Anotation This thesis deals with autism spectrum disorders and social residential services, special homes. In the theoretical part, the basic information is about social services in Czech Republic. Another chapter is devoted to autism and its forms. The following chapter is devoted to workers in direct care and burnout syndrom, which are at risk. In the theoretical part I further develop the theme of problem behavior and subsequent restrictive measures that are working with people affected by autism sometimes unavoidable. In the practical part describes the day to day running of The Home Libčice, which is designed for stay of persons with autism associated with problem behavior. TITLE Efficacy of physiotherapeutic interventions in stroke patients with pusher and neglect syndrome. OBJECTIVES The aim of the thesis is to summarise current literature concerning two peculiar phenomena following stroke - pusher syndrome and neglect. Furthermore the work focuses on evaluation of efficacy of physiotherapeutic interventions. METHODS The thesis represents a critical review of a literature and it consists of three parts. The first one summarize issues concerning the pusher syndrome and neglect in general. In a special part you can find a detailed description of physiotherapeutic methods that are widely used in a rehabilitation treatment of these two neuronal symptoms. This work is concluded by evaluating results of effects of the therapeutic interventions that are discussed with respect to the future research. RESULTS Up till now there are no clinical trials concerning efficiacy of physiotherapeutic methods in a treatment of a pusher syndrome. The similar situation seems to be in case of a neglect syndrome as number of available clinical trials testing the efficiacy of physiotherapeutic techniques is very limited. The thesis deals with problems of burnout among special educators working in special primary schools with students with multiple disabilities. There are defined basic concepts in the theoretical part of the work - burn-out syndrome, its symptoms, causes and stages of development. Separate chapters are devoted to the diagnosis of burn-out syndrome and prevention. Furthermore, the concept of a special educator and a multiple disability is explained in the theoretical section and a special primary school is described here. The practical part focuses on quantitative research into burn-out syndrome in two selected groups - special educators in special primary schools and teachers in ordinary primary schools. The results of the investigation are then compared with each other.

Grundskollärares uppfattning om integrering av elever med Asperger syndrom

Abstract The purpose for this essay has been to illuminate some teachers opinions of integration of students with Aspergers syndrom in the compulsory school, then compare it with the statue documents and see similarities and differences. The studie has been of a quality character, based on quality intervjues to get the teachers understanding at a deeper level. The questions in the intervju are based on open questions with purpose not to control the person during the intervju. The most obvious result which was that it appered two perspective of integration of students with Aspergers syndrom. One in the short run and one in the long run. In the long run it was the integration of the individual into the society and in the short run integration in school. For the perspective in the long run, the opinions from the teachers in compulsory school is good corresponding to the statue documents, that the integration is necessary for our democratic and human values that our sociaty are based on. In the short run, during the years when the students are in school, it came out great difficulties to integrate children with Aspergers syndrom. To proceed an integration that works out for the students, the teachers took site from the individual and not from the society. If we do not look at integration from an individual base we are probably not going to succeed to integrate in society. Searchwords: Aspergers syndrom, neuropsychiatric funktional disorders, integration, inclusion, segregation. / Sammanfattning Syftet med min uppsats har varit att belysa några grundskollärares uppfattningar om integrering av elever med Asperger syndrom för att sedan jämföra med styrdokumenten och se om de stämmer överens. Studien har varit av kvalitativ karaktär, baserad på kvalitativa intervjuer för att få lärares uppfattningar på ett djupare plan. Intervjufrågorna baseras på öppna frågor med syfte att inte styra de intervjuade. Det mest framträdande resultatet var att det framkom två perspektiv på integrering av elever med Asperger syndrom. Ett långsiktigt och ett kortsiktigt perspektiv. Det långsiktiga är en integrering av individen i samhället och det kortsiktiga är en integrering i skolan. I det långsiktiga perspektivet så stämmer grundskollärarnas uppfattning väl överens med styrdokumenten, att integrering är nödvändig för att värna våra demokratiska och humanistiska värden som samhället bygger på. I det kortsiktiga perspektivet, elevens skolgång, framkom stora svårigheter vad det gäller att integrera elever med Asperger syndrom. Som utgångspunkt för att få en fungerande integrering av eleven, så utgick lärarna från individen och inte samhället. Om vi inte ser på integration utifrån individen så kommer vi troligtvis inte att lyckas med integrationen i samhället. Being in a relationship where the partner has Asperger's syndrome makes you indirectly affected by their disability as it affects not only the disabled but also the partner. The partner may be faced with a lack of understanding from their partner with Asperger's syndrome and from its surroundings, where the syndrome including causes, limits ability in social interaction and communication. The study is a qualitative study based on six asynchronous interviews conducted via email with partners who are in a relationship with a partner who has Asperger's syndrome. The aim of the study was to increase understanding and to highlight partners experience to be in a relationship where the other partner has Asperger's syndrome and how outside support from the environment is perceived. Results of the study showed that all six partners felt that everyday life was structured around the partner with Asperger's syndrome. There was a perceived feeling of loneliness because of the partner with Asperger's syndrome not caring about the partner and of being the one taking the greatest responsibility in everyday life for it to work. There was a feeling of stress, anxiety, lost confidence and frustration but there was also a strong desire and commitment to their partner with Asperger's syndrome. There was a strong love for the partner with Asperger's syndrome and a willingness to get the couple relationship to work. The results show that partners need and want support from the environment. It is important that they receive the support they need. The type of support needed varies but the important thing is that it is flexible, tailored and personalized for the couple or the individual partner's needs. The results indicate that there is a benefit for the couple relationship when the partner with Asperger's syndrome has a diagnosis. Both parties can thus get a better understanding of why certain things are the way they are and then work from there.

