La santé psychologique des travailleurs de la santé et des services sociaux au Québec en temps de pandémie : regard longitudinal sur la réaffectation des postes durant la COVID-19

Contexte. La pandémie du coronavirus COVID-19 a déclenché une crise sanitaire majeure depuis le début de l’année 2020. Les connaissances limitées et la rapidité de propagation du virus ont engendré une réaction précipitée de la santé publique du Québec créant des conditions éprouvantes pour les travailleurs de la santé et des services sociaux. Avant même l’arrivée de cette nouvelle réalité, les chiffres démontraient une problématique majeure quant à la prévalence et aux impacts engendrés par les problèmes psychologiques chez les travailleurs québécois. Les études ayant suivi les précédentes pandémies ont permis d’identifier plusieurs facteurs de risque engendrés par de telles crises chez les travailleurs de la santé et des services sociaux. Parmi les facteurs de risques importants au travail, on retrouve l’augmentation du stress vécu face à de nouvelles tâches, la perte de repères et l’absence de soutien, éléments qui sont généralement liés à une réaffectation des postes. Il est encore à ce jour difficile de bien comprendre l’ensemble des processus et l’interinfluence des facteurs qui affectent la santé psychologique des travailleurs en temps de crise. L’évolution de ces facteurs dans le temps n’est pas documentée. Objectif. Le présent projet évalue l’impact de la réaffectation des postes sur le développement de la détresse psychologique des travailleurs de la santé et des services sociaux du Québec ainsi que l’évolution hebdomadaire de ces aspects au cours de la pandémie COVID-19. Méthode. L’étude porte sur 169 travailleurs du réseau de la santé et des services sociaux du Québec provenant de 3 institutions différentes et occupant une variété de postes. Le projet suit un devis quantitatif longitudinal suivant un plan à série temporelle. Les données ont été collectées par une application mobile d’autosurveillance. Des analyses par modèles linéaires mixtes, puis des analyses de modération ont été effectuées. Résultats. La relation entre la réaffectation des postes et la détresse psychologique ne s’avère pas significative. L’exposition à la maladie et aux décès ne s’illustre pas comme modérant cette relation. La dépression est cependant associée au niveau de stress au travail ainsi qu’à la qualité de vie personnelle et professionnelle. L’anxiété est également associée à ces facteurs. Conclusion. Certaines hypothèses peuvent expliquer l’absence de relation entre la réaffectation des postes et la détresse : professionnels habitués au stress, omniprésence de la maladie au quotidien, niveau de stress constamment élevé. Le manque d’information sur la nature de la réaffectation ainsi que sur le niveau de base limite l’interprétation des résultats. Il est cependant possible que la réaffectation n'engendre aucun impact significatif sur la détresse vécue. Les travailleurs de la santé du réseau québécois ont possiblement fait preuve d’une grande adaptation, atténuant ainsi l’impact attendu de la réaffectation. / Background. The COVID-19 coronavirus pandemic has triggered a major health crisis since the beginning of 2020. health and social services. Even before the arrival of this new reality, the figures showed a major problem about the prevalence and impacts caused by psychological problems among Quebec workers. Studies following previous pandemics have identified several risk factors for such crises in health and social services workers. Among the major risk factors at work are the increase in stress experienced in the face of new tasks, the loss of bearings and the absence of support, elements which are generally linked to a reassignment of positions. It is still difficult to fully understand all the processes and the inter-influence of the factors that affect the psychological health of workers in times of crisis. The evolution of these factors over time is not documented. Objectives. This project considers the impact of job reassignment on the development of psychological distress among health and social services workers in Quebec as well as the weekly evolution of these aspects during the COVID-19 pandemic. Methods. The study focuses on 169 workers in the Quebec health and social services network from 3 different institutions and occupying a variety of positions. The project follows a longitudinal bill of quantities following a time series plan. The data was collected by a mobile self-monitoring application. Analyzes by mixed linear models, then moderation analyzes were carried out. Results. The relationship between job reassignment and psychological distress is not found to be significant. Exposure to illness and death does not appear to moderate this relationship. Depression is, however, associated with the level of stress at work as well as with the quality of personal and professional life. Anxiety is also associated with these factors. Conclusion. Some hypotheses may explain the lack of relationship between job reassignment and distress: professionals accustomed to stress, omnipresence of the disease in daily life, constantly high stress level. The lack of information on the nature of the reassignment as well as on the base level limits the interpretation of the results. It is however possible that the reassignment does not generate any significant impact on the distress experienced. Healthcare workers in the Quebec network may have shown great adaptation, thus mitigating the expected impact of the reassignment.

