Bedside Reporting: Improving PracticeWichman, Lori 01 January 2017 (has links)
Bedside reporting is one way to improve communication among the health care team. At the study site, at least 50% of bedside reporting was being conducted at the door of the patient's room instead of at the patient's bedside. The project question addressed whether a computer-based education and training video on bedside reporting and a standardized bedside reporting checklist would increase the rate of bedside shift-to-shift reporting among a medical surgical unit (MSU) to 100%. The project addressed the implementation of standardizing bedside reporting through education and training using Agency for Health Care Research and Quality's (AHRQ) Guide to patient and family engagement. The project also promoted use of a standardized tool to conduct bedside reporting and a surveillance tool to ensure bedside reporting was being conducted. Data was collected through surveys and surveillance. The data was tabulated for frequencies displayed in percentages. Post-implementation findings indicated that bedside reporting went from 0% to 86% during the 2-week surveillance period. MSU nursing staff improved their knowledge and skill on how to conduct bedside reporting, but their attitude did not change as they thought the practice of bedside reporting was not an effective use of their time. Findings may be used to increase involvement of patients and families in their inpatient health care.

Tidigare studier berör främst funktion, praktik, innehåll och relevans i den muntliga rapporten. När det gäller omvårdnad i den muntliga rapporten hittades ingen vetenskaplig litteratur. Författarna i den här studien anser att omvårdnad är lika viktig som medicinen i patientvården. Syftet är att belysa omvårdnad i den muntliga rapporten mellan sjuksköterskor. En observationsstudie utfördes på sex avdelningar på ett sjukhus i Skaraborg. Data spelades in på band för att transkriberas, analyseras och tolkas. Resultatet visar att omvårdnad förekommer i den muntliga rapporten mellan sjuksköterskor genom sex huvudteman och tio underteman. Det rapporterades om patienternas tillstånd, vilka åtgärder sjuksköterskorna utfört och vad sjuksköterskorna som skulle påbörja sitt arbetspass skulle tänka på och göra under sitt skift. Studien tolkar att den information som sjuksköterskor delar med varandra under en rapport är omvårdnad då den möjliggör en god patientvård. / Prior studies mainly concern the function, practice, content and relevance of the shift report. When it comes to care in the shift report no scientific literature were found. The authors of this study consider care to be just as important as medicine in the patient care. The aim of this study is to shed light on care in the nurse-to-nurse shift report. An observational study was carried out in six divisions at a hospital. The data was recorded on tape to be transcribed, analyzed and interpreted. The results imply, through categorization into six main themes and ten sub themes, that care is evident during the nurse-to-nurse shift report. Patient’s condition and which actions the nurses had executed were reported, as well as what the nurses, who were about to start their work shift, had to perform and keep in mind. The information being shared in the nurse-to-nurse shift report is in this study interpreted as care seeing that it make a good patient care possible.

Impact of a Standardized Tool on Handoff Quality in Nurse Change-of-Shift ReportsAyala, Wilma Lonzame 01 January 2017 (has links)
Within the context of healthcare delivery, human lives are at risk when health care professionals fail to communicate effectively. Audits by The Joint Commission from 1995 to 2005 revealed that 65% of sentinel events occurred as a result of staff miscommunication, prompting the requirement that standardized handoff tools be deployed. Therefore, this project was completed to improve safety in the hospital through implementation of a standardized tool that could enhance the quality of nurse handoff communication. Mohorek & Webb's (2015) linear model of communication, which emphasizes the importance of encoding, transmission, and decoding in the communication process, provided the framework for this project. Participants included 11 registered nurses (RNs) and 14 licensed vocational nurses (LVNs) who completed the pre-and-post intervention survey using the Handover Evaluation Scale. The RNs reflected significant difference in improvement in the perceived quality of handoff following the implementation of standardized handoff tool: pre-test (M = 66.91, SD = 7.27) compared to post-test (M = 80.91, SD = 7.45); t(10) = -5.09, p = 0.000. On the other hand, there was no statistically significant change noted with the LVN group before implementation of standardized handoff tool: pre-test (M = 70.71, SD = 9.72) compared to post- test implementation (M = 73.57, SD = 7.73); t(13) = -1.06, p = 0.309. The findings resulting from this project suggest that there are important differences in provider types when using standardized handoff tools and that more attention to this dynamic is warranted. The social change mandate of protecting the safety of patients is enhanced through effective communication among nurses and was demonstrated in the project.

