Pressão intraocular média obtida por três diferentes avaliadores utilizando os tonômetros de rebote e de aplanação em equinos

A mensuração da pressão intraocular (PIO) é essencial para o exame oftálmico, pois permite o diagnóstico de doenças oftálmicas como uveíte e glaucoma, além de auxiliar no monitoramento das cirurgias intraoculares. Objetivou-se comparar a tonometria de rebote com a tonometria de aplanação em equinos. Além disso, determinar a reprodutibilidade dos valores obtidos em ambos os aparelhos. A PIO foi aferida em 30 equinos da raça Crioula, machos ou fêmeas, com idade entre um e 16 anos utilizando o tonômetro de rebote (TonoVet®) e o tonômetro de aplanação (Tonopen Avia®). Previamente a aferição da PIO, todos os animais foram submetidos ao exame oftálmico incluindo, avaliação dos reflexos pupilares, teste da lágrima de Schirmer, prova da fluoresceína e biomicroscopia com lâmpada de fenda portátil. Foram utilizados apenas bulbos oculares hígidos. A análise estatística foi feita com ANOVA e teste t de Student. A reprodutibilidade foi avaliada pelo cálculo do coeficiente de correlação intraclasse (ICC). A média da PIO foi de 26,16 ± 5,13 mmHg utilizando o tonômetro de rebote e de 22,55 ±7,32 mmHg com o tonômetro de aplanação. A média da PIO de acordo com o avaliador utilizando o tonômetro de rebote foi de 25,27 ± 4,9 mmHg para o examinador A, 25,80 ± 5,2 mmHg para o examinador B e de 27,40 ± 5,1 mmHg para o examinador C. Com o tonômetro de aplanação as médias obtidas foram de 20,03 ± 4,5 mmHg com o examinador A, 19,67 ± 6,0 mmHg com o examinador B e de 27,97 ± 10,3 mmHg com o examinador C (p<2,3). O ICC foi de 0,18 para o tonômetro de aplanação e de 0,41 para o tonômeto de rebote. Os valores da pressão intraocular média obtidos com o tonômetro de rebote são superestimados quando comparados aos valores obtidos com o tonômetro de Tonopen Avia® em equinos saudáveis. Comparativamente ao Tonopen Avia® os valores obtidos com o Tonovet® demonstraram maior reprodutibilidade. / The measurement of intraocular pressure (IOP) is essential for the ophthalmic examination, as it allows the diagnosis of eye diseases such as uveitis and glaucoma, as well as aid in the monitoring of intraocular surgery. This study aimed to compare the mean intraocular pressure (IOP) values in horses obtained by three different examiners using the rebound and applanation tonometer’s. Each examiner measured the IOP of 30 “Crioulo” horses, males or females, aged one to 16 years old, using rebound tonometer and applanation tonometer. Previous to the experiment an ophthalmic examination was performed in horses. Only animals without alterations were selected. Three veterinarians measured IOP with rebound (Tonovet®) and applanation tonometer’s (TonoPen Avia®). Comparisons were made using ANOVA and student t test. Intraclass correlation coefficient (ICC) was calculated for reproducibility. IOP measurements resulted in mean values of 26.16 ± 5.13 mmHg for rebound tonometer and 22.55 ±7.32 mmHg for applanation tonometer. These values were different on statistical analysis. Rebound tonometer mean values, according with the examiner, were: A 25.27 ± 4.9 mmHg, B 25.80 ± 5.2, C 27.40 ± 5.1 mmHg. For applanation tonometer the mean IOP for examiner A (20.03 ± 4.5 mmHg), B (19.67 ± 6.0 mmHg), C (27.97 ± 10.3 mmHg). With the Tonopen Avia® tonometer examiner C measured higher IOP’s than examiners A and B. Intraclass correlation coefficient was 0.41 for rebound tonometer was 0.18 for applanation tonometer. It was observed that the values obtained with the Tonovet® were overestimated compared to Tonopen Avia® in healthy horses. When compared with Tonopen Avia® values obtained with the Tonovet® showed a higher reproducibility.

