Incorporation of the traditional healers into the national health care delivery system / Martha Gelemete Pinkoane

The process for the incorporation, integration or collaboration of traditional healers into the National Health Care Delivery System of South Africa was marred by an array of mixed attitudes from all the parties concerned, namely traditional healers, patients, biomedical personnel, and the policy makers. The variety of approaches for inclusion of the traditional healers into the National Health Care System of South Africa was a further indication of the complexity of the situation. The possibility of functioning together between traditional healers and biomedical personnel existed before 1990 when the two groups met in Johannesburg in 1986 to discuss ways by which functioning together can be established. A series of meetings and discussions followed after which came the promulgation of the Chiropractors Homeopaths and Allied Health Services Professionals Act of 1996, which gives traditional healers their due recognition but does not include them as part of health care providers. The process of functioning together is a recommendation made by the World Health Organization and the most used terms for this functioning together is, incorporation, integration and collaboration. The process of incorporation can be realised by ensuring that both biomedical personnel and traditional healers remain autonomous, not controlling each other, respecting the existence of one another, as well as each other's own methods of healing. Integration was another method whereby the two health care systems can function together, even though integration differs in context from incorporation. Integration means that the traditional healers will have to function within the health care system under the directions of the biomedical personnel, whereby the patient receives a combination of both treatment methods depending on the problem or diagnosis. The third modality of getting the two health care systems to function together could be by collaboration. Collaboration was seen as a two sided effort whereby the healing methods of one are brought to fore and the most effective one is chosen to cure the patient's identified problem at that time. For the process of functioning together to be meaningful, it was necessary to get the government to review licensing the traditional healer's practices, so as to identify the healing techniques that are of value and use these to treat the patients. It was not really possible to clearly separate the three approaches because they all addressed the issue of having the two health care systems function together to increase health care services and fulfil the patients' health needs. For the purpose of this research the word incorporation was used. In South Africa the traditional healer is identified as the health care choice of 80-9036 of the black population. If this large number of black people uses traditional healing, then it becomes necessary to investigate the manner in which the traditional healer can be utilized effectively in the National Health Care Delivery System of South Africa to render the services that the patient needs for his/her health needs. It is for this reason that the researcher aimed at investigating the existing models of incorporation of traditional healers, the perceptions and attitudes of the traditional healers, biomedical personnel, patients and the policy makers regarding incorporation, their views on how this incorporation should be achieved, as well as how the incorporation of traditional healers into the National Health Care Delivery System of South Africa could be realised. A qualitative research design and theory generating approach was followed, and the research was conducted in two stages. In stage one qualitative research, participants were traditional healers, biomedical personnel, patients and policy makers, selected by means of non-probable purposive voluntary sampling. Data was collected by means of conducting semi-structured interviews with all the participants in the three identified provinces of South Africa. Field notes were recorded after each interview session. Data analysis was achieved by open coding. A co-coder and the researcher analysed the data independently after which consensus discussions took place to finalise the analysed data. Ethical principles were applied according to the guidelines of the Democratic Nurses Organisation of South Africa and the Department of Health. Aloenxerto ósseo cortical desvitalizado com nitrogênio líquido – estudo experimental em ovelhas

