The psychosocial impact on rural grandmothers caring for their grandchildren orphaned by HIV/AIDS

This exploratory study investigated the psychosocial impact on rural grandmothers of Gutu, Zimbabwe, caring for their grandchildren orphaned by HIV/AIDS. The participants included 12 paternal and maternal grandmother-caregivers from four districts of Gutu, whose ages ranged from 56 to 76 years with orphans in their care ranging from infants to 18 years. The present study made use of Erikson's psychosocial theory of development on late adulthood. Data were gathered using semi-structured open-ended interviews in the participants' homes. Interpretive analysis was used to analyse the audio-taped data. Findings reveal that most grandmothers are experiencing a personal toll in dealing with the late adult crisis of integrity versus despair, including finding it difficult to resolve the grief of losing children while engaging in full time grandparenting in a stigmatising society. Participants reported a need for support and interventions tailored to their unique needs. Counselling, social support, financial assistance, and skills and knowledge about HIV/AIDS are therefore recommended. / Psychology / M.Sc. (Psychology)

Ignoring a Silent Killer: Obesity & Food Security in the Caribbean (Case Study: Barbados)

Obesity and obesity-related diseases – such as type 2 diabetes – have become the most crucial indicators of population health in the 21st century. Formerly understood as ‘diseases of affluence’, obesity is now prevalent in the Global South posing serious risk to socioeconomic development. This is particularly true for rapidly developing countries where nutrition transitions are most apparent. There are many factors which impact on risk of obesity (e.g. gender, culture, environment, socioeconomic status, biological determinants). The problem is further aggravated within small island developing states where food security is exacerbated by factors associated with globalization and development. The thesis examines the surge of obesity and type 2 diabetes within Caribbean populations, using Barbados as a case study. A holistic approach was applied using an ecological health model. Moving away from the lifestyle model, the theoretical framework underpinning included sub-theories (e.g. social constructivism, feminism, post-colonial theory, concepts of memory and trauma).

This thesis sets out to explore the lives of homosexuals in the AIDS epidemic in Sweden in Jonas Gardell´s trilogy Torka aldrig tårar utan handskar (Don´t Ever Dry Tears Without Gloves). How is queer space able to unfold in connection to concrete places? The thesis utilizes the perspective of queer theory, where the useful terms of queer space and heteronormativity are found.   The analysis is divided into two parts based on the important themes of life and death found in the trilogy. The metal ring at the central station called Bögringen (The Gay Ring) and other public places such as parks and streets make up the first part. It becomes evident that these spaces are vital to the very existence of the homosexual lives of the men in the book-series. In these places, they are able to create their own (queer) space, which exists in parallel to the heteronormative surroundings. The concrete spaces that become these queer spaces are nevertheless often challenged by their surroundings, both verbally and physically.   Funerals make up the second half of the analysis. Here, two opposite funerals are being looked closer at. These are the funerals of two of the most important men in the trilogy: Rasmus and Paul. In Rasmus´funeral, there is no room for anything (or anyone) that might be associated with homosexuality and AIDS – a queer lifestyle ­– at all, and the boyfriend and all the other friends of the deceased Rasmus are not welcome. Everything queer is banished. The other funeral, Paul´s funeral, celebrates everything that is different, and here, it is the heteronormative, conservative views and thoughts that are being pushed aside. This funeral brings hope (and power) to the queers.

Le travail présenté dans ce manuscrit traite des mécanismes d'émergence des maladies infectieuses. Une première partie du texte est consacrée à la réflexion menée sur ces processus, et à leurs conséquences pour la gestion des maladies. Dans une deuxième partie nous exposons les travaux de modélisation réalisés ; l’objectif est de tester si des changements dans l'expression d'une infection peuvent être expliqués par la dynamique de circulation de l'agent infectieux dans la population hôte, à travers la protection de groupe. Nous montrons que l'apparition de la maladie de Carré chez les lions du Serengeti peut être due à une perte d’immunité de groupe suite à l’extinction du virus. Nous étudions ensuite les conséquences sur le patron épidémique d'une protection partiellement croisée entre souches. Enfin, nous intégrons à cette problématique la structure spatiale de la population hôte, pour expliquer des observations faites sur les systèmes lagomorphes/lagovirus en France. / The study presented here aims to suggest and to test several mechanisms to explain how infectious diseases impact could increase, in relation to microparasite circulation and loss of herd protection. We first introduce the main characteristics of host/microparasite interaction and display some knowledge about emerging infectious diseases, and their consequences for diseases control. Stochastic modelling is then used to study epidemic patterns under several hypotheses and for different host/parasite systems: i) we explain canine distemper emergence in Serengeti lions through a break in virus circulation and the resulting loss of herd immunity; ii) we study the consequences of partial crossprotection induced in hosts by different strains in a small homogeneous host population; and iii) we extend this study to spatially structured host populations to explain data observed in lagomorphs/lagoviruses in France.

