Étude comparative de l'anatomie des plaies de greffe de cornée par tomographie de cohérence optique (OCT)

Cette thèse porte sur l’étude de l’anatomie de la cornée après 3 techniques de greffe soient, la greffe totale traditionnelle (GTT) et des techniques de greffe lamellaire postérieur (GLP) telles que la greffe lamellaire endothéliale profonde (DLEK) et la greffe endothélium/membrane de Descemet (EDMG) pour le traitement des maladies de l’endothélium, telles que la dystrophie de Fuchs et de la kératopathie de l’aphaque et du pseudophaque. Dans ce contexte, cette thèse contribue également à démontrer l’utilité de la tomographie de cohérence optique (OCT) pour l’étude de l’anatomie des plaies chirurgicales la cornée post transplantation. Au cours de ce travail nous avons étudié l'anatomie de la DLEK, avant et 1, 6, 12 et 24 mois après la chirurgie. Nous avons utilisé le Stratus OCT (Version 3, Carl Zeiss, Meditec Inc.) pour documenter l’anatomie de la plaie. L'acquisition et la manipulation des images du Stratus OCT, instrument qui à été conçu originalement pour l’étude de la rétine et du nerf optique, ont été adaptées pour l'analyse du segment antérieur de l’oeil. Des images cornéennes centrales verticales et horizontales, ainsi que 4 mesures radiaires perpendiculaires à la plaie à 12, 3, 6 et 9 heures ont été obtenues. Les paramètres suivants ont été étudiés: (1) Les espaces (gap) entre les rebords du disque donneur et ceux du receveur, (2) les dénivelés de surface postérieure (step) entre le les rebords du disque donneur et ceux du receveur, (3) la compression tissulaire, (4) le décollement du greffon, 6) les élévations de la surface antérieure de la cornée et 7) la pachymétrie centrale de la cornée. Les mesures d’épaisseur totale de la cornée ont été comparées et corrélées avec celles obtenues avec un pachymètre à ultra-sons. Des mesures d’acuité visuelle, de réfraction manifeste et de topographie ont aussi été acquises afin d’évaluer les résultats fonctionnels. Enfin, nous avons comparé les données de DLEK à celles obtenues de l’EDMG et de la GTT, afin de caractériser les plaies et de cerner les avantages et inconvénients relatifs à chaque technique chirurgicale. Nos résultats anatomiques ont montré des différences importantes entre les trois techniques chirurgicales. Certains des paramètres étudiés, comme le sep et le gap, ont été plus prononcés dans la GTT que dans la DLEK et complètement absents dans l’EDMG. D’autres, comme la compression tissulaire et le décollement du greffon n’ont été observés que dans la DLEK. Ceci laisse entrevoir que la distorsion de la plaie varie proportionnellement à la profondeur de la découpe stromale du receveur, à partir de la face postérieure de la cornée. Moins la découpe s’avance vers la face antérieure (comme dans l’EDMG), moins elle affecte l’intégrité anatomique de la cornée, le pire cas étant la découpe totale comme dans la GTT. Cependant, tous les paramètres d’apposition postérieure sous-optimale et d’élévation de la surface antérieure (ce dernier observé uniquement dans la GTT) finissent par diminuer avec le temps, évoluant à des degrés variables vers un profil topographique plus semblable à celui d’une cornée normale. Ce processus paraît plus long et plus incomplet dans les cas de GTT à cause du type de plaie, de la présence de sutures et de la durée de la cicatrisation. Les valeurs moyennes d’épaisseur centrale se sont normalisées après la chirurgie. De plus, ces valeurs moyennes obtenues par OCT étaient fortement corrélées à celles obtenues par la pachymétrie à ultra-sons et nous n’avons remarqué aucune différence significative entre les valeurs moyennes des deux techniques de mesure. L’OCT s’est avéré un outil utile pour l’étude de l’anatomie microscopique des plaies chirurgicales. Les résultats d’acuité visuelle, de réfraction et de topographie des techniques de GLP ont montré qu’il existe une récupération visuelle rapide et sans changements significatifs de l’astigmatisme, contrairement à la GTT avec et sans suture. La GLP a permis une meilleure conservation de la morphologie de la cornée, et par conséquence des meilleurs résultats fonctionnels que la greffe de pleine épaisseur. Ceci nous permet d’avancer que la GLP pourrait être la technique chirurgicale à adopter comme traitement pour les maladies de l’endothélium cornéen. / This thesis aims to study the anatomy of the corneal wound following 3 techniques of corneal graft: traditional penetrating keratoplasy (PK) and two techniques of posterior lamellar keratoplasy (PLK) which are deep lamellar endothelial keratoplasy (DLEK) and Endothelial-Descemet’s Membrane Graft (EDMG) for the treatment of the endothelial corneal diseases, such as Fuch’s dystrophy and aphakic and pseudopakic bullous keratopathy. In this context, this thesis also contributes to show the utility of the time domain optical coherence tomography (TD-OCT) for studying the anatomy of surgical wounds after corneal transplantation. In this work we studied the anatomy of DLEK, before and 1,6,12 and 24 months after surgery. We used the Stratus OCT. (Version 3, Carl Zeiss, Meditec Inc.) to document the anatomy of the wound. The acquisition and the handling of the images of the Stratus OCT, an instrument originally designed for the study of the retina and the optic nerve, were adapted to analyse the anterior segment of the eye. Vertical and horizontal central images of the cornea, in addition to 4 radial measurements perpendicular to the wound at 12, 3, 6 and 9 hours were obtained. The following parameters were studied: (1) the gap between the edges of the donor disc and those of the recipient, (2) posterior surface mismatch (step) between the edges of the disc donor and those of the recipient, (3) tissue compression, (4) graft detachment, 6) elevations of the anterior corneal surface and 7) the central pachymetry of the cornea. Measurements of the total thickness were compared and correlated with those obtained with an ultrasound pachymeter. Measurements of visual acuity, manifest refraction and topography were also acquired in order to evaluate the functional results. Lastly, we compared the data of DLEK with those obtained from the EDMG and the PK, in order to characterize the wounds and to highlight the advantages and disadvantages relative to each surgical technique.Our anatomical results showed important differences between the three surgical techniques. Some of the studied parameters, like the step and the gap, were more pronounced in PK than in DLEK and completely absent in the EDMG group. Others, like tissue compression and graft detachment were observed only in the DLEK group. This let us predict that the distortion of the wound varies proportionally with the depth of recipient posterior stromal dissection. The less dissection towards the anterior surface (as in EDMG), the less it affects the anatomical integrity of the cornea, the worst case being full thickness trephination as in PK. However, all the parameters of sub-optimal posterior surface apposition and anterior surface elevation (this last only observed in PK) ended up decreasing with time, evolving with variable degrees to a topographic profile more similar to that of a normal cornea. This process appears longer and more incomplete in the cases of PK because of the type of wound, the presence of sutures and the longer healing period. The mean values of central thickness were normal after surgery. Moreover, these mean values obtained by OCT. were strongly correlated with those obtained by ultrasound pachymetry and we did not notice any significant difference between the mean values of the two measurement techniques. OCT proved to be a useful tool for the study of the microscopic anatomy of the corneal surgical wounds. The results of vision, refraction and topography of the techniques of posterior lamellar grafts showed that there was a fast visual recovery and without significant changes in astigmatism, contrary to PK with and without sutures. Posterior lamellar grafts allowed a better conservation of the morphology of the cornea, and consequently better functional results than PK. This enabled us to conclude that posterior lamellar corneal grafts could be the surgical technique of choice for the treatment of corneal endothelial diseases.

