Relação entre periodontite e síndrome metabólica / Relationship between periodontitis and metabolic syndrome

Os objetivos desse estudo foram: (1) avaliar se o diagnóstico da periodontite crônica pode auxiliar na identificação de pacientes com síndrome metabólica, e (2) verificar o efeito da terapia periodontal não-cirúrgica sobre os componentes utilizados para o diagnóstico da síndrome metabólica nos pacientes com periodontite crônica. No estudo 1 foram avaliados 33 pacientes com periodontitecrônica (idade média 50,3, DP 7,9 anos) e 36 pacientes controles (gengivite/saudável) (idade média 39,7, DP 10,3 anos), sem diagnóstico de síndrome metabólica. Os pacientes foram avaliados clinica e laboratorialmente para verificar possível associação entre a presença de periodontite e diagnóstico precoce de síndrome metabólica. Os parâmetros clínicos usados foram: Índice de placa visível (IPV), índice de sangramento gengival (ISG), profundidade de bolsa à sondagem (PBS) e nível de inserção clínica (NIC). Os níveis séricos de proteína C Reativa (PCR), glicemia em jejum, colesterol e triglicerídeos foram analisados. Também foram verificados peso, altura, circunferência da cintura, Índice de Massa Corporal (IMC) e pressão arterial. No estudo 2, os pacientes com periodontite crônica foram tratados através da terapia periodontal não-cirúrgica e reavaliados 90 dias após tratamentopara nova avaliação de exames clínicos (PBS, NI, IPV, ISG). Os dados depeso, altura, circunferência da cintura, IMC e pressão arterial e as avaliações séricas foram repetidas e comparadas aos do dia 0. No estudo 1 foi constatado que o nível sérico de glicose e o número de itens da síndrome metabólica presentes foram estatisticamente maiores no grupo teste do que no grupo controle. No estudo 2, os níveis de glicose, colesterol, LDL, PCR e número de itens da síndrome metabólica presentes reduziram significantemente e o HDL aumentou significantemente após a terapia periodontal não-cirúrgica. Assim, podemos concluir que o diagnóstico de periodontite crônica aumenta a chance de diagnóstico de síndrome metabólica e que o tratamento periodontal foi eficaz em melhorar alguns componentes da síndrome metabólica. / The aims of this study were: (1) to assess whether the diagnostic of chronic periodontitis mayidentify patients with metabolic syndrome, and (2) to verify the effect of non-surgical periodontal therapy on the components used for the diagnostic of the metabolic syndrome in patients with chronic periodontitis. In study 1, 33 patients with chronic periodontitis (age 50.3, DP 7.9 years) and 36 patients and controls (gingivitis/healthy) (age 39.7, DP 10.3 years), without diagnostic of metabolic syndrome were examined.The patients were evaluated clinically and laboratory to verify possible association between the presence of periodontitis and early diagnose of metabolic syndrome. The clinical parameters used were: visible plaque index (VPI), gingival index (GI), bleeding on probing (BOP), probing pocket depth (PPD) and clinical attachment loss (CAL). Serum levels of C-reactive protein (CRP), fasting glucose, cholesterol and triglycerides were analyzed. Weight, height, waist circumference, body mass Index (BMI) and blood pressure were also assessed.In study 2, patients with chronic periodontitis received non-surgical periodontal therapy and 90 days after treatment were reevaluated to new clinical assessment (VPI, GI, BOP, PPD and CAL). The data of weight, height, waist circumference, BMI and blood pressure and serum assessments were repeated and compared to day 0.In study 1,the serum glucose level and the number of items of the metabolic syndrome present were statistically higher in the test group as compared to values observed in the control group. In study 2, the levels of glucose, cholesterol, LDL, and number of items in the metabolic syndrome present significantly reduced and HDL increased significantly after non-surgical periodontal therapy. In conclusion, chronic periodontitis diagnostic increases the chance of diagnostic of metabolic syndrome and periodontal treatment was effective in improving some components of metabolic syndrome.