Spatial structure of complex network and diffusion dynamics / Structure spatiale du réseau complexe et dynamique de diffusion

Dans le développement récent des sciences de réseau, réseaux contraints spatiales sont devenues un objet d'une enquête approfondie. Spatiales des réseaux de contraintes sont intégrées dans l'espace de configuration. Leurs structures et les dynamiques sont influencées par la distance spatiale. Ceci est prouvé par les données empiriques de plus en plus sur des systèmes réels montrant des lois exponentielles ou de distribution d'énergie distance spatiale de liens. Dans cette thèse, nous nous concentrons sur la structure de réseau spatial avec une distribution en loi de puissance spatiale. Plusieurs mécanismes de formation de la structure et de la dynamique de diffusion sur ces réseaux sont pris en considération. D'abord, nous proposons un réseau évolutif construit en l'espace de configuration d'un mécanisme de concurrence entre le degré et les préférences de distance spatiale. Ce mécanisme est décrit par un a^'fc- + (1 — a)^'lL_,1, où ki est le degré du noeud i et rni est la distance spatiale entre les noeuds n et i. En réglant le paramètre a, le réseau peut être fait pour changer en continu à partir du réseau spatiale entraînée (a = 0) pour le réseau sans échelle (a = 1). La structure topologique de notre modèle est comparé aux données empiriques de réseau de courrier électronique avec un bon accord. Sur cette base, nous nous concentrons sur la dynamique de diffusion sur le réseau axé sur spatiale (a — 0). Le premier modèle, nous avons utilisé est fréquemment employée dans l'étude de la propagation de l'épidémie: ['spatiale susceptible-infecté-susceptible (SIS) modèle. Ici, le taux de propagation entre deux noeuds connectés est inversement proportionnelle à leur distance spatiale. Le résultat montre que la diffusion efficace de temps augmente avec l'augmentation de a. L'existence d'seuil épidémique générique est observée, dont la valeur dépend du paramètre a Le seuil épidémique maximum et le ratio minimum fixe de noeuds infectés localiser simultanément dans le intervalle 1.5 < a < 2.Puisque le réseau spatiale axée a bien défini la distance spatiale, ce modèle offre une occasion d'étudier la dynamique de diffusion en utilisant les techniques habituelles de la mécanique statistique. Tout d'abord, compte tenu du fait que la diffusion est anormale en général en raison de l'importante long plage de propagation, nous introduisons un coefficient de diffusion composite qui est la somme de la diffusion d'habitude constante D des lois de la Fick appliqué sur différentes distances de transfert possibles sur le réseau. Comme prévu, ce coefficient composite diminue avec l'augmentation de a. et est une bonne mesure de l'efficacité de la diffusion. Spatial constrained networks are embedded in configuration space. Their structures and dynamics are influenced by spatial distance. This is proved by more and more empirical data on real Systems showing exponential or power laws spatial distance distribution of links. In this dissertation, we focus on the structure of spatial network with power law spatial distribution. Several mechanisms of structure formation and diffusion dynamics on these networks are considered. First we propose an evolutionary network constructed in the configuration space with a competing mechanism between the degree and the spatial distance preferences. This mechanism is described by a ki + (1 — a), where ki is the degree of node i and rni is the spatial distance between nodes n and i. By adjusting parameter a, the network can be made to change continuously from the spatial driven network (a = 0) to the scale-free network (a = 1). The topological structure of our model is compared to the empirical data from email network with good agreement. On this basis, we focus on the diffusion dynamics on spatial driven network (a = 0). The first model we used is frequently employed in the study of epidemie spreading : the spatial susceptible-infected-susceptible (SIS) model. Here the spreading rate between two connected nodes is inversely proportional to their spatial distance. The result shows that the effective spreading time increases with increasing a. The existence of generic epidemic threshold is observed, whose value dépends on parameter a. The maximum épidemic threshold and the minimum stationary ratio of infected nodes simultaneously locate in the interval 1.5 < a < 2. Since the spatial driven network has well defined spatial distance, this model offers an occasion to study the diffusion dynamics by using the usual techniques of statistical mechanics. First, considering the fact that the diffusion is anomalous in general due to the important long-range spreading, we introduce a composite diffusion coefficient which is the sum of the usual diffusion constant D of the Fick's laws applied over different possible transfer distances on the network. As expected, this composite coefficient decreases with increasing a and is a good measure of the efficiency of the diffusion. Our second approach to this anomalous diffusion is to calculate the mean square displacement (l²) to identify a diffusion constant D' and the degree of thé anomalousness y with the help of the power law {l²} = 4D'ty. D' behaviors in the same way as D, i.e., it decreases with increasing a. y is smaller than unity (subdiffusion) and tends to one (normal diffusion) as a increases.

