Avaluació de factors mol·leculars associats a la resposta a anticossos monoclonals anti-EGFR: integració amb tractaments de radioteràpia en càncer de cap i coll

Antecedents: Radioteràpia i cisplatí concomitants poden obtenir un alt control loco-regional i supervivència al càncer de cap i coll (CCC) localment avançat, particularment a la orofaringe, en casos VPH positius i en no fumadors. Diverses revisions recents qüestionen el paper de cetuximab i radioteràpia en aquesta població, suggerint resultats inferiors, cosa que requereix confirmació addicional. El CCC VPH negatiu sembla obtenir encara pitjors resultats amb les estratègies vigents. Aquest treball investiga les associacions existents entre la informació clínica de llarg seguiment d'una cohort de CCC localment avançat i la relacionada amb la senyalització de HER3 i PI3K/mTOR en aquesta mostra. Material i mètodes: S'han avaluat 106 casos tractats amb radioteràpia i cisplatí (n=70) o anti-EGFR (n=36). En 84 d'aquests casos s'ha pogut avaluar l'estat VPH, i 46 tenien material suficient per l'estudi de marcadors. Els resultats s'han analitzat amb el mètode de Kaplan-Meier, el model de Cox i metodologia de riscos competitius. Resultats: El factor més influent en els resultats clínics ha estat el fàrmac administrat, amb diferències clares en tots els paràmetres avaluats (FLR, SLP, SG i SE). Mentre els resultats obtinguts amb cisplatí són similars als d'altres sèries les dades amb anti-EGFR són bastant inferiors. Confirma l'aspecte rellevant de l'efecte dels anti-EGFR en la FLR, essent aquesta la causa més rellevant de fallida a distància. Val a dir que una majoria àmplia d'aquesta mostra era VPHneg , que la presència de tumors d'orofaringe no era massa alta, i que la presència de tumors T4 i fumadors era bastant superior a la d'altres sèries. Pel que fa als marcadors avaluats, indiquen que la ruta PI3K/mTOR és biològicament activa i que s'associa a pitjor pronòstic; que pS6, un element distal d'aquesta ruta, podria ser un indicador d'activitat; la baixa presència de mutacions de PIK3CA (molts casos són VPH negatius); la potencial implicació de HER3 com a mecanisme compensatori; i la disfunció de p53. Conclusions: Els resultats han estat clarament millors, tot i que subòptims, amb radioteràpia i cisplatí en una mostra predominantment VPH negativa. És necessari seguir investigant la millor manera d'integrar els agents anti-EGFR en aquests tumors. Seria d'interès poder avaluar l'efecte clínic del bloqueig de mTOR i de HER3, i la utilitat de pS6 com a marcador d'activitat al CCC VPH negatiu. / Background: Concomitant radiation and cisplatin can achieve high loco-regional control and survival in locally advanced head and neck cancer (HNC), particularly in the oropharynx, in HPV-positive tumors and non-smokers. Recent publications challenge the role of cetuximab plus radiation in this population suggesting inferior outcomes, which needs to be further confirmed. HPV-negative HNC seems to do far worse with current strategies. This study evaluates potential associations between long-term clinical data of a locally advanced HNC cohort and HER3-PI3K/mTOR signaling in that cohort. Material and Methods: 106 HNC patients received definitive treatment with radiation and cisplatin (n=70) or anti-EGFR monoclonal antibody (n=36). 84 patients had tissue available for HPV analysis, and 46 for the biomarker study. Outcomes were analyzed by the Kaplan-Meier method, Cox model and competing risks analysis tools. Results: The strongest factor influencing clinical outcome was the concomitant systemic treatment administered, with significant differences in all the endpoints evaluated (loco-regional failure, progression-free survival, overall survival, specific survival). While cisplatin obtained similar outcomes to those in other series, results with anti-EGFR were clearly inferior. This adds to evidence that anti-EGFR contribute to loco-regional failure (LRF), being LRF the main cause of distant failure. Key characteristics in this sample were: mostly HPV-negative tumors, moderate number of oropharynx cancers, and major presence of T4 tumors and smokers. With regard to biomarkers, PI3K/mTOR signaling is active and confers worse prognosis; pS6 (downstream of this pathway) may indicate an active signaling state; low presence of PIK3CA mutations (being most HPV-negative); potential role of HER3 as a compensatory mechanism; and p53 dysfunction. Conclusions: Radiation and cisplatin achieved better outcomes in this study, being suboptimal and in a predominantly HPV-negative sample. Aditional efforts are needed to best integrate anti-EGFR agents in HNC. Exploratory analyses suggest that mTOR and HER3 blockade may positively affect clinical outcome in HPV-negative HNC.

Del liderazgo transformacional al liderazgo prosocial: evidencias obtenidas en la dirección de organizaciones sanitarias catalanas