Transexualismo é definido como o desejo de viver e ser aceito como pertencente ao sexo oposto. É usualmente acompanhado por sentimento de desconforto ou inadequação em relação ao sexo anatômico, associado a desejo de se transformar cirúrgica e hormonalmente para tornar o corpo o mais congruente possível com o sexo pretendido. Com essa alteração, surge ainda a necessidade de adequação jurídica da pessoa física por meio de processo cível para mudança do nome e do sexo jurídicos, adequando-se, assim, o status civil/social. O objetivo deste trabalho foi observar como os laudos psicológicos poderiam ou não colaborar nas decisões judiciais, de acordo com a análise de sentenças judiciais para mudança de prenome em transexuais masculinos e femininos, submetidos ou não a cirurgia de transgenitalização. Realizamos levantamento por amostragem, que inicialmente caracterizou as variáveis sociais e demográficas dessa população e, posteriormente, sistematizamos os dados encontrados. A população estudada, que pleiteia a mudança do registro civil, foi mapeada a partir de dados de sentenças e processos para mudança de prenome, traçando-se um perfil nacional segundo os critérios definidos pelo SUS. Buscamos a amostra nos quatro centros credenciados para o processo transexualizador (Portaria no 457, de 19 de agosto de 2008). Encontramos 55 sujeitos que entraram com processo de mudança de prenome e/ou prenome e sexo, matriculados/acompanhados nos referidos hospitais. Observamos uma proporção de de 9:2 entre homem para mulher (MTF) e mulher para homem (FTM), semelhante ao que se relata na literatura internacional (4:1). Quanto às sentenças, apenas 5,50% (três) dos processos foram indeferidos, correspondentes a sujeitos do sexo biologico masculino que ainda não haviam sido submetidos à cirurgia. Observamos uma associação entre cirurgia e deferimento da mudança de prenome (p = 0,001). Em apenas 10,90% (seis) sentenças analisadas, havia menção ao laudo psicológico, que, portanto, foi omitido nas demais, 82,10% (49). Quando presentes, os laudos não foram elaborados por profissionais dos centros credenciados para o processo transexualizador, descaracterizandose como objetos deste estudo. Os resultados sugerem que os laudos psicológicos não influem na decisão judicial nos processos de mudança de prenome entre pacientes transexuais e que a realização prévia da cirurgia de readequação sexual está associada a um maior índice de êxito no pleito dos pacientes / Transsexualism is defined as a desire to live and be accepted as a member of the opposite sex, usually accompanied by a sense of discomfort (or inappropriateness) with one\'s anatomic sex, and a wish to have surgical and hormonal treatment to make one\'s body as congruent as possible with one\'s preferred sex. This change triggers the need for legal adjustment of the individual through civil proceedings for change the legal name and sex/gender. This study\'s objective was to observe whether psychological appraisals may or may not influence the judicial decision, by the analysis of the judges\' sentences for the change of forename in transsexual men and women, whether or not they underwent reassignment surgery. We performed a sample study where we initially characterized the social and demographic variables of this population, and then we systematized the data found. The population studied, who presented a plea to change theirs forename at the civil register, was mapped from data of trial sentences. From this data we present a national profile of patients according to criteria for the processo transexualizador, the treatment as laid down by the Brazilian National Health Care System. We seeked our sample at the four centers licensed for the \"processo transexualizador\" (Ordinance 457 of the Brazilian Health Care Ministry, dated August 19, 2008). We have found 55 subjects which entered pleas for changes of forename and / or pleas for changes of forename and sex. This subjects should be registered patients or be monitored at these hospitals. We found a proportion of 9:2 male to female (MTF) and female to male (FTM) patients, this finding is similar to reports from the international literature (4:1). When we analyzed for outcome of the judgments, we found that 5.50% (three) cases were rejected. This three cases belonged to biological male gender and had not yet been submitted to surgery. We observed a association between surgery and plea for first name\'s change (p = 0.001). Only 10.90% (six) of the sentences analyzed mentioned the psychological appraisal, which was omitted in 82.10% (49) of the sentences. We also found that when the psychological appraisals were present they were not issued by professionals working at licensed centers, therefore these did not meet the criteria for the study. Our findings suggest that the psychological appraisals do not affect the judicial decision over the pleas for forename changes among transsexual patients and the prior execution of the reassignment surgery is associated to a higher success rate in the patients\' litigations

Cell-Free Synthesis of Proteins with Unnatural Amino Acids: Exploring Fitness Landscapes, Engineering Membrane Proteins and Expanding the Genetic Code

Unnatural amino acids (uAA) expand the structural and functional possibilities of proteins. Numerous previous studies have demonstrated uAA as a powerful tool for protein engineering, but challenges also remain. Three notable such challenges include: (1) the fitness of uAA-incorporated proteins are difficult to predict and time-consuming to screen with conventional methods, (2) uAA incorporation in difficult-to-express proteins (e.g. membrane proteins such as G-protein coupled receptors) remain challenging, and (3) the incorporation of multiple types of uAA are still limited. In response, we pose cell-free protein synthesis (CFPS), a rapid and versatile in vitro expression system, as a platform to explore solutions to these challenges. The "cell-free" nature of CFPS enables it to accelerate protein expression and tolerate extensive modifications to its translational environment. In this work, these advantages were utilized to address the aforementioned challenges by: (1) rapidly expressing and screening uAA-containing proteins, (2) incorporating uAA in functional G-protein coupled receptor in the presence of membrane-mimicking lipid additives, and (3) engineer the translational environment extensively towards multiple uAA incorporation.