Avaliação de estratégias de treinamento para utilização do ICDAS e de técnicas de padrão-ouro para validação dos métodos de detecção de lesões de cárie em superfícies oclusais

O objetivo geral desta pesquisa foi avaliar estratégias de treinamento para a utilização do ICDAS, bem como, as técnicas de padrão-ouro utilizadas para validação dos métodos de detecção de lesões de cárie em superfícies oclusais. Para isso, dois estudos foram conduzidos. O primeiro estudo teve como objetivos: (1) avaliar o desempenho de estudantes de odontologia na detecção in vitro de lesões de cárie em superfícies oclusais utilizando o ICDAS após diferentes estratégias de treinamento e (2) avaliar a influência do treinamento de estudantes de odontologia através de um objeto digital de aprendizagem no desempenho em detectar in vitro lesões de cárie em superfícies oclusais utilizando o ICDAS. Já o segundo estudo teve como objetivo avaliar a influência do treinamento do examinador na reprodutibilidade de duas técnicas de padrão-ouro utilizadas para validação de cárie oclusal de acordo com três critérios de classificação histológica. Para o primeiro estudo, sessenta e quatro estudantes de odontologia que não tinham experiência em detecção de cárie através do ICDAS foram selecionados. Inicialmente, estes estudantes realizaram exames in vitro de 80 dentes permanentes para detecção de lesões de cárie em superfícies oclusais de acordo com o ICDAS utilizando apenas uma tabela com a descrição dos sete escores deste sistema. Todos os participantes foram aleatoriamente alocados em três grupos de acordo com a estratégia de treinamento para detecção de lesões de cárie utilizando o ICDAS: G1 (n= 21): e-learning do ICDAS; G2 (n= 22): e-learning do ICDAS + objeto digital de aprendizagem (ODA) sobre o ICDAS; G3 (n=21): nenhuma estratégia de treinamento (controle). Duas semanas depois, os alunos pertencentes ao G1 e G2 foram treinados de acordo com o grupo alocado. Transcorridas duas semanas deste treinamento, todos os participantes reavaliaram os dentes de acordo com o ICDAS. Os dentes foram preparados para validação histológica, na qual foi realizada a avaliação da extensão das lesões de cárie. Os resultados mostraram que na comparação nos grupos entre antes e depois da estratégia de treinamento foram encontrados valores significantemente maiores de sensibilidade e especificidade para G1 e G2, de sensibilidade para o G3 (no ponto de corte D2) e na área abaixo da curva ROC para G2. Na comparação entre os grupos em cada ponto de corte, o G1 apresentou especificidade maior no ponto de corte D3 que os outros dois grupos. Já a sensibilidade do G1 no ponto de corte D2 foi inferior ao G2 e G3. No segundo estudo foi selecionada uma amostra de 210 imagens digitais (105 de hemissecções dentárias sem corante e 105 com corante) de 105 dentes permanentes, além de uma amostra de seis cirurgiões-dentistas (três examinadores treinados para avaliação histológica de hemissecções dentárias e três examinadores da área odontológica, porém sem treinamento para avaliação histológica). Cada uma das 105 imagens digitais com corante e 105 sem corante foi avaliada em um monitor de computador pelos seis examinadores e classificadas de acordo com três critérios de classificação histológica. Cada uma das avaliações foi repetida pelos participantes para que a reprodutibilidade pudesse ser calculada. Assim, 12 exames independentes foram realizados, com um intervalo de 24 horas entre cada um. Para a análise dos dados, os seis participantes foram divididos em dois grupos: G1: examinadores treinados para avaliação histológica e G2: examinadores sem treinamento para avaliação histológica. Nos resultados, a média dos valores de reprodutibilidade intraexaminador no G1 foram superiores ao G2 em todas as variáveis. A reprodutibilidade intraexaminador foi menor para a técnica sem corante em ambos os grupos. Os valores médios de reprodutibilidade interexaminador no G1 variaram de 0,60 a 0,68 e no G2, os valores variaram de 0,34 a 0,69. Os resultados mostraram diferença estatisticamente significante entre as avaliações dos examinadores treinados e sem treinamento em 29,6% dos casos. Com base nos resultados obtidos no primeiro estudo, pode-se concluir que houve uma melhora no desempenho dos estudantes em detectar in vitro lesões de cárie em superfícies oclusais após ambas as estratégias de treinamento, em especial, quando o elearning foi associado ao ODA, visto que melhorou a relação entre sensibilidade e especificidade, além de contribuir no desempenho dos estudantes na detecção de lesões de cárie em todos os pontos de corte avaliados. A partir dos resultados do segundo estudo, concluiu-se que: (1) o