Tyrimo tikslas – nustatyti antibiotikų skyrimo ypatumus Lietuvos pirminės sveikatos priežiūros įstaigose gydant suaugusių žmonių (nuo 18 m. amžiaus) kvėpavimo takų infekcijas. Tyrimo uždaviniai: 1. Nustatyti kvėpavimo takų infekcijų struktūrą. 2. Įvertinti mikrobiologinių tyrimų ir kitų diagnostinių tyrimų naudojimą suaugusiems sergantiems kvėpavimo takų infekcijomis (KTI). 3. Nustatyti antibiotikų skyrimo dažnumą gydant suaugusių KTI privačiuose centruose ir poliklinikose. 4. Nustatyti skiriamų antibiotikų struktūrą gydant suaugusių KTI privačiuose centruose ir poliklinikose. Tyrimo metodika ir apimtis. Atliktas aprašomasis retrospektyvinis paplitimo tyrimas. Daugiapakopės atsitiktinės atrankos būdu atrinktos 1767 ambulatorinės kortelės suaugusių asmenų, kurie 2004 m. gydėsi ambulatorinėse įstaigose. Surinkti ir išanalizuoti bendri duomenys apie ligonį (amžius, lytis), duomenys apie 1 metų bėgyje nustatytas infekcijas, paskirtą antimikrobinį gydymą, atliktus diagnostinius tyrimus. Duomenų tvarkymui ir analizei naudotos Epi Data, SPSS, WinPepi, Epi 6 programos. Rezultatai. Iš viso tirtiems asmenims diagnozuota 1141 infekcija, iš kurių didžiausią dalį (84,1%, 959) sudarė KTI. Daugiau diagnozuota VKTI (66,0%, 633) nei AKTI (34,0%, 326). Dažniausiai diagnozuota tarp VKTI buvo ūmi virusinė KTI (41,5%), faringitai (28,9%), o tarp AKTI - ūmūs bronchitai (52,1 %). [toliau žr. visą tekstą] / The aim of this study was to describe prescription of antimicrobials to treat adult respiratory tract infections in Lithuanian primary health care. The tasks of the investigation: 1. To define the structure of respiratory tract infections (RTIs). 2. To rate the use of microbiological and other diagnostic tests in adult patients with RTIs. 3. To determine the antibiotic prescription rates for adult RTIs in private centers and polyclinics. 4. To define the structure of prescribed antibiotics for the treatment adult RTIs in private centers and polyclinics. Methods and volume of the investigation. The retrospective prevalence study was carried. Randomly selected 1767 medical records of adult pacients, who visited physician during 2004 year were analysed. Data about patients (age, sex), infection, antimicrobial treatment, diagnostic tests were registered and analysed. Data were managed and analysed using Epi Data, SPSS, Win Pepi, Epi 6 statistical programmes. Results. There were 1141 infections diagnosed overall. RTIs were the most prevalent and composed 84,1% (959) of all diagnosed infections. Upper respiratory tract infections (URTIs) were more frequent (66,0%, 633) than lower respiratory tract infections (LRTIs) (34,0%, 326). Mechanism of action and utilization of isothiocyanates from mustard against Escherichia Coli O157:H7

E. coli O157:H7 has been found to survive in dry sausages and cause disease. Isothiocyanates have been studied for their capacity to eliminate pathogens from foods and are attractive from the consumer perspective because of their natural origin. There is a need to better understand how isothiocyanates kill microorganisms and their behaviour in food matrices. It was found that glutathione and cysteine naturally present in meat can react with AIT, forming a conjugate with no or low bactericidal activity against an E. coli O157:H7. In addition, AIT presented higher anti-E. coli activity at lower pH values; therefore, it should be more efficient in acid foods. AIT was also found to inhibit the activity of thioredoxin reductase and acetate kinase; hence, enzymatic inhibition may represent a way in which AIT kills E. coli O157:H7. Mustard powder is used as a spice (active myrosinase) and/or binder (inactive myrosinase) in meat products. Both of these powders killed E. coli O157:H7 in dry fermented sausage. This was not expected since the powder lacking myrosinase is not able to produce isothiocyanates. Starter cultures and E. coli were found to consume significant amounts of glucosinolates. Pediococcus pentosaceus UM 121P and Staphylococcus carnosus UM 123M (higher myrosinase-like activity) were compared against P. pentosaceus UM 116P + S. carnosus UM 109M for their ability in reducing E. coli viability in dry sausage. Sausage batches containing powders of hot mustard, cold mustard, autoclaved mustard and no powder were prepared. Both pairs of starters yielded similar results. Reduction >5 log CFU/g of E. coli O157:H7 occurred after 31 d for hot powder and 38 d for cold powder; there was no reduction in the control. E. coli O157:H7 itself has greater effect on glucosinolate degradation than either pair of starters, which may be more important in determining its survival. Autoclaved powder caused >5 log CFU/g reduction after 18 d. This may be the result of synergistic/additive interaction among E. coli O157:H7 myrosinase-like activity, the presence of newly formed/released antimicrobials in the autoclaved powder and the multiple hurdles present in the dry sausage. Autoclaved mustard powder has potential as a novel food ingredient for the meat industry.