The impact of topical prostaglandin analogs on the biomechanical properties of the cornea in patients with open angle glaucoma

Justification: Le glaucome entraîne une perte progressive de la vision causée par la détérioration du nerf optique. Le glaucome est répandu dans le monde et cause la cécité dans environ sept millions de personnes. Le glaucome touche plus de 400 000 Canadiens et sa prévalence augmente avec le vieillissement de la population.1,2 Il s'agit d'une maladie chronique surnoise dont les symptômes se manifestent uniquement lors des stades avancés et qui peuvent mener à la cécité. Présentement, le seul moyen possible d’arrêter la progression du glaucome au stade initial est de diminuer la pression intra-oculaire (PIO). Les analogues de prostaglandines (APG) topiques sont fréquemment utilisées comme traitement de première ligne. Cependant, la recherche démontre que cette classe de médicaments peut changer certaines propriétés de la cornée, et possiblement influencer la mesure de la PIO.3 Objectif: À déterminer si l'utilisation d'APG affecte les propriétés biomécaniques de la cornée. La conclusion sera basée sur l'analyse intégrée des résultats obtenus de l'analyseur Reichert oculaire Réponse (ORA), la tonométrie par applanation de Goldmann (TAG) et la pachymétrie ultrasonographique. Le deuxième objectif potentiel de cette étude est de déterminer la corrélation, le cas échéant, entre les propriétés biomécaniques de la cornée, l'épaisseur de la cornée centrale (ECC) et la PIO chez les patients subissant un traitement d’APG topique. L'hypothèse principale de cette étude est que l’APG influence les propriétés de la cornée telles que l'épaisseur centrale, l'élasticité et la résistance. Patients et méthodes : Soixante-dix yeux de 35 patients, âgés de 50-85 ans, atteints de glaucome à angle ouvert (GAO) et traités avec APG topique ont été examinés. Seulement les sujets avec une réfraction manifeste entre -6,00 D et +4,25 D ont été inclus. Les critères d'exclusion sont: patients avec n'importe quelle autre maladie de la cornée de l’œil, telles que la dystrophie endothéliale de Fuch’s et kératocône, ou tout antécédent de traumatisme ou d'une chirurgie de la cornée, ainsi que le port de lentilles de contact. Nous avons demandé aux patients atteints du glaucome qui ont des paramètres stables et qui utilisent l’APG dans les deux yeux de cesser l’APG dans l'œil moins affecté par la PIO, et de continuer l’utilisation d’APG dans l'œil contralatéral. Le meilleur œil est défini comme celui avec moins de dommage sur le champ visuel (CV) (déviation moyenne (DM), le moins négatif) ou une PIO maximale historique plus basse si la DM est égale ou encore celui avec plus de dommage sur la tomographie par cohérence optique (TCO, Cirrus, CA) ou la tomographie confocale par balayage laser (HRT, Heidelberg, Allemagne). Toutes les mesures ont été prises avant la cessation d’APG et répétées 6 semaines après l’arrêt. Les patients ont ensuite recommencé l’utilisation d’APG et toutes les mesures ont été répétées encore une fois après une période supplémentaire de 6 semaines. Après commencer ou de changer le traitement du glaucome, le patient doit être vu environ 4-6 semaines plus tard pour évaluer l'efficacité de la goutte.4 Pour cette raison, on été décidé d'utiliser 6 semaines d'intervalle. Toutes les mesures ont été effectuées à l’institut du glaucome de Montréal par le même technicien, avec le même équipement et à la même heure de la journée. L'œil contralatéral a servi comme œil contrôle pour les analyses statistiques. La tonométrie par applanation de Goldmann a été utilisée pour mesurer la PIO, la pachymétrie ultrasonographique pour mesurer l'ECC, et l’ORA pour mesurer les propriétés biomécaniques de la cornée, incluant l'hystérèse cornéenne (HC). L’hypothèse de l'absence d'effet de l'arrêt de l’APG sur les propriétés biomécaniques a été examiné par un modèle linéaire à effets mixtes en utilisant le logiciel statistique R. Les effets aléatoires ont été définies à deux niveaux: le patient (niveau 1) et l'œil de chaque patient (niveau 2). Les effets aléatoires ont été ajoutés au modèle pour tenir compte de la variance intra-individuelle. L’âge a également été inclus dans le modèle comme variable. Les contrastes entre les yeux et les temps ont été estimés en utilisant les valeurs p ajustées pour contrôler les taux d'erreur internes de la famille en utilisant multcomp paquet dans R. Résultats: Une augmentation statistiquement significative due l 'HC a été trouvée entre les visites 1 (sur APG) et 2 (aucun APG) dans les yeux de l'étude, avec une moyenne (±erreur standard) des valeurs de 8,98 ± 0,29 mmHg et 10,35 ± 0,29 mmHg, respectivement, correspondant à une augmentation moyenne de 1,37 ± 0,18 mmHg (p <0,001). Une réduction significative de 1,25 ± 0,18 mmHg (p <0,001) a été observée entre les visites 2 et 3, avec une valeur moyenne HC finale de 9,09 ± 0,29 mmHg. En outre, une différence statistiquement significative entre l’oeil d’étude et le contrôle n'a été observée que lors de la visite 2 (1,01 ± 0,23 mmHg, p <0,001) et non lors des visites 1 et 3. Une augmentation statistiquement significative du facteur de résistance conréen (FRC) a été trouvée entre les visites 1 et 2 dans les yeux de l'étude, avec des valeurs moyennes de 10,23 ± 0,34 mmHg et 11,71 ± 0,34 mmHg, respectivement. Le FRC a ensuite été réduit de 1,90 ± 0,21 mmHg (p <0,001) entre les visites 2 et 3, avec une valeur moyenne FRC finale de 9,81 ± 0,34 mmHg. Une différence statistiquement significative entre l’oeil d’étude et le contrôle n'a été observée que lors de la visite 2 (1,46 ± 0,23 mmHg, p <0,001). Une augmentation statistiquement significative de l'ECC a été trouvée entre les visites 1 et 2 dans les yeux de l'étude, avec des valeurs moyennes de 541,83 ± 7,27 µm et 551,91 ± 7,27 µm, respectivement, ce qui correspond à une augmentation moyenne de 10,09 ± 0,94 µm (p <0,001). L'ECC a ensuite diminué de 9,40 ± 0,94 µm (p <0,001) entre les visites 2 et 3, avec une valeur moyenne finale de 542,51 ± 7,27 µm. Une différence entre l’étude et le contrôle des yeux n'a été enregistré que lors de la visite 2 (11,26 ± 1,79 µm, p <0,001). De même, on a observé une augmentation significative de la PIO entre les visites 1 et 2 dans les yeux de l'étude, avec des valeurs moyennes de 15,37 ± 0,54 mmHg et 18,37 ± 0,54 mmHg, respectivement, ce qui correspond à une augmentation moyenne de 3,0 ± 0,49 mmHg (p <0,001). Une réduction significative de 2,83 ± 0,49 mmHg (p <0,001) a été observée entre les visites 2 et 3, avec une valeur moyenne de la PIO finale de 15,54 ± 0,54 mmHg. L’oeil de contrôle et d’étude ne différaient que lors de la visite 2 (1,91 ± 0,49 mmHg, p <0,001), ce qui confirme l'efficacité du traitement de l’APG. Lors de la visite 1, le biais de la PIO (PIOcc - PIO Goldmann) était similaire dans les deux groupes avec des valeurs moyennes de 4,1 ± 0,54 mmHg dans les yeux de contrôles et de 4,8 ± 0,54 mmHg dans les yeux d’études. Lors de la visite 2, après un lavage de 6 semaines d’APG, le biais de la PIO dans l'œil testé a été réduit à 1,6 ± 0,54 mmHg (p <0,001), ce qui signifie que la sous-estimation de la PIO par TAG était significativement moins dans la visite 2 que de la visite 1. La différence en biais PIO moyenne entre l'étude et le contrôle des yeux lors de la visite 2, en revanche, n'a pas atteint la signification statistique (p = 0,124). On a observé une augmentation peu significative de 1,53 ± 0,60 mmHg (p = 0,055) entre les visites 2 et 3 dans les yeux de l'étude, avec une valeur de polarisation finale de la PIO moyenne de 3,10 ± 0,54 mmHg dans les yeux d'études et de 2,8 ± 0,54 mmHg dans les yeux de contrôles. Nous avons ensuite cherché à déterminer si une faible HC a été associée à un stade de glaucome plus avancé chez nos patients atteints du glaucome à angle ouvert traités avec l’APG. Lorsque l'on considère tous les yeux sur l’APG au moment de la première visite, aucune association n'a été trouvée entre les dommages sur le CV et l'HC. Cependant, si l'on considère seulement les yeux avec un glaucome plus avancé, une corrélation positive significative a été observée entre la DM et l'HC (B = 0,65, p = 0,003). Une HC inférieure a été associé à une valeur de DM de champ visuelle plus négative et donc plus de dommages liés au glaucome. Conclusions : Les prostaglandines topiques affectent les propriétés biomécaniques de la cornée. Ils réduisent l'hystérèse cornéenne, le facteur de résistance cornéen et l'épaisseur centrale de la cornée. On doit tenir compte de ces changements lors de l'évaluation des effets d’APG sur la PIO. Plus de recherche devrait être menées pour confirmer nos résultats. De plus, d’autres études pourraient être réalisées en utilisant des médicaments qui diminuent la PIO sans influencer les propriétés biomécaniques de la cornée ou à l'aide de tonomètre dynamique de Pascal ou similaire qui ne dépend pas des propriétés biomécaniques de la cornée. En ce qui concerne l'interaction entre les dommages de glaucome et l'hystérésis de la cornée, nous pouvons conclure qu' une HC inférieure a été associé à une valeur de DM de CV plus négative. Mots Clés glaucome - analogues de prostaglandines - hystérèse cornéenne – l’épaisseur de la cornée centrale - la pression intraoculaire - propriétés biomécaniques de la cornée. / Rationale: Glaucoma is a chronic disease that causes a gradual loss of vision due to progressive