Layou ABSTRACT This dissertation presents a comprehensive analysis of the Staffing for Adequate Fire and Emergency Responders (SAFER) grant program's impact on Virginia's local governments, emphasizing their fiscal strategies, emergency management practices, and demographic relationships. By incorporating theories of fiscal federalism and the political economy, this study explores how SAFER grants, provided by the FEMA, influence local tax policies, economic stability, and the staffing levels in fire and emergency medical service departments, against the backdrop of a critical need for enhanced public safety post the 9/11 terrorist attacks. The second chapter provides a review of the literature tracing the evolution of fiscal federalism from its roots in the political philosophies of the Federalist Papers to its contemporary interpretations and applications. The study found gaps in the literature, providing opportunities for further exploration of how the current scholarship has been built upon the different generations of fiscal federalism. This study seeks to contribute to the literature by examining the relationship that federal grants have with local governments' preparation for emergencies through their local fire and emergency medical service staffing. The dissertation constructs a novel dataset covering SAFER grant allocations, local tax rates, and fire and emergency medical service budgets across Virginia's cities and counties from 2016 to 2022. It evaluates the grants' effects on budgetary decisions and staffing levels, utilizing a methodological framework that includes regression analysis, correlation coefficients, and event series analysis. The third chapter focuses on the socio-political and economic determinants affecting the distribution of the SAFER grant funding, revealing that jurisdictions with higher median household incomes receive more funding, contrary to expectations that economically disadvantaged areas are prioritized. This chapter also highlights the disparities in resource allocation and examines the equitable distribution between volunteer and professional fire departments and the relationship of political leanings on fund allocation. The fourth chapter explores the SAFER grants within the principal-agent framework, investigating their impact on local government tax policy and economic stability. It explores whether SAFER grants lead to changes in local tax policies and how they contribute to fiscal stability, especially during crises such as the 2007-2008 financial crisis and the COVID-19 pandemic. The fifth chapter assesses the SAFER grants' effectiveness in increasing the number of full-time equivalent (FTE) employees and improving public safety. It scrutinizes whether these grants supplement or supplant local budgets and their broader implications for budget allocations and incident reduction, which tie closely with the federal government's national preparedness goals. This dissertation advances emergency management scholarship by offering new insights into the allocation of SAFER grants in Virginia and its impact. Layou GENERAL AUDIENCE ABSTRACT This dissertation investigates how federal money, specifically the SAFER grants aimed at improving fire and emergency services, impacts local communities in Virginia. The SAFER grants are designed to provide a financial boost to a community's fire department in hiring more firefighters. This study goes beyond hiring to determine who receives the grant money and why. It examines if richer areas are more likely to receive funds than poorer ones. The study also considers whether a community's size or the number of emergencies they handle influences the amount of funding they receive. By analyzing the data from Virginia, this study sheds light on the process behind these grants. It finds that wealthier areas often receive more funding, supporting prior research and challenging the idea that such grants primarily benefit economically disadvantaged regions. Additionally, the study examines whether there exists a fair balance in funding between volunteer-based and professional fire departments and how politics may play a role in who receives more or less funding. Furthermore, this dissertation explores the ripple effects of the SAFER grants. It assesses whether these grants encourage local governments to adjust their spending and tax policies. The findings suggest that receiving a SAFER grant does not necessarily lead local governments to increase taxes or decreases spending in other areas. Instead, these governments appear to use these funds strategically to enhance public safety without overburdening local taxpayers. Ultimately, this dissertation not only provides valuable insights into the allocation and impact of the SAFER grants but also raises important questions about equity, fairness, efficiency, and the role of government at all levels in ensuring public safety.

Redesigning the Barranquilla's public emergency care network to improve the patient waiting time

