Metodologia: Os dados do presente trabalho foram obtidos do Projeto SB Brasil 2003 (MS, 2004) que incluiu no estudo epidemiológico todas as capitais brasileiras. Foi realizada amostragem probabilística por conglomerados para a seleção das escolas e das crianças. Resultados: Foram examinadas 102 crianças de 12 anos das escolas publicas de Manaus. O CPO-D em Manaus (2003) foi de 3,20 (±3,93), sendo dentes cariados o maior componente do índice (54,5%). A prevalência de fluorose nos escolares foi de 8%. Conclusão: No presente estudo o CPOD encontrado não atingiu a meta da OMS para o ano de 2000 (escolares aos 12 anos - CPOD '< ou =' 3,0) e ficou distante de atingir a meta da OMS para 2010 (CPOD < 1,0), necessitando assim implementação de atividades preventivas e curativas para saúde bucal destes escolares, bem como constante monitoramento destas condições, e a utilização de um método coletivo de uso do fluoreto. A maioria dos escolares aos 12 anos estavam livres de fluorose (92%). Capitulo 2 - Objetivo: Verificar a prevalência das alterações de coloração de esmalte de caráter extrínseco bem como a sua relação com a qualidade da agua na cidade de Caapiranga/AM. Observação: O resumo, na íntegra, poderá ser visualizado no texto completo da tese digital / Abstract: Chapter 1 - Goal: Understand the oral health status of the 12 year old scholars in relation to dental caries and fluorosis in Manaus-AM, which has no fluoridation of public water supply in the year 2003. Methodology: The data in this paper was obtained from the SB Brazil Project 2003 (MS, 2004) that included, in this epidemiological study, all Brazilian capitals. Was performed by cluster random sampling for selection of schools and children, allowing the production of inferences. Results: We examined 102 12 year old children of public schools in Manaus. DMFT in Manaus (2003) was 3.20 (+ or - 3.93), and decayed teeth the largest component of the index (54.5%). The prevalence of fluorosis in children was 8%, and only 6 students had very mild fluorosis (5.88%) and 01 mild fluorosis (0.98%). Conclusion: In this study the DMFT found did not reach the WHO target for the year 2000 (school for 12 years - DMFT < or - 3.0) and was far from attaining the goal of WHO in 2010 (DMFT <1.0), thus requiring the implementation of preventive and curative oral health for these students, as well as constant monitoring of these conditions, and using a collective method of use of fluoride. The distribution of fluoride in South African groundwater and the impact thereof on dental health

The most appropriate and widely used source of drinking water for the rural populations of South Africa is groundwater. Pilot studies and surveys conducted by the Department of Water Affairs and Forestry (DW AF) indicated that there are a number of boreholes across the country that contain apart from fluoride, levels of nitrate, some heavy metals, total dissolved solids, sulphates and faecal coliform (in isolated regions) that could pose a health risk if the water is used for drinking purposes. Very few boreholes have been tested for heavy metals or toxic organic substances. However considering the levels of fluoride, in general, groundwater is of acceptable quality except for some provinces in which elevated levels of natural groundwater fluoride occurs. Very high levels of fluoride, >4 mg/l occur in some groundwater sources in all nine provinces of South Africa, especially in the Limpopo, North-West, Eastern Cape, Northern Cape, Western Cape and KwaZulu Natal provinces. A superficial inspection reveals that most of the local people in those areas suffer from dental fluorosis at varying degrees. The main aim of this study is to determine the distribution of the fluoride ion concentration levels in South African groundwater and the impacts thereof on dental health. The available data is used to assess the distribution of the various fluoride ion concentration levels in some national groundwater sources. Areas of particularly high or low fluoride levels are identified. Results from an epidemiological survey carried out by the National Department of Health (NDOH) are used concurrently with the fluoride data to determine the percentage morbidity of dental fluorosis in each area The results are compared in order to determine if any relationship exists between the occurrence of fluoride in drinking water and the incidences of dental fluorosis. Vegter's lithostratigraphy and the simplified geology of South Africa are used to interpret the results and assess the role of surface geology in the release and distribution of fluorides in groundwater. Comparing two orthodontic brackets’ bond to fluorosed and non-fluorosed enamel - an in vitro study

