenças significativas quanto às variáveis demográficas, comorbidades psiquiátricas, gravidade do transtorno ou nível de juízo crítico. As queixas dermatológicas mais freqüentes, em média duas, foram: discromias, acne, quanto à forma do corpo e ao envelhecimento. No grupo Cosmiatria a maioria dos pacientes já havia se submetido a tratamentos ou cirurgias cosméticas com resultados insatisfatórios. Os achados desse estudo apontam para uma maior prevalência em pacientes dermatológicos, principalmente nos que procuram tratamentos cosméticos, sugerindo que possam ser mais obsessivos e ter pior nível de juízo crítico em relação aos seus sintomas. Considerando a alta prevalência do Transtorno Dismórfico Corporal em pacientes dermatológicos e que os tratamentos cosméticos raramente melhoram seus sintomas, o treinamento dos profissionais para a investigação sistemática, diagnóstico e encaminhamento para tratamento psiquiátrico parece fundamental / Cosmetic concerns are increasingly pervading the contemporary societies, focusing on body shape and the skin perfection. Dermatologists and plastic surgeons are frequently consulted to evaluate and treat these concerns. Therefore it is important to be aware of the Body Dysmorphic Disorder, also known as dysmorphophobia, little studied until recently. This disorder is relatively common sometimes causing impairment involving a distorted perception of the body image characterized by an excessive preoccupation with an imagined or minimal appearance defect. Most of the patients experience some degree of impairment in social or occupational functioning and as a result, their obsessive thoughts, may lead to repetitive behaviors and in severe cases, to attempted suicide. Most individuals have poor insight and they do not acknowledge the defect is minimal or inexistent and seek out cosmetic treatments for a psychiatric disorder. The prevalence of this disorder among general population ranges from 1 to 2 % and in dermatological and cosmetic surgery patients ranges from 2, 9 to 16%. This study assessed the prevalence of Body Dysmorphic Disorder in dermatological patients. Trained interviewers used questionnaires (BDDQ) and semi-structured clinical interviews (SCID) to access patients seeking clinical or surgical cosmetic treatments (Cosmetic group, n=150), seeking dermatology in general (General group, n=150) and a control group of 50 subjects. Three independent psychiatrists assigned the best estimate diagnosis and 32 patients were diagnosed with the disorder (Cosmetic 14%, General 6, 7% and Control 2%). Prevalence differences in the three groups were significant, also for Cosmetic and Control groups. In the logistic regression a higher prevalence has been noticed in the Cosmetic group (when compared with General and Control groups), in patients that are not currently married and with less body mass index. The severity of the symptoms assessed by a validated scale (BDD-YBOCS) was moderate and the obsessions were significantly higher in the Cosmetic group as compared to the General one. Beliefs were assessed by a scale (BABS) and insight were significantly poorer in the Cosmetic group. None of the patients had been previously diagnosed. Psychiatric co-morbidities were frequent, mostly Major Depressive Disorder and Obsessive-Compulsive Disorder. Comparing two patients subgroups with just Body Dysmorphic Disorder and patients with Obsessive-Compulsive Disorder as comorbidity show no differences according to demographic variables, Axis I psychiatric comorbidity, symptoms severity and level of insight. The most frequent dermatological concerns were dyschromias, acne, the shape of the body and ageing. In the Cosmetical group most of the patients have performed clinical or surgical cosmetic treatments with poor results. Our findings provide further support for higher prevalence of Body Dysmorphic Disorder in dermatological patients, mainly those seeking cosmetic treatments and suggests that those patients might be more obsessive and having poorer insight into their symptoms. Taking into account the high prevalence of Body Dysmorphic Disorder in dermatological patients and cosmetic treatments that rarely improve symptoms there are compelling reasons for training professionals to systematically investigate, diagnose and refer these patients to adequate psychiatric treatment

