Analysis of accident and emergency services in Hong Kong: the level of inappropriate utilization and why?. / CUHK electronic theses & dissertations collection

Objectives: This study has been conducted to determine the levels of inappropriate use of the A&E for conditions that could be treated by GPs, the nature of the morbidity pattern of those conditions, the reasons why primary care services were not being utilized, and also examined the validity (i.e. sensitivity and specificity) of patient classifications undertaken by nurses at the time of admission within this local context. Study design and setting: A cross sectional study was conducted over a one year period and subjects were randomly selected from four A&E departments located across the four principle geographic regions of Hong Kong by stratified, two-stage sampling. Main outcome measure: The gold standard in differentiating true emergency cases and GP cases was based on a retrospective record review conducted independently by a panel of emergency physicians. A random sub-sample of those classified as GP cases was interviewed and compared to a matched (via morbidity status) sample of primary care patients who had attended a hospitals' GOPC in order to determine factors distinguishing these two patient groups. Multiple Logistic Regression was used to distinguish the difference between GP cases and matched GOPC primary care patients on significance and odds ratios of the variables. The morbidity pattern according to ICPC was tabulated and analysed for the 'true' A&E cases and non-urgent cases. Sensitivity, specificity and positive predictive values were computed for both non-weighted and weighted conditions. Results: The level of GP cases was found to be 57% with a significant higher proportion of patients in younger age group, and late evening. The morbidity pattern of those top 10 diagnoses of non-urgent cases was very similar to the Hong Kong general practice morbidity pattern for self limiting conditions. Closure of the clinic was the main reason for GP cases attending A&E. Other major reasons were deterioration of symptoms, GPs' inability to diagnose efficiently and patients' wish to continue medical treatment in the same hospital. Affordability was the most pronounced reason for utilising the GOPC, but did not apply to the A&E GP patients. The most accurate weighted nurses' triage classification had the average sensitivity of 75%, specificity of 65.7%, and positive predictive value of 54%. The most accurate weighted patients' self-triage classification yielded a sensitivity of 43.3%, specificity of 49.2%, and a positive predictive value of 38.6%. Conclusion: The reasons for high level of utilisation of A&E services are complex and reflect problems of delivery of GP services. There is an urgent need for the GPs to set up a network system to provide out of hours services, and also for a better interfacing between primary and secondary care, and between public and private sectors, so that patients can be referred back to GPs. Expérience de soins de patients aînés atteints de cancer à l’urgence / Health care experience of older cancer patients in the context of emergency room visits

Résumé: Introduction: Cette étude s’intéresse à l’expérience de soins de patients aînés de 70 ans et plus atteints de cancer entourant une visite à l’urgence. Elle repose sur plusieurs résultats d’études démontrant que ces personnes présentent des besoins spécifiques non comblés, les amenant à utiliser les services d’urgence pour y répondre. Peu d’études se sont intéressées à leur expérience de soins et encore plus rarement dans le contexte d’une visite à l’urgence. But : Mieux comprendre l’expérience de soins des patients aînés atteints de cancer entourant une visite à l’urgence. Cette étude comprend quatre objectifs soit : 1) établir le profil des patients de 70 ans et plus ayant consulté l’urgence pour un problème lié à leur cancer dans les 12 mois précédant l’étude; 2) identifier les raisons et les facteurs amenant ces patients à consulter les services d’urgence; 3) identifier les stratégies de coping utilisées avant d’aller à l’urgence et 4) décrire l’expérience de soins de ces patients à l’urgence. Méthode: Une étude à devis mixte simultané a été entreprise. Une analyse descriptive des bases de données administratives a été réalisée pour documenter le profil sociodémographique, clinique et l'utilisation des services des patients aînés atteints de cancer de 70 ans et plus qui ont visité l'urgence d'un centre hospitalier au Québec, le Canada (n = 792, Objectifs 1-2). Ensuite, des entrevues semi-structurées ont suivi (n = 11) et ont été analysées par une analyse de contenu en profondeur (Objectifs 2, 3, & 4). Résultats: Un total de 792 patients aînés atteints de cancer ont visité l'urgence pour un total de 1572 visites. Les raisons de consultation les plus fréquentes étaient de nature respiratoire (15,8%), digestive (13,4%) et les troubles cardiovasculaires (8,2%). Plus de la moitié des visites était réalisée de jour. L’anxiété, les problèmes d’accessibilité et la détérioration importante de la santé étaient des facteurs qui influençaient le recours aux services d’urgence. Les patients ont été en mesure de déployer une multitude de stratégies de coping pour faire face à leurs problèmes de santé. Par ailleurs, l’expérience de soins est un processus unique, vécu de manière très individuelle. Conclusion: Cette étude décrit l’expérience de soins de patients aînés atteints de cancer à l’urgence et souligne plusieurs domaines d’amélioration des services en périphérie de l’urgence, mais aussi au sein même du service d’urgence. / Abstract: Introduction: Older cancer patients are known to have specific unmet needs due to the complexity of their health care, leading them to use emergency services. However, it is known that emergency rooms are not well-suited to the needs of the elderly. Few studies have focused on the health care experience of older cancer patients and even less so in the context of emergency room (ER) visits. Purpose: This study aims to better understand the experience of older cancer patients in the context of ER visits for unexpected health deterioration related to cancer. We sought to: 1) establish the profile of patients aged 70 years and older who made ER visits for problems related to their cancer in the 12 months preceding the study; 2) identify the reasons and factors that motivate older patients with cancer to make ER visits; 3) identify coping strategies used by older cancer patients prior to ER visits; and 4) describe the health care experience of older cancer patients in the context of ER visits. Methods: A concurrent mixed-method design was used. Descriptive analysis of administrative databases was first conducted to document the socio-demographic, clinical, and service utilization profile of elderly cancer patients aged 70 years and older who visited the ER of a hospital in Québec, Canada (n = 792, Objectives 1-2). Semi-structured interviews were subsequently conducted (n = 11) and then analysed using in-depth content analysis (Objectives 2, 3, & 4). Results: The sample of 792 older cancer patients made a total of 1,572 ER visits. The most frequent medical reasons for ER visits were respiratory (15.8%) and digestive (13.4%) concerns, and cardiovascular conditions (8.2%). Content analysis of the qualitative data suggested that older cancer patients made most of the ER visits when experiencing high levels of anxiety, when other cancer care services were unavailable, or because of a serious life-threatening health condition. Patients were able to use a variety of coping strategies to deal with health issues. Furthermore, the care process is experienced uniquely for each individual. Conclusion: This study describes the health care experience of older cancer patients in the context of ER visits and suggests areas of improvement both outside of and within emergency services.

Introdução: A Síndrome de Burnout representa um importante fenômeno psicossocial da atualidade que ocorre como consequência do estresse laboral crônico. Demonstra ser um fenômeno bastante complexo e multifatorial. Os profissionais de saúde estão entre os profissionais mais acometidos pelo Burnout, e entre eles, encontram-se os profissionais que atuam nos serviços de emergências, que estão trabalhando constantemente em contato direto nos eventos traumáticos. Devido essa particularidade do serviço, podem estar mais suscetíveis ao comprometimento de seu bem-estar físico e emocional e, somado as variáveis individuais e ambientais desenvolverem Burnout. Objetivo: Avaliar a validade fatorial do Maslach Burnout Inventory - Human Services Survey (MBI-HSS) na amostra, verificar a prevalência de Burnout em profissionais de saúde e sua relação com as variáveis laborais, individuais e consequências para a saúde (adoecimento). Métodos: Estudo transversal, analítico de abordagem quantitativa. Avaliaram-se 282 profissionais de saúde (equipe de enfermagem, médicos e radioperadores) atuantes nos serviços de urgência e emergência (serviço móvel, pré-hospitalar