Disruption of thyroid hormone action by environmental contaminants in vertebrates

Thyroid hormones (THs) are important hormones involved in developmental processes, including foetal brain maturation. THs are also involved in the maintenance of homeostasis. One in three people in Canada are considered to have some form of thyroid disorder. One reason for the high level of thyroid disorders may be the increasing amount of anthropogenic chemicals released into the environment that affect normal hormone action. Amphibian metamorphosis is completely dependent on TH and provides a model to study such chemicals. This thesis uses the Rana catesbeiana tadpole as a model to study potential TH disrupting chemicals by developing a novel screening assay called the cultured tail fin biopsy assay, or the “C-fin” assay. The C-fin assay uses tail biopsies from premetamorphic tadpoles, Taylor-Kollros stage VI-VIII. The biopsies are cultured in serum-free media along with the test chemical for 48 hours. QPCR is used to measure the mRNA steady-state levels of selected gene transcripts. Two TH-responsive gene transcripts were measured: the up-regulated gene transcript, thyroid hormone receptor β (TRβ) and the down-regulated gene transcript, Rana larval keratin type I (RLKI). Heat-shock protein 30 (HSP30) and catalase (CAT) were used as indicators of cellular stress. Another model system used in this thesis is rat pituitary cells, or GH3 cells. QPCR was used to measure the mRNA steady-state levels of three TH-responsive genes growth hormone (GH), deiodinase I (DIOI), and prolactin (PRL); heat-shock protein 70 (HSP70) was used as an indicator of cellular stress. Nanoparticles, used in various consumer products, were one class of chemicals examined. Using the C-fin assay, nanosilver and quantum dots (QDs) caused perturbations in TH-signalling and also showed signs of cellular stress. There was no overt toxicity observed as was determined by the normalizer, house-keeping gene transcript, ribosomal protein L8. The GH3 cells also detected TH disrupting effects by both nanosilver and QDs; however, nanosilver did not appear to cause cellular stress whereas QDs did. Nitrate and nitrite, major waterway contaminants, were also examined and there were no TH-perturbations observed using the C-fin assay. Finally, two antimicrobials used in many consumer products, triclocarban (TCC), triclosan (TCS) and its metabolite, methyl-TCS (mTCS) were examined using both the C-fin assay and GH3 cells. Both the C-fin assay and the GH3 cells determined mTCS to be more potent than TCS in disrupting TH action. TCC also caused perturbations in TH-signalling as well as causing a significant amount of cellular stress. Overall the C-fin assay and the GH3 cells proved to be excellent models in studying the potential disruptors of the TH axis. The C-fin assay and GH3 cells detected novel TH disruptors and gave further insight into already known disruptors of the TH axis.

ELISA and spectrometric analysis, is expensive enough, but precise enough to determine the exact concentration of antibiotics.

