Human resources capacity in the Ministry of Health and Social Services in Namibia

The purpose of this study was to examine the extent to which human resources capacity of the Ministry of Health and Social Services (MoHSS), Namibia, influences health care services delivery to the Namibian population. A qualitative research model using exploratory and descriptive study designs was adopted. Data were collected through semi-structured interviews with 46 health workers from two referral hospitals and two directorates in Windhoek District. The study found that there is severe staff shortage in the MoHSS, which has resulted in high workload and poor health care. Health worker migration, new services and programmes, emerging diseases, and population growth were reported to have contributed to staff shortage and high workload in the MoHSS. Study findings suggested a need to create more posts to accommodate emerging needs, and to introduce an effective retention strategy to attract and retain health professionals with scarce skills, and those working under difficult conditions. / Public Administration and Management / M. In order for a company to operate efficiently, it's need to focus the attention on integrity. Integrity problem is very bright, how bright is the rupture between the values and the fact. The axle of problems is how to overcome the distraction-discrepancies between words (values) and action (the fact). The aim of work: To evaluate the integrity of the phenomenon and its management in the organization. The goals of work: 1. to define the integral essence of professional activity and conception; 2. to indicate by text stage company installing the model of integrity; 3. to reveal the integrity of the Ministry of Health, Welfare and Sport in the Netherlands. Analýza průběhu epidemie pandemické chřipky v Jihočeském kraji / Analysis of the flu pandemic in the South Bohemian Region

In the thesis there are chronologically processed data about incidence of influenza Pandemic (H1N1) 2009 from its first incidence in spring 2009 in Mexico until the official end of the 6th phase of pandemic announced by the WHO in august 2010. These data were collected by means of secondary analysis. The thesis is focused on the Region of South Bohemia from the first proved incidence of Pandemic influenza (H1N1) 2009 here. The data necessary to meet the objectives of the work and to answer the research questions were collected in cooperation with the Regional Hygienic Station of the South Bohemia in České Budějovice. 3 deaths were analysed in the context of incidence of Pandemic influenza (H1N1) 2009 in the Region of South Bohemia and anti-epidemic measures were assessed. Differences in 121 people with Pandemic influenza (H1N1) 2009 in the Region of South Bohemia from the point of view of age and sex were described. In 52 people with the flu from the Region of South Bohemia ?traveller? history was recorded (these people were infected during their stays abroad) and most of them had stayed in Germany. Indicators of morbidity (ARI) in the Region of South Bohemia and in the whole Czech Republic were also processed and then graphically compared.

Pohled dětské sestry na zdravotnické reformy / View of nurses on the health - care reform

This thesis analyses the reforms of health and social care in the Czech Republic, focusing on the impacts of the reform measures on providers and consumers of health care. Health reforms are currently very up to date, but have not yet been adequately processed and published. Health care of the modern state which Czech republic undoubtedly is must provide affordable and quality health care to all children and people of the society. There is however a necessary role of the state as the representative of the entire health system and relevant legislation. In this thesis we studied the prepared government reforms. The aim of this work was to understand this difficult area and bring this issue further primarily to medical professionals. The results of the research can serve professionals in health social area and be published in scientific journals. The thesis contains a theoretical and empirical part. In the theoretical part we focused on health care systems after 1989, the need for a new reform, the economic development, the health legislation and we also mentioned problems of the current health care system in the Czech Republic. In more detail, we focused on the amended laws related to the new changes in the healthcare system of the Czech Republic. Regarding the reform measures of the Ministry of Labour and Social Affairs, we focused on the change in the institutional child care and thus a possible replacement of nurseries foster care. For the empirical part research was chosen qualitative method of semi-structured interview with nurses. The research sample consisted of 20 university educated nurses who work mainly on children's wards in hospitals in Czech Budejovice, Tabor, Czech Krumlov and General University Hospital in Prague. In the thesis we stated six goals and six research questions. First, we wanted to determine nurses opinions on the big planned new health and social reform. Furthermore, we were interested in awareness nurses would planned changes in the health and social sphere with other goals we wanted to see what would nurses would like to change in the health system of the Czech Republic, how well informed Theky are about the changes in health care since 1989, what they know about the problems of institutional care and what their opinion on the possible cancelation of nurseries and they replace ment of institutional care by foster care is. From the research it was shown that the specifics of the health reform were seen by the respondents from different point of views. During the qualitative investigation it was found that the knowledge of university educated nurses regarding the planned health reforms was not fully sufficient. With the year of 2014 crucial changes occurred in the Czech health system. In particular, we do not pay medical fees for inpatient stay (1st. January 2014). By the media discussed many medical fees majority of the respondents was aware of but they didnt know the other reforms. In this thesis we focused on the lack of (un) awareness of nurses of planned the health reforms. Our hope is to convince the wider professional public that basic knowledge of politics in the Czech Republic is a very important issue.

