O objetivo deste estudo foi analisar o comportamento dos níveis plasmáticos de grelina, em relação aos fatores de risco cardiometabólico, em uma população multiétnica de eutróficos e de obesos..A grelina é um peptídeo produzido predominantemente pelas células oxínticas gástricas, que desempenha importante papel na homeostase energética, promovendo estímulo do apetite e aumento do peso corporal, além de participar do controle do metabolismo lipídico e glicídico, interagindo diretamente com os fatores de risco cardiometabólico. Este é um estudo transversal. Duzentos indivíduos entre 18 e 60 anos com diferentes graus de índice de massa corporal (IMC) compuseram a amostra, assim dividida: cem eutróficos (IMC < 25 kg/m2) e 100 obesos (IMC &#8805; 30 kg/m2). Todos foram avaliados para parâmetros antropométricos, determinação da pressão arterial (aferida por método oscilométrico através de monitor automático) e variáveis metabólicas (métodos usuais certificados). A grelina acilada foi mensurada pela técnica de sanduíche ELISA; a leptina, pelo método Milliplex MAP. O marcador inflamatório proteína C reativa ultrassensível(PCRUS)foi estimado por nefelometria ultrassensível. A insulina foi determinada por quimioluminescência e o HOMA-IR calculado pelo produto insulinemia (U/ml) X níveis de glicemia de jejum (mmol/L) / 22.5. Foram excluídos do estudo aqueles com história de comorbidades crônicas, doenças inflamatórias agudas, dependência de drogas e em uso de medicação nos dez dias anteriores à entrada no estudo. As concentrações de grelina acilada mostraram tendência de redução ao longo dos graus de adiposidade (P<0,001); a leptina se comportou de maneira oposta (P<0,001). Os níveis de grelina se correlacionaram negativamente com IMC (r = -.36; P<0,001), circunferência da cintura (CC) (r=-.34; P<0,001), relação cintura/quadril (RCQ) (r=-.22; P=0,001), diâmetro abdominal sagital (DAS) (r=-.28; P<0,001), pressão arterial sistólica (PAS) (r=-.21; P=0,001), insulina (r=-.27; P<0,001), HOMA-IR (r=-.24; P=0,001) e PCRUS (r=-.29; P<0,001); e positivamente com o HDL-colesterol (r=.30; P<0,001).A PCRUS acompanhou o grau de resistência insulínica e os níveis de grelina também mostraram tendência de redução ao longo dos tercis de resistência insulínica (P=0,001). Em modelo de regressão linear múltipla as principais associações independentes da grelina acilada foram sexo feminino (P=0,005) e HDL-colesterol (P=0,008), ambos com associação positiva e IMC (P<0,001) (associação negativa). Esses achados apontam para uma associação da grelina acilada com melhor perfil metabólico, já que seus níveis se correlacionaram positivamente com HDL-colesterol e negativamente com indicadores de resistência insulínica e atividade inflamatória. / The aim of this study was to analyze the behaviour of ghrelin levels in relation to cardiometabolic risk factors, in a multiethnic population of lean and obese subjects. Ghrelin is a peptide produced mainly by oxyntic gastric cells. It has an important role in energetic balance, stimulating appetite and weight gain, with a role in lipid and carbohydrate metabolism. It interacts directly with the cardiometabolic risk factors. This is a cross-sectional study. Two hundred individuals between 18 and 60 years with varying degrees of body mass index (BMI) comprised the sample, divided as follows: one hundred eutrophic (BMI < 25 kg/m2), 50 men and 50 women and 100 obese (BMI &#8805; 30 kg/m2), 50 men and 50 women. All were evaluated by anthropometric parameters, blood pressure determination (measured by the oscilometric method using an automatic monitor) and metabolic variables (usual methods certificates). The acylated ghrelin was measured by sandwich ELISA technique; leptin by Milliplex MAP method. The inflammatory marker sensitive C reactive protein (hsCRP) was estimated by ultrasensitive nephelometry. Insulin was determined by quimioluminescency and HOMA-IR calculated as the product of insulin (U/ml) X fasting glucose levels (mmol/L) / 22.5. Those subjects with a history of chronic comorbidities, acute inflammatory diseases, drug addiction and on medication in the ten days prior to study entry were withdrawn from the study. There was a trend of decreasing acylated ghrelin (P<0,001) and increasing leptin levels (P<0,001), respectively, along increasing degrees of adiposity. Acylated ghrelin levels were negatively correlated with BMI (r = -.36; P<0,001), waist circumference (r=-.34; P<0,001), waist-to-hip ratio (r=-.22; P=0,001), sagittal abdominal diameter (r=-.28; P<0,001) , systolic blood pressure (r=-.21; P=0,001) , insulin (r=-.27; P<0,001), HOMA-IR (-.24; P=0,001) and high sensitive C reactive protein (hsCRP) (r=-.29; P<0,001); the correlation of acylated ghrelin with HDL-cholesterol was positive (r=.30; P<0,001).The hsCRP followed insulin resistance degree and acyated ghrelin levels also showed decreasing linear trend along increasing HOMA-IR tertiles (P=0,001). In a linear multiple regression model the independent positive correlates of ghrelin were female sex (P=0,005) and HDL-cholesterol (P=0,008), while BMI associated negatively and independently with ghrelin levels (P<0,001). These findings suggest an association of ghrelin with a better metabolic profile, since its levels were positively correlated with HDL-cholesterol and negatively associated with insulin resistance and inflammatory activity indicators.