e hospitalar) de um município do interior do estado de São Paulo (Brasil). Procedeu-se a avaliação por meio da autoaplicação dos seguintes instrumentos: questionário sociodemográfico de condições de trabalho e saúde, o Inventário de Burnout de Maslach, Childhood Trauma Questionnaire, Inventário de Sintomas de Stress, Escala de Estresse Percebido, Psychiatric Screeming Questionnaire, Questionário de Estilo de Vida Fantástico, Escala Hospitalar de Ansiedade e Depressão e Escala de Reajustamento Social de Homes- Rahe. Os dados foram analisados por meio de análise fatorial confirmatória (AFC) e testes Qui-Quadrado de Pearson ou do teste Exato de Fisher para proceder os testes correlacionais. Adotou-se um nível de significância de 5%. Resultados: Os resultados obtidos mostram que o MBI-HSS é um instrumento confiável e fatorialmente válido para medir o Burnout. Todos os itens que compõem a escala apresentaram assimetria e curtose adequadas, atestando a sensibilidade psicométrica desses itens na amostra. A análise fatorial confirmou a estrutura trifatorial proposta para o MBI-HSS. Os itens 9, 12, 15 e 16 apresentaram peso fatorial abaixo do considerado adequado e foram removidos do modelo. O modelo hierárquico de segunda ordem com as modificações mencionadas apresentou ajustamento adequado aos dados e pode ser considerado o melhor e mais parcimonioso modelo testado de acordo com os índices da teoria da informação. O Burnout foi identificado em 13,2% da amostra e 24,5% dos participantes sofreram estresse precoce na infância, 11,3% apresentaram um estilo de vida considerado regular, 35,5% apresentaram ansiedade, 27,7% apresentaram depressão, 46,5% apresentaram sintomas de estresse e 37,9% apresentam fortes chances de adoecer e também apresentam transtornos mentais comuns. Houve associação entre Burnout e as seguintes variáveis: escolaridade, estresse precoce, sintomas de estresse, fase do estresse, tipo de sintomas, percepção de estresse, sofrimento mental comum, estilo de vida, ansiedade e depressão. As correlações foram evidenciadas entre Burnout e: tipo de serviço (p=0,032; OR: 0,187), escolaridade (p=0,029; OR: 2,313), percepção do estresse (p=0,037; OR: 1,67) e chances razoáveis de adoecer (p=0,031; OR: 0,279). Considerações finais: Os resultados confirmaram ambas as hipóteses do estudo, confirmando o modelo tridimensional que foi proposto para o MBI-HSS e demonstrando ser um instrumento confiável e fatorialmente válido para medir o Burnout e que algumas variáveis se associaram e/ou correlacionaram à variável desfecho, comportando-se como mediadoras do processo de desenvolvimento do Burnout / Introduction: Burnout syndrome represents an important psychosocial phenomenon of current that occurs as a consequence of chronic stress. It proves to be a very complex and multifactorial phenomenon. Health professionals are among the most affected by Burnout, and among them, are professionals working in emergency services who are constantly working in direct contact with traumatic events. Because of this particularity of service, they may be more susceptible to compromising their physical and emotional well-being and, in addition to individual and environmental variables, develop Burnout. Objective: To evaluate the factorial validity of Maslach Burnout Inventory - Human Services Survey (MBI-HSS) in the sample, to verify the prevalence of Burnout in health professionals and its relationship with labor and individual variables and consequences for health (illness). Methods: Cross- sectional, analytical, quantitative approach. Were avaluated a total of 282 health professionals (nursing staff, physicians and radio operators) working in the emergency services (mobile, prehospital and hospital services) of a city in the interior of São Paulo (Brazil). The following instruments were used: the sociodemographic questionnaire on working and health conditions, the Maslach Burnout Inventory, the Childhood Trauma Questionnaire, the Stress Symptom Inventory, the Self- Perceived Stress Scale, Psychiatric Screening Questionnaire, Fantastic Lifestyle Questionnaire, Hospital Anxiety and Depression Scale, and Homes-Rahe Social Readjustment Scale. Data were analyzed by means of confirmatory factorial analysis (CFA) and Pearson\'s Chi-square test or Fisher\'s Exact test to perform correlational tests. Adopted a significance level of 5%. Results: The results obtained show that MBI-HSS is a reliable and factorially valid instrument for measuring Burnout. All the items that compose the scale presented adequate asymmetry