Mucosal immune system comprises not only the major compartment of the immune system but also important interface with the outer environment. It is responsible in maintaining an intricate balance with the danger and non-danger stimuli of the outer world by employing specific anatomical features and unique functional mechanisms. Mucosal immune system has been long understudied, perhaps due to the limited accessibility, and its biological importance is thus still underevaluated. However, it has become evident that it is important to study mucosal immune system not only in local mucosal affections but also when uncovering pathogenic mechanisms and novel prevention strategies of organ specific autoimmune diseases such as type 1 diabetes. Thus, the first, more clinically oriented part of this thesis is focused on mucosal immune system of the upper respiratory tract in disease conditions - in nasal polyposis (NP). Because there is a substantial accumulation of eosinophils and neutrophils in the most frequent type of NP, we investigated and described increased expression of chemokine receptors CCR1 and CCR3 in NP versus nasal mucosa. Die Anwendung der Akustischen Rhinometrie beim Hund in der klinischen Veterinärmedizin

Die akustische Rhinometrie ist in der Humanmedizin eine etablierte, nicht-invasive Methode zur Bestimmung von Nasenhöhlenvolumina und definierten Querschnittsflächen in nasalen Atemwegen beim Menschen. Sie basiert auf dem Vergleich beziehungsweise der Analyse der Amplituden von Schallwellen (definiert über die Querschnittsflächen) als Funktion der Zeit (definiert durch den Abstand in die Nasenhöhle), die durch Reflexionen einer Ausgangs-schallwelle in der Nasenhöhle entstehen. Das Ziel dieser Studie war es, erstmals den Nutzen der akustischen Rhinometrie für die klinische Veterinärmedizin bei Hunden zu überprüfen. Die Hauptschwerpunkte lagen in der Quantifizierung des geometrischen Aufbaus der Nasenhöhle von gesunden (n=15) und kranken (n=32) Hunden über die Auswertung der Flächenabstandskurven, der Definition von minimalen Querschnittsflächen (MCA1, MCA2) und Zuordnung zu anatomischen Landmarken auf CT-Schnittbildern. Für die Messungen wurde das Rhinometer SRE 2000 (RhinoMetrics, Dänemark) verwendet. Voruntersuchungen zu Wiederholbarkeit und Genauigkeit der Messmethode erfolgten an zylindrischen Stufenmodellen (n=3). Für jeden Beagle konnte eine deutliche Korrelation der ersten minimalen Querschnittsflächen zur Spitze der Concha nasalis ventralis nachgewiesen werden. Die zweite minimale Querschnittsfläche ließ sich keiner anatomischen Landmarke zuordnen. Es bestand kein statistisch signifikanter Unterschied in Nasenhöhlenvolumina, Auftreten der MCA1 und MCA2 und ihrer entsprechenden Dimension der jeweils rechten und linken Nasenhöhle im Gruppenvergleich. Nach lokaler Applikation von Xylometazolin lag die Größenzunahme des Volumens der rechten Nasen-höhle bei 19,4% [3,9-24,7%] (n=13) und der Linken bei 23,7% [15,4-36,4%] (n=12). Von insgesamt 32 untersuchten Hunden zeigten acht Hunde eindeutig einseitige Veränderungen (25%) in den CT-Schnittbildern. Anhand der Kurvensymmetrie ließ sich die pathologisch veränderte Nasenhöhlenseite nur in vier von acht Fällen zweifellos zuordnen. Besonders bei Erkrankungen mit Rhinorrhoe oder Ansammlungen von Sekret innerhalb der Nasenhöhle kam zu Fehlinterpretationen der Flächenabstandskurven. Oszillationen traten bei 22% der Patienten auf. Die Ergebnisse lassen Rückschlüsse zu, dass sich die akustische Rhinometrie aufgrund einer guten Wiederholbarkeit und einfachen Anwendung für wissenschaftliche Studien eignet, bei denen der Schwerpunkt auf intraindividuellen Vergleichen liegt. Obwohl die Daten objektiv ermittelt werden, ist die Anwendung dieser Methode in der klinischen Veterinärmedizin bei Hunden nur in Kombination mit anderen Untersuchungsmodalitäten sinnvoll. Fehlmessungen treten bereits bei einfach strukturierten Modellen auf; am caninen Patienten nehmen sie durch die ansteigende Diskrepanz zum idealen theoretischen Modell weiter zu. Primäre pathologische Veränderungen in der Nasenhöhlengeometrie werden nicht erkannt oder durch sekundäre Veränderungen maskiert. Verschiedene Krankheitsbilder weisen gleiche oder ähnliche strukturelle Veränderungen in der Nasenhöhle auf und können durch Messungen mit akustischen Schallwellen nicht unterschieden werden. Die Interpretation von Absolutwer-ten ist kritisch. Die Quantifizierung einer intranasalen Stenose ist nicht möglich.