Analisar a associação recíproca entre fatores de risco cardiometabólico, níveis de adipocitocinas (leptina e adiponectina de alto peso molecular), endocanabinoides (anandamida [AEA] e 2-araquidonoilglicerol [2-AG]), compostos canabimiméticos (N-oleoiletanolamina [OEA] e N-palmitoiletanolamina [PEA]) e polimorfismos em genes codificadores de componentes do sistema endocanabinoide (enzima de degradação de endocanabinoides FAAH [gene FAAH] e receptor endocanabinoide CB1 [gene CNR1]) e do receptor PPAR-&#945; [gene PPARA], em indivíduos com diferentes graus de adiposidade. Duzentos indivíduos, entre 18 e 60 anos, com diferentes graus de índice de massa corporal (IMC) compuseram a amostra, dividida em dois grupos: cem eutróficos (IMC < 25 kg/m2) e 100 obesos (IMC &#8805; 30 kg/m2), com 50 homens e 50 mulheres em cada grupo. Os obesos ficaram assim distribuídos: grau 1, com IMC < 35 kg/m2 (n=54), 27 homens e 27 mulheres; grau 2, com IMC < 40 kg/m2 (n=32), 16 homens e 16 mulheres e grau 3, com IMC &#8805; 40 kg/m2 (n=14), 7 homens e 7 mulheres. Todos os indivíduos foram recrutados entre funcionários, estudantes e residentes do Hospital Universitário Pedro Ernesto, bem como voluntários do quadro da Polícia Militar do Estado do Rio de Janeiro e selecionados com base em amostra de conveniência. Todos foram avaliados por parâmetros antropométricos, determinação da pressão arterial, análises laboratoriais e genotipagem, para determinar seu perfil metabólico, níveis de endocanabinoides e adipocitocinas e rastreamento dos polimorfismos FAAH 385C>A, CNR1 3813A>G e PPARA 484C>G. Foram excluídos do estudo aqueles com história de comorbidades crônicas, doenças inflamatórias agudas, dependência de drogas de qualquer natureza e em uso de medicação nos dez dias anteriores à entrada no estudo. A atividade inflamatória, avaliada pela proteína C reativa ultrassensível (PCRUS), acompanhou o grau de resistência insulínica. Os níveis de PEA se associaram negativamente com a adiposidade visceral e resistência insulínica, sugerindo um melhor perfil metabólico, enquanto que os níveis de 2-AG se associaram positivamente com a PCRUS, apontando para piora nesse perfil. Os polimorfismos estudados não se associaram com o fenótipo obeso ou insulinorresistente. A presença do alelo 3813G no gene CNR1 mostrou associação independente com níveis reduzidos de adiponectina em obesos, sugerindo pior perfil metabólico nesse grupo. A presença do alelo 484G no gene PPARA, associando-se com níveis mais elevados de IMC e LDL-colesterol nos eutróficos pode indicar maior predisposição desses indivíduos para o desenvolvimento de obesidade e dislipidemia aterosclerótica. O genótipo homozigoto AA na posição 385 do gene FAAH e os níveis de PCRUS foram as principais associações, diretas e independentes, com os níveis de AEA, indicando claramente disfunção da enzima de degradação da AEA e, possivelmente, contribuindo para um perfil cardiometabólico mais vulnerável em portadores dessa variante genética. / To analyze the reciprocal association of cardiometabolic risk factors, levels of adipocytokines (leptin and high molecular weight adiponectin), endocannabinoids (anandamide [AEA] and 2-arachidonoylglycerol [2-AG]), cannabimimetic compounds (N-oleoylethanolamine [OEA] and N-palmitoylethanolamine [PEA]) and polymorphisms in genes encoding components of the endocannabinoid system (endocannabinoid degradation enzyme FAAH [FAAH gene] and endocannabinoid receptor CB1 [CNR1 gene]) and the PPAR-&#945; receptor (PPARA gene) in subjects with varying degrees of adiposity. Two hundred individuals between 18 and 60 years with varying degrees of body mass index (BMI) comprised the sample, divided in two groups: one hundred eutrophic (BMI < 25 kg/m2) and 100 obese (BMI &#8805; 30 kg/m2), 50 men and 50 women per group. The obese were distributed as follows: grade 1, with BMI < 35 kg/m2 (n = 54), 27 men and 27 women; grade 2, with BMI between &#8805; 35 and < 40 kg/m2 (n = 32), 16 men and 16 women and grade 3, with BMI &#8805; 40 kg/m2 (n = 14), 7 men and 7 women. All subjects were recruited from staff, students and residents of Pedro Ernesto University Hospital, as well as volunteers from Military Police of Rio de Janeiro State and selected based on a convenience sample. All were evaluated by anthropometric parameters, blood pressure determination, laboratory analysis and genotyping, to determine their metabolic profile, endocannabinoid and adipocytokine levels and investigate the polymorphisms FAAH 385C>A, CNR1 3813G>A and PPARA 484C>G. Those with a history of chronic comorbidities, acute inflammatory diseases, drug addiction of any kind and on medication in the ten days prior to study entry were withdrawn from the study. The inflammatory activity as assessed by high sensitive C reactive protein (hsCRP), accompanied the degree of insulin resistance. The levels of PEA negatively associated with visceral adiposity and insulin resistance, suggesting a better metabolic profile, whereas 2-AG levels were positively associated with hsCRP, pointing to a worse metabolic profile. The polymorphisms studied were not associated with the obese or insulin resistant phenotype. The presence of the allele 3813G in the CNR1 gene was independently associated with reduced levels of adiponectin in obese patients, suggesting a worse metabolic profile in this group. The presence of the allele 484G in the PPARA gene associating with higher levels of BMI and LDL-cholesterol in eutrophics may indicate a predisposition for the development of obesity and atherosclerotic dyslipidemia in these individuals. The homozygous genotype AA in position 385 of the FAAH gene, along with levels of hsCRP, were the main direct and independent associations with AEA levels, clearly indicating dysfunction of the degradation enzyme of AEA and possibly contributing to a more vulnerable cardiometabolic profile in individuals with this variant genotype.

