Naplňování kritérií pro saturaci potřeb pacienta / Fulfilling the criteria for saturation of the patient's needs

The thesis deals with the realization of the criteria for saturation of the patient's needs. In the current system of nursing care, the principles of modern nursing are failing to realize and therefore we want to find out the main obstacles of this situation and the possibilities of their removal. For the purpose of the thesis there were set three objectives. The first of them has set the objective of finding out the most common problems in meeting the criteria for saturation of the patient's needs. The research showed that the nurses see the problem in a lack of staff at the wards, a lack of time, excess of documentation, excessive demands for nurses. The purpose of the second objective was to find out what supports the realization of the criteria for saturation of the patient's needs. Results have shown that what helps nurses is a positive work environment, motivation, nurse´s personality, own scale of values, salary, good leadership, organization, nursing care. The third objective was to find out the opinion of nurses on meeting the criteria for saturation of the patient's needs. It turned out that the majority of respondents think that nurses do not effectively satisfy all the needs of patients for a number of reasons: there are few nurses and they are overloaded, the nurses do their best but still cannot satisfy all the patients´ needs. We also found out that the higher patients´ needs are omitted. Less than a third of the nurses indicated that nurses effectively meet all the patients´ needs. For the qualitative part of the research, we set two research questions in advance. Which criteria nurses use in patients´ needs saturation? The research showed that time, which is very valuable for nurses and very often is in short supply, is an important criteria for the saturation of the patient's needs. Furthermore, it is knowledge and manual dexterity when nurses additionally reported a lack of education of some nurses especially within the nursing process. As the next criteria, it was mentioned the cooperation with the patient as it is necessary to engage patient himself in care planning. The next criteria is monitoring of the patient´s response, which is included in every phase of the nursing process. Furthermore, the nurses agreed on criteria, which include communication as a key skill of every nurse, then individual care, a suitable environment and utilities, education, critical thinking, effective nursing process and empathy. The second research question tried to find out which obstacles prevent nurses in effective saturation of the patients´ needs. We found out that nurses are concerned about a lack of staff at wards and the excess of administrative load, which is related to a lack of time for patients. These three factors are also obstacles for nurses and prevent them from effective satisfying of the patients´ needs. The next obstacles in satisfying the needs are burnout, reluctance of nurses to work, excessive demands for nurses, poor workplace relationships, a lack of evaluation both in a form of praise and appreciation, and finance. We also found a lack of information, which is related to a lack of time, as nurses do not have a chance to collect all the information concerning the patient. As another possible obstacle, nurses also mentioned personal problems. The results of the thesis can be used as a small contribution to currently ongoing effort of the Czech Association of Nurses.

Efektivita používání pomůcek pro inkontinentní nemocné v následné péči / The effectiveness of aids for incontinence patients in aftercare

This thesis deals with the effectiveness of use of aids intended for incontinent ill people in subsequent care. The thesis is divided into a theoretical part and experience part. The experience part of the thesis has been implemented by means of qualitative research. The data have been obtained on the basis of two different half pen and paper interviews with patients/clients who are using incontinence aids prescribed by a doctor on a coupon or by a matron or a ward sister from the subsequent care department in hospitals with the state-funded organization and in hospitals with the joint-stock company organization. The result of the qualitative research showed, that both groups of respondents use mostly only plastic pants or nappies out of the whole assortment of aids. For patients/clients living at home the reason is that on the basis of anamnesis the general doctors prescribe the incontinence aids only in the most serious incontinence cases. At the subsequent care department the situation is caused by a big number of immobile fully incontinent patients/clients. The results show, that the quantity of absorb aids prescribed by general doctors is insufficient. It does not reach the quantity limits given in the documents by VZP (Public Health Insurance Company). Taking into consideration the low income of seniors, the respondents have to pay considerable amounts of money for incontinence aids which are necessary for them. There is an absolute lack of absorption aids in the hospital with the state funded organization. The incontinence aids are financed by three different ways. Patients/clients themselves have to get and pay the absorption aids at the Geriatrics department I and subsequent care department 2. The hospital provides them only with one-off mats and hygienic aids free of charge. The reason for this is a low limit of material. The respondents regard this system of financing negatively. It burdens patients/clients and nurses too. If patients/clients do not have the aids, the hospital has to lend the aids, make a list of debts and collects them back. On this account conflict situations often arise in families. At these departments there are reserved five social beds which enable patients/clients the prescription of aids by a doctor on a coupon likewise the patients who are ill and cure at home. At the stopgap department patients/clients pay a monthly fee; this includes the costs on incontinence aids too. At the subsequent care department 2A, B of the hospital with the join-stock company organization the aids are fully paid by the hospital. There is available a sufficient assortment and quantity of absorb and hygienic aids, which enable an individual attitude.