GOVERNO ELETRÔNICO E INFORMAÇÃO SANITÁRIA NO BRASIL / THE ELETRONIC GOVERNMENT AND THE SANITARY INFORMATION IN BRAZIL

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / The use of technological tools by governments as a way to access the citizen and inform him about matters of public interest is a relatively recent movement and is linked to a culture of transparency, in which the citizen has the power to control the paths taken by their state. This governance model is termed Electronic Government and in Brazil it has been adopted for more than 15 (fifteen) years. The advances in this area have been strengthened with the advent of the Brazilian Information Access Law, Law nº. 12.527/11, which states that access to information is the rule and secrecy to the exception. In this way, all Brazilian government agencies are linked to the provisions of this law that determines the maximum disclosure and transparency in Brazil. Among these, the Ministry of Health draws attention for, in addition to dealing with theme of universal interest, make heavy use of electronic resources on the Internet, and cited here only three: the Portal of Health, the health blog and the Ministry of Page Health on Facebook. So, the question is: the Ministry of Health, in using these online environments, follows the guidelines of the culture of access to information? Moreover, how happens the relationship between him and the Internet citizens who use their official websites? In order to determine what is e-government and how the new information and communication technologies help in the realization of the right to access to information, specifically in the Brazilian Ministry of Health, were used the method of deductive approach, the method of a monographic procedure and techniques of indirect documentation of research (literature and documents) as well systematic intensive direct observation in the environments analyzed on the Internet. It is concluded that despite using up of electronic tools intensively and Law nº 12.527/11 represent an advance in public transparency of the country, Brazil is still far short in other respects, requiring other protection measures for the user to ensure the democratic rule of law which aims to be. / O uso de ferramentas tecnológicas por Governos como forma de acessar o cidadão e informá-lo acerca de assuntos de interesse público é um movimento relativamente recente e vincula-se a uma cultura da transparência, em que o cidadão possui o poder de controlar os caminhos tomados por seu Estado. Este modelo de governança é denominado Governo Eletrônico e no Brasil ele vem sendo adotado há mais de 15 (quinze) anos. Os avanços neste tema foram reforçados com o advento da Lei de Acesso à Informação Brasileira, Lei nº 12.527/11, que determina que o acesso à informação é a regra e o sigilo a exceção. Desta maneira, todos os órgãos públicos brasileiros estão vinculados aos preceitos desta lei, que determina a máxima divulgação e transparência no Brasil. Dentre estes, o Ministério da Saúde chama a atenção por, além de tratar de tema de interesse universal, fazer uso intenso de recursos eletrônicos na Internet, sendo citado aqui apenas três: o Portal da Saúde, o blog da Saúde e a página do Ministério da Saúde no Facebook. Assim, questiona-se: o Ministério da Saúde, na utilização destes ambientes online, segue as diretrizes da cultura do acesso à informação? Ademais, como se dá a relação entre ele e os cidadãos internautas que fazem uso de seus sítios oficiais? Com o objetivo de determinar o que é governo eletrônico e de que forma as novas tecnologias da informação e da comunicação auxiliam na efetivação do direito ao acesso à informação, especificamente no Ministério da Saúde Brasileiro, foram utilizados o método de abordagem hipotético dedutivo, o método de procedimento monográfico e as técnicas de pesquisa de documentação indireta (bibliográfica e documental) bem como a observação direta intensiva sistemática nos ambientes analisados na Internet. Conclui-se que apesar de utilizar-se de ferramentas eletrônicas de forma intensa e da Lei nº 12.527/11 representar um avanço na transparência pública do país, o Brasil ainda está muito aquém em outros aspectos, sendo necessárias outras medidas de proteção ao usuário para se garantir o Estado Democrático de Direito que visa ser.