Les investigacions en el camp de lideratge, independentment de les diferents escoles, enfocaments o teories, coincideixen en que no hi pot haver una empresa, centre, equip o organització amb èxit, sense un bon lideratge. A més aquest és també essencial per al manteniment i l'eficàcia de les organitzacions. Aquest estudi intenta basar-se en el Lideratge Transformacional per fer una aproximació empírica al Lideratge Prosocial, (Roche, 2015), centrat en el sector sanitari català des d’una perspectiva quantitativa. Presentant aquests objectius: Determinar la diferència i la semblança o la complementarietat de les dimensions del Lideratge Transformacional i del Lideratge Prosocial i Determinar com es relacionen amb les corresponents variables de resultat: Satisfacció en el Treball, Esforç Extra i l'Eficiència del Líder amb els dos diferents lideratges: Transformacional i Prosocial en directius i empleats. Per intentar arribar a aquests objectius vam aplicar qüestionaris MLQ (Qüestionari Lideratge Multifactorial) i el PLQ (Qüestionari Lideratge Prosocial). A directius i empleats dels dos consorcis hospitalaris de Catalunya que es componen d'un total de 14 organitzacions sanitàries Tenint en compte les característiques d'aquestes organitzacions de salut i el que ens dibuixen els resultats obtinguts en aquests dos consorcis hospitalaris, podem dir que es tracta d’un entorn organitzatiu que permet l'estudi i implementació del Lideratge Prosocial com un bon model de gestió. Els resultats obtinguts (per les similituds que hi ha entre les dimensions nuclears d’un i de l’altre, i des de les característiques que hauria de tenir el líder transformacional) també ens permeten afirmar que el Lideratge Transformacional té les bases per arribar a obtenir un Lideratge Prosocial, concloent que un líder transformacional podria esdevenir líder prosocial i a la inversa actuar com un líder prosocial ja implica un lideratge transformacional. Basat en els resultats, podem reflexionar sobre la importància d'un canvi de paradigma en la gestió directiva en el context de la sanitat catalana, finalment assenyalant quina hauria de ser la línia de recerca futura. / Las investigaciones en el campo del liderazgo independientemente de las diferentes escuelas, enfoques o teorías, coinciden en que no puede existir una empresa, centro, equipo u organización exitosa, sin un buen liderazgo. Además éste es fundamental para el mantenimiento y eficacia de las organizaciones. En este estudio se intenta partir del Liderazgo Transformacional, para realizar una aproximación empírica al Liderazgo Prosocial (Roche, 2015), enfocándolo en el sector sanitario catalán desde una perspectiva cuantitativa. Plateándonos estos objetivos: Determinar la diferencia, similitud o complementariedad de las dimensiones del Liderazgo Transformacional y del Liderazgo Prosocial y determinar, cómo se relacionan las correspondientes variables de resultado: Satisfacción en el trabajo, Esfuerzo Extra y Eficacia del Líder, con los dos Liderazgos: Transformacional y Prosocial, tanto en directivos/as como en empleados/as. Para intentar llegar a estos objetivos aplicamos los cuestionarios MLQ (Cuestionario Multifactorial del Liderazgo y el PLQ (Cuestionario de Liderazgo Prosocial) a empleados/as y a directivos/as de dos Consorcios hospitalarios de Cataluña que están compuestos por un total de 14 organizaciones sanitarias. Teniendo en cuenta las características de estas organizaciones sanitarias y lo que nos dibujan los resultados obtenidos de estos dos consorcios hospitalarios, podemos afirmar que se trata de un entorno organizacional que posibilita el estudio e implementación del Liderazgo Prosocial como un buen modelo de gestión. Los resultados alcanzados (por las similitudes que hay entre las dimensiones nucleares de uno y otro, y desde las características que debería tener el líder transformacional) nos permiten también afirmar, que el Liderazgo Transformacional tiene las bases para llegar a obtener un liderazgo Prosocial, concluyendo que un líder transformacional podría devenir líder prosocial, y actuar como un líder prosocial ya implica Liderazgo Transformacional. A partir de los resultados reflexionamos sobre la importancia de un cambio de modelo en la gestión directiva en el contexto sanitario catalán, apuntando finalmente cuál debería ser la línea de investigación futura. / The research in the field of leadership, regardless of the different schools, approaches or theories, agree that there can not be a company, center, team or organization successful, without good leadership. In addition this is fundamental to the maintenance and effectiveness of organizations. In this study, there is an attempt based in the transformational leadership to carry out an empirical approximation to the Prosocial Leadership (Roche, 2015) focusing on the Catalan health sector from a quantitative perspective, with these objectives: to determine the difference, similarity or complementarity among the dimensions of the transformational leadership, and those of the prosocial leadership and determine how to relate the corresponding outcome variables: job satisfaction, extra effort and effectiveness of the leaders in the two different leadership taking in account both samples: managers/as well as employees. In an attempt to reach these goals we apply questionnaires MLQ (Multifactorial Questionnaire of the Leadership) and the PLQ (Prosocial Leadership Questionnaire) to employees and directors of two hospital consorcia of Catalonia, which are composed of a total of 14 health care organizations. In view of the characteristics of these health care organizations and what we draw the results of these two consortiums hospital, we can say that it is an organizational environment that allows the study and implementation of the Prosocial Leadership as a good model of management. The results achieved (by the similarities that exist between the nuclear dimensions of one and another, and from the characteristics that should have the transformational leader) allow us to affirm that transformational leadership has the basis for a further prosocial leadership, concluding that a transformational leader could become prosocial leader, and a prosocial leader already implies prosocial transformational leadership. On the basis of the results we consider the importance of a possible shift of paradigm in the directive management in the Catalan health context, pointing at what should be the line of future research.

Capacidad funcional, marcha y descripción del músculo en individuos ancianos

Introducción: El envejecimiento es uno de los mayores retos de salud a los que nos enfrentamos. Fragilidad y sarcopenia son dos conceptos íntimamente relacionados con el envejecimiento y ampliar el conocimiento sobre ellos permitirá desarrollar estrategias específicas preventivas y terapéuticas para minimizar sus consecuencias a nivel sanitario, económico y social. Los objetivos principales de este estudio fueron determinar las características propias de marcha y equilibrio relacionadas con el estado de fragilidad y determinar las características musculares ecográficas y sonoelastográficas del músculo en individuos frágiles. Material y Método: Se diseñaron dos estudios complementarios observacionales transversales en ancianos robustos, prefrágiles y frágiles aplicando los criterios de fragilidad de Fried. Para el estudio de la marcha y equilibrio se incluyeron 62 individuos. Se analizaron los parámetros temporoespaciales de la marcha utilizando una plataforma de presiones incluida en una cinta sin fin. El equilibrio se valoró mediante posturografía, baropodometría y test clínicos. Para el estudio del músculo por ecografía y sonoelastografía se incluyeron 48 ancianos. Para la validación de la técnica ecográfica, se realizó un estudio de concordancia intra- e inter-observador en una población de 15 jóvenes. Resultados: Los ancianos frágiles presentan un aumento de la fase de apoyo (p<0,05) y de doble soporte (p=0,004), disminuyen la fase de oscilación (p<0,05), la longitud del paso (p=0,004) y de zancada (p=0,005), y la velocidad de marcha (p=0,001). Presentan también un aumento de la velocidad de balanceo del centro de presiones (p<0,001) y peores resultados de los test clínicos (p<0,001). Los ancianos frágiles presentan menor grosor (p=0,019), área seccional (p=0,049) y compresibilidad muscular (p=0,034) que el resto ancianos. Se observa la tendencia a aumentar la ecogenicidad muscular (p=0,087) con la fragilidad y a la aparición con ella de zona de alta dureza en el elastograma (p=0,095). Conclusiones: Los ancianos frágiles presentan cambios en los parámetros temporoespaciales de la marcha, cambios en la capacidad de equilibrio y en las características ecográficas y sonoelastográficas musculares. Estos hallazgos sugieren la posibilidad de diseñar intervenciones específicas para mejorar los déficits de la población anciana frágil. / Introduction: Aging is nowadays one of the biggest challenges in terms of health. Frailty and sarcopenia are two concepts closely related to aging. Increasing knowledge about them, would allow us to develop specific preventive and therapeutic strategies to minimize the consequences on health, economics and social resources. The main objectives in this study were to determine the gait and balance characteristics related to the frailty state and to determine the sonographic and sonoelastographic features of the muscle in frail subjects. Methods: Two complementary observational studies were designed in robust, prefrail and frail elderly subjects following the Fried Frailty criteria. 62 subjects were included for the gait and balance study. The spatiotemporal parameters of gait were analyzed using posturography, baropodometry and clinical test. For the study of the muscle sonography and sonoelastography, 48 elderly subjects were selected. A study of intra- and inter-observer concordance was conducted in a sample of 15 young volunteers to validate the ultrasound technique. Results: Frail subjects show an increase of the stance phase (p < 0,05) and of the double support (p=0,004), a decreased swing phase (p <0.05), decreased step length (p = 0.004), decreased stride length (p = 0.005), and decreased gait speed (p = 0.001). They also show an increase of the average velocity of the center of pressure (p < 0,001) and worse results of the clinical test (p <0.001) compared to prefrail and robust subjects. The frail population show a decreased muscle thickness (p = 0.019), less cross-sectional area of the muscle (p = 0.049) and less muscle compressibility (p = 0.034) compared to the rest of the elderly subjects. There is also a tendency to increase echogenicity of the muscle (p=0,087) and to the appearance of high hardness areas on the elastogram (p=0,095) in relation to the frailty phenotype. Conclusions: Frail elderly show changes in the spatiotemporal parameters of gait, in balance ability and changes in the muscle ultrasound characteristics (echogenicity and sonoelastography). These findings suggest the possibility of designing specific interventions to improve the specific deficits of the frail elderly population.

