Professional Quality of Life, Moral Distress, and Turnover Intent of Healthcare Providers Working within the Context of Medical Assistance in Dying

Background: Despite a longstanding debate regarding personal choice towards death and dying, MAiD was legalized in Canada on June 17th, 2016. Since its integration into the healthcare system, there has been a dearth of literature surrounding providers’ experience with the procedure. Healthcare providers are uniquely positioned within the context of the MAiD experience. Aim: To explore the experiences of health care providers (nurses and physicians) caring for patients undergoing MAiD as well as to explore the relationships between the concepts, Moral Distress (MD), Professional Quality of Life (Burnout (BU), Secondary Traumatic Stress (STS) and Compassion Satisfaction (CS)), and Intent to Turnover (TO). Design: This explorative and descriptive cross-sectional study consisted of a self-reported survey that uses both scale items and open-ended questions. Setting/ Participants: The target population included all nurses and physicians who were active members of a targeted Regional MAiD Network practicing within a designated geographical location within Canada. Results: N=38 Questionnaires completed. The averages of our measured constructs include Moral Distress (Composite) x= 2.9 (SD 2.03), Moral Distress (Frequency): x= 1,18 (SD .102), Moral Distress (Intensity): x= 1.61 (SD .28) Burnout: x= 2.08 (SD 0.5), Secondary Traumatic Stress x= 2.22 (SD 0.48), Compassion Satisfaction x= 4.18 (SD 0.43) and Turnover Intent: x= 2.22 (SD 0.77). Significant positive correlations were found between inter-scale constructs of Moral Distress (Composite, Frequency, and Intensity), Moral Distress, Burnout, and Secondary Traumatic Stress, and between Burnout and Turnover Intent. Significant negative correlations were also found between Professional Quality of life Inter-Scale constructs of Compassion Satisfaction and Burnout as well as between Compassion Satisfaction and Turnover Intent. Conclusion: MAiD providers in our study expressed feelings of their work being “rewarding” and “deeply satisfying”, further reflective in their below-average rates of Moral Distress, Burnout, Secondary Traumatic Stress, Turnover Intent, and higher-than-average rates of Compassion Satisfaction. Although the procedure remains controversial, these participants expressed enjoying their work. These reported positive aspects of their roles persisted despite the challenges the COVID-19 pandemic presented.

WHAT KEEPS US WELL? PROFESSIONAL QUALITY OF LIFE AND CAREER SUSTAINING BEHAVIORS OF MUSIC THERAPY PROFESSIONALS

Self-care can be seen as not only critical for individual professionals, but also for the growth of the helping professions and the quality of care which clients receive. The purpose of this study was to investigate use of career-sustaining behaviors and the levels of professional quality of life in music therapy professionals. This study investigated research questions regarding use of career sustaining behaviors and levels of professional quality of life, the relationship between these variables, the differences in the use of career sustaining behavior by demographics, and the use of music as a self-care strategy. An online survey was sent to all professional members of the American Music Therapy Association. A total of 403 participants were included in the study for the purposes of data analysis. Findings from the study indicate that music therapy professionals are in the average to low ranges for burnout and secondary traumatic stress. However, a portion of the sample was identified to be at risk for these factors. Differences existed in the use of career sustaining behaviors between demographic variables, indicating self-care behaviors vary among professionals. The field of music therapy should further investigate these areas to best provide opportunities for professional self-care.

Qualité de vie professionnelle et bien-être au travail en protection de la jeunesse : l’influence des rôles de genre sur la violence au travail et ses conséquences

L'exposition à la violence au travail peut avoir des conséquences professionnelles négatives pour les travailleurs en protection de la jeunesse. Bien que des études antérieures aient identifié des facteurs de protection et de risque en lien avec cet enjeu, les différences individuelles liées aux rôles de genre restent encore à être explorées. Allant au-delà du sexe, ce mémoire vise à examiner comment les rôles de genre influencent l'exposition à la violence au travail, la qualité de vie professionnelle et le bien-être au travail chez les travailleurs en protection de la jeunesse. Un échantillon randomisé stratifié par sexe de 301 travailleurs en protection de la jeunesse canadiens (hommes: 15,6%, femmes: 84,4%) a été recruté afin que les participants remplissent des questionnaires validés portant sur l’exposition à la violence au travail, les rôles de genre, la qualité de vie professionnelle et le bien-être au travail. Les différences de moyennes pour l’exposition à la violence au travail, la qualité de vie professionnelle et le bien-être au travail selon le rôle de genre ont ensuite été évalué à l’aide d’ANCOVA en contrôlant pour l'expérience clinique et le type de travail. L'effet modérateur des rôles de genre sur les autres variables a été évalué par le biais de régressions linéaires multiples hiérarchiques. L'androgynie (masculinité élevée et féminité élevée) était associée à des scores plus élevés sur les indicateurs positifs de la qualité de vie professionnelle et du bien-être au travail. Cependant, les rôles de genre n'ont montré aucun effet modérateur significatif sur la relation entre l'exposition à la violence et les indicateurs de bien-être psychologique au travail. Les résultats montrent comment les analyses comparatives entre les genres peuvent fournir davantage d’information sur les mécanismes entourant la qualité de vie professionnelle et le bien-être au travail. Ils suggèrent également que l'androgynie pourrait être liée à des avantages psychosociaux potentiels pour les travailleurs en protection de la jeunesse. / Exposure to workplace violence puts child protection workers at risk for adverse occupational outcomes. While previous studies have identified protective and risk factors, individual differences in gender roles have yet to be explored. Moving beyond sex, the present thesis aims to examine the ways in which gender roles influence exposure to workplace violence, professional quality of life and wellbeing at work among child protection workers. A randomised sample stratified by sex of 301 Canadian child protection workers (Male: 15.6%, Female: 84.4%) completed validated questionnaires of exposure to workplace violence, gender roles, professional quality of life, and wellbeing at work. Mean differences in workplace violence, professional quality of life, and wellbeing at work according to gender role were then assessed using ANCOVAs controlling for clinical experience and type of work. The moderating effect of gender roles on other variables was assessed through hierarchical multiple linear regressions. Androgyny (high masculinity and high femininity) was associated with higher scores on positive indicators of professional quality of life and wellbeing at work. However, gender roles showed no significant moderating effect on the relationship between exposure to violence and indicators of psychological wellbeing at work. Results show how gender-based analyses can provide additional insights into the dynamics surrounding professional quality of life and wellbeing at work. They also suggest that androgyny could be related to potential psychosocial benefits for child protection workers.