The research focuses on the interactions in the virtual environment between users and the Ministry of Health about public health policies. In a mediated and networked society, the Ministry has the challenge to change its communication logics historically driven by unidirectional campaigner model. In this sense, the intention is to discuss the potential and limits of social participation in social media related to the cycle of public health policies. For this, the empirical object used in this study is the interaction on the first stage of the vaccination campaign against HPV (Human Papillomavirus) in the Ministry of Health fanpage on Facebook social networking site held in 2014. In order to understand this communication policy and these interactions, this study puts into context the social struggles for the construction of the Unified Health System (SUS); the formation of the Communication and Health field; and the National Policy on Social Participation of the Federal Government to the digital environment. The paper uses the theoretical and methodological framework of Social Theory of Discourse and Bourdieu s notions of field and symbolic power. / Análise da política de comunicação do Ministério da Saúde nas redes sociais na internet. A pesquisa tem como foco as interações no ambiente virtual entre os usuários e o Ministério da Saúde a respeito das políticas públicas de saúde. Em uma sociedade midiatizada e conectada em rede, o Ministério tem o desafio de alterar sua lógica de comunicação historicamente pautada pelo modelo campanhista unidirecional. Nesse sentido, o intuito é debater sobre as potencialidades e os limites da participação social nas mídias sociais em relação ao ciclo das políticas públicas de saúde. Para isso, utilizam-se como objeto empírico as interações sobre a primeira etapa da campanha de vacinação contra o HPV (Papilomavírus Humano) na fanpage do Ministério da Saúde no site de redes sociais Facebook realizada em 2014. Com o propósito de compreender essa política de comunicação e essas interações, contextualizam-se as lutas sociais para a construção do Sistema Único de Saúde (SUS); a formação do campo da Comunicação e Saúde; e a Política Nacional de Participação Social do Governo Federal para o ambiente digital. O trabalho utiliza os referenciais teórico-metodológicos da Teoria Social dos Discursos e as noções de campo e poder simbólico de Bourdieu.

Investigamos o contexto político-institucional que condicionou a incorporação do Método da Roda na instituição, buscamos identificar especificidades e singularidades da construção do Apoio em diferentes Secretarias e Departamentos do Ministério da Saúde; e propusemos uma matriz analítica com os modos de organização e de operação do Apoio Institucional na gestão federal. O estudo situa-se na categoria de pesquisa qualitativa, dentro do campo da pesquisa social em saúde. Definimos a Hermenêutica-Dialética como abordagem metodológica por considerarmos coerente com o referencial teórico utilizado - Apoio Paidéia e Dialética Marxista - e com os objetivos desta investigação. Analisamos documentos oficiais, Diretrizes Nacionais, Cadernos de Formação, Políticas e Programas que tratam do Apoio Institucional no Ministério da Saúde, produzidos por vários setores. Concluímos que o estudo mais aprofundado sobre o Estado é necessário para compreendermos o mundo da gestão e do trabalho em saúde. Entendemos que os sujeitos (individuais e coletivos) são influenciados e determinados pela macroestrutura, inclusive estatal, com disputa de hegemonia no conjunto de instituições, nas práticas sociais, na ideologia, na cultura, na política, na educação etc. Identificamos dilemas decorrentes de dificuldades, limites e contradições que os gestores do SUS vivenciam cotidianamente ao buscarem estratégias de mudança da lógica tradicional de governo (racionalidade gerencial hegemônica), seja na gestão democrática e participativa, seja na relação entre entes federados. Os documentos analisados expressam desejo da mudança, consistência teórica-política adequada a esse desejo e priorização de parte dos dirigentes. Entretanto, apresentam contradições dentro das mesmas equipes dirigentes, entre secretarias e políticas distintas, mostram dificuldade de integração entre as áreas do Ministério da Saúde e deste com os gestores descentralizados (estados e municípios) / Abstract: This study aims to understand the formulation of the Institutional Support by the Ministry of