A configuração do programa academia da saúde como estratégia para a promoção à saúde / The configuration of the health academy program as a strategy for health promotion

A população brasileira caminha para mudança no perfil demográfico, acompanhada de processo de transição epidemiológica com crescente número de acometidos por doenças crônicas não transmissíveis. Entre as estratégias do Ministério da Saúde está o Programa Nacional de Promoção à Saúde com intuito de promover a qualidade de vida e reduzir vulnerabilidade e riscos à saúde. Aliado a este, em 2011 é lançado o Programa Academia da Saúde. Os polos do programa são espaços dotados de equipamentos, estrutura e profissionais, com objetivo de promover à saúde e produzir o cuidado. Estudo buscou analisar o programa Academia da Saúde em dois municípios do estado de São Paulo em relação ao ciclo da política e identificar limites na inserção na rede de atenção à saúde. Para a análise referente ao ciclo da política, os dados foram extraídos por meio de revisão bibliográfica e análise documental. Já para o estudo de caso empírico, visando à identificação dos limites do programa, foram realizadas entrevistas semi-estruturadas com gestores e profissionais. Foram entrevistados 05 gestores e 10 profissionais. A análise foi feita a partir das dimensões: contexto político e histórico, modelo de organização e modelo de gestão dos polos através de aproximações sucessivas. Foram criados banco de dados na plataforma Stata 12, realizando-se uma análise descritiva simples das informações processadas. A entrada na agenda nos municípios estudados vai ao encontro das políticas adotadas no âmbito do governo federal por meio dos métodos de indução utilizados pelo ministério da saúde. O processo de indução se acentuou pela pouca presença do governo de estado na tomada de decisão e implantação de programas federais. O envolvimento estadual ocorreu de forma formativa. Os projetos de implantação são inespecíficos, trazendo poucos dados sobre sua proposta de ação, integração com a rede de atenção à saúde, mais especificamente à Atenção Básica, e com a rede intersetorial de apoio à comunidade. A articulação com a rede de assistência ocorre de forma irregular e frágil. A falta de conhecimento dos objetivos do Programa por parte dos profissionais e gestores, o que atrapalhava as relações de trabalho. Apesar de pertencer a rede de atenção, o programa está contextualmente deslocado, uma vez que não existe integração entre esse serviço e outros. Os serviços são realizadas de forma isolada por cada profissional e por cada equipamento de saúde. Apesar de introduzido em um contexto de mudanças de relações entre os entes federativos configura-se como uma ação que ainda é implantada por meio de forte indução federal ao município e através de incentivos e recursos financeiros para subsidiar uma agenda federal / The Brazilian population is heading towards change in demographic profile, accompanied by epidemiological transition with increasing number of affected by chronic diseases. One of the Ministry of Health\'s strategies is the National Program Promoting Health with a view to promoting quality of life and reduce vulnerability and risk to health. Allied to this, in 2011 the Ministry of Health launched the Health Academy Program. The program centers are spaces with equipment, structure and professionals, in order to promote health and care delivery. This study investigates the Health Academy program in two cities in the state of São Paulo in relation to the political cycle and identify limits on inclusion in the network of health care. For the analysis regarding the policy cycle, the data were extracted through literature review and document analysis. As for the empirical case study, aimed at identifying the program limits, semi-structured interviews with managers and professionals were held. We interviewed 05 managers and 10 professionals. The analysis was made from the dimensions: political and historical context, organizational model and management model of the poles through successive approximations. A simple descriptive analysis of the information processed were created database in Stata 12 platform is performing. The agenda entry in the cities studied meets the policies adopted within the federal government through the induction methods used by the health ministry. The induction process was accentuated by the poor state of government presence in decision making and implementation of federal programs. The state involvement occurred formative way. The implementation of projects are nonspecific, bringing little data on its proposed action, integration with the network of health care, specifically the Primary, and the intersectoral network of community support. The articulation with the assistance of network occurs irregularly and fragile. Lack of knowledge of the aims for professionals and managers, which hindered labor relations. Although attention belong to the network, the program is shifted contextually, since there is no integration between the service and others. Services are held in isolation for each professional and each health unit. Although introduced in a context of changing relations between the federal entities is configured as an action that is still implemented through strong federal induction to the municipality and through incentives and financial resources to support a federal agenda

O cuidado a pessoas em situação de rua: a experiência da Rede de Atenção Psicossocial da Sé / Health care for homeless population: the experience of health services network of Sé