I am who I am because I am a sister: exploring sister relationships in middle adulthood

With the focus on development, research has often been limited to the time of life between the ages of birth to 18. Sibling research in adulthood has often been limited to examining siblings’ interactions in a particular context. Most of the research has examined siblings dealing with caregiving, family businesses, finances, or parental treatment. How siblings feel about their relationship, how the relationship has enhanced their lives, and what meaning individuals ascribe to that relationship through their lifetime has been understudied. This study focused on the meaning ascribed to a relationship between sisters by those in the relationship as well as the importance of sisterhood to the individual’s identity or perception of who they are because of the relationship. Participants responded to questions designed to gather information about what it means to them to be a sister in middle adulthood. The sisters indicated that the relationship held meaning for them though out their adult life. Parents were found to have influenced the relationship. In addition the sister relationship impacted the development of a sisters identity in multiple ways. For most sisters, they could not imagine who they would have become without the influence of their sisters.

Edukační proces pro sestry v pediatrické primární péči a rodiče dětí s epilepsií / The education process for nurses in pediatric primary practice and parents of children with epilepsy

The epilepsy occurs in the whole world without exception of age, race, geographical location and socio-economic structure. The incidence of epilepsy depends on age and influences each time the quality of life of the child and its next relatives. Our children and young people perceive, experience and evaluate their life usually from different points of view than adults including the parents. For this reason the main target of the nurse effort should be the education and involving the family into the process of healing. Also the preventive and educational care is connected with this. The target of thesis was to map if the education by the nurse concerning the regimen measures has influence on knowledge extent of the family of an epileptic child. The work focused, as for quantitative examination, on nurses in the surgeries of physicians for the children and young people (pediatrists) in South Bohemia. By the technology of the questionnaire the possibilities of provided primary and secondary care for children with epilepsy by the nurse and the scope of the co-operation of the nurse with the clubs of epileptics were mapped. Within the qualitative examination, based on the talks with parents of the children with epilepsy, the contribution of the knowledge of regimen measures handed on through the education by nurses in the primary pediatric care was examined. It follows from the results of the quantitative examination that the nurses in the outpatient departments of pediatrists do not devote attention to primary or secondary prevention of epilepsy and simultaneously they do not co-operate with the clubs of epileptics. It was impossible to verify all three hypotheses. It followed from the results of the quantitative examination that nurses in outpatient departments of physicians for children and young people do not sufficiently educate the parents of children with epilepsy. If the education by the nurse in the pediatric primary care has been performed, it was a contribution for parents each time. The results of the thesis lead us to reflection concerning the role of the nurse as educator and simultaneously as defender of the client- child in the outpatient departments of pediatrists. The results of the thesis: I would like to address the nurses in primary care, that they should assert themselves more, educate themselves and get involved into the care for children with epilepsy and hereby help to children to improve their childhood in the way necessary for their healthy growth and development.

Para realizar tal tarefa, foram abordados conceitos estéticos do barroco, de gêneros como a loa e o auto sacramental, além de aspectos críticos, intertextuais, assim como vivenciais, históricos e sociais, que envolvem sua obra geral como poeta, dramaturga e mulher de vanguarda, considerada até mesmo uma precursora do feminismo em termos americanos. A pesquisa enfoca ainda, os temas contidos nesse auto, ligados à mitologia asteca e à clássica; ao mistério do dogma católico, à tentativa de evangelização, às alegorias, metáforas e outros recursos do barroco, presentes nesse texto teatral. Realizou-se a análise desse texto com o apoio teórico filosófico, dramático e poético de Walter Benjamin e de Jacques Derrida, numa reflexão moderna e contemporânea, respectivamente; do poeta Octavio Paz e do dramaturgo Guillermo Schmidhuber, nos contextos biográficos, líricos e teatrais. ______________________________________________________________________________ ABSTRACT / This dissertation presents the reading and analysis of the auto sacramental El Divine Narcissus (1691) of Sor Juana Inés de la Cruz, a play of the spanish literature drawn up for Corpus Christi, a Catholic celebration of great importance in the evolution of the theater. For this task, the aesthetics of the Baroque, from genres such as the loa and auto sacramental were covered, as well as critical and intertextual aspects, just as experiential, historical and social aspects, involving her general work as a poet, playwright and avant-garde woman. She is considered a precursor of american feminism, in a retrospective look with philosophical, poetic and dramatic theoretical support. The research also looks through the optics linked to Aztec and Classic mythologies; to Catholic dogma mystery, to tentativ evangelization, to allegories, to metaphors and others resources of the Baroque. The theory of the philosophers Jacques Derrida and Walter Benjamin, as well as the studies of the poet Octavio Paz and of playwright Guillermo Schmidhuber were used as study support.