Sendo assim é importante conhecer o Transtorno Dismórfico Corporal, inicialmente chamado de dismorfofobia que foi pouco estudado até recentemente. Esse transtorno é relativamente comum, por vezes incapacitante, e envolve uma percepção distorcida da imagem corporal caracterizada pela preocupação exagerada com um defeito imaginário na aparência ou com um mínimo defeito corporal presente. A maioria dos pacientes apresenta algum grau de prejuízo no funcionamento social e ocupacional e como resultado de suas queixas obsessivas com a aparência podem desenvolver comportamentos compulsivos, em casos mais graves há risco de suicídio. O nível de juízo crítico é prejudicado, não reconhecem que seu defeito é mínimo ou inexistente e freqüentemente procuram tratamentos cosméticos para um transtorno psíquico. A prevalência do transtorno na população geral é de 1 a 2% e em pacientes dermatológicos e de cirurgia cosmética de 2,9 a 16%. Neste estudo investigou-se a prevalência do Transtorno Dismórfico Corporal em pacientes dermatológicos. Entrevistadores treinados avaliaram com questionários e entrevistas semi-estruturadas (SCID) pacientes que procuravam tratamentos cosméticos clínicos e cirúrgicos (grupo Cosmiatria, n=150), que procuravam a dermatologia em geral (grupo Geral, n=150) e grupo controle de 50 pacientes. Três psiquiatras independentes fizeram a melhor estimativa diagnóstica (best estimate diagnosis). Foram diagnosticados 32 pacientes (Cosmiatria 14%; Geral 6,7%; Controle 2%). As diferenças entre as prevalências nos três grupos foram significativas, bem como entre o grupo da Cosmiatria e o Controle. A regressão logística mostrou maior prevalência na Cosmiatria do que no grupo Geral e Controle, em indivíduos solteiros e com menor índice de massa corpórea. A gravidade foi moderada (em escala validada) e as obsessões foram mais significativas no grupo da Cosmiatria do que no Geral. A aplicação de escala de avaliação de crenças mostrou que o nível de juízo crítico estava mais prejudicado nos pacientes do grupo da Cosmiatria. Nenhum paciente havia sido diagnosticado previamente. As comorbidades psiquiátricas foram freqüentes, principalmente o Transtorno Depressivo Maior e o Transtorno Obsessivo Compulsivo. A comparação de subgrupos de pacientes que tinham o Transtorno Dismórfico Corporal ou este associado ao Transtorno Obsessivo-Compulsivo não mostrou diferenças significativas quanto às variáveis demográficas, comorbidades psiquiátricas, gravidade do transtorno ou nível de juízo crítico. As queixas dermatológicas mais freqüentes, em média duas, foram: discromias, acne, quanto à forma do corpo e ao envelhecimento. No grupo Cosmiatria a maioria dos pacientes já havia se submetido a tratamentos ou cirurgias cosméticas com resultados insatisfatórios. Os achados desse estudo apontam para uma maior prevalência em pacientes dermatológicos, principalmente nos que procuram tratamentos cosméticos, sugerindo que possam ser mais obsessivos e ter pior nível de juízo crítico em relação aos seus sintomas. Considerando a alta prevalência do Transtorno Dismórfico Corporal em pacientes dermatológicos e que os tratamentos cosméticos raramente melhoram seus sintomas, o treinamento dos profissionais para a investigação sistemática, diagnóstico e encaminhamento para tratamento psiquiátrico parece fundamental / Cosmetic concerns are increasingly pervading the contemporary societies, focusing on body shape and the skin perfection. Dermatologists and plastic surgeons are frequently consulted to evaluate and treat these concerns. Therefore it is important to be aware of the Body Dysmorphic Disorder, also known as dysmorphophobia, little studied until recently. This disorder is relatively common sometimes causing impairment involving a distorted perception of the body image characterized by an excessive preoccupation with an imagined or minimal appearance defect. Most of the patients experience some degree of impairment in social or occupational functioning and as a result, their obsessive thoughts, may lead to repetitive behaviors and in severe cases, to attempted suicide. Most individuals have poor insight and they do not acknowledge the defect is minimal or inexistent and seek out cosmetic treatments for a psychiatric disorder. The prevalence of this disorder among general population ranges from 1 to 2 % and in dermatological and cosmetic surgery patients ranges from 2, 9 to 16%. This study assessed the prevalence of Body Dysmorphic Disorder in dermatological patients. Trained interviewers used questionnaires (BDDQ) and semi-structured clinical interviews (SCID) to access patients seeking clinical or surgical cosmetic treatments (Cosmetic group, n=150), seeking dermatology in general (General group, n=150) and a control group of 50 subjects. Three independent psychiatrists assigned the best estimate diagnosis and 32 patients were diagnosed with the disorder (Cosmetic 14%, General 6, 7% and Control 2%). Prevalence differences in the three groups were significant, also for Cosmetic and Control groups. In the logistic regression a higher prevalence has been noticed in the Cosmetic group (when compared with General and Control groups), in patients that are not currently married and with less body mass index. The severity of the symptoms assessed by a validated scale (BDD-YBOCS) was moderate and the obsessions were significantly higher in the Cosmetic group as compared to the General one. Beliefs were assessed by a scale (BABS) and insight were significantly poorer in the Cosmetic group. None of the patients had been previously diagnosed. Psychiatric co-morbidities were frequent, mostly Major Depressive Disorder and Obsessive-Compulsive Disorder. Comparing two patients subgroups with just Body Dysmorphic Disorder and patients with Obsessive-Compulsive Disorder as comorbidity show no differences according to demographic variables, Axis I psychiatric comorbidity, symptoms severity and level of insight. The most frequent dermatological concerns were dyschromias, acne, the shape of the body and ageing. In the Cosmetical group most of the patients have performed clinical or surgical cosmetic treatments with poor results. Our findings provide further support for higher prevalence of Body Dysmorphic Disorder in dermatological patients, mainly those seeking cosmetic treatments and suggests that those patients might be more obsessive and having poorer insight into their symptoms. Taking into account the high prevalence of Body Dysmorphic Disorder in dermatological patients and cosmetic treatments that rarely improve symptoms there are compelling reasons for training professionals to systematically investigate, diagnose and refer these patients to adequate psychiatric treatment

<p>УВОД: Diabetes mellitus (DM) је водећа глобална епидемија 21. века, сложена болест коју карактерише поремећај метаболизма и хронична хипергликемија, која доводи до развоја микроваскуларних и макроваскуларних компликација. Повишене вредности гликемије у ДМT2 доводе до поремећаја ћелија крви и њихових параметара. Истраживања су показала да хема-толошки параметри имају допринос у настанку оштећења васкуларног ендотела и учествују у развоју дегенеративних промена и путем других механизама код пацијената са дијабетесом. Циљ истраживања је да се утврде могуће промене параметара комплетне крвне слике (ККС) у зависности од гликорегулације, дужине трајања болести и њихова повезаност са микро и макро ангиопатским комликацијама код пацијената са шећерном болести тип 2 (ДМT2). МЕТОДЕ: Студија је била проспективна у трајању од једне године, од 2016. до 2017. године. Истраживањем је обухваћено укупно 137 испитаника, од којих 90 болује од ДМТ2, а 47 је здравих, оба пола, старијих од 40 година, спроведена у Дому здравља &ldquo;Др Милорад Мика Павловић&rdquo;, Инђија, Србија. Да би се утврдила могућа корелација између параметара ККС, гликорегулације у ДМT2 и дегенеративних компликација, испитаници су подељени на више начина: на групу оболелих од ДМT2 и групу здравих; групе оболелих са вредностима HbA1c&le;7% и оне са вредностима HbA1c &gt;7%, као и на групе са и без дегенеративних компликација. Коришћењем стандардних биохемијских поступака анализирани су параметри ККС, параметри гликорегулације, липидни статус, а вршена су и антропометријска мерења. Подаци од пацијената прикупљени су путем упитника и електронског картона пацијента. РЕЗУЛТАТИ: Утврђена је статистички значајна разлика између група са и без шећерне болести за WBC, еозинофилне гранулоците, хемоглобин, MCH, MCHC, SE у 2016. год, неутрофилне гранулоците, моноците, RDW, PDW, SE у 2017. год. У групи чија је дужина трајања ДМT2 већа од 6 година утврђене су повишене вредности MCHC и PDW. У односу на гликорегулацију уочене су значајне разлике у PMDW, великим тромбоцитима и RDW у групи са HbA1c&gt;7%. У односу на дегенеративне микроваскуларне компликације постоје значајне разлике у броју лимфоцита и неутрофилних гранулоцита, а за макроваскуларне компликације у вредностима