treinamento do examinador parece influenciar na reprodutibilidade das duas técnicas de padrão- ouro estudadas; (2) a reprodutibilidade tende a ser menor quando se faz uso da técnica de padrão-ouro sem corante, independentemente do treinamento do examinador e (3) o critério de classificação histológica não parece ter influência na reprodutibilidade. / The aim of this study was to evaluate learning strategies for the use of ICDAS, as well as gold standard techniques used to validate occlusal caries detection methods. Therefore, two studies were carried out. The aims of the first study were: (1) to evaluate the performance of undergraduate dental students on ICDAS in vitro occlusal caries detection after different learning strategies; and (2) to evaluate the influence of undergraduate dental students training through a digital learning tool on the performance in detecting in vitro occlusal caries lesions using ICDAS. The second study aimed to evaluate the influence of the examiner's training on the reproducibility of two gold standard techniques used for validation of occlusal caries detection methods according to three histological classification criteria. For the first study, sixty-four undergraduate dental students who had no experience in caries detection using ICDAS were selected. Initially, the students performed in vitro examinations of 80 permanent teeth for the occlusal caries detection according to ICDAS using as guide a table describing all the seven scores of this system. All participants were randomly allocated into three groups according to the learning strategy for caries detection using ICDAS: G1 (n = 21): ICDAS e-learning program; G2 (n = 22): ICDAS e-learning program + digital learning tool (DLT); G3 (n = 21): no learning strategy (control). Two weeks later, students from G1 and G2 were trained according their group. After two weeks, all participants reassessed the teeth according to ICDAS. The teeth were prepared for histological validation, in which the evaluation of caries lesions depth was carried out. The results showed that, in the comparison in the groups between before and after the learning strategy, significant higher values of sensitivity and specificity for G1 and G2, of sensitivity for G3 (at D2 thresholds) and in the area under the ROC curve to G2 were observed. The comparison among the groups at each threshold showed that G1 presented higher specificity at the D3 threshold than the other two groups. The sensitivity of G1 at the D2 threshold was lower than G2 and G3. In the second study, a sample of 210 digital images (105 without coloring and 105 with dye enhancing) were selected from 105 permanent teeth, as well as a sample of six dentists (three trained examiners for the histological evaluation of teeth sections and three examiners of the dental area, but without training for histological evaluation). Each of the 105 digital images with dye enhancing e 105 without coloring was evaluated on a computer screen by the six examiners and classified according to three histological classification criteria. Each evaluation was repeated by the participants so that reproducibility could be calculated. Thus, 12 independent exams were performed, with a 24-hour interval. For data analysis, the six examiners were divided into two groups: G1: examiners trained for histological evaluation and G2: examiners without training for histological evaluation. In the results, mean values of intra-examiner reproducibility in G1 were higher than G2 in all variables. Intra-examiner reproducibility was lower for the dyefree technique in both groups. Mean values of inter-examiner reproducibility in G1 ranged from 0.60 to 0.68 and in G2, values ranged from 0.34 to 0.69. The results showed a statistically significant difference between the evaluations of the trained and untrained examiners in 29,6% of the cases. Based on the results obtained in the first study, it can be concluded that there was an improvement in the students' performance in detecting in vitro occlusal caries lesions after both learning strategies, especially when e-learning was associated with a DLT, since it improves the relation between sensitivity and specificity, besides contributing to the students' performance in the caries detection in all the thresholds studied. From the results of the second study, it was concluded that: (1) the training of the examiner seems to influence the reproducibility of both gold standard techniques studied; (2) reproducibility tends to be lower when using the gold standard technique without coloring, regardless of examiner training, and (3) the histological classification criterion does not seems to influence reproducibility.

