GLI EFFETTI DELL'EXPRESSIVE WRITING SUI SINTOMI DEPRESSIVI ETRAUMATICI DA STRESS POST-PARTUM / The effects of Expressive Writing on depression and post-partum traumatic stress symptoms

Il presente studio intervento randomizzato è stato condotto su 140 donne assegnate - a seguito del parto - alla condizione di scrittura espressiva (EW) o alla condizione di scrittura neutra non-emotiva (NW). L’obiettivo era indagare gli effetti della scrittura espressiva sull’esperienza del parto sia sul funzionamento psicologico – sintomi depressivi e postraumatici da stress- (Studio 1a), sia sul funzionamento genitoriale - stress che la madre incontra nell’esercizio del suo ruolo e nell’interazione con il bambino – (Studio 2) al follow-up a tre mesi dal parto. Inoltre, sono state indagate le differenze individuali (moderatori) e i meccanismi psicologici- linguistici (mediatori) associati al cambiamento positivo a seguito della scrittura espressiva (Studio 1b). Gli studi hanno evidenziato che i sintomi depressivi e di intrusione connessi al trauma si riducevano nelle donne assegnate alla condizione EW nel confronto con la condizione NW e che a beneficiare dell’intervento erano le donne con sintomi acuti post-partum (Studio 1a). L’elaborazione cognitiva dell’esperienza stressante del parto attraverso le sessioni di scrittura è stata identificata come il meccanismo chiave, responsabile degli effetti benefici sulla sintomatologia depressiva postnatale (Studio 1b). Infine, il terzo studio ha identificato un effetto positivo della scrittura espressiva sulla qualità dell’interazione madre-bambino moderato da alcune caratteristiche individuali. / One hundred forty women participated in the present randomized intervention study. Right after giving birth women were assigned to the Expressive Writing (EW) intervention group or to the non-emotional neutral writing (NW) condition. The aim was to investigate the effects in the childbirth experience of expressive writing both on psychological functioning - depression and post-partum traumatic stress symptoms - (Study 1a), and on parental functioning - the stress experienced by mothers in their parental role and in the interaction with the newborn - (Study 2) at a three-month follow-up after delivery. Individual differences were investigated as moderators and psycholinguistic mechanisms as mediators associated with positive changes following expressive writing (Study 1b). Results indicate that depression and intrusive symptoms related to the trauma decreased in EW women compared to NW women, and that acute post-partum symptom women benefited the most (Study 1a). Cognitive elaboration of the childbirth traumatic experience taking place during writing sessions is the key mechanism responsible for the beneficial effects on postnatal depression symptoms (Study 1b). Finally, the third study indicated a positive effect of expressive writing on the quality of mother-child interaction. The effect resulted moderated by some individual characteristics.

Développement et évaluation d'un programme psychoéducatif de formation aux habiletés parentales pour les parents de jeunes enfants ayant un trouble du spectre de l'autisme / Development and evaluation of a French parent training program in young children with autism spectrum disorder

Les troubles du spectre de l’autisme (TSA) affectent la qualité de vie familiale. Des programmes de formation aux habiletés parentales (PFHP) sont susceptibles d’entraîner une amélioration du stress parental et du comportement de l’enfant. Cette étude vise à développer et évaluer un PFHP francophone basé sur l’analyse appliquée du comportement. Ce PFHP a été évalué auprès d’un groupe pilote de 6 parents. Une augmentation des connaissances sur les TSA et les pratiques éducatives est observée ainsi qu’une amélioration du comportement de l’enfant. Les parents considèrent les objectifs ciblés comme importants, les procédures transmises comme acceptables et les effets observés comme satisfaisants. Le PFHP a été amélioré et évalué auprès de 18 parents. Une augmentation des connaissances et des pratiques éducatives est observée avec une amélioration des compétences sociales des enfants. Pour 16 parents, une amélioration au niveau du stress est remarquée. Un manuel de ce PFHP a été élaboré. / Parents of children with ASD face many challenges that affect their well-being. Parent trainings (PT) in young children with ASD are known for improving the child’s behaviors and reducing parenting stress. The purpose of this study is to develop and evaluate a French parent training that is based on applied behavior analysis. An initial efficacy study was conducted with 6 parents. Parents have significantly improved their knowledge in ASD and behavioral intervention strategies. They reported an improvement in their child’s behavior. The PT was considered by the parents as effective, the methods used as acceptable and the objectives targeted as important. This promising PT was then improved and evaluated on 18 more parents. Parents have again improved their knowledge in ASD and behavioral strategies whereas children’s socialization skills have improved as well. For 16 parents, the PT has an additional decreasing effect on stress. A manual has been developed with this PT.

