Background: Although the negative impact of sleep apnea on the clinical course of acute ischemic stroke (AIS) is well known, data regarding non-invasive ventilation in acute patients are scarce. Several studies have shown its tolerability and safety, yet no controlled randomized sequential phase studies exist that aim to establish the efficacy of early non-invasive ventilation in AIS patients.
Methods/design: We decided to examine our hypothesis that early non-invasive ventilation with auto-titrating bilevel positive airway pressure (auto-BPAP) positively affects short-term clinical outcomes in AIS patients. We perform a multicenter, prospective, randomized, controlled, third rater- blinded, parallel-group trial. Patients with AIS with proximal arterial obstruction and clinically suspected sleep apnea will be randomized to standard stroke care alone or standard stroke care plus auto-BPAP. Auto-BPAP will be initiated within 24 hours of stroke onset and performed for a maximum of 48 hours during diurnal and nocturnal sleep. Patients will undergo unattended cardiorespiratory polygraphy between days three and five to assess sleep apnea. Our primary endpoint will be any early neurological improvement on the NIHSS at 72 hours from randomization. Safety, tolerability, short-term and three-months functional outcomes will be assessed as secondary endpoints by un-blinded and blinded observers respectively.
Discussion: We expect that this study will advance our understanding of how early treatment with non-invasive ventilation can counterbalance, or possibly reverse, the deleterious effects of sleep apnea in the acute phase of ischemic stroke. The study will provide preliminary data to power a subsequent phase III study.

Is CPAP a feasible treatment modality in a rural district hospital for neonates with respiratory distress syndrome?Hendriks, Hans Jurgen 12 1900 (has links)
Thesis (MMed) -- Stellenbosch University, 2010. / ENGLISH ABSTRACT: Introduction: Limited facilities exist at rural hospitals for the management of newborn infants with respiratory distress syndrome (RDS). Furthermore, the secondary and tertiary hospitals are under severe strain to accept all the referrals from rural hospitals. Many of these infants require intubation and ventilation with a resuscitation bag which must be sustained for hours until the transport team arrives. Not only is lung damage inflicted by the prolonged ventilation, but transferring the infant by helicopter and ambulance is expensive.
CPAP (continuous positive airway pressure), a non-invasive form of ventilatory support, has been used successfully at regional (Level 2) and tertiary (Level 3) neonatal units, to manage infants with RDS. It is cost-effective for infants with mild to moderate grades of RDS to be managed at the rural hospital instead of being transferred to the regional secondary or tertiary hospital. CPAP was introduced to Ceres Hospital, a rural Level 1 hospital, in February 2008 for the management of infants with RDS.
Aim: To determine the impact of CPAP on the management of infants with RDS in a rural level 1 hospital and whether it can reduce the number of referrals to regional hospitals.
Study setting: Nursery at Ceres District Hospital, Cape Winelands District, Western Cape.
Study design: Prospective cohort analytical study with an historic control group (HCG).
Patients and Methods: The study group (SG) comprised all neonates with respiratory distress born between 27/02/2008 and 26/02/2010. The infants were initially resuscitated with a Neopuff® machine in labour-ward and CPAP was commenced for those with RDS. The survival and referral rates of the SG were compared to an historic control group (HCG) of infants born between 1/2/2006 to 31/01/2008 at Ceres Hospital.
Results: During the 2 years of the study, 51 neonates received CPAP (34 <1800g, 17>1800g). Twenty (83%) of the SG infants between 1000g and 1800g and 23 (68%) of the infants between 500g and 1800g survived. Those <1800g that failed CPAP, had either a severe grade of RDS which required intubation and ventilation or were <1000g. Seventeen (33%) of the infants that received CPAP, were in the >1800g group. Thirteen (76%) of these infants were successfully treated with CPAP only. The four infants that failed CPAP suffered from congenital abnormalities and would not have benefited from CPAP. There was no statistically significant difference in the survival between the SG and HCG (80%) (p=0.5490) but the number of referrals decreased significantly from 21% in the HCG to 7% in the SG (p=0.0003). No complications related to CPAP treatment, such as pneumothorax, were noted. The nursing and medical staff quickly became proficient and confident in applying CPAP and were committed to the project.