Na ortopedia veterinária, o enxerto ósseo é comumente utilizado em uma variedade de procedimentos ou afecções ortopédicas como artrodeses, fusões espinhais, reparo de fraturas, tratamentos de união atrasada e não-união de fraturas, preenchimento de falhas ósseas, osteotomias corretivas e reconstruções após a ressecção de neoplasia óssea. O uso de enxerto ósseo alógeno, comparado com o enxerto autógeno, tem como vantagens menor morbidez e dor pós-cirúrgica, diminuição do tempo cirúrgico e, principalmente, fornecimento de maior volume ósseo para a reconstrução adequada de uma grande falha óssea. As desvantagens do seu uso são os riscos de reação imunológica e transmissão de doenças, além da necessidade da formação de um banco de ossos, o que aumenta os custos e necessita de espaço apropriado. O objetivo do presente trabalho foi avaliar a taxa e a forma de incorporação do aloenxerto ósseo cortical submetido ao congelamento em nitrogênio líquido e inserido em tíbias de ovelhas, visando à preservação do membro locomotor. Seis ovelhas clinicamente sadias, da raça Corriedale, com idade estimada entre dois e três anos, foram submetidas à avaliação radiográfica pré-operatória das tíbias para que fosse descartada qualquer alteração óssea. Simultaneamente, realizou-se a ostectomia da diáfise tibial de duas ovelhas para a retirada de um segmento de 7 cm que, após a remoção do periósteo e da medula óssea, foi imerso em nitrogênio líquido, tendo como finalidade a desvitalização óssea, e implantado imediatamente no outro paciente. A estabilização entre os fragmentos e o implante ósseo foi realizada com placa cirúrgica de compressão dinâmica (PCD). Após os procedimentos cirúrgicos, foram realizadas avaliações clínicas e radiográficas a cada 30 dias até o 180º dia de pós-operatório. Aos 60 dias de pós-operatório já se observava o completo uso funcional do membro operado. A união radiográfica das interfaces proximal e distal e a consolidação óssea ocorreram em tempo médio de 95 dias em todos os animais. Conclui-se que o aloenxerto ósseo cortical submetido ao congelamento em nitrogênio líquido é um método apropriado para a preservação do membro locomotor, tendo em vista que a incorporação do mesmo ocorreu gradualmente e a taxa foi de 100%. / Bone grafting is employed in veterinary orthopedics with a variety of purposes as for arthrodesis, spinal fusions, fracture repairs, treatment of delayed or non-unions, filling bone defects, supporting corrective osteotomy, and as replacement in limb sparing techniques for tumor resection. The allograft bone, in contrast to the autologous bone graft, is associated with less site-donor morbidity and pain, reduced surgical time, and adequate bone supply to fill greater defects. Still, the allogeneic bone, besides requiring the establishment of a bone bank, can be a source of diseases and may induce immunogenic response. This study evaluated the allogeneic cortical bone graft incorporation after submission of the harvested fragment to a bout freezing in liquid nitrogen. Six adult clinically healthy sheep were submitted to a 7cm ostectomy of the tibial diaphysis. After removal of periosteum and bone marrow, the fragment was submersed in liquid nitrogen and implanted in another sheep missing a same-sized segment at the corresponding bone. Stabilization of the allograft in the host bone was accomplished by a dynamic compressive plate (DCP). Clinical and radiographic evaluations were performed in the immediate post-operatory period and in every 30 days for six months after surgery. The proximal and distal host-graft interfaces showed radiographic union at a mean postoperative time of 95 days in all the animals. The cortical bone allograft submitted to liquid nitrogen freezing provided adequate bone healing in the sheep model, representing a feasible alternative for limb preservation techniques.

Die navorser maak van die k:walitatiewe navorsingsmetode gebruik deur individuele onderhoude met 'n aantal dosente te voer. Hierdie inligting is met behulp van 'n oudio-bandopnemer opgeneem en verbatim transkripsies is daarvan gemaak. Na die ontleding van die transkripsies is faktore geidentifiseer wat emstige stres by die deelnemers veroorsaak tydens die veranderingsproses. Ten slotte is aanbevelings in hierdie verband gemaak. / Change is a constant in all sphere of people's lives. To keep abreast of the changing world and to meet the needs of its people, vast changes are also being implemented in education in South Mrica. Institutions involved in teacher training have also undergone far-reaching restructuring over the past few years. Legislation requires that tertiary education institutions be incorporated into the Higher education sector, which means incorporation into a university or technikon. This study focuses on establishing how the lecturers employed at a tertiary education institution " experience this process of change. The researcher has used the qualitative research method by conducting individual interviews with a number oflecturers. This information was taped and verbatim transcriptions were made of the interviews. Analysis of these transcriptions made it possible to identify factors that have caused serious stress in participants during the process of change. Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / A presente pesquisa busca compreender o papel do Estado e do Capital Incorporador no processo de reestruturação urbana do bairro de Campo Grande, na cidade do Rio de Janeiro, através da produção de moradias. Para isto, parte das reflexões sobre a atuação destes dois agentes. O incorporador como responsável pela construção e comercialização de habitações, e o Estado apresentado como medidor de conflitos e provedor de políticas públicas voltadas para a requalificação urbana e solução da questão habitacional. Ambos, também agem no processo de valorização, onde a produção da mercadoria solo urbano e a renda da terra são atributos herdados pela ação destes agentes. Neste sentido, a expansão do capital de incorporação com o auxilio do Estado, principalmente com o advento do Programa Minha Casa, Minha vida, estimulou o crescimento da atuação do mercado imobiliário no bairro. Como fatores atrativos estimulado pelos investidores estão a importante centralidade construída ao longo do tempo e o atual e futuro subcentro planejado, ou seja, os shoppings West Shopping e o ParkShopping Campo Grande. Contudo, este processo de expansão imobiliária em curso no bairro vem atraindo novos moradores e maior atuação do capital privado, como a construção de torres empresariais. / This paper aims to understand the role of state and capital of incorporation in the process of urban restructuring in the neighborhood of Campo Grande in the city of Rio de Janeiro, through the production of housing. For this, start with considering the performance of these two agents. The incorporator as responsible for building and sale of homes. And the State presented as a measure of conflict and provider of public policies for the urban requalification and solving the housing issue. Both also act on the recovery process, where the production of the urban land and rent of land attributes are inherited from by the action of these agents. In this sense, the expansion of capital of incorporation with the assistance of the state, especially with the advent of the program My House, My life, stimulated the growth performance of the property market in the neighborhood. And as attractive factors stimulated by investors are important central built over time and the current and future planned sub-center, or malls West Shopping and the ParkShopping Campo Grande. However, this process of ongoing real estate boom in the neighborhood is attracting new residents and larger role of private capital, such as building towers.

