Therapeutic orientation bias & personal values: A study on Swedish psychologists

Psychodynamic therapy (PDT) and Cognitive behavioral therapy (CBT) are the main Therapeutic Orientations (TO) in Sweden and this study aims to explore the relationship between psychologists personal value preferences and their inclination toward PDT/CBT. Furthermore, the study explored if there is a tendency to explicitly appreciate one TO and depreciate the other (explicit bias) and additionally, if that tendency is associated with personal value preferences. Participants consisted of a voluntary response sample of clinical psychologists in Sweden (N = 151). The subjects answered an online survey advertised in the largest Swedish group for psychologists only on a social network. The survey was comprised from three already existing questionnaires (Portrait Values Questionnaire (PVQ57), Therapeutic orientation questionnaire (TOQ-R) and Therapeutic orientation- and experiences questionnaire (TOES). Results showed that inclination toward CBT was significantly associated with the personal value preferences security and power and that there was a clear tendency of explicit bias. The sample was skewed toward appreciating CBT and depreciating PDT and the personal value preference benevolence was higher for the unbiased group than it was for the biased group. This study shows accordance with previous research with the exception of there being less extensive differences regarding personal value preferences and inclination towards TO. / Psykodynamisk terapi (PDT) och kognitiv beteendeterapi (KBT) är de mest tillämpade terapiorienteringarna (TO) i Sverige och denna studie syftar till att utforska sambandet mellan psykologers personliga värderingspreferenser och deras benägenhet för PDT/KBT. Därtill utforskas huruvida det finns en tendens att uttryckligen uppskatta den ena TO och nedvärdera den andra (explicit bias) och dessutom, om den tendensen är förknippad med personliga värderingspreferenser. Deltagarna bestod av ett frivilligt svarsurval av kliniska psykologer i Sverige (N = 151). Försökspersonerna svarade på en webbenkät som hänvisades till i den största sociala nätverksgruppen för svenska psykologer. Undersökningen använde sig av tre redan existerande frågeformulär. Webbenkäten bestod av tre redan existerande frågeformulär: Portrait Values Questionnaire (PVQ57), Therapeutic orientation questionnaire (TOQ-R) and Therapeutic orientation- and experiences questionnaire (TOES). Resultaten visade att en benägenhet för KBT var signifikant förknippad med personliga värderingspreferenserna säkerhet och makt samt att det fanns en tydlig tendens till explicit bias. Urvalet lutade mot att uppskatta KBT och nedvärdera PDT samt att personliga värderingspreferensen välvilja var högre för gruppen bland psykologer utan bias än de med hög bias. Denna studie visar överensstämmelse med tidigare forskning, men uppvisar mindre skillnader vad gäller personliga värderingspreferenser och föredragen TO.

Faisabilité et efficacité d’une thérapie cognitivo-comportementale pour insomnie et anxiété chez les ainés via e-SPACE : plateforme en ligne pour la promotion de la santé

La population âgée est vulnérable face à des problèmes d’insomnie qui peuvent parfois être accompagnés d’anxiété. La thérapie cognitivo-comportementale pour insomnie (TCCi) est le traitement de première ligne pour l’insomnie mais reste peu accessible. Le développement d’une TCC pour l’insomnie et l’anxiété délivrée sur la plateforme en ligne e-SPACE fournit l’accessibilité d’une intervention novatrice pour les personnes âgées isolées. Les objectifs étaient : 1) d’évaluer la convivialité et l’acceptabilité de e-SPACE 2) vérifier l’efficacité de la TCC en ligne pour l’insomnie et l’anxiété. Ainsi, 92 participants âgés insomniaques répartis aléatoirement dans deux groupes (groupe 1 TCCi en ligne N = 47 et groupe 2 liste d’attente N = 45) ont complété : - Agenda de sommeil, Index de Sévérité de l’Insomnie (ISI) et Inventaire d’anxiété gériatrique (GAI) avant et après thérapie/attente pour vérifier l’efficacité - Questionnaire de satisfaction adapté de l’échelle de mesure de la convivialité (SUS) et du questionnaire du modèle d’acceptation technologique 2 (TAM-2), pour vérifier la convivialité et l’acceptabilité La moyenne de score SUS indique que la plateforme e-SPACE est conviviale. Les questions du TAM-2 confirment que son acceptabilité repose sur sa facilité d’utilisation, son utilité et la démontrabilité des résultats. La diminution significative des scores ISI (F = 19.855, p < 0.001) et GAI (F = 11.332, p = 0.001) uniquement chez le groupe 1 démontre l’efficacité de la thérapie. Cependant, il reste une marge d’amélioration afin que la plateforme soit plus adaptée et plus efficace à traiter les symptômes d’insomnie et d’anxiété pour les aînés. / The elderly population is vulnerable to insomnia problems that can sometimes come along with anxiety. Cognitive-behavioural therapy for insomnia (CBTi) is considered as the first-line treatment for insomnia but is still not widely available. The development of an internet-delivered CBT for insomnia and anxiety on the e-SPACE online platform provides accessibility to an innovative intervention for isolated seniors. The objectives were : 1) to assess the usability and acceptability of e-SPACE 2) to verify the efficacy of the online CBT for insomnia and anxiety. Thereby, 92 elderly participants with insomnia were randomised into two groups (group 1 online CBTi N = 47 and group 2 waiting list N = 45) and completed : - Sleep Diary, Insomnia Severity Index (ISI) and Geriatric Anxiety Inventory (GAI) before and after therapy/waiting to verify efficacy - Satisfaction questionnaire adapted from the System Usability Scale (SUS) and the extension of Technology Acceptance Model (TAM-2) to verify usability and acceptability The means of SUS score indicates that the e-SPACE platform is user-friendly. The TAM-2 questions confirm its acceptability is predominantly based on perceived ease of use, perceived usefulness and the result demonstrability. The significant decrease of ISI (F = 19.855, p < 0.001) and GAI (F = 11.332, p = 0.001) scores in group 1 alone confirms the effectiveness of the therapy. However, there is room for improvement so that the platform is more suitable and more effective in treating the symptoms of insomnia and anxiety for seniors.