Perception and Production of French Nasal Vowels by German Native Speakers

This thesis explores the challenges faced by German native speakers in perceiving and producing French nasal vowels. French has three to four nasal vowels, while German lacks them entirely. The study analyzes German speakers of different proficiency levels using a perception task and production measurements. The results indicate that German speakers struggle in perceiving nasal contrasts, but can perceive nasality in itself. Acoustic analysis reveals deviations in nasalance and formant values compared to native French speakers. However, L2 speakers successfully master other markers of nasality. The findings highlight the interplay between perception and production, impacting the acquisition of nasal vowels. These results contribute to second language research, emphasizing the difficulty of nasal vowels for L2 learners and their implications for language teaching. Overall, nasal vowels pose a consistent challenge, varying based on speakers' native languages.

Towards an empirical and typological exploration of the sonority of nasal vowels

Ce mémoire est une étude phonologique et phonétique visant à ordonner les voyelles nasales en fonction de leur degré en sonorité. Jusqu'à présent, la sonorité des voyelles nasales n’a pas été abordée dans la littérature phonologique et phonétique; ainsi, cette étude vise à combler cette lacune de la recherche en menant une étude typologique et expérimentale des différentes qualités des voyelles nasales. La première partie du mémoire comprend un examen du comportement et de la distribution des voyelles nasales dans les langues du monde afin d'élucider leur place dans l’échelle de sonorité. La partie typologique montre que, dans trois des langues étudiées, seules les voyelles nasales hautes subissent des processus phonologiques comme l’harmonie nasale et la réduction vocalique tandis que, dans deux des langues étudiées, seule la voyelle nasale basse subit des processus phonologiques comme le changement vocalique déplacement de la voyelle et l’attraction de l’accent. La deuxième partie est une expérience nasométrique dans laquelle l’intensité des voyelles nasales et orales est mesurée. L’analyse d’intensité permet de déterminer le niveau de sonorité de chaque voyelle nasale car l’intensité est considérée comme le corrélat physique le plus saillant de la sonorité. L'analyse statistique descriptive effectuée sur l'ensemble des données des voyelles organisées selon leur hauteur (c'est-à-dire haute, mi-haute, mi-basse ou basse) et leur type (nasal ou oral) montre que [a] est la voyelle orale la plus haute en intensité relatif. Quant aux voyelles nasales, [ĩ] est la plus élevée en intensité. Ainsi, suite à l’analyse statistique inférentielle, nous avons établi une échelle permettant de classer les voyelles orales et nasales en fonction de leur intensité. / This thesis is a phonological and phonetic study to classify nasal vowels according to their rank in sonority. To date, the sonority of nasal vowels has not been covered in the phonology and phonetics literature; thus, this study aims to fill that research gap through conducting a typological and experimental investigation of different qualities of nasal vowels. The first part of this thesis includes an examination of the behaviour and the distribution of nasal vowels in the world’s languages to elucidate the place of nasal vowels in sonority hierarchy. The typological part of this study shows that, in three selected languages, only high nasal vowels undergo phonological processes such as nasal harmony and vowel reduction while, in two other selected languages, only the low nasal vowel undergoes phonological processes like vowel shift and attraction of stress. The second part of this study presents the findings of a nasometric experiment in which the intensity of nasal vowels and oral vowels is measured. Analysing intensity helps to determine the level of sonority of each nasal vowel, because intensity is considered the most salient physical correlate of sonority. The descriptive statistical analysis performed on the data set of vowels organized according to height (i.e., high, mid-high, mid-low, and low) and type (nasal or oral) shows that, [a] is the highest in relative intensity. As for nasal vowels, [ĩ] is the highest in intensity. Thus, according to the inferential statistical analysis, we established a scale that classifies oral and nasal vowels according to their intensity.