Navigating the neoliberal settler city : Palestinian mobility in Jerusalem between exclusion and incorporation

The mobility of Palestinian residents of Jerusalem is usually understood in terms of exclusion, reflecting their lack of access to urban services more broadly as well as the restrictive mobility regime at work across the Palestinian territories. Yet after fifty years of Israeli occupation, a more complex and contradictory situation has emerged in the city. This dissertation uses mobility as a vehicle to arrive at a more integrated understanding of the paradoxical manner in which Palestinian Jerusalemites are simultaneously excluded from and incorporated into the city and to analyse how they negotiate their interstitial and often contradictory position. The thesis approaches the question of Palestinian quotidian movement by engaging with theoretical work on mobility and embodied movement as well as from empirical study including eight months of on-site research. In its three core sections, the work examines in detail several manifestations of the restriction, facilitation, and contested nature of mobility. In the first section, a discussion of Palestinian exclaves and enclaves of the city shows the continuities of mobility’s exclusionary effects on both sides of the Separation Wall. This limitation of movement leads to a restriction of spatio-political possibilities – but at the same time, Palestinians expand the horizon of what is possible through everyday and leisure practices. The second section employs two case studies of recent public transport developments in East Jerusalem to examine how incorporation is operationalised through everyday movements across urban space. The third section analyses the paradoxical role of mobility as the result of a tension between the settler colonial and the neoliberal logics concurrently at work in the city. On the one hand, the restriction of movement gradually renders the Palestinians as external to their city. On the other, the facilitation and regulation of mobility in East Jerusalem also serves to normalise Israeli rule and constitute Palestinians as incorporated urban residents, thereby undermining long-term aspirations for autonomy in the east of the city. The examination of the manner in which mobilities are contested in Jerusalem shows that movement, although often associated with freedom and independence, is essential for negotiating the terms of interdependence in the city.

Crossing Boundaries to Education: Haitian Transnational Families and the Quest to Raise the Family Up

Nearly 175, 000 Haitian immigrants have settled in South Florida since the 1970s. Their lives are often lived transnationally with persistent connections and obligations to family members in Haiti. Yet, traditional theories of immigrant assimilation focus on the integration of immigrants into host countries, giving little consideration to relationships and activities that extend into migrants' countries of origin. Conversely, studies of transnational families do not explicitly address incorporation into the receiving country. This dissertation, through the experiences of Haitian immigrants in South Florida, reveals a transnational quest “to raise the family up” through migration, remittances, and the pursuit of higher levels of education. I argue that familial duties and obligations, which have cultural foundations in the Haitian lakou, structure the activities of Haitian transnational families as they pursue socioeconomic advancement through migration and education. With the support of transnational families, many students cross boundaries to academic achievement and improve their opportunities for socioeconomic mobility in the US. With higher levels of education, these individuals contributed to a more favorable incorporation into the United States for their extended families, as well. The data were collected through participant observation and 78 in-depth interviews documenting the migration histories of 27 Haitian immigrant families in South Florida. This dissertation contributes to the existing literature on Haitian immigrants in the United States and to an understanding of the transnational dimensions of immigrant incorporation more broadly.