Assim, foi objetivo do presente estudo avaliar, através do MEF bidimensional, a distribuição interna das tensões nas estruturas de suporte da PPREL associada a um implante osseointegrado, em função da reprodução homogênea e não-homogênea do ligamento periodontal (LP). Para isso, 6 modelos (M) foram criados no programa AutoCAD 2005 (Autodesk Inc.; USA), representando: MA - hemiarco contendo somente o dente 33 e o rebordo desdentado para distal; MB - semelhante ao A, com uma PPREL substituindo os dentes ausentes; MC - semelhante ao B, com um implante (Sistema Bränemark - 3,75 mm x 10,0 mm) na região retromolar suportando a base da prótese; MD, ME e MF semelhantes aos modelos A, B e C, respectivamente, com exceção do LP, que foi modelado de forma não-homogênea, com a representação de 4 conjuntos de fibras: fibras da crista, horizontais, oblíquas e apicais. Após isso, os modelos foram exportados para o programa de elementos finitos ANSYS 8.0, e carregados com forças estritamente verticais de 50N em cada ponta de cúspide, fracionadas em 5 pontos de 10N. Os mapas gerais de tensões evidenciaram máxima e mínima concentração de tensões de (em MPa): MA (58,976 e 0,0045); MB (77,863 e 0,0551); MC (207,195 e 0,0057); MD (110,33 e 0,0137); ME (191,36 e 0,0044) e MF (326,60 e 0,0076), com deslocamento máximo de (mm): MA (0,0631); MB (0,1263); MC (0,1142); MD (0,2709); ME (0,1960) e MF (0,2019). / The application of many hypotheses in a finite element analysis (FEA) has been critical to obtain precise results in the removable partial denture treatment associated with an osseointegrated implant. In view of that, the purpose of the present study was to evaluated, through 2-D finite element analysis, the stress distribution in the support structures of a free-end saddle removable partial denture (RPD) associated with an osseointegrated implant, in function of the homogenous and non-homogenous condition for the periodontal ligament. For this, 6 models (M) were created in the AutoCAD software (Autodesk Inc.; USA), representing: MA - hemiarch contain only the natural tooth (33) and edentulous space in the posterior region; MB - similar to MA, with an RPD replacing the absent teeth; MC - similar to MB, with an osseointegrated implant (Branemark System - 3.75 x 10.00 mm) positioned in a posterior region in order to support the RPD base; MD, ME and MF - identical to MA, MB and MC, respectively; however, in these models, the periodontal ligament was considered an non-homogenous structure by the incorporation of four fibers groups in a sagital plane: alveolar crest fibers, horizontals, oblique, and apical fibers. After this, the models were exported to the finite element program (ANSYS 8.0) and loaded with verticals forces (50N) in each cusp. The results showed maximum and minimum stress (MPa): MA (58.976 and 0.0045); MB (77.863 and 0.0551); MC (207.195 and 0.0057); MD (110.33 and 0.0137); ME (191.36 and 0.0044) and MF (326.60 and 0.0076). Logo, a DP deve ser tratada para restabelecer a sa?de dos tecidos periodontais e diminuir as complica??es do diabetes. Portanto, objetiva-se avaliar clinicamente o efeito da terapia periodontal de descontamina??o da boca completa (Full Mouth Scaling and Root Planning - FMSRP) em pacientes diab?ticos tipo II portadores de periodontite cr?nica durante 12 meses. Trinta e um participantes no grupo controle (GC), sem diabetes e com periodontite cr?nica, e 12 no grupo teste (GT) foram avaliados no baseline, aos 03, 06, 09 e 12 meses. Foram analisados os seguintes par?metros cl?nicos periodontais: ?ndice de sangramento a sondagem (ISS), ?ndice de placa vis?vel (IPV), profundidade de sondagem (PS), n?vel cl?nico de inser??o (NCI) e recess?o gengival (RG). Para o GT, tamb?m foram realizados exames laboratoriais para avaliar os par?metros sangu?neos de glicose em jejum e hemoglobina glicada (HbA1c). Os resultados foram analisados de duas formas distintas: uma utilizando todos os s?tios presentes na boca e outra apenas com os s?tios doentes. Foram utilizados o teste de Mann-Whitney, Friedman e Wilcoxon com um n?vel de signific?ncia de 5%. Na an?lise intergrupo de todos os s?tios, percebe-se que n?o houve diferen?a significativa ao longo do tempo em rela??o ? PS, ISS, IPV, NCI e RG (p>0,05). Entretanto, ao avaliar os s?tios doentes, observamos diferen?a significativa somente para o NCI, sendo maior para o GT. A an?lise intragrupo de todos os s?tios mostrou uma redu??o estatisticamente significativa para a PS, IPV e ISS em ambos os grupos. Para os s?tios