L'objectif de cette thèse est d'optimiser l'utilisation d'actionneurs redondants pour un avion de transport lorsqu'une défaillance des actionneurs arrive en vol. La tolérance aux pannes résulte ici de la redondance des actionneurs présents sur l'avion. Différents concepts et méthodes classiques liés aux chaînes de commande de vol tolérantes aux pannes sont d'abord examinés et de nouveaux concepts utiles pour l'analyse requise sont introduits. Le problème qui est abordé ici est de développer une méthode de gestion des pannes des commandes de vol dans le cas d'une défaillance partielle des actionneurs, qui va permettre à l'avion de poursuivre en toute sécurité la manœuvre prévue. Une approche de commande en deux étapes est proposée et appliquée à la fois à l'évaluation de la manœuvrabilité restante et à la conception de structures de commande tolérante aux pannes. Dans le premier cas, une méthode d'évaluation hors ligne des qualités de vol basée sur la commande prédictive est proposée. Dans le second cas, une structure de commande tolérante aux pannes basée sur la commande non linéaire inverse et la réaffectation des actionneurs en ligne est développée. Dans les deux cas, un problème de programmation linéaire quadratique (LQ) est formulé. Différents cas de pannes sont considérés lorsqu'un avion effectue une manœuvre classique. Trois solveurs numériques sont appliqués aux solutions en ligne et hors ligne des problèmes LQ qui en résultent.

Flexible-schedule-based TDMA protocols for supporting fault-tolerance, on-demand TDMA slot transfer, and peer-to-peer communication in wireless sensor networks

[Truncated abstract] This thesis develops a scheduled protocol (time division multiple access, TDMA) called flexible-schedule-based TDMA Protocol (FlexiTP), to address the problem of providing end-to-end guarantees on data delivery, whilst also respecting severe resource constraints of wireless sensor networks. FlexiTP achieves this balance through a distributed, synchronised, and loose slot structure in which sensor nodes can build, modify, or extend their schedules based on their local information. In FlexiTP, it is not necessary to predetermine the number of slots required for a network. FlexiTP's local repair scheme allows nodes to adjust their schedules dynamically and autonomously to recover from node and communication faults. Hence, it maintains a reliable and selforganising multihop network. Most sensor network protocols designed for data gathering applications implicitly assume a periodic rate of data collection from all nodes in the network to the base station. However, nodes may want to report their data more rapidly or slowly depending on the significance and importance of their data to the end-user. The problem is that traditional TDMA-based protocols are not flexible to changes in traffic patterns because of their rigid slot structure schemes. This thesis aims to solve this problem by developing an ondemand TDMA slot transfer method that leverages the flexible-slot structure algorithm of FlexiTP to transfer time slots from one part of the network to another part. ... While these communication patterns are sufficient for monitoring applications, individual sensor nodes may need to send their data to multiple destination nodes across the network in order to execute a distributed cooperative-function based on their local environment. This peer-to-peer communication pattern makes sensor networks more reactive to triggers from the environment. This thesis attempts to solve the problem of lack of peer-to-peer communication in the design of a TDMA-driven protocol by extending the idea of on-demand TDMA slot transfer method to allow each sensor node in the network to claim extra time slots to communicate with any other nodes (peers) in the network, without going through the base station. Nodes in the network may have different priorities of data because of event-triggering sensor readings or various types of sensor readings (e.g., light, temperature, and humidity) they provide. When nodes with high priority packets increase the frequency of their data collections, the network bandwidth may be dominated by these nodes. It is desirable to allow nodes with low priority packets to aggregate their packets and so enabling these nodes to send their data to the base station under the current available network bandwidth. This thesis proposes an on-demand data aggregation algorithm that enables sensor nodes to perform an in-network-aggregation based on their current sensing requirements and network capacity constraints. In summary, this thesis describes the design, implementation, and evaluation of protocols for wireless sensor networks that focus on achieving energy-efficiency, provisioning performance assurances, and supporting reactivity and adaptability in constantly changing environment.

The aim included seeking out how participants’ wellbeing was affected by being within this liminal space. Six participants were recruited in Johannesburg, Pretoria and Cape Town. Thematic analysis was used to derive themes from the transcripts. Themes included coming out; relationships; wellbeing; misgendering/misnaming; support; public and private medical care; and liminality.