Conclusion: CPAP can be safely and successfully practised in infants with mild to moderate RDS in a rural Level 1 hospital. The survival rate stayed the same as the HCG, even though a higher risk infants were treated in the SG. The transfers were significantly reduced from 21% to 7%. Le pédalage « excentrique », caractérisé par une meilleure efficience, pourrait permettre à ces patients de se réentrainer plus efficacement. Toutefois, les modalités de prescription et les effets du pédalage excentrique méritent d’être étudiés.Chez le sujet sain, nos résultats ont montré un retard de réactivation parasympathique au décours du pédalage excentrique par rapport au concentrique réalisés à même puissance mécanique, avec une influence de la fréquence de pédalage sur les réponses cardio-respiratoires et autonomes. À même puissance métabolique, le pédalage excentrique se caractérise par un stress cardio-vasculaire et respiratoire plus important que lors du pédalage concentrique, et des réponses neuro-végétatives en faveur d’une activité sympathique plus élevée. La fonction vasculaire est altérée à l’issue du pédalage excentrique. Chez le patient atteint de BPCO, comme chez le sujet sain, le pédalage excentrique a un effet tachypnéisant.Nous pensons que la tension musculaire accrue nécessaire en pédalage excentrique à l’atteinte d’une puissance métabolique équivalente à l’exercice concentrique serait à l’origine d’une plus grande activité sympathique et d’une contrainte hémodynamique périphérique, expliquant l’augmentation de la contrainte cardio-vasculaire en pédalage excentrique. / New exercise modalities are required for patients severely limited during exercise such as chronic obstructive pulmonary disease patients (COPD). Eccentric “cycling”, characterized by better efficiency, could allow COPD patients to train more effectively. Nonetheless, prescription modalities and effects of eccentric cycling deserve to be more studied.In healthy subjects, our results showed a delayed parasympathetic reactivation after eccentric compared to concentric cycling matched for mechanical power, together with an influence of pedalling frequency on autonomic and cardiorespiratory responses. At similar metabolic power, eccentric cycling is characterized by a greater cardiovascular and respiratory stress than during concentric cycling, and altered autonomic nervous systems responses in favour to greater sympathetic activity. Vascular function is altered after eccentric cycling. In COPD patients, as in healthy subjects, eccentric cycling impose tachypneic breathing pattern.We believe that the higher muscle tension during eccentric cycling necessary to reach metabolic power similar to concentric leads to greater sympathetic activity and peripheral hemodynamic constraint, likely explaining the increased constraints on cardiovascular system during eccentric cycling.

Previous issue date: 2007-06-29 / Stretching by the global postural re-education method (GPR) has been widely used as a
physical therapeutic procedure for postural alterations, but its action on the cardio-respiratory
system has been little documented. However, scientific evidence is necessary to validate it as
an alternative treatment. Within this context, three studies were carried out with the objective
of verifying the effect of stretching the respiratory muscle chain in the open-arm, open hip
joint angle posture by the GPR method on cardio-respiratory responses. Twenty young
(22.7±2.5 years old), healthy, sedentary (VO2max: 30.2±4.3 mL.kg.min-1) men with BMI =
25.3±1.5 kg/m2 were studied, divided at random into two groups of ten: the control group (CG)
that did not take part in the stretching and the group submitted to treatment (GPR-G). The
intervention consisted of two weekly stretching sessions, each of 30 min, for eight weeks,
giving a total of 16 sessions. In the first study, the maximal respiratory pressure and the
respiratory coefficient were assessed by way of a manovacuometer and thoracoabdominal
cirtometry, respectively. Only the GPR-G presented greater values for inspiratory (IPmax)
and expiratory (EPmax) maximal pressures and for axillary, xyphoid and abdominal
cirtometry after the intervention period. The results showed that the method caused a
beneficial effect on respiratory muscle force and on thoracoabdominal mobility, contributing to
an improvement in respiratory mechanics. In the second study, pulmonary function was
assessed by spirometry. The manoeuvres of slow vital capacity (SVC), forced vital capacity
(FVC) and maximal voluntary ventilation (MVV) were carried out. The results showed there
was a significant increase in the values for SVC, inspiratory capacity (IC), FVC, forced
expiratory volume in the first second (FEV1) and MVV for the GPR-G after the training period,
providing evidence that the proposed stretching was efficient in promoting an increase in
pulmonary capacity and volume. In the third study, the effect of stretching on the autonomic
modulation of the heart rate (HR) and on the ventilatory and metabolic variables was
assessed at rest and during the cardio-pulmonary exercise test respectively. The ventilatory
and metabolic variables were captured using an automatic metabolic system and the HR
using a one-channel heart monitor. The exercise test was carried out using a cycle
ergometer, with 20 to 25 W/min increments up to physical exhaustion. Intervention did not
change the variables studied either at rest or during the cardio-pulmonary exercise test, for
either of the groups. These results showed that the respiratory muscle chain stretching
exercise did not promote any adaptations with respect to HR neither at rest and the
ventilatory and metabolic variables during the exercise, which could be attributed to the noninvolvement
of the major muscle groups in the type of protocol used. Finally, the results
obtained in the three studies gave evidence that stretching the respiratory muscle chain by
the GPR method was efficient in promoting an increase in respiratory muscle strength,