doentes, a an?lise intragrupo mostrou uma redu??o estatisticamente significativa na PS, ISS e NCI em ambos os grupos. Apesar de haver diminui??o nos valores no ISS nos dois grupos, apenas no GC pode-se perceber uma redu??o estatisticamente significativa. N?o houve altera??o no grupo teste em rela??o ? HbA1c e a glicemia em jejum nos per?odos avaliados. Portanto, a terapia periodontal mostrou-se eficiente para os cuidados da sa?de bucal. / There is a bidirectional association between periodontal disease (PD) and diabetes mellitus, in which diabetes favors the development of PD and PD, if left untreated, can worsen the metabolic control of diabetes. Thus, periodontal disease should be treated to restore periodontal health and reduce the complications of diabetes. Therefore, the objective is assess the effect of full mouth periodontal therapy decontamination (Full Mouth Desinfection - FMD) in diabetic type II patients with chronic periodontitis during 12 months. Thirty-one patients in group one (G1) and 12 in group two (G2) were followed at baseline, 03, 06 09 and 12 months. There following clinical parameters were accessed: probing on bleeding (BOP), visible plaque index (PI), probing depth (PD), clinical attachment level (CAL) and gingival recession (GR). For diabetic patients, there were also made laboratory tests to evaluate blood parameters: fasting glucose and glycated hemoglobin. The results had been analyzed in two ways: all sites in the mouth and another with diseased sites. The Mann-Whitney, Friedman and Wilcoxon tests were used with 5% significance. Intergroup analysis of all sites it is clear that there was no significant difference over time concerning PD, BOP, PI, CAL and RG. However, when evaluating the diseased sites, we observed significant difference for CAL and PD, with higher values in G1. The intragroup analysis for all sites showed a statistically significant reduction at PD, PI and BOP in both groups. Intragroup analysis of periodontal affected sites showed a statistically significant reduction in PD, BOP and CAL in both groups. There was also a statistically significant increase in RG values. There was no significant change concerning glycated hemoglobin and fasting glucose in the G1. Therefore, it can be concluded that there were improvements in periodontal parameters over the 12 months of research, but without changes in glycemic levels of diabetic patients. Thus, periodontal therapy proved effective in maintaining oral health.

Avaliação dos efeitos do probiótico Bifidobacterium animalis subsp. lactis HN019 como terapia adjuvante no tratamento da periodontite experimental em ratos / Effects of the probiotic Bifidobacterium animalis subsp. lactis HN019 as an adjunct to treatment of experimental periodontitis in rats

Probióticos do gênero Lactobacillus estão sendo amplamente investigados no tratamento da periodontite. Contudo, os efeitos de microrganismos do gênero Bifidobacterium ainda são pouco conhecidos. Este estudo avaliou os efeitos do probiótico Bifidobacterium animalis subsp. lactis (B. lactis) HN019 como adjuvante à raspagem e alisamento radicular (RAR) no tratamento da periodontite experimental (PE) em ratos. No dia 0 do experimento, 32 ratos foram alocados em 4 grupos: C (controle), PROB (probiótico), PE-RAR e PE-RARPROB. Nos grupos PE-RAR e PE-RAR-PROB, a PE foi induzida pela colocação de ligaduras de seda ao redor dos primeiros molares inferiores dos animais. No 14&deg; dia, as ligaduras foram removidas e realizou-se a RAR. Nos animais dos grupos PROB e PE-RARPROB, o probiótico B. lactis HN019 foi administrado diariamente (10 mL/dia de 109 unidades formadoras de colônia) por 15 dias tendo seu início no 14&deg; dia do experimento. Os animais de todos os grupos foram submetidos à eutanásia 29 dias após o início do experimento. As hemimandíbulas e amostras de intestino delgado foram coletadas. Foram realizadas análises histomorfométricas, microtomográficas e imunohistoquímicas. Foram investigados, também, os efeitos microbiológicos de B. lactis HN019 no biofilme associado às ligaduras durante o desenvolvimento da PE. Todos os dados foram analisados estatisticamente. O Grupo PE-RAR-PROB apresentou menores reabsorção óssea alveolar e perda de inserção conjuntiva quando comparado ao Grupo PE-RAR, bem como menor número de osteoclastos, maior expressão de citocinas anti-inflamatórias e menor expressão de citocinas pró-inflamatórias (p <0,05). No grupo PE-RAR-PROB, os valores médios da profundidade da cripta do jejuno e duodeno foram