No primeiro estudo utilizou-se 30 isolados clínicos e uma cepa de referência de C. albicans para obtenção de suspensões celulares (108ufc/mL) que foram plaqueadas nos meios específicos. Três variáveis independentes (nº de replicatas, cor de fundo e instrumento de leitura) originaram oito protocolos de leitura. A reprodutibilidade intra e interexaminadores da medida de produção de enzima (Pz) foi avaliada por meio do Coeficiente de Correlação Intraclasse (CCI). A enzima proteinase apresentou dois protocolos com ―Boa‖ reprodutibilidade intraexaminador (CCI ≥ 0,71) para todos os examinadores e o protocolo que utilizou leituras em triplicata sobre as fotografias contra luz natural atribuiu maior reprodutibilidade interexaminador para os valores de Pz-proteinase. Três protocolos com ‗Boa' concordância intraexaminador de Pz-fosfolipase foram selecionados para a análise interexaminador. Tanto as leituras em paquímetro como as realizadas em computador, em triplicata, mostraram reprodutibilidade ‗Muito Boa'. No estudo II, a concentração de CUR a 0,1µM foi utilizada associada a iluminação LED para promover dose subletal de IF. O grupo tratamento (C+L+) teve suas células fotossensibilizadas com CUR e iluminadas. O grupo controle positivo consistiu de amostras que receberam apenas CUR (C+L-), e o controle negativo não recebeu nem CUR nem luz (C-L-). A formação de biofilme foi analisada ...(Resumo completo, clicar acesso eletrônico abaixo) / This study aimed to: I. to establish a reproducible protocol using the agar-Based methodology to evaluate the production of the phospholipase and proteinase by the species Candida albicans (C. albicans); II. to analyze the expression of virulence factors after photodynamic inactivation (PDI) mediated by curcumin (CUR). In the first study 30 clinical isolates and a reference strain of the C. albicans were used to obtain cell suspensions (108CFU/mL) that were plated in specific media. Three independent variables (number of replicates, background color and reading instrument) originated eight reading protocols. The intra- and inter-examiner reproducibilityof the enzyme production measure (Pz) was assessed using the intraclass correlation coefficient (ICC). The enzyme proteinase presented two protocols with good intra-rater reproducibility (ICC≥0.71) and the readings in triplicate against natural light showed the highest inter-examiner reproducibility for Pz-proteinase values. Three protocols with 'Good' intra-rater agreement for Pz-phospholipase were selected for interexaminer analysis. Caliper and computer readings, performed in triplicate, showed 'Very Good' reproducibility. In Study II, CUR at 0,1μM was used with LED light to promote sublethal dose of the PDI. The treatment group (C+L+) had its cells photosensitized with CUR and illuminated. The control group consisted of the samples that received only CUR (C+L-), and another which received neither CUR or light (C-L-). The adhesion ability was evaluated by means of CFU/mL and the XTT method. Biofilm formation was analyzed by means of CFU/mL, XTT and Crystal Violet (CV). Simulations parallèles de Monte Carlo appliquées à la Physique des Hautes Energies pour plates-formes manycore et multicore : mise au point, optimisation, reproductibilité / Monte Carlo parallel simulations applied to the High Energy Physics for manycore and multicore platforms : development, optimisation, reproducibility

Lors de cette thèse, nous nous sommes focalisés sur le calcul à haute performance, dans le domaine très précis des simulations de Monte Carlo appliquées à la physique des hautes énergies, et plus particulièrement, aux simulations pour la propagation de particules dans un milieu. Les simulations de Monte Carlo sont des simulations particulièrement consommatrices en ressources, temps de calcul, capacité mémoire. Dans le cas précis sur lequel nous nous sommes penchés, la première simulation de Monte Carlo existante prenait plus de temps à simuler le phénomène physique que le phénomène lui-même n’en prenait pour se dérouler dans les conditions expérimentales. Cela posait donc un sévère problème de performance. L’objectif technique minimal était d’avoir une simulation prenant autant de temps que le phénomène réel observé, l’objectif maximal était d’avoir une simulation bien plus rapide. En effet, ces simulations sont importantes pour vérifier la bonne compréhension de ce qui est observé dans les conditions expérimentales. Plus nous disposons d’échantillons statistiques simulés, meilleurs sont les résultats. Cet état initial des simulations ouvrait donc de nombreuses perspectives d’un point de vue optimisation et calcul à haute performance. Par ailleurs, dans notre cas, le gain de performance étant proprement inutile s’il n’est pas accompagné d’une reproductibilité des résultats, la reproductibilité numérique de la simulation est de ce fait un aspect que nous devons prendre en compte.C’est ainsi que dans le cadre