and kurtosis, attesting the psychometric sensitivity of these items in the sample. The factorial analysis confirmed the proposed three-factor structure for MBI-HSS. Items 9, 12, 15 and 16 presented a factorial weight below that considered not adequate and were removed from the model. The second-order hierarchical model with the mentioned modifications presented adequate adjustment to the data and can be considered the best and most parsimonious model tested according to the indices of information theory. Burnout was identified in 13.2% of the sample and 24.5% of the participants suffered from early childhood stress, 11.3% had a regular lifestyle, 35.5% presented anxiety, 27.7% presented depression, 46.5% had symptoms of stress and 37.9% % have a strong chance of becoming ill and also have common mental disorders. There was association between Burnout and the following variables: schooling, early stress, stress symptoms, stress phase, type of symptoms, perceived stress, common mental suffering, lifestyle, anxiety and depression. Correlations were evidenced between Burnout and: type of service (p = 0.032, OR: 0.187), schooling (p = 0.029, OR: 2,313), stress perception (p = 0.037, OR: 1.67) and reasonable chances of becoming ill (P = 0.031, OR: 0,279). Conclusions: The results confirmed both hypotheses of the study, confirming the three-dimensional model that was proposed for MBI-HSS and demonstrating that it is a reliable and factorially valid instrument to measure Burnout and that some variables were associated and / or Correlated with the outcome variable, behaving as mediators of the Burnout development process

Nätverksanalys i GIS-miljö för insatskartor : En fallstudie baserad på Uppsala kommuns vägnät

Att maximera sjukhusens och räddningstjänstens täckning av befolkningen är en viktig säkerhetsfråga som diskuteras inom bland annat hälsovårdsplaneringen. Geografiska informationssystem (GIS) kan användas som ett hjälpmedel för att analysera och presentera andelen befolkning som är utanför sjukvårdens täckningsområden och även visa variationer på täckningsområdena när anläggningar slås ihop eller läggs ned. Syftet med arbetet är att presentera en ny, vektorbaserad nätverksanalys i GIS-miljö för att undersöka hur täckningsområdena för räddningstjänsten i Uppsala kommun påverkas. För att utföra nätverksanalysen användes data från Nationella Vägdatabasen (NVDB) över Uppsala kommun. Attributen som användes från NVDB innehöll information om framförallt hastighetsbegränsningar, väghierarki, tid, restriktioner och gatunamn. Täckningsområdena beräknades med ArcGIS och tilläggsprogrammet Network Analyst och resulterade i en översiktlig karta med räddningstjänstens insatstider och även enskilda insatskartor. Jämfört med tidigare resultat av insatskartor för samma områden, gav nätverksanalysen mer noggranna resultat med utökade täckningsområden för varje räddningstjänststation. Antalet täckta tätorter ökades och även andelen tätortsbefolkning. Metoden som beskrivs är användbar för att beräkna geografiska täckningsområden och andelen population som tjänas inom en specifik tid. Det är ett särskilt viktigt verktyg som kan nyttjas av politiker vid till exempel beslutsfattande. / Maximizing the catchment areas of emergency services is an important safety question that is discussed in health planning. Geographic Information Systems (GIS) can be used as a support to analyse and present the percentage of the population that is outside the hospital catchment area and also show catchment area variations by restructuring the organization. The aim of this thesis is to present a new, vector-based network analysis and to investigate how the catchment areas for the emergency service in Uppsala municipality will be affected. To perform the network analysis, data over Uppsala municipality was used from Nationella Vägdatabasen (NVDB). The attributes that were used contained information about speed limits, road hierarchy, restrictions, time and road names. ArcGIS and the extension Network Analyst were used to calculate the catchment areas. The results obtained from the analysis were shown in a regional chart and individual maps with the emergency service drive time for each station. In comparison with previous results, the network analysis presents more accurate catchment areas. The method described is very useful when calculating geographic catchment areas and the percentage of the population that is served within a certain time. It is an important tool that can be used by politicians within decision making.