Funktionella neurologiska symtom är vanliga och funktionsnedsättande symtom som ofta anses svårbehandlade. En vanlig förklaringsmodell har varit att den drabbade varit med om något stressande och traumatiskt som lett till kroppsliga symtom. Denna förklaringsmodell leder till frågan om psykologisk behandling med fokus på trauma kan påverka funktionella neurologiska symtom. En systematisk litteraturöversikt, i enlighet med riktlinjer från Statens beredning för medicinsk och social utvärdering, har genomförts för att undersöka om psykologisk traumabehandling med kognitiv beteendeterapi och Eye Movement Desensitization and Reprocessing Therapy kan minska funktionella neurologiska symtom vid symtom på komorbid posttraumatiskt stressyndrom. Resultaten av litteraturöversikten består av totalt 10 artiklar, merparten fallstudier. Slutsatsen av litteraturöversikten tyder på att den forskning som gjorts visar på minskning av funktionella neurologiska symtom efter behandling av symtom på posttraumatiskt stressyndrom. Detta forskningsområde är dock väldigt litet och består främst av fallstudier med något bristande kvalitet. Trots detta anser vi att det finns underlag för att rekommendera att det bör finnas ett fokus på erfarenheter av trauma vid bedömning av funktionella neurologiska symtom.

Les enfants qui sont victimes d’agressions sexuelles peuvent développer des symptômes à la suite de cet événement traumatique. Que ces symptômes apparaissent dans les jours qui suivent ou après plusieurs années, il est important d’agir le plus tôt possible pour éviter que ces enfants développent des problèmes susceptibles d’impacter négativement leur fonctionnement et leur bien-être à court et à long terme. La thérapie cognitivo-comportementale est une thérapie qui gagne en popularité dans les dernières années, grâce à plusieurs études qui ont pu démontrer son efficacité dans le traitement de diverses conditions. Elle est identifiée comme une thérapie de choix pour le traitement des victimes d’agressions sexuelles (Sousa-Gomes et al., 2022). L’objectif de cette revue systématique de la littérature est d’analyser l’efficacité de la thérapie cognitivo-comportementale à réduire les symptômes associés à l’agression sexuelle chez les enfants de moins de 6 ans qui en ont été victimes, en comparaison avec l’absence de traitement. La recherche systématique s’est effectuée à l’aide des bases de données PsycINFO, MEDLINE, Social Work Abstracts, Social Services Abstract et Web of Science. Aucune étude ne correspondait à l'entièreté des critères d’inclusion et d’exclusion de cette revue. Cependant, sept études ont été lues en intégralité et ont été discutées. Cette revue permet ainsi de faire le point sur la littérature actuelle quant à l'efficacité de la thérapie cognitivo-comportementale chez les enfants de différents groupes d'âge, ainsi qu’en la comparant à d'autres thérapies. Cette revue ne comparera pas la thérapie cognitivo-comportementale à l’absence de thérapie, car les études qui incluent un groupe contrôle dans ce contexte se font très rares. Puis, il existe très peu d'études portant uniquement sur une population d’enfants d'âge préscolaire, ce qui rend impossible de tirer des conclusions sur les enfants âgés de 0 à 6 ans spécifiquement. Puisque ces deux conditions sont rarement utilisées dans les études, la combinaison de ces deux conditions est encore plus rare. / Children who are victims of sexual abuse may develop symptoms following this traumatic event. Whether these symptoms manifest in the days following or after several years, it is crucial to take action as early as possible to prevent these children from developing problems that could negatively impact their short-term and long-term functioning and well-being. Cognitive-behavioral therapy has gained popularity in recent years, thanks to several studies demonstrating its effectiveness in treating various conditions. It is identified as a preferred therapy for the treatment of victims of sexual abuse (Sousa-Gomes et al., 2022). The objective of this systematic literature review is to analyze the effectiveness of cognitive-behavioral therapy in reducing symptoms associated with sexual abuse in children under the age of 6 who have been victims, compared to the absence of treatment. The systematic search was conducted in the PsycINFO, MEDLINE, Social Work Abstracts, Social Services Abstract, and Web of Science databases. No study met all the inclusion and exclusion criteria of this review. However, seven studies were read in their entirety and discussed. This review provides an overview of the current literature regarding the effectiveness of cognitive-behavioral therapy in children of different age groups, as well as comparing it to other therapies. This review will not compare cognitive- behavioral therapy to the absence of therapy, as studies including a control group in this context are very rare. Moreover, there are very few studies focusing solely on a preschool-aged population, making it impossible to draw conclusions specifically about children aged 0 to 6. Since these two conditions are rarely used in studies, the combination of these two conditions is even rarer.