de cette thèse, après un état de l’art sur le profilage, l’optimisation et la reproductibilité, nous avons proposé plusieurs stratégies visant à obtenir plus de performances pour nos simulations. Dans tous les cas, les optimisations proposées étaient précédées d’un profilage. On n’optimise jamais sans avoir profilé. Par la suite, nous nous intéressés à la création d’un profileur parallèle en programmation orientée aspect pour nos besoins très spécifiques, enfin, nous avons considéré la problématique de nos simulations sous un angle nouveau : plutôt que d’optimiser une simulation existante, nous avons proposé des méthodes permettant d’en créer une nouvelle, très spécifique à notre domaine, qui soit d’emblée reproductible, statistiquement correcte et qui puisse passer à l’échelle. Dans toutes les propositions, de façon transverse, nous nous sommes intéressés aux architectures multicore et manycore d’Intel pour évaluer les performances à travers une architecture orientée serveur et une architecture orientée calcul à haute performance. Ainsi, grâce à la mise en application de nos propositions, nous avons pu optimiser une des simulations de Monte Carlo, nous permettant d’obtenir un gain de performance de l’ordre de 400X, une fois optimisée et parallélisée sur un nœud de calcul avec 32 cœurs physiques. De même, nous avons pu proposer l’implémentation d’un profileur, programmé à l’aide d’aspects et capable de gérer le parallélisme à la fois de la machine sur laquelle il est exécuté mais aussi de l’application qu’il profile. De plus, parce qu’il emploi les aspects, il est portable et n’est pas fixé à une architecture matérielle en particulier. Enfin, nous avons implémenté la simulation prévue pour être reproductible, performante et ayant des résultats statistiquement viables. Nous avons pu constater que ces objectifs étaient atteints quelle que soit l’architecture cible pour l’exécution. Cela nous a permis de valider notamment notre méthode de vérification de la reproductibilité numérique d’une simulation. / During this thesis, we focused on High Performance Computing, specifically on Monte Carlo simulations applied to High Energy Physics. We worked on simulations dedicated to the propagation of particles through matter. Monte Carlo simulations require significant CPU time and memory footprint. Our first Monte Carlo simulation was taking more time to simulate the physical phenomenon than the said phenomenon required to happen in the experimental conditions. It raised a real performance issue. The minimal technical aim of the thesis was to have a simulation requiring as much time as the real observed phenomenon. Our maximal target was to have a much faster simulation. Indeed, these simulations are critical to asses our correct understanding of what is observed during experimentation. The more we have simulated statistics samples, the better are our results. This initial state of our simulation was allowing numerous perspectives regarding optimisation, and high performance computing. Furthermore, in our case, increasing the performance of the simulation was pointless if it was at the cost of losing results reproducibility. The numerical reproducibility of the simulation was then an aspect we had to take into account. In this manuscript, after a state of the art about profiling, optimisation and reproducibility, we proposed several strategies to gain more performance in our simulations. In each case, all the proposed optimisations followed a profiling step. One never optimises without having profiled first. Then, we looked at the design of a parallel profiler using aspect-oriented programming for our specific needs. Finally, we took a new look at the issues raised by our Monte Carlo simulations: instead of optimising existing simulations, we proposed methods for developing a new simulation from scratch, having in mind it is for High Performance Computing and it has to be statistically sound, reproducible and scalable. In all our proposals, we looked at both multicore and manycore architectures from Intel to benchmark the performance on server-oriented architecture and High Performance Computing oriented architecture. Through the implementation of our proposals, we were able to optimise one of the Monte Carlo simulations, permitting us to achieve a 400X speedup, once optimised and parallelised on a computing node with 32 physical cores. We were also able to implement a profiler with aspects, able to deal with the parallelism of its computer and of the application it profiles. Moreover, because it relies on aspects, it is portable and not tied to any specific architecture. Finally, we implemented the simulation designed to be reproducible, scalable and to have statistically sound results. We observed that these goals could be achieved, whatever the target architecture for execution. This enabled us to assess our method for validating the numerical reproducibility of a simulation.