Health of Brazil and how this strategy of Paideia Method has been incorporated in the official discourse of the federal management of the SUS. We have researched the political and institutional context which led to the incorporation of the Paideia Method in the institution, also have tried to identify the specificities and peculiarities of the Support construction in different Secretaries and Departments of the Ministry of Health. Besides, we have proposed an analytical matrix with the manners of organization and operation of the Support Institutional of the federal management. The study is a qualitative research within the social research on health. We have defined the Dialectical Hermeneutics as the methodological approach as we consider it coherent to the theoretical framework which was used- Paideia Support and Marxist Dialectic - and also to the goals of this research. We have analyzed official documents, National Guidelines, Training Booklets and Policies and Programs which are concerned with the Institutional Support of the Ministry of Health produced by various sectors. We have concluded that a deeper study on the State is necessary to understand the world of management and furthermore the work on health. We have understood that the subjects (individual and collective) are influenced and determined by the macrostructure, including state, with fight for hegemony in the set of institutions, social practices, ideology, culture, politics, education etc. We have identified dilemmas which come from the difficulties, limits and contradictions that SUS managers experience daily while seeking strategies of changes of the government traditional logic (hegemonic managerial rationality), either in democratic and participative management or in the relationship between federal entities. The documents analyzed reflect a desire for change, political-theoretical consistency which suits this desire and priority of the leaders. Apresenta-se como estudo qualitativo de caráter exploratório, com base em estudo de caso da experiência de implantação, no ano de 2010, do NASF no Território-Escola Manguinhos, vinculado à Fundação Oswaldo Cruz e fruto da parceria com a Secretaria Municipal de Saúde do Rio de Janeiro. Seu objetivo central é analisar o processo de trabalho do NASF do Território-Escola Manguinhos, a partir da contribuição do apoio matricial proposto por Campos (1998) sob a perspectiva de quem apoia, é apoiado e do gestor da atenção básica. Busca conhecer, em contexto, a realidade dos trabalhadores em saúde, de forma a observar o modo de organização proposto pela equipe do NASF, com vistas a contribuir para aprimoramento do conhecimento no que se refere à atenção primária e ao trabalho de equipes de saúde da família. Construiu-se o desenho desta pesquisa após identificação dos aportes teóricos que sustentam a análise do objeto desta investigação, assim como dos elementos que se pretende explorar e da estratégia de análise desses materiais. Compreende-se que a metodologia de estudo de caso proposta é estratégica para a área da gestão em saúde, por descrever em profundidade e em detalhe o objeto em seu contexto. Logo, as estratégias para a elaboração do material empírico são a análise histórico-estrutural do campo e do processo de implementação do NASF, a análise documental e entrevistas semiestruturadas aplicadas aos gestores e aos profissionais do NASF e do ESF. Para a análise e a interpretação do material elaborado no campo, partiu-se do compromisso com a fidedignidade à compreensão dos dados empíricos, referindo-se às relações sociais e dinâmicas e vivas, segundo o proposto por Minayo (2002). Optou-se pela análise hermenêutico-dialética proposta pela autora, por possibilitar o exame e interpretação do discurso dos sujeitos entrevistados com base nas práxis. Conclui com proposições a partir das semelhanças e contradições dos discursos dos diferentes participantes implicados na organização do NASF Manguinhos / Abstract: This research analyses the work process performed by the Family Health Care Support Unit (NASF) based upon the concept of matrix support suggested by Campos. This is presented as a qualitative study of explanatory character, based on NASF¿s establishing experience at the Manguinhos school territory case study, associated with Oswaldo Cruz Foundation, and as a product of a partnership with Rio de Janeiro¿s City Health Secretariat. The study¿s main objective is to analyze the contribution of a