Orthodontic attachments must be able to bond to a wide range of tooth and prosthetic surfaces. Despite the high prevalence of fluorosis in many parts of South Africa (Louw A, Chikte U 1997), only limited information is available on the integrity of the bond between orthodontic brackets and fluorosed teeth. The objective of this study was to measure and compare Shear Bond Strengths (SBSs) of metal and ceramic orthodontic brackets on fluorosed and non-fluorosed teeth. One hundred and twenty (60 fluorosed and 60 non-fluorosed) extracted premolar teeth were divided into four groups A to D, consisting of 30 teeth in each group. BluGloo® was used as an orthodontic adhesive to bond brackets on the buccal surface of each tooth. The experimental groups consisted of Group A, in which Nu-Edge® metal brackets were used and Group B, in which InspireIce® ceramic brackets were bonded to fluorosed teeth. Group C and D consisted of Nu-Edge® metal brackets and InspireIce® ceramic respectively, bonded to non-fluorosed teeth. Bonding techniques were kept the same and standardised for all four groups. An Instron testing device was used to debond and measure the SBSs. SBSs were compared using ANOVA with posthoc analysis done using Dunnett’s C test for pairwise comparisons. Significance was set at P<0.05. The results showed that SBS of Group B>Group C>Group D>Group A. Ceramic brackets bonded to fluorosed teeth had the highest SBS with a mean of 15.78 (SD=9.07) Megapascals (MPa), while metal brackets bonded to fluorosed teeth produced the lowest SBS of 8.41 (SD=4.68) MPa. The SBSs of ceramic brackets bonded to fluorosed teeth was significantly higher than that of SBS of metal brackets bonded to fluorosed teeth, but not significantly different from SBSs obtained from either brackets bonded to non-flurosed teeth. The BluGloo adhesive if used to bond ceramic brackets to fluorosed teeth can produce adequate SBS for clinical use. The recommendation from this study is that ceramic brackets can be used efficiently to bond to fluorosed teeth. A follow up study should be carried out to assess the nature of enamel damage caused during debonding of flourosed teeth. The genetic susceptibility/resistance to fluorosis among different inbred mouse strains

Indiana University-Purdue University Indianapolis (IUPUI) / Fluoridation of community water supplies for the purpose of preventing dental caries remains one of the top 10 public health interventions of the last century. However, exposure (ingestion) of greater than optimal amounts of fluoride from a variety of sources has led to an increase in the prevalence of dental fluorosis. We propose that dental fluorosis represents a complex condition caused by environmental and genetic factors. Purpose: To assess the role of genetics in the pathogenesis of dental fluorosis using genetically separate inbred strains of mice. Methods: Twelve genealogically disparate strains of mice were treated with 0 ppm, 25 ppm, and 50 ppm of fluoride in their drinking water. Each mouse was given weekly dental fluorosis evaluations. After 60 days of treatment, femurs were collected for fluoride analysis. Mandibular incisors were isolated for quantitative light induced fluorescence (QLF) studies and fluoride analysis. Digital and 35 mm images were taken of all mouse incisors in order to apply and compare the Dean's Index and the modified Thylstrup and Fejerskov Index (TFI), both indices of dental fluorosis. Skeletal radiographs were taken on the euthanized mice and later examined for extra skeletal calcifications and other gross bony deformities. Results: Differences in dental fluorosis susceptibility/resistance were identified between the strains, ranging from mild, moderate, to severe dental fluorosis. Furthermore, we found clustering of strains into distinct phenotypic groups. The A/J mouse strain was highly susceptible, with a rapid onset and severe development of dental fluorosis compared with the other strains tested. The 129P3/J mouse strain was least affected with negligible dental fluorosis. From the skeletal radiographs, no gross skeletal lesions or evidence of bone dysplasia were noted. Similar body burden of fluoride, as judged from analysis of mineralized tissues, was seen in all strains despite differences in their predispositions to develop dental fluorosis. Both the Dean's and TF indices are useful for classifying the stage or severity of fluorosis in mice, and there are advantages to the use of digital images over conventional 35 mm slide images. Both indices correlate well with the amount of fluoride exposure during amelogenesis; however, these indices are not promising indicators of fluoride burden during amelogenesis. Conclusions: QLF proved to be an innovative and useful tool for the quantification of dental fluorosis. Furthermore, these observations support the role of a genetic component in the pathogenesis of fluorosis.