O cuidado em saúde dirigido à população em situação de rua requer ações intersetoriais, que considerem as características desse grupo e respondam às suas necessidades. As diretrizes propostas pelas Políticas Públicas de Saúde e Saúde Mental oferecem subsídios para o cuidado e destacam a importância das ações serem desenvolvidas no contexto de vida das pessoas, de modo a favorecer o exercício da cidadania e dos direitos. Este estudo teve como objetivos conhecer as ações dirigidas às pessoas em situação de rua que apresentam transtorno mental, desenvolvidas pelas equipes do Consultório na Rua (CR) da Unidade Básica de Saúde (UBS) Sé e do Centro de Atenção Psicossocial (CAPS) Adulto II Sé; identificar obstáculos e pontos de força encontrados no cotidiano de trabalho e conhecer a opinião dos usuários sobre o cuidado recebido. A pesquisa, de caráter qualitativo, empregou os seguintes procedimentos metodológicos: revisão integrativa da literatura; pesquisa documental; entrevistas semi-estruturadas com profissionais e usuários; observação participante e elaboração do caderno de campo. Os dados foram coletados entre fevereiro e abril de 2016. Os resultados evidenciaram que a maioria das ações oferecidas pelos serviços considera as características da população e busca responder às suas necessidades. A construção e manutenção do vínculo entre profissionais e usuários foram compreendidas como eixos do trabalho, que contribuem para o estabelecimento de relações humanizadas e influenciam positivamente a realização de ações. A construção do trabalho intersetorial apresentou-se como um desafio, por depender da organização e do alinhamento de cada serviço com as normativas e da relação estabelecida entre eles. Dois aspectos se destacaram nos resultados: a sobrecarga e o risco de adoecimento dos profissionais e a influência da atual forma de gestão dos serviços na produção do cuidado. Concluiu-se que as ações produzidas pelo CAPS Adulto II Sé e pelas equipes de CR da UBS Sé se encontram alinhadas às diretrizes das Políticas, porém é necessário cuidado às equipes e envolvimento da gestão nos processos de trabalho, para que a responsabilidade da produção de ações de qualidade não recaia unicamente sobre os profissionais / Health care for homeless npopulation requires intersectoral actions that considers this group characteristics and attends his needs. Public Health and Mental Health Policies offer a guideline to actions and indicates the importance of these actions to be developed according to people life context, in a way to promote citizenship and access to rights. The goals of this research were to get known the actions developed by Psychosocial Care Center to Adults II Sé and Consultation Office in the Streets teams and offered to homeless population that presents mental disorders; to identify obstacles and points of force presents in the daily work; and know the patients opinion about the received care. This qualitative research used methodological procedures such as integrative literature review; documental research; semi-structured interview developed with professionals and patients; participant observation and field notebook construction. The data was collected between february and april/2016. The results showed that most part of actions offered by professional teams take into consideration this population needs and try to answer to these needs. The construction and maintenance of a bound between professionals and patients were comprehended as the center point of work, which contribute to establish humanized relations and positively influence actions. The construction of intersectoral work was comprehended as a challenge, due to services organization, alignment between services and guidelines and relation among different services. In the results, two aspects were highlighted: burden and risk of illness of professionals and the influence of current services structure on delivered care. As a conclusion, actions offered by professional teams are in accordance to Policies guidelines, although it is necessary to offer better care to professionals and higher involvement of administration on work processes, in order to avoid the risk of responsibility for quality actions fall over professionals only

Esta tesis doctoral tiene por objetivo el estudio de la actividad innovadora de los sectores económicos desde la perspectiva de los vínculos tecno-productivos que se establecen entre ellos. Dentro de este objetivo general, cobra una destacada importancia el territorio en el cual se encuentran estos sectores, siendo España y la Comunidad Valenciana los casos analizados. Ante estos objetivos, se plantean las siguientes preguntas que guían la investigación: a) ¿Qué rol desempeñan los servicios empresariales intensivos en conocimiento en la generación y difusión de innovación en el sistema económico? ¿Qué contribución cuantitativa realizan tales sectores al sistema de creación y generación de conocimiento? b) ¿Cuáles son los patrones de innovación de sectores menos intensivos en conocimiento, clasificados típicamente como tradicionales, pero con gran arraigo e influencia territorial (en particular el sector agroalimentario)? ¿A qué fuentes recurren con mayor intensidad cuando el territorio presenta también bajas intensidades tecnológicas o baja capacidad de absorción (la Comunidad Valenciana)? c) ¿Qué patrones de innovación se corresponden con distintas capacidades