Abstract The purpose of military service is to have enough manpower to form military and in order to maintain national security. Thus, the military service system has direct influence on the quality of military and has direct impact on the result of the conflict and survival of nation. History has told us the importance of military establishment, but most people still emphasis on ¡§force employment¡¨ and ignore military establishment, In recent years, when we talked about defense policy reform, you will always hear abolish conscript service and start volunteer service; most of disagreements think conscript is not suitable for the current condition in Taiwan, at the same time that volunteer force will bring us more professional military, and mobilization will only need during war time. The purpose of this research paper is to analysis the possibility of adopt full volunteer service, through the study of others¡¦ experience, lesson learned and would trend. Through the research discovered that volunteer service is the modern trend, and at current time period we can not suddenly switch to full volunteer, it should be divided into different phases and progressively transform from conscript to volunteer service, and reaching the final goal of full volunteer force and professional military establishment. Respiratory infections and cold exposure in asthmatic and healthy military conscripts

Abstract The purpose was to study respiratory infections in a cold environment among young Finnish men. The seasonal variation in the occurrence of respiratory tract infections is well-known, but the impact of cold exposure is obscure. The burden of respiratory tract infections is especially apparent during military service, but the possible risk factors for infections are not. A total of 892 young military conscripts, 224 men with physician-diagnosed asthma, from the intake groups of July 2004 and January 2005 in Kainuu Brigade, were recruited for the study. In Kajaani area, the average daily temperature is above 10°C only from June to August and all conscripts serve during the cold season, too. The previous history of respiratory tract symptoms, infections, smoking habits and cold sensations were obtained with a questionnaire. Blood samples were taken for determination of the markers of inflammation and infection and peak expiratory flow, height and weight were measured. Data on respiratory tract infections requiring a physician consultation and results of a 12-min running test were collected. The temperature data was obtained from the nearest meteorological station located ca. 15 km from the garrison. At the beginning of the service, asthmatic men reported to have experienced more respiratory tract symptoms and were in poorer physical condition according to the 12-min running test compared to non-asthmatic men. However, 48% of men with asthma were without medication. After the 180–362 -day service, both men with and without asthma had enhanced their physical fitness as determined with the 12-min running test. At the same time, the levels of high sensitive C-reactive protein as a marker of low-grade inflammation, decreased. Infection episodes requiring physician consultation were more common among men with, rather than without, asthma. Chlamydia pneumoniae infections were mostly mild upper respiratory tract infections, common cold and sinusitis, and were as common in asthmatic as in non-asthmatic men. However, prolonged Chlamydia pneumoniae infections were more common among asthmatic men. Obesity and previous respiratory tract infections were independent risk factors for frequent infections among men with 180-day service. There was a typical seasonal variation in respiratory tract infections among conscripts: most infections occurred in the wintertime. The men with 180-day service had most infections during the first three months of the service, both in the July and January intake groups. Temperature was significantly associated with the occurrence of respiratory infection episodes. The most common temperature for the onset of an episode was in the outdoor temperature range of 0°C to –5°C. Respiratory tract infections were preceded by linearly decreasing outdoor temperature, the coldest day being the day before physician consultation.