El objetivo de este estudio fue evaluar el conocimiento sobre fluorosis en Cirujanos Dentistas en Lambayeque, 2020. El estudio tiene un enfoque tipo cuantitativo, descriptivo, prospectivo, transversal y observacional. La población total fue de 210 cirujanos dentistas en donde se realizó a través de un programa Fisterra y se aplicó el tamaño muestral, quedando una muestra ajustada a las pérdidas de 122. Asimismo, se realizó un muestreo aleatorio simple, aplicando el 10% para la prueba piloto quedando 12 participantes. Por tanto, el estudio fue aplicando una encuesta virtual a 110 (100%) cirujanos dentistas, de los cuales, solo respondieron 103 (93%). El análisis estadístico se realizó utilizando Kuder Richardson KR-20, donde se aplicaron las pruebas estadísticas Chi Cuadrado. Se encontró la adecuación y el poder predictivo de 0.706 una confiabilidad aceptable, mostrando que el instrumento fue confiable. Además, se realizó la comparación de conocimiento según sexo, años de experiencia y especialidad, en donde no se encontraron diferencias significativas, ya que estos factores no son tan relevantes para obtener una buena capacidad de conocimientos. Este estudio proporcionará información para estudios posteriores en cuanto a fluorosis dental, para mejorar el conocimiento. Para finalizar, se necesita esfuerzos para crear conciencia en la comunidad odontológica para que estén informados en cuanto a esta patología bucal que cada día es más común en la cita odontológica. Se concluyó que el conocimiento sobre fluorosis en cirujanos dentistas es regular, y que no existe significancia estadística.

Ion specific analysis and laboratory techniques were used to determine the fluoride content of the water from 517 sources. The teeth of 3 068 children were examined clinically and photographically. The fluorosis was scored according to the Tooth Fluorosis Index and Dean's classification. Graphical techniques, descriptive statistics and nonparametric analysis of variance were used to describe the occurrence and variation patterns of the key variables in the different spatial units is each study area. The relation between the variables as well as their intra and interareal covariation were analysed. The fluoride content of the groundwater in the Pilanesberg and Hammanskraal areas was relatively high to high; in the southern Free State it was relatively low. Spatial variability and significant differences over small distances typify the fluoride content of the groundwater, irrespective of the aquifer. Central statistics inadequately describe the fluoride quality of the natural drinking water. The development of new water sources causes long term variation in the fluoride content of the available drinking water. Fluorosis prevalence varied form 62% to 87%, with more severe fluorosis in Pilanesberg and Hammanskraal than in the southern Free State. Significant intra-areal spatial variation is an attribute of fluorosis in all three study areas. In all cases the variation pattern in the fluoride content of the drinking water partly explained the spatial pattern in the occurrence of fluorosis. Fluoride in surface water and groundwater in southeast Sweden : sources, controls and risk aspects

The aim of this thesis is to determine the sources, controls and risk aspects of fluoride in surface water and groundwater in a region of southeastern Sweden where the fluorine-rich 1.45 Ga circular Götemar granite (5 km in diameter) crops out in the surrounding 1.8 Ga granites and quartz monzodiorites (TIB rocks). The materials of this thesis include both primary data, collected for the purpose of this thesis, and a large set of secondary data, retrieved from the Swedish Nuclear Fuel and Waste Management Co., the Swedish Geological Survey and the Kalmar County Council. A characteristic feature of the area is high fluoride concentrations in all kinds of natural waters, including surface waters (such as streams) and groundwater in both the Quaternary deposits (regolith groundwater) and bedrock fractures (fracture groundwater). A number of potential sources and controls of the high fluoride concentrations were investigated, including a variety of geological, mineralogical, mineral-chemical and hydrological features and processes. For the stream waters and regolith groundwater, high fluoride concentrations were correlated with the location of the Götemar granite. This finding is explained by the discharge of fluoride-rich groundwater from fractures in the bedrock and/or the release of fluoride due to the weathering of fluorine-bearing minerals in the Quaternary deposits; however, the Quaternary deposits had considerably lower fluoride concentrations than the underlying bedrock. The high fluoride concentrations in the fresh fracture groundwater (up to 7.4 mg/L) in the TIB-rocks are proposed to be the result of long residence times and the alteration/dissolution of fluorine-bearing primary and secondary minerals along the fracture walls. In terms of risk aspects, this thesis shows that fluoride can add to the transport and inorganic complexation of aluminium in humic-rich, acidic streams. Additionally, 24 % of the children in households with private wells in Kalmar County were assessed to be at risk of excess fluoride intake based on the WHO drinking water guideline value (1.5 mg/L). However, the risk increased significantly when instead the US EPA reference dose (0.06 mg/kg-day) was used, both when all relevant exposure pathways were taken into account as well as water consumption alone. Hence, it is shown that the risk of an excess intake of fluoride is strongly dependent on the basis for evaluation.