Glycoconjugates : synthesis and investigation of carbohydrate-protein interactions

To study the functions of glycoconjugates in biological systems reliable and efficient protocols for glycoconjugate synthesis are needed. To reach this goal we have developed methods for solid-phase synthesis of glycoconjugates that can be monitored with gel-phase 19F spectroscopy using fluorinated linkers, building blocks, and protecting groups. We have developed a new fluorine containing linker suitable for solid-phase synthesis of glycoconjugates. The linker was more acid-labile than similar linkers in order to enable cleavage under mild conditions of the target compound from the linker resin.  A carbamate-based strategy has been applied to attach a spacer carrying an amino group to a fluorinated Wang linker for synthesis of amino-functionalized glycoconjugates using thioglycoside donors with fluorinated protective groups. Cleavage from the solid support was performed with trifluoroacetic acid and subsequent protecting group removal gave the target compound. The terminal amine was conjugated with didecyl squarate and this derivative can be attached to various proteins and solid surfaces carrying primary or secondary amines. To evaluate this methodology we have immobilized glycoconjugates in amino-functionalized microtiter plates and successfully probed them with lectin. In addition, a novel fluorine containing protecting group has been designed, synthesized and evaluated. The protecting group was used for protection of the unreactive 4-OH in a galactose building block that was applied in the synthesis of 6-aminohexyl galabioside and was removed with TBAF in THF. Adenovirus serotype 8 (Ad8), Ad19, and Ad37 cause the severe ocular infection, epidemic keratoconjunctivities (EKC). During infection, the adenoviruses interact with sialic acid containing glycoconjugates on the epithelial cells via fiber structures extending from the viral particles. The virus particle most likely binds to the host cell in a multivalent way by simultaneously using multiple fiber proteins and binding sites. Multivalent sialic acid containing conjugates could efficiently inhibit Ad37 cell attachment and subsequent infection of human corneal epithelial (HCE) cells. Three compact tri- and tetravalent sialic acid conjugates were prepared and evaluated as inhibitors of adenoviral host cell attachment and subsequent infection and all conjugates were potent as anti-adenoviral agents. The conjugates can readily be synthesized from accessible starting materials. A crystal structure of the Ad37 fiber knob protein and the trivalent sialic acid conjugate showed that the three binding sites were all occupied by one sialic acid residue each.