Oropharyngeal carriage of respiratory bacteria among military conscripts

Abstract The aims of this work were to study the carriage of respiratory bacteria and to identify risk factors affecting pharyngeal colonisation by these pathogens among young Finnish men during military service, and also to investigate the role of mannose-binding lectin (MBL) concentrations and MBL2 gene polymorphisms in the carriage of respiratory bacteria. A total of 892 military recruits entering the Kainuu Brigade, including 224 men with asthma, were followed up prospectively to the end of their military service. Carriage of Streptococcus pneumoniae, Neisseria meningitidis and beta-haemolytic streptococci appeared to be higher during and at the end of military service than at the beginning. Smoking was found to be a significant risk factor for colonisation by these bacteria. S.pneumoniae was more common in the asthmatic than military conscripts in the non-asthmatic ones at the beginning of military service. A low MBL level increased the risk of carrying N. meningitidis and beta-haemolytic streptococci during military service among non-smokers but not among smokers. Low MBL levels producing MBL2 haplotypes seemed to be associated with the carriage of N. meningitidis and S. pneumoniae. Characterisation of all the oropharyngeal N.meningitidis isolates obtained (n=215) by phenotypic and genotypic methods showed that most of them belonged to the carriage-associated ST-60 clonal complex. Clonal complexes ST-41/44, ST-32, and ST-23, which have previously been associated with disease, also accounted for a third of the carriage strains. Furthermore, a significant association was indicated between an acute upper respiratory infection and oropharyngeal carriage of the virulent meningococcal ST-23 clone. In conclusion, the results reported here show a significant increase in bacterial carriage during military service and provide new information on the association between MBL and carriage of respiratory bacteria. These findings also highlight the importance of smoking cessation, especially among military conscripts, who have been found to be a risk group for invasive bacterial diseases, and they also point to the importance of meningococcal vaccination for military recruits and the need for an efficacious vaccine against serogroup B meningococci. / Tiivistelmä Hengitystieinfektiot ovat yleisiä varusmiespalvelun aikana. Myös oireeton bakteerien nielukantajuus on lisääntynyt. Useimmiten infektiot ovat lieviä virusinfektioita, mutta bakteerien nielukantajuus voi johtaa myös vaikeisiin bakteeritulehduksiin. Tämän väitöskirjatyön tarkoituksena oli tutkia bakteerien nielukantajuutta varusmiespalveluksen alkaessa ja päättyessä sekä mahdollisten hengitystieinfektioiden aikana ja näin saada uutta tietoa bakteerien nielukantajuuteen vaikuttavista tekijöistä. Lisäksi tavoitteena oli selvittää mannoosia sitovan lektiinin (MBL) sekä MBL2-geenin polymorfismien yhteyttä bakteerien nielukantajuuteen. Työn tarkoituksena oli myös feno- ja genotyypittää varusmiehiltä palveluksen aikana eristetyt meningokokkikannat ja verrata niitä vastaavana ajankohtana invasiivista tautia sairastaneista henkilöistä eristettyihin meningokokkikantoihin. Tutkimuksessa seurattiin prospektiivisesti 892 varusmiestä, jotka suorittivat asepalveluksen Kainuun Prikaatissa vuosina 2004–2006. Tutkimukseen osallistuneista varusmiehistä 224:llä oli astma. Tutkimuksessa havaittiin, että oireeton bakteerien nielukantajuus lisääntyy merkitsevästi varusmiespalveluksen aikana. Lisäksi havaittiin, että tupakointi oli merkittävä itsenäinen riskitekijä pneumokokin, meningokokin sekä beta-hemolyyttisten streptokokkien nielukantajuudelle varusmiespalveluksen aikana. Astmaatikkojen pneumokokin nielukantajuus varusmiespalveluksen alussa oli yleisempi kuin terveiden varusmiesten. Tutkimuksessa osoitettiin myös pienen seerumin MBL-pitoisuuden sekä MBL2-geenin polymorfismin eksoni 1:n alueella ja geenin säätelyalueella olevan riskitekijöitä meningokokin, pneumokokin sekä beta-hemolyyttisten streptokokkien nielukantajuudelle tupakoimattomilla varusmiehillä. Meningokokin nielukannoista jopa kolmasosa kuului genotyyppiryhmään, jonka on aiemmissa tutkimuksissa havaittu liittyvän invasiiviseen tautiin. Tutkimuksessa osoitettiin myös tietyn hyperinvasiivisen meningokokin genotyypin (ST-23) liittyvän hengitystieinfektioepisodeihin. Tässä väitöskirjatyössä osoitettiin, että bakteerien nielukantajuus lisääntyy merkitsevästi varusmiespalveluksen aikana ja että oireettomilla varusmiehillä tavataan myös hyperinvasiivisia meningokokkikantoja. Tutkimus antoi myös uutta tietoa hyperinvasiivisten meningokokin genotyyppien liittymisestä hengitystieinfektioihin sekä MBL:n vaikutuksesta bakteerien nielukantajuuteen. Tutkimushavainnot tukevat tupakoimattomuuden edistämisen tärkeyttä myös varusmiespalveluksen aikana.