Interaction entre le stress parental maternel et paternel en lien avec les problèmes de comportement intériorisés et extériorisés de l’enfant

De nombreuses études montrent une association robuste entre le stress parental et les difficultés socioaffectives de l’enfant. Toutefois, la majorité de ces études ont seulement pris en considération le stress parental de la mère en omettant complètement de considérer celui du père. Bien que les pères soient de plus en plus inclus dans la littérature, plusieurs études récentes sur le stress parental déplorent l’absence de données paternelles. Pourtant, il est bien établi que les pères s’investissent de façon importante dans le développement de leurs enfants et qu’ils peuvent ressentir des niveaux de stress parental comparables à ceux des mères. Ainsi, l’objectif de la présente étude était d’examiner l’association entre le stress parental des deux parents et le développement de problèmes de comportement extériorisés et intériorisés chez l’enfant. L’interaction entre le stress parental paternel et maternel a également été vérifiée dans l’association avec les problèmes de comportement de l’enfant. L’échantillon de l’étude était composé de 157 enfants et leurs parents. Afin d’évaluer leurs niveaux de stress parental, les deux parents ont complété le Parental Stress Index- Short Form alors que leur enfant était âgé de 18 mois. Ils ont ensuite évalué la sévérité des problèmes extériorisés et intériorisés chez leur enfant alors qu’il était âgé de 4 ans, à l’aide du Child Behavior Checklist. Les résultats suggèrent que seul le stress parental paternel prédit la sévérité des problèmes extériorisés (mais non intériorisés) chez l’enfant. Les résultats suggèrent également une interaction entre le stress parental du père et de la mère, de sorte que l’association entre le stress paternel et les comportements extériorisés diminue à mesure que le stress maternel augmente. En somme, la présente étude souligne le rôle du stress paternel quant au développement socioaffectif de l’enfant, particulièrement pour les problèmes extériorisés. / Numerous studies highlight a robust association between parenting stress and child socioemotional difficulties. However, the majority of these studies have only considered maternal parenting stress, failing to consider paternal stress. Although fathers are increasingly considered in recent literature, several recent studies on parenting stress note the absence of paternal data. Yet, it is well established that fathers are involved in their children's development and experience levels of parenting stress comparable to those of mothers. Thus, the purpose of the present study was to examine the association between both parents’ parenting stress and child externalizing and internalizing behavior problems. The interaction between paternal and maternal parenting stress was also examined in association with child behavior problems. The study sample consisted of 157 children and their parents. To assess their levels of parenting stress, both parents completed the Parental Stress Index-Short Form when their child was 18 months old. They then assessed their child’s externalizing and internalizing behaviors when children were 4 years old, using the Child Behavior Checklist. The results suggest that only paternal parenting stress predicts subsequent child externalizing (though not internalizing) behaviors. The results also suggest an interaction between paternal and maternal parenting stress, such that the association between paternal stress and externalizing behaviors decreases as maternal stress increases. In sum, this study underscores the role that paternal stress may play in child socio-emotional development, particularly in the development of externalizing behaviors.