thoracoabdominal mobility and pulmonary function, reflecting in an increase in respiratory
mechanics, this fact being attributable to the specificity of the training. In this way, the findings
of the present study, reinforce the importance of the GPR method in the maintenance of the
functional capacity of the respiratory chain muscles, suggesting that it could be used as an
alternative treatment for dysfunctions of the respiratory mechanics. / O alongamento pelo método de reeducação postural global (RPG) tem sido muito utilizado
como conduta fisioterapêutica em alterações posturais, porém, sua ação sobre o sistema
cardiorrespiratório é pouco documentada. No entanto, as evidências científicas são
necessárias para sua validação como alternativa de tratamento. Dentro desse contexto,
foram realizados três estudos com o objetivo de verificar o efeito do alongamento da cadeia
muscular respiratória na postura rã no chão com os braços abertos do método de RPG sobre
as respostas cardiorrespiratórias. Foram estudados 20 homens jovens (22,7±2,5 anos),
saudáveis, sedentários (VO2max: 30,2±4,3 mL.kg.min-1) e com IMC = 25,3±1,5 kg/m2,
divididos aleatoriamente em dois grupos de dez: grupo controle (G-C) que não participou do
alongamento e grupo submetido ao treinamento (G-RPG). A intervenção consistiu em duas
sessões semanais de alongamento de 30 min cada, durante oito semanas, totalizando 16
sessões. No primeiro estudo, foram avaliadas as pressões respiratórias máximas e o
coeficiente respiratório, por intermédio da monovacuometria e da cirtometria
toracoabdominal, respectivamente. Somente o G-RPG apresentou maiores valores das
pressões inspiratórias (PImáx) e expiratórias (PEmáx) máximas e da cirtometria axilar,
xifoideana e abdominal após o período de intervenção. Os resultados mostraram que o
método teve um efeito benéfico na força muscular respiratória e na mobilidade
toracoabdominal, contribuindo para a melhora da mecânica respiratória. No segundo estudo,
foi avaliada a função pulmonar por intermédio da espirometria. Foram realizadas as
manobras de capacidade vital lenta (CVL), capacidade vital forçada (CVF) e ventilação
voluntária máxima (VVM). Os resultados mostraram que houve aumento significativo dos
valores da CVL, capacidade inspiratória (CI), CVF, volume expiratório forçado no primeiro
segundo (VEF1) e VVM do G-RPG após o período de treinamento, evidenciando que o
alongamento proposto foi eficiente para promover aumento dos volumes e capacidades
pulmonares. No terceiro estudo, foi avaliado o efeito do alongamento nas variáveis
ventilatórias e metabólicas durante o teste de exercício cardiopulmonar e na modulação
autonômica da freqüência cardíaca (FC) em repouso. As variáveis ventilatórias e
metabólicas foram captadas a partir de um sistema metabólico automático, e a FC a partir de
um monitor cardíaco de um canal. O teste de exercício foi realizado em cicloergômetro, com
incrementos de 20 a 25 W/min até a exaustão física. A intervenção não modificou as
variáveis estudadas em nenhum dos grupos. Esses resultados mostram que o exercício de
alongamento da cadeia muscular respiratória não promoveu adaptações na FC e em sua
variabilidade em repouso e nas variáveis ventilatórias e metabólicas durante o exercício, o
que pode ser atribuído ao não envolvimento de grandes grupos musculares no tipo de
protocolo utilizado. Finalizando, os resultados obtidos nos três estudos evidenciaram que o
alongamento da cadeia muscular respiratória pelo método de RPG mostrou-se eficiente para
promover aumento da força muscular respiratória, mobilidade toracoabdominal e função
pulmonar, refletindo na melhora da mecânica respiratória, sendo que tal fato pode ser
atribuído à especificidade do treinamento. Desse modo, nossos achados reforçam a
importância do método de RPG para manutenção da capacidade funcional dos músculos da
cadeia respiratória, sugerindo que a postura utilizada pode ser coadjuvante no tratamento
das disfunções da mecânica respiratória.

Previous issue date: 2015-04-16 / Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) / Interval training (IT), combined with pharmacological therapy, has been strongly recommended for the management of coronary artery disease (CAD) and
control of cardiovascular risk factors. However, the best protocol to be used it is still unclear. In this regard, the IT protocols based on ventilatory anaerobic
threshold (VAT) can be an effective and safe strategy to prescribe the intensity of the exercise training as well as to promote improvement of the aerobic
functional capacity, metabolic profile, cardiac autonomic modulation and to reduce inflammatory markers. Moreover, the adaptive responses to IT may
differ between individuals. Part of this biological variability may be explained by the genetic polymorphisms involved in the synthesis of structural proteins or on the circulation enzymes. Based on this context, this thesis involved three studies. The first and second studies were conducted to investigate if the IT,
based on intensities between 70 and 110% of the workload obtained in the VAT, could be effective to improve functional aerobic capacity and metabolic
profile. Moreover, it was investigated if there was a relationship between the improvement in cardiac autonomic modulation with the reduction of the Creactive protein (CRP) in patients with CAD and/or cardiovascular risk factors. The results of the first study showed that the IT program increased aerobic
functional capacity and reduced body mass and body mass index. The second study found that after the IT program, there was an increase of the
parasympathetic autonomic modulation that was associated with a reduction in CRP levels. Given these findings, the third study was designed to investigate
the improvement of the functional aerobic capacity and changes in lipid profile induced by IT, in relation to the apolipoprotein B (APOB) and the angiotensinconverting
enzyme (ACE) genes polymorphisms. In this study, it was observed that the increase of the functional aerobic capacity was associated with the