Este trabalho propõe-se à reorganização e releitura de textos benjaminianos que se referem direta ou indiretamente à questão da percepção. Objetiva levar à maturação e sistematização de elementos para uma teoria da percepção benjaminiana que se encontra dispersa em alguns de seus diversos estudos. Aborda o conceito de percepção colocando-o em situação constelacional com outros conceitos como Estética, comentário, critica, percepção ótica, percepção tátil. Discute sobre o conceito de percepção em alguns momentos da tradição filosófica. Reflete sobre os regimes escópicos da modernidade. E discute sobre a compreensão benjaminiana da percepção como leitura. Aborda uma questão fundamental no pensamento benjaminiano sobre os meios audiovisuais que é o conceito de aura. Intenta situar os termos da discussão controversa sobre o declínio da aura e discorre sobre a forma de percepção que predominaria na recepção cinematográfica: a percepção de choque. Avalia a legibilidade da cidade, através de uma reflexão sobre a flânerie e a visão surrealista, entendidas como práticas sociais inspiradoras da original reflexão de Benjamin sobre a percepção. / The present thesis aims to show the result of Walter Benjamins theory of perception texts organization edited in several publishing. Thus, the general concept of perception is divided into other concepts such as Aesthetics, Comment, Criticism, optical perception and Philosophical perception, presenting the scopic regimes of modernity. Besides, it highlights Benjamins concept of perception meaning the same as reading. It also covers a fundamental subject in his thoughts on visual arts: the aura concept. Regional pulmonary function analysis using image registration and 4DCT

Current radiation therapy (RT) planning for limiting lung toxicity is based on a uniform lung function with little consideration to the spatial and temporal pattern of lung function. Establishment of relationships between radiation dose and changes in pulmonary function can help predict and reduce the RT-induced pulmonary toxicity. Baseline measurement uncertainty of pulmonary function across scans needs to be assessed, and there is a great interest to compensate the pulmonary function for respiratory effort variations. Respiratory-gated 4DCT imaging and image registration can be used to estimate the regional lung volume change by a transformation-based ventilation metric which is computed directly from the deformation field, or a intensity-based metric which is based on CT density change in the registered image pair. In this thesis, we have evaluated the reproducibility of regional pulmonary function measures using two repeated 4D image acquisitions taken within a short time interval for both transformation-based and intensity-based metrics. Furthermore, we have proposed and compared normalization schemes that correct ventilation images for variations in respiratory effort and assess the reproducibility improvement after effort correction. The major contributions of this thesis include: 1) develop and validate a process for establishing measurement reproducibility in 4DCT-based ventilation, 2) evaluate reproducibility of the transformation-based ventilation measurement, 3) evaluate reproducibility of the intensity-based ventilation measurement, 4) develop and compare different ventilation normalization methods to correct for respiratory effort variation across scans.

Optimalan broj taksivozila predstavlja prvi korak ka<br />postavljanju &bdquo;zdravih odnosa&ldquo; između svih činioca sistema taksi<br />transporta u gradovima i omogoćuje ostvarenje reproduktivne<br />sposobnosti operatera. Tendinopathien der Achilles- oder Patellarsehne sind häufig in Sportarten mit zahlreichen repetitiven Belastungen im Dehnungs-Verkürzungs-Zyklus der unteren Extremität zu finden. Sowohl eine möglicherweise alterierte belastungsspezifische neuromuskuläre Antwort (NMA), als auch funktional begründete Therapiemaßnahmen mit möglichen positiven Effekten sind aktuell ungeklärt. Ziel der Arbeit war deshalb die Untersuchung der belastungsspezifischen neuromuskulären Antwort bei Athleten mit Tendinopathie der Achilles- oder Patellarsehne im Vergleich zu beschwerdefreien Athleten. Zusätzlich sollten mögliche funktionale und therapeutische Effekte eines sensomotorischen Trainings im randomisierten, kontrollierten und prospektiven Studiendesign überprüft werden. 51 Sportler mit unilateraler Tendinopathie (Achilles-/Patellarsehne n = 35/16) und 33 gesunde Sportler wurden zur Beurteilung der belastungsspezifischen neuromuskulären Antwort eingeschlossen. Zur Klärung der Effekte eines sensomotorischen Trainings im Längsschnitt konnten 26 Sportler mit Tendinopathie randomisiert zu einer Kontrollgruppe (n = 14) und einer Therapiegruppe mit sensomotorischem Training (n = 12) zugeordnet werden. Nach einer ersten biomechanischen Messung M1 (Belastungssituationen: Lauf-, Stabilisations-, Kraftbelastung) und der Erhebung der subjektiven Schmerzsymptomatik folgte eine 8-wöchige Therapiephase mit einer abschließenden Re-Test-Messung M2 identisch zu M1. Das sensomotorische Training war auf die gesamte untere Extremität ausgerichtet und wurde nach Einweisung regelmäßig kontrolliert. Die Erfassung der NMA erfolgte über die Quantifizierung der muskulären Aktivität (EMG). Zusätzlich wurde die