Oral Health Related Quality of Life Outcomes of Orthodontics in Children

Contemporary conceptual models of health emphasize the importance of patient-based outcomes and recognize the complexity involved in their assessment. Various health conditions, personal, social, and environmental factors, are all thought to contribute to individual’s quality of life. However, the impact of orthodontic treatment on Oral Health-related Quality of Life (OH-QOL) outcomes in children has not yet been systematically studied. Hence, this research was planned to assess the effect orthodontic treatment has on pediatric OH-QOL outcomes. Further, the important moderational role of children’s psychological assets on OH-QOL reports is explored. Following completion of a preliminary study to confirm the psychometric properties of the Child Perception Questionnaire (CPQl1-14), the current two-phase study was undertaken. This consisted of a cross-sectional study examining the relationship among Self-Esteem (SE), malocclusion, and OH-QOL, and a longitudinal study examining the influence of orthodontics and children’s Psychological Wellbeing (PWB) on OH-QOL reports. This PhD dissertation is presented in the “Publishable Style”. The journals which hold the copyrights for the papers published from this thesis have given permission for the reproduction of the text and figures for this dissertation. The preliminary data confirmed that the CPQ11-14 is sensitive to change when used with children receiving orthodontic treatment. Our cross-sectional findings indicated that the impact of malocclusion on OH-QOL is substantial in children with low SE and identified SE as a salient determinant of OH-QOL in children seeking orthodontic treatment. Longitudinal data, on the other hand, detected significant improvement of OH-QOL outcomes after orthodontic treatment. As postulated, these improvements were most evident for the social and emotional domains of OH-QOL. However, covariate analysis emphasized the important role psychological factors play in moderating OH-QOL reports, as children with better PWB were more likely to report better OH-QOL regardless of their orthodontic treatment status. These results substantiate the validity of contemporary models of patient-based outcomes linking biological, personal, social, and environmental factors. Researchers and clinicians are encouraged to adopt this forward thinking approach when dealing with children with oro-facial conditions. Further studies with larger samples and longer follow-ups would be of value to expand on these findings.

En meningsfull tillvaro Syftet med denna studie har varit att undersöka i vilken grad en omsorgsmottagare upplever en meningsfull tillvaro utifrån beviljade hemtjänstinsatser. Studien har bedrivits genom en halvstrukturerad, kvalitativ intervjumetod med omsorgsmottagare, omsorgsgivare samt biståndshandläggare och analyser av empirin har utgått från symbolisk interaktionism och dramaturgisk rollteori. Vi har funnit att det som är centralt för den enskilde för att känna meningsfullhet är den sociala interaktionen tillsammans med andra människor, att boendemiljön är trivsam och önskvärd samt att den enskilde har en känsla av trygghet. Trots det faktum att omsorgs- och serviceinsatser, i olika grad, påverkar dessa aspekter är studiens viktigaste slutsats att de enskilda vanligtvis inte sammankopplar beviljade insatser med känslan av meningsfullhet. / A meaningful existence The purpose of this study was to examine the degree to which a care recipient is experiencing a meaningful existence by established homecare services. The study was conducted through a structured, qualitative interview method with care recipients, carers and care managers, analysis of empirical data has been based on symbolic interaction and dramaturgical perspective. We have found that crucial for the individual to feel meaningfulness is social interaction with other people, that the living environment is satisfying and desirable, and that the individual also possesses a sense of security. Despite the fact that the home care services, to varying degrees, affect these aspects, the study's main conclusion is that individuals usually do not associate the granted proceedings with the sense of meaningfulness

The social capital theory argues that the development performance of a particular community can be explained directly by the prevailing level of social capital, and that the underlying levels of trust, social norms and networks are sufficient to explain development. On the other hand, the school of thought that supports the need for intermediary actors argues that social capital is unable to influence development performance at higher levels. If a larger impact and a more precise outcome are expected, intermediary actors who facilitate interest formation, aggregation and representation are necessary. Without them, social capital remains largely inactive and dormant. In exploring the possible link between the two variables, this thesis supports the second premises, introducing the notion of intermediary actors that might activate the stock of social capital and its performance on household wellbeing. The need to explore the relationship between the two variables demanded empirical research. The research was conducted in the rural villages of Oromia regional State of Ethiopia. Based on the empirical evidence, the relationship between the stock of social capital and performance in household wellbeing is generally positive. A large stock of social capital is generally accompanied by a higher level of performance in household wellbeing. However, I argue that social capital is only truly social when activated by an intermediary development actor. Failing this, though it contributes significantly to village solidarity and unity, social capital remains inactive and dormant. Therefore, the general conclusion of this thesis is that social capital matters, but its utilisation by intermediary agencies matters more.</p> </font></font></p>

Aggressive tendencies and violent behaviour become internalised and adopted as acceptable ways to resolve conflict situations. Chronic, continuous exposure to violence results in physical, psychological and emotional disturbances, such as depression, anxiety, lowered self-confidence, sleep disturbances, decreased attention and concentration spans. This study addressed the form of violence known as community violence, i.e. violence that children experience within their communities (either as witnesses or as victims). This study investigated the effects of this negative environmental experience and investigated potential mediating and moderating variables that could influence the harmful effects of such experiences. The variables considered as mediating and/or moderating variables were social support and self-esteem. The theoretical framework adopted for this study was Bronfennbrenner&rsquo / s Bioecological Systems theory.</p>