Tesis por compendio / [ES] La oportunidad en la atención es uno de los críticos de mayor relevancia en la satisfacción de los pacientes que acuden a los servicios de Urgencias. Por tal motivo, las instituciones prestadoras de servicio y las organizaciones gubernamentales deben propender conjuntamente por una atención cada vez más oportuna a costos operacionales razonables. En el caso de la Red Pública en Servicios de Urgencias de Barrannquilla, compuesta por 8 puntos de atención y 2 hospitales, la tendencia marca un continuo crecimiento de la oportunidad en la atención con una tasa de 3,08 minutos/semestre y una probabilidad del 93,13% de atender a los pacientes después de una espera mayor a 30 minutos. Lo anterior se constituye en un síntoma inequívoco de la incapacidad de la Red para satisfacer los estándares de oportunidad establecidos por el Ministerio de Salud, hecho que podría desencadenar el desarrollo de sintomatologías de mayor complejidad, el incremento de la probabilidad de mortalidad, el requerimiento de servicios clínicos más complejos (hospitalización y cuidados intensivos) y el aumento de los costos asociados al servicio. En consecuencia, la presente tesis doctoral presenta el rediseño de la Red Pública en Servicios de Urgencias anteriormente mencionada a fin de otorgar a la población diana un servicio eficiente y altamente oportuno donde tanto las instituciones prestadoras del servicio como los organismos gubernamentales converjan efectivamente. Para ello, fue necesaria la ejecución de 4 grandes fases a través de las cuales se consolidó una propuesta orientada al desarrollo efectivo y sostenible de las operaciones de la Red. Primero, se caracterizó la Red Pública de Servicios de Urgencias en Salud considerando su comportamiento actual en términos de demanda y oportunidad de la atención. Luego, a través de una revisión sistemática de la literatura, se identificaron los enfoques metodológicos que se han implementado para la mejora de la oportunidad y otros indicadores de rendimiento asociados al servicio de Urgencias. Posteriormente, se diseñó una metodología para la creación de redes de Urgencias eficientes y sostenibles la cual luego se validó en la Red Pública sudamericana a fin de disminuir la oportunidad de atención promedio en Urgencias y garantizar la distribución equitativa de los beneficios financieros derivados de la colaboración. Finalmente, se construyó un modelo multicriterio que permitió evaluar el rendimiento de los departamentos de Urgencia e impulsó la creación de estrategias de mejora focalizadas en incrementar su respuesta ante la demanda cambiante, los críticos de satisfacción y las condiciones de operación estipuladas en la ley. Los resultados de esta aplicación evidenciaron que los pacientes que acceden a la Red tienden a esperar en promedio 201,6 min con desviación de estándar de 81,6 min antes de ser atendidos por urgencia. Por otro lado, de acuerdo con la revisión de literatura, la combinación de técnicas de investigación de operaciones, ingeniería de la calidad y analítica de datos es ampliamente recomendada para abordar este problema. En ese sentido, una metodología basada en modelos colaterales de pago, simulación de procesos y lean seis sigma fue propuesta y validada generando un rediseño de Red cuya oportunidad de atención promedio podría disminuir entre 6,71 min y 9,08 min con beneficios financieros promedio de US$29,980/nodo. En último lugar, un modelo compuesto por 8 criterios y 35 sub-criterios fue diseñado para evaluar el rendimiento general de los departamentos de Urgencias. Los resultados del modelo evidenciaron el rol crítico de la infraestructura (Peso global = 21,5%) en el rendimiento de los departamentos de Urgencia y la naturaleza interactiva de la Seguridad del Paciente (C + R = 12,771). / [CA] L'oportunitat en l'atenció és un dels crítics de major rellevància en la satisfacció dels pacients que acudeixen als serveis d'Urgències. En el cas de la Xarxa Pública en Serveis d'Urgències de Barrannquilla, composta per 8 punts d'atenció i 2 hospitals, la tendència marca un continu creixement de l'oportunitat en l'atenció amb una taxa de 3,08 minuts / semestre i una probabilitat de l' 93,13% d'atendre els pacients després d'una espera major a 30 minuts. Els resultats d'aquesta aplicació van evidenciar que els pacients que accedeixen a la Xarxa tendeixen a esperar de mitjana 201,6 min amb desviació d'estàndard de 81,6 min abans de ser atesos per urgència. D'altra banda, d'acord amb la revisió de literatura, la combinació de tècniques d'investigació d'operacions, enginyeria de la qualitat i analítica de dades és àmpliament recomanada per abordar aquest problema. Therefore, hospitals and governmental organizations should jointly aim to provide timely attention at reasonable costs. In the case of Barranquilla's Pubic Emergency Service Network, composed by 8 Points of care (POCs) and 2 hospitals, the trend evidences a continuous growing of the waiting time with a rate of 3,08 min/semester and a 93,13% likelihood of serving patients after waiting for more than 30 minutes. This is an unmistakable symptom of the network inability for satisfying the standards established by the Ministry of Health, which may trigger the development of more complex symptoms, increase in the death rate, requirement for more complex clinical services (hospitalization and intensive care unit) and increased service costs. This doctoral dissertation then illustrates the redesign of the aforementioned Public Emergency Service Network aiming at providing the target population with an efficient and highly timely service where both hospitals and governmental institutions effectively converge. It was then necessary to implement a 4-phase methodology consolidating a proposal oriented to the effective and sustainable development of network operations. First, the Public Emergency Service Network was characterized considering its current behavior in terms of demand and waiting time. A systematic literature review was then undertaken for identifying the methodological approaches that have been implementing for improving the waiting time and other performance indicators associated with the emergency care service. Following this, a methodology for the creation of efficient and sustainable emergency care networks was designed and later validated in the Southamerican Public network for lessening the average waiting time and ensuring the equitable distribution of profits derived from the collaboration. Ultimately, a multicriteria decision-making model was created for assessing the performance of the emergency departments and propelling the design of improvement strategies focused on bettering the response against the changing demand conditions, critical to satisfaction and operational conditions. The results evidenced that the patients accessing to the network tend to wait 201,6 min on average with a standard deviation of 81,6 min before being served by the emergency care unit. On the other hand, based on the reported literature, it is highly suggested to combine Operations Research (OR) methods, quality-based techniques, and data-driven approaches for addressing this problem. In this sense, a methodology based on collateral payment models, Discrete-event simulation, and Lean Six Sigma was proposed and validated resulting in a redesigned network whose average waiting time may diminish between 6,71 min and 9,08 min with an average profit US$29,980/node. Lately, a model comprising of 8 criteria and 35 sub-criteria was designed for evaluating the overall performance of emergency departments. Styrning av förbättringsarbete på akutmottagningar : En studie om möjliggörande och tvingande styrning / Management of process-improvements in emergency departments : A study about enabling and coercive control