Función cognitiva en la esclerosis múltiple: correlatos neurofisiológicos y herramientas de evaluación neuropsicológica

La esclerosis múltiple (EM) es una enfermedad inflamatoria, desmielinizante y neurodegenerativa del sistema nervioso central y principal causa de discapacidad en el adulto joven. Aunque su causa es desconocida, se ha observado que se desarrolla en personas con una susceptibilidad genética que se exponen a ciertos factores medioambientales. En la historia natural de la enfermedad, las alteraciones cognitivas son frecuentes y se manifiestan en un 40 a 65% de pacientes. Están presentes desde fases iniciales de la enfermedad, hasta las formas progresivas de la misma. Estas alteraciones hacen que los pacientes tengan dificultades para mantener el empleo y que sean dependientes para las actividades diarias. Por esto, es necesaria una evaluación neuropsicológica que permita realizar adaptaciones en el trabajo o desarrollar programas de estimulación cognitiva. Existen varias pruebas para estudiar las alteraciones cognitivas en la EM, sin embargo, muchas requieren de personal especializado y un mínimo de 10 minutos para su aplicación. La presente tesis doctoral plantea que la presencia de deterioro cognitivo es un evento precoz en la evolución de la EM y que puede ser detectado tanto mediante pruebas cognitivas de cribado cómo por estudios neurofisiológicos. La tesis doctoral consta de dos estudios sobre la evaluación cognitiva de pacientes con EM. En el primero, se comparan el SDMT y el PASAT, dos pruebas de cribado de deterioro cognitivo para conocer cuál de las dos es más sensible y específica para detectar alteraciones cognitivas en un seguimiento de un año. En el segundo, se compara el rendimiento de pacientes con EM en fases iniciales de la enfermedad con un grupo control, mediante potenciales relacionados con eventos. En el primer estudio se incluyeron 237 pacientes y 57 controles. Todos los participantes se valoraron inicialmente con una batería neuropsicológica extensa. Después de un año, 196 pacientes se exploraron nuevamente. Los resultados mostraron que el SDMT es más sensible y específico que el PASAT para detectar deterioro cognitivo y que además tiene mejor correlación con el resto de pruebas administradas. Así mismo, se observó que el SDMT es un test más fácil y rápido de administrar y que además puede ser contestado por todas los sujetos. Por otro lado, el PASAT requiere repetir varias veces las instrucciones, haciendo la prueba más larga y se encontró que un 5.3% de los controles no la quiso contestar. En el segundo estudio, se incluyeron 27 pacientes con EM en fases iniciales y 31 controles. Se valoró el sistema de monitorización del desempeño a través de la negatividad relacionada con el error (ERN), que es un potencial cerebral relacionado con eventos que se observa después de un error de comportamiento. También se realizó una exploración neuropsicológica completa. No se observaron diferencias entre los grupos en la mayoría de pruebas neuropsicológicas ni en la tarea ODDBALL auditiva. En la tarea de STOP el desempeño conductual de los pacientes y los controles fue similar, sin embargo, la amplitud de la ERN asociada con los errores de STOP fue significativamente mayor en los pacientes. Esto se observó ya que después de la presentación de una señal de STOP, los errores de comisión estaban asociados con un aumento en la onda negativa. Este hallazgo indica que los pacientes con EM en fases iniciales utilizan áreas cerebrales más extensas para mantener el mismo rendimiento cognitivo que los controles sanos. Los resultados de esta tesis doctoral apoyan, por un lado, la utilización del SDMT como prueba de cribado de deterioro cognitivo en la EM y, por otro, sugieren la presencia de mecanismos compensatorios cerebrales en etapas iniciales de la enfermedad, cuando las alteraciones cognitivas no se manifiestan en una exploración neuropsicológica. / Multiple sclerosis (MS) is an inflammatory, demyelinating and neurodegenerative central nervous system disease and is the principal cause of disability in young adults. Even though its aetiology is unknown, it has been observed that it develops in people with genetic susceptibility that is exposed to certain environmental factors. In the natural history of the disease, cognitive impairment is frequent and may be present in 40 to 65% of the patients. It can be observed from the initial phases of the disease until the progressive forms of MS. Patients with cognitive impairment have difficulties to maintain their job and they usually need help in the activities of daily living. Therefore, a neuropsychological assessment is needed in order to make adaptations in patient’s jobs or to develop cognitive stimulation programs. There are different tests to evaluate cognitive impairment in MS patients, nevertheless, many of them require specialized personnel and minimum 10 minutes to be administrated. This doctoral thesis suggests that the presence of cognitive impairment is a common event in MS evolution that can be detected with cognitive screening tests as well as with neurophysiological studies. The doctoral thesis consists of two studies regarding cognitive evaluation of MS patients. The first one, compares the SDMT and the PASAT, two known screening tests for cognitive impairment in MS, in order to assess which one is more sensitive and specific to detect cognitive impairment after one-year follow-up. The second one, compares cognitive performance of MS patients in early stages of the disease and a control group, using event related potentials. In the first study 237 patients and 57 healthy subjects were included. All participants were assessed at baseline with a comprehensive neuropsychological battery. After one year, 196 patients were assessed a second time. The results showed that the SMDT is more sensitive and specific than the PASAT to detect cognitive impairment and it also has a better correlation with the other tests that were administered. It was also observed that the SDMT is easier and faster to administer than the PASAT and in addition, all the subjects were able to answer it. On the other hand, instructions to answer the PASAT needed to be repeated several times, making the test longer and we found that 5.3% of controls didn’t want to answer it. In the second study, 27 patients with MS in initial phases of the disease and 31 controls were included. The performance monitoring system through the error-related negativity (ERN), that is an event-related brain potential observed following behavioural errors was evaluated. A comprehensive neuropsychological evaluation was also performed. No differences between groups were observed in most of the neuropsychological tests or on the auditory ODDBALL task. In the STOP task behavioural performance of patients and controls was similar, nevertheless, the amplitude of the ERN associated with STOP errors was significantly higher in the patients group. This was observed because after de STOP signal presentation, commission errors were associated with an increase in the negative wave. This finding indicates that patients in early stages of MS use larger brain areas in order to maintain the same cognitive performance than healthy controls. The results of this doctoral thesis support the SDMT as screening test of cognitive impairment in MS patients and suggest the presence of compensatory cerebral mechanisms in the early stages of the disease, when cognitive impairment is not yet observed in a neuropsychological assessment.

