Validação de conteudo do diagnostico de enfermagem "Conflito no desempenho do papel de mãe" : uma perspectiva do periodo neonatal

À medida que a equipe de saúde considera que o quadro clínico do bebê permite, a mãe passa a assumir grande parte dos seus cuidados. Assumir o papel materno pode não ser uma experiência fácil, devido às características situacionais e ambientais, nas quais o cuidado foi dado quase que predominantemente pela equipe de enfermagem. Assim, este estudo teve como objetivos: realizar validação de conteúdo do diagnóstico de enfermagem, proposto pela NANDA, "conflito no desempenho do papel de mãe" e suas características definidoras junto a enfermeiros que atuam em unidades de cuidados neonatais e pesquisadores da área; identificar as características definidoras maiores e menores deste diagnóstico e verificar se o julgamento dos enfermeiros foi influenciado por fatores como idade, tempo de atuação como enfermeiro, tempo de atuação em neonatologia, realização de cursos sobre diagnóstico de enfermagem, participação em cursos de pós-graduação, realização de pesquisa e utilização de diagnósticos. Acredita-se que este assunto torna-se de grande interesse porque o crescimento prático e teórico da enfermagem requer um maior domínio dos diagnósticos, como apresenta a literatura consultada, sendo a unidade de internação neonatal um local carente de estudos sobre este assunto. Foi utilizado o modelo de Validação de Conteúdo Diagnóstico proposto por FEHRING, o qual se baseia na obtenção de opiniões de enfermeiros (peritos, de acordo com os critérios deste modelo, seja por sua experiência assistencial, seja pelos estudos realizados) acerca do grau em que certas características definidoras são indicativas ou não de um determinado diagnóstico. A amostra utilizada neste estudo foi por conveniência, composta por 59 enfermeiros. O estudo validou as características definidoras estabelecidas pela NANDA para este diagnóstico. Quatro características definidoras foram consideradas maiores por terem obtido média ponderada maior ou igual a 0,80: 1. mãe expressa preocupação(ões)/ sentimento(s) de inadequação para suprir as necessidades físicas e emocionais da criança durante a hospitalização; 2. mãe expressa preocupação(ões)/sentimento(s) de inadequação para suprir as necessidades físicas e emocionais da criança em casa; 3. mãe expressa preocupação(ões) com relação a mudanças no papel materno e 4. mãe expressa preocupação(ões) em relação à saúde da família. As demais obtiveram média ponderada menor que 0,80, mas maiores que 0,50, sendo assim classificadas como menores. Nenhuma característica definidora foi excluída. Não houve correlação entre o perfil dos enfermeiros e o julgamento que fizeram das características definidoras. As dificuldades para a elaboração das definições operacionais e os comentários dos enfermeiros demonstraram o quanto as características definidoras propostas pela NANDA precisam ter sua linguagem reavaliada para que se tornem mais objetivas e descrevam melhor as respostas dos clientes frente aos problemas de saúde e processos de vida. Recomenda-se que a validação de conteúdo, seguida por validação clínica, se dê em diferentes culturas e locais de assistência para abranger as diversidades das clientelas atendidas / Abstract: The technological progress of medical and nursing assistance has made possible the survival of many newboms with medical complications. As the staff considers baby's condition is evolving well, the mother has more opportunity of taking care of him or her. To assume maternal role might not be an easy experience in the neonatal care unit due to the special characteristics of the situation and the environment and because of the mother's fragility, being distant from the child and taking care less often than the nursing staft. The aims of this study were: validating the content of the NANDA nursing diagnosis "Parental role conflict" and its defining characteristics, with nurses which work in neonatal care units and researchers of this area; identifying minor and major defining characteristics of this diagnosis and, finally, verifying if the scores attributed by nurses to