matriarchal support suggested by Campos (1998) applied in NASF¿s work process in the Manguinhos school territory, by the supporter¿s, supported¿s and basic health care manager¿s perspective. It explores into the health care workers¿ reality in context observing the organization methods proposed by NASF, focusing on the improvement of knowledge on primary health care and family health care team work. The research¿s design occurred with the identification of theoretical contributions that give foundation to the investigation¿s object analysis, the elements about to be explored and the materials analysis strategy. It is understood that the case study method suggested is strategic to the health care management field for its depth on object¿s description and detail, in context. Therefore empiric material consists of NASF¿s establishing process and the field¿s structural-historical analysis, documents¿ and semi-structured interviews done to management and NASF¿s and ESF¿s crew analysis. The analysis and interpretation of field generated material was committed to reliability on empirical data¿s comprehension, concerning social and dynamic and alive relations, according to Minayo (2002). An hermeneutic-dialectic analysis was chosen as suggested by the author, for the reason that it makes possible to consider and interpret the interviewed subjects¿ speech, praxis based. No âmbito da Atenção Básica, o financiamento dos medicamentos é regulamentado pelo Componente Básico da Assistência Farmacêutica (CBAF) e sua execução é feita pelos entes federados de acordo com a legislação deste Componente. O objetivo geral deste trabalho foi: analisar os preços de medicamentos do CBAF adquiridos pelos municípios que compõem o Projeto QualiSUS-Rede. O Projeto de Formação e Melhoria da Qualidade de Rede de Atenção à Saúde (QualiSUS-Rede) constitui uma intervenção do Ministério da Saúde para promover a implantação de quinze RAS, cuja conformação contempla 17 estados brasileiros. A "Pesquisa Assistência Farmacêutica nas RAS" é parte da Intervenção Sistêmica da Assistência Farmacêutica no Projeto QualiSUS-Rede, e consiste em estudo transversal realizado por meio de entrevistas realizadas com Secretários Municipais de Saúde e equipe de Assistência Farmacêutica nos municípios de 15 Regiões do Projeto QualiSUS-Rede, cuja equipe da Intervenção Sistêmica é composta por entrevistadores/pesquisadores específicos para cada Região. A metodologia deste estudo consiste em análise secundária do estudo transversal "Pesquisa da Assistência Farmacêutica nas RAS", em que foram analisadas as variáveis relativas à forma de pactuação para o repasse da contrapartida estadual do CBAF (compra centralizada no Estado ou descentralizada aos municípios) e o preço unitário dos medicamentos do CBAF adquiridos no último processo licitatório pelos municípios das Regiões QualiSUS-Rede. Para as análises, os medicamentos do CBAF foram identificados segundo a classificação Anatômica Terapêutica e Química (Anatomical Therapeutic Chemical - ATC), a variação de preços dos medicamentos das Regiões foi identificada, o percentual médio dos preços unitários foram comparadas com o Banco de Preços em Saúde (BPS) e o Teste não paramétrico de Wilcoxon foi realizado para testar a hipótese da existência de diferença na média dos preços unitários entre as Regiões, em função do tipo de pactuação para a execução dos recursos do CBAF. A Região que realiza compra centralizada pelo Estado (RSCJN/CE) apresentou menor variação de preços unitários e maior frequência de média de preços unitários abaixo do BPS. O Teste de Wilcoxon foi significativo para 67% dos medicamentos adquiridos por ambas Regiões no último processo licitatório, comprovando a hipótese de diferença entre as médias das Regiões em função do tipo de pactuação. O estudo evidenciou que o tipo de pactuação para repasse da contrapartida estadual dos recursos do CBAF aos municípios, na forma de aquisição centralizada de medicamentos pelo Estado, é relevante na obtenção de menores preços unitários dos medicamentos do CBAF. Palavras-chave: Preços de medicamentos, Assistência Farmacêutica, Componente Básico da Assistência Farmacêutica, QualiSUS-Rede / Abstract: The Pharmaceutical Assistance, as one of the goals from the Brazilian Public Health System (called SUS), has been consolidating over the years, through the National Medicines and Pharmaceutical Assistance Policies. The Basic Component of Pharmaceutical Assistance (BCPA) regulates