Efectos diferenciales de la EFT (Emotional Freedom Techniques) en los patrones narrativos de los pacientes en psicoterapia

L'objecte d'estudi d'aquesta recerca era analitzar els efectes diferencials de l'aplicació de la tècnica de EFT en d'un procés de psicoteràpia cognitiu-constructivista integradora en comparació d'un grup de pacients als quals s'ha aplicat també un procés psicoterapéutic però sense EFT amb la finalitat de poder explorar i comprovar què variables milloren diferencialment, si les hi ha (simptomatologia, benestar subjectiu, funcionament interpersonal i risc) i quins canvis narratius diferencials es produeixen després de la teràpia, si es produeix algun. En aquest estudi han participat 18 clients (17 dones i un home) en el cas de l'aplicació de EFT i sense EFT, 239 persones (191 dones i 48 homes). Els instruments d'evolució que es van aplicar han estat el CORE-OM (adaptat per Feixas et al., 2012), la autocaracterizació (Kelly, 1955), la Graella d'Avaluació Narrativa (Botella, 2008). Els resultats han mostrat que hi havia una gravetat de la demanda inicial del grup de teràpia amb i sense EFT; en els casos dels pacients que rebien psicoteràpia combinada amb EFT, es donava una claredat menor en la meta final valorada, un major grau de conflicte entre objectius diferents així com una menor intel·ligibilitat. / El objeto de estudio de esta investigación era analizar los efectos diferenciales de la aplicación de la técnica de EFT en de un proceso de psicoterapia cognitivo-constructivista integradora en comparación con un grupo de pacientes a los que se ha aplicado también un proceso psicoterapéutico pero sin EFT con el fin de poder explorar y comprobar qué variables mejoran diferencialmente, si las hay (sintomatología, bienestar subjetivo, funcionamiento interpersonal y riesgo) y qué cambios narrativos diferenciales se producen tras la terapia, si se produce alguno. En este estudio han participado 18 clientes (17 mujeres y un hombre) en el caso de la aplicación de EFT y sin EFT, 239 personas (191 mujeres y 48 hombres). Los instrumentos de evolución que se aplicaron han sido el CORE-OM (adaptado por Feixas et al., 2012), la autocaracterización (Kelly, 1955), la Rejilla de Evaluación Narrativa (Botella, 2008). Los resultados han mostrado que había una gravedad de la demanda inicial del grupo de teràpia con y sin EFT; en los casos de los pacientes que recibían psicoteràpia combinada con EFT, se daba una claridad menor en la meta final valorada un mayor grado de conflicto entre objetivos diferentes así como una menor inteligibilidad. / The object of study of this research was to analyze the differential effects of the application of EFT's in a cognitive process of psychotherapy - constructivista of integration in comparison with a group of patients to whom a process has been applied combined psychotherapy but without EFT in order to be able to explore and verify what variables they improve differentially, if they are (symptomatology, subjective well-being, interpersonal functioning and risk) and what narrative differential changes take place(are produced) after the therapy, if someone takes place(is produced). In this study there have taken part 18 clients (17 women and a man) in case of EFT's application and without EFT, 239 persons (191 women and 48 men). The instruments of evolution that were applied have been the CORE-OM (adapted by Feixas et to., 2012), the autocharacterization (Kelly, 1955), the Grid of Narrative Evaluation (Bottle, 2008). The results have showed that there was a gravity of the initial demand of the group of therapy with and without EFT; in the cases of the patients who were receiving psychotherapy combined with EFT, one was giving a minor clarity in the final valued goal; a major degree of conflicto between different goals as well as a minor inteligibility.

En este trabajo se realiza un estudio integral sobre el Síndrome de Asperger y el trato que el mismo recibe a nivel forense y judicial en nuestro territorio. Así, se realiza un estudio sobre el propio Síndrome, intentando comprender mejor su realidad a nivel de diagnóstico, de prevalencia y etiología así como sus comorbilidades y aspectos de carácter cognitivo, para poder avanzar a una mayor comprensión de los aspectos vinculados con el entorno forense en caso de que los afectados por este trastorno se vean implicados en procesos de carácter judicial y/o policial. Por otro lado, se realiza una aproximación a la realidad de las personas afectadas por este Síndrome, teniendo en cuenta su propia visión de sus circunstancias así como la opinión de sus familiares y profesionales especialistas en este Síndrome, a fin de arrojar luz sobre la posibilidad de que este trastorno presente unos elevados índices de cifra negra en cuanto a la implicación en agresiones que no estén siendo notificadas; así como para obtener información sobre procesos de agresión a otros o procesos en los que los afectados hayan sufrido algún tipo de victimización. El objetivo, por tanto, es comprender mejor la realidad del Síndrome para plantear un óptimo afrontamiento del mismo por parte de los agentes implicados en los procesos forenses, policiales y judiciales, generando un sistema que acoja la propia idiosincrasia del Síndrome como parte del proceso. Así, de esta tesis doctoral se puede deducir concluir que existe la posibilidad de realizar una valoración psicológica forense específica en el caso de los afectados por Síndrome de Asperger, que integra a los diferentes agentes implicados en un proceso de victimización o agresividad del afectado, a fin de garantizar que reciba un trato jurídico óptimo y, asimismo, que garantice que se ha realizado un análisis comprensivo de sus situación que incluya elementos esenciales de la Psicológica Básica, Evolutiva, Clínica, Social y Forense. Así, entendemos que esta tesis doctoral debe ser un punto de partida para futuras investigaciones pretendiendo aportar un marco inicial que refleja la realidad del Síndrome en nuestro territorio tanto a nivel demográfico como sociológico, clínico y de comorbilidad asociado a funciones ejecutivas y, sobretodo, específico del ámbito forense. Así, hemos ahondado en elementos que históricamente habían sido debatidos sobre la presencia real de criterios de comorbilidad en el síndrome y su incidencia forense, sobre una posible cifra negra delictiva en estos afectados, sobre si podían tener o no capacidad de testimoniar y bajo que premisas, o sobre si nos hallábamos ante un Síndrome eminentemente violento ante los casos de valoración forense. / This paper presents a comprehensive study of Asperger’s Syndrome and the forensic and legal treatment that receives in our Country. Thus, we focus our study on the syndrome itself, trying to understand its diagnostic reality, prevalence and etiology, comorbidities and cognitive aspects, so we can progress to a greater understanding of the issues related to the forensic, legal or police environment for those affected by this disorder and who are involved in legal processes. On the other hand, we make an approach to the reality of the people affected by this syndrome, focusing in their own view of their circumstances and the opinions of their families and specialists in this syndrome, in order to shed light on the possibility that this disorder present high rates of attacks that are not being reported (in which those affected have suffered some type of victimization or, or that they have committed non reported attacks). The aim, therefore, is to understand properly the reality of the syndrome to create a proper answer for forensics, law enforcement and judicial processes, creating a system that embraces the idiosyncrasies of the syndrome as part of the process. Thus, from this thesis we can conclude that it is possible to make a specific forensic psychological assessment in the case of those affected by Asperger’s Syndrome, integrating the different agents involved in the process of victimization or aggression, to ensure that they receive an optimal legal treatment and also ensure that there has been a comprehensive analysis of the situation involving essential elements of Basic Psychology, Clinical Psychology, Social and Forensics. So, we believe that this thesis should be a starting point for future research, pretending to provide an initial framework that reflects the reality of the syndrome in our territory in demographically, sociologically, clinical and comorbidity areas, associated with executive functions and, above all, the forensic field. So, we have delved into elements that historically had been discussed about the real presence of comorbidity criteria in Asperger’s Syndrome, the forensic impact in non-revealed crimes, under what kind of criteria they have the capacity to testify, or whether we are dealing with a highly violent syndrome in cases of forensic assessment.