O experimento foi desenvolvido nas dependências da FCAV/UNESP – Campus de Jaboticabal. Para tanto foram conduzidos quatro estudos, sendo que dois foram dedicados em selecionar uma bactéria probiótica produtora de bacteriocina e avaliar a interação de substâncias antimicrobianas como EDTA, àcido acético e Nisina “in vitro” sobre a capacidade inibitória de Salmonella Enteritidis e Salmonella Gallinarum em diferentes tempos e o terceiro e quarto experimento estudam o potencial de aplicação do probiótico e do antimicrobiano em aves. O isolado de Lactobacillus acidophilus C1, demonstrou ser produtor de bacteriocina e inibir a multiplicação de Salmonella Enteritidis e Salmonella Gallinarum. Dentre os antimicrobianos testados o que apresentou efeito na eliminação de Salmonella Enteritidis foi a combinação de Ácido Acético + nisina e Ácido Acético + Lactobacillus acidophilus C1. O uso do probiótico no estudo em aves reduziu a excreção de Salmonella sp, mas não sua eliminação no trato intestinal das aves, já que é possível constatar sua presença no tratamento controle. A combinação de nisina e ácido acético não levou a redução da multiplicação de Salmonella sp. nas aves. O estudo permitiu verificar que existe um bom potencial de aplicação do probiótico citado, assim como o uso de alguns antimicrobianos na segurança alimentar. / The commercial poultry aims at obtaining high productivity at low cost and offering quality products to the consumer. A pathogenic bacterium which has worried the poultry sector in recent times is Salmonella. Therefore, the present study aimed at characterizing six isolates of Lactobacillus acidophilus in combination with antimicrobials in the inhibition of Salmonella enteritidis and Salmonella gallinarum. The experiment was developed in the dependencies of FCAV/UNESP – Campus of Jaboticabal. In order to do this, four studies were conducted, being two of them dedicated to select a bacteriocin-procucer probiotic bacterium and evaluate the interaction of antimicrobial substances such as EDTA, acetic acid and “in vitro”Nisin on the inhibitory capacity of Salmonella enteritidis and Salmonella gallinarum in different periods and the third and fourth experiment study the potential of application of the probiotic and antimicrobial in poultry. The isolate of Lactobacillus acidophilus C1, showed to be bacteriocin producer and inhibit the multiplication of Salmonella enteritidis and Salmonella gallinarum. Among the antimicrobial tested, the one which presented effect in the elimination of Salmonella enteritidis, was the combination of Acetic Acid + nisin and Acetic Acid + Lactobacillus acidophilus C1. The use of the probiotic in the study of birds reduced the counting of Salmonella sp, but not its elimination in the intestinal tract of the birds, as we can see its presence in the control treatment. The combination of nisin and acetic acid did not show the reduction of the multiplication of Salmonella sp. in birds. The study has shown that there is a good potential of application of the mentioned probiotic, as well as the use of some antimicrobial in food safety.

Avaliação microbiológica de queijos tipo Minas Frescal comercializados na região do Triângulo Mineiro /

As análises compreenderam a determinação do Número Mais Provável de coliformes a 35ºC e a 45ºC e pesquisa de E. coli. Os resultados mostraram a presença de coliformes a 45ºC acima dos valores permitidos pela legislação em 30% dos queijos com SIF, 70% dos queijos sem SIF e 61,4% dos queijos temperados. Das Enterobacteriaceae isolados nos queijos Minas Frescal verificou-se presença de E. coli, Proteus, Providencia, Serratia, Klebsiella e Enterobacter. Foram isoladas e identificadas 1.243 E. coli e dessas amostras foram separadas aleatoriamente 330 estirpes, para pesquisar a presença do gene stx1, stx2 e eae, a detecção dos genes codificadores de adesinas (pap, afa e sfa) pela reação em cadeia da polimerase e o teste de sensibilidade aos antimicrobianos. Nenhuma estirpe testada identificou presença de stx e eae não havendo E. coli produtoras de toxina e uma estirpe apresentou o gene afa. Para o teste de sensibilidade as E. coli de todos os queijos