damage to the optic nerve. It is widespread in the world and causes blindness in about seven million people. In addition, it affects more than 400,000 Canadians and its prevalence is increasing with the aging of the population.1, 2 Glaucoma becomes symptomatic only in the more advanced stages of the disease, and currently, the goal of treatment is to halt the progression of the disease by lowering the intra-ocular pressure (IOP). Topical prostaglandin analogs (PGA) are currently the first line treatment, however research has shown that this class of medications may change certain properties of the cornea, and hence the measurement of IOP.3 Aim: To determine whether the use of topical prostaglandin analogs (PGA) affects the biomechanical properties of the cornea. The conclusion will be based on the integrated analysis of the data collected from the Reichert Ocular Response Analyzer (ORA), Goldmann tonometry and ultrasound pachymetry. The second potential aim of this study is to determine the correlations, if any, between the biomechanical properties of cornea, the Central Corneal Thickness (CCT) and IOP in patients undergoing topical PGA treatment. The main hypothesis of this study is that the PGA drops influence the properties of the cornea such as central thickness, elasticity and resistance. Patients and Methods: In this study, seventy eyes of 35 patients, aged 50 - 85 years, with open angle glaucoma (OAG) and treated with topical PGA were examined. Only subjects with a manifest refraction between -6.00 D and +4.25 D were included. Exclusion criteria included patients with any other corneal eye disease, such as Fuch’s endothelial dystrophy or keratoconus, or any past history of corneal trauma or surgery. Contact lens wearers were also excluded. Patients with stable glaucoma parameters who were using topical PGA in both eyes prior to the start of the study were asked to discontinue the PGA in the best eye and to continue the application of PGA to the contralateral eye. The "best" eye, representing the eye with the least amount of glaucoma-related damage, was selected based on the results of the Humphrey Visual Field (HFA, Carl Zeiss Meditec, Inc., Dublin, CA), Heidelberg Retinal Tomography (HRT II, Heidelberg Engineering GmbH, Heidelberg, Germany), Optical Coherence Tomography (CIRRUS HD-OCT, Carl Zeiss Meditec, Inc., Dublin, CA) and maximum IOP. The contralateral eye served as a control for statistical analyses. Corneal measurements were taken before PGA cessation and repeated 6 weeks after cessation. Patients then restarted the use of PGA and all measurements were repeated once more after an additional 6 weeks. After starting or changing glaucoma treatment the patient should be seen approximately 4-6 weeks later to assess efficacy of the drop.4 For this reason it was decided to use 6 weeks interval. All measurements were performed at The Montreal Glaucoma Institute by the same trained technician, with the same equipment and at the same time of day. Goldmann applanation tonometry was used to measure the patient’s intraocular pressure (IOP), ultrasound pachymetry was used to measure central corneal thickness (CCT) and the ORA provided the measurements of the corneal biomechanical properties, including corneal hysteresis (CH). The hypothesis of no effect regarding the discontinuation of PGA on the biomechanical properties was examined by a linear mixed-effect model using the nlme package in R. Random-effects was defined on two levels: the patient (level-1) and the eye within each patient (level-2). Those random-effects were added to the model to account for the intra-individual variance due to the repeated-measure design. Age was also included in the model as a covariate. Contrasts between the eyes and times were estimated using adjusted p-values to control for familywise error rate using multcomp package in R. Results: A statistically significant increase in CH was found between Visit 1 (on PGA) and Visit 2 (no PGA) in the study eyes, with mean (±Standard Error) values of 8.98 ± 0.29 mmHg and 10.35 ± 0.29 mmHg, respectively, corresponding to a mean increase of 1.37 ± 0.18 mmHg (p < 0.001). A significant reduction of 1.25 ± 0.18 mmHg (p < 0.001) was also observed between Visits 2 and 3, with a final mean CH value of 9.09 ± 0.29 mmHg. In addition, a statistically significant difference between the study and control eyes was only observed at Visit 2 (1.01 ± 0.23 mmHg; p < 0.001) and not at Visits 1 and 3. A statistically significant increase in Corneal Resistance Factor (CRF) was found between Visits 1 and 2 in the study eyes, with mean values of 10.23 ± 0.34 mmHg and 11.71 ± 0.34 mmHg, respectively. CRF was then reduced by 1.90 ± 0.21 mmHg (p < 0.001) between Visits 2 and 3, with a final mean CRF value of 9.81 ± 0.34 mmHg. A statistically significant difference between the study and control eyes was only observed at Visit 2 (1.46 ± 0.23 mmHg; p < 0.001). A statistically significant increase in CCT was found between Visits 1 and 2 in the study eyes, with mean values of 541.83 ± 7.27 µm and 551.91 ± 7.27 µm respectively, corresponding to a mean increase of 10.09 ± 0.94 µm (p < 0.001). CCT then decreased by 9.40 ± 0.94 µm (p < 0.001) between Visits 2 and 3, with a final mean value of 542.51 ± 7.27 µm. A difference between the study and control eyes was only recorded at Visit 2 (11.26 ± 1.79 µm; p < 0.001). Similarly, a significant increase in IOP was observed between Visits 1 and 2 in the study eyes, with mean values of 15.37 ± 0.54 mmHg and 18.37 ± 0.54 mmHg respectively, corresponding to a mean increase of 3.0 ± 0.49 mmHg (p < 0.001). A significant reduction of 2.83 ± 0.49 mmHg (p < 0.001) was observed between Visits 2 and 3, with a final mean IOP value of 15.54 ± 0.54 mmHg. The control and study eyes only differed at Visit 2 (1.91 ± 0.49 mmHg; p < 0.001), confirming the effectiveness of PGA treatment. At Visit 1, the IOP bias (IOPcc – Goldmann IOP) was similar in both groups, all eyes at that time being on long term PGA medication, with mean values of 4.1 ± 0.54 mmHg in the control eyes and 4.8 ± 0.54 mmHg in the study eyes. At Visit 2, after a 6 week washout of PGAs, the IOP bias in the tested eye was reduced to 1.6 ± 0.54 mmHg (p<0.001), meaning that underestimation of IOP by Goldmann tonometry was significantly less than at Visit 1. The difference in mean IOP bias between the study and control eyes at Visit 2, however, did not reach statistical significance (p= 0.124). A marginally significant increase of 1.53 ± 0.60 mmHg (p = 0.055) was observed between Visits 2 and 3 in the study eyes, with a final mean IOP bias value of 3.10 ± 0.54 mmHg in the study eyes and 2.8 ± 0.54 mmHg in the control eyes. We then tried to determine if a low CH was associated with signs of more severe glaucoma progression among our open-angle glaucoma patients treated with PGA. When considering all eyes on PGA at the time of the first Visit, no association was found between VF damage and CH. However, when considering only eyes with more advanced glaucoma, a significant positive correlation was observed between VF MD and CH (B = 0.65; p = 0.003). A lower CH was associated with a more negative visual field MD value and thus greater glaucoma-related damage. Conclusions: Topical prostaglandin analogs reduce CH, CRF, CCT and IOP in glaucomatous eyes. The changes in CH and CCT influence the measurement of IOP, and therefore, these changes should be taken into account when evaluating IOP lowering response to PGA medications. To discern an interaction between IOP and corneal hysteresis, further research should be conducted with intraocular pressure control. It can be achieved by using systemic medications that decrease IOP and do not influence biomechanical properties. It can also be achieved by using Pascal dynamic contour tonometry or a similar tonometer that does not depend on the biomechanical properties of the cornea. Regarding the interaction between severity of glaucoma damage and corneal hysteresis, it was demonstrated that a lower CH was associated with a more negative visual field MD value. The damage of ocular surface represents one of the most common causes of decreased quality of vision or even blindness. If the injury is extensive and includes the region of limbus, niche of limbal stem cells (LSC), LSC deficiency occurs and the natural corneal regeneration is stopped. Conjunctival epithelium migrates into the injured area. Neovascularization, local inflammation and corneal opacity occur. Corneal transplantation is an insufficient treatment in such case. If the injury is bilateral, the allogenic limbal graft or LSC transplantation is required. In such cases systemic immunosuppressive drugs with many negative side-effects must be administered. The search for an adequate autologous substitution is important for avoid immunosuppressive medication. Mesenchymal stem cells (MSC) represent a perspective substitution for the reason of their immunomodulatory properties and the capability to differentiate in many cell types. There is possibility to isolate sufficient number of these cells from adipose tissue or bone marrow which are relatively easily accessible. Our goal was to observe local production of cytokines and other molecules which are present in inflammatory reaction after the chemical burn of the murine cornea and after the treatment with stem cells growing on nanofiber scaffold....