"They Are More Afraid of Losing Women Than of Having Women" : How the structural transformation of the Swedish Armed Forces has affected the experience for female recruits

When interviewing female conscripts, Emma Lindgren Lauritzon (2012) found that they perceived their basic military training in the Swedish Armed Forces as a negative experience. The women felt that the environment was hostile and that they served on the conditions of men. This is especially alarming since the unequal conscription was replaced by voluntary service for all in 2010, making the Swedish defence dependent on attracting the whole population to fill up its necessary positions. Therefore, this thesis examines how female recruits experience the new basic military training and if the structural transformation, with a more gender equal entrance, has had an impact on the individual level. Eight semi-structured interviews were conducted, designed and presented by a deductive thematic analysis, and later interpreted through feministic and gender organizational theories. The results indicate a positive development in their experiences, and more so – a new development of the women feeling especially wanted and needed. This perceived dependency on women is arguably in the risk of backfiring, generating guilt if they disappoint the organisation. This study gives vital insight to the organisation in question, the Swedish Armed Forces, and also several new and interesting aspects to build future research on regarding women in the military.

Susceptibility to respiratory tract infections in young men: the role of inflammation, mannose-binding lectin, interleukin-6 and their genetic polymorphisms

Abstract Respiratory tract infections are the most common acute illnesses, and innate immunity and inflammation are important in defence against these infections. Mannose-binding lectin (MBL) mediates innate immune defences by recognising microbial structures. MBL deficiency caused by polymorphisms in the MBL2 gene has been associated with susceptibility to recurrent infections. Interleukin-6 (IL-6) is a mediator of inflammatory response. Polymorphisms in the IL-6 and IL-6 receptor (IL-6R) genes have been previously associated mainly with metabolic disorders and cardiovascular diseases. Chlamydia pneumoniae is a common pathogen in acute respiratory tract infections, but it also has a tendency to cause persistent infections, which have been associated with cardiovascular diseases and its risk factors, such as obesity. The aims of this study were to investigate if selected polymorphisms of the MBL2, IL-6 and IL-6R genes are associated with respiratory tract infections and markers of C. pneumoniae infection, and to study if persistent C. pneumoniae infection is connected with an elevated body mass index (BMI) in 893 Finnish male military conscripts. Respiratory tract infections were followed during their military service and serum samples were collected at the beginning and end of their service and during each infectious episode. A variation in serum MBL levels between different MBL2 genotypes and a MBL deficiency in homozygous exon 1 variant genotypes (at codons 52, 54 and 57) were observed. Low MBL levels and MBL2 polymorphisms in exon 1 and promoter region were found to be risk factors for susceptibility to respiratory tract infections as well as for positivity and a rise in C. pneumoniae antibodies during military service. Associations between IL-6R gene polymorphisms in the promoter region (-183G/A) and in intron 1 and respiratory tract infections were found. In addition, the IL-6 -174G/C polymorphism was