Contribuição de alimentos minimamente processados e ultraprocessados no risco cardiometabólico em adultos jovens brasileiros / Contribution of minimally processed and ultraprocessed foods in the cardiometabolic risk in young brazilian adults

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPES / Objective: to associate food consumption with second degree of processing and cardiometabolic risk in young adults. Methods: A cross-sectional study comparing 120 young adults aged 18 to 25 years classified by the presence of cardiometabolic risk. Food consumption was assessed using a semi-quantitative food frequency questionnaire and classified according to the extent of food processing. Biochemical, clinical, anthropometric, body composition, and lifestyle comparisons were performed using the Mann-Whitney test; food groups and tertiles of fresh food grams, minimally processed, processed and ultraprocessed by the Kruskal-Wallis test. Associations between second grade food consumption and cardiometabolic risk components were assessed by multivariate logistic regression. Results: high consumption of ultraprocessed foods was observed among young adults, regardless of whether or not they had cardiometabolic risk, determining the highest consumption of lipids in cardiometabolic risk (p=0.04). The higher consumption of ultraprocessed foods was a risk factor for abdominal obesity regardless of sex, physical activity level and alcoholic beverage (OR = 0.92, CI=0.78-1.08). In natura and minimally processed foods were protective for changes in LDL-C levels (OR=0.70; CI=0.50-0.98). Conclusion: fat from ultraprocessed foods represented a greater caloric contribution to cardiometabolic risk. In addition, ultraprocessed foods were a risk factor for abdominal obesity regardless of gender, physical activity and alcoholic beverage, and in natura and minimally processed foods were protective for changes in LDL-C levels. / Objetivo: associar o consumo de alimentos segundo grau de processamento e o risco cardiometabólico em adultos jovens. Métodos: estudo transversal comparativo com 120 adultos jovens com idade compreendida entre 18 e 25 anos categorizados pela presença do risco cardiometabólico. O consumo alimentar foi avaliado mediante questionário de frequência alimentar semi-quantitativo e classificado segundo extensão do processamento dos alimentos. Comparações bioquímicas, clínicas, antropométricas, de composição corporal e de estilo de vida foram realizadas pelo teste de Mann-Whitney; grupos alimentares e tercis dos gramas de alimentos in natura, minimamente processados, processados e ultraprocessados pelo teste de Kruskal-Wallis. Associações entre o consumo de alimentos segundo grau de processamento e os componentes do risco cardiometabólico foram avaliadas por regressão logística multivariada. Resultados: foi observado o elevado consumo de alimentos ultraprocessados entre os adultos jovens, independente de ter ou não risco cardiometabólico, determinando o maior consumo de lipídeos no risco cardiometabólico (p=0,04). O maior consumo de alimentos ultraprocessados foi fator de risco à obesidade abdominal independente de sexo, nível de atividade física e bebida alcoólica (OR=0,92; IC=0,78-1,08). Os alimentos in natura e minimamente processados foram protetores às alterações nas concentrações de LDL-c (OR=0,70; IC=0,50-0,98). Conclusão: gorduras provenientes de alimentos ultraprocessados representaram maior contribuição calórica no risco cardiometabólico. Além disso, alimentos ultraprocessados foram fator de risco à obesidade abdominal independente de sexo, atividade física e bebida alcoólica e, os alimentos in natura e minimamente processados foram protetores à alterações dos níveis de LDL-c. / Aracaju, SE

Efeitos de um programa de intervenção no estilo de vida sobre o perfil de risco cardiometabólico de uma população nipo-brasileira de alto risco cardiovascular / Impact of lifestyle intervention program on cardiometabolic profile of a high risk Japanese-Brazilian population.