本研究以美國心理學會(APA)暴力預防處與美國幼兒教育協會所共同發展的ACT親職教育方案(Adults and Children Together- Raising Safe Kids Program)為範本,將一個在美國與世界各地發展將近15年的方案首度引進臺灣並應用於臺灣的家庭中,期待能夠藉由這一套社會認知取向的預防性介入「ACT親職教育方案」來增進父母的親職教養態度與知識,降低父母親的親職壓力,並進一步了解親職壓力與兒童行為問題的關聯性。 研究參與者為30位育有0-12歲孩子的父母,其中15位家長接受為期8週的ACT親職教育團體作為實驗處理,另外15位家長則未接受任何處理,有效樣本共23位,實驗組家長12位,對照組家長11位。本研究採用親職壓力量表簡式版與兒童行為檢核表實施前、後測,並針對實驗組實施三個月後的追蹤測量並輔以質性問卷蒐集更完整的資料。 本研究採用無母數統計法Wilcoxon等級檢定與Mann-Whitney U檢定,檢驗親職壓力感受在前測、後測及追蹤測量之間的差異,另以Spearman相關分析檢視親職壓力與兒童行為問題之間的關聯性。研究結果顯示,ACT親職教育為實驗組家長帶來正向的影響發生在三個月後的追蹤測量,特別是在「困難兒童分量表」的親職壓力上有顯著的改善,且親職壓力總分與兒童行為整體問題呈現高度正相關。 / This study investigated whether the ACTdults and Children Together: Raising Safe Kids (ACT-Rprogram causes effects on reducing parenting stress and examined the correlation between parenting stress and child behavioral problems. The ACT program, developed by American Psychological Association in collaboration with the National Association for the Education of Young Children, is a child maltreatment prevention program for parents of young children. Twenty-three parents having children aged 0-12 completed the Parenting Stress Index-Short Form (PSI-SF) and Child Behavior Checklist (CBCL). Twelve of these parents took part in eight-week ACT-RSK program and learned effective parenting including child development, anger management, social problem-solving skills, non-violent discipline and effects of violent media on children. The remaining eleven parents were placed on the wait listreceived treatment as usual. Wilcoxon signed rank test and Mann–Whitney U test were used to respectively compare the intra-group data and inter-group data. Results indicated a positive impact especially on the difficult child subscale of parenting stress during three-month follow-up. Qualitative data collected through open questions in three-month follow-up questionnaire also demonstrated that parents perceived numerous benefits to the ACT-RSK program and felt less stress in the area of parent-child interaction.

Cependant, peu d’études ont investigué le degré de concordance des comportements de la mère et du père dans des couples intacts ainsi que les antécédents ou déterminants de cette concordance. Le présent mémoire contient un article empirique qui poursuit deux buts. Premièrement, l’article a examiné le niveau de concordance (similarité) entre la qualité des comportements des mères et des pères dans des couples intacts. Deuxièmement, l’article a investigué les antécédents familiaux de la concordance des comportements maternels et paternels: la satisfaction conjugale et le stress parental des mères et des pères. L’étude a été menée auprès de 44 familles intactes (mère-père-enfant). Les résultats indiquent que la concordance mère-père est modérée (r = .31). De plus, un degré significatif de concordance a été trouvé chez les couples dont les partenaires rapportent une satisfaction conjugale élevée ainsi qu’un faible niveau de stress parental, mais pas chez les couples moins bien ajustés. Les implications théoriques et empiriques sont discutées. / There is compelling evidence that the quality of both maternal and paternal parenting behaviour is important for child development. Yet, little is known of the degree of similarity between maternal and paternal parenting behaviour in intact couples, and about the antecedents or determinants of this parental concordance. The present master’s thesis includes an empirical article pursuing two aims. First, the article examined the concordance (similarity) between the quality of maternal and paternal behaviours with their toddler. Second, the article investigated the parental determinants of this concordance: marital satisfaction and parenting stress. The sample included 44 families (mother-father-toddler). Results indicated that the concordance between the quality of maternal and paternal behaviour was moderate (r = .31). Furthermore, significant mother-father concordance was found among more satisfied and less stressed parents, but not among less adjusted parents. The theoretical and empirical implications are discussed.

過去研究指出,親職壓力是造成兒童虐待的危險因子之一。本研究探討:親職壓力因應策略與親職同理心對管教行為的影響,或此兩變項是否能調節親職壓力對管教行為的影響,欲透過研究結果來加強兒童虐待的預防。本研究請每位受試者填寫五種量表,分別為:基本資料、親職同理心、親職壓力、親職壓力因應與親職衝突策略量表,利用零相關和階層迴歸,分析142位育有國小子女之單親母親所填寫的量表。研究結果部分符合預期:親職壓力因應策略中的「主動面對問題」、「逃避隱藏情緒」可以調節親職壓力對非暴力管教行為的影響,「情緒認知調整」亦在親職壓力和嚴重攻擊管教行為間扮演調節變項。除了調節作用外,親職壓力因應策略對管教行為有獨特的預測力:「主動面對問題」使用經常性越高,輕度攻擊的使用次數越多,「負面情緒反應」使用經常性越高,心理攻擊和輕度攻擊的使用次數越多。「親職同理心」則對管教行為沒有獨特的預測力,也不具有調節作用。另外亦發現「親職同理心」和「逃避隱藏情緒」可以預測親職壓力。本研究針對研究結果進行討論,包括管教行為與兒童虐待的關係、親職壓力因應策略與管教行為間的關係、親職同理心扮演的角色,以及單親母親兒童虐待行為的預防。 / According to previous studies, parenting stress is one of the risk factors of child abuse. This study explored the influence of parenting stress- coping styles and parental empathy on parenting practices, and examined the moderating effect of these variables on the relationship between parenting stress and parenting practices. The subjects were asked to complete the questionnaires including: basic information, parental empathy scale, parenting stress scale, parenting stress-coping scale, and Parent-Child Conflict Tactics Scales. Zero-order correlation and hierarchical regression analysis were used to analyze the scales completed by 142 single mothers. The results indicated that “Facing the problems actively” and “Evading hidden feelings” could moderate the relationship between parenting stress and non-violent discipline. “Adjustment in sentimental cognition” could moderate the relationship between parenting stress and severe assault. Furthermore, some of the parenting stress- coping styles had main effect on parenting practices: “Facing the problems actively” had a positive effect on minor assault. “Negative sentimental reaction” had a positive effect on psychological aggression and minor assault. There were no main effects nor moderator effect of parental empathy. The relationship between parenting practices and child abuse, the influence of parenting stress- coping styles on parenting practices, the role of parental empathy, and the prevention of child abuse in single-mother households were discussed in this study.