associated with persistently elevated C. pneumoniae antibodies and with slightly elevated serum C-reactive protein (CRP) levels, pointing to chronic C. pneumoniae infection. Furthermore, persistent C. pneumoniae antibodies as a suggestive marker of chronic infection, and elevated serum CRP levels as a marker of systemic inflammation, were associated with an elevated BMI. In conclusion, the findings support the role for MBL in susceptibility to infections and provide new information about the association between MBL and common respiratory tract infections. The results also suggest that the 5’ area of the IL-6R gene may be a possible candidate region for respiratory tract infection susceptibility, and that IL-6 genetics may be associated with C. pneumoniae infection. The study also provides new information about the role of possible chronic C. pneumoniae infection in obesity. / Tiivistelmä Hengitystieinfektiot ovat yleisimpiä äkillisiä sairauksia, ja synnynnäisellä immuunivasteella ja tulehduksella on tärkeä rooli puolustuksessa näitä infektioita vastaan. Synnynnäiseen immuniteettiin kuuluva mannoosia sitova lektiini (MBL) tunnistaa infektioita aiheuttavien mikrobien rakenteita. MBL2-geenin polymorfismien aiheuttaman MBL-proteiinin puutteen on todettu altistavan toistuville infektioille. Interleukiini-6 (IL-6) on tulehduksen välittäjänä toimiva sytokiini. IL-6- ja IL-6-reseptori (IL-6R) -geenien polymorfismit on aikaisemmin yhdistetty lähinnä metabolisiin häiriöihin sekä sydän- ja verisuonitauteihin. Chlamydia pneumoniae eli keuhkoklamydia on yleinen hengitystieinfektioiden aiheuttaja, mutta se voi myös aiheuttaa kroonisia infektioita, jotka on yhdistetty sydän- ja verisuonitauteihin sekä niiden riskitekijöihin kuten lihavuuteen. Työn tarkoituksena oli tutkia tiettyjen MBL2-, IL-6- ja IL-6R-geenien polymorfismien yhteyttä hengitystieinfektiohin ja keuhkoklamydiavasta-ainetasoihin sekä keuhkoklamydiainfektion yhteyttä painoindeksiin 893 suomalaisella varusmiehellä. Hengitystieinfektioita seurattiin palveluksen aikana, ja seeruminäytteet kerättiin palveluksen alussa, lopussa ja jokaisen infektion aikana. Tutkimuksessa havaittiin vaihtelua seerumin MBL-pitoisuudessa eri MBL2-genotyyppien välillä sekä MBL:n puute homotsygooteissa eksoni 1 -alueen varianttigenotyypeissä (kodoneissa 52, 54 ja 57). Alhaiset MBL-tasot sekä MBL2-geenin polymorfismit eksoni 1 -alueella ja säätelyalueella olivat riskitekijöitä hengitystieinfektioalttiudelle sekä keuhkoklamydiavasta-aineiden esiintymiselle ja vasta-aineiden nousulle palveluksen aikana. IL-6R-geenin polymorfismit säätelyalueella (-183G/A) ja introni 1 -alueella liittyivät hengitystieinfektioihin. Lisäksi IL-6-geenin -174G/C polymorfismi oli yhteydessä jatkuvasti kohonneisiin keuhkoklamydiavasta-aineisiin sekä seerumin C-reaktiivisen proteiinin (CRP) tasoihin, jotka mahdollisesti osoittaisivat kroonista keuhkoklamydiainfektiota. Lisäksi krooniseen keuhkoklamydia-infektioon viittaavat vasta-ainetasot sekä tulehdukseen liittyvä kohonnut CRP-pitoisuus olivat yhteydessä ylipainoon. Tutkimuksen tulokset tukevat aikaisemmin havaittua MBL:n vaikutusta infektioalttiuteen ja lisäksi antavat uutta tietoa MBL:n yhteydestä tavallisiin hengitystieinfektioihin. Tulokset viittaavat myös siihen, että IL-6R-geenin 5’-alueella voi olla yhteyttä hengitystieinfektioalttiuteen ja että IL-6-polymorfismi olisi yhteydessä keuhkoklamydiainfektioon. Tutkimus antaa myös uutta tietoa mahdollisen kroonisen keuhkoklamydiainfektion liittymisestä ylipainoon.

En gradvis förändring : En förändring av yrkesofficersprofessionen genom införandet av 15 månaders värnpliktiga