Bakgrund: Sjukvården i Sveriges står inför en problematik, vilken kan hänföras till få vårdplatser och knappa resurser, vilket i sin tur lett till överbelastade akutmottagningar. Denna problematik ställer krav på att utveckla förbättringar i syfte att öka effektiviteten och kvaliteten på mottagningarna. Styrningen av förbättringsprocesser består av både möjliggörande och tvingande inslag, vilka kan komma att antingen samspela eller ge upphov till styrspänningar.  Syfte: Syftet med studien är att identifiera och analysera olika aspekter av förbättringsarbete på akutmottagningar, såsom hur de styrs och dess potentiella utmaningar. Studien syftar även till att undersöka de anställdas upplevelser av de möjliggörande och tvingande inslagen av styrningen samt ge förslag på hur styrningen av förbättringsarbete kan utvecklas i framtiden.  Metod: Studien har innefattat semistrukturerade intervjuer med diverse chefer, läkare, sjuksköterskor och politiker i Region Kronoberg och Region Kalmar, vilket gör det till en multipel flerfallstudie av kvalitativ karaktär. I metodavsnittet beskrivs det hur det empiriska materialet samlats in. Slutsats: Förbättringsarbete analyserades utifrån aspekterna: kvalitet, flöde och effektivitet, där det tydliggjordes hur förbättringsarbetet består av att möta de utmaningar som finns samtidigt som kostnadseffektivitet och vårdkvalitet behövs tas i beaktning. Vidare identifierades möjliggörande styrning på akutmottagningarna i form av värdegrunden och visionen, och i den professionella autonomin. Den tvingande styrningen kunde istället ses i budgeten och verksamhetsplaner, regler och riktlinjer samt i RETTS-systemet. Fem stycken styrspänningar identifierades mellan professionell autonomi och formella regelverk, personalbrist och överbelastning, effektiviseringskrav och resursbrist, läkare och sjuksköterskor, samt medicinsk personal och administrativa samt politiska aktörer. Dessa spänningar och relationer mellan aktörerna i regionerna förklarades med hjälp av aktör-nätverksteori. / Background: Swedish healthcare is facing problems associated with few hospital beds and scarce resources, which in turn has led to an overload in the emergency departments. This issue has created a demand for process improvement aimed at increasing the efficiency and quality of the emergency departments. Management of process improvement consists of both enabling and coercive elements, which may either interact or create tensions in governance. Purpose: The purpose of the essay is to identify and analyze various aspects of improvement work in emergency departments, such as how it is managed and its potential challenges. The study also aims to examine employees’ experiences of the enabling and coercive elements of management, and to provide suggestions on how the controls can be developed in the future. Method: The study has involved semi-structured interviews with various managers, physicians, nurses and politicians in Region Kronoberg and Region Kalmar, making it a multiple case study of qualitative nature. The methodology section describes how the empirical data was collected.  Conclusion: Improvement work was analyzed based on the aspects of quality, patient flow and efficiency, clarifying how the improvement work consists of meeting the challenges that exist while at the same time taking cost efficiency and quality of care into account. Furthermore, enabling control was identified in emergency departments in the form of the value and vision, and in the professional autonomy. Coercive controls could instead be seen in the budget, the operational plans, rules and guidelines, and the RETTS-system. Five tensions were identified between professional autonomy and formal regulations, staff shortages and overcrowding, efficiency demands and resource shortages, doctors and nurses, and medical staff and administrative as well as political actors. These tensions and relationships between actors in the regions were explained using actor-network theory.