Among the other aspects of this theme, we focus the ideological and political process of elaboration of the positivist project of organization of the work relations, looking for to understand to which extent certain positivist ideals of this project had been incorporated and making up the historical construction of national labor law. Within the set of comtian positivist theory, the ideal of proletariat incorporation into the modern society occupies a core position intrinsically entailed to the political realization of the theory. According to Comte, become the working class integrated part of the capitalist society used to imply realizing the deep moral reform in the political and economic concepts proposed by positivism, reorganizing the society to the extent that turns harmonious the relations between the capital and work. In Brazil, the national positivists claim that the solution for the working issue had such a first challenge overcome the slavery. But the problem could not be solved simply with the abolition, it was necessary a project which integrated the former slavers into the society. Reforming the positivist theory by Augusto Comte from the national needs, the Brazilian positivists made up to the nation a reorganizing project of the working relations, which embraces a large proposal of social reform. Getting a spot in Rio Grande do Sul, such a theory was given an important political expression through Júlio de Castilhos and Borges de Medeiros period of governorship, reaching out the 1907 generation that took part in the revolution of 1930, and led by Getúlio Vargas assumed the pos-revolutionary government. The State configuration from 1930 revealed the assimilation of the positivist ideas of the proletariat incorporation, the social harmony, and the collaboration among the classes. The positivist project if constitutes, thus, in an important source of the ideological inspiration about the working right in Brazil. / O presente estudo teve como objetivo uma maior compreensão dos problemas relacionados a Questão Social no Brasil. Dentre os vários aspectos dessa temática, enfocamos o processo político e ideológico de elaboração do projeto positivista de organização das relações de trabalho, procurando compreender em que medida certos ideais deste projeto foram incorporados e permearam a construção histórica do direito trabalhista nacional. No conjunto da teoria positivista comtiana, o ideal de incorporação do proletariado à sociedade moderna ocupa uma posição central intrinsecamente vinculada à realização política do ideário. Para Comte, fazer do operariado parte integrante da sociedade capitalista implicava em realizar a profunda reforma moral nas concepções políticas e econômicas propostas pelo positivismo, reorganizando a sociedade na medida em que tornava harmoniosas as relações entre capital e trabalho. No Brasil, segundo os positivistas nacionais, a solução da questão do trabalho tinha como primeiro desafio superar a escravidão. Mas o problema não se esgotava com a mera abolição; era preciso um projeto que integrasse os ex-escravos à sociedade. Reformulando a teoria positivista de Augusto Comte a partir das necessidades nacionais, os positivistas brasileiros construíram para o país um projeto de reorganização das relações de trabalho que abarcava uma proposta ampla de reforma social. Encontrando espaço no Rio grande do Sul, tal ideário ganhou importante expressão política nos governos de Júlio de Castilhos e de Borges de Medeiros, chegando à geração de 1907 que participou da revolução de 1930. Sob a liderança de Getúlio Vargas, políticos dessa geração assumiram o governo pós-revolucionário. A configuração do Estado, a partir de 1930, revelava a assimilação das idéias positivistas de incorporação do proletariado, de harmonia social e de colaboração entre as classes. O projeto positivista se constitui, assim, em uma fonte importante de inspiração ideológica do direito trabalhista no Brasil.

Para avaliar os efeitos de incorporação do fósforo e dos outros corretivos, utilizaram-se dois manejos de solo: o preparo convencional e o cultivo mínimo. A pesquisa foi fundamentada em duas hipóteses: I) A aplicação de parte do fósforo em área total e a outra parte no fundo do sulco, em solos de textura média, proporcionam maior aproveitamento do fósforo e outros nutrientes a curto e longo prazo. II) A incorporação do fósforo e dos outros corretivos de solo proporcionam uma melhor condição para o desenvolvimento do sistema radicular. O delineamento experimental utilizado foi o de blocos inteiramente casualizados com parcelas subdivididas em: tratamento principal, manejo do solo, preparo convencional e cultivo mínimo, tratamento secundário, dose área total 0; 100; 200 kg.ha-1 P2O5, tratamento terciário, dose no sulco 0; 90; 180 kg.ha-¹ P2O5. Foram avaliados os parâmetros químicos do solo, produtividade de colmos e açúcar, tecnológicos e exportação de nutrientes. O programa estatístico utilizado foi o AgroEstat – Sistema para Análises Estatísticas de ensaios agronômicos. Os resultados mostraram que o cultivo mínimo proporcionou uma maior concentração do fósforo na camada superficial em relação ao preparo convencional e o excesso de fósforo causou uma diminuição na qualidade tecnológica e na produção de açúcar. / Taking into consideration the polemics involved on which is the most efficient phosphorus application method in Brazilian tropical soils, the present research suggests an efficient phosphorus application method in sugarcane production in an environment of medium potential, emphasizing phosphorus application methods and rates, aiming the best availability of this element in the soil solution. In order to evaluate the phosphorus incorporation effects and other soil correction chemicals, two soil managements were adopted; the conventional and the minimum tillage. Therefore, this research were based on two hypothesis: I) the application of part of the phosphorus over the soil and other part in the sugarcane furrow, in sandy soils, allowed higher phosphorus and other nutrients utilization. II) The phosphorus incorporation and incorporation of other element allowed better sugarcane root systems development. The experimental design used was completely randomized block design, with split-split plot, where the main plot was soil management (either conventional or minimum tillage), secondary was phosphorus rate over the soil (0; 100; 200 kg ha-1 P2O5), and tertiary was phosphorus rate on in sugarcane row (0; 90; 180 kg ha-1 P2O5). Soil chemical parameters were evaluated, as well as stems and sugar yield, technological aspects and nutrient extraction. AgroEstat was used for the statistical analysis. The results showed that minimum tillage provided higher phosphorus concentration on the soil top layer compared to the conventional tillage and the phosphorus excess caused reduction in the technological quality and yield of sugarcane.