presence of the ACE I allele, whereas patients with polymorphism -7673G>A, in the APOB gene, presented increase of the low-density lipoprotein (LDL) levels over time. Considering the findings from these studies, the IT protocol promoted improvement of cardiac autonomic modulation, increase of aerobic functional capacity, reduction of inflammation e improvement of metabolic profile. These benefits on the cardiovascular and metabolic systems contribute to control of cardiovascular risk factors and primary and secondary prevention of CAD. Moreover, this IT protocol provides new possibilities on cardiac rehabilitation field, with respect to exercise training prescription at the VAT level. / O treinamento físico intervalado (TFI) tem sido fortemente recomendado, combinado com a terapia farmacológica, para o manejo da doença arterial
coronariana (DAC) e controle dos fatores de risco cardiovascular. Entretanto, ainda há controvérsias com relação ao melhor protocolo a ser utilizado. Neste
sentido, os protocolos de TFI preconizados a partir do limiar de anaerobiose ventilatório (LAV) pode ser uma estratégia efetiva e segura para determinar a
intensidade de treinamento físico, além de ser capaz de promover melhora da capacidade funcional aeróbia, do perfil metabólico, da modulação autonômica cardíaca e redução de marcadores inflamatórios. Além disso, as respostas adaptativas promovidas pelo TFI podem diferir entre os indivíduos. Parte dessa variabilidade biológica pode ser explicada pelos polimorfismos genéticos envolvidos na síntese de proteínas estruturais e enzimas circulantes. Baseado neste contexto, esta tese envolveu três estudos. O primeiro e o segundo estudo
foram conduzidos para investigar se o TFI, baseado em intensidades entre 70 a 110% da potência obtida no LAV, pode ser uma estratégia que contribua
efetivamente para o ganho da capacidade funcional aeróbia e melhora do perfil metabólico; e se há uma relação entre a melhora da modulação autonômica da
frequência com a redução da proteína C-reativa (PCR-us) em pacientes com DAC e/ou fatores de risco cardiovascular. Os resultados do primeiro estudo
mostram que, o programa de TFI promoveu aumento da capacidade funcional aeróbia e redução da massa corporal e do índice de massa corporal. Já o
segundo estudo, verificou que aumento da modulação autonômica parassimpática foi associado com a redução dos níveis de PCR-us após o programa de TFI. Diante desses achados, o terceiro estudo foi dedicado no sentido de investigar a melhora da capacidade funcional aeróbia e as mudanças do perfil lipídico, frente ao TFI, em relação aos polimorfismos do gene da apolipoproteína B (APOB) e da enzima conversora de angiotensina (ECA). Neste estudo, foi observado que o aumento da capacidade funcional
aeróbia foi associado com a presença do alelo I do gene da ECA, enquanto que o aumento dos níveis da lipoproteína de baixa densidade (LDL), ao longo
do tempo, foi evidenciado nos pacientes com o polimorfismo -7673G>A do gene da APOB. Considerando os achados apresentados nos três estudos, o TFI proposto promoveu melhora da modulação autonômica cardíaca, aumento
da capacidade funcional aeróbia, redução da inflamação e melhora do perfil metabólico. Tais benefícios sobre o sistema cardiovascular e metabólico
contribuem efetivamente o controle dos fatores de risco cardiovascular, uma vez que a atenção primária à saúde tem se tornado uma estratégia efetiva para
o manejo da DAC. Além disso, o protocolo de TFI vislumbra novas possibilidades para a reabilitação cardíaca no que concerne a prescrição do
TFI individualizado.

Análise da existência de máxima fase estável de lactato nos exercícios resistidos leg press 45º e supino reto / Analysis of the existance of a maximal lactate steady state on resistance exercises leg press 45° and bench pressRodrigo Ferro Magosso 26 July 2010 (has links)
O objetivo deste estudo foi verificar se existe máxima fase estável de lactato (MFEL) nos exercícios LP e SR e analisar o comportamento de variáveis ventilatórias ventilação (Ve) e consumo de oxigênio (VO2), bem como lactacidemia, freqüência cardíaca (FC) e percepção subjetiva de esforço (PSE) na intensidade da MFEL e comparar as respostas entre os exercícios. Foram avaliados 12 homens jovens (20-33 anos) saudáveis e com experiência mínima de 6 meses em treinamento resistido, que passaram por teste de 1 repetição máxima (1RM), teste crescente para determinação do limiar anaeróbio (AT) e mais três sessões para a determinação da MFEL. Todas as sessões foram realizadas no mesmo horário do dia, separadas por 48 a 72 horas de intervalo. O valor de 1RM no LP foi de 298,0 ± 36,6 kg enquanto que no SR foi 93,7 ± 21,8 kg. A intensidade máxima atingida no teste crescente no LP foi 70,00 ± 12,25% de 1RM e no SR 50,00 ± 8,16% de 1RM, sendo esta diferença estatisticamente significativa (p0,05). A lactacidemia final também foi significativamente maior (p0,05) no LP comparada ao SR (8,36 ± 2,34 vs. 5,43 ± 1,12 mmol/L, respectivamente). O AT, expresso em percentual de 1RM, no LP foi de 27,9 ± 3,7% e no SR de 24,1 ± 4,4% de 1RM, porém esta diferença não foi estatisticamente significante. A lactacidemia no momento do teste crescente em que o AT era atingido também não apresentou diferença significativa, sendo 2,29 ± 0,58 mmol/L no LP e 2,52 ± 0,65 no SR. A intensidade da MFEL, no LP foi de 29,17 ± 7,02% de 1RM, enquanto que no SR a intensidade da MFEL foi de 21,67 ± 4,44% de 1RM, sendo esta diferença estatisticamente significativa (p0,05). No LP não houve diferença significativa entre as intensidades do AT e da MFEL (27,9 ± 3,7% e 29,2 ± 7,0% de 1RM, respectivamente). No SR intensidade do AT foi significativamente maior (p0,05) que a intensidade da MFEL (24,1 ± 3,4% e 21,7 ± 4,4% de 1RM, respectivamente). Durante a realização de exercício na MFEL, a ocorreu aumento de Ve, VO2, FC e PSE seguidos de estabilização, sem diferenças entre os exercícios. Estes resultados demonstram que é possível verificar MFEL nos exercícios LP e SR, e que no LP a intensidade é maior. As diferenças encontradas no teste crescente e na intensidade da MFEL são provavelmente causadas pelas características dos exercícios. / The purpose of the study was to verify if there is a maximal lactate steady state (MLSS) for the Leg Press (LP) and Bench Press (BP) exercises and to analyse ventilatory responses ventilation (Ve) and oxygen uptake (VO2), heart rate (HR), blood lactate concentration (BLC) and ratings of perceived exertion (RPE) to those exercises performed on MLSS intensity and to compare those responses for the exercises. 