Application de l'immunolocalisation à la recherche de la cellule souche endothéliale cornéenne humaine / Application of the immunolocalization for researching the human corneal endothelial stem cells

Le contrôle de la transparence de la cornée dépend de l'intégrité de l'endothélium cornéen mono-stratifié qui est classiquement considéré dès la naissance, dépourvu de capacité de régénération chez l’homme. Dans des conditions pathologiques conduisant à la cécité par œdème cornéen, les pertes significatives en cellules endothéliales (CE) ne sont pas remplacée efficacement, ce qui signifie que ni de nouvelles CE provenant de cellules souches (CS), ni la division des cellules voisines des lésions ne peuvent contribuer à la régénération endothéliale. Toutefois, plusieurs travaux ont prouvé depuis 25 ans que les CE possédaient une capacité proliférative résiduelle ex vivo et deux équipes ont suggéré l’existence de CS ou de progéniteurs à la périphérie de l’endothélium cornéen. Dans notre travail de thèse, nous avons tout d'abord optimisé, en la systématisant, une technique d’immunomarquage spécialement adaptée à l'endothélium cornéen intact de cornées montées à plat. A l’issue de ces développements, nous disposons de protocoles simples de fixation à température optimale et de démasquages antigéniques susceptibles de permettre la révélation de nombreuses protéines. A partir d’une importante série de cornées humaines non conservées et d’autres conservées en organoculture, et grâce à cet outil désormais efficace, nous avons étudié le cycle cellulaire des CE et la localisation de potentielles CS sur l’endothélium cornéen humaine. Nos résultats démontrent que dans ces conditions, les CE expriment de façon homogène des régulateurs positifs (PCNA, MCM2, cycline D1, cycline E et cycline A) et des régulateurs négatifs du cycle cellulaire (P16, P27); certaines particularités ont par ailleurs pu être décrites de façon innovante, comme la localisation cytoplasmique diffuse de MCM2, paranucléaire de la cycline D1, l’absence de P21. L’ensemble des marquages pourrait suggérer que les CE sont arrêtées en fin de G1, après le point de restriction et que de nombreux mécanismes de réparation de l’ADN sont mis en jeux dans les CE exposées à un stress oxydant important tout au long de l’existence. Nous avons identifié une nouvelle organisation de la micro-anatomie de la périphérie et de l'extrême périphérie de l’endothélium où des cellules regroupées en multiples clusters pluristratifiés semblent alimenter des colonnes de CE radiaires longues d'un millimètre. Ces éléments, associés à l’observation d'une moindre différenciation et d’une compétence proliférative plus élevés en périphérie suggèrent un nouveau modèle d’homéostasie endothéliale humaine in vivo: toute la vie, des CS périphériques alimentent de façon très lente la périphérie cornéenne en CE qui migrent de façon centripète pour assurer la stabilité du centre cornéen dont les propriétés optiques primordiales sont sous-tendues par un endothélium qui ne perd que 0,6% de CE par an. A la différence de l’épithélium cornéen, ce système ne peut être accéléré lors de circonstances pathologiques. Les perspectives de nos travaux sont désormais d’essayer d’isoler de l’extrême périphérie les CS endothéliales ou les progéniteurs et de les cultiver en recréant un microenvironnement favorable. La possibilité de produire un grand nombre de CE in vitre non pas à partir de CE sénescentes prélevées sur la totalité de l’endothélium comme cela a été tenté par la passé, mais cette fois à partir de CS ou des progéniteurs ouvriraient la voie d'une véritable thérapie cellulaire endothéliale. L'enrichissement des greffons pendant la durée de leur conservation à la banque de cornée pourrait constituer une première étape majeure avant d’envisager créer de novo un endothélium sur un support greffable pour une greffe endothéliale du 3e type qui deviendrait ainsi enfin indépendante des aléas de la découpe du greffon. Enfin, l’ïdentification de la CS endothéliale et de son microenvironnement permettra aussi d'envisager une thérapie cellulaire in vivo pour traiter les stades précoces des pathologies endothéliales cornéennes / The control of corneal transparency depends on the integrity of the mono-stratified corneal endothelium, which is considered devoid of regenerative capacity after birth in humans. In pathological conditions leading to blindness by irreversible corneal edema, the significant losses of endothelial cells (ECs) are not replaced efficiently, indicating that neither new ECs derived from stem cells (SC) nor the division of ECs neighboring the lesions can contribute to a form of regeneration. However, several works of the last 25 years demonstrated that ECs possess residual capacity of proliferation ex vivo, and more recently, two teams suggested the existence of SC or endothelial progenitors located in the corneal periphery. In this thesis work, we have firstly optimized an immunostaining technique specially adapted to intact endothelium of flat mounted whole corneas. Consequently, we now have simple protocols of fixation at the right temperature, and of antigen retrieval that allow detecting multiple proteins with a clear subcellular localization. Using important series of non-stored and of organ cultured corneas, and thanks to this technique, we investigated the cell cycle status of ECs and the location of potential SC in human corneal endothelium. Our results indicate that ECs homogeneously express positive regulators (PCNA, MCM2, cyclin D1, cyclin E and cyclin A) as well as negative regulators of the cell cycle (P16, P27); several original descriptions have been made: diffuse cytoplasmic location of MCM2, paranuclear location of cyclin D1, absence of P21. The expression patterns suggest that ECS could be arrested after the restriction point of the G1 phase and that numerous mechanism of DNA repair are stimulated in ECs exposed to an important oxidative stress throughout live. We identified a novel organization of the micro-anatomy of the endothelial periphery and extreme-periphery, where cells accumulate in multiple small multilayered clusters connected radial centripetal cell rows of nearly 1 mm of length. Associated with the observation of a lesser differentiation and an increased proliferation capacity in this area, these elements suggest a novel model of endothelial homeostasis in humans: during life, SC continuously and extremely slowly sustain the endothelial