The thesis titled "Occupational safety and health" is focused on summarizing the current legal regulations that impose rights and obligations on employers and employees in the field of occupational health and safety. Introductory chapter emphasizes the understanding of the importance and function of safety aspects, historical context and role of the state. In the crucial second chapter the Czech legal regulations are analyzed in detail and the basic concepts of this issue (prevention of risk, work categorization, training...) are characterized. Following the legislation chapter three introduces options above standard approaches, by which the employer can be inspired or directly certified in task of the safety system solution. In the next chapter the analysis of the current legislation of "Occupational Services" is presented and also compared with the previous one. The theoretical part is concluded with an article concerning work accidents and occupational diseases, in which are also defined possibilities of accident insurance. In the case study, I focused on the application of the legal regulations for a particular employer, evaluation of the current state and the proposal for possible remedy measurements.

Síndrome de Burnout e estratégias de enfrentamento em hospital público de urgência e emergência / Burnout and coping strategies in urgency and emergency hospital preceptors

This undergraduate thesis – TCC, composed of a scientific article and an intervention product analyzes the occurrence of Burnout Syndrome dimensions and associations with factors that compose the coping strategies used by preceptors in an emergency hospital and emergency room of Maceió. The study consisted of a quantitative descriptive cross-sectional study. There was a non-probabilistic sample of 124 professionals from social service, nursing, medicine and psychology areas who, besides assistance to the users, play the role of preceptors within their routine activities. Descriptive statistics, chi-square test (χ²), analysis of variance (ANOVA) with post hoc tests, and r Pearson correlation analysis were used. We verified that 66.1% of the subjects (f = 82) work in more than one health institution, and have an average workload of 51.83 hours per week (SD = 15.08). Within coping strategies, the positive reassessment factor was the most prevalent for our sample, demonstrating an emotional focus to deal with the stressors in the environment. The second factor evidenced was problem solving, more effective in the daily services. Regarding MBI, it was evidenced that the sample studied presented a mean score for the three dimensions of burnout: 60 (48.4%) emotional exhaustion, 57 (46.0%) depersonalization and 54 (43.5%) professional achievement. Burnout syndrome is personal and institutional; therefore, it is necessary that institutions rethink management forms, and provide more adequate working conditions to minimize agents causing stress and increase resources for the use of more appropriate coping strategies. After obtaining results, the intervention product aimed to promote actions meant to reduce stress levels created by daily work activities, preventing diseases and generating general welfare. An informative primer on burnout and suggestions for stress management was also made. / Este trabalho Acadêmico de Conclusão de Curso – TACC, composto por um artigo científico e um produto de intervenção, analisa qual a ocorrência das dimensões da síndrome de burnout, bem como as associações com os fatores que compõem as estratégias de coping utilizadas pelos preceptores em um hospital de urgência e emergência de Maceió. A pesquisa consiste em um estudo de delineamento descritivo transversal quantitativo. Conta-se com uma amostra por conveniência de 124 profissionais das áreas de serviço social, enfermagem, medicina e psicologia que, além da assistência aos usuários, exercem o papel de preceptores dentro de suas atividades de rotina. São utilizadas estatísticas descritivas; teste qui-quadrado (χ²); análise de variância (ANOVA) com testes post hoc; análise de correlação r Pearson. Verifica-se que 66,1% dos sujeitos (f=82) atuam em mais de uma instituição de saúde e possuem carga horária média de 51,83 horas semanais (DP=15,08). Dentro das estratégias de coping, o fator reavaliação positiva é o mais prevalente para a amostra, demonstrando um enfoque emocional para lidar com os estressores no ambiente. O segundo fator evidenciado é a resolução de problemas, mais eficaz no cotidiano dos serviços. Quanto ao MBI, evidencia-se que a amostra estudada apresenta escore médio para as três dimensões de burnout: 60 (48,4%) exaustão emocional, 57 (46,0%) despersonalização e 54 (43,5%) realização profissional. A síndrome de burnout é de ordem pessoal e institucional, portanto, faz-se necessário que as instituições repensem as formas de gestão, assim como proporcionem condições laborais mais adequadas, minimizando os agentes causadores de estresse e ampliando os recursos para utilização de estratégias de coping mais apropriadas. Após a obtenção dos resultados, o produto de intervenção tem como objetivo promover ações que visem à diminuição dos níveis de estresse gerados pelo cotidiano das atividades laborais, prevenindo doenças e gerando bem-estar geral. Também é formatada uma cartilha informativa sobre burnout e sugestões de administração do estresse.