Introdução: Altas prevalências de diabetes mellitus tipo 2 e outros fatores de risco cardiovascular previamente detectadas na população nipo-brasileira motivaram a implementação de um programa de intervenção em mudança de estilo de vida. Objetivos: Avaliar os efeitos de 2 anos de intervenção comportamental sobre o perfil cardiometabólico desses indivíduos, independente da condição de tolerância à glicose no início do estudo. Métodos: A existência de dados relativos a 2000-2005 permitiu conhecer o comportamento de variáveis no período pré-intervenção. Em 2005, 728 indivíduos iniciaram o seguimento, sendo, 650 reavaliados em 2006 e 500 em 2007. Nestas ocasiões, os indivíduos foram submetidos a exames médico e laboratorial e consulta com nutricionista e educador físico. O programa constou de atendimentos individualizados e em grupo, enfocando as metas do programa: redução 5% de peso corporal; prática de 150minutos/semana de exercício; ingestão de <10% de ácidos graxos saturados do valor calórico total; ingestão de 400g/dia de frutas, verduras ou legumes. Resultados: Comparado ao período pré-intervenção, o comportamento de variáveis antropométricas e metabólicas durante o programa foi significantemente mais favorável. O impacto do programa dependeu do alcance de metas em indivíduos sem diabetes. As frequências de intolerância à glicose e diabetes reduziram após um ano de intervenção (de 58,4% para 35,4%; p<0,001 e de 30,1% para 21,7%; p<0,001, respectivamente). Após excluir os indivíduos com diabetes no início do estudo, 71,7% dos participantes mantiveram ou regrediram seu estado de tolerância à glicose (\"non-progressors\") no final de 2 anos de seguimento. A presença de intolerância à glicose e níveis mais baixos de PCR no início do estudo associaram-se independentemente com a não deterioração da tolerância à glicose, ajustado para idade e variáveis antropométricas. As mudanças em peso e estilo de vida não se associaram com a nãodeterioração da tolerância à glicose após intervenção. Conclusões: A intervenção trouxe benefícios no perfil cardiometabólico de nipo-brasileiros de alto risco cardiovascular após o primeiro e o segundo anos do programa, independente do grau de tolerância à glicose no início do estudo. As mudanças em fatores de risco cardiovascular foram proporcionais ao sucesso no alcance das metas do porgrama. A maioria dos indivíduos sem diabetes manteve ou melhorou a tolerância à glicose após 2 anos de intervenção. Os achados sugerem que níveis mais baixos de PCR e a presença inicial de intolerância à glicose podem ser características preditivas dos indivíduos que se beneficiam desta estratégia de intervenção em termos de não-deterioração do metabolismo da glicose, independente da adiposidade corporal. Esse estudo deve encorajar profissionais da saúde a instituir estratégias simples de intervenção comportamental em grupos populacionais com alto risco cardiometabólico, independente do estado de tolerância à glicose. / Introduction: High prevalence rates of diabetes mellitus type 2 and other cardiovascular risk factors, previously reported in the Japanese-Brazilian population, motivated an intervention program for change in lifestyle. Aims: To evaluate the effect of 2-year behavioral intervention on cardiometabolic profile of these individuals, independent of the glucose tolerance status at the beginning of the study. Methods: The availability of data regarding the period 2000-2005 provided information about the behavior of variables during the pre-intervention period. In 2005, 728 individuals started in the program; 650 were re-examined in 2006 and 500 in 2007. In each occasion, participants were scheduled for laboratory procedures, medical exams and visits with nutritionist and physical educator. Participants were scheduled for individualized dietary counseling and group sessions, focusing the achievement of the goals: 5% weight loss from baseline for individuals with excessive adiposity; 150 minutes/week of physical activities; intake £ 10% of saturated fat of total energy consumed per day; intake of 400g/day of fruits and vegetables. Results: Compared to the pre-intervention period, the changes in metabolic variables induced by the program were significantly more favorable. The impact of the program was dependent on the number of goals achieved in individuals without diabetes at baseline. Prevalence rates of glucose tolerance disturbances decreased after the first year of intervention (from 58.4% to 35.4%, p<0.001; and from 30.1% to 21.7%, p<0.001, respectively). After excluding individuals with diabetes at baseline, 71.7% of participants improved or maintained their glucose tolerance status (non-progressors) after 2-year intervention. Lower levels of CRP and the diagnosis of glucose intolerance at baseline were associated with improvement or maintenance of glucose tolerance status, adjusted age and anthropometric variables. Changes in lifestyle and anthropometric variables after intervention were not associated with non-deterioration of glucose tolerance status. Conclusion: The intervention induced benefits on cardiometabolic profile of the japanese-Brazilians with high cardiovascular risk after the first and second years of the program, independent of the glucose tolerance status at baseline. Most of the participants maintained or improved the glucose tolerance status following the intervention. Our findings suggest that lower levels of CRP and the presence of glucose intolerance at baseline may be predictive characteristics of those who benefit from this intervention strategy regarding non-deterioration of glucose metabolism, independent of body adiposity. This study should encourage health care providers to make efforts to achieve and maintain a healthy diet and physically active lifestyle in subsets of the population at high cardiometabolic risk.