Concordança entre avaluadors de la personalitat de subjectes amb discapacitat intel·lectual

L’objectiu d’aquest estudi consisteix en avaluar el grau d’acord entre diferents avaluacions (auto i hetero) de la personalitat de subjectes amb discapacitat intel·lectual (DI). Amb aquesta finalitat s’ha dut a terme un estudi transversal amb múltiples informants en una mostra procedent de tres entitats d’usuaris amb DI. Entre l’any 2010 i el 2012 es van obtenir 61 autoinformes de les persones amb DI, 60 heteroinformes dels familiars (FAM) i 61 heteroinformes de l’educador principal (AV1). En el cas de 22 participants, es van recollir dos heteroinformes més d’altres educadors. Aquesta darrera avaluació, juntament amb la de l’educador principal, va constituir la variable denominada Mitjana d’Avaluadors (AV3). L’instrument utilitzat ha estat el Qüestionari de Personalitat de Zuckerman-Kuhlman (ZKPQ). Tots els heteroavaluadors van completar la versió d’adults del ZKPQ i les persones amb DI van respondre una versió adaptada per aquest col·lectiu. Els valors de tendència central i dispersió de les escales del ZKPQ i de cadascun dels diferents avaluadors ens indiquen que les persones amb DI tenen capacitat per reconèixer aspectes de la seva personalitat, però tendeixen a sobreestimar les puntuacions que s’adjudiquen a si mateixos. Els valors de l’Alpha de Cronbach, indicador de la consistència interna, ens mostren que l’avaluació de l’AV1 i la del FAM són les que ofereixen més garanties psicomètriques. El grau d’acord entre avaluadors es va mesurar mitjançant el Coeficient de Correlació Intraclasse (CCI) de tipus absolut. Els resultats van concloure que hi ha un baix grau d’acord entre l’autoavaluació i la resta d’avaluadors (FAM, AV1 i AV3). Per altra banda, l’acord entre FAM i AV1 va ser baix, mentre que el grau d’acord entre FAM i Mitjana d’Avaluadors (AV3) va ser alt a excepció d’una escala. / El objetivo de este estudio consiste en evaluar el grado de acuerdo entre diferentes evaluaciones (auto y hetero) de la personalidad de sujetos con discapacidad intelectual (DI). Los valores de Alpha de Cronbach, indicador de la consistencia interna nos muestran que la evaluación del EV1 y la del FAM son las que ofrecen más garantías psicométricas. El grado de acuerdo entre evaluadores midieron mediante el Coeficiente de Correlación Intraclase (CCI) de tipo absoluto. Los resultados concluyeron que hay un bajo grado de acuerdo entre la autoevaluación y el resto de evaluadores (FAM, EV1 i EV3). Por otro lado, el acuerdo entre FAM i EV1 fue bajo, mientras que el grado de acuerdo entre FAM y el Promedio de Evaluadores (EV3) fue alta a excepción de una escala. / The aim of this study was to analyze the degree of concordance of multiple informants in the assessment of personality in individuals with intellectual disabilities (ID). For this purpose, a cross-sectional study was carried out with multiple informants (self- and hetero-informants) in a sample from three different associations with ID users. Between 2010 and 2012, 61 self-reports were obtained from people with ID, 60 external reports from relatives (FAM), and 61external reports from the educator that best knew the user (AV1). From 22 of these 61 participants, two more external reports were collected from other educators. These two assessments, together with that of the main educator, constituted the variable called Average of Evaluators (AV3). To assess personality, we used the Zuckerman-Kuhlman Personality (ZKPQ) questionnaire. All external evaluators completed the adult version of the ZKPQ, and people with ID responded to a version adapted for this purpose. Means, standard deviations and internal consistencies of the ZKPQ scales indicate that people with ID are able to recognize aspects of their own personality, but they tend to overestimate the scores they endorse to themselves. The values of Cronbach's Alpha, an indicator of internal consistency, show that the evaluations from the AV1 and the FAM are those that offer more psychometric guarantees. The degree of agreement between evaluators was measured using the Intra-class Correlation Coefficient (ICC). Results indicated that there is a low level of agreement between the self-evaluation and that of the external evaluators (FAM, AV1 and AV3). Furthermore, the agreement between FAM and AV1 was low, while the degree of agreement between FAM and the Average of Evaluators (AV3) was high, with the exception of a single scale.