sectoriales para crear nuevas empresas? O en otras palabras, ¿el patrón de generación/absorción de conocimiento de cada sector afecta a su capacidad para crear nuevas empresas? Para dar respuesta a estas preguntas se ha utilizado el enfoque del "embodied knowledge". Esta aproximación permite mensurar el conocimiento "incorporado" en los productos y servicios que cada sector utiliza como inputs en sus procesos productivos y que indirectamente contribuyen a su propia intensidad o esfuerzo tecnológico y de innovación. This master thesis investigates the subject of Corporate Social Responsibility (CSR) in Kazakhstan and selected states of European Union (EU). Successful implementation of CSR depends on a number of factors influencing it. Firstly, CSR is regarded as a part of market strategic decision making. Thus, there should be certain level of awareness about CSR on this particular market. Secondly, CSR is highly dependent on the states' public policies and activities of civic sector organizations (CSOs). In this thesis I examine the interaction of the state, the market and civic sector in influencing and creating conditions for the development of CSR in Kazakhstan. One chapter is dedicated to CSR in Kazakhstan's oil and gas sector. This thesis also investigates and compares major differences regarding CSR in Kazakhstan and selected EU states. The aim of this thesis is to present current situation of CSR development in Kazakhstan and formulate recommendations for the successful development of CSR in this country.

Subject of the research - the mechanism of interaction between public authorities and non-profit organizations in the field of supporting families with children in the Sverdlovsk region. The goal is to analyze the specifics of interaction between public authorities and non-profit organizations in the field of supporting families with children and develop recommendations for improving this interaction. The solution of the tasks set in the work was carried out on the basis of the application of such research methods as the analysis of theoretical and regulatory documents, the analysis of statistical data. A sociological study was carried out using the method of secondary data analysis and expert survey. The study examined theoretical and regulatory issues of interaction between government bodies and non-profit organizations in the field of supporting families with children; the analysis of the state of this interaction in the Sverdlovsk region is carried out, the main problems are identified. Объектом исследования являются получатели социальных услуг, проживающие в сельской местности. Предмет исследования: структура и качество социальных услуг для получателей социальных услуг, проживающих в сельской местности. Целью магистерской диссертации является выявление уровня удовлетворенности получателей социальных услуг, проживающих в сельской местности (на примере ГАУСО СО «КЦСОН Пригородного района»). В процессе работы проведен анализ научной литературы и нормативно-правовой базы в сфере социального партнерства в социальной сфере, проведено социологическое исследование, которое выделило структуру социальных проблем получателей социальных услуг, усовершенствована модель социального партнерства в интересах получателей социальных услуг, проживающих в сельской местности. В социологическом исследовании приняло участие 200 получателей социальных услуг, проживающих в сельской местности – подопечные ГАУСО СО «КЦСОН Пригородного района». В результате исследования было выявлено, что среди главных сложностей, с которыми сталкиваются получатели социальных услуг, проживающие в сельской местности, являются доступность медицинского обслуживания, возможности для приобретения новых умений и навыков, возможности для организации досуга. Для решения данной проблемы нами усовершенствована модель социального партнерства в интересах получателей социальных услуг, проживающих в сельской местности, после апробации которой проведено повторное анкетирование, которое доказало действенность данной модели. / The subject of the study are recipients of social services living in rural areas. The subject of the study: the structure and quality of social services for recipients of social services living in rural areas. The purpose of the master's thesis is to identify the level of satisfaction of recipients of social services living in rural areas (on the example of GAUSO SO "KCSON Prigorodny District"). In the process, an analysis of scientific literature and the regulatory framework in the field of social partnership in the social sphere was carried out, a sociological study was carried out, which highlighted the structure of social problems of recipients of social services, and the model of social partnership was improved for the benefit of recipients of social services living in rural areas. The sociological study was attended by 200 recipients of social services living in rural areas - wards of GAUSO SO "KCSON Prigorodny District." As a result of the study, it was revealed that among the main difficulties faced by recipients of social services living in rural areas are the availability of medical care, opportunities for acquiring new skills and skills, and opportunities for organizing leisure activities. To solve this problem, we have improved the model of social partnership for the benefit of recipients of social services living in rural areas, after testing of which a repeated survey was carried out, which proved the effectiveness of this model.