Patrones de tabiques sinusales evaluados con tomografías computarizadas de haz cónico en un centro de formación odontológica, Chiclayo, 2017-2019

El objetivo del estudio fue determinar los patrones de tabiques sinusales evaluados con tomografía computarizada de haz cónico (CBCT). El estudio fue descriptivo, transversal y observacional. Se evaluaron 157 CBCT del Centro Especializado en Formación Odontológica de la Universidad Católica Santo Toribio de Mogrovejo, conformado por 95 tomografías del sexo femenino y 62 del sexo masculino. Se incluyeron tomografías donde ambos senos maxilares eran visibles, se excluyeron las que presentaron artefactos técnicos que dificultaron la visualización del seno maxilar. El estudio fue aprobado por el Comité de Ética en investigación de la Facultad de Medicina de la Universidad Católica Santo Toribio de Mogrovejo. Se realizó la prueba piloto con el objetivo de calibrar al investigador, se obtuvo una de >0.81 mediante el coeficiente Kappa de Cohen. Los resultados mostraron que la orientación de tabique que predomina fue el coronal con 14,6% y medio con 20,4% en una vista sagital, con mayor presencia en el lado izquierdo con 29,9%, predominando el sexo femenino con 28,4% y finalmente la mayoría de los pacientes fueron desdentados parciales con 74,1%. Concluyendo que el patrón de tabique sinusal más frecuente fue el coronal.

The relationship between circulating biomarkers of nitric oxide and endothelin-1 and hemodynamic function in obstructive sleep apnea

Obstructive sleep apnea (OSA) is a disorder that affects a significant portion of middle-aged adult population. Patients exhibit recurring episodes of upper airway obstruction during sleep that decrease blood oxygen concentration (hypoxia) and are terminated by brief arousals. Epidemiologically, OSA has been extensively linked to cardiovascular dysfunction and is an independent risk factor for the development of hypertension. The proposed mechanism of cardiovascular dysfunction in patients is chronic sympathoexcitation and altered vascular tone, with a predominance of the vasoconstrictor endothelin-1 (ET-1) and removal of the vasodilator nitric oxide (NO). Means to reduce the effects of ET-1 and increase synthesis of NO may have beneficial effects on the cardiovascular co-morbidity commonly associated with OSA. OBJECTIVES: The major aim of this study was to assess the relative importance of circulating biomarkers of ET-1 and NO in hemodynamic function in OSA patients. Potential production of ET-1 by circulating mononuclear cells was also measured to assess their contribution to plasma ET-1 levels. Biomarker levels before and after 12 wk of continuous partial airway pressure (CPAP) therapy were used to assess standard treatment. Mild/moderate exercise training was initiated with CPAP therapy in a subgroup of OSA patients to evaluate the potential benefits of physical activity on hemodynamic function and NO and ET-1 levels. METHODS: Overall, 16 newly diagnosed OSA patients (5 female, 11 male; age 45.4 ± 2.7 yr; RDI 24.6 ± 4.0 events/hr) were selected for study. Seven apparently healthy control volunteers (5 female, 2 male; age 39.43 ± 2.6 yr) screened for OSA served as control subjects. Blood pressure was recorded over one complete day and prior to, during, and following maximal exercise testing on a cycle ergometer. Blood samples were taken prior to exercise testing and assessed for nitrate and nitrite by HPLC and for big endothelin-1 and ET-1 by ELISA. Relative gene expression of preproendothelin-1 was measured by real-time RT-PCR. Following initial testing, patients were stratified into either a standard therapy group (nCPAP) or a standard therapy group with a mild/moderate intensity aerobic training regimen (nCPAP+Ex). Baseline testing was repeated following 12 wk of treatment. Statistical significance was set at p < 0.05 a priori. RESULTS: 24 hr ambulatory systolic and diastolic blood pressure were elevated in OSA patients vs. control subjects (systolic: 128.9 ± 3.8 mmHg vs. 108.8 ± 1.3 mmHg, respectively; diastolic: 97.5 ± 2.0 mmHg vs. 82.1 ± 1.9 mmHg, respectively). OSA patients experienced significant elevations in systolic (OSA 209.7 ± 5.7 mmHg; Control 174.5 ± 6.2 mmHg) and mean arterial pressures (OSA 125.8 ± 3.2 mmHg; Control 109.05 ± 4.5 mmHg) at peak exercise. No differences in nitrate, nitrite, or big endothelin-1 were noted. Plasma endothelin-1 concentrations were below assay detection limit. Big endothelin-1 levels were significantly correlated with BMI in both OSA patients (r=0.955; p=0.001) and control subjects (r=0.799; p=0.045). Relative gene expression of preproendothelin-1 was not elevated in OSA patients (0.40 ± 0.20 fold increase over control subjects). Group nCPAP usage was above minimum therapeutic threshold, but was non-uniform in both groups, with an overall range of 182 to 495 min mean usage per night. A mild/moderate exercise training program failed to elicit a training response through standard hemodynamic or cardiopulmonary indices. Plasma nitrite levels rose from 55.3 ± 4.7 μg/ml to 71.0 ± 7.6 μg/ml in the nCPAP group. CONCLUSIONS: Moderate OSA is associated with elevated blood pressure at rest and during exercise stress that bears no relationship to circulating biomarkers of NO and ET-1 or immune preproendothelin production in patients without diagnosed hypertension. nCPAP therapy failed to elicit significant improvements in hemodynamic function, with or without moderate exercise. Plasma nitrite levels rose following nCPAP therapy, indicating a possible increase in basal nitric oxide formation. Higher intensity exercise regimens may be needed to elicit the positive benefits of exercise training in OSA patients without significant cardiovascular dysfunction. / Ph. D.