Bakgrund: Sjuksköterskor har i sitt dagliga arbete till uppgift att ge och ta emot rapport om patienterna hen vårdar. En god kommunikation krävs för att säkerställa att relevant information förmedlas och inte patientsäkerheten hotas. Syfte: Att beskriva strategier för hur kommunikationen kan förbättras vid överrapportering mellan sjuksköterskor inom den somatiska slutenvården. Metod: Litteraturöversikten är baserad på 15 vetenskapliga artiklar med kvalitativ, kvantitativ och mixad ansats. Artiklarna identifierades via sökningar i databaserna Cinahl, PubMed och Web of Science. Resultat: Tre strategier framkom: personlig överrapportering, användandet av standardiserade formulär och överrapportering vid patientens säng. Strategierna gav en effektivare, säkrare och mer tidssparande överrapportering. Slutsats: En god kommunikation mellan sjuksköterskorna är oerhört viktigt för att bibehålla en trygg och säker vård för patienterna. För att få en förbättrad kommunikation mellan sjuksköterskor vid överrapportering kan olika strategier användas. Då sjuksköterskorna ofta har en hög arbetsbelastning så krävs det att överrapporteringarna är strukturerade för att inte tappa bort viktig information. / Background: Information handover about patients is a central task in registered nurses’ daily work. Communication is important to safeguard that relevant information is transferred and that patient safety is not violated. Aim: To describe strategies that can improve communication at handover between registered nurses in somatic hospital wards. Method: The literature review is based on 15 scientific articles including qualitative, quantitative and mixed approach, identified in the databases Cinahl, PubMed and Web of Science. Results: Three strategies were identified: individual handover, the use of standardized forms, and bedside nursing handover. These strategies provided a more efficient, safer and more time-saving handover. Conclusion: Different strategies can be used for enhanced handover communication between registered nurses. Registered nurses often have a heavy workload which requires that the handovers are well structured in order not to lose important information. A good communication between nurses are extremely important to maintain a safe and secure care for patients.

Communication competence and medical communication competence served as the theoretical framework for this research that seeks to identify specific communication behaviors associated with what nurses say constitute a communicatively competent patient handoff at the nursing change of shift. Data collected from 286 nurses responding to an online modified Medical Communication Competence Scale posted at www.allnurses.com supported the hypotheses that information exchange (information giving, seeking and verifying) and socioemotional communication behaviors are rated more highly in the best patient handoffs than in the worst ones. Research questions found that the incoming nursing role rated behaviors associated with information verifying and socioemotional communication higher than did the outgoing nursing role, and that the worst handoffs were those in which the incoming nursing role gave the lowest ratings for information-giving behaviors. Additional insight into other communication-related characteristics associated with quality handoffs were provided as well, including location, tools/type and environment for the patient handoff at the nursing change of shift. These findings offer a foundation for future research into development of communication-based standardized patient handoff processes and training that ultimately may reduce patient care errors caused by communication failures during the patient handoff at the nursing change of shift.

The Move from Recorded to Bedside Shift Report: Evaluating Barriers to Full ImplementationCipra, Elizabeth Jackson 01 January 2016 (has links)
Many sentinel events in acute care centers can be attributed to miscommunication of patient information at shift change. A growing body of evidence supports the implementation of bedside shift report as the standardized approach to ensure effective communication by staff. The purpose of this study was to identify the barriers that impede nurses from fully participating in bedside shift report in order to create an educational initiative to develop the nursing staff's proficiency in performing bedside report. Lewin's change theory served as the framework for the project. Data for the staff development project were collected using a focus group approach with 18 nurse participants. A classic method of analysis, defined and outlined by Krueger and Casey (2009), was used to identify themes and categorize results. A second evaluator of the data supported thematic findings. Nurses revealed barriers consistent with the literature including frequent interruptions, patient confidentiality, sensitive issues, and inconsistent report content. Barriers identified by this study, but not evident in the reviewed literature, included staff unavailability to answer call lights and having to receive report and transfers during report. An education plan was developed based on perceived barriers and evidence in the literature. Implications for positive social change include interventions to refine the current practice of bedside shift report in order to promote effective and efficient communication at change of shift. Findings can inform nursing units in the hospital and other facilities to achieve patient-centered care and improved outcomes. It is critical that nurses understand the positive impact that consistent, timely, and effective shift reports have on providing safe, quality patient care.