Study Abroad and Student-Athlete Choice

The focus of this case study was a study abroad program for student-athletes at a high academically achieving, small liberal arts college in the mid-west region of the United States. The program is designed to maintain a culture of internationalism and multiculturalism by exposing as many student-athletes as possible to study abroad. I reviewed literature to extract an appropriate theoretical framework along with variables that aligned with the purpose of the study; structural and organizational characteristics of the institution, student's background and pre-college traits, interaction with agents of socialization and institutional environment, and quality of effort. I used the semi-structured interview process to interview 9 senior student-athletes (3 female, 6 male; 7 White, 1 African American/White, 1 Chilean/White) who participated in study abroad during the 2015-2016 academic school year at the researched institution and to interview 5 administrators who facilitate the athletic department at the institution. I found that certain critical elements emerged as necessary to create and maintain a study abroad program geared specifically to the needs of the student-athlete population. I also found strong implications for adaptable elements that could generate opportunities for student-athletes to study abroad at a higher rate. These elements serve as a recommended framework and set of initial guidelines for student-athletes and athletic departments nationwide.

SMTs are working under the most difficult conditions, especially during the transformation process in education. They are often not well-prepared for tasks they must perform and are neither given sufficient training. The focus of this research is to examine the role of HODs in the management of HRD. The EMD curriculum delivery nexus prompts the need to examine curriculum and education management and development, theories, practices and policies that support the implementation of the curriculum. HODs that are determined to build learning organisations should be able to create a culture of collaborative learning, where the acquisition of skills and knowledge is seen as an investment for the future. For transformation to take place in schools, educators should become major focal points for producing the momentum for change. Educational transformation and curriculum change development is taking place at a rapid pace. The whole transformation process is characterised by policy formulation rather than policy implementation. The concern is that the state of readiness of implementation at school level has not been investigated. The establishment of sound legal and regulatory framework to facilitate the transformation process can only become effective if schools are ready for the implementation of the new curriculum. A qualitative research methodology was employed for this study. The key objective of the research was to elicit the perceptions, by means of questionnaires and interviews, that educators and SMTs have of the role that HODs play on the management of HRD to support curriculum change, development and delivery, as well as the role of District Officials in this regard. The population of this study consisted of educators from primary schools in three education districts in the Free State province. The sample employed in this study consisted of sixty educators, including Heads of Department (HODs). Twenty educators per education district were randomly selected. It is evident that HODs require competencies such as staff provisioning, maintaining good human relations and providing an intensive HRD programme so that they manage educators effectively, which, in turn will result in their effective management of departments. The recommendations of this study are incorporated in a Human Resource Development (HRD) programme put forward by the researcher to add to the current body of knowledge in Human Resource Management in Education.