Influences de l'écrit sur la perception auditive : le cas de locuteurs hindiphones apprenant le français / Influences of written information on auditory perception : a case study of Hindi native speakers learning French

S’il est aujourd’hui communément admis que la perception de la parole s’effectue d’une manière plus optimale en mode audiovisuel qu’en mode auditif seul (Benoît, Mohamadi et Kandel, 1994, Schwartz, Berthommier et Savariaux, 2004), la nature des informations visuelles dont il est le plus souvent question est la mimo-gestuelle articulatoire fournie par le locuteur en face à face. Cependant, dans une situation d’enseignement d’une langue étrangère, un autre type d’aide visuelle intervient généralement : la forme écrite des éléments oraux. Pourtant en didactique des langues étrangères, la question du passage à l’écrit est loin d’être consensuelle et certains didacticiens se prononcent en faveur d’un entraînement intensif de la prononciation au tout début de l’apprentissage, avant que l’apprenant ne soit confronté au code écrit (Lauret, 2007). Notre hypothèse est que la dimension facilitante de la forme écrite pour certains publics ne doit pas être négligée, même en début d’apprentissage. Notre recherche se fonde sur des expérimentations menées auprès d’apprenants hindiphones. Tenant compte des spécificités de ce public, nous pensons que l’écrit peut, dans certains cas, faciliter sa réception orale des sons du français en début d’apprentissage, condition nécessaire et préalable à leur production (Renard, 1979). Les tests que nous avons conçus obligent les apprenants à recentrer leur attention sur la graphie de sons (les voyelles nasales [ɑ̃] et [ɔ̃]) dès le début de leur apprentissage au moyen de diverses focalisations visuelles écrites (Fort, Spinelli, Savariaux et Kandel, 2010). Les propositions didactiques que nous formulons à la suite reposent sur la suite logique perception – (transcription graphique) — production même si cette présente étude se centre sur l’évaluation de la perception des sons. / It is commonly admitted today that speech perception is more performing in an audiovisual context than in a visual one (Benoît, Mohamadi and Kandel, 1994, Schwartz, Berthommier and Savariaux, 2004). Visual information in this situation often consists of the speaker’s articulatory and facial gestures provided by the face-to-face interaction. However, when learning a foreign language, another type of visual help is generally available to identify oral forms: their written forms. And yet, in the field of didactics of foreign languages, the issue of the oral-written transition is far from being consensual and some didacticians favour training the pronunciation skills of the learner at the beginning of the learning process, before he is confronted to the written code (Lauret, 2007). Our hypothesis is that the facilitating effect of written forms should not be neglected, even at the beginning of a foreign language learning process. Our research is based on the case of Hindi speakers. Taking into consideration the specificities of this population, we think that written information can, in some cases, facilitate the oral reception of French sounds in the beginning of the learning process, which would be a preliminary condition to their production (Renard, 1979). We have conceived a series of test, forcing the Hindi speaking learners to refocus their attention on the nasal vowels [ɑ̃] and [ɔ̃]’s written form from the beginning of the learning process, using different forms of visual written focuses (Fort, Spinelli, Savariaux and Kandel, 2010). Our didactic proposal relies on the following process: perception – (written form transcription) – production, even though the present study is centred on the evaluation of speech perception.