Depuis son introduction en santé publique en 1978 et son intégration en promotion de la santé en 1986 avec la Charte d’Ottawa, la gouvernance intersectorielle (GI) est devenue une des stratégies les plus utilisées en santé publique et en promotion de la santé pour aborder les déterminants sociaux de la santé. La GI met en relation des acteurs de différents secteurs avec des intérêts divers qui décident de travailler ensemble dans le but de réaliser un objectif commun. La réalisation de cet objectif nécessite que les acteurs harmonisent leurs intérêts organisationnels pour aboutir à un intérêt collectif mutuellement bénéfique. Il a été démontré que la conciliation des intérêts est un des facteurs déterminants de l'efficacité des processus de gouvernance intersectorielle. S’il est traditionnellement reconnu que dans un processus de gouvernance intersectorielle les acteurs concilient leurs intérêts organisationnels et collectifs, à notre connaissance, l’étude des stratégies d’influence déployées par les acteurs intersectoriels pour aligner leurs intérêts est un peu négligée. Cette thèse se propose de combler cette lacune. La thèse porte sur un cas de gouvernance intersectorielle aux niveaux régional et local à Montréal : l’Initiative montréalaise pour le développement social coordonnée par un comité de quatre partenaires provenant de quatre secteurs différents. Nous nous intéressons plus précisément au processus de révision du Cadre de référence de l’Initiative montréalaise (IM) qui a été marqué par une crise et qui a eu de grands impacts sur le fonctionnement et la dynamique du processus de gouvernance de l’IM. L’objectif principal de la thèse est d’identifier les stratégies d’influence développées par les acteurs intersectoriels pour concilier leurs intérêts organisationnels et collectifs dans un processus délibératif. Plus spécifiquement, nous nous proposerons i) de retracer et de décrire les évènements critiques qui ont marqué le processus délibératif de révision de l’Initiative montréalaise de 2011 à 2015 ; ii) de caractériser les évènements critiques du point de vue des acteurs de chaque secteur afin de reconstruire les stratégies d’influence déployées pour concilier leurs intérêts et d’examiner les conditions d’utilisation des stratégies d’influence. Pour atteindre nos objectifs, nous avons mobilisé la théorie de l’acteur-réseau (TAR) combinée au modèle conceptuel de Dewulf et Elbers (2018) et de l’approche de Heckscher (2013). Pour la collecte de données, nous avons eu recours à la recherche documentaire, la technique d’incidents critiques, des entrevues semi-structurées et des groupes de discussion. Les résultats révèlent que pour reconstruire à postériori un processus de gouvernance intersectorielle à l’aide d’évènements critiques, il est important d’identifier les évènements en lien avec les controverses survenues au cours de son déroulement. Concernant les stratégies d’influence, l’étude montre que les secteurs ont déployé deux grandes catégories de stratégies que nous avons qualifiées de stratégies intersectorielles et de stratégies intra sectorielles. Les premières comprennent les stratégies communes et les stratégies de médiation. Les stratégies intra sectorielles regroupent les stratégies unilatérales créatives, les stratégies de pouvoir et les stratégies multilatérales. Les deux catégories de stratégies se différencient principalement par leurs objectifs, leur lieu de déploiement et par leurs mécanismes de base. Pour ce qui est des conditions d’utilisation, la recherche indique que : i) le contexte interne de la gouvernance intersectorielle caractérisé par l’existence ou non de conflit entre les intérêts sectoriels et collectifs; ii) les contraintes organisationnelles définies par les valeurs, missions et objectifs de l’organisation d’appartenance des acteurs et iii) les atouts qui désignent les ressources matérielles, la légitimité et l’autorité dont disposent les secteurs constituent les principales conditions dans lesquelles les stratégies d’influence sont utilisées. À noter également que les différents secteurs ont eu recours aux mêmes stratégies d’influence - exception faite des stratégies de pouvoir - dans les mêmes conditions. Ces résultats apportent un nouvel éclairage au fonctionnement interne de la gouvernance intersectorielle pour la santé. Ils mettent en évidence l’importance des stratégies d’influence