Modeling human Usher syndrome during Drosophila melanogaster development

Human Usher syndrome is a severe and congenital form of syndromic deafness that affects 1 person in 25,000 people in the world population. Normally the stereocilia, microvillar protrusions of the apical membrane of inner ear hair cells, are organized into coherent bundles. This precise organization is critical for mechanosensing, i.e. for hearing. Mutation in any of the five known Usher syndrome genes is sufficient to alter the precise organization of stereocilia, a condition that results in deafness. To date, however, the molecular mechanisms responsible for the splaying of stereocilia and genesis of the disease are not well understood. Here, I identified Drosophila melanogaster genes related to human Usher syndrome and characterized some of them (Cad99C, DSANS and crinkled) during Drosophila development, in the processes of microvilli morphogenesis in the follicular and wing imaginal disc epithelia. Cadherin Cad99C is a transmembrane protein with putative cell adhesion properties. Similar to its human ortholog Protocadherin 15, Drosophila Cad99C localizes to microvillar protrusions in the follicular epithelium. In this epithelium, Cad99C is required for the proper morphogenesis and organization of microvilli into bundles, similar to human Protocadherin 15. Further, overexpression of the full-length Cad99C or of a deleted version, devoid of the cytoplasmic region, promotes microvilli bundling. This finding suggests that Cad99C establishes adhesive interactions between microvilli via its extracellular region. Interestingly, morphological alteration of follicle cell microvilli associates with defective deposition of the vitelline membrane, an extracellular matrix that protects the embryo from osmotic stresses. These findings suggest that microvilli are normally required for the even deposition of the extracellular matrix. In order to test whether Cad99C is involved in microvilli morphogenesis and bundling in other tissues, I analyzed the function of Cad99C in a larval tissue, the wing imaginal disc. Cad99C overexpression, but not Cad99C removal, is sufficient to alter microvilli morphology and organization in the columnar epithelium of the wing imaginal disc. Likely, other molecules can compensate for Cad99C loss of function in this tissue. To possibly get some insights on the molecular function of other Usher syndrome proteins, I analyzed the function of Drosophila SANS and crinkled in the follicular epithelium, where both these genes are expressed. crinkled is the ortholog of myosinVIIa, that encodes a motor protein of the actin cytoskeleton. DSANS is related to human SANS and encodes a cytoplasmic protein of unknown function. It has been puzzling how removal of SANS, a cytoplasmic protein, could impair adhesion and bundling of stereocilia. To study the function of DSANS, I generated null mutant flies and observed that, in the absence of DSANS, delivery of Cad99C to microvilli is impaired. Cad99C localization is however unperturbed in crinkled mutant follicle cells. By immunostaining, DSANS immunoreactivity was detected diffusively in the cytoplasm and in dot-like structures, possibly corresponding to vesicles. In conclusion, DSANS is a cytoplasmic protein that is required for the efficient delivery of Cad99C to microvilli protrusions. Taken together, the analysis that I here performed of Drosophila Usher syndrome related genes indicates two novel molecular mechanisms of function for the corresponding human Usher syndrome proteins. First, human Protocadherin 15, like Drosophila Cad99C, could be involved in establishing adhesive interactions between microvilli protrusions of the inner ear (stereocilia). Removal of Protocadherin 15 would then cause splaying of stereocilia due to lack of inter-stereocilia adhesive links. Second, the analysis here performed suggests that SANS is involved in the efficient delivery of Protocadherin 15 to stereocilia. Mutations in SANS would then lead to splaying of stereocilia and deafness due to poor localization of Protocadherin 15 to stereocilia.