periphery with new ECs that migrate toward centre forming cells rows. These cells ensure the stability of the center, which optical fundamental properties require a perfect stability, as indicated by an annual cell loss of only 0.6%. Contrary to the corneal epithelium, this system is incapable of accelerations in case of important cell loss. Further studies are necessary to understand this limitation. Our works offer several perspectives: the next step is to isolate the SC or the progenitors from the extreme periphery and to cultivate them in an adapted microenvironment. The possibility to cultivate endothelial cells directly from SC or progenitors and not, as previously tried in the past, from senescent EC from the whole endothelium open the way of a true endothelial cell therapy. The increase of endothelial cell density during corneal storage by eye banks could be a first step before developing bioengineered endothelium on a specific carrier that could be implanted in the recipient eye. Finally, the identification of SC and of its microenvironment would allow developing the basis of an in vivo cell therapy able to treat early stages of endothelial dysfunctions

Transplante de córnea no Brasil: progresso e dificuldades em 16 anos / Corneal transplantation in Brazil: progress and difficulties in 16 years

Introdução: As doenças da córnea são responsáveis por cerca de 5% da cegueira reversível no mundo e o transplante de córnea (TC) é importante para o tratamento dessas enfermidades. A partir de fontes de dados oficiais e públicas, foram analisados o progresso e as dificuldades relacionados ao TC no Brasil nos últimos 16 anos, bem como desigualdades regionais, gastos do Sistema Único de Saúde (SUS) e indicadores de qualidade dos bancos de tecido ocular (BTOs). Métodos: Estudo retrospectivo e analítico com dados sobre TCs e BTOs no Brasil, no período de janeiro de 2001 a dezembro de 2016, divulgados pelo Sistema Nacional de Transplantes (SNT), pela Associação Brasileira de Transplante de Órgãos (ABTO) e pela Agência Nacional de Vigilância Sanitária (ANVISA). Para verificação de existência de tendência, comparação de médias entre as regiões e verificação da diferença de médias, foram utilizados o teste de Cochran-Armitage, a Análise de Variância e as comparações múltiplas de Duncan, respectivamente. Em todos os testes foi utilizado um nível de significância de 5%. Resultados: No Brasil, houve aumento: de 2,4 vezes no número de TCs (de 6.193 - 35,2 pmp para 14.641 - 71,0 pmp - p < 0,001); de 50,7% na eficácia do atendimento à demanda populacional por TCs (de 35,3% para 53,2% - p < 0,001); de 27,8% no número de globos e córneas in situ doados (de 24.608 - 127,1 pmp para 31.450 - 152,6 pmp - p < 0,001); de 31,7% nas córneas preservadas (de 21.012 para 27.674); de 2,4 vezes no gasto financeiro total com TCs (de R$ 9.179.688 para R$ 22.060.973); e de 2,2 vezes no gasto unitário com TC (de R$ 716 para R$ 1.603). A fila de espera para TC reduziu em 45,4% (de 23.549 - 123,0 pmp para 12.865 - 62,4 pmp - p < 0,001). As duas principais causas para a não doação foram as contraindicações médicas (média de 42,5%) e a recusa familiar (média de 36,6%). As principais causas de descarte de córneas foram a sorologia positiva para hepatite B (média de 33%), validade tecidual (média de 30,9%) e qualidade imprópria do tecido (16,8%). A Eficácia na Preservação de Córnea (EPC), o Coeficiente de Descarte de Córnea (CDC) e a Eficácia no Fornecimento de Córnea para Transplante (ECT) foram em média 88%, 37% e 63% ao longo dos anos, respectivamente. Os melhores índices foram apresentados nas regiões Sul, Centro-Oeste e Sudeste e os piores no Norte e Nordeste. Conclusão: No Brasil, o pequeno número de doações e a grande taxa de descarte de córneas são as principais dificuldades ao adequado atendimento à demanda populacional por TCs. Contudo, o país aumentou a capacidade de transplantar córneas e reduziu as filas de espera em 16 anos / Introduction: Corneal diseases account for about 5% of reversible blindness in the world and Corneal Transplantation (CT) is important for the treatment of these diseases. From official and public data sources, the progress and difficulties related to CT in Brazil in the last 16 years were analyzed, as well as regional inequalities, expenses for the Unified Health System (SUS) and quality indicators of ocular tissue banks (OTBs). Methods: A retrospective and analytical study with data on CT and OTB in Brazil, from January 2001 to December 2016, published by the National Transplantation System (SNT), the Brazilian Organ Transplantation Association (ABTO) and the National Surveillance Agency Sanitary (ANVISA). The Cochran-Armitage test, the Analysis of Variance and the Duncan\'s multiple comparisons were used to verify the existence of trend, comparison of means between regions and verification of the mean difference, respectively. A significance level of 5% was used in all tests. Results: In Brazil, there was an increase: of 2.4 times in the number of CTs (from 6,193 - 35.2 pmp to 14,641 - 71.0 pmp - p < 0.001); of 50.7% in the efficacy of meeting the population demand for CTs (from 35.3% to 53.2% - p < 0.001); of 27.8% in the number of donated globes and corneas in situ (from 24,608 - 127.1 pmp to 31,450 - 152.6 pmp - p < 0.001); of 31.7% in preserved corneas (from 21,012 to 27,674); of 2.4 times in the total finance expense with CTs (from R$ 9,179,688 to R$ 22,060,973); and 2.2 times the unit expense with CT (from R$ 716 to R$ 1,603). The waiting list for CT decreased by 45.4% (from 23,549 - 123.0 pmp to 12,865 - 62.4 pmp - p < 0.001). The two main causes for non-donation were medical contraindications (mean of 42.5%) and family refusal (mean of 36.6%). The main causes of corneal discard were positive serology for hepatitis B (mean of 33%), tissue validity (mean of 30.9%) and inadequate tissue quality (16.8%). Efficacy in Corneal Preservation (EPC), Corneal Discarding Coefficient (CDC) and Efficacy Supply of Corneas for Transplantation (ECT) averaged 88%, 37%, and 63% over the years, respectively. The best indexes were presented in the South, Midwest and Southeast regions and the worst in the North and Northeast. Conclusion: In Brazil, the small number of donations and the high rate of discard of corneas are the main difficulties to the adequate attendance to the population demand by CTs. However, the country increased the ability to transplant corneas and reduced waiting lists in 16 years