testados apresentaram índices de resistência à tetraciclina. Neste trabalho, avaliou-se o efeito inibitório do Origanum vulgare e da Petroselinum sativum testando-os sobre coliformes a 35ºC e a 45ºC do queijo produzido com leite cru e dos queijos produzidos com leite cru acrescidos de E. coli (ATCC 25922). Os resultados dessas plantas no leite cru não promoveram efeito inibitório sobre os coliformes a 35ºC, no entanto, promoveram um efeito inibitório significativo nos coliformes a 45ºC. No leite inoculado com E. coli, os resultados apresentaram efeito inibitório para os coliformes a 35ºC e a 45ºC / Abstract: In the present study, 111 samples of cheese of Fresh "Minas" type were analyzed. The analysis include the determination of the most likely number of coliforms at 35ºC and at 45ºC and research of E. coli. The results showed the presence of coliforms at 45ºC above the values allowed by the legislation in 30% of the cheese with SIF, 70% of the cheese without SIF and 61.4% of the seasoned cheese. Out of the Enterobacteriaceae isolated in the cheese of Fresh "Minas" type and the E. coli, Proteus, Providencia, Serratia, Klebsiella e Enterobacter presence was determinated. 1,234 E. coli were isolated and identified and 330 stocks were randomly separated out of these samples to research the presence of the stx1, stx2 and eae gene, the detection of the adhesine codifier genes (pap, afa and sfa), through the chain reaction of polymerase and the sensibility test to antimicrobials. No tested stock identified the presence of stx and eae, no E. coli toxin productors and a stock showed the afa gene. For the sensibility test, the E. coli of all the tested cheese showed endurance index to tetracycline. In this study, it was evaluated the inhibitory effect of the Origanum vulgare and the Petroselinum sativum by testing them on the coliforms at 35ºC and at 45ºC of the cheesed produced with raw milk and the ones produced with raw milk and E. coli (ATCC 25922). The results of these plants in the raw milk did not provoke inhibitory effect on the coliforms at 35ºC, however, they provoked a significant inhibitory effect in the coliforms at 45ºC. Correlação entre concentração inibitória mínima e níveis urinários de antimicrobianos para o tratamento de infecções no trato urinário / Correlation between minimum inhibitory concentration and urinary levels of antimicrobials for treatment of infections in urinary tract

Introdução: As Infecções no trato urinário (ITU) estão entre as doenças infecciosas mais comumente diagnosticadas e são responsáveis por uma grande proporção do consumo de agentes antimicrobianos, sendo a Escherichia coli o microorganismo mais freqüentemente envolvido. Os antimicrobianos utilizados no tratamento das ITU sofrem concentração no processo de eliminação, ocasionando altos níveis de antimicrobiano na urina. No entanto, os testes que avaliam a susceptibilidade aos antimicrobianos são baseados em concentrações séricas. Assim, é de interesse a padronização de um teste que utilize níveis urinários (discos de alta potência) dos antimicrobianos frente aos uropatógenos. Objetivo: padronizar teste de susceptibilidade aos antimicrobianos para E. coli de ITU, com concentrações de antimicrobianos atingidas na urina. Métodos: foram utilizados 204 isolados bacterianos resistentes a pelo menos um dos seguintes antimicrobianos: ampicilina, cefalotina, ciprofloxacino, gentamicina, sulfametoxazol-trimetoprima(cotrimoxazol) para os quais foi determinada a MIC através de diluição em agar. Através da análise das MIC obtidas e das concentrações que os antimicrobianos atingem na urina, foram estabelecidas as concentrações dos discos de alta concentração. As seguintes concentrações de disco foram utilizadas: ampicilina (500, 400, 300 μg), cefalotina (200, 100 μg) e ciprofloxacino (200, 150 μg). Resultados: a ampicilina apresentou MIC ≥ 512μg/mL para cerca de 50% dos isolados. Dos discos testados, o de 500 μg mostrou melhor coeficiente de correlação de Spearman (rs: -0,659) e foi escolhido para a determinação do ponto de corte através de curva ROC (receiver operating characteristic). Pela curva, um diâmetro de halo de 9 mm separaria as amostras