Canine Platelet Concentrates: An In Vitro Study to Effectively Provide a Source of Functional PlateletsSink, Carolyn A. 04 April 2002 (has links)
This study monitored the storage lesion of 15 units of canine platelet concentrates harvested by differential centrifugation. Canine platelet concentrates were stored at 20-24°C in a platelet rotator for a total of 9 days; the storage lesion of three second generation platelet storage containers was compared. The battery of in vitro tests used to monitor the storage lesion were selected from previous studies performed with human platelet concentrates separated by differential centrifugation. Based on these tests, canine platelet concentrates exhibited a storage lesion similar to human platelet concentrates. Metabolic analytes demonstrated decreasing pH, carbon dioxide, bicarbonate and glucose concentrations concurrent with increasing oxygen and lactate dehydrogenase activity over the 9-day period. Platelet structural changes were monitored by mean platelet volume, which began to increase on Day-5. Platelet function appeared to be compromised, as indicated by aggregation studies using collagen and adenosine diphosphate as agonists. Product sterility was maintained.
There was no consensus of data supporting superior performance of one platelet storage container. This study indicates that canine platelet concentrates may be harvested by differential centrifugation of whole blood. In vitro studies utilizing three second-generation platelet storage bags support a previous study and concurs that canine platelet concentrates stored at 20-24°C using continuous agitation are viable for at least 5 days. / Master of Science

OUTCOMES ASSOCIATED WITH BLOOD COMPONENT TRANSFUSION IN ADULT TRAUMA PATIENTSJones, Allison R 01 January 2015 (has links)
The purpose of this dissertation was to evaluate outcomes associated with blood component (BC) transfusion in adult trauma patients. Specific aims were to: 1) explore the relationship between traumatic injury, hemorrhage, and BC transfusion, focusing on consequences of the component storage lesion through presentation of a conceptual model; 2) systematically review research literature comparing outcomes of massively transfused major trauma patients based on ratios of BCs received; 3) evaluating the relationship between type of blood transfusion trauma patients received (whole blood versus BCs) and mortality likelihood after controlling for demographic and clinical variables; 4) evaluating the relationship between volume and ratio of BCs transfused to trauma patients and development of inflammatory complications (ICs) after controlling for demographic and clinical variables.
Specific aim one was addressed through the development of a conceptual model, depicting the current state of knowledge regarding the storage lesion, and short-/long-term outcomes of traumatic injury, hemorrhage, and blood transfusion. The second specific aim was addressed through a systematic review of studies that grouped critically injured, massively transfused patients based on ratios of BCs they received, and compared clinical outcomes among groups. Findings from this analysis revealed increased survival likelihood with massive transfusion of BCs in a 1:1:1 (packed red blood cells [PRBCs], fresh frozen plasma [FFP], platelets [PLTs]) fashion. The third specific aim involved a secondary analysis of the National Trauma Data Bank to evaluate the relationship between type of transfusion trauma patients received (whole blood versus BCs) and mortality. Patients who received BCs experienced a higher mortality likelihood compared with those who received whole blood. The fourth specific aim was addressed through a secondary analysis of the Inflammation and Host Response to Injury Trauma-Related Data Base, to evaluate the relationship between volume and ratio of BCs transfused and development of ICs in patients with major trauma. Findings revealed that total transfused volume of PRBCs, injury severity, and comorbidities were associated with development of ICs. There were no differences in time to complication between PRBCs:FFP or PRBCs:PLTs ratio groups.

Sabendo-se que um percentual expressivo da população brasileira é portadora de ferro deficiência, e tendo em vista as inovações tecnológicas envolvendo os processos hemoterápicos este projeto teve como objetivo a avaliação da dinâmica de ferro com o monitoramento de parâmetros tais como: hematócrito, hemoglobina, dosagem de ferro e ferritina pré transfusionais e quatro meses após a doação, em quatro diferentes grupos de estudo:A,C,Ce D. Foi feita pelo teste ANOVA simples e para as variáveis sem distribuição normal pelo teste não paramétrico de Mann-Whitney e o teste de Kruskal Wallis, teste T-Pareado e Wilcoxon. No primeiro momento de análise (M1), com análise intra-grupos, as variações estatísticas foram presentes apenas nos parâmetros de Hb (p. 0,017), onde as variações estiveram presentes quando comparados os grupos A x D (p. 0,034) e C x D (p. 0,028) e Ht (p. <0,01) onde as variações estiveram presentes quando comparados os grupos A x D (p. 0,034) e C x D (p. 0,028). Knowing that a significant percentage of the population is disabled iron, and in view of the technological innovations involving haemotherapic processes this project aimed to evaluate the iron dynamics with the monitoring of parameters such as hematocrit, hemoglobin, iron dosing and