Komparace ošetřovatelské péče o apalicky nemocné v domácí a ústavní péči z pohledu pečujícího / Comparison of nursing care about apallic patients in domestic and institutional care from carer?s viewpoint

As a result of urgent medicine acceleration and neurosurgery care improvement individuals that had minimum chance of life saving in the past survive brain insults. Increasing occurrence of people with apallic syndrome is a consequence of this progress. This thesis compares domestic and institutional care of apallic people. It examines whether the care quality and complexity are comparable. As the domestic care is more economical for the government it has to be supported in the times of insufficient funds for health care. Quantitative research technique of non-standardized interview was used for data collection. The research sample consisted of six healthcare workers and six family members caring for apallic patients. One respondent had a son in institutional care, however she took him temporarily to domestic care. Questions for healthcare staff and for caring family members were prepared separately. The results show that apallic patients in domestic care achieve substantially better results. The physical load is comparable, mental load is higher for family members. The basal stimulation, which is extremely important for apallic patients, is purposefully applied in institutional care only, as it is provided by trained staff there. The results of this work were presented at the 4th National Student Conference organized by the Faculty of Healthcare Studies of the Pardubice University. Výkon profese v intenzivní péči a její vliv na životní styl sester / The profession in intensive care and its impact on the lifestyle of nurses

The diploma thesis has been focused on the issue of nurses working within intensive care and on the impact of performing a job in this branch on their lifestyles. The jobs of nurses working within the intensive branches clearly belong among the demanding ones. A nurse often becomes a witness of dying patients while there is hardly any chance of curability and stabilization of their conditions. All of that and many other factors affect her personal and professional life, as well as her current lifestyle. To perform her job at as high level as possible, a nurse must be motivated and directed by management of the health centre, she must be educated enough in her branch and the job should be a hobby for her, where she must feel certain and safe and the job should satisfy her. This diploma thesis has defined three following intentions. The first one highlighted mapping of a lifestyle of nurses working within intensive care. The second one was determination of psychical load of nurses working within intensive care. The third one included comparison of lifestyle of nurses working within intensive care and lifestyle of nurses working at standard wards. There were 4 hypotheses defined for the investigation survey. The first one analyzed if a job within intensive care affects diet of the nurses. This hypothesis proved true. The second one, if nurses working within intensive care consume more alcoholic drinks than the sisters working at standard wards do. This hypothesis did not prove true. The third one examined whether nurses working within intensive care smoke tobacco products more than the sisters working at standard wards do. This hypothesis proved true. And the fourth and the last hypothesis analyzed if a job within intensive care is psychically demanding for the nurses. This hypothesis proved true.