12 young healthy men with a minimal experience of 6 months of resistance training volunteered for the study. Volunteers underwent a 1 repetition maximum test (1RM), na incremental test to determine anaerobic threshold (AT) and three more sessions to determine MLSS. Session were performed on the same time of Day and separeted by a 48-72h interval. 1RM values for LP and BP were 298,0 ± 36,6kg and 93,7 ± 21,8 kg, respectively. The maximal intensity in the incremental test was 70 ± 12,25% of 1RM for LP and 50,00 ± 8,16% of 1RM for BP, which was a significant difference (p0,05). Post-test blood lactate concentration (BLS) was also significantly higher for LP (8,36 ± 2,34 vs. 5,43 ± 1,12 mmol/L, respectively) (p0,05). AT intensity and was not significantly different for LP and BP (27,9 ± 3,7% vs. 24,1 ± 3,6, respectively). BLC on AT was not significantly different for LP (2,29 ± 0,58 mmol/L) and BP (2,52 ± 0,65). MLSS intensity on LP (29,17 ± 7,02% of 1RM) was significantly higher (p0,05) compared to MFEL on BP (21,67 ± 4,44% of 1RM). When compared to AT, MLSS intensity for LP was not significantly different, but for BP it was significantly lower. During exercise on MLSS, oxygen uptake and ventilation, HR and RPE rose significantly compares to rest and then stabilized. There were no differences between exercises. These results show that it is possible to identify a MLSS on the LP and BP exercises, and that for LP this intensity is higher. The differences found between exercises can be atributted to the intrinsic caractheristics of each exercise.

Previous issue date: 2011-03-29 / Introdução: A força muscular respiratória é geralmente estimada a partir das pressões geradas nas vias aéreas pela contração dos músculos respiratórios. Dentre os testes disponíveis para este fim, o mais amplamente utilizado na prática clínica é a avaliação das pressões respiratórias estáticas máximas (PREM) ao nível da boca: pressão inspiratória máxima e pressão expiratória máxima. Atualmente, recomenda-se a utilização de transdutores de pressão em substituição aos manovacuômetros aneróides durante as medidas das PREM. Entretanto, os parâmetros de definição de pressão máxima a serem empregados a partir da utilização deste tipo de equipamento ainda são motivo de debate. Uma das possíveis vantagens relacionadas à utilização dos transdutores de pressão é a visualização do próprio esforço realizado por parte dos indivíduos. Porém, não foram encontrados na literatura consultada estudos que avaliassem objetivamente os efeitos desta estratégia sobre os valores pressóricos mensurados. Objetivos: Investigar diferentes parâmetros de definição de pressão máxima descritos na literatura para as medidas das PREM, bem como avaliar a influência do estímulo visual sobre o desempenho dos voluntários durante estas medidas. Materiais e métodos: Estudo transversal, do qual participaram 49 sujeitos saudáveis, de ambos os sexos, com média de idade de 23,08 ± 2,5 anos, índice de massa corporal que não caracterizasse desnutrição ou obesidade, e função ventilatória dentro dos limites da normalidade. Os voluntários foram recrutados na comunidade acadêmica da Universidade Federal de Juiz de Fora, e todos assinaram o termo de consentimento livre e esclarecido previamente às avaliações. Para as medidas das PREM, foi utilizado um transdutor de pressão da marca EMG System do Brasil Ltda. Os testes foram realizados em duas ocasiões, com intervalo mínimo de duas semanas, com e sem a utilização do estímulo visual. Os parâmetros de definição de pressão máxima avaliados foram estimados a partir de algoritmo matemático desenvolvido para a pesquisa, a saber: pressão de pico, pressão de platô, pressão média máxima e pressão segundo a área. Além disso, foram avaliados ainda o tempo para atingir o valor de pico e a área normalizada. Para análise estatística, foram empregados a média e o desvio padrão para caracterização da amostra, além dos testes de
Friedman, Wilcoxon, e da correlação de Spearman para análise das variáveis. Resultados: Foram encontradas diferenças significativas entre todos os parâmetros de definição de pressão máxima estudados (p < 0,05). Com relação à utilização do estímulo visual, este esteve relacionado a maiores valores pressóricos apenas no primeiro dia de avaliação (p < 0,05), indicando neste caso o efeito do aprendizado. Conclusão: Foram evidenciadas diferenças significativas entre todos os parâmetros de definição de pressão máxima estudados. Além disso, a utilização do estímulo visual durante os testes pareceu não resultar em melhoria do desempenho dos sujeitos avaliados. / Introduction: Respiratory muscle strength is usually estimated from the pressures generated on the airways by the respiratory muscles contraction. Among the available tests, the most widely used in clinical practice is the assessment of the maximal static respiratory pressures (MSRP) generated at the mouth: maximum static inspiratory pressure and maximum static expiratory pressure. Nowadays, the use of pressure transducers during the measurements of the MSRP, in substitution to the aneroid manometers, is recommended. However, the definition parameters of maximum pressure that should be applied when using this device are still to be defined. One of the possible advantages related to the use of pressure transducers is the real time visualization, by the subjects, of their own effort. Nevertheless, studies that objectively evaluated the effects of this strategy