Connections to the land: the politics of health and wellbeing in Arviat Nunavut

Connections to the Land: the Politics of Health and Wellbeing in Arviat, Nunavut is about traditional knowledge as process. The thesis examines the relationships between Inuit Qaujimajatuqangit (IQ) [“the Inuit way of doing things”; traditional knowledge (TK);], Inuit perceptions of health and wellbeing and the land; and what the relationships mean for integrated coastal and ocean management. Among Arviat Inuit (Arviarmiut), IQ, health and wellbeing and the land are tightly interconnected. When one relationship is stressed, disruptions occur throughout the whole system. IQ is embedded in Inuit perceptions of health and wellbeing, and to be healthy, Inuit maintain they must interact with the land in Inuit ways. In 2004, issues surrounding the nature of IQ, its control, production, documentation and legitimation were contested by Arviarmiut. Inuit strongly resented input from academics, resource managers, scientists or other southern “experts”. Arviarmiut sensitivities regarding IQ were in part the result of the rapid social change that had occurred over the last half century. Social systems suffered upheaval as colonial processes and institutions impacted values, networks, families and identity. The rate of change did not allow for time for traditional systems to adapt, and aspects of social change happened out of sync with each other resulting in dysfunction. Arviat’s history of relocation, uneven social change and expanded communication ability all affected the formation of IQ. Through participatory research, participant observation, interviews and network-building with Inuit organizations, the research explores IQ as process and the interconnections with the land and wellbeing. It recommends greater integration of Inuit into resource management planning and decision-making in ways consistent with IQ; and allowing Inuit to decide what processes and policies are most appropriate for them.

Dual-response approach to work stress: An investigation of organisational stressors, individual moderators and wellbeing outcomes.

This study demonstrates the complex place stress has in the workplace by investigating both positive (eustress) and negative (distress) stress responses. An international sample of 140 individuals was recruited from various industries and organisational levels and these individuals participated in a confidential online survey. A moderated mediation model was proposed in which organisational stressors (person-job fit and role overload) influenced employee affective wellbeing directly and indirectly through stress responses, moderated by individual factors (work-family conflict and self-efficacy). Person-job fit influenced eustress which had positive effects on employee affective wellbeing. Role overload influenced distress which negatively impacted affective wellbeing. Self-efficacy moderated these relationships, with high levels increasing stress responses both negative and positive. Work-family conflict moderated relationships by reducing the positive effect of eustress and increasing the negative effect of distress. The findings not only advance current knowledge but have implications for organisational stress management practices.

Defending Starlight as a Cultural Resource: the use of environmental legislation in Aotearoa/New Zealand

In 2012, a large area within the Mackenzie District of Te Waipounamu/South Island of Aotearoa/New Zealand was designated as the Aoraki Mackenzie International Dark Sky Reserve (AMIDSR). While lighting restrictions within the AMIDSR promise to protect starlight visibility in that location for the foreseeable future, this thesis considers instruments provided by the Resource Management Act 1991 (RMA) that could be applied in protection of starlight as a natural resource of cultural significance to Māori. The celestial realm is a vital element of Māori cosmology. Tātai aroraki/Māori astronomy was responsible for the Polynesian discovery and settlement of this nation. Traditionally the moon and stars also played a role in mahika kai/food gathering and other cultural practices. Tātai aroraki is a body of knowledge that was developed through continued observation and use of the natural resource of starlight, and both the knowledge and the natural resource itself are embedded within the whole mahika kai resource chain. Interviews with local kaumātua/elders and a Māori astrophysicist were conducted to determine whether tātai aroraki is still practiced and important to contemporary Kāi Tahu, an iwi/tribe with close ties to the Mackenzie District. Results confirmed that despite the eroding effects that colonisation, urbanisation and new technologies have had on traditional environmental knowledge in general, remnants of tātai arorangi remain and are still used by a few Kāi Tahu individuals and families. For many Māori, their cultural identity is closely linked to traditional knowledge and practices, a form of cultural capital. A strong cultural identity is an important element of cultural wellbeing. Applying the RMA to the protection of starlight from light pollution would protect a resource important for mahika kai and therefore indirectly enhance the potential for Kāi Tahu to provide for their cultural wellbeing. As the RMA is a national statute this has implications for iwi in other regions of Aotearoa who have similar astronomical traditions. This thesis extends previous research on the AMIDSR within a growing body of scholarship on starlight protection. It also makes a contribution to RMA scholarship and Actor-Network Theory literature on the natural environment by including the celestial realm within resource networks.

The sample consisted of 568 Grade nine learners and data was collected through the administration of a questionnaire compiled through the combination of five scales: the Children’s Hope Scale, the Recent Exposure to Violence Scale (REVS), the KIDSCREEN-52, social support scale and Rosenberg’s self-esteem scale. The data was analysed through regression and multiple regression.