PDW-а. Istraživanja su pokazala da hema-tološki parametri imaju doprinos u nastanku oštećenja vaskularnog endotela i učestvuju u razvoju degenerativnih promena i putem drugih mehanizama kod pacijenata sa dijabetesom. Cilj istraživanja je da se utvrde moguće promene parametara kompletne krvne slike (KKS) u zavisnosti od glikoregulacije, dužine trajanja bolesti i njihova povezanost sa mikro i makro angiopatskim komlikacijama kod pacijenata sa šećernom bolesti tip 2 (DMT2). METODE: Studija je bila prospektivna u trajanju od jedne godine, od 2016. do 2017. godine. Istraživanjem je obuhvaćeno ukupno 137 ispitanika, od kojih 90 boluje od DMT2, a 47 je zdravih, oba pola, starijih od 40 godina, sprovedena u Domu zdravlja &ldquo;Dr Milorad Mika Pavlović&rdquo;, Inđija, Srbija. Da bi se utvrdila moguća korelacija između parametara KKS, glikoregulacije u DMT2 i degenerativnih komplikacija, ispitanici su podeljeni na više načina: na grupu obolelih od DMT2 i grupu zdravih; grupe obolelih sa vrednostima HbA1c&le;7% i one sa vrednostima HbA1c &gt;7%, kao i na grupe sa i bez degenerativnih komplikacija. Korišćenjem standardnih biohemijskih postupaka analizirani su parametri KKS, parametri glikoregulacije, lipidni status, a vršena su i antropometrijska merenja. Podaci od pacijenata prikupljeni su putem upitnika i elektronskog kartona pacijenta. REZULTATI: Utvrđena je statistički značajna razlika između grupa sa i bez šećerne bolesti za WBC, eozinofilne granulocite, hemoglobin, MCH, MCHC, SE u 2016. god, neutrofilne granulocite, monocite, RDW, PDW, SE u 2017. god. U grupi čija je dužina trajanja DMT2 veća od 6 godina utvrđene su povišene vrednosti MCHC i PDW. U odnosu na glikoregulaciju uočene su značajne razlike u PMDW, velikim trombocitima i RDW u grupi sa HbA1c&gt;7%. U odnosu na degenerativne mikrovaskularne komplikacije postoje značajne razlike u broju limfocita i neutrofilnih granulocita, a za makrovaskularne komplikacije u vrednostima PDW-a. ZAKLJUČAK: Na osnovu dobijenih rezultata našeg istraživanja može se zaključiti da postoji povezanost između pojedinih hematoloških parametara i glikoregulacije, oboljevanja od šećerne bolesti kao i povezanost sa komplikacijama kod pacijenata sa DMT2.</p> / <p>BACKGROUND: Diabetes mellitus (DM) is the leading global epidemic of the 21st century,a complex disease characterized by metabolism disorders and chronic hyperglycaemia, that leads to the development of microvascular and macrovascular complications. Elevated blood glucose level in T2DM lead to disturbance of blood cells and its parameters. Previous studies have reported that haematological parameters have contributed to the development of vascular endothelial damage and are involved in the development of degenerative changes through other mechanisms in patients with diabetes. The aim of the research is to determine possible changes in the complete blood count (CBC) parameters depending on glycemic control, the duration of the disease and their association with micro and macroangiopathic complications in patients with Type 2 diabetes mellitus (T2DM). METHODS: The study was prospective from 2016. to 2017. year. The study included a total of 137 subjects, 90 with T2DM and 47 healthy, of both gender over the age of 40 years, from the Health Care Center &quot;Dr Milorad Mika Pavlović&quot; Indjija, Serbia. The subjects were divided into several ways, in order to notice the possible correlation between the CBC parameters and glucose control in T2DM, with and without T2DM, two groups with HbA1c&le;7% and with HbA1c&gt;7%, and with and without complications. We analysed CBC parameters, parameters of glycoregulation, lipid status using standard biochemical methods, performed anthropometric measurements and collected patients data by questionnaire and electronic patient card. RESULTS: There were statistical difference between group with T2DM and healthy subjects for WBC, eos, Hgb, MCH, MCHC, ESR in 2016. and neutro, mono, RDW, PDW, ESR in 2017. In the group of T2DM patients with duration of disease longer than 6 years we found elevated value of MCHC, PDW. In relationship to glycoregulation, significant differences in PMDW, large platelets and RDW were found in the group HbA1c&gt;7%. According to degenerative complications significant differences were revealed in lympho, neutro in the group with microvascular complication, and PDW in the group with macrovascular complications. CONCLUSION: Based on the resuluts of our research, it can be concluded that there is an association between particular haematological parameters and glycoregulation, diabetes mellitus, as well as relationship with degenerative complications in patients with T2DM.</p>