Abnormal numerical processing in math-anxious individuals: Evidence from event-related brain potentials

This PhD thesis comprises five studies aiming to investigate differences between low math- anxious (LMA) and high math-anxious (HMA) individuals in numeric processing by means of behavioral and event-related potential (ERPs) measures. The excellent temporal resolution of the ERP technique was expected to provide detailed information that would shed light about the difficulties HMA individuals face when they have to deal with numbers. The first study aimed to adapt into Spanish and validate the Shortened Mathematics Anxiety Rating Scale (sMARS; Alexander & Martray, 1989) as a starting point of this thesis, in order to make sure that the construct of math anxiety (MA) was going to be assessed with an instrument providing valid and reliable measures. The adaptation into Spanish of the sMARS scale gave sound evidence of its good psychometric properties: strong internal consistency, high 7-week test-retest reliability and good convergent/discriminant validity. Study II aimed to investigate, with the ERP technique, the use of the plausibility strategy in math-anxious individuals by studying Faust et al. (1996)´s finding on flawed scores for dramatically incorrect solutions (large-split) in an arithmetic verification task. We were able to replicate, for the first time, those findings, by finding a greater percentage of flawed scores for large-split solutions for the HMA group as compared to the LMA one. Moreover, ERP analysis showed that large-split solutions generated a P600/P3b component of larger amplitude and delayed latency for the HMA group as compared to the LMA one. Given the functionality of this component, this finding suggested that large-split solutions demanded more cognitive resources and required more time to be processed for the HMA group than for the LMA one. These findings were interpreted according to the Attentional Control Theory (ACT; Eysenck, Derakshan, Santos, & Calvo, 2007): HMA individuals, being more influenced by the stimulus-driven attentional system, would have succumbed to the distractor nature of the large-split solution, devoting more time (P600/P3b latency) and cognitive resources (P600/P3b amplitude) to process this clearly wrong solution, instead of using the plausibility strategy. Study III consisted of finding the electrophysiological correlates of numeric interference in LMA and HMA individuals, by means of a numeric Stroop task. We found that HMA individuals needed more time to solve this task as compared to their LMA peers, suggesting that they were distracted by the task-irrelevant dimension of the stimuli (i.e. physical size of numbers). ERP data analysis showed that LMA and HMA individuals differed in the way they adapted to conflict: the LMA group presenting a greater N450 component for the interference effect preceded by congruence than when preceded by incongruity while the HMA group showed the same enhancement but for the subsequent Conflict sustained potential. These results suggested that both groups showed a different implementation of attentional control, which was executed in a proactive way by LMA individuals and in a reactive way by HMA ones. A reactive recruitment of attentional control in HMA individuals would have made them more influenced by bottom-up input (i.e. stimulus-driven attentional system), making them more vulnerable to distraction. The two remaining studies of this PhD thesis aimed to explore two possible factors contributing to the development of MA. Given that errors are crucial for mathematical learning, because of its cumulative nature, one concept building on the next, Study IV aimed to assess whether LMA and HMA individuals differed in the way they processed a numeric error as compared to a non- numeric one. We found that HMA individuals showed an increased error-related negativity (ERN) when they committed an error in the numeric Stroop task, but not in the classical Stroop task. Furthermore, standardized low resolution electromagnetic tomography (sLORETA) analysis showed significant greater voxel activation at the right insula for the errors committed in the numerical task as compared to the classical one for the HMA group and not differences at all for the LMA one. Given that the right insula has been associated with the discomfort with one´s own physiological responses and given that errors are considered to generate a cascade of physiological responses, this finding suggests that HMA individuals´ may have experienced a discomfort with the physiological responses generated by a numeric error. This negative bodily reaction towards numeric errors may be at the base of the development of negative attitudes towards mathematics and of the tendency of HMA individuals to avoid math-related situations. Finally, Study V aimed to investigate, by means of an emotional Stroop task, whether MA is characterized by an attentional bias towards math-related information, given that an attentional bias towards threatening information is considered to be a contributory factor in the origin and maintenance of several types of anxiety. This study showed that HMA individuals showed a clear tendency of responding slower to math-related words as compared to neutral words. Given that this slowdown in an emotional Stroop task has traditionally been interpreted as an attentional bias towards threatening or emotional stimuli, this study demonstrates that MA is also characterized by an attentional bias, in this case, towards math-related words, which could probably be at the base of its development and maintenance. To sum up, this PhD thesis has shown that MA is characterized by a vulnerability to distraction, which was shown when a large-split solution was presented for a simple addition task (Study II) and when physical size interfered with numerical magnitude in a numeric Stroop task (Study III). Moreover, HMA individuals also showed a reactive recruitment of attentional control after conflict detection (Study III), a greater sensitivity or emotional response to numeric errors (Study IV) and a clear tendency of an attentional bias towards math-related stimuli (Study V). / Esta tesis doctoral se compone de cinco estudios cuyo objetivo era investigar las diferencias en el procesamiento numérico entre individuos con alta ansiedad a las matemáticas (AAM) y aquellos con baja ansiedad a las matemáticas (BAM) a través de medidas conductuales y de potenciales evocados cerebrales (ERPs). Esperábamos que la excelente resolución temporal de esta técnica nos permitiera obtener información más específica sobre los problemas a los que se enfrentan los individuos con AAM cuando han de procesar números. El primer estudio pretendía adaptar al español y validar la escala sMARS (Alexander & Martray, 1989), como punto de partida de esta tesis, para asegurarnos de que el constructo de la ansiedad a las matemáticas (AM) fuera medido con un instrumento que nos proporcionara medidas válidas y fiables. La adaptación al español de esta escala dio evidencias de sus buenas propiedades psicométricas: alta consistencia interna, alta fiabilidad test-retest de 7 semanas, y alta validez convergente/discriminante. El Estudio II pretendía investigar, con la ayuda de los ERPs, el uso de la estrategia de plausibilidad en los individuos con AAM, estudiando el hallazgo de Faust et al. (1996) en su medida de puntuaciones anómalas (flawed scores) para las soluciones exageradamente incorrectas (large- split solutions). En primer lugar, reproducimos el patrón obtenido por dichos autores. Además, el análisis de ERPs mostró que las soluciones exageradamente incorrectas generaban un componente P600/P3b de mayor amplitud y de latencia más tardía para el grupo de AAM comparado con el de BAM. Dada la funcionalidad de este componente, estos resultados sugirieron que las soluciones exageradamente incorrectas demandaron más recursos cognitivos y requirieron más tiempo para ser procesadas en el grupo de AAM que en el de BAM. Estos resultados fueron interpretados de acuerdo a la Teoría del Control Atencional (ACT; Eysenck, Derakshan, Santos, & Calvo, 2007): los individuos con AAM, estando más influenciados por el sistema atencional ligado a estímulos (stimulus-driven attentional system), habrían sido más vulnerables a la distracción, y habrían sucumbido a la naturaleza distractora de las soluciones exageradamente incorrectas, empleando más recursos cognitivos y más tiempo (reflejado por la amplitud y la latencia del componente P600/P3b) para procesar esta solución implausible, en lugar de utilizar la estrategia de plausibilidad. Por otro lado, el Estudio III consistió en investigar el correlato electrofisiológico de la interferencia numérica en individuos con AAM, por medio de una tarea de Stroop numérico. En este estudio encontramos que los individuos con AAM necesitaban más tiempo para resolver la tarea que los individuos con BAM, sugiriendo que éstos se distraían con la dimensión irrelevante de la tarea (esto es, el tamaño físico). El análisis de ERPs demostró que los individuos con AAM y BAM presentaban una adaptación al conflicto diferente: el grupo de BAM mostró una mayor amplitud del componente N450 para la interferencia precedida por congruencia respecto a la precedida por incongruencia, mientras el grupo de AAM mostró dicho aumento de amplitud, pero para el componente CSP. Estos resultados sugirieron que los grupos implementaban el control atencional de un modo diferente: de una manera proactiva por el grupo de BAM y de una manera reactiva por el grupo de AAM. Un uso reactivo del control atencional en individuos con AAM los habría hecho más influenciables por el sistema atencional ligado a estímulos y, por tanto, más vulnerables a la distracción. Los dos estudios restantes de esta tesis doctoral pretendían explorar dos factores que podrían contribuir al desarrollo de la AM. Dado que los errores son cruciales para el aprendizaje de las matemáticas, el Estudio IV pretendía evaluar si los individuos con AAM y BAM diferirían en la manera en que procesan un error numérico respecto a otro no numérico. En este estudio encontramos que los individuos con AAM mostraron un componente ERN de mayor amplitud cuando cometían un error en la tarea numérica que cuando lo cometían en una tarea no numérica. Además el estudio con sLORETA mostró una mayor activación de la ínsula derecha para los errores cometidos en la tarea numérica respecto a la tarea no numérica, sólo para el grupo de AAM. Dado que la ínsula derecha se ha asociado al desagrado con las respuestas fisiológicas y dado que se considera que los errores generan una cascada de dichas respuestas, este hallazgo fue interpretado como indicador del malestar que los individuos con AAM habrían experimentado respecto a su respuesta fisiológica ante errores numéricos. Esta reacción corporal negativa hacia errores numéricos podría estar en la base del desarrollo de actitudes negativas hacia las matemáticas y de la tendencia de los individuos con AAM a evitar situaciones con contenido numérico. Finalmente, el Estudio V pretendía estudiar si la ansiedad a las matemáticas podría desarrollarse a través de los mismos mecanismos por los que se ha sugerido que se desarrollarían otros tipos de ansiedad investigando, por medio de la tarea de Stroop emocional, si la AM se caracteriza por un sesgo atencional hacia información relacionada con las matemáticas. Este estudio mostró que los individuos con AAM eran más lentos en indicar el color de la tinta de palabras relacionadas con las matemáticas comparado con palabras neutras, mientras que no hubo diferencias en este sentido para el grupo de BAM. Dado que este enlentecimiento en los tiempos de respuesta en la tarea de Stroop emocional se interpreta como un sesgo atencional hacia información emocional u amenazante, este estudio demuestra que la ansiedad a las matemáticas también se caracteriza por un sesgo atencional, que podría jugar algún papel en su desarrollo, mantenimiento o empeoramiento. Para resumir, esta tesis doctoral ha mostrado que la AM se caracteriza por una vulnerabilidad a la distracción, la cual se mostró cuando se presentó una solución exageradamente incorrecta para una tarea de sumas simples (Estudio II) y cuando el tamaño físico interfería con la magnitud numérica en una tarea de Stroop numérico (Estudio III). Además, los individuos con AAM también mostraron un uso reactivo del control atencional tras la detección del conflicto (Estudio III), mayor sensibilidad o respuesta emocional al error (Estudio IV) y un sesgo atencional hacia palabras relacionadas con las matemáticas (Estudio V).