significativamente maiores que aqueles do grupo PE-RAR. A proporção de bactérias aeróbias/anaeróbias foi maior nas amostras de biofilme de animais tratados com B. lactis HN019 em relação àquelas de animais não tratados (p <0,05). Dentro dos limites deste estudo, pode-se concluir que a utilização de B. lactis HN019 como adjuvante à RAR promove benefícios histológicos, microtomográficos e imunológicos adicionais no tratamento da PE em ratos, bem como melhora a morfologia intestinal. / Lactobacillus probiotics have been investigated in periodontitis. However, the effects of the genus Bifidobacterium on periodontitis are hardly known. This study evaluated the effects of the probiotic Bifidobacterium animalis subsp. lactis (B. lactis) HN019 as an adjunct to scaling and root planing (SRP) in rats with experimental periodontitis (EP). At baseline, 32 rats were assigned to 4 groups: C (control), PROB, EP-SRP and EPSRP- PROB. In groups EP-SRP and EP-SRP-PROB, the mandibular first molars of the animals received a ligature. At day 14, the ligatures were removed and SRP was performed. Animals of groups PROB and EP-SRP-PROB were orally administered with 10 mL/day of 109 colony forming units of B. lactis HN019 for 15 days, starting at day 14. Animals were euthanized at day 29. The jaws and samples of the duodenum, jejunum, and ileum were resected. Histomorphometric, microtomographic and immunohistochemical analyses were performed. Microbiological effects of B. lactis on biofilm were also evaluated. Data were statistically analyzed. Group EP-SRP-PROB presented reduced alveolar bone resorption and attachment loss when compared with Group EP-SRP (p<0.05). Group EP-SRP-PROB showed significantly fewer osteoclasts, increased expression of anti-inflammatory cytokines and reduced expression of proinflammatory cytokines compared with Group EP-SRP (p<0.05). In group EP-SRPPROB, the mean values of crypt depth of the jejunum and dudoenum were significantly higher than the ones from group EP-SRP. B. lactis promoted a higher ratio between aerobic and anaerobic bacteria in biofilm samples (p<0.05). Within the limits of this study it can be concluded that the use of B. lactis HN019 as an adjunct to SRP promotes additional histologic, microtomographic and immunologic benefits in the treatment of EP in rats and improves the intestinal morphology.

A coexistência de doenças sistêmicas, como a hipertensão, pode levar a uma inflamação exacerbada, maior reabsorção óssea e dano sistêmico pronunciado. Além disso, células imunes residentes têm importante papel na progressão da DP, porém, a participação dos mastócitos (MC) ainda não é bem compreendida. Objetivos: Avaliar o papel dos MC sobre o metabolismo ósseo local (mandíbula) e sistêmico (fêmur), em modelo animal normotenso (ratos Wistar) e hipertenso (ratos SHR) com DP. Métodos: Ratos machos Wistar e SHR (10 semanas) foram utilizados. A depleção de MC foi conduzida pelo pré-tratamento com o composto 48/80 e a DP foi induzida por ligadura bilateral nos primeiros molares inferiores, mantida por 15 dias. Foi realizada a identificação de MC no tecido gengival, por coloração com Azul de Toluidina. A perda óssea alveolar e parâmetros de arquitetura óssea na mandíbula e fêmur foram avaliados por microtomografia computadorizada, a expressão gênica de marcadores de formação, remodelamento e reabsorção na mandíbula e fêmur foram avaliada por RT-PCR em tempo real, e a produção de citocinas nos tecidos de interesse foi avaliada por ELISA. Principais resultados: Observamos significativa perda óssea induzida por DP, principalmente no SHR, em comparação ao Wistar, e a depleção de MC foi capaz de prevenir esta perda. A DP levou a expressão aumentada de Opn, Opg, Rankl e Rank, Trap, Ctsk, Vtn, Itga5 e Itgb5, além de aumento na produção de TNF-α, IL-6, IL-10 e CXCL3 na mandíbula, o que foi reduzido em animais depletados de MC. No fêmur, a DP não levou a alterações da arquitetura óssea, mas foi capaz de aumentar a expressão de Runx2, Opn, Opg, Rankl, Rank, Trap, Mmp2, Mmp9, Vtn, Itga5 e Itgb5, o que também foi significativamente reduzido pela depleção de MC. Conclusões: Nossos dados sugerem um importante papel dos MC nas consequências ósseas locais (mandíbula) da DP, especialmente nos animais SHR, além dos efeitos sistêmicos (fêmur) onde os MC possivelmente têm participação na modulação da expressão de marcadores ósseo, alterados pela DP, o que foi mediado por vias diferentes das observadas na resposta local. / Introduction: Periodontal disease (PD) is an