Sedan början av 2000-talet har Försvarsmakten genomfört en mängd förändringar med sin organisation och personal. Nedskärningarna av förband tillsammans med ett ökat fokus på internationella insatser i början av 00-talet har under slutet av 10-talet förändrats till en tillväxt av förband samt en återgång till ett nationellt fokus. Yrkesofficerarnas profession har förändrats genom införandet av ett tvåbefälssystem under slutet av 00-talet från ett enbefälssystem, likväl som att värnplikten lades vilande 2010 och anställda soldater introducerades i organisationen. Sedan 2018 är värnplikten återinförd med utbildning av soldater och gruppbefäl, 9 respektive 11 månader. Från och med sommaren 2021 återinförs 15 månaders värnpliktiga, som övertar befattningar i krigsorganisationen som tidigare bemannats av yrkesofficerare. Studien utforskar vad detta införande innebär för yrkesofficersprofessionen, vilka konsekvenser som uppstår likväl som hur införandet bör implementeras. Genom intervjuer av åtta respondenter från Trängregementet i Skövde utforskas införandet ur yrkesofficerarnas perspektiv. Studien tar hjälp av Bolman och Deal’s fyra perspektiv på organisation och ledarskap samt Runsten och Werr’s modell för kunskapsintegration för att förstå och tolka organisationsförändringen. Införandet av 15 månaders värnpliktiga omfattar yrkesofficerarnas förväntningar på förändringen, som baseras från tidigare erfarenheter men innehåller även farhågor och positiva förväntningar. Erfarenheten att utbilda dessa värnpliktiga saknas idag på grundutbildningskompanierna, denna kunskap återfinns hos äldre yrkesofficerare i högre nivåer - varvid ett utbyte av gamla erfarenheter i dagens kontext är en framgångsfaktor. Införandet påverkar även utbildning och övningar för förbanden, då värnplikten sker under en längre tid samt att de värnpliktiga ställer ett ökat ansvarstagande hos plutonchefer. Professionen för främst specialistofficerare förändras, genom att värnpliktiga innehar samma grad som specialistofficerare likväl som att expertisen för specialistofficerare kommer ta längre tid att uppnå. En maktförskjutning sker från yrkesofficerare till värnpliktiga, vilket främst påverkar unga specialistofficerare. Studien rekommenderar Försvarsmakten att överse nuvarande styrdokument och uppdatera dessa, samt vidare utveckla yrkesofficersprofessionen. Studien rekommenderar Försvarsmakten att med nuvarande indelning i trebefälssystemet att sergeantsgraden är lämplig för de värnpliktiga och att en förstegsutbildning är bäst lämpad för att utbilda och öva sig till ställföreträdande plutonchef. Om Försvarsmakten inför värnpliktiga plutonchefer i framtiden bör detta föregås av en analys utifrån den militära professionen och trebefälssystemet. / Since the early 2000’s the Swedish Armed Forces has conducted several changes concerning its organization and personnel. The downsizings of units as well as an increased focus on international operations in the early 2000’s have changed to a growth of units and a return to a national focus. The professions of professional officers have changed with the transformation from a single-command system to a two-officer system in the late 00s, as well as the pause of national conscript service in 2010 and introduction of professional soldiers within the organization. The national conscript service was reintroduced in 2018, for soldiers and squad leaders between 9 and 11 months. During the summer of 2021, 15 months conscript service will be reintroduced. These new conscripts will take over the positions previously manned by professional officers in the wartime organization. The study explores the reintroduction’s affects for professional officers, what consequences that occur as well as how the reintroduction should be implemented. Through interviews with eight respondents from the Logistics regiment in Skövde the reintroduction is explored through the professional officers’ view. The study seeks guidance from Bolman and Deal’s four perspectives on organization and leadership and also Runsten and Werr’s model for integration of knowledge, to be able to understand and interpret the organizational change. The reintroduction of 15 months conscripts includes professional officers’ expectations of the change, which is based from earlier experiences but also includes concerns and hopes. There is a lack of experience to train this new category of conscripts today at company level, the ability to do so is found in older officers in higher echelons – therefore an exchange of old experiences in the context of today is a criterion for success. The reintroduction also affects the training and the field exercises, because the conscript service is prolonged and the conscripts require an increased responsibility from the platoon leader. The profession for non-commissioned officers (NCO) changes - through the use of the same rank by conscripts and NCO’s. It will also take longer time for NCO’s to achieve expertise within their profession. A shift of power occurs from professional officers to conscripts, which mostly affects junior NCO’s. The study recommends the Swedish Armed Forces to overlook its current steering documents and to update these, and also to further develop its military profession for all officers. The study recommends the Armed Forces that with the current officer system, the rank of sergeant is well suited for these conscripts and the conscripts service should be based upon a pre-training concept to be able to succeed as deputy platoon leader. If the Armed Forces introduce conscripts as platoon leaders in the future, an analysis should be conducted prior to the change, concerning the military profession and the three-officer system.