An Analysis and Critique of Mental Health Treatment in American State Prisons and Proposal for Improved Care

Mental health treatment in state prisons is revealed to be highly variable, under-funded, and systematically inadequate. Existing literature exposes this injustice but fails to provide a comprehensive proposal for reform. This paper attempts to fill that gap, outlining a cost-effective, evidence-based treatment proposal, directly addressing the deficits in care revealed through analysis of our current system. In addition, this paper provides historical overviews of the prison system and mental health treatment, utilizing theoretical perspectives to contextualize this proposal in the present state of affairs. Lastly, the evidence is provided to emphasize the potential economic and social benefits of improving mental health treatment in state prisons. Significant findings suggest a clear financial, legal, and moral incentive for states to address this issue, while the proposal provides a viable method of doing so.

Depression drabbar 10–20% av gravida kvinnor och kan leda till konsekvenser för bådebarnet, modern och samhället. IKBT för depression är en effektiv behandling, däremot har patientersom genomgått IKBT uttryckt behov av ytterligare stöd. Syftet med studien var att undersökaupplevelser av att erhålla extrastödsamtal under IKBT inom DANA-studien. Genom en kvalitativansats genomfördes intervjuer med nio deltagare med depression under graviditeten. Tematisk analysanvändes vilket resulterade i två huvudteman; Individanpassning efter behov och Integrering avbehandling. Resultatet visade att extrastödet utgjorde en möjlighet till flexibel individanpassningsamt ett stöd utifrån att IKBT upplevdes som en krävande behandling. Kommunikation upplevdessom avgörande för helhetsupplevelsen av behandlingen och extrastöd utgjorde en möjlighet till flerperspektiv utifrån tillgång till olika yrkeskompetenser. Resultatet ger en ökad förståelse avupplevelsen av extrastödet och dess funktion, samt vad deltagarna upplever som viktigt för att kunnagenomföra behandlingen. / Depression affects 10-20% of pregnant women and can cause consequences for the child,mother, and society. ICBT for depression is an effective treatment, however, patients that haveundergone ICBT have expressed a need for additional support. The purpose of this study was toexamine experiences of receiving extra-counseling during ICBT within the DANA-study. Through aqualitative approach, interviews were conducted with nine participants with depression duringpregnancy. Thematic analysis was used and resulted in two main themes: Adaptation to individualneeds and Integration of treatment. The result shows that extra-counseling provided an opportunityto adapt the treatment to specific needs, while functioning as a support to the ICBT which wasexperienced demanding. Communication was crucial to the overall experience, and extra-counselingwas a possibility to gain perspective from different professions. This provides increasedunderstanding of experiences of extra-counseling, its function, and what is experienced as importantto accomplish the treatment.

Rapid response in psychological treatments for binge-eating disorder

Objective: Analysis of short- and long-term effects of rapid response across three different treatments for binge-eating disorder (BED). Method: In a randomized clinical study comparing interpersonal psychotherapy (IPT), cognitive-behavioral guided self-help (CBTgsh), and behavioral weight loss (BWL) treatment in 205 adults meeting DSM-IV criteria for BED, the predictive value of rapid response, defined as ≥ 70% reduction in binge-eating by week four, was determined for remission from binge-eating and global eating disorder psychopathology at posttreatment, 6-, 12-, 18-, and 24-month follow-up. Results: Rapid responders in CBTgsh, but not in IPT or BWL, showed significantly greater rates of remission from binge-eating than non-rapid responders, which was sustained over the long term. Rapid and non-rapid responders in IPT and rapid responders in CBTgsh showed a greater remission from binge-eating than non-rapid responders in CBTgsh and BWL. Rapid responders in CBTgsh showed greater remission from binge-eating than rapid responders in BWL. Although rapid responders in all treatments had lower global eating disorder psychopathology than non-rapid responders in the short term, rapid responders in CBTgsh and IPT were more improved than those in BWL and non-rapid responders in each treatment. Rapid responders in BWL did not differ from non-rapid responders in CBTgsh and IPT. Conclusions: Rapid response is a treatment-specific positive prognostic indicator of sustained remission from binge-eating in CBTgsh. Regarding an evidence-based stepped care model, IPT, equally efficacious for rapid and non-rapid responders, could be investigated as a second-line treatment in case of non-rapid response to first-line CBTgsh.