Bakgrund: För att säkerställa säker och kontinuerlig vård är det väsentligt att kommunikation och samverkan mellan sjukvårdspersonal fungerar. Exempel på sådan samverkan kan äga rum vid överrapportering mellan två sjuksköterskor under skiftbyte, vilket innebär att sjuksköterskor delger varandra väsentlig patientinformation. Informationsöverföring är ett särskilt utsatt riskområde i vården och ett kritiskt moment för att uppnå god patientsäkerhet. Det finns olika typer av överrapportering, det går även att anpassa överrapporteringen med olika modeller och verktyg. God kommunikation är ett av sjuksköterskans viktigaste redskap i omvårdnadsarbetet. Syfte: Studiens syfte är att belysa grundutbildade sjuksköterskors upplevelser av överrapportering och dess inverkan på patientsäkerheten. Metod: Litteraturstudien baserades på tolv vetenskapliga artiklar med kvalitativ ansats som sedan utgjorde studiens resultat. Artiklarna inhämtades från två olika databaser, med inriktning på omvårdnad. Artiklarna kvalitetsgranskades med SBU:s granskningsmall som underlag. Data analyserades i fem steg som tog isär artiklarnas helhet till delar. Slutligen bildade delarna en ny helhet. Resultat: Utifrån analysen kunde två teman identifieras och utifrån dem kunde tio tillhörande underkategorier formas. De två temana var Bygga en gemensam patientbild och Överrapportering. Konklusion: Överrapportering är en mångsidig process. Sjuksköterskor beskrev många upplevda faktorer som kunde påverka kvalitén på överrapporteringen och därmed hur överrapporteringen påverkade patientsäkerheten. Sjuksköterskor upplevde att en fullgod överrapportering hade positiv inverkan på patientsäkerheten. / Background: To ensure safe and continuous care it’s vital that communication and coordination amongst medical personnel is working. This type of coordination can take place during shift report between two nurses at shift change, during which nurses share vital patient information. Transfer of information is an especially vulnerable risk area within health care and a crucial moment to ensure patient safety. There are different types of shift report, different models and instruments can also be used to customize the shift report. Adequate communication is one of nurses’ most important tool in nursing care. Aim: The aim of the study is to highlight nurses’ experiences of shift report and its impact on patient safety. Method: The literature review was based on twelve scientific articles with qualitative study design that created the structure for the literature review’s result. The articles were retrieved from two databases focusing on nursing care. The articles’ quality was assessed using a template from the SBU. The data was analyzed in five steps where the essence from the articles’ results was extracted from a whole into individual parts. Finally, the parts were assembled making a new whole. Result: Two themes were identified from the analysis, and based on them ten subcategories could be created. The themes were Building a mutual patient picture and Experiences of shift report. Conclusion: Shift report is a many-sided process. Nurses described having experienced many different factors that could affect the quality of the shift report and therefore the shift report’s effect on patient safety. Nurses experienced that an adequate shift report had a positive impact on patient safety.

Bakgrund: Överrapportering mellan skiftbyten inom vården har en viktig roll när det gäller överföring av information till nästkommande sjuksköterska. Inom hälso- och sjukvården finns olika rapporteringssätt, samtal mellan sjuksköterskor avskilt på en expedition och rapportering vid sängkanten hos patienten. I ett flertal studier har sjuksköterskors uppleverser av överrapportering vid sängkanten undersökts, vilket har visat att det ger en ökad personcentrering i omvårdnaden, ökad patientsäkerhet och minskade missförstånd vid kommunikationen. Syfte: Studiens syfte var att beskriva hur vuxna patienter inom slutenvården upplever sjuksköterskors överrapportering vid sängkanten i samband med skiftbyten. Metod: Litteraturöversikten har utgått från 12 kvalitativa originalartiklar som kvalitetsgranskats och analyserats där två av dessa både är kvalitativa och kvantitativa. Resultat: Totalt fyra kategorier identifierades i resultatet: mer personcentrering, ökad trygghet, ett observandum och upplevda hinder. Majoriteten av patienterna upplevde att överrapportering vid sängkanten ökade deras delaktighet och involvering i vården. Genom att bidra med information och klargöra missförstånd i kommunikationen upplevde patienterna minskad osäkerhet och ökad trygghet. De ansåg att medicinska uttryck som användes av sjuksköterkorna kunde förhindra deras förståelse. Språkbarriärer och patienters hälsotillstånd identifierades även som hinder i överrapporteringarna. Känslig information ansågs inte som något hinder, förutsatt att informationen hanterades med varsamhet, respekt och personlig hänsyn till patienten utifrån vårdsituation. Slutsats: I litteraturöversikten har resultat av patienters upplevelser gällande överrapportering vid sängkanten identifierats, vilka övervägande var positiva upplevelser. Det identifierades också negativa aspekter av arbetssättet. / Background: Handovers between shift changes in healthcare play an important role in the transfer of information to the next nurse. In healthcare, there are different ways of reporting, conversations between nurses in a separate office and reporting at the patient´s bedside. A number of studies have investigated nurses´experiences of bedside reporting, which have shown that it provides increased person-centeredness in nursing, increased patient safety and reduced misunderstandings in communication. Purpose: The purpose of the study was to describe how adult patients in inpatient care experience nurse´handover at the bedside in connection with shift changes. Method: The literature review is based on 12 original qualitative aticles who were quality reviewed and analyzed, two of which are both qualitative and quantitative. Results: A total of four main categories was identified: more person-centered care, increased security, an observandum and perceived obstacles. The majority of patients felt that bedside handover increased their participation and involvement in their care. By providing information and clarifying misunderstandings in communication, patients experienced reduced uncertainty and increased safety. They felt that medical terms used by nurses could hinder their understanding. Language barriers and patients´health conditions were also identified as obstacles in the transfer of information. Sensitive information was not considered a barrier, provided it was handled with care, respect and personal consideration for the patient based on the care situation. Conclusion: In the literature review, results of patients´experiences regarding bedside over-reporting have been identified, which werw predominantly positive experiences. Negative aspects regarding the topic were also identified.