the defining characteristics were influenced by factors as age, experience in nursing, time of neonatology practice, continuing education about nursing diagnoses, post-graduation, development of researches and the use of nursing diagnoses. This issue is really important because practical and professional nursing development require a greater domain of nursing diagnoses, according to literature, and neonatal care units are deprived of this kind of study. The diagnostic content validation model proposed by FEHRING was used in this work, which consists on obtaining opinion from nursing experts about how much certain defining characteristics are indicative or not of a diagnosis. It was a convenient sample of 59 nurses. It was recognized that the study validated the defining characteristics recognized by NANDA. Results showed that four defining characteristics were considered major because achieved scores of 0,80 and higher: 1. Mother expresses concem(s)/feeling(s) of inadequacy to provide for child's physical and emotional needs during hospitalization; 2. Mother expresses concem(s)/feeling(s) of inadequacy to provide for child's physical and emotional needs at home; 3. Mother expresses concem(s) about changes in parental role; 4. Mother expresses concem(s) about family health. No defining characteristic was eliminated. There was not correlation between nurses' profile and the judgement of the defining characteristics. The difficulties of operational definitions elaborating and the commentaries of nurses showed that the defining characteristics proposed by NANDA need assessment to become more objective and describe more accurately patients' answers concerning health problems and life issues. This thesis focuses on the welfare system of women during maternity and parental leave, primarily for specific benefits, which are in maternity and parenthood provided. System of state social support to women subject to conditions childbirth, child benefits and parental pay. For the childbirth, the work highlights specific situations that may arise in life. They are taking a child into permanent parental care, surrogate motherhood and the death of a child. For child benefits is given consideration above its level and subsequently at the parental pay instance of fraudulent conduct involving the unauthorized receipt of benefits or readers are informed on the progress of the amendment to the law on state social support. Sickness insurance system is provided benefit maternity pay, for example, which analysed the possible claim women on maternity benefits in specific situations (pregnant student, pregnant person exercising custodial sentence, etc.). For each benefit is each explained the general conditions for entitlement to, and also its amount. Although the work is devoted mainly to women, and a separate section of this paper deals with the men and their opportunities to take the place of women and care for the child. This thesis focuses on mapping and evaluation of psychosocial climate by expectant mothers in the "Česko-německá horská nemocnice s.r.o.". It is a theoretical and empirical project. The theoretical part provides knowledge about psychological changes, which the woman goes through during her pregnancy, childbirth and the postpartum period. The further space is devoted to the needs of the expectant mother, the stress in the perinatal period and woman's psychopathology in the mentioned periods. Socio-cultural links in the perinatal period are also discussed. The vital importance lies in describing psychosocial aspects in the perinatal care as well as communication with the woman in the perinatal period. The research part of the thesis is aimed at the analysis of psychosocial climate in the maternity hospital in Vrchlabí. A questionnaire of psychosocial climate of the KLI-P maternity hospital was used for the research. 51 mothers participated in the evaluation plus extra 10 to complete the qualitative questionnaire. The analysis is enriched by two midwives' point of view of the approach to delivering mothers in the Vrchlabí maternity hospital. No Care for Distance : The (Market) Logic of Regionalizing Maternity Care