Knowledge and attitudes of Ball State University pre-service elementary education teachers toward emergency care in the school setting

Unintentional injuries are the leading cause of death for children aged 5-19 Twenty-two million children are injured each year and approximately one quarter of these injuries occur on school premises. Schools must provide nursing services to children who attend school, but ratios of registered nurses to students is higher than the 1:750 recommended ratio. Current school teachers believe pre-service teachers should be trained in emergency care in teacher training programs. Yet, no research has been conducted to evaluate pre-service teachers’ knowledge and attitudes toward emergency care. The purpose of the study was to investigate pre-service teachers’ knowledge of and attitude toward emergency care in the school setting. A cross sectional group-comparison survey design was used. A 40-item questionnaire was administered to pre-service elementary teachers at Ball State University located in Muncie, IN. The questionnaire consisted of questions from “Emergencies in the school setting: Are public school teachers adequately trained to respond?” and Urban public school teachers’ attitudes and perceptions of the effectiveness of CPR and automated external defibrillators. Sub-group comparisons were made using bivariate and multivariate analyses of similar demographic, attitude, and knowledge questions. Findings indicated that pre-service teachers have a positive attitude toward emergency care, low levels of knowledge about emergency care, and a low level of willingness to provide emergency care in schools. In addition, when comparing pre-service teachers who had received emergency care training to those who did not, a statistically significant difference was found in their knowledge about emergency care. Emergency care training has limited influence on pre-service teachers’ attitudes and willingness to provide care. / Department of Physiology and Health Science

Proposes the design of regional services in the optics of prehospital care with emphasis on the completeness of care and integration of services, qualification and the humanization of attention. Based on data from DATASUS and IBGE, year 2003, in conformity with a Minister of Health Order nº 2048/2003-GM-MS, were defined 12 regional clusters of urgency/emergency, with the same number of centrals regulation (11 to be implanted), 133 ambulances are needed for basic support (USB in portuguese) and 28 ambulances for advanced support (USA in portuguese). The necessary staff will be: 396 physicians, 396 nurses, 737 assistant/technical nursing, 660 ambulance drivers, 11 pharmacists, 110 telephone operators, among others. It was analyzed the admissions quantitative made in 20 poles hospitals in relation to the intervals of distance from the place of domicile. The occupation of beds is, in its majority, made by people of the municipality where the hospital is located. Based on the features of the Act Order of a Minister of Health nº 1101/GM-MS and according to the pole-regional grouping suggested, an analysis was made to verify the amount of beds available. It was obeyed that the number of beds is insufficient, more worrying is the situation of the ICU beds. As for the types of procedures of emergency, in the comparison between the poles, by frequency, there is greater focus on admissions for carrying out deliveries, cesarean sections and curettage post-abortion. It is suggested that the implementation of services should be done gradually with priority to the most underprivileged regions. In the re agreement of occupation of the beds in poleregional hospitals there should be clear a fair percentage for the types of procedures to be used by residents of the municipalities in regional coverage area, with definition of a standardized portfolio of hospital services, targeting self-sufficiency of emergency care at micro and macrorregional health in the state of Ceara. / Este estudo, descritivo, teve como objetivo elaborar uma proposta para organização do Sistema Estadual de Urgência e Emergência no estado do Ceará-Brasil, baseado na implantação de serviços de atendimento pré-hospitalar de urgência e emergência. Propõe o desenho de regionalização dos serviços na ótica do atendimento pré-hospitalar com ênfase na integralidade da atenção e integração dos serviços, da qualificação e da humanização da atenção. Com base nos dados do Datasus, e IBGE, ano 2003 e de acordo com o estabelecido pela Portaria GM-MS 2048/2003, foram definidos 12 pólos regionais de urgência e emergência com o mesmo número de centrais de regulação (11 a serem implantadas). São necessárias 133 ambulâncias de suporte básico (USB) e 28 ambulâncias de suporte avançado (USA). O pessoal necessário será: 396 médicos, 396 enfermeiros, 737 auxiliares/técnico de enfermagem, 660 condutores socorristas, 11 farmacêuticos, 110 telefonistas, entre outros. Foi analisado o quantitativo de internações realizadas nos 20 hospitais pólos em relação a intervalos de distância do local da residência. A ocupação dos leitos é em sua maioria feita pelos residentes dos municípios onde o hospital está localizado. Com base nos parâmetros do Ato Portaria nº 1101/GM-MS e de acordo com o agrupamento pólo-regional sugerido foi analisado o quantitativo de leitos disponíveis, verificou-se que o número de leitos é insuficiente, sendo mais preocupante a situação dos leitos de UTI. Quanto aos tipos de procedimentos de urgência e emergência, na comparação entre os pólos, por freqüência, existe maior concentração nas internações para realização de partos, cesarianas e curetagem pós-aborto. Sugere-se que a implantação seja feita de forma gradual com prioridade para as regiões mais desassistidas. Na repactuação da ocupação dos leitos dos hospitais pólo-regionais deverá haver definição clara de um percentual eqüitativo para os tipos de procedimentos por internação a serem utilizados pelos moradores dos municípios da área de cobertura regional, com definição de um “portfólio” padronizado de serviços hospitalares, almejando auto-suficiência do atendimento de urgência e emergência a nível micro e macrorregional de saúde no estado do Ceará.