Search for new antiviral compounds using fragment screening methodology

Tesi realitzada a l'Institut de Biologia Molecular de Barcelona (IBMB-CSIC) i a l'Institut de Recerca Biomèdica de Barcelona (IRBB) / Picornaviridae are among the most diverse and oldest known viral families that include many important pathogens of humans and animals. They are small, icosahedral (+)ssRNA viruses, causing a variety of diseases, such as encephalitis, and poliomyelitis. Vaccines are available for poliovirus, hepatitis A virus and foot-and mouth disease virus, but no effective prophylaxis is implemented for other picornaviruses. Thus far, anti-viral research has focused on the capsid, whereas inhibitors targeting non-structural proteins (i.e. proteases, helicases, polymerases) have remained largely unaddressed. The project was focused on structural and biochemical characterization of the enterovirus-B93 (EVB93) 3C protease alone and in complex with several covalent inhibitors. The second objective was to identify the first non-covalent potent inhibitors of the EV-B93 3C protease and their further biochemical, antiviral, and structural evaluation. This work studied the in-vitro proteolytic activity of the EV-B93 3C protease, alone and in the presence of two known covalent inhibitors - rupintrivir and compound 1, as well as three low molecular weight covalent inhibitors - NZO, NZN and DB5_60. The crystal structures of the EV-B93 3C protease alone and in complex with rupintrivir, compound 1, and NZN molecule were solved at high resolution (1.57, 1.50, 1.32, and 1.73 Å, respectively). The structures revealed that the protein adapts a chymotrypsinlike fold similarly to other picornavirus 3C proteases and possesses His-40, Glu-71 and Cys-147 as a catalytic triad. The STD NMR-based fragment screening was performed to select non-covalent binders of the EV-B93 3C protease. Validation and profiling of the most promising non-covalent hits were done using thermal shift assay (TSA), surface plasmon resonance (SPR), and proteolytic activity assay. 44 analogs of the most potent molecule were evaluated in the in-vitro proteolytic activity assay. The most active compound displayed IC50 value of 5 flM. Further chemical optimization was performed resulting in more efficient inhibitor with similar IC50 value. Selected analogs were tested in the in-vitro proteolytic assay against analogous 3C proteases from the following viruses: human rhinovirus-A49, enterovirusD68, aichivirus A, porcine sapelovirus, and equine rhinitis B virus. All compounds exhibited good inhibitory activity against three of the tested proteases. Furthermore, in a cell-based proteolytic assay and an antiviral assay the compounds did not exhibit either proteolytic or antiviral activity, which may be explained by several factors such as lack of cell permeability, low solubility and/or high toxicity. Extensive co-crystallization and soaking trials were performed to obtain crystal structures of noncovalent complexes of the EV-B93 3C protease with the most potent compounds. Regrettably, no additional electron density was identified in the proteolytic active site. Bioinformatics docking simulations suggested potential binding mode of the optimized compound. These pointed to the presumed pockets occupied by the compound that interact with the two conserved residues from the catalytic triad. Since the most potent compound is a relatively large and rigid molecule, it is unable to bind to the protease without its previous rearrangement, which is unfavorable in the crystalline state of the protein. This observation may explain the inability of the non-covalent molecules to co-crystallize with EV-B93 3C protease. The results obtained in this study may aid the design of potent, noncovalent antivirals targeting enteroviral 3C proteases. / Los Picornaviridae son una de las familias de virus más diversas y conocidas desde hace más tiempo. Esta familia incluye importantes agentes patógenos que afectan a humanos y a animales. Los Picornaviridae son virus pequeños, icosaédricos, de ARN de cadena sencilla de sentido positivo y causan una gran variedad de enfermedades, tales como encefalitis y poliomielitis. Se dispone de vacunas para el poliovirus, el virus de la hepatitis A y el virus de la fiebre aftosa, pero no se ha implementado ninguna profilaxis efectiva para otros picornavirus. Hasta ahora, la investigación antiviral se ha centrado en la cápside, mientras que los inhibidores dirigidos a proteínas no estructurales (como las proteasas, las helicasas y las polimerasas) están todavía por explorar. Este proyecto se centró en la caracterización estructural y bioquímica de la proteasa 3C de enterovirus B93 (EV-B93) y de los complejos de esta proteasa con varios inhibidores covalentes. Los estados mentales psicóticos y las anomalías de la experiencia subjetiva: Relación entre los síntomas básicos de Huber de alto riesgo y la sintomatología clínica a lo largo del contínuum de la psicosis