The relationship between personality variables and work performance of credit controllers in a bank

The primary aim of the research has been to determine whether there is a relationship between personality variables as measured by the Occupational Personality Questionnaire (OPQ32i) and the work performance of credit controllers in a bank. Work performance was measured by means of internal company data sources. A literature review was used to verify whether there is a theoretical relationship between personality and work performance and strong evidence was found. The sample consisted of 89 credit controllers. The relationship between personality variables and work performance was determined by means of correlation studies and multiple regression analyses. Results are reported both in terms of statistical significance and effect sizes. Fostering a framework to embed the records management function into the auditing process in the South Africa public sector

Proper records management plays a significant role in the auditing process, risk management and wider corporate governance. Despite this role, in South Africa, many governmental bodies are issued with disclaimer reports every year by the Auditor-General of South Africa (AGSA) due to a lack of supporting documentation. This problem is exacerbated by the exclusion of records management from the criteria for a sound financial management infrastructure in many governmental bodies. The other dilemma is that some records such as financial records, personnel records and electronic records usually fall outside the jurisdiction of the organisation’s records manager. Utilising the King Report III as a framework, this study sought to develop a framework to embed records management practices into the auditing process in the public sector of South Africa, with a view to entrench a culture of clean audits. The study relied on mixed methods research (MMR), with the quantitative study conducted first through informetrics analysis of audit reports, while the qualitative paradigm was used to substantiate numerical data. Data collection adopted a multi-approach with four key sources of data: a questionnaire, interviews, literature review and publicly available data from the consolidated general reports of AGSA. The study revealed that most governmental bodies have established internal audit units, audit committees and records management units, which did not work in unison. In most governmental bodies records management did not form part of the audit scope and records management professionals were not part of the audit committees. As a result, most governmental bodies continued to receive negative audit opinions from AGSA. The study recommends that records management community should utilise auditing and risk management as a springboard to propel records management to the new heights. A further empirical study on the role of auditing and risk management in records management that embraces both the private and public sectors is recommended. / Information Science / D. Litt. et Phil. The focus is on Germany, but the analysis is put in a wider, comparative context. This is done with reference to South Africa, as well as Europe and the American system. This thesis demonstrates and discusses the existing legal norms to regulate Internet provider liability for illegal content on the Internet and the international efforts to deal with this issue. In the introduction it is shown how the Internet has given rise to a new form of global communication and the accompanying legal problems. This is followed by an examination of the different functions Internet providers have. A survey of some of the important crimes affecting the Internet and also some Internet-specific offences put the more general issue of liability in a more specific context. Traditional and new forms of crimes are discussed. This section is followed by an analysis of Internet provider liability under German criminal law and Germany's Teleservices Act. From an international criminal law perspective some international instruments, like the Cybercrime Convention of the Council of Europe, is discussed. National legislation, especially in the context of the European Union, must always be put in the proper regional and international context. The thesis concludes with some thoughts on alternative, or perhaps complementary, methods to fight illegal and criminal conduct on the Internet. This is done not as a critique of the responses to Internet crime, but rather to strengthen the many hands trying to reduce Internet crime. / AFRIKAANSE OPSOMMING: Hierdie tesis handeloor die strafregtelike aanspreekliheid van Internet diensverskaffers. Die fokus val op Duitsland, maar die analise word ook geplaas in 'n wyer, vergelykende konteks. Dit word gedoen met verwysing na Suid-Afrika, sowel as Europa en die VSA. Die tesis demonstreer en bespreek die bestaande regsnorme wat Internet diensverskaffers reguleer met spesifieke verwysing na aanspreeklikheid vir onwettige inhoud op die Internet en internasionale pogings om hierdie probleem aan te spreek. Ter inleiding word daar aangetoon hoe die Internet aanleiding gee tot nuwe vorme van globale kommunikasie en die regsprobleme wat dit tot gevolg het. Dit word gevolg deur 'n ondersoek na die verskillende funksies van Internet verskaffers. 'n Ontleding en bespreking van Internet-spesifieke misdrywe plaas die meer algemene vraagstuk in 'n meer gefokusde konteks. Tradisionele en nuwe vorme van misdaad word bespreek. Hierdie afdeling word gevolg deur 'n ontleding van Internet diensverskaffer aanspreeklikheid ingevolge Duitse reg en die Duitse wetgewing op die terrein van telediens. Uit 'n internasionale strafreg oogpunt word sekere internasionale instrumente, soos die Cybercrime Convention van die Raad van Europa, bespreek. Nasionale wetgewing, veral in die konteks van die Europese Unie, word ook in die relevante regionale en internasionale konteks geplaas. Die tesis word afgesluit met sekere gedagtes oor alternatiewe, of moontlik komplimentêre, metodes in die stryd teen Internet-kriminaliteit. Dit moet nie gesien word as kritiek op die huidige stand van sake nie, maar eerder as 'n poging om die talle rolspelers in die stryd teen Internet misdaad se hande te sterk.