resistentes (R) das susceptíveis (S) a altos níveis de ampicilina com 100% de sensibilidade e especificidade de 50%. A cefalotina apresentou MIC inferior a 256μg/mL para 78% dos isolados sendo que o “rs” para os discos de 100 e 200 μg foi o mesmo: -0,744. Pela curva ROC, halos de 17mm (disco de 200μg) e 13mm (disco de 100μg) discriminariam amostras R de S com 100% de sensibilidade e 96,9% de especificidade. O ciprofloxacino apresentou MIC ≤ 128 μg/mL para mais de 50% dos isolados, sendo que o “rs” foi melhor com o disco de 200μg (rs:-0,916). Um halo de 21mm separaria as amostras S e R com 100% de sensibilidade e 78,9% de especificidade. Para o cotrimoxazol, 95% das amostras se mostraram resistentes a altos níveis e os testes com disco de alta potência não foram realizados. Para a gentamicina o baixo nº de amostras (n=22) prejudicou a análise, mas cerca de 63% dos isolados seriam susceptíveis a altos níveis. Conclusão: O presente estudo demonstrou que o uso de discos de alta concentração pode ser útil para avaliar a susceptibilidade de isolados urinários. Isto tem importância em laboratórios clínicos onde, embora haja dificuldade na realização da MIC, testes de disco-difusão estão incorporados na rotina. / Introduction: The urinary tract infections (UTI) are among the most common infectious diseases and are responsible for a high antimicrobial usage. The Escherichia coli is the main microorganism involved in UTI. The antimicrobials used for treatment of UTI concentrate in the urinary tract and this leads to high levels of them in the urine. However, the susceptibility tests are based on the serum levels of the antimicrobials. Therefore, it is of interest to develop a test which correlates the urinary levels of antibiotics (discs with high levels) with the uropathogens. Objective: to propose a susceptibility test of E. coli from UTI using doses of the antimicrobials reached in the urine. Methods: a total of 204 clinical isolates of E.coli resistant, according to conventional antimicrobial susceptibility test to, at least, one of the following antimicrobials: ampicillin, cefalotin, ciprofloxacin, gentamicin and trimetoprima- sulfametoxazol. The MIC was determined by the agar dilution technique. According to the results of the MICs and the urinary levels of the antimicrobials it was possible to establish the concentration of the high potency discs. The following concentration of discs were used: ampicilina (500, 400, 300 μg), cefalotin (200, 100 μg) and ciprofloxacin (200, 150 μg). Results: ampicillin presented MIC ≥ 512μg/mL for around 50% of E. coli isolates. The disc containing 500 μg of ampicillin displayed the best coeficient of correlation (rs: -0,659) and it was chosen to establish diameter breakpoint according to the ROC curve. A diameter of 9 mm would discriminate high level ampicillin resistant isolates with 100% and 50% of sensitivity and specificity, respectively. Cefalotin presented MIC below 256 μg/mL for 78% of E. coli isolates. Its “rs” for discs containing 100 and 200μg was the same: - 0,744. According to the ROC curve zone diameters of 17mm (disk of 200μg) e 13mm (disk of 100μg) were able to discriminate isolates resistant with 100% and 96.9% of sensitivity and specificity, respectively. Ciprofloxacin presented MIC ≤ 128 μg/mL for more than 50% of isolates and the best “rs” was achieved with the 200μg (r:-0,916) disk. A zone diameter of 21mm would discriminate susceptible isolates with 100% and 78.9% of sensitivity and specificity. respectively. A total of 95% of E. coli isolates proved to be resistant to high levels of cotrimoxazole and, therefore, tests with high potency disks were not performed. Due to the small numbers of isolates resistant to gentamycin (n=22) we were not able to perform further analysis although 63% of E coli isolates proved to be susceptible to high levels of this antimicrobial. Conclusion: This study indicated that the susceptibility test with disks containing high levels of antimicrobials may be useful to evaluate the susceptibility of urinary tract isolates This may be of importance as the clinical laboratory may not be able to perform the MIC test for routine isolates.