upon the pressure values obtained were not found in the literature. Objectives: To investigate different definition parameters of maximum pressure described in the literature for the MSRP measurements, as well as to assess the influence of the visual feedback upon the performance of the volunteers. Methods: Transversal study, in which 49 healthy subjects from both sexs took part, mean age 23,08 ± 2,5 years, body mass index that did not characterize obesity or malnutrition, and ventilatory function within normal limits. The volunteers were recruited from the academic community of the Federal University of Juiz de Fora, and all gave their informed written consent prior to the measurements. The assessments of the MSRP were made using an EMG System do Brasil Ltda. pressure transducer. The tests were conducted in two occasions, with a two-week minimum interval, with and without visual feedback. The definition parameters of maximum pressure were estimated from a mathematical algorithm developed to the research and were, namely, peak pressure, plateau pressure, mean pressure and pressure according to the area. It was also evaluated the time to reach the peak pressure and the normalized area. For statistical analysis, it was used the mean and the standard deviation for sample characterization, and the Friedman and Wilcoxon tests, as well as the Spearman correlation, for the data analysis. Results: There were found significant differences among all the definition parameters of maximum pressure studied. Regarding the tests undertaken with visual feedback,
they were related to higher pressure values only in the first day of assessments (p > 0,05), indicating, in this case, the learning effect. Conclusion: There were found significant differences among all the definition parameters of maximum pressure studied. In addition, the use of visual feedback during the testes seems to be unable to improve the volunteers’ performance.

Conselho Nacional de Desenvolvimento Científico e Tecnológico / The essential oil (EOs) extracted from plants Hesperozygis ringens and Lippia alba possess
anesthetic and sedative properties and is an alternative to traditional anesthetics used in
aquaculture for ease of handling and/or reduce stress. In this sense, the study aimed to
investigate the effects of these EOs on the physiology of fish, through physiological, biochemical
and endocrine indicators. In the article 1 was determined (a) the anesthetic activity of the EOs of
H. ringens (EOHR) and L. alba (EOLA) and (b) its effects on silver catfish (Rhamdia quelen) after
induction and recovery from anesthesia. Fish were subjected to one of the following treatments
for each EO: basal group, control or anesthetized (150, 300 or 450 uL L-1 EO), evaluating the
ventilatory rate (VR) during the induction period and thereafter transferred to anesthetics-free
tanks for recovery from anesthesia. At 0, 15, 30, 60 and 240 min of recovery, samples of plasma
and gills were collected to measure metabolic indicators and ionregulatory enzymes, respectively.
In the article 2, the effects of prolonged exposure to low EOHR concentrations were studied on
silver catfish. After 6 h of exposure to 0 (control), 30 or 50 uL L-1 EOHR added to water, it was
analyzed: VR, metabolic indicators of stress in plasma, enzyme activity in liver, and expression of
pituitary hormones (growth hormone - GH, prolactin - PRL and somatolactina - SL). In the
manuscript, (a) evaluated the effectiveness of anesthesia EOLA on gilthead sea bream (Sparus
aurata) and (b) we investigated the effects of 35 uL L-1 EOLA and 2-phenoxyethanol (2-PHE) on
the stress response in gilthead sea bream undergoing persecution. After 4 h of exposure, the
plasma was sampled (for the determination of cortisol, metabolites and osmolality), brain and
pituitary (to evaluate the expression of endocrine indicators). In the article 1, anesthesia with EOs
caused changes in some parameters measured in silver catfish, but did not prevent the
restoration of most of the indicators assessed after 240 min of recovery. In the article 2, 50 uL L-1
EOHR led to an increase of glucose, lactate, protein and osmolality, as well as an increase in
metabolic enzyme activity and reduced expression of GH and SL. In the manuscript, gilthead sea
bream exposed to EOLA, stressed or not, exhibited higher levels of cortisol, glucose, lactate and
osmolality. EOLA exposure added to the stress reduced the expression levels of CRH-BP
(corticotropin releasing hormone bound to protein). PRL expression was reduced in the stressed
control group and after exposure to EOLA and 2-PHE in fish not stressed. Higher expression of
pro-opiomelanocortin (POMC) "a" and "b" were observed in fish stressed and exposed to EOLA
and 2-PHE, respectively. We conclude that: (1) the EOLA is more efficient for silver catfish that
EOHR in anesthesia concentrations; (2) for sedating the fish, it is recommended 30 uL EOHR L-1
(or less); (3) the EOLA was effective as an anesthetic for gilthead sea bream at 100-300 uL L-1,
but for 4 h exposure, the 2-PHE was more effective in preventing the stress response. / Os óleos essenciais (OEs) extraídos das plantas Hesperozygis ringens e Lippia alba possuem
propriedades anestésica e sedativa, constituindo uma alternativa aos anestésicos
tradicionalmente usados em aquicultura para facilitar o manejo e/ou reduzir o estresse. Neste
sentido, o estudo teve por objetivo investigar os efeitos desses OEs sobre a fisiologia de peixes,
através de indicadores fisiológicos, bioquímicos e endócrinos. No artigo 1 determinou-se (a) a
atividade anestésica dos OEs de H. ringens (OEHR) e L. alba (OELA) e (b) seus efeitos em
jundiá (Rhamdia quelen) depois da indução e recuperação da anestesia. Os peixes foram
submetidos a um dos seguintes tratamentos para cada OE: grupo basal, controle ou anestesiado
(150, 300 ou 450 μL L-1 OE), avaliando-se a taxa ventilatória (TV) durante o período de indução
e, posteriormente, transferidos para aquários sem anestésicos para recuperação da anestesia.