Este trabajo de tesis proporciona importantes contribuciones al conocimiento de un subgrupo de alteraciones llamadas Síntomas Básicos de Huber. El ámbito de investigación es el continuum de la psicosis, un enfoque de máxima relevancia en la investigación de la psicopatología centrado en estudiar las diversas manifestaciones sutiles, preclínicas y clínicas de los trastornos del espectro esquizofrénico. En la parte teórica se ha desarrollado un modelo comprensivo donde se integran diversos componentes del continuum de la psicosis: los rasgos esquizotípicos de la personalidad y las manifestaciones preclínicas y clínicas de la psicosis, incluyendo los Síntomas Básicos. Se clarifica que estos constructos no describen entidades taxonómicas independientes, sino que describen gradaciones en cuanto a desadaptación, duración, frecuencia y necesidad de tratamiento que reflejan un amplio conjunto de factores etiopatogénicos. Esta tesis se focaliza específicamente en la fenomenología subjetiva de los Síntomas Básicos. Aunque han estado tradicionalmente infra-investigados, en los últimos años han sido foco de intensa y prometedora investigación dentro del ámbito de detección precoz de psicosis. A diferencia de la evaluación psicopatológica convencional, focalizada en las manifestaciones clínicas u objetivas de un síntoma, este modelo fenomenológico se centra en la vivencia subjetiva de alteraciones sutiles en la motivación, el afecto, el pensamiento, el habla, la tolerancia al estrés, la percepción corporal o la actividad motora. Por tanto, mediante este modelo, se recupera la “primera persona" del sufrimiento psicopatológico, es decir, se toma en consideración la percepción subjetiva del síntoma por parte del sujeto, además de la evaluación objetiva por parte del clínico. Desde un punto de vista empírico, esta tesis hace aportaciones innovadoras y relevantes en el marco de la psicosis incipiente. Se ha utilizado para la evaluación de los Síntomas Básicos la Schizophrenia Proneness Instrument (SPI-A), un instrumento recientemente elaborado por el grupo alemán líder en el estudio de este campo. La SPI-A está específicamente diseñada para evaluar aquellos Síntomas Básicos de riesgo que han mostrado en estudios prospectivos mayor riesgo para psicosis. Además de la SPI-A, se ha usado una extensa batería de pruebas que evalúan los síntomas psicóticos subclínicos y clínicos convencionales en tres grupos de participantes que reflejan distintos estadios del continuum de la psicosis: participantes de una muestra no clínica, una muestra de participantes con estado mental de alto riesgo para la psicosis, y pacientes con primer brote de psicosis. Los principales resultados de esta tesis indican que los Síntomas Básicos aportan valiosa información acerca de la formación del síntoma psicótico y de la relación entre los planos subjetivos y clínico-objetivos del fenómeno psicótico, especialmente en sujetos con estado mental de alto riesgo. Además, se constata que la fenomenología de los Síntomas Básicos es altamente relevante en la elaboración de nuevas estrategias de evaluación y tratamiento que podrían contribuir a mejorar la detección e intervención precoz de las psicosis. / This thesis provides important contributions to the knowledge of a subset of disorders called Basic Symptoms of Huber. The field of research is the continuum of psychosis, an approach highly relevant in psychopathology research focused on studying the various subtle, preclinical and clinical manifestations of the schizophrenia spectrum disorders. In the theoretical portion of this work, a comprehensive model is developed in which various components of the continuum of psychosis are integrated: schizotypal personality traits and preclinical and clinical manifestations of psychosis, including Basic Symptoms. It clarifies that these constructs do not describe taxonomic independent entities but describe gradations in terms of maladjustment, duration, frequency and treatment needs that reflect a wide range of etiopathogenic factors. This thesis focuses specifically on the phenomenology of Basic Symptoms. Although traditionally they have been under-investigated, in recent years the Basic Symptom model has been the focus of intense and promising research in the field of early detection of psychosis. Unlike conventional psychopathological assessment, focused on clinical or objective manifestations of a symptom, this phenomenological model addressed the subjective experience of subtle disturbances in motivation, affect, thinking, perception, speech, stress tolerance, body perception or motor activity. Therefore, with this model, the "first person" of psychopathological suffering is recovered; that is, the subjective perception of a symptom by the subject is revealed, and considered alongside clinical or objective assessment. From an empirical point of view, this thesis makes innovative and relevant contributions in the context of incipient psychosis. For the evaluation of Basic Symptoms, the Schizophrenia Proneness Instrument (SPI-A) was used, which is an assessment recently developed by the leading German group to study this field. The SPI-A is specifically designed to assess those Basic Symptoms that have been shown in prospective studies to indicate an increased risk for psychosis. In addition to the SPI-A, an extensive battery of tests assessing conventional clinical and subclinical psychotic symptoms was used in three groups of participants who reflect different stages of the continuum of psychosis: participants of a non-clinical sample, participants with at-risk mental state for psychosis, and patients with a first episode of psychosis. The main results of this thesis show that the Basic Symptoms provide valuable information about the formation of psychotic symptoms and the relationship between subjective and clinical dimensions of psychotic phenomenon, especially in patients with at-risk mental states. In addition, the phenomenology of Basic Symptoms is highly relevant in the development of new strategies for evaluation and treatment that may help improve early detection and intervention of psychosis.