des acteurs dans la conciliation des intérêts et du même coup dans la dynamique interne de la gouvernance. À ce titre, nous pensons que les chercheurs gagneraient à inclure le concept de stratégie à la liste des facteurs qui favorisent ou qui entravent le fonctionnement de la gouvernance intersectorielle régionale/locale en promotion de la santé. / Intersectoral governance (IG) has become one of the most widely used strategies in public health and health promotion to address the social determinants of health since its introduction into public health in 1978 and its integration into health promotion in 1986 formulated/highlighted in the Ottawa Charter. Intersectoral governance brings together actors from different sectors with diverse interests to work together to achieve a common goal. Achieving this goal requires that actors align their organizational interests to achieve a mutually beneficial collective interest. It has been shown that the reconciliation of interests is one of the determining factors in the effectiveness of cross-sectoral governance processes. While it is traditionally recognized that actors in an intersectoral governance process reconcile their organizational and collective interests, to the best of our knowledge, the study of strategies deployed by intersectoral actors to align their interests is somewhat neglected. This thesis proposes to fill this gap. The thesis focuses on a case of intersectoral governance in Montreal: the Montreal Initiative for Social Development coordinated by a committee of four partners from four different sectors. Specifically, we focus on the process of revising the Montreal Initiative's (MI) Terms of Reference, which was affected by a crisis and had major impacts on the functioning and dynamics of the MI's governance process. The main objective of the thesis is to identify the strategies developed by intersectoral actors to reconcile their organizational and collective interests in a deliberative process. More specifically, we will propose to a) reconstruct and describe the critical events that marked the deliberative process of revising the Montreal Initiative from 2011 to 2015; b) characterize the critical events from the point of view of the actors in each sector in order to reconstruct the strategies deployed to reconcile their interests and to examine the conditions of use of the strategies. To achieve our objectives, we mobilized the actor network theory (ANT), using the conceptual model of Dewulf and Elbers (2018) and the approach of Heckscher (2013). For data collection, we conducted literature review, critical incident technique, semi-structured interviews, and focus groups. In terms of critical events, our results reveal that it is important to identify events related to controversies that occurred during the process. Regarding influence strategies, the study shows that the sectors have deployed two main categories of strategies including, intersectoral strategies and intra-sector strategies. The former includes joint strategies and mediation strategies. The latter, intra-sectoral strategies consist of creative unilateral strategies, power strategies and multilateral strategies. The two categories of strategies differ mainly in their objectives, their place of deployment and their basic mechanisms. Regarding the conditions of use, the results indicate that: i) the internal context of intersectoral governance are characterized by the existence or not of conflict between sectoral and collective interests; ii) organizational constraints are defined by the values, missions and objectives of the organization that the actors belong; and iii) assets, which refer to the material resources, legitimacy and authority available to the sectors, constituting the main conditions under which influence strategies are used. It should be also noted that the different sectors used the same strategies - except for power strategies - under the same conditions. These results shed new light on the internal workings of intersectoral governance. This study highlights the importance of the actors' strategies in reconciling interests and, at the same time, in the internal dynamics of governance. As such, we believe that researchers would benefit from including the concept of strategy in the list of factors that promote or hinder the functioning of intersectoral governance in health promotion.