Nos tempos 0, 15, 30, 60 e 240 min de recuperação foram realizadas amostragens de plasma e
brânquias para medir indicadores metabólicos e enzimas ionorregulatórias, respectivamente. No
artigo 2, os efeitos da exposição prolongada de jundiás a baixas concentrações do OEHR foram
estudados. Após 6 h de exposição a 0 (controle), 30 ou 50 μL L-1 OEHR adicionado à água,
analisou-se: TV, indicadores metabólicos e de estresse em plasma, atividade enzimática em
fígado e expressão de hormônios hipofisários (hormônio do crescimento - GH, prolactina - PRL e
somatolactina - SL). No manuscrito (a) avaliou-se a eficácia anestésica do OELA em dourada
(Sparus aurata) e (b) investigaram-se os efeitos de 35 μL L-1 de OELA e 2-fenoxietanol (2-PHE)
sobre a resposta ao estresse em douradas submetidos à perseguição. Após 4 h de exposição,
foram amostrados plasma (para determinação dos níveis de cortisol, metabólitos e
osmolalidade), cérebro e hipófise (para avaliar a expressão de indicadores endócrinos). No artigo
1, a anestesia com os OEs provocou alterações em alguns parâmetros medidos em jundiás, mas
não impediu a restauração da maioria dos indicadores avaliados após 240 min de recuperação.
No artigo 2, 50 μL L-1 do OEHR provocou a elevação dos níveis de glicose, lactato, proteína e
osmolalidade, bem como aumento na atividade de enzimas metabólicas e redução na expressão
do GH e SL. No manuscrito, douradas expostos ao OELA, estressados ou não, exibiram maiores
níveis de cortisol, glicose, lactato e osmolalidade. A exposição ao OELA somado ao estresse
reduziu os níveis de expressão de CRH-BP (hormônio liberador de corticotrofina ligado à
proteína). A expressão de PRL foi reduzida no grupo controle estressado e após a exposição ao
OELA e 2-PHE em peixes não estressados. Maiores expressões de pro-opiomelanocortina
(POMC) a e b foram observadas em peixes estressados e expostos ao OELA e 2-PHE,
respectivamente. Conclui-se que: (1) o OELA é mais eficiente para jundiás que o OEHR em
concentrações para anestesia; (2) para sedar os peixes, recomenda-se 30 μL L-1 do OEHR (ou
menos); (3) o OELA foi eficaz como anestésico para dourada entre 100-300 μL L-1, mas para 4 h
de exposição o 2-PHE foi mais eficiente em prevenir a resposta ao estresse.