Το πρόβλημα της κλιματικής αλλαγής αποτελεί έναν από τα κυριότερα σημεία έντονου ενδιαφέροντος για τις περισσότερες χώρες. Μάλιστα, τα επόμενα χρόνια αναμένεται να δοθεί μεγαλύτερη προσοχή στην ανάπτυξη πολιτικών που θα μειώνουν τις εκπομπές ρυπογόνων αέριων ρύπων. Η εξοικονόμηση ενέργειας, ως μέτρο πολιτικής, θα συνεχίσει να αποτελεί μια σημαντική στρατηγική ανάπτυξης για την οικονομία της χώρας μας, μια και συνδέεται σε σημαντικό βαθμό, με την κατανάλωση ενέργειας όπως και με την μείωση των εκπομπών αερίων ρύπων. Επιπλέον, συντονισμένες προσπάθειες τόσο από την Ευρωπαϊκή Ένωση όσο και από άλλους οργανισμούς (IEA, OECD) θέτουν σε βασικό άξονα προτεραιότητας την μείωση της κατανάλωσης ενέργειας, την χρησιμοποίηση εναλλακτικών μορφών και ανανεώσιμων πηγών και την μείωση των ρυπογόνων εκπομπών ιδιαίτερα στον βιομηχανικό κλάδο. Στα πλαίσια της παρούσας διατριβής αναλύονται θέματα που σχετίζονται με την υιοθέτηση τεχνολογιών εξοικονόμησης ενέργειας από Βιομηχανικές επιχειρήσεις. Κεντρικό στοιχείο αυτής της προσέγγισης είναι το “Παράδοξο της Ενεργειακής Αποδοτικότητας”. Τρία συγκεκριμένα θέματα εξετάζονται σε αυτή την κατεύθυνση. Πρώτον, η διερεύνηση των παραγόντων που οδηγούν στην εμφάνιση του “παραδόξου της ενεργειακής αποτελεσματικότητας” και συγκεκριμένα αν οι αποφάσεις των επιχειρήσεων για υιοθέτηση τέτοιων τεχνολογιών συνυπολογίζουν το στοιχείο της αποδοτικότητας των επενδεδυμένων κεφαλαίων. Δεύτερον, και με δεδομένο ότι στα αποτελέσματα του προηγούμενου σταδίου ανάλυσης αναδεικνύουν την σημαντικότητα του παράγοντα της πληροφορίας, αναπτύσσεται μια εκτενής προσέγγιση που αφορά τόσο το περιεχόμενο όσο και τον ρόλο του παράγοντα της πληροφορίας στην διαδικασία υιοθέτησης ΤΕΕ. Τρίτο, διερευνάται η επίδραση της υιοθέτησης των ΤΕΕ στην παραγωγική αποτελεσματικότητα και παραγωγικότητα των βιομηχανικών επιχειρήσεων. Για τις ανάγκες της ανάλυσης αυτών των θεμάτων αναπτύσσονται δύο επιμέρους μικροοικονομικά υποδείγματα και μια μέθοδος μέτρησης της παραγωγικότητας σε ετερογενής τεχνολογίες. Το πρώτο μικροοικονομικό υπόδειγμα διερευνά την διαδικασία λήψης επενδυτικών αποφάσεων σε ΤΕΕ υπό το πρίσμα της συσχέτισης της επενδυτικής επιλογής με την κερδοφορία, σε πλαίσιο μερικής παρατηρησιμότητας. Το δεύτερο μικροοικονομικό υπόδειγμα επανατοποθετεί την έννοια της πληροφορίας και διερευνά τους παράγοντες που προσδιορίζουν το επίπεδο πληροφόρησης της επιχείρησης για ΤΕΕ. Τέλος για την μέτρηση της επίδρασης των ΤΕΕ στην παραγωγική αποτελεσματικότητα και παραγωγικότητα αναπτύσσεται μια μέθοδος που λαμβάνει ρητά υπόψη της την τεχνολογική ετερογένεια. Η διερεύνηση των τριών αυτών ζητημάτων βασίζεται στην ανάλυση εμπειρικών δεδομένων που αφορούν επιχειρήσεις οι οποίες ενέταξαν στην παραγωγική τους διαδικασία τεχνολογίες εξοικονόμησης ενέργειας στην περίοδο 1990-2004. Οι επενδύσεις αυτές επιδοτήθηκαν κυρίως στα πλαίσια του Β’ και Γ’ Κοινοτικού Πλαισίου Στήριξης. Τα δεδομένα συλλέχθηκαν με την μέθοδο των προσωπικών συνεντεύξεων (ερωτηματολόγια). Συμπληρωματικά δεδομένα αντλήθηκαν από την βάση δεδομένων της ICAP. / The improvement for energy efficiency is generally viewed as an important option to reduce greenhouse gas emissions and environmental damage caused by other pollutants (e.g. NOX,SOX). Moreover, is clearly interwoven with the exploitation of new and innovative technologies through the production process and its consequent paradox, the so called “energy efficiency paradox”. This paradox has recently attracted the interest of researchers and organizations (IEA,OECD) in an attempt to bring to light the source of it, the causalities between the adoption of energy efficient technology (EET) and the behaviour of firms . Three research questions have been examined in the specific Phd Thesis. Our first main research question were examined by formulating and testing the following hypothesis: the decision of the firms to adopt or not EET, is correlated to their profitability. Our second research project develops in two stages. The first stage aims at examining the factors influencing retrieval of information concerning EETs by manufacturing firms, while at the second stage we distinguish between readily available and emerging energy efficiency technologies and examine the factors affecting information acquisition for each one of these two broad sets of technologies. Finally, in order to disentangle firm’s heterogeneity we developed a methodological framework to calculate total factor productivity and its components differences arising from EETs adoption. Our first research question examines the energy efficiency paradox demonstrated in Greek manufacturing firms through a partial observability approach. Maximum likelihood estimates that arise from an incidental truncation model reveal that the adoption of the energy saving technologies is indeed strongly correlated to the returns of assets that are required in order to undertake the corresponding investments. The source of the energy efficiency paradox lies within a wide range of factors. Policy schemes that aim to increase the adoption rate of energy saving technologies within the field of manufacturing are significantly affected by differences in the size of firms. Finally, mixed policies seem to be more effective than polices that are only capital subsidy or regulation oriented. Answering the second research question, we aim to redefine the notion of awareness regarding the adoption of EETs. In a second stage we explore the crucial factors that affect the information level of EET adopters, distinguishing between epidemic and emerging technologies information. Our empirical findings reveal that the main factor that exerts positive influence on the level of information acquired by the firms may be encompassed in a set of variables that reflect what may be called a “business culture” regarding the EET Finally, we examined the impact of EETs adoption to Greek manufacturing firms operating under heterogeneous technology sets and we measured the components of total factor productivity (TFP) and its components arising from scale and technological differences. In order to examine our research questions we formulate a unique database. Our database came to light from the necessity of the Greek government to conserve energy in manufacturing and to reduce dangerous emissions in order to meet the criteria of the Kyoto Protocol. An extensive questionnaire was addressed to the 298 firms across the country that adopt EETs that have been subsidized from (i) the Support Frameworks for Regional and Industrial Development, (ii) the Energy Operational Program (OPE), which was part of the second European Union Support Framework (1994-2000) and (iii) the Operational Program ‘Competitiveness’, which is part of the third European Union Support Framework (2000-2006). Finally, 161 of them agreed to be interviewed on the basis of the questionnaire. Face to face interviews took place in the first six months of 2004. Epidemic models and inference for the transmission of hospital pathogens