Introdução: Doenças cardiometabólicas são problemas de saúde pública mundial com repercussões sociais e econômicas. Dieta inadequada é apontada como um dos principais fatores de risco modificáveis, de importância central na prevenção destas doenças. Apesar da constatação de que certos nutrientes se associam a doenças, ainda existem dúvidas quanto às relações causais. Objetivos: 1) Revisar a participação dos alimentos e nutrientes na gênese e controle de doenças crônicas não-transmissíveis integrantes da síndrome metabólica (manuscrito 1); 2) Descrever de forma detalhada a estrutura do programa de intervenção interdisciplinar em hábitos de vida, desenvolvido para usuários da rede pública de saúde do município de São Paulo (manuscrito 2); 3) Investigar o efeito de mudanças em fatores dietéticos sobre o metabolismo glicídico, decorrentes de intervenção em hábitos de vida em indivíduos de risco cardiometabólico da rede pública de saúde (manuscrito 3). Métodos: Foram incluídos adultos com diagnóstico de pré-diabetes ou síndrome metabólica sem diabetes em ensaio clínico aleatorizado envolvendo dois tipos de intervenções de 18 meses: tradicional e intensiva. A intervenção tradicional consistiu de consultas trimestrais com endocrinologista; na intensiva, além dos atendimentos médicos, os indivíduos participaram de sessões psicoeducativas em grupo com equipe multiprofissional. As orientações dietéticas visaram à adequação do consumo de gorduras totais (redução de saturadas e trans e elevação de insaturadas) e aumento na ingestão de fibras. As variáveis coletadas nos momentos pré- e pós-intervenção incluíram dados sócio-demográficos, antropométricos, dietéticos e bioquímicos (glicemia de jejum e póssobrecarga, perfil lipídico, insulina, adiponectina, proteína C reativa, IL-6, TNF-, apolipoproteína A1 e B). A dieta foi avaliada por recordatórios de 24 horas, processados pelo Nutrition Data System software. Foram empregados os testes: t de Student, coeficientes de correlações, ANOVA com p de tendência e análises de regressão múltipla para identificar fatores dietéticos associados à melhora do metabolismo glicídico. Resultados: O manuscrito 1 consiste de um capítulo de livro (Nutrição em Saúde Coletiva) abordando aspectos epidemiológicos de doenças do espectro da síndrome metabólica com ênfase em fatores alimentares e estado nutricional. O manuscrito 2 detalha a estrutura, a equipe integrante, as orientações preconizadas e as principais estratégias psicoeducativas utilizadas no programa de intervenção em hábitos de vida desenvolvido (artigo metodológico submetido e website). Análises do manuscrito 3 mostraram que, ao final da intervenção, o aumento na ingestão de ácidos graxos monoinsaturados e fibras solúveis associaramse de forma independente à redução da glicemia de jejum e os primeiros também à melhora da glicemia pós-sobrecarga. Ajustes por medidas antropométricas não alteraram os resultados, o que ocorreu, porém, após inclusão da variável mudança na insulina nos modelos (artigo original). Conclusão: Justifica-se divulgar programas de intervenção como o aqui desenvolvido considerando-se o atual cenário das doenças cardiometabólicas na atualidade. No presente estudo, o aumento na ingestão de ácidos graxos monoinsaturados e de fibras solúveis promoveu benefícios no metabolismo glicídico, independente da adiposidade, ao final da intervenção no estilo de vida. Possíveis mecanismos mediadores destes processos podem incluir principalmente a melhora da sensibilidade à insulina. Dessa forma, nossos achados sugerem fortemente que um aumento factível no consumo destes nutrientes deva ser estimulado como estratégia para a proteção do metabolismo glicídico em indivíduos de risco cardiometabólico / Introduction: Cardiometabolic diseases are global public health problems with social and economic repercussions. Unhealthy diet is seen as one of the main modifiable risk factors, being of central importance for the prevention of these diseases. Despite the evidence that certain nutrients are associated with diseases, causal relationships are still under discussion. Objectives: 1) To review the involvement of foods and nutrients in the genesis and control of non-communicable chronic diseases, that comprise the metabolic syndrome (manuscript 1); 2) To describe in detail the structure of the interdisciplinary intervention program on lifestyle, developed for costumers of public health system of Sao Paulo city (manuscript 2); 3) To investigate the impact of changes in dietary factors on glucose metabolism, induced by a lifestyle intervention in individuals at cardiometabolic risk attended by the public health system (manuscript 3). Methods: Adults with prediabetes or metabolic syndrome without diabetes were included in a randomized clinical trial involving two types of 18-month interventions: a traditional or an intensive one. The traditional intervention consisted of quarterly consultations with an endocrinologist, while in the intensive participants also attended psychoeducational group sessions with a multidisciplinary team, in addition to the medical visits. The dietary recommendations aimed at the adequate intake of fat (reduction of saturated and trans and increase of unsaturated fatty acids) and increase in fiber intake. Socio-demographic, anthropometric, dietary and biochemical (fasting and post-load plasma glucose, lipid profile, insulin, adiponectin, C-reactive protein, IL-6, TNF-, apolipoprotein A1 and B) data were collected at baseline and after 18-month of follow-up. Diet was assessed by 24-h dietary recalls, processed by Nutrition Data System software. Student t test, correlation coefficients, ANOVA with p for trend and multiple linear regressions analyses were employed to identify dietary factors associated with glucose metabolism improvement. Results: Manuscript 1 consists of a book chapter (Nutrition in Public Health) about epidemiological aspects of diseases of the spectrum of metabolic syndrome with emphasis on dietary factors and nutritional status. Manuscript 2 details the structure, professional team, recommendations and the main psychoeducative strategies used in the lifestyle intervention program developed (methodological article submitted and website). Analyses of manuscript 3 showed that the intervention-induced increase in monounsaturated fatty acids and soluble fiber intake were independently associated with reduction on fasting plasma glucose and the former also with 2h-plasma glucose improvement. Adjustment for anthropometric measurements did not change these results but did after including change in insulin in the models (original article). Conclusion: Intervention programs such as the developed herein should be reported taking into consideration the current scenario of cardiometabolic diseases. In the present study, increases in monounsaturated fatty acids and soluble fiber intake promoted benefits in glucose metabolism, independent of adiposity, at the end of the lifestyle intervention. Possible mechanisms mediating these processes may mainly include improvement in insulin sensitivity. Therefore, our findings strongly suggest that a feasible increase in consumption of these nutrients should be encouraged as a strategy for the protection of glucose metabolism in individuals at cardiometabolic risk