Pesquisa do Estado da Bahia - FAPEB / Introduction: Dental fluorosis is an alteration in dental enamel mineralization, related to fluoride (F?) intake. Objectives: To investigate the impact of F? addition in toothpaste on the prevalence of dental fluorosis and to compare methods for F? determination in samples of diet and toothpaste. Methods: Two studies were developed: 1) systematic review of the literature, with meta-analysis, performed by two reviewers independently searching the four electronic databases (MEDLINE / PubMed; MEDLINE / Ovid; Bireme and Web of Science). The quality of articles eligible for Strengthening the Reporting of Observational Studies in Epidemiology (STROBE), and by the classification modified by Irani (2013); 2) a laboratory study that determined the F? concentration in the diet and dentifrice samples using two extraction methods: HMDS facilitated microdiffusion and hydrolysis in 0.01M HCl at times 30, 60 and 120 min of incubation. For analysis of the data, the one-way ANOVA test was applied. Results: 1) In the systematic review, eleven articles were eligible, and, when STROBE was applied, the number of items included ranged from 14 and 19, with an average of 16.3 and a standard deviation of 4.03; in the analysis of the classification modified by Irani (2013), two articles were classified as high quality and the rest of intermediate quality; 2) the method of extraction using 0.01M HCl, in the time 30 min, resulted in similar values to that expected by inactivation of 50% of the dentifrice?s F?. And in time 120 min resulted in similar values to HMDS facilitated microdiffusion. Conclusion: The majority of eligible articles in the systematic review found an association between the age of fluoride toothpaste use and dental fluorosis, but the prevalent severity of fluorosis was very mild, ie it does not imply dental fluorosis as a public health problem. It's the method of F? extraction using 0.01M HCl, in the experimental study, was promising to be used in evaluation of F? in dietary intake and dentifrices. / Introdu??o: Fluorose dent?ria ? uma altera??o na mineraliza??o do esmalte dental relacionada ? ingest?o de fluoreto (F?). Resultados: 1) Na revis?o sistem?tica, onze artigos foram eleg?veis, e quando aplicado o STROBE o n?mero de itens inclu?dos variou entre 14 e 19, com m?dia de 16,3 e desvio padr?o 4,03; na an?lise da classifica??o modificada por Irani (2013), dois artigos foram classificados como de alta qualidade e o restante de qualidade intermedi?ria. 2) A t?cnica de extra??o em HCl 0,01M no tempo 30 minutos resultou em valores semelhantes ao esperado pela inativa??o de 50% do F? do dentifr?cio. E o tempo de 120 minutos resultou em resultados semelhantes ? t?cnica de microdifus?o facilitada por HMDS. Conclus?o: A maoiria dos artigos eleg?veis da revis?o sistem?tica encontraram associa??o entre idade do uso do dentifr?cio fluoretado e fluorose dental, por?m a severidade prevalente da flurose foi o grau muito leve, ou seja, n?o implica considerar fluorose dental como um problema de sa?de p?blica. E o estudo laboratorial sugere que o m?todo de extra??o de F? em HCl 0,01M ? promissor para ser utilizado em trabalhos de avalia??o da ingest?o de F? pela dieta e dentifr?cios.