pre transfusion ferritin months after the donation, in four different study groups: A, B, C and D. Statistical analyses was made by ANOVA and simple test for variables without normal distribution using the nonparametric Mann-Whitney and Kruskal Wallis test, Paired t-test and Wilcoxon. At first analysis (M1) and intra-group analysis, statistical variations were present only in Hb parameters (p. 0.017), where variations were present when comparing the x groups D (p. 0.034) and C x D (p. 0028) and HT (p. <0.01) in which variations were present when comparing the groups D x (p. 0.034) and C x D (p. 0028). In the second stage of analysis (M2) was identified differences between the groups, however, due to the low statistical significance was not possible to identify the exact difference per group. Evaluation of the platelet yield index on interim platelet units from Reveos by comparison with platelet count from SysmexAhlin, Jennifer January 2023 (has links)
Background: Platelets are the smallest cells in the blood and are involved in the wound healing process. Platelet transfusion is an important part of treating illnesses such as thrombocytopenia and to prevent and stop major bleedings. Platelets for transfusion are received after processing of whole blood through automated or semi-automated systems and pooling of interim platelet units received after the processing. A limit is set for the minimum total estimated platelet yield in a pool to ensure that the final product consists of enough platelets. Aim: The aim of this study was to evaluate the platelet yield index received from Reveos automatic blood processing system by comparing it to the fluorescent flow cytometry method on Sysmex cell counter. Material and method: Whole blood from 147 donors were processed in the Reveos automated blood processing system into the separate components. All the interim platelet units received a platelet yield index from Reveos. A sample from each interim platelet unit was collected and analyzed on Sysmex in the channel for fluorescent flow cytometry. Result: The difference between the platelet count from the two methods was 45 ±26 x109/unit (p<0,001). Platelet yield index from Reveos was 67 ±17 x109/unit and the platelet count from Sysmex was 112 ±35 x109/unit. The r2 from the scattergram was 0,49. Conclusion: The platelet yield index from Reveos underestimates the platelet yield compared to fluorescent flow cytometry from Sysmex. The limit for the total platelet yield in a pool of interim platelet units could therefore be decreased.

Trauma-induced coagulopathy (TIC) is a recently described condition which traditionally has been diagnosed by the common coagulation tests (CCTs) such as prothrombin time/international normalized ratio (PT/INR), activated partial thromboplastin time (aPTT), platelet count, and fibrinogen levels. The varying sensitivity and specificity of these CCTs have led trauma coagulation researchers and clinicians to use Viscoelastic Tests (VET) such as Thromboelastography (TEG) to provide Targeted Thromboelastographic Hemostatic and Adjunctive Therapy (TTHAT) in a goal directed fashion to those trauma patients in need of hemostatic resuscitation. This review describes the utility of VETs, in particular, TEG, to provide TTHAT in trauma and acquired non-trauma-induced coagulopathy.

INTRODUÇÃO: sangramento por coagulopatia é problema comum durante o transplante hepático (TH). O uso adequado da monitorização da coagulação pode reduzir a transfusão de hemocomponentes, como o Plasma Fresco Congelado (PFC). Exames Convencionais da Coagulação (ECC), tais como Tempo de Protrombina (TP) e Tempo de Tromboplastina Parcial Ativada (TTPa), são os testes mais amplamente utilizados para monitorizar a coagulação durante o TH, mas algumas limitações têm sido apontadas acerca do seu uso em pacientes cirróticos. OBJETIVO: investigar o uso de ECC como variáveis preditoras da indicação de transfusão de PFC durante o TH em pacientes cirróticos. MÉTODO: analisou-se coorte histórica de 297 transplantes hepáticos com enxertos provenientes de doadores cadáveres. Foram incluídos receptores cirróticos de uma única instituição durante nove anos (2002-2010). A infusão profilática de ácido épsilon-aminocaproico (20 mg/kg/h) e outros pré-requisitos hemostáticos foram mantidos na cirurgia. O TP [expresso na forma de Percentual de Atividade da Protrombina (TP%) e de Relação Normalizada Internacional (INR)] e o TTPa foram medidos no pré-operatório e no fim de cada fase do TH. Os participantes só receberam transfusão de PFC quando se diagnosticou coagulopatia, independentemente dos resultados dos ECC. Os pacientes foram distribuídos em dois grupos, de acordo com a ocorrência de transfusão intraoperatória de PFC. Examinou-se o comportamento dos resultados dos ECC durante a cirurgia. Analisaram-se os fatores de risco para a transfusão de PFC por análises uni e multivariada. Os resultados pós-operatórios de ambos os grupos foram comparados. A acurácia dos ECC para predizer o uso de PFC em cada fase da cirurgia foi investigada por curvas ROC. Além disso, pontos de corte dos ECC não associados à coagulopatia foram calculados para cada fase da cirurgia. RESULTADOS: a análise multivariada demonstrou que hematócrito pré-operatório (odds ratio [OR] = 0,90, P < 0,001), fibrinogênio pré-operatório (OR = 0,99, P < 0,001) e ausência de carcinoma hepatocelular (OR = 3,57, P = 0,004) foram