Avaliação multidimensional da dor no pós-operatório da ceratectomia fotorrefrativa e fatores preditivos de dor / Improved multidimensional pain evaluation and predictors of early postoperative pain after photorefractive keratectomy

OBJETIVOS: Validar o uso de questionários multidimensionais, como o Inventário Resumido da Dor (BPI) e o Questionário de Dor de McGill (MPQ) no pós-operatório da ceratectomia fotorefrativa (PRK). Comparar o perfil da dor no pós-operatório da PRK entre os dois olhos operados sob as mesmas condições e verificar preditores de dor como sexo, estado de ansiedade, conhecimento prévio da cirurgia e equivalente esférico do erro refrativo (EEER). MÉTODOS: Oitenta e seis olhos de 43 pacientes submeteram-se à PRK com intervalo de 14 dias entre cada olho. Uma hora antes da cirurgia, os pacientes responderam ao Inventário de Estado de Ansiedade (IDEA). No pós-operatório os pacientes receberam tratamento usual para dor e responderam aos questionários Escala Visual Analógica (EVA), BPI e MPQ após uma, 24, 48, 72 e 96 horas. Estudaram-se a consistência interna e as correlações de cada questionário. Compararam-se as pontuações de dor e a ansiedade entre primeiros e segundos olhos operados usando o teste de Wald, pareados através do teste t de Student. Utilizou-se o teste de Wald para comparar o comportamento da dor de acordo com sexo e EEER. RESULTADOS: Os questionários MPQ e BPI demonstraram alta consistência interna. Os questionários apresentaram pontuações mais elevadas na primeira mensuração da EVA (4.93 ± 2.38), MPQ - Índice de Estimativa de Dor (PRI) (26.95 ± 10.58), BPI - Índice de Intensidade de Dor (IID) (14.53 ± 7.36) e o BPI - índice de Interferência Funcional de Dor (IIFD) (22.30 ± 15.13), reduzindo-se gradativamente a cada momento subseqüente de avaliação. O MPQ-PRI na subescala subjetiva, apresentou curva de dor com redução lentificada. Todas as escalas apresentaram redução média estatisticamente significativa de um momento para o outro (p < 0.05) no pósoperatório, exceto no MPQ-PRI Subjetivo. Observaram-se correlações positivas entre as subescalas BPI e MPQ com a EVA (p < 0.05). Não houve diferença estatisticamente significativa nas pontuações de dor da EVA, BPI e MPQ-PRI entre ambos os olhos para todos os momentos avaliados. Os pacientes estavam menos ansiosos antes da PRK do segundo olho (p < 0.001), mas isto não apresentou correlação com níveis de dor após a cirurgia. O sexo e o conhecimento prévio do procedimento cirúrgico não influenciou significativamente em qualquer das escalas de dor. O EEER entre -3D to -5D correlacionou-se (p=0.035) com o BPI. CONCLUSÃO: O BPI e o MPQ apresentaram boas propriedades psicométricas em relação a confiabilidade e validade. Questionários multidimensionais fornecem uma avaliação mais abrangente sobre o perfil de dor após a PRK, se comparados à EVA, principalmente nos aspectos afetivos e cognitivos. O perfil da dor pósoperatória da PRK apresentou-se similar em ambos os olhos sob as mesmas condições. O EEER entre -3D to -5D foi o único fator preditor deste estudo para elevado nível de dor pós-operatória / PURPOSE: to validate the use of multidimensional questionnaires, such as the Brief Pain Inventory (BPI) and the McGill Pain Questionnaire (MPQ) in the postoperative photorefractive keratectomy (PRK). To compare the profiles of postoperative PRK pain between both eyes operated under the same conditions and to verify the preoperative predictors of pain such as gender, anxiety, knowledge of the procedure, and spherical equivalent refractive error (SERE). METHODS: eighty-six eyes of 43 patients with myopia underwent PRK in both eyes at an interval of 14 days between the procedures. One hour before surgery, subjects answered the State Anxiety Inventory (SAI). After surgery, usual PRK pain treatment was given and subjects answered to the Visual Analogue Scale (VAS), BPI and MPQ pain questionnaires at one, 24, 48, 72 and 96 hours intervals. The internal consistency was evaluated and convergent validity of each questionnaire was assessed using correlation testing. Pain scores and anxiety were compared between each eye using the Wald test and paired Student t test. Wald test was also used to test gender and SERE for each eye separately. RESULTS: both BPI and MPQ questionnaires showed internal consistency higher than 0.70. Subjects reported higher postoperative pain scores at the first measurement of the VAS (4.93 ± 2.38), MPQ - Pain Rating Index (26.95 6 10.58), BPI - Pain Severity Index (14.53 ± 7.36), and BPI- Pain Interference Index (22.30 ± 15.13) with decreasing scores at each subsequent observation period in all scales. All scales showed statistically significant (p < 0.05) pain reduction from one measurement to the next postoperatively, except the MPQ-PRI Evaluative. The majority of the scales and subscales showed a statistically significant (p < 0.05) direct correlation with the VAS at all of the evaluation periods. There were no statistically significant differences between the two eyes at all examination intervals regarding the VAS, BPI, and MPQ scores. Subjects were less anxious on average before the second surgery compared to the first surgery (p < 0.001), but this finding was not related to pain ratings after surgery. Gender and knowledge of the procedure did not significantly interfere with any scale of pain. The SERE between -3 D (diopters) and -5 D (p=0.035) revealed interference on the BPI. CONCLUSION: the BPI and the MPQ showed good psychometric properties regarding reliability and validity. The multidimensional questionnaires expanded the assessment of the PRK postoperative pain profile, compared to VAS, mainly in cognitive and affective aspects. The profiles of postoperative pain after PRK were similar between both eyes under the same conditions. In this study, a high SERE was the only predictor for increased pain after PRK