Work-related exposures and disorders among physical therapists: experiences and beliefs of professional representatives assessed using a qualitative approach

Background According to international study results, physical therapists are afflicted with work-related musculoskeletal, psychosocial and dermal disorders as well as infections. The few existing studies in German-speaking regions focus mainly on dermal and psychosocial exposures and resulting complaints. An overview of all relevant work-related exposures and complaints of physical therapists is currently lacking. We sought to identify work-related exposures based on the subjective experiences and beliefs of physiotherapeutic representatives, in order to identify relevant work-related complaints and diseases. Likewise we aimed to compare the international evidence with the actual situation of physical therapists in Germany. Methods Two complementary qualitative approaches were used: 1) a focus group discussion with representatives of professional physiotherapy associations as well as health and safety stakeholders and 2) qualitative semi-structured telephone interviews incorporating currently employed physical therapists. The group discussion was conducted applying a moderation technique, and interviews were analyzed using the content analysis approach by Mayring. Results The focus group discussion with five participants and the 40 semi-structured interviews with physical therapists identified comparable results. The main exposures of physiotherapeutic work were considered to be musculoskeletal (e.g., awkward body postures during treatment, patient transfers, passive mobilization), psychosocial (e.g., statutory audit of prescriptions and the associated conflicts with doctors and health insurance providers) and partly dermal and infectious (e.g., wet work and risk of infection) factors. Diseases of the spine, wrist or finger joints, burnout syndrome and infections were mentioned as possible consequences. Conclusions The subjective data generated by both groups (focus group discussion and interviews) were comparable and consistent with the current state of research. The results provide new insight regarding work-related exposures and diseases of physical therapists working in Germany. These findings aided the design of a German-wide representative survey of practicing physical therapists.

6 ABSTRACT Background The existence of risks of new work-related disorders and occupational diseases in the society is constantly increasing in relation to the fast and continuous development of working conditions, techniques used, materials and overall acceleration of the working pace. Aim of the study The aim of the study is to analyse new risks in the working environment and to evaluate early diagnostic possibilities of new occupational diseases including their prevention. The study also compares the current situation in this field in the Czech Republic and other European countries. Materials and methods The doctoral dissertation is a multiple manuscript thesis focusing on one common topic - new occupational diseases. The first publication studied the acknowledgement of low-back pain disease caused by overload as an occupational disease. Specialists from European countries were inquired via questionnaire focusing on diagnostic and evaluation criteria of this disease in their countries. Representatives from countries acknowledging low-back pain diseases caused by overload provided further evaluation criteria and number of cases. Text in Afrikaans / The purpose of the research was to determine the extent to which health issues, and particularly work stress, are managed at a welfare organization. The group investigated consisted of 49 registered social workers employed at nine welfare organizations in the Nelson Mandela Metropole. The research process was directed by quantitative research, the research information being obtained through a literature study and structured questionnaire. A number of health issues were identified in the workplace. Based on the empirical investigation it was concluded that health management does not enjoy a high priority at welfare organisations. The health issues that have the most impact on the respondents' work ability are work stress and burnout. It is recommended that organisations compile a holistic policy for health management. There is a need for implementation of employee assistance and wellness programmes for health management. Such programmes promote the productivity and general wellbeing of social workers. / Die doel van die navorsingstudie was om te bepaal tot watter mate gesondheidskwessies, in die besonder werkstres, by 'n welsynsorganisasie bestuur word. Die ondersoekgroep het bestaan uit 49 geregistreerde maatskaplike werkers werksaam by nege welsynsorganisasies in die Nelson Mandela Metropool. Die navorsingsproses is deur kwantitatiewe navorsing gerig en die navorsingsinligting is met behulp van 'n literatuurstudie en gestruktureerde vraelys bekom. Daar word tans 'n aantal gesondheidskwessies in die werkplek geidentifiseer. Na aanleiding van die empiriese ondersoek is daar tot die gevolgtrekking gekom dat gesondheidbestuur nie 'n hoe prioriteit geniet by welsynsorgnisasies nie. Daar word aanbeveel dat organisasies 'n holistiese beleid ten opsigte van gesondheidsbestuur opstel. Patterns of occupational exposure to patients' body fluids among health care workers in Tikuranbesa University Hospital, Addis Ababa, Ethiopia