Preterm birth and cardiometabolic risk factors in adolescence and early adulthood

Abstract About 11% of infants are born preterm (before 37 weeks of gestation) worldwide. Adults born preterm with very low birth weight show enhancement of cardiometabolic risk factors such as elevated blood pressure and impaired glucose regulation compared with their peers born at term. Not all the cardiometabolic risk factors related to preterm birth are known, or whether they apply to those born less preterm, although about 80% of premature infants are born late preterm. The association between preterm birth and cardiometabolic risk factors in adolescence and adulthood was investigated in three cohort studies: The Helsinki Study of Very Low Birth Weight Adults, the Northern Finland Birth Cohort 1986, and the ESTER study. Preterm birth over its whole range has a long-term impact on a child’s health in later life: adults born preterm with very low birth weight had lower resting energy expenditure, but higher resting energy expenditure per unit lean body mass than their peers born at term. Adolescent girls born before 34 weeks of gestation had higher blood pressure and boys have elevated levels of LDL cholesterol and apolipoprotein B. Adults born preterm were more likely to be obese and to have hypertension or metabolic syndrome than their peers born at term. In addition to conventional biomarkers of cardiometabolic disorders, they had alterations in other cardiometabolic biomarkers, such as uric acid and liver transaminases. Adolescents and adults born preterm are at greater risk of developing cardiometabolic disorders than their peers born at term. Most of the cardiometabolic risk factors related to preterm birth are modifiable. Favorable early life circumstances of premature infants, such as optimal nutrition and reduction of stress in neonatal intensive care units, might reduce the risk of later cardiometabolic disorders. In addition, children and adults born preterm might particularly benefit from primary prevention such as screening for additional risk factors and promotion of healthy lifestyles. / Tiivistelmä Noin joka yhdeksäs lapsi maailmassa syntyy ennenaikaisesti, ennen 37. raskausviikkoa. Keskosena syntyneillä aikuisilla on todettu enemmän joitakin sydän- ja verisuonisairauksien riskitekijöitä kuin heidän täysaikaisena syntyneillä ikätovereillaan. Näistä eniten on tutkittu etenkin kohonneen verenpaineen ja heikentyneen sokerin siedon esiintyvyyttä, mutta kaikkia myöhempien sairauksien riskitekijöitä ei tunneta. Suurin osa aiemmista keskostutkimuksista on tehty hyvin tai erittäin ennenaikaisesti syntyneillä, vaikka yli 80% keskosista syntyy lievästi ennenaikaisena. Ei ole juurikaan tutkimuksia siitä, ovatko sydän- ja verisuonitautien riskitekijät lisääntyneet myös tässä suuressa lievemmin ennenaikaisesti syntyneiden joukossa. Ennenaikaisella syntymällä sinänsä on pitkäaikaiset vaikutuksen syntyneen lapsen terveyteen myös nuoruudessa ja aikuisuudessa: Hyvin pienipainoisena ennenaikaisesti syntyneillä on korkeampi lepoenergian kulutus rasvatonta painoyksikköä kohden kuin täysiaikaisena syntyneillä ikätovereilla. Hyvin ennenaikaisena (ennen 34. raskausviikkoa) syntyneillä tytöillä on 16-vuotiaina korkeampi verenpaine, ja pojilla suuremmat LDL-kolesterolin ja apolipoproteiini B:n pitoisuudet. Keskosena syntyneet puolestaan täyttivät aikuisina todennäköisemmin lihavuuden, verenpainetaudin ja metabolisen oireyhtymän kriteerit. Perinteisten sydän- ja verisuonitautien riskitekijöiden lisäksi heillä oli muutoksia myös monissa muissa sydän- ja verisuonitautien merkkiaineissa, kuten uraatin ja maksa-arvojen pitoisuuksissa. Correlates of Vitamin D Status in Healthy Older Adults Living in Miami-Dade and its Association with Non-Skeletal Outcomes: A Cross-Sectional Study

Examining factors that affect vitamin D status in the fast-growing elderly population of Miami-Dade, Florida, is needed. Vitamin D deficiency in older adults has been linked to correlates of disability, including falls and fractures, and cardiovascular disease. The purpose of this study was to determine the proportion of vitamin D insufficient individuals and their relationship with vitamin D insufficiency in older adults (n=97) living in Miami-Dade. We evaluated the association between vitamin D status and 1) dual task physical performance to understand the link between vitamin D and cognition in the context of mobility; and 2) cardiometabolic risk, measured by galvanic skin response, pulse oximetry, and blood pressure to create a composite score based on autonomic nervous system and endothelial function. Participants completed baseline assessments that included serum levels of vitamin D, anthropometrics, body composition, dual task physical performance and cardiometabolic risk. Surveys to evaluate vitamin D intake, sun exposure, physical activity, and depressive symptoms were completed. Spearman’s correlations, independent t-tests, paired t-tests, repeated measures ANOVAs, and multiple logistic and linear regressions were used to examine the relationship of vitamin D insufficiency (25(OH)D /ml) and sufficiency (25(OH)D ≥30 ng/ml) with determinants of vitamin D status, dual task physical performance variables and cardiometabolic risk scores. Although the proportion of vitamin D insufficient individuals was lower when compared to the prevalance of the general United States elderly population, it was still common in healthy community-dwelling older adults living in Miami-Dade County, especially among Hispanics. Factors that affected skin synthesis (ethnicity, and sun exposure), and bioavailability/metabolism (obesity) were significant predictors of vitamin D status. Vitamin D insufficiency was not significantly correlated with worse dual task physical performance; however, cognitive performance was worse in the vitamin D insufficient group. Our results suggest a relationship of vitamin D insufficiency with executive dysfunction, and support an association with cardiometabolic risk using an innovative electro-sensor complex, possibly by modulating autonomic nervous system activity and vascular function, thus affecting cardiac performance.