Ocular biomechanics of the anterior segment

The thesis investigates methods of examining corneal biomechanics using non-contact tonometry and introduces novel techniques to investigate corneal material properties in vivo. A comprehensive systems analysis of the CorvisST (CST) and Ocular Response Analyser (ORA) was performed. Pressure sensors were used to characterisation the airflow produced by the CST and the ORA. Distinct differences were observed between the central airflow pressures between the two devices: the CST pressure was higher and of shorter duration. Scheimpflug high-speed imaging via the CST allowed components of the corneal deformation to be investigated and the development of a 3D deformation matrix (time, depth and spatial resolution) through tracing of the anterior and posterior corneal surface. Measures of whole eye movement (WEM) with CST were found to be robust. WEM demonstrated an asymmetric profile and a correction method was developed to address the corneal deformation matrix for this asymmetry. Novel methods for characterisation of intrinsic material characteristics of the cornea were developed using numerical and graphical analytical procedures. Application of these parameters was tested on enucleated porcine eyes across a wide range of manometry internal ocular pressure (MIOP). The dynamic E-Modulus was found to be most affected by MIOP change. To investigate the in vivo distribution and heterogeneity of the corneal biomechanics, a novel set-up allowed the mapping of corneal biomechanics across the cornea using the CST (central, paracentral, peripheral) and ORA (central, peripheral). Biometric and demographic grouping of subjects allowed detection of discriminating factors between individuals. The results suggest that the in vivo cornea of healthy human adults can be characterised as a viscoelastic, damped system for longitudinal strain and a highly oscillating system for lateral strain. The cornea is approximately homogenous for measures of rigidity and dynamic E-Modulus but other corneal material characteristics (longitudinal and lateral strain, hysteresis, damping and compressibility) demonstrated regional differences. The experimental design employed allowed for strict control of biometric and biomechanical intersubject variables, based on gold-standard techniques as well as newly-developed methods, thereby creating a normative database for future use.

Alterações biomecânicas da córnea de suínos induzidas pela confecções de lamelas pediculadas de diferentes espessuras por laser de femtossegundo / Biomechanical changes after flap creation with different thicknesses with the femtosecond laser in swinish cornea

Objetivo: Investigar as alterações biomecânicas da córnea de suínos induzidas pela confecção de lamelas pediculadas de diferentes espessuras pelo laser de femtossegundo. Métodos: Para a formação dos dois grupos, 12 olhos de porcos foram usados: lamelas pediculadas de 100 e de 300 micrômetros confeccionadas pelo laser de femtossegundo. Cada olho foi submetido aos seguintes exames, antes da criação das lamelas: topografia por rasterstereography, Ocular Response Analyzer (ORA), tomografia do segmento anterior por coerência óptica para a avaliação paquimétrica corneal e das lamelas criadas e sistema de velocidade de onda (SVO), que mede a velocidade de propagação de ondas acústicas entre dois transdutores posicionados na superfície corneal antes e imediatamente, após a feitura da lamela. O primerio passo foi desenhado para o estudo das diferenças em relação à histerese corneal, fator de resistência corneal, mudanças na curvatura e velocidade de propagação de onda acústica entre córneas com lamelas finas e espessas. Posteriormente, as lamelas foram amputadas, e as medidas do sistema de velocidade de onda foram repetidas. Resultados: A média de espessura das lamelas ± desviopadrão (DP) foi de 108,5±6,9 (8,5% da espessura total) e 307,8±11,5 m (22,9% da espessura total), para os grupos de lamelas finas e espessas, respectivamente (p< 0,001). Histerese corneal e o fator de resistência corneal não apresentaram diferença estatística, após a criação de lamelas finas (p = 0,81 e p = 0,62, respectivamente). Histerese corneal foi significantemente mais baixa, depois da confecção de lamelas mais espessas (8,0±1,0 para 5,1±1,5 mmHg para medidas pré e pós-operatórias, respectivamente, p = 0,003, diminuição de 36,25%) e fator de resistência corneal também mostrou significante diminuição nesse grupo, após o procedimento cirúrgico; valores médios pré e pós-operatórios de 8,2±1,6 e 4,1±2,5 mmHg respectivamente (p= 0,007), diminuição de 50%. A ceratometria média simulada apresentou maiores valores, após a confecção das lamelas mais espessas em relação ao pré-operatório (ceratometria pré e pós-operatória de 39,5±1 D e 45,9±1,2 D, respectivamente, p= 0,003). Para o grupo de lamelas finas, não houve diferença estatisticamente significante (ceratometria pré e pós-operatória de 40,6±0,6 D e 41,4±1,0 D, respectivamente, p=0,55). Em relação ao Sistema de Velocidade de Onda, após a criação das lamelas e sua amputação, houve diminuição da velocidade de propagação acústica, embora na maior parte das posições não fosse estatisticamente significante. Conclusão: Nas condições experimentais estabelecidas por este estudo, a criação de lamelas de maior espessura pareceu exercer efeito mais relevante sobre a biomecânica da córnea de suínos / Purpose: To study the impact of programmed flaps at two different thicknesses on the biomechanical properties of the swine corneas. Methods: Twelve pig eyes were enrolled in this study and were formed two groups: 100m and 300 m flaps performed with the femtosecond laser. Each eye had the following procedure before the flap creation: raster photograph topographic maps, Ocular Response Analyzer (ORA), Optical Coherence Tomography to measure the pachymetry and flap thickness and Surface Wave Velocity system which is a prototype system that measures sonic wave propagation time between two transducers positioned on the corneal surface before and after flap creation. This first step was designed to investigate the differences in respect to corneal hysteresis, corneal resistance factor, curvature change and ultrasonic wave propagation between the groups with thinner and thicker flaps. After this initial procedure, flap amputation was performed and new measurements with the surface wave velocity system were taken again. Results: Measured flap thicknesses averaged 108.5±6.9 (8.5% of the total cornea) and 307.8±11.5 m (22.9% of the total cornea) for thin and thick flap groups, respectively (p< 0.001). Hysteresis and corneal resistance factor did not change significantly after flap creation in the thin flap group (p = 0.81 and p = 0.62, respectively). With thicker flaps, both parameters decreased significantly from 8.0±1.0 to 5.1±1.5 mmHg (p=0.003, reduction of 36.25%) and from 8.2±1.6 to 4.1±2.5 mmHg, respectively (p = 0.007), reduction of 50%. Simulated keratometry values increased in the thick flap group (from 39.5±1 D to 45.9±1.2 D, p=0.003) after flap creation and not in the thin flap group (from 40.6±0.6D to 41.4±1.0D, p= 0.55). Regarding surface wave velocity analysis, the surgical procedures induced lower values in some positions although most of them did not present statistically different results. Transplante de córnea no Brasil: progresso e dificuldades em 16 anos / Corneal transplantation in Brazil: progress and difficulties in 16 years