A wave of maternity unit closure is sweeping through the North Atlantic zone, leaving rural communities without the care they crucially need. In its wake resistance grows, mobilizing against closures in the face of a discourse of economic efficiency and neoliberal austerity. To understand the issue, research on maternity care and geography offer useful insights on the particular costs and consequences of losing access to care but is less useful for engaging the causes behind them. Not suffering from a lack of critical engagement, Marxist theory enables the wave to be understood in terms of changing political incentives and the ways these have come about. The present essay brings the two fields together in an effort to aid local resistance in rural communities, concluding that regionalization does not operate on a logic of its own as is otherwise stated but on the logic of markets, imposed on governments by the neoliberalization of the Western world and beyond. The essay aims to provide the political-economic framework needed to confrontt he logic of markets, neoliberalism, and the capitalist political-economic system that underline the closures.

The teaching of the delivery procedure by the simulation method

(Advanced Midwifery and Neonatal Nursing) / With the introduction of the four year comprehensive course, the students at a particular college of nursing receive instruction on normal labour at the beginning of their second year. They are then allocated to the maternity wards of a particular hospital for their midwifery nursing science practica. The practical placement of these students to the midwifery clinical area is variable as a few may be allocated to the practice immediately after having received their theoretical instruction, a few two months later and the rest four, six, eight and ten months later. A major problem thus facing midwifery educators at a particular college of nursing, is to select an appropriate method of teaching that would assist the student nurses in acquiring the psychomotor skills that are necessary for the management of the delivery procedure. The goal of this study was to evaluate the competency levels of the second year student nurses, in the application of the psychomotor skills that are necessary for the management of the delivery procedure, after having made use of different teaching strategies, namely: * the traditional teaching method, and * the aforementioned method combined with the simulation teaching method. The hypothesis stated that there would be no difference in the application of the necessary psychomotor skills for the first delivery procedure, between the control and the experimental groups.

D.Cur. / The midwife is the pivot around whom training activities in midwifery revolve. Her knowledge and skill form an inseparable part of the effectiveness of her practice. The external and internal environment of the midwife play a vital role regarding the demands placed upon her. The internal and external environment of the midwife is dynamic, and change continually. The external environment in the past ten years has undergone marked changes: health policy has changed; legal-ethical changes have occurred; the roles of the private sector and fee-for-service practitioners have changed; a new government with other objectives has placed the emphasis on new needs at professional and lay levels. These changes place new demands on the midwife's practice. Because the internal and external environments are in interaction, changes in the internal environment are continually being made, for example the knowledge and competency required by the midwife must continuously be improved so that she can comply with the new demands in her practice. The training of the midwife ought to be such that she is adequately equipped to fulfil the demands set by practice. The goal of this study is: an exploration and description of the knowledge and skill required by the midwife to be able to accept her responsibility in the new health care service of South Africa and set guidelines for the basic training of midwife who is to function independently.

The maternity capital's impact on birth intervals in Russia : Survival analysis of the transition from the 1st to 2nd child

From 2007 up until now, Russia’s period fertility rate (TFR) increased at a relative constant pace. This increase coincided with the implementation and execution of pronatalist measures, mostnotably the maternity capital program. In this study, two distinct time periods (pre- and postpolicy) were compared in order to discover the effect of this program on fertility. This study is based on data from the Russian Generations and Gender Survey (GGS), conducted in 2011. Employing event-history analysis for the transition to a 2nd childbirth, this study assesses whether the maternity capital policy has had an impact on the interval between the 1st and 2nd birth and the overall 2nd birth risk. Moreover in this study I aimed to determine whether the 2nd birth risk was different according to woman’s educational level and ages at the time of 1st birth (entry into the 1st motherhood) in the policy period. The analysis pointed out that the transition to the 2nd birth event did not precipitate due to the introduction of maternity capital. Moreover my research indicated that the introduced policy did not increase the overall risk of having a 2nd child. The analysis of the interaction between the post-policy period and woman’s education or age at the 1st birth also did not reveal any significant difference.

Human society is dependent on women who ensure their sustainability in the role of mothers. At the same time women participate in the economic prosperity of the family and society. The combination of these two roles puts them at a disadvantage in the labour market. This thesis aims at analyzing the current situation of women from the point of view of motherhood and position on the labour market. The theoretical part deals with two primary areas including social policy and the labour market. Social policy gives information about family policy and state support to mothers in comparison with other countries. The practical part, which is based on methodology, is dedicated to the analysis of the current situation of women and is divided according to the origin of the data. The secondary data were obtained from the public statistical database and the primary one through the questionnaire survey. The outcome of the work reveals significant factors influencing the age at which women have their first child, the dependence between parental leave income, the length of parental leave, and other answers to the research questions.