Relations and effects of dietary protein and body composition on cardiometabolic health

<p>Obesity has ascended to become the primary modifiable cause of death in the United States. New evidence has called into question the utility of BMI – the typical index of obesity – in predicting cardiometabolic disturbances. The distribution of body fatness may be just as important as the total quantity. Intermuscular adipose tissue (IMAT) has emerged as a distinct subset of adipose in skeletal muscle that may be particularly metabolically deleterious. Typically, sections of either the calf or thigh are used as proxy measurements for whole-body IMAT in investigations. However, IMAT dispersion may not be consistent across tissues, instead infiltrating specific muscle or muscle compartments, and these have may have different metabolic consequences. The study described in Chapter 2 was designed to address this possibility and investigate and compare associations among thigh and calf IMAT stores with indices of cardiometabolic health. The strength of the relationship between IMAT and glucose control-related indices of cardiometabolic health was dependent upon anatomic location. Specifically, thigh IMAT is a better predictor of cardiometabolic risk that calf IMAT. </p> <p>Skeletal muscle has gained increased recognition in recent years for its importance in promotion of health and wellness throughout the life course. While treatment models addressing issues of declining muscle mass and strength with age previously focused on older adults, the importance of utilizing a life course model to promote skeletal muscle health at all ages was more recently recognized. There is consistent evidence that higher-protein diets modestly improve body composition. However, women are at greater risk for not meeting protein requirements and seem to be less willing to adopt strategies to achieve greater protein intake, such as protein supplementation, for fear that it may cause ‘bulkiness’. Therefore, the study described in Chapter 3 was designed to critically evaluate the effect of whey protein supplementation on body composition changes in women via a systematic review & meta-analysis of published randomized controlled trials. It was hypothesized that whey protein supplementation would moderately improve body composition but would not cause excessive muscle hypertrophy. Consistent with our hypothesis, whey protein supplementation improved body composition by modestly (<1%) increasing lean mass, without influencing fat mass.</p> <p>Dietary protein and skeletal muscle are conceptually inseparable; protein is often only considered in terms of how it impacts skeletal muscle-related outcomes. However, it is of interest to determine if the proposed beneficial effects of increased dietary protein consumption extend beyond skeletal muscle. Consumption of higher protein diets result in lower resting blood pressure, but the potential for protein to attenuate acute exercise blood pressure responses is unclear. The study described in Chapter 4 was designed to investigate the effects of meals with different amounts of protein on blood pressure responses to exercise in a randomized, cross-over trial. We hypothesized that consuming the higher-protein meal would attenuate the blood pressure responses to exercise and result in a more robust post-exercise hypotensive response. Contrary to our hypothesis, a higher-protein meal does not attenuate exercise-induced blood pressure responses compared to a lower-protein meal. These findings build upon previous research suggesting that the beneficial effect of chronically elevated protein intake on blood pressure is typically not observed in an acute setting by extending these findings to encompass blood pressure responses to acute responses to exercise.</p> <p>The three studies packaged herein utilize different techniques and report on different outcomes, but conceptual threads unite these works which augment the collective findings. Future researchers investigating the effects of protein on skeletal muscle anabolism can: 1) learn of the importance of proper reflection on surrogate measures and potential for anatomic-specific effects from the IMAT findings (Chapter 2), 2) appreciate the relevance of energy and training states in modulating responses from the WP meta-analysis (Chapter 3), and 3) recognize the importance of holistic approaches and employing challenges to reveal heterogeneity from the protein and BP trial (Chapter 4). Taken together, the research presented in this dissertation forwards our understanding of the relations and effects of dietary protein with different components of body composition on cardiometabolic health. </p>

Impact d’une intervention nutritionnelle précoce pendant les traitements du cancer sur la qualité de la diète des enfants