as únicas variáveis preditoras independentes para a transfusão de PFC durante o TH. As mortalidades precoce e tardia, a permanência em UTI e a incidência de reoperações por sangramento microvascular foram semelhantes entre os grupos. Os ECC demonstraram baixa acurácia global para a predição de transfusão de PFC durante o TH (as áreas sob as curvas ROC não chegaram a 70%, independentemente do teste da coagulação e do momento da aferição). Pontos de corte de ECC com alta especificidade para a não transfusão de PFC foram determinados em cada fase do TH para TP% (39,4, 27,8 e 20,3), INR (2,14, 2,62 e 3,52) e TTPa (50,5, 80,2 e 119,5 segundos). CONCLUSÕES: os únicos preditores independentes para a transfusão de PFC durante o TH foram hematócrito pré-operatório, fibrinogênio pré-operatório e ausência de carcinoma hepatocelular. Behavior of CCA results during LT were examined in both groups. Univariate and multivariate analyses of risk factors associated with FFP transfusion were performed. Post-operative outcomes were compared between groups. Accuracy of CCA to predict FFP transfusions was investigated using receiver operating characteristic (ROC) curves. Also, alert values of CCA unassociated with coagulopathy in each phase of surgery were calculated. RESULTS: Multivariate analysis showed that preoperative hematocrit (odds ratio [OR] = 0.90, P < 0.001), preoperative fibrinogen (OR = 0.99, P < 0.001) and absence of hepatocellular carcinoma (OR = 3.57, P = 0.004) were the only significant predictors for FFP transfusion. Short- and long-term survival, ICU stay and incidence of early reoperations for bleeding were similar between the groups. Vid blodgivning donerar blodgivare blod frivilligt. Blodet kan sedan användas inom sjukvården för exempelvis blodtransfusion, vilket kräver blodprodukter kompatibla med patienten. Förekomst av leukocyter i blodprodukter medför en ökad risk för febrila transfusionsreaktioner hos transfunderade patienter. Därför krävs det att vid framställning leukocytreducera blodprodukter och utföra kvalitetskontroll. Med analysen B-leukocytpartikelkoncentration (LPK) kan totalantalet leukocyter i helblod beräknas. Flödescytometri är en metod som kan analysera optiska och fluorescerande egenskaper hos exempelvis celler i en suspension, vilket kan användas för att kvantifiera cellantal. BD Leucocount™-Kit (BD Biosciences) är avsett för flödescytometrisk analys av antalet kvarvarande leukocyter i leukocytreducerade blodprodukter. Vid framställning av blodprodukter ska helblodspåsen vila vid rumstemperatur i minst 3 timmar efter blodgivning. I Falun används antingen ett dagsprogram där produktion sker samma dag som blodgivningen, eller ett övernattningsprogram där produktion sker dagen därpå. Prover från 505 kontrollerade erytrocytenheter, samlade i Falun, har påvisat en skillnad i leukocytkoncentration beroende på vilket program som använts. Anledningen till att erytrocytenheternas leukocytinnehåll skiljer sig är inte känt. Syftet med denna studie är därav att undersöka om vilotiden har någon effekt på leukocytkoncentrationen i helblodspåsar. LPK varierade mellan helblodspåsarna. Ett ökande leukocytantal observerades över tid i majoriteten av helblodspåsar, inklusive medelvärde. Däremot kunde inte hypotesprövning påvisa statistisk signifikans. Hypotesen om att leukocytantalet ökar över tid går emot grundläggande hematologi. Utifrån resultaten i denna studie kan inte hypotesen bevisas. Vidare studier bör genomföras. / During blood donation, blood donors donate blood voluntarily. The blood can then be used in healthcare for, for example, blood transfusions, which requires blood products compatible with the patient. The presence of leukocytes in blood products increases the risk of febrile transfusion reactions in transfused patients. Therefore, leukocyte-reduction in blood products is necessary during production. Each blood center must perform quality control on produced blood products. With the analysis B-leukocyte particle concentration (LPK), the total number of leukocytes in whole blood can be calculated. Flow cytometry is a method that can analyze the optical and fluorescent properties of, for example, cells in a suspension, which can be used to quantify cell numbers. The BD Leucocount™-Kit (BD Biosciences) is intended for flow cytometric analysis of the number of leukocytes remaining in leukocyte-reduced blood products. When producing blood products, the whole blood bag should rest at room temperature for at least three hours after the donation. In Falun, either a day program is used where production takes place on the same day as the blood was donated, or an overnight program where production takes place the next day. Samples from 505 controlled erythrocyte units, collected in Falun, have shown a difference in leukocyte concentration depending on the program used. The reason why the leukocyte content of erythrocyte units differs is not known. The purpose of this study is therefore to investigate whether the resting period has any effect on the leukocyte concentration in whole blood bags. The LPK varied between the whole blood bags. An increasing leukocyte count was observed over time in most of the whole blood bags. However, hypothesis testing did not show statistical significance. The hypothesis that leukocyte counts increase goes against basic hematology. Based on the results of this study, the hypothesis cannot be proven.