The primary objective of this dissertation is to utilise, adapt and extend current stochastic models and statistical inference techniques to describe the transmission of nosocomial pathogens, i.e. hospital-acquired pathogens, and multiply-resistant organisms within the hospital setting. The emergence of higher levels of antibiotic resistance is threatening the long term viability of current treatment options and placing greater emphasis on the use of infection control procedures. The relative importance and value of various infection control practices is often debated and there is a lack of quantitative evidence concerning their effectiveness. The methods developed in this dissertation are applied to data of methicillin-resistant Staphylococcus aureus occurrence in intensive care units to quantify the effectiveness of infection control procedures. Analysis of infectious disease or carriage data is complicated by dependencies within the data and partial observation of the transmission process. Dependencies within the data are inherent because the risk of colonisation depends on the number of other colonised individuals. The colonisation times, chain and duration are often not visible to the human eye making only partial observation of the transmission process possible. Within a hospital setting, routine surveillance monitoring permits knowledge of interval-censored colonisation times. However, consideration needs to be given to the possibility of false negative outcomes when relying on observations from routine surveillance monitoring. SI (Susceptible, Infected) models are commonly used to describe community epidemic processes and allow for any inherent dependencies. Statistical inference techniques, such as the expectation-maximisation (EM) algorithm and Markov chain Monte Carlo (MCMC) can be used to estimate the model parameters when only partial observation of the epidemic process is possible. These methods appear well suited for the analysis of hospital infectious disease data but need to be adapted for short patient stays through migration. This thesis focuses on the use of Bayesian statistics to explore the posterior distributions of the unknown parameters. MCMC techniques are introduced to overcome analytical intractability caused by partial observation of the epidemic process. Statistical issues such as model adequacy and MCMC convergence assessment are discussed throughout the thesis. The new methodology allows the quantification of the relative importance of different transmission routes and the benefits of hospital practices, in terms of changed transmission rates. Evidence-based decisions can therefore be made on the impact of infection control procedures which is otherwise difficult on the basis of clinical studies alone. The methods are applied to data describing the occurrence of methicillin-resistant Staphylococcus aureus within intensive care units in hospitals in Brisbane and London

The participants included 12 paternal and maternal grandmother-caregivers from four districts of Gutu, whose ages ranged from 56 to 76 years with orphans in their care ranging from infants to 18 years. The present study made use of Erikson's psychosocial theory of development on late adulthood. Data were gathered using semi-structured open-ended interviews in the participants' homes. Interpretive analysis was used to analyse the audio-taped data. Findings reveal that most grandmothers are experiencing a personal toll in dealing with the late adult crisis of integrity versus despair, including finding it difficult to resolve the grief of losing children while engaging in full time grandparenting in a stigmatising society. Participants reported a need for support and interventions tailored to their unique needs. Counselling, social support, financial assistance, and skills and knowledge about HIV/AIDS are therefore recommended. / Psychology / M.Sc. (Psychology)