inflammatory disorder of the tissues supporting the teeth, which start from the bacterial biofilm accumulation, and results in local and systemic damage. The coexistence of systemic diseases, such as hypertension, can lead to exacerbated inflammation, increased alveolar bone resorption and increased systemic damage. In addition, resident immune cells have an important role in PD progression, however the mast cells (MC) participation is still not well understood. Aims: To evaluate the role of MC in the local (mandible) and systemic (femur) bone metabolism in a normotensive (Wistar) and hypertensive (SHR) rats with PD. Methods: Males Wistar and SHR rats (10-week old) were used. MC depletion was conducted by compound 48/80 treatment and PD was induced by bilateral ligature placed in the lower first molars, which was maintained for 15 days. The identification of MC in the gingival tissue was done by Toluidine Blue staining, alveolar bone loss and bone architecture parameters of mandible and femurs were evaluated by micro-computed tomography, bone markers gene expression on the mandible and femur were evaluated by real time RT-PCR, and the cytokines production was evaluated by ELISA. Key results: We observed a more significant PD-induced bone loss in SHR, compared to Wistar, and MC depletion was able to prevent this loss. PD increased the expression of Opn, Opg/ Rankl/Rank, Trap, Ctsk, Vtn, Itga5 and Itgb5, as well as increased production of TNF-α, IL-6, IL-10 and CXCL3 in the mandible, what was prevented by MC depleted animals. In the femur, PD did not change bone architecture parameters, but was able to increase expression of Runx2, Opn, Opg/Rankl/Rank, Trap, Mmp2, Mmp9, Vtn, Itga5 and Itgb5, which also was significantly reduced by MC depletion. Conclusions: Our data suggest an important role of MC in the local bone consequences (mandible) of PD, especially in SHR, as well the systemic effects (femur), where MC may have a role in bone dynamics by modulating the expression of bone markers, altered by PD, through different mechanism from those observed in the local response. / 15/03965-2

Periodontal diseases in a representative urban population in south Brazil

Background and Aims: There is little information about the epidemiology and risk factors of periodontal diseases in Latin America in general, and Brazil in particular. The principal aims of this study were to: 1) describe the prevalence and severity of periodontal attachment loss and gingival recession, and to assess the contribution of demographic, behavioral, and environmental exposures to the occurrence of periodontal disease outcomes in a sample representative of the urban population in the state of Rio Grande do Sul in south Brazil; and 2) report the epidemiology and risk indicators of aggressive periodontitis in this population. Methods: A representative sample consisting of 1,586 subjects 14-103 years of age (mean 38 y) and comprising 45.3% males and 54.7% females was selected using a multi-stage, probability, cluster sampling strategy. The subjects were interviewed using a structured questionnaire and underwent a full-mouth, six sites per tooth clinical examination in a mobile examination center. Results: Moderate and severe clinical attachment loss and gingival recession were widespread among adults in this population. The prevalence and extent of attachment loss ³5 and ³7 mm were 79% and 52% subjects, and 36% and 16% teeth; and for gingival recession ³3 mm and ³5 mm were 52% and 22% subjects, and 17% and 6% teeth, respectively. Aggressive periodontitis was diagnosed in 5.5% of subjects, which is significantly higher than the reported prevalence in most other populations. Among the main risk indicators for chronic as well as aggressive destructive periodontal diseases were: older age, low socioeconomic status, dental calculus, and smoking. Cigarette smoking accounted for an important part of periodontal disease burden, particularly in adults, and should be considered an important target in any prevention strategy aimed at reducing the burden of periodontal diseases. Partial recording methods consistently underestimated the prevalence of attachment loss in the population, and the extent of underestimation was dependent on the type of system used and the threshold of attachment loss. Conclusions: Destructive periodontal diseases are prevalent in this Brazilian population. Suitable disease prevention and health promotion programs should be established to improve the periodontal health in this population.