Social-emotional and behavioral development problems in 1 to 2-year-old children in Northern Finland:reports of mothers, fathers and healthcare professionals

Abstract Background and aims: Growing evidence supports the existence of clinically significant problems in social-emotional/behavioral (SEB) development among infants and toddlers and the importance of early identification of these problems. There is a lack of research on the occurrence and identification of problems in the SEB domain among the Finnish general population of 1 to 2-year-old children. The present study examined these important issues. Given the dearth of earlier research knowledge, particular emphasis was focused on analyzing possible moderating effects of the assessed child’s and informant’s gender on the results. Subjects and methods: Oulu toddler (N = 208, age 18 months), Oulu infant (N = 227, age 12 months), and Oulu Province (N = 1008, age 12 months) samples were collected during 2008–2013 in collaboration with child health centers. Both mothers and fathers completed questionnaires assessing their child´s SEB development (e.g. the Brief Infant-Toddler Social and Emotional Assessment; Briggs-Gowan & Carter, 2006) and the family’s sociodemographic characteristics. Child healthcare nurse (CHCN) worry reports concerning the children’s development and family well-being were also gathered. Main results and conclusions: In parental ratings, girls obtained higher SEB competence scores than boys, whereas boys got higher SEB problem scores than girls, particularly among the toddler sample. Thus, boys may have an elevated risk of parent-reported problem behaviors and delays in SEB competences even before the age of 2 years. Compared to fathers, mothers were prone to rate the toddlers higher in both SEB competences and problems, especially with regard to externalizing behavior problems and problems in boys. Elevated interparental rating differences were associated with relatively elevated maternal parenting stress. Therefore, it is recommended to gather data on young children’s SEB development from both parents when possible and to focus clinical attention on the issue if the parental reports differ significantly. Parents and CHCNs seldom recognized SEB problems and delays in competency as problematical in 1-year-old infants, even in the case of infants who were screened to be in the of-concern range on the age-appropriate measure of SEB development. Thus, further efforts are needed for developing the identification of early problems in SEB development by both parents and preventive child healthcare professionals in Finland. / Tiivistelmä Tausta ja tavoitteet: Kasvava näyttö tukee käsitystä, että kliinisesti merkittäviä sosioemotionaalisen ja käyttäytymisen (SEK) kehityksen ongelmia esiintyy jo vauva- ja taaperoikäisillä ja että näiden ongelmien varhainen tunnistaminen on tärkeää. SEK-kehityksen ongelmien esiintymisestä ja tunnistamisesta 1–2-vuotaiden väestössä Suomessa puuttuu tutkimustietoa. Tämä tutkimus arvioi näitä tärkeitä aiheita. Tutkimuksessa huomioitiin erityisesti sekä tutkittavan lapsen että arvioitsijan sukupuolen mahdollinen moderoiva vaikutus tuloksiin, koska aiempaa tutkimustietoa tästä näkökulmasta on vähän. Tutkittavat ja menetelmät: Oulu taapero (N = 208, ikä 18 kk), Oulu vauva (N = 227, ikä 12 kk) ja Oulun lääni (N = 1008, ikä 12 kk) -aineistot kerättiin vuosina 2008–2013 yhteistyössä lastenneuvoloiden kanssa. Sekä äidit että isät täyttivät lomakkeet, jotka arvioivat lapsen SEK-kehitystä (mm. the Brief Infant-Toddler Social and Emotional Assessment; Briggs-Gowan & Carter, 2006) ja perheen sosiodemografisia taustatekijöitä. Myös terveydenhoitajan raportit lapsen kehitykseen ja perheen hyvinvointiin liittyvistä huolista kerättiin. Päätulokset ja johtopäätökset: Vanhempien arvioimina tytöt saivat korkeampia pistemääriä SEK-taidoissa kuin pojat, kun taas pojat saivat korkeampia pistemääriä SEK-ongelmissa kuin tytöt, etenkin taaperoaineistossa. Pojilla saattaakin olla kohonnut riski vanhempien raportoimille käytösongelmille ja SEK-kehityksen viiveille jopa ennen 2 vuoden ikää. Isiin verrattuina äidit olivat taipuvaisia antamaan korkeampia pisteitä sekä SEK-taito- että -ongelmaosioissa taaperoikäisten aineistossa, etenkin koskien ulospäin suuntautuvia käytösongelmia ja poikien ongelmia. Suurentuneet eroavaisuudet vanhempien antamissa arvioissa olivat yhteydessä suhteellisesti kohonneeseen vanhemmuuden stressiin äideillä. Onkin suositeltavaa kerätä tietoa pienen lapsen SEK-kehityksestä kummaltakin vanhemmalta, jos mahdollista, ja kiinnittää kliinistä huomiota asiaan, jos vanhempien antamat tiedot eriävät merkittävästi. Vanhemmat ja terveydenhoitajat tunnistivat SEK-ongelmia ja taitojen puutteita harvoin huolenaiheiksi 1-vuotiailla, vaikka nämä vauvat olisivat seuloutuneet huolialueelle ikätasoon soveltuvalla SEK-kehityksen mittarilla. Tarvitaankin lisätoimia, jotta vanhemmat ja terveydenhuollon ammattilaiset oppivat paremmin tunnistamaan varhaisia SEK-kehityksen ongelmia Suomessa.