L’objectif principal du présent mémoire réside dans l’exploration des liens concomitants existant entre les trois dimensions de l’épuisement professionnel telles que définies par Maslach (1981) et la consommation de substances psychoactives, plus précisément au niveau de la consommation épisodique excessive et hebdomadaire à risque d’alcool et de la consommation de médicaments psychotropes. À partir d’un échantillon composé de 1809 travailleurs provenant de 83 milieux de travail québécois, des profils-types correspondant à des formes particulières de comorbidité de santé mentale au travail sont identifiés grâce à la méthode d’analyse en classes latentes. Ainsi quatre profils-types sont dégagés: un premier regroupant les individus dits «sains», dont les scores aux différentes dimensions de l’épuisement professionnel sont faibles et dont la consommation de substances psychoactives est modérée; deux autres correspondant à des formes intermédiaires de risques; et un quatrième rassemblant des travailleurs dits «fragiles» dont les scores pour chacune des dimensions de l’épuisement professionnel se situent dans le quintile le plus élevé et dont les probabilités de consommation de substances psychoactives sont grandes. De plus, cette recherche s’est penchée sur l’identification de facteurs de risque et de protection associés à chacun des profils-types. À cet effet, les résultats des analyses corroborent la plupart des associations retrouvées au sein de la littérature quant aux facteurs du travail (composantes des modèles du stress professionnel de Karasek et Theorell (1990) ainsi que de Siegrist (1990)), hors travail (statut matrimonial, obligations parentales, revenu du ménage) et certaines caractéristiques individuelles (âge et genre). De faibles récompenses et un fort degré de surinvestissement de la part de l’individu se révèlent être des facteurs de risque particulièrement significatifs pour les formes intermédiaires et à risque de comorbidité de la santé mentale au travail. Dans une moindre mesure, une faible utilisation des compétences, des demandes psychologiques élevées, un soutien social inadéquat et le jeune âge expliquent une part de la variation observée entre les différents profils-types. Enfin, les résultats soutiennent une conceptualisation tridimensionnelle de l’épuisement professionnel. / The main objective of this master is to explore the concurrent associations between two phenomena related to stress, burnout and the use of psychoactive substance. Particularly, this research focuses on the three dimensions of burnout as defined by Maslach (1981), on two hazardous alcohol patterns consumption, binge drinking and weekly alcohol abuse, and on psychotropic drugs utilization. From a sample of 1809 workers issued from 83 workplaces in Quebec, typical profiles corresponding to different forms of comorbidity are identified with the use of latent class analysis. Four typical profiles were identified. The first one referred to “healthy” individuals who presented the lowest scores for the three dimensions of burnout without misuse of any psychoactive substances. Two more groups corresponded to intermediate health risk forms of comorbidity patterns. The fourth one was composed of “fragile” workers, with the highest burnout scores and the highest probabilities for misusing alcohol and psychotropic drugs. In addition, this research identified some risk and protective factors associated with each profiles. Multivariate analyses results corroborated most associations found in the occupational stress literature on organizational work conditions (model components of occupational stress developed by Karasek and Theorell (1990) and Siegrist (1996)), non-work determinants (marital status, parental status and household income), and on individual characteristics (age and gender). Low rewards and high overcommitment appeared as particularly significant risk factors for the intermediate and severe forms of comorbidity. Low skills utilization, low social support at work, high psychological demands and a young age explained to a lesser extent part of the variation observed between the comorbidity profiles. Results supported a three-dimensional conceptualization of burnout.