Utilização de redes neurais artificiais para a classificação da resistência a antimicrobianos e sua relação com a presença de 38 genes associados a virulência isolados de amostras de Escherichia coli provenientes de frangos de corte

A Escherichia coli patogênica aviária (APEC), pertence à família Enterobacteriacea, é responsável por vários processos patológicos nas aves, atuando como agente primário ou secundário na aerossaculite, pericardite, perihepatite, peritonite, salpingite, onfalite, celulite, entre outros. O presente estudo aborda a resistência a antimicrobianos de amostras de E. coli (APEC) de uma forma inovadora, utilizando como ferramenta as redes neurais artificiais, metodologia inserida na linha de pesquisa do CDPA (Centro de Diagnóstico e Pesquisa em Patologia Aviária). A utilização de inteligência artificial, especificamente, as redes neurais artificiais (RNAs), está sendo crescentemente empregada como ferramenta para a análise de dados não lineares e multivariados, característica comum em fenômenos biológicos. O objetivo do presente trabalho foi demonstrar que é possível predizer o uso de antimicrobianos, utilizando trinta e oito genes responsáveis por distintos fatores de virulência, oriundos das amostras de Escherichia coli isoladas de frango de corte, através das redes neurais artificiais (RNAs). Além disso, verificou-se a relação entre o índice de patogenicidade (IP) e a resistência aos quatorze antimicrobianos que fazem parte do banco de dados usado para o desenvolvimento deste estudo. Neste trabalho foram utilizados os dados disponíveis referentes a 256 amostras de E. coli isoladas de camas de aviários, lesões de celulite e quadros respiratórios de frangos de corte. Para a confecção das redes neurais artificiais as entradas escolhidas foram: os índices de patogenicidade, as lesões induzidas em pintos de um dia de idade, a caracterização dos genes associados à patogenicidade, o bioquimismo, a origem das amostras e por fim, a motilidade. As redes neurais artificiais foram criadas realizando associações entre as variáveis de entrada com o objetivo de encontrar o modelo mais ajustado. As saídas utilizadas de acordo com Salle (2009) foram o comportamento das cepas de Escherichia coli frente aos 14 antimicrobianos. Para verificar se existia diferença significativa entre as médias dos índices de patogenicidade (IP) e as amostras sensíveis e resistentes aos 14 antimicrobianos utilizados neste estudo, realizou-se análise estatística com o auxílio do software JMP® 9.0.1 (SAS Institute Inc., 2010). Os resultados obtidos demonstram que as redes neurais artificiais foram capazes de realizar a classificação correta do comportamento das amostras com amplitude de 74,22% a 98,44%, desta forma tornando possível predizer a resistência antimicrobiana da Escherichia coli, através de modelo das RNAs. A análise estatística realizada para verificar a relação entre o IP e a resistência aos 14 antimicrobianos demonstrou que estas são variáveis independentes. Ou seja, podem haver picos no IP sem alteração na resistência antimicrobiana, ou até mesmo o contrário, alterações na resistência antimicrobiana sem mudanças no IP. / The avian pathogenic Escherichia coli (APEC), belongs to the family Enterobacteriacea, is responsible for various pathological processes in poultry, acting as an agent in the primary or secondary lesion such as: sacculitis, pericarditis, perihepatitis, peritonitis, salpingitis, omphalitis, cellulitis, among others. This study addresses the antimicrobial resistance of E. coli (APEC) strains in an innovative way, using tools such as artificial neural networks, methodology embedded in the CDPA´s search line (Center for Diagnostics and Research in Avian Pathology). The use of artificial intelligence, specifically artificial neural networks (RNAs), is being increasingly used as a tool for data analysis and nonlinear multivariate, common feature in biological phenomena. The objective of this study was to demonstrate that it is possible to predict the use of antimicrobials, using thirty-eight distinct genes responsible for virulence factors, derived from Escherichia coli isolates from broiler, through artificial neural networks (ANNs). Besides, it was found the relationship between pathogenicity index (PI) and resistance to fourteen antimicrobial forming part of the database used for the development of this study. In this study was used the data available for 256 samples of E. coli isolated from broiler litter, lesion of cellulitis and respiratory symptoms in broilers. To make the neural network inputs have been chosen: the indices of pathogenicity, the induced lesions in chicks at day old, characterization of genes associated with pathogenicity, biochemism, the source of samples and finally motility. Artificial neural networks have been created making associations between the input variables in order to find the best adjusted model. The outputs used according Salle (2009) was the behavior of Escherichia coli strains compared to 14 antimicrobials. To check whether there was a significant difference between the average indices for pathogenicity (IP) and the sensitive and resistant samples to 14 antimicrobials used in this study, statistical analysis was performed with the help of software JMP ® 9.0.1 (SAS Institute Inc., 2010). The results show that artificial neural networks were able of performing correct classification of the behavior of the samples with an amplitude of 74.22% to 98.44%, thereby making it possible to predict the antibiotic resistance of Escherichia coli, using ANNs model. The statistical analysis performed to assess the relationship between IP and resistance to 14 antibiotics showed that these variables are independent. That is, it can happen peaks in IP without change in antimicrobial resistance, or even the opposite, changes in antimicrobial resistance without changes in IP.