Det finns överlag en brist på compassion-inriktade interventioner för vård- och omsorgspersonal. Tidigare studier tyder på att dessa interventioner ger goda resultat, men fler studier behövs. Hittills har exempelvis inga svenska studier undersökt effekter av Compassionate Mind Training (CMT) på individnivå eller med samvetsstress som utfallsmått. Studien syftade följaktligen till att undersöka om en internetbaserad CMT-kurs var en hjälpsam metod för att minska samvetsstress och arbetsrelaterad stress, samt öka självmedkänsla och professionell livskvalitet hos svensk vård- och omsorgspersonal. Studien var designad som en inomindividsstudie och utgick från en Single-Case Experimental Design (SCED), med vissa anpassningar. Totalt sex deltagare mötte inklusionskriterierna och deltog i en för- och eftermätning med självskattningsformulären Stress of Conscience Questionnaire (SCQ), Copenhagen Psychosocial Questionnaire II (COPSOQ-II), Self-Compassion Scale (SCS) och Professional Quality of Life Measure (ProQOL-5) samt fyra veckovisa mätningar under kursens gång. Slutsatsen var att CMT-kursen har varit hjälpsam i att minska samvetsstress och arbetsrelaterad stress samt i att öka självmedkänsla och professionell livskvalitet. Vidare forskning med starkare experimentell kontroll och uppföljningsmätningar behövs dock för att bekräfta resultaten. / There is overall a lack of compassion-based interventions for healthcare personnel. Previous studies suggest these interventions lead to good outcomes, however there is a need for more research. There are for example no Swedish studies that have yet examined the effect of Compassionate Mind Training (CMT) either on an individual level or with stress of conscience as outcome measure. Hence, the study aimed to evaluate whether a CMT-course is a helpful method for reducing stress of conscience and work-related stress, as well as increasing self-compassion and professional quality of life for healthcare personnel. The study had a within-subject design and used a Single-Case Experimental Design (SCED) with some adjustments. Six participants met the inclusion criteria and completed Stress of Conscience Questionnaire (SCQ), Copenhagen Psychosocial Questionnaire II (COPSOQ-II), Self-Compassion Scale (SCS) and Professional Quality of Life Scale (ProQOL-5) before and after the CMT-course in addition to four weekly questionnaires during the course. The conclusion was that CMT was helpful in reducing stress of conscience and work-related stress as well as increasing self-compassion and professional quality of life. However, research with more experimental control and follow up measurements are needed to confirm the results. / ICOP

Limited research has investigated the stress-related experiences of South African nursing students. Moreover, there is a scarcity of empirical studies that have reported on the development and evaluation of psycho-educational stress-management programmes that focus on both pathogenic, as well as positive and meaning-related factors among nursing students. The aim of this investigation was to study compassion fatigue, burnout, compassion satisfaction and meaning in life among a sample of nursing students with the aim of developing, and then empirically evaluating, a psycho-educational stress-management programme. The psycho-educational stress-management programme was articulated from a logotherapy-based perspective. A research design, consisting of three interdependent phases, was used to pursue the aim of the study. The aim of phase 1 was to describe the (1) prevalence of, and (2) correlations between, the deleterious and positive and meaningful effects of caring and among a sample of 80 nursing students (Mage = 22.40 years, SD = 11.1, female = 91.25%). The results indicated that participants may benefit from a logotherapy-based psycho-educational stress-management programme. The purpose of phase 2 of the study was to develop a logotherapy-based psycho-educational stress-management programme for nursing students. The goal of the logotherapy-based psycho-educational stress-management programme was to assist participants to develop the skills, knowledge and abilities that may be required to address deleterious challenges, and enhance positive and meaning-related opportunities. In phase 3 the logotherapy-based psycho-educational stress-management programme was presented to a sample of 42 first year nursing students (Mage = 20.21, SD = 1.57, female = 79.31%). A convergent parallel mixed methods research design was used to evaluate the efficacy of the logotherapy-based psycho-educational stress-management programme. Quantitative results indicated (1) a reduction in compassion fatigue and burnout, and (2) an increase in compassion satisfaction and meaning in life, scores over the course of the programme. Qualitative analysis supported the quantitative results. It was subsequently concluded that the logotherapy-based psycho-educational stress-management programme was effective in assisting participants to address the deleterious , as well embrace the positive and meaning-related effects of caring.