More recently, planned mentoring has become an important form of intervention with young people (Philip, 2003). Although mentoring can be an effective strategy for dealing with youth, the mentoring is only as good as the relationship that develops out of the process between mentors and mentees and the match that is made between the two parties. The number of mentor programmes that is running continues to grow yet the quality of these programmes remains unknown as this area lacks agreed upon sets of standards and / bench marks that could be used to determine the effectiveness of these programmes (Sipe, 1988 -1995). Victorian outlines: the crisis of individuation in nineteenth-century literature and art

This dissertation explores how in the mid-nineteenth-century, the outline, an element in art, became a symbolic form for the relationship between the individual and society. Artists like J.M.W. Turner and the Pre-Raphaelites either blurred or overstressed their outlines to highlight tensions in the ideal of the liberal individual. Writers like Alfred Tennyson, Charlotte Brontë, and George Eliot responded with their own experiments in characterization and plot. In attending to the conceptual analogies between the outline of figures in art, the outline of literary character, and the outline of the individual in society, I explore the gradual decline of the cultural narrative of personal development and the waning of the belief in the ability to individuate in an ever-burgeoning society. Because of the ease with which the outline’s symbolic meaning traverses the boundaries of painting, literature, and lived experience, it fosters a verbal-visual vocabulary for experiencing sociality to which the visually literate Victorians were intuitively attuned. Chapter one surveys the debate about outline, tracing a line from Blake to Ruskin. Focusing on outlines in Turner’s Snow-Storm and Millais’s Isabella, I demonstrate painting’s ability to formally articulate social critique. The following chapters explore some ways in which texts experiment with outline: diffusion, evasion, and superimposition. For instance, in In Memoriam A.H.H., rather than regain a distinguishable selfhood after loss, Tennyson gradually blends his own self with the selves that surround him, in line with Turner’s aerial perspective. In Villette, Brontë draws on her experience as a failed artist and her reading of Ruskin’s Modern Painters to fashion a uniquely feminine method of characterization, which I call negative space. She defines Lucy Snowe by filling in the space around her, leaving her to come out in relief. Finally, inspired by the photographic technique of double-exposure, Middlemarch develops characters by superimposing their identities upon one another, so that their outlines can no longer protect their illusion of singularity. Middlemarch shows how the most superficial social impulse of projecting our own preconceived notions onto our neighbors, can actually enrich rather than diminish their identities, and thus help to develop society as a whole. / 2027-03-31T00:00:00Z

In vitro chemically-induced DNA damage in cancer patients and healthy individuals. The effect of genotoxic compounds in cells from polyposis coli, colon cancer patients and healthy individuals.

In the present study DNA damage was measured in peripheral blood lymphocytes from polyposis coli and colorectal cancer patients, treated with different dietary and environmental compounds and compared with lymphocytes from healthy individuals. In addition, confounding factors such as age, gender, alcohol intake and smoking habits were taken into consideration. The assays used in this study included the Comet assay, the Micronucleus assay, the Micronucleus ¿ FISH assay and the sister chromatid exchange assay. The food mutagens, PhIP and IQ, as well as titanium dioxide nanoparticles (TiO2 NPs) induced a dose dependent increase in the DNA damage and chromosomal abnormalities in all tested groups regardless of confounding factors. Prior to experiments physicochemical characterisation of nanoparticles was conducted. In the presence of the flavonoids, quercetin and rutin that were acting in an antioxidant manner, the DNA damage resulting from the highest doses of food mutagens was significantly reduced. Thus, dietary supplementation with flavonoid-rich vegetables and fruits may prove very effective in protection against oxidative stress. The polyposis coli and colon cancer patients were more susceptible to food mutagens, PhIP and IQ, as well as TiO2 NPs, and in the majority of cases had a higher level of DNA damage in the Comet assay and higher cytogenetic damage in the Micronucleus assay. In the final project, twelve frequently encountered (NewGeneris) chemical compounds were evaluated to establish their damaging potential in lymphocytes and spermatozoa from healthy donors. The highest damage was produced by DNA reactive aldehydes, food mutagens and benzo[a]pyrene when assessed with the neutral and alkaline Comet assay with and without metabolic activation. / EU NewGeneris Programme and United Kingdom - India Education and Research Initiative (UKIERI)