Introdução: A Síndrome de Burnout representa um importante fenômeno psicossocial da atualidade que ocorre como consequência do estresse laboral crônico. Demonstra ser um fenômeno bastante complexo e multifatorial. Os profissionais de saúde estão entre os profissionais mais acometidos pelo Burnout, e entre eles, encontram-se os profissionais que atuam nos serviços de emergências, que estão trabalhando constantemente em contato direto nos eventos traumáticos. Devido essa particularidade do serviço, podem estar mais suscetíveis ao comprometimento de seu bem-estar físico e emocional e, somado as variáveis individuais e ambientais desenvolverem Burnout. Objetivo: Avaliar a validade fatorial do Maslach Burnout Inventory - Human Services Survey (MBI-HSS) na amostra, verificar a prevalência de Burnout em profissionais de saúde e sua relação com as variáveis laborais, individuais e consequências para a saúde (adoecimento). Métodos: Estudo transversal, analítico de abordagem quantitativa. Avaliaram-se 282 profissionais de saúde (equipe de enfermagem, médicos e radioperadores) atuantes nos serviços de urgência e emergência (serviço móvel, pré-hospitalar e hospitalar) de um município do interior do estado de São Paulo (Brasil). Procedeu-se a avaliação por meio da autoaplicação dos seguintes instrumentos: questionário sociodemográfico de condições de trabalho e saúde, o Inventário de Burnout de Maslach, Childhood Trauma Questionnaire, Inventário de Sintomas de Stress, Escala de Estresse Percebido, Psychiatric Screeming Questionnaire, Questionário de Estilo de Vida Fantástico, Escala Hospitalar de Ansiedade e Depressão e Escala de Reajustamento Social de Homes- Rahe. Os dados foram analisados por meio de análise fatorial confirmatória (AFC) e testes Qui-Quadrado de Pearson ou do teste Exato de Fisher para proceder os testes correlacionais. Adotou-se um nível de significância de 5%. Resultados: Os resultados obtidos mostram que o MBI-HSS é um instrumento confiável e fatorialmente válido para medir o Burnout. Todos os itens que compõem a escala apresentaram assimetria e curtose adequadas, atestando a sensibilidade psicométrica desses itens na amostra. A análise fatorial confirmou a estrutura trifatorial proposta para o MBI-HSS. Os itens 9, 12, 15 e 16 apresentaram peso fatorial abaixo do considerado adequado e foram removidos do modelo. O modelo hierárquico de segunda ordem com as modificações mencionadas apresentou ajustamento adequado aos dados e pode ser considerado o melhor e mais parcimonioso modelo testado de acordo com os índices da teoria da informação. O Burnout foi identificado em 13,2% da amostra e 24,5% dos participantes sofreram estresse precoce na infância, 11,3% apresentaram um estilo de vida considerado regular, 35,5% apresentaram ansiedade, 27,7% apresentaram depressão, 46,5% apresentaram sintomas de estresse e 37,9% apresentam fortes chances de adoecer e também apresentam transtornos mentais comuns. Houve associação entre Burnout e as seguintes variáveis: escolaridade, estresse precoce, sintomas de estresse, fase do estresse, tipo de sintomas, percepção de estresse, sofrimento mental comum, estilo de vida, ansiedade e depressão. As correlações foram evidenciadas entre Burnout e: tipo de serviço (p=0,032; OR: 0,187), escolaridade (p=0,029; OR: 2,313), percepção do estresse (p=0,037; OR: 1,67) e chances razoáveis de adoecer (p=0,031; OR: 0,279). Considerações finais: Os resultados confirmaram ambas as hipóteses do estudo, confirmando o modelo tridimensional que foi proposto para o MBI-HSS e demonstrando ser um instrumento confiável e fatorialmente válido para medir o Burnout e que algumas variáveis se associaram e/ou