Lien bidirectionnel entre des caractéristiques personnelles des parents et leurs pratiques parentales dans un contexte d’intervention

Les parents d’enfants aux prises avec un trouble déficitaire de l’attention avec ou sans hyperactivité (TDA/H) sont à risque de dépression. Ces parents rapportent aussi des relations familiales plus problématiques, des expériences plus stressantes, un sentiment d’auto-efficacité plus faible à l’égard de leur rôle parental et des pratiques parentales plus coercitives ou inadéquates en comparaison aux parents d’enfants sans ce diagnostic. Plusieurs recherches ont relevé que les parents d’enfants ayant un TDA/H qui ont participé à un programme d’entraînement aux habiletés parentales (PEHP) rapportent une amélioration générale des difficultés énumérées précédemment. Le changement d’attitude et de pratiques parentales est souvent relié à une diminution des symptômes du TDA/H chez les enfants. L’intervention peut donc contribuer à améliorer la condition du parent et par le fait même celle de son enfant. Toutefois, le TDA/H est un trouble chronique qui peut affecter certaines caractéristiques personnelles du parent pouvant interférer avec la capacité de ce dernier à bénéficier d’une intervention. Peu d’études s’attardent aux caractéristiques du parent pouvant affecter l’efficacité de l’intervention. Le but de la présente étude est d’étudier le lien bidirectionnel entre certaines caractéristiques personnelles (dépression, stress, sentiment d’auto-efficacité) et les pratiques parentales dans un contexte d’intervention qui s’adresse aux parents d’enfants ayant un TDA/H. Les résultats démontrent que le sentiment d’auto-efficacité initial du parent est prédicteur d’une discipline inconstante, et ce, indépendamment de la condition expérimentale. De plus, les pratiques positives initiales sont prédicteurs d’un sentiment d’auto-efficacité élevé au post-test et ce, indépendamment de la condition expérimentale. / When compared to parents of non-diagnosed children, parents of children with ADHD face a higher risk of experiencing parenting stress, depression, and family functioning problems. The objective of the present study is to examine the bidirectional link between parental characteristics (depression, stress, self-efficacy) and parental practices within the context of a parent training program. Baseline parent self-efficacy predicted use of inconsistent discipline regardless of whether parents had followed the training program. Furthermore, baseline positive parental practices predicted parent self-efficacy, regardless of experimental condition.