Les survivants du cancer pédiatrique présenteront, au cours de leur vie, des séquelles cardiométaboliques liées à leur maladie et aux traitements qu’ils ont reçus. Nous pensons qu’il est possible d’améliorer la qualité de leur alimentation et leur profil de santé cardiométabolique en intervenant dès le début de leur parcours en oncologie. Ce projet d’inscrit dans l’initiative VIE (Valorisation-Implication-Éducation) dont l’objectif est d’offrir un support aux patients et leur famille afin de prévenir les effets secondaires aigus et tardifs des traitements antinéoplasiques. Objectifs. Évaluer l’impact d’une intervention nutritionnelle précoce en oncologie pédiatrique sur la qualité alimentaire des patients. Évaluer l’évolution des indicateurs de santé cardiométabolique après un an d’intervention nutritionnelle. Méthodologie. Soixante-deux enfants traités en oncologie au CHU Ste-Justine ont été recrutés dans l’étude moins de 12 semaines après leur diagnostic et ont été suivis tous les deux mois par une nutritionniste de recherche. Les données ont été collectées à l’évaluation initiale et après un an d'interventions nutritionnelles auprès de 36 patients. L’intervention nutritionnelle consistait en accompagnement individualisé ayant pour but d’encourager une alimentation équilibrée durant le traitement du cancer chez l’enfant. La collecte de données comprenait des journaux alimentaires, des informations cliniques, dont la pression artérielle, des prélèvements sanguins et des prises de mesures anthropométriques. La qualité alimentaire était déterminée à l’aide des scores de qualité alimentaire : Diet Quality Index (DQI) et Healthy Diet Indicator (HDI) calculés à partir des données provenant des journaux alimentaires. Les paramètres anthropométriques et biochimiques ont été comparés aux normes pour les enfants en bonne santé du même sexe et du même âge afin de déterminer l’évolution de leur profil cardiométabolique. Résultats. Lors de l'évaluation initiale, l'âge moyen était de 7,9 ans, 52,8% étaient des garçons et 50% souffraient de leucémie aiguë lymphoblastique. Le nombre moyen de suivis était de 4,72  1,06. Par rapport à l’évaluation initiale, dont la moyenne et l’écart-type sont de 48.53 ± 10.45 sur une possibilité maximale de 100 points, la différence avec l’évaluation initiale était de 5,22 ± 9,95 points (P= 0,003) et de 4,75 ± 12,98 points (P= 0,04) pour l’évaluation de 6 mois. De même, la proportion de participants ayant une adhésion modérée et forte (par rapport à une faible adhésion) au score HDI était plus importante après six mois (n= 12 ; 35,29% ; P= 0,027) et un an d’intervention (n=14 ; 39% ; P= 0,012) que lors de l’évaluation initiale (n=5 ; 13,89%). Entre l’évaluation initiale et l’évaluation effectuée après un an d’intervention, une augmentation du score Z moyen du poids et de l’IMC et des concentrations moyennes de HDL-C et du 25-hydroxy vitamine D ont été observés. Conclusion. Dans l'ensemble, cette étude confirme qu'une intervention nutritionnelle d'un an, déployée tôt après le diagnostic de cancer pédiatrique, permet d'améliorer la qualité de l’alimentation des enfants. / Pediatric cancer survivors will have lifelong cardiometabolic sequelae related to their disease and their treatments. We believe that improving their dietary quality and cardiometabolic health profile is possible by intervening early in their oncology journey. This project is part of the VIE (Valorisation-Implication-Éducation) initiative, which aims to support patients and their families to prevent antineoplastic treatments' acute and late side effects. Objectives. Evaluate the impact of an early nutritional intervention in pediatric oncology on the nutritional quality of patients. To assess changes in cardiometabolic health indicators after one year of nutritional intervention. Methodology. Sixty-two children treated in oncology at CHU Ste-Justine were recruited less than 12 weeks after their cancer diagnosis and were followed every two months by a registered dietitian. The nutritional intervention consisted of individualized support to encourage a balanced diet during childhood cancer treatment. Data was collected at baseline and after one year of nutritional intervention for 36 participants. Data collection included food diaries, clinical information including blood pressure, blood sampling and anthropometric measurements. Dietary quality was determined using the Diet Quality Index (DQI) and Healthy Diet Indicator (HDI) scores calculated from data obtained from food journals. Anthropometric and biochemical parameters were compared with sex and age norms for healthy children to determine their cardiometabolic profile evolution. Results. At initial assessment, the mean age was 7.9 years, 52.8% were male, and 50% had leukemia. The mean number of dietitian follow-up visits during the intervention was 4.72  1.06. Compared with the initial assessment, which had a mean and standard deviation of 48.53 ± 10.45 out of a maximum possibility of 100 points, the difference with the initial assessment was 5.22 ± 9.95 points (P = 0.003) and 4.75 ± 12.98 points (P = 0.04) for the 6-month assessment. Similarly, the proportion of participants with moderate and strong adherence (vs. low adherence) to the HDI score was greater at six months (n= 12; 35.29%; P=0.027) and one year evaluation (n=14; 39%; P= 0.012) than at the initial assessment (n=5; 13.89%). Between the initial and 1-year assessments, an increase in mean Z-score weight and BMI and mean HDL-C and 25-hydroxy vitamin D concentrations were observed. Conclusion. Overall, this study confirms that a 1-year nutrition intervention deployed early after diagnosis of pediatric cancer improves children's diet.