the financing of drugs to the Primary Care and its implementation has been occurring by the federal entities in accordance with the rules of this Component. The research focus was to: analyze the BCPA drug prices purchased by municipalities that composed the QualiSUS-Network Project. The Training Project and Quality Betterment of Health Care Network (QualiSUS-Rede) is an intervention of the Ministry of Health to promote the deployment of fifteen Health Attention Networks (HAN), whose conformation has 17 states. The "Research Pharmaceutical Services in HAN" is part of Systemic Intervention from Pharmaceutical Assistance in QualiSUS-Network Project, and consists of cross-sectional study conducted through interviews with Municipal Secretaries of Health and Pharmaceutical Assistance team in the municipalities of 15 Project Regions QualiSUS-Network, whose staff of Systemic Intervention consists of interviewers / researchers for each specific region. The methodology of this study consists of secondary analysis of cross-sectional study "Research Pharmaceutical Assistance in HAN", in which was analyzed the variables focused on the form of the current pact to the transfer of state counterpart of BCPA (centralized or decentralized purchasing in the state to municipalities) and the unit price of BCPA of drugs purchased in the last bidding process by the municipalities of the Regions QualiSUS-Network. For the analyzes, the BCPA medications were identified according to the international classification Anatomical Therapeutic Chemical (Anatomical Therapeutic Chemical - ATC), the drug prices variation of the Regions has been identified, the average unit prices were compared with the Bank of Prices Health (BPH) and the nonparametric Wilcoxon test was used to test the hypothesis of difference in average unit prices of the Regions, according to the type of pact for implementing the BCPA resources. The Region that performs centralized purchasing by the state (RS CJN / CE) showed less variation in unit prices and higher mean frequency of unit prices below the BPH. The Wilcoxon test was significant for 67% of drugs purchased by both Regions in the last bidding process, proving the hypothesis of difference between the means of the Regions on the type of pact. The study showed that the type of pact to transfer the state counterpart of CBAF resources to municipalities in the form of centralized procurement of medicines by the State, it is important to obtain lower unit prices CBAF medications. El Perú posee un sistema de salud fragmentado, compuesta por seis instituciones, entre ellas el Ministerio de Salud (MINSA), el cual en el marco de la Ley N° 30024, Ley que crea el Registro Nacional de Implementación de Historias Clínicas Electrónicas y la Ley de Gobierno Digital, desarrolla el Plan Nacional de Implementación el Sistema de Información de Historias Clínicas Electrónicas SIHCE e-Qhali para las IPRESS del Primer Nivel de Atención 2019-2021, con la finalidad de contribuir a mejorar la calidad de la atención de salud a los usuarios de los servicios de salud a través del mencionado sistema; sin embargo, durante dicho proceso se han presentado diversas limitaciones que han impedido el avance de dicha implementación. En ese sentido, la presente investigación toma como caso de estudio una IPRESS de Lima Norte con el objetivo de diagnosticar la situación de la Implementación del Sistema de Información de Historias Clínicas Electrónicas (SIHCE) e-Qhali en el Centro Materno Infantil El Progreso, durante el periodo del 2019 al 2020, a través del análisis de las fases de Preparación, Planificación, Ejecución, Seguimiento y Control considerados en el precitado plan de implementación; así como la utilidad de dicho sistema en el CMI El Progreso. / Peru has a fragmented health system, made up of six institutions, including the Ministry of Health (MINSA), which within the framework of Law N°. 30024, Law that creates the National Registry for the Implementation of Electronic Medical Records and the Digital Government Law, develops the National Plan for the Implementation of the Electronic Medical Records Information System - SIHCE e-Qhali for the IPRESS of the First Level of Care 2019-2021, in order to contribute to improving the quality of health care to the users of health services through the aforementioned system; however, during said process, various limitations have arisen that have impeded the advancement of said implementation.