Développement langagier des tout-petits : étude de cas de l'articulation problème – solution chez les acteurs de la petite-enfance d’un territoire pluriethnique de Montréal

Ce mémoire porte sur la perception des acteurs multisectoriels de la petite enfance concernant le développement langagier des tout-petits et les interventions qu’ils proposent pour le soutenir. L’origine de cette question vient de l’Enquête sur la maturité scolaire des enfants montréalais. En route pour l'école! (DSPM, 2008a) qui révélait que les tout-petits montréalais étaient plus vulnérables dans les domaines du développement cognitif et langagier et des habiletés de communication et connaissances générales que ceux de l’échantillon normatif canadien, jumelée à la préoccupation des acteurs eux-mêmes relativement à cette problématique. Les objectifs sont de décrire le portrait qu’ont ces acteurs du développement langagier des enfants de leur milieu et analyser ces représentations sur le plan de leur articulation problème – solution. Pour y répondre, un cadre conceptuel écologique mettant en relation quatre niveaux d’influence (enfant, famille, voisinage, environnement général) et cinq axes d’intervention a été conçu. La méthodologie adoptée fut une étude de cas qualitative à base d’analyses documentaires et d’entrevues (n=10) auprès des six secteurs engagés dans l’intervention intersectorielle en petite enfance dans un CSSS montréalais. Les résultats montrent que ces acteurs connaissent les facteurs influençant le développement langagier des tout-petits. Les interventions actuelles ou souhaitées sont cohérentes avec les écrits scientifiques et partiellement stratégiques par rapport aux facteurs de risques identifiés. Des actions doivent toutefois être posées pour soutenir davantage les jeunes familles, dont celles visant à hausser la qualité du voisinage ou encore, l’accès à des services publics de qualité. / This thesis examines perceptions of intersectoral stakeholders in early childhood regarding language development in young children, and the initiatives they suggest to support such development. The question originates from a survey on the school readiness of Montréal children, Enquête sur la maturité scolaire des enfants montréalais. En route pour l'école! (DSP, 2008a), which revealed that, with regard to language development and communication skills, young Montréal children were more vulnerable than the Canadian normative sample. It also stems from stakeholders’ concerns regarding this issue. The goals are to describe the representation of language development held by these stakeholders and to analyse their conceptualisation of the problem and the solution to language development. An ecological conceptual framework was developed, comprising four levels of influence (child, family, neighbourhood, overall environment) and five areas of intervention. The methodology was a qualitative case study based on a literature synthesis and interviews (n=10) carried out in six sectors involved in early childhood intersectoral initiatives in a Montréal Health and Social Services Centre (CSSS). Results show that stakeholders are aware of the factors influencing language development in young children. Current or desired initiatives correspond with the literature and are mostly strategic with respect to identified risk factors. However, actions must be taken to provide more support to young families, including actions designed to improve neighbourhoods and enhance access to quality public services.

O modelo é denominado Estratégia de Desenvolvimento Sustentável, o qual é composto pelas etapas de formação de equipe multidisciplinar, diagnóstico situacional, proposição e alinhamento de intervenções, monitoramento e avaliação, reconhecimento de desempenho e renovação e expansão de atividades e parcerias. A metodologia de pesquisa-ação foi utilizada para a implementação das etapas, combinada com a abordagem de estudo de caso para o relato da experiência e apresentação de resultados. Os resultados indicaram que a Estratégia de Desenvolvimento Sustentável viabilizou melhorias na articulação entre secretarias do executivo municipal, identificação de dados relevantes para uso no processo de planejamento urbano e estímulo ao alinhamento entre representantes dos setores público, setor privado e sociedade civil na elaboração e execução de iniciativas relacionadas à sustentabilidade urbana. Neste sentido foi possível identificar a viabilidade do modelo proposto, bem como sua possível adaptação e replicação a outros municípios. / This research deals with the creation and implementation of a intersectoral engagement framework focused on sustainable urban development in the municipality of Guaíba in the state of Rio Grande do Sul. The framework is called the Sustainable Development Strategy, which is composed by the steps of multidisciplinary team composition, situational awareness, proposition and alignment of interventions, monitoring and evaluation, general review and expansion and renewal of activities and partnerships. The action-research methodology was used for the implementation of the steps, combined with the case study approach for reporting the experience and presenting its results. The results indicated that the Sustainable Development Strategy enabled improvements in the articulation between municipal executive secretariats, identification of relevant data for use in the urban planning process, and encouragement of alignment among representatives of the public sector, private sector and civil society in the elaboration and execution of initiatives related to urban sustainability. In this sense, it was possible to identify the feasibility of the proposed model, as well as its possible adaptation and replication to other municipalities.