correlacionaram à variável desfecho, comportando-se como mediadoras do processo de desenvolvimento do Burnout / Introduction: Burnout syndrome represents an important psychosocial phenomenon of current that occurs as a consequence of chronic stress. It proves to be a very complex and multifactorial phenomenon. Health professionals are among the most affected by Burnout, and among them, are professionals working in emergency services who are constantly working in direct contact with traumatic events. Because of this particularity of service, they may be more susceptible to compromising their physical and emotional well-being and, in addition to individual and environmental variables, develop Burnout. Objective: To evaluate the factorial validity of Maslach Burnout Inventory - Human Services Survey (MBI-HSS) in the sample, to verify the prevalence of Burnout in health professionals and its relationship with labor and individual variables and consequences for health (illness). Methods: Cross- sectional, analytical, quantitative approach. Were avaluated a total of 282 health professionals (nursing staff, physicians and radio operators) working in the emergency services (mobile, prehospital and hospital services) of a city in the interior of São Paulo (Brazil). The following instruments were used: the sociodemographic questionnaire on working and health conditions, the Maslach Burnout Inventory, the Childhood Trauma Questionnaire, the Stress Symptom Inventory, the Self- Perceived Stress Scale, Psychiatric Screening Questionnaire, Fantastic Lifestyle Questionnaire, Hospital Anxiety and Depression Scale, and Homes-Rahe Social Readjustment Scale. Data were analyzed by means of confirmatory factorial analysis (CFA) and Pearson\'s Chi-square test or Fisher\'s Exact test to perform correlational tests. Adopted a significance level of 5%. Results: The results obtained show that MBI-HSS is a reliable and factorially valid instrument for measuring Burnout. All the items that compose the scale presented adequate asymmetry and kurtosis, attesting the psychometric sensitivity of these items in the sample. The factorial analysis confirmed the proposed three-factor structure for MBI-HSS. Items 9, 12, 15 and 16 presented a factorial weight below that considered not adequate and were removed from the model. The second-order hierarchical model with the mentioned modifications presented adequate adjustment to the data and can be considered the best and most parsimonious model tested according to the indices of information theory. Burnout was identified in 13.2% of the sample and 24.5% of the participants suffered from early childhood stress, 11.3% had a regular lifestyle, 35.5% presented anxiety, 27.7% presented depression, 46.5% had symptoms of stress and 37.9% % have a strong chance of becoming ill and also have common mental disorders. There was association between Burnout and the following variables: schooling, early stress, stress symptoms, stress phase, type of symptoms, perceived stress, common mental suffering, lifestyle, anxiety and depression. Correlations were evidenced between Burnout and: type of service (p = 0.032, OR: 0.187), schooling (p = 0.029, OR: 2,313), stress perception (p = 0.037, OR: 1.67) and reasonable chances of becoming ill (P = 0.031, OR: 0,279). Conclusions: The results confirmed both hypotheses of the study, confirming the three-dimensional model that was proposed for MBI-HSS and demonstrating that it is a reliable and factorially valid instrument to measure Burnout and that some variables were associated and / or Correlated with the outcome variable, behaving as mediators of the Burnout development process