Kinetik (z.B. Maximalkraft) belastungsspezifisch erfasst. Eine reduzierte NMA konnte für die Sportler mit Tendinopathie über veränderte EMG-Zeit- und Amplitudenmessgrößen, eine reduzierte aktive Stabilisationsfähigkeit und Maximalkraft (p < 0,05) nachgewiesen werden. In Abhängigkeit der Lokalisation (Achilles-/Patellarsehen) bzw. der Seite (Beschwerdeseite/gesunde Seite) ergaben sich keine relevanten Differenzen. Das sensomotorische Training zeigte eine Optimierung der NMA (z.B. erhöhte Maximalkraft) bei Tendinopathie der Achilles- oder Patellarsehne. Die Überprüfung der Beschwerdesymptomatik wies in allen Schmerz-Scores nach der Therapie reduzierte Werte und damit einen positiven therapeutischen Effekt gegenüber der Kontrollgruppe auf. Zusammenfassend kann eine systematisch reduzierte NMA bei Lauf-, Stabilisations- und Kraftbelastung der Sportler mit Tendinopathie nachgewiesen werden. Das sensomotorische Training ist funktional und therapeutisch als effiziente Therapiemaßnahme zu erachten. / Tendinopathies of the achilles- or patellartendon are common overuse unjuries in sports including high numbers of repetitive stretch-shortening-cycles of the lower limb. Possible alterations of the neuromuscular response (NMR) to physical stress and functionally justified therapies with potential positive effects are currently unknown. The aim of the study was therefore the analysis of the neuromuscular response in situations with physical stress in athletes with tendinopathie of the achilles or patellartendon compared to symptom free athletes. In addition, possible functional and therapeutic effects of a sensorimotor training were studied in a prospective, randomized and controlled study. 51 athletes with unilateral tendinopathy (achilles-/patellartendon n = 35/16) and 33 healthy athletes were included to assess the stress related neuromuscular response (NMR). To clarify the effects of a sensorimotor training 26 athletes with tendinopathy were randomized to a control group (n = 14) and a therapy group with sensorimotor training (n = 12). After initial biomechanical measurements M1 (situations: running, active postural stability and maximum strength test) and the collection of subjective complaints (pain) an 8-week treatment phase followed with a final re-test M2 measure identical to M1. The sensorimotor training was arranged for the entire lower extremity, and was checked after an initial briefing on a regular basis. The assessment of the NMR was quantified by muscle activity (EMG). In addition, the kinetics (e.g. maximum force) was acquired in the stabilization and maximum strength test. A reduced NMR could be proven for athletes with tendinopathy with changed EMG time and amplitude measurements, a reduced active postural stabilization ability and reduced maximum strength (p <0.05). No relevant differences could be observed in comparing localization (achilles-/patellartendon) and side (leg with complains/healthy leg). The sensorimotor training showed an optimization of NMR (e.g. increased maximum strength) for patient with tendinopathy of the achilles- or patellartendon. The examination of the complaints showed reduced values in all pain scores after therapy. The treatment had therefore positive therapeutic effects compared to the control group. In summary, a systematically reduced NMR in running, stabilization and strength tests are evident for athletes with tendinopathy. The sensorimotor training can be considered as an efficient treatment with functional and therapeutic effects.

Advancements in high throughput protein profiling using surface enhanced laser desorption/ionization time of flight mass spectrometry

Surface-enhanced laser desorption/ionization time-of-flight mass spectrometry (SELDI)is one of several proteomics technologies that can be used in biomarker discovery studies. Such studies often have the goal of finding protein markers that predict early onset of cancers such as cervical cancer. The reproducibility of SELDI has been shown to be an issue in the literature. There are numerous sources of error in a SELDI experiment starting with sample collection from patients to the signal processing steps used to estimate the protein mass and abundance values present in a sample. This dissertation is concerned with all aspects of signal processing related to SELDI's use in biomarker discovery projects. In chapter 2, we perform a comprehensive study of the most popular preprocessing algorithms available. Next, in chapter 3, we study the basic statistics of SELDI data acquisition. From here, we propose a quadratic variance measurement model for buffer+matrix only spectra. This model leads us to develop a modified Antoniadis-Sapatinas wavelet denoising algorithm that demonstrates superior performance when compared to MassSpecWavelet, one of the leading techniques for preprocessing SELDI data. In chapter 4, we show that the quadratic variance model 1) extends to real pooled cervical mucus QC data from a clinical study, 2) predicts behavior and reproducibility of peak heights, and 3) finds four times as many reproducible peaks as the vendor-supplied preprocessing programs. The quadratic variance measurement model for SELDI data is fundamental and promises to lead to improved techniques for analyzing the data from clinical studies using this instrument.