Optimisation des interactions patient-ventilateur en ventilation assistée : intérêt des nouveaux algorithmes de ventilation / Patient-ventilator interactions optimization : new ventilation algorithms contributionCarteaux, Guillaume 30 November 2015 (has links)
En ventilation assistée, les interactions patient-ventilateur, qui sont associés au pronostic, dépendent pour partie des algorithmes de ventilation. Objectifs : Caractériser l'intérêt potentiel des nouveaux algorithmes de ventilation dans l'optimisation des interactions patient-ventilateur : 1) en ventilation invasive, deux modes et leurs algorithmes nous ont semblé novateurs et nous avons cherché à personnaliser l'assistance du ventilateur en fonction de l'effort respiratoire du patient au cours de ces modes proportionnels : ventilation assistée proportionnelle (PAV+) et ventilation assistée neurale (NAVA) ; 2) en ventilation non-invasive (VNI) nous avons évalué si les algorithmes VNI des ventilateurs de réanimation et des ventilateurs dédiés à la VNI diminuaient l'incidence des asynchronies patient-ventilateur. Méthodes : 1) En PAV+ nous avons décrit un moyen de recalculer le pic de pression musculaire réalisée par le patient à chaque inspiration à partir du gain réglé et de la pression des voies aériennes monitorée par le respirateur. Nous avons alors évalué la faisabilité clinique d'ajuster l'assistance en ciblant un intervalle jugé normal de pression musculaire. 2) Nous avons comparé une titration de l'assistance en NAVA et en aide inspiratoire (AI) en se basant sur les indices d'effort respiratoire. 3 et 4) En VNI, nous avons évalué l'incidence des asynchronies patient-ventilateur avec et sans l'utilisation d'algorithmes VNI : sur banc d'essai au cours de conditions expérimentales reproduisant la présence de fuites autour de l'interface ; en clinique chez des patients de réanimation. Résultats : En PAV+, ajuster le gain dans le but de cibler un effort respiratoire normal était faisable, simple et souvent suffisant pour ventiler les patients depuis le sevrage de la ventilation mécanique jusqu'à l'extubation. En NAVA, l'analyse des indices d'effort respiratoire a permis de préciser les bornes d'utilisation et de comparer les interactions patient-ventilateur avec l'AI dans des intervalles d'assistance semblables. En VNI, nos données pointaient l'hétérogénéité des algorithmes VNI sur les ventilateurs de réanimation et retrouvaient une meilleure synchronisation patient-ventilateur avec l'utilisation de ventilateurs dédiés à la VNI pour des qualités de pressurisation par ailleurs identiques. Conclusions : En ventilation invasive, personnaliser l'assistance des modes proportionnels optimise les interactions patient-ventilateur et il est possible de cibler une zone d'effort respiratoire normale en PAV+. En VNI, les ventilateurs dédiés améliorent la synchronisation patient-ventilateur plus encore que les algorithmes VNI sur les ventilateurs de réanimation, dont l'efficacité varie grandement selon le ventilateur considéré. / During assisted mechanical ventilation, patient-ventilator interactions, which are associated with outcome, partly depend on ventilation algorithms.Objectives: : 1) during invasive mechanical ventilation, two modes offered real innovations and we wanted to assess whether the assistance could be customized depending on the patient's respiratory effort during proportional ventilatory modes: proportional assist ventilation with load-adjustable gain factors (PAV+) and neurally adjusted ventilator assist (NAVA); 2) during noninvasive ventilation (NIV): to assess whether NIV algorithms implemented on ICU and dedicated NIV ventilators decrease the incidence of patient-ventilator asynchrony.Methods: 1) In PAV+ we described a way to calculate the muscle pressure value from the values of both the gain adjusted by the clinician and the airway pressure. We then assessed the clinical feasibility of adjusting the gain with the goal of maintaining the muscle pressure within a normal range. 2) We compared titration of assistance between neurally adjusted ventilator assist (NAVA) and pressure support ventilation (PSV) based on respiratory effort indices. During NIV, we assessed the incidence of patient-ventilator asynchrony with and without the use of NIV algorithms: 1) using a bench model; 2) and in the clinical settings.Results: During PAV+, adjusting the gain with the goal of targeting a normal range of respiratory effort was feasible, simple, and most often sufficient to ventilate patients from the onset of partial ventilatory support until extubation. During NAVA, the analysis of respiratory effort indices allowed us to precise the boundaries within which the NAVA level should be adjusted and to compare patient-ventilator interactions with PSV within similar ranges of assistance. During NIV, our data stressed the heterogeneity of NIV algorithms implemented on ICU ventilators. We therefore reported that dedicated NIV ventilators allowed better patient-ventilator synchronization than ICU ventilators, even with their NIV algorithms engaged.Conclusions: During invasive mechanical ventilation, customizing the assistance during proportional ventilatory modes with the goal of targeting a normal range of respiratory effort optimizes patient-ventilator interactions and is feasible with PAV+. During NIV, dedicated NIV ventilators allow better patient-ventilator synchrony than ICU ventilators, even with their NIV algorithm engaged. ICU ventilators' NIV algorithms efficiency is however highly variable among ventilators.

Background: Although the negative impact of sleep apnea on the clinical course of acute ischemic stroke (AIS) is well known, data regarding non-invasive ventilation in acute patients are scarce. Several studies have shown its tolerability and safety, yet no controlled randomized sequential phase studies exist that aim to establish the efficacy of early non-invasive ventilation in AIS patients.
Methods/design: We decided to examine our hypothesis that early non-invasive ventilation with auto-titrating bilevel positive airway pressure (auto-BPAP) positively affects short-term clinical outcomes in AIS patients. We perform a multicenter, prospective, randomized, controlled, third rater- blinded, parallel-group trial. Patients with AIS with proximal arterial obstruction and clinically suspected sleep apnea will be randomized to standard stroke care alone or standard stroke care plus auto-BPAP. Auto-BPAP will be initiated within 24 hours of stroke onset and performed for a maximum of 48 hours during diurnal and nocturnal sleep. Patients will undergo unattended cardiorespiratory polygraphy between days three and five to assess sleep apnea. Our primary endpoint will be any early neurological improvement on the NIHSS at 72 hours from randomization. Safety, tolerability, short-term and three-months functional outcomes will be assessed as secondary endpoints by un-blinded and blinded observers respectively.
Discussion: We expect that this study will advance our understanding of how early treatment with non-invasive ventilation can counterbalance, or possibly reverse, the deleterious effects of sleep apnea in the acute phase of ischemic stroke. The study will provide preliminary data to power a subsequent phase III study.