Introdução: As doenças da córnea são responsáveis por cerca de 5% da cegueira reversível no mundo e o transplante de córnea (TC) é importante para o tratamento dessas enfermidades. A partir de fontes de dados oficiais e públicas, foram analisados o progresso e as dificuldades relacionados ao TC no Brasil nos últimos 16 anos, bem como desigualdades regionais, gastos do Sistema Único de Saúde (SUS) e indicadores de qualidade dos bancos de tecido ocular (BTOs). Métodos: Estudo retrospectivo e analítico com dados sobre TCs e BTOs no Brasil, no período de janeiro de 2001 a dezembro de 2016, divulgados pelo Sistema Nacional de Transplantes (SNT), pela Associação Brasileira de Transplante de Órgãos (ABTO) e pela Agência Nacional de Vigilância Sanitária (ANVISA). Para verificação de existência de tendência, comparação de médias entre as regiões e verificação da diferença de médias, foram utilizados o teste de Cochran-Armitage, a Análise de Variância e as comparações múltiplas de Duncan, respectivamente. Em todos os testes foi utilizado um nível de significância de 5%. Resultados: No Brasil, houve aumento: de 2,4 vezes no número de TCs (de 6.193 - 35,2 pmp para 14.641 - 71,0 pmp - p < 0,001); de 50,7% na eficácia do atendimento à demanda populacional por TCs (de 35,3% para 53,2% - p < 0,001); de 27,8% no número de globos e córneas in situ doados (de 24.608 - 127,1 pmp para 31.450 - 152,6 pmp - p < 0,001); de 31,7% nas córneas preservadas (de 21.012 para 27.674); de 2,4 vezes no gasto financeiro total com TCs (de R$ 9.179.688 para R$ 22.060.973); e de 2,2 vezes no gasto unitário com TC (de R$ 716 para R$ 1.603). A fila de espera para TC reduziu em 45,4% (de 23.549 - 123,0 pmp para 12.865 - 62,4 pmp - p < 0,001). As duas principais causas para a não doação foram as contraindicações médicas (média de 42,5%) e a recusa familiar (média de 36,6%). As principais causas de descarte de córneas foram a sorologia positiva para hepatite B (média de 33%), validade tecidual (média de 30,9%) e qualidade imprópria do tecido (16,8%). A Eficácia na Preservação de Córnea (EPC), o Coeficiente de Descarte de Córnea (CDC) e a Eficácia no Fornecimento de Córnea para Transplante (ECT) foram em média 88%, 37% e 63% ao longo dos anos, respectivamente. Os melhores índices foram apresentados nas regiões Sul, Centro-Oeste e Sudeste e os piores no Norte e Nordeste. Conclusão: No Brasil, o pequeno número de doações e a grande taxa de descarte de córneas são as principais dificuldades ao adequado atendimento à demanda populacional por TCs. Contudo, o país aumentou a capacidade de transplantar córneas e reduziu as filas de espera em 16 anos / Introduction: Corneal diseases account for about 5% of reversible blindness in the world and Corneal Transplantation (CT) is important for the treatment of these diseases. From official and public data sources, the progress and difficulties related to CT in Brazil in the last 16 years were analyzed, as well as regional inequalities, expenses for the Unified Health System (SUS) and quality indicators of ocular tissue banks (OTBs). Methods: A retrospective and analytical study with data on CT and OTB in Brazil, from January 2001 to December 2016, published by the National Transplantation System (SNT), the Brazilian Organ Transplantation Association (ABTO) and the National Surveillance Agency Sanitary (ANVISA). The Cochran-Armitage test, the Analysis of Variance and the Duncan\'s multiple comparisons were used to verify the existence of trend, comparison of means between regions and verification of the mean difference, respectively. A significance level of 5% was used in all tests. Results: In Brazil, there was an increase: of 2.4 times in the number of CTs (from 6,193 - 35.2 pmp to 14,641 - 71.0 pmp - p < 0.001); of 50.7% in the efficacy of meeting the population demand for CTs (from 35.3% to 53.2% - p < 0.001); of 27.8% in the number of donated globes and corneas in situ (from 24,608 - 127.1 pmp to 31,450 - 152.6 pmp - p < 0.001); of 31.7% in preserved corneas (from 21,012 to 27,674); of 2.4 times in the total finance expense with CTs (from R$ 9,179,688 to R$ 22,060,973); and 2.2 times the unit expense with CT (from R$ 716 to R$ 1,603). The waiting list for CT decreased by 45.4% (from 23,549 - 123.0 pmp to 12,865 - 62.4 pmp - p < 0.001). The two main causes for non-donation were medical contraindications (mean of 42.5%) and family refusal (mean of 36.6%). The main causes of corneal discard were positive serology for hepatitis B (mean of 33%), tissue validity (mean of 30.9%) and inadequate tissue quality (16.8%). Efficacy in Corneal Preservation (EPC), Corneal Discarding Coefficient (CDC) and Efficacy Supply of Corneas for Transplantation (ECT) averaged 88%, 37%, and 63% over the years, respectively. The best indexes were presented in the South, Midwest and Southeast regions and the worst in the North and Northeast. Conclusion: In Brazil, the small number of donations and the high rate of discard of corneas are the main difficulties to the adequate attendance to the population demand by CTs. However, the country increased the ability to transplant corneas and reduced waiting lists in 16 years

Alterações biomecânicas da córnea de suínos induzidas pela confecções de lamelas pediculadas de diferentes espessuras por laser de femtossegundo / Biomechanical changes after flap creation with different thicknesses with the femtosecond laser in swinish cornea

Objetivo: Investigar as alterações biomecânicas da córnea de suínos induzidas pela confecção de lamelas pediculadas de diferentes espessuras pelo laser de femtossegundo. Métodos: Para a formação dos dois grupos, 12 olhos de porcos foram usados: lamelas pediculadas de 100 e de 300 micrômetros confeccionadas pelo laser de femtossegundo. Cada olho foi submetido aos seguintes exames, antes da criação das lamelas: topografia por rasterstereography, Ocular Response Analyzer (ORA), tomografia do segmento anterior por coerência óptica para a avaliação paquimétrica corneal e das lamelas criadas e sistema de velocidade de onda (SVO), que mede a velocidade de propagação de ondas acústicas entre dois transdutores posicionados na superfície corneal antes e imediatamente, após a feitura da lamela. O primerio passo foi desenhado para o estudo das diferenças em relação à histerese corneal, fator de resistência corneal, mudanças na curvatura e velocidade de propagação de onda acústica entre córneas com lamelas finas e espessas. Posteriormente, as lamelas foram amputadas, e as medidas do sistema de velocidade de onda foram repetidas. Resultados: A média de espessura das lamelas ± desviopadrão (DP) foi de 108,5±6,9 (8,5% da espessura total) e 307,8±11,5 m (22,9% da espessura total), para os grupos de lamelas finas e espessas, respectivamente (p< 0,001). Histerese corneal e o fator de resistência corneal não apresentaram diferença estatística, após a criação de lamelas finas (p = 0,81 e p = 0,62, respectivamente). Histerese corneal foi significantemente mais baixa, depois da confecção de lamelas mais espessas (8,0±1,0 para 5,1±1,5 mmHg para medidas pré e pós-operatórias, respectivamente, p = 0,003, diminuição de 36,25%) e fator de resistência corneal também mostrou significante diminuição nesse grupo, após o procedimento cirúrgico; valores médios pré e pós-operatórios de 8,2±1,6 e 4,1±2,5 mmHg respectivamente (p= 0,007), diminuição de 50%. A ceratometria média simulada apresentou maiores valores, após a confecção das lamelas mais espessas em relação ao pré-operatório (ceratometria pré e pós-operatória de 39,5±1 D e 45,9±1,2 D, respectivamente, p= 0,003). Para o grupo de lamelas finas, não houve diferença estatisticamente significante (ceratometria pré e pós-operatória de 40,6±0,6 D e 41,4±1,0 D, respectivamente, p=0,55). Em relação ao Sistema de Velocidade de Onda, após a criação das lamelas e sua amputação, houve diminuição da velocidade de propagação acústica, embora na maior parte das posições não fosse estatisticamente significante.
