INTERCEPTOR TARGET MISSILE TELEMETRY

This test tool must be highly reliable, low cost and simple and must perform any task that the developing interceptor missile planners require. The target missile must have ample power and guidance resources to put the target in a specified place in the sky at a desired time. The telemetry and measurement system for the target missile must have the same requirements as its interceptor missile but must be flexible enough to accept new requirements as they are applied to the target and its interceptor. The United States Army has tasked Coleman Aerospace to design and build this type of target missile. This paper describes and analyzes the telemetry and instrumentation system that a Hera target missile carries. This system has been flying for the past two years, has completed seven out of seven successful test flights and has accomplished all test objectives to date. The telemetry and instrumentation system is an integral part of the missile self-test system. All preflight checks and flight simulations are made with the on-board three-link telemetry system through a radio frequency (RF) link directly through the missile antenna system to a ground station antenna. If an RF transmission path is not available due to test range restrictions, a fiber-optic cable links the pulse code modulator (PCM) encoder to the receiving ground stations which include the bitsync, decommutator and recorders. With this capability, alternative testing is not limited by RF test range availability. The ground stations include two mobile stations and a factory station for all testing including preflight testing of the missile system prior to flight test launches. These three ground stations are built in a single configuration with additional equipment in the mobile units for use at remote locations. The design, fabrication, testing and utilization of these ground stations are reviewed. The telemetry system is a modification of the classical PCM system and will operate with its interceptor missile at least into the first decade from the year 2000.

Beyond ‘Cesarean Overuse’ : Hospital-Based Audits of Obstetric Care and Maternal Near Miss in Tehran, Iran

With one in two women delivering surgically, Iran has one of the highest rates of cesarean section (CS) worldwide. CS overuse in low-risk pregnancies potentially exposes women to Maternal Near-Miss (MNM) morbidity with minimal health benefits. This thesis studied obstetric care quality and MNM at hospitals with high rates of CS in Tehran, Iran. In Study I, we investigated whether audits of CS indications and feedback influenced CS rates at a general hospital. Subsequent to the audit, a 27% reduction in the risk of primary CS was found. In Study II, characteristics of MNM were investigated at university hospitals between 2012 and 2014. During a 26-month period, 82 MNM cases were identified using the WHO MNM approach. Severe postpartum hemorrhage (35%), severe preeclampsia (32%), and placenta previa including abnormally invasive placenta (10%) were the main three causes of MNM. Iran has a huge influx of migrants from Afghanistan. Women with antepartum CS and those who lacked health insurance, almost all Afghans, had increased risk of MNM. In Study III, audits examined whether MNM care quality differed between 54 Iranians and 22 Afghans and whether near-miss events were preventable. A majority of MNM cases (62%) arrived at hospital in a moribund state and obstetric care was more suboptimal for Afghans than Iranians (adjusted odds ratio 5.1, 95% confidence interval 1.2–22.6). Moreover, MNM was commonly (71%) potentially preventable and professionals with suboptimal practice were involved in 85% of preventable cases. In Study IV, a qualitative interview study was conducted to explore care experiences of Afghan MNM survivors. Discrimination, insufficient medical attention, and ineffective counseling were the main experiences. To a lesser extent, poverty and low education were perceived as contributing factors to delays in accessing care. This thesis emphasizes the importance of high-quality care for preventing undesirable maternal outcomes. The audit method along with interviews was useful to determine quality and equity gaps in care provision. Policymakers and professionals should consider these gaps when structuring programs to reduce adverse maternal outcomes.

No sistema de saúde, o Hospital Terciário/Quaternário é centro de referência para pacientes em situações graves que as colocam em risco de morte. A Maternidade do HC-FMB-Unesp é um dos centros terciário/quaternário de referência para patologias obstétricas do DRS-VI do Estado de São Paulo. A análise da frequência de situações de risco que chegam à maternidadegestantes portadoras de hipertensão arterial, bem com a assistência que receberam até serem referenciadas e ao chegarem à maternidade, poderá identificar deficiências e propor o seu aprimoramento, reduzindo-se assim o risco do binômio mãe-feto. Objetivo: Determinar a frequência de casos de morbidade materna grave não near missque evoluíram para situação de near miss, associados à hipertensão arterial e propor protocolo clínico de assistência pré-natal, bem como aprimorar o protocolo de assistência praticada no centro terciário com finalidade de reduzir o risco do binômio mãe-feto. Over the past 20 years, the concept of near miss has been addressed in maternal health as an adjunct to confidential maternal death surveys. Objective: To determine the frequency of evolution of cases of severe maternal morbidity not near miss (SMM not MNM) for maternal near miss (MNM) associated with hypertension in the reference center of the Polo Cuesta and Jurumirim Vale subareas of the Regional Health Division - VI (DRS-VI) of the State of São Paulo. Methods: A cross-sectional retrospective study was carried out between 2015 and 2016 at a tertiary/quaternary reference center for the Polo Cuesta and Jurumirim Vale sub-areas of DRS-VI in the state of São Paulo. A total of 167 pregnant women or postpartum women with a diagnosis of hypertension with signs of severity were included. In addition to the frequency of near miss, demographic, clinical and origin data were obtained according to the subarea of the regional health study population. The problem of placing patients in the homes of paid caretakers is multifarious and complex. Among the most basic of the questions facing the agency planning to place patients is: what kind of patient to place in what kind of a home? When stated criteria are lacking for the selection of patients for both types of settings from the same hospital, the social worker's judgments become the gauge by which selection is made. This situation prevails at the Veterans Administration Hospital at Gulfport, Mississippi. Delineation of the factors contributing to successful placement would be useful in the selection and preparation of patients for community placement. The data of this study are primarily related to the successful community placement made by sixty white male veterans. Answers were sought to the following questions by an analysis of the data collected on 34 descriptive items. (1) How does the literature present and describe halfway house and foster home care as a form of community placement for male veteran patients? (2) Do the 30 veteran patients who have made a successful adjustment in the foster home care differ significantly from the 30 veteran patients who have made a successful adjustment to halfway house care when distributed on 10 selected social or baseline characteristics?