Background: Accidental exposure to patients’ body fluids (BFs) is an occupational hazard among health care workers (HCWs). The study aimed at describing the patterns of exposure to patients’ BFs among HCWs at a university hospital in Ethiopia. Methods: A contextual descriptive cross-sectional design was used for this study. Self-administered questionnaires were used to collect data. Results: The one year and professional life prevalence of occupational exposure to patients’ BFs among HCWs was 33.5% and 66.5% respectively. Circumstances that led to participants’ exposures to patients’ BFs include needle stick injuries to fingers and splashes to the eyes (82.4%); conducting procedures included blood withdrawal (10.8%) and inserting intravenous infusions (8.1%) and recapping of used needles (12.2%). Conclusion: Findings of this study generally indicated that occupational exposures to patients’ BF of different types and circumstances were common among all categories of HCWs in the study site. This high finding of BF exposure should not be over looked. HCWs should follow the universal precaution protocol and PEP need to be strengthened / Health Studies / M.A. (Public Health)

A cross-sectional study of tuberculosis among workers in Tygerberg Academic Hospital, Western Cape province, South Africa

Thesis (MMed)--Stellenbosch University, 2013. / ENGLISH ABSTRACT: Introduction: The morbidity and mortality associated with tuberculosis (TB) disease is of grave consequences for the health and employment of afflicted individuals. Healthcare workers are identified amongst high risk groups in communities. The prevalence/incidence of TB is dependent on the presence of associated risk factors which varies in diversity and intensity in different communities and workplaces. Understanding the risk factors operating in any given environment is indispensable to any tuberculosis control programme. Objective: The objective of this study was to describe the occurrence and trends of TB disease as well as to determine the risk factors associated with the disease among Tygerberg hospital employees. Method: A cross-sectional descriptive study design with a nested case-control component was used to determine the occurrence (and trends) and risk factors of TB disease respectively. Occurrence and trends of tuberculosis: The frequencies, distribution and trends of TB disease from 2008 to 2011 were obtained by calculating and comparing the annual incidence rates for each variable. Cases were identified from the occupational health clinic TB register, while the various denominator data were obtained from the Human Resource database. Determination of risk factors: Cases were recruited from the occupational health clinic TB register and controls were randomly selected from unaffected workers during the study period. Self-administered risk factor questionnaires were completed by both cases and controls. Multivariate logistic regression analysis was used to determine the association between known and suspected risk factors and the occurrence of TB disease amongst employees. Results: Sixty six cases of TB disease occurred in the workforce during the study period resulting in an annual average incidence rate of 397/100,000 population (95% CI: 307/100,000-505/100,000). Twenty three (34.8%) of the 66 cases occurred in Housekeeping staff, making them the most affected sub-group [1181/100,000 population (95% CI: 747/100,000-1768/100,000)]. The rate of TB disease in nurses was 1.7 times (95%CI: 1.4-2.0) that of doctors. Workers in the 40-49 years age-group experienced the highest incidence [490/100,000 population (95%CI: 329.6/100,000-706.8/100,000)] of TB disease compared to the other age-groups. There was no obvious difference in gender occurrences. Disease rates varied among different racial groups, with the highest rate in black employees [1473/100,000 population, (95%CI: 924/100,000-1981/100,000)]. Distribution of TB disease in the institution was widespread, with security department being the most affected [2500/100,000 population (95%CI: 311/100,000- 9262/100,000)]. There was a downward but statistically insignificant (annual range 9-23; p=0.28) trend in the rate of disease occurrence over the study period. No previous training on TB prevention (OR: 2.97, 95% CI: 1.15 - 7.71), HIV (OR: 67.08, 95% CI: 7.54 – 596.64) and working without knowledge of TB risk profile of the workplace (OR: 8.66, 95% CI: 1.10 – 67.96) were associated with TB disease occurrence. Conclusion: Occurrence of TB disease among Tygerberg hospital employees was low compared to that of the general population of its drainage areas. Disease occurrence in the facility was wide and varied with respect to occupational groups, workplaces and time. Well-established risk factors for TB infection (and disease) were found to be determinants of disease occurrence in the facility.
