Na clínica contemporânea, percebe-se que as compulsões, as patologias do agir e a experiência de vazio estão cada vez mais presentes, contaminadas pela pressão de ideais de consumo ou constituídas sob a égide desses ideais, na promessa de completude presente nos valores atuais da cultura. Tais manifestações tornam-se ainda mais evidentes com a erupção de formas de sofrimento marcadas pela repetição de experiências dolorosas que limitam e empobrecem a vida dessas pessoas. Nesse contexto, quadros clínicos como a personalidade borderline e as adicções ganham relevância e exigem dos profissionais do campo da saúde mental produções acadêmicas para maior compreensão e cuidado desses fenômenos psicopatológicos. Frente a isto, o objetivo desta pesquisa consistiu em refletir sobre a relação entre as denominadas personalidades borderline e as adicções, a partir de uma investigação fundamentada em uma leitura psicanalítica. Pretende-se ainda enriquecer os instrumentos teórico-clínicos de que se dispõe, de forma a favorecer uma maior compreensão diagnóstica, imprescindível no manejo desses casos. Para tanto, esses conceitos foram estudados a partir da leitura hermenêutica entendida como construção dialética de conhecimento numa relação de intersubjetividade com as condições de sofrimento que podem ser agrupadas sob as nomeações aqui referidas e ilustradas através da utilização de material clínico. A noção de relação adictiva foi utilizada como conceito articulador, pois comporta elementos centrais da dinâmica psíquica própria das adicções e da personalidade borderline. A partir desse estudo, formulou-se a proposta de compreensão da organização borderline de personalidade como uma modalidade de adicção, cujo objeto é uma pessoa, mesmo que coisificada / Compulsive behavior, acting out pathologies and feelings of emptiness are increasingly frequent in contemporary psychoanalytical practice. Such symptoms are contaminated or even brought forth by the pressure of strongly rooted ideals of current culture regarding consumerism and its void promises of providing inner fulfillment. The result becomes even more evident in the outbursts of modern-day ways of suffering characterized by the repetition of painful experiences that limit and deplete the lives of these patients. Hence the understanding of clinical diagnosis and treatment of disorders such as addictions and borderline personality syndrome becomes an important academic requirement for mental health professionals. The goal of this research is therefore to reflect on the relation between borderline personality and addictions from a psychoanalytical perspective and thus, through the development of both theoretical and clinical knowledge, contribute to improve diagnosis, which is essential for the handling of such cases. In order to achieve such goals, these concepts have been studied from a hermeneutic approach understood as a dialectic construction of knowledge , with great attention to relations of intersubjectivity between them and conditions of suffering that may be brought together under the classification referred here. Additionally, those ideas have also been illustrated by clinical observations. The notion of addictive relations has proved to be a valuable starting point in the articulation of these studies, since it contains essential elements of the mental dynamics of patients suffering both from addictions and borderline personality syndrome. Amor patológico: aspectos clínicos e de personalidade / Pathological love: clinical and personality aspects

O amor patológico caracterizado pelo comportamento de prestar cuidados e atenção ao parceiro, de maneira repetitiva e desprovida de controle, em um relacionamento amoroso é um quadro pouco estudado cientificamente, apesar de não ser raro e de gerar sofrimento importante. Com o intuito de compreender os aspectos clínicos e de personalidade associados ao amor patológico, os seguintes fatores foram analisados: impulsividade, personalidade, satisfação com o relacionamento amoroso, estilo de amor e tipo de apego. Além de revisar a literatura, os dados obtidos, por meio da aplicação de vários instrumentos em 50 sujeitos com amor patológico e 39 sujeitos saudáveis evidenciaram que indivíduos com amor patológico apresentam alta impulsividade (p<0.001; Escala de Impulsividade de Barratt), elevada auto-transcendência, isto é, senso de fazer parte de uma realidade maior (p<0.001; Inventário de Temperamento e Caráter), e manutenção de relacionamento amoroso com baixa qualidade (p<0.001; Escala de Avaliação do Relacionamento Adaptada). Assim, essa população necessita, por parte dos centros de tratamento especializados, de avaliação e abordagem terapêutica efetivas, as quais devem contemplar as características clínicas e de personalidade específicas dessa condição / Pathological love characterized by the behavior of providing repetitive and uncontrolled care and attention to the partner in a romantic relationship is a rarely studied condition, despite not being rare and causing suffering. In order to know the clinical and personality aspects associated with pathological love, the following factors were analyzed: impulsivity, personality, satisfaction with romantic relationship, love style, and type of attachment. Besides revisiting the literature, the data obtained from the application of various instruments in 50 subjects with pathological love and 39 healthy subjects showed that individuals with pathological love present higher impulsivity (p<0.001; Barratt Impulsiveness Scale), elevated selftranscendence, that is, hold sense of communion with a wider reality (p<0.001; Temperament and Character Inventory) and keep dissatisfactory romantic relationships (p<0.001; Adapted Relationship Assessment Scale). Hence, this population needs effective assessment and therapeutic strategies, which must take into account the specific clinical and personality characteristics of the condition

Dada a prevalência estimada na população em geral, potencialmente projetada em até 16% da população adulta, e as severas consequências negativas oriundas de tal patologia, esta tese se propôs a investigar de forma mais precisa as relações entre tais compradores compulsivos e seus objetos de consumo, notadamente as preferências declaradas por tais indivíduos quanto à preferência pela posse, de natureza temporária, ou propriedade definitiva de produtos, traduzidas por meio de situações hipotéticas e experimentais de aluguel e compra, respectivamente, lacuna ainda inexplorada na literatura inerente à oniomania. Considerando esta mesma literatura sobre o comprar compulsivo, em que premissas teóricas constroem uma narrativa baseada na aquisição / compra de produtos (Faber, O’Guinn & Krych, 1987; Glatt & Cook, 1987; Valence, d'Astous & Fortier, 1988; Krueger, 1988; O’Guinn & Faber, 1989; d'Astous, Maltais & Roberge, 1990; Christenson, Faber, de Zwaan, Raymond, Specker & Eckern, 1994; McElroy, Keck Jr, Pope Jr, Smith & Strakowski, 1994; Faber & Christenson, 1996; Black, 2001; 2007; Dittmar, 2005; Koran, Faber, Aboujaoude, Large & Serpe, 2006; Saraneva & Saaksjarvi, 2008; Tavares, Lobo, Fuentes, & Black, 2008; Kukar-Kinney, Ridgway & Monroe, 2009; Trautmann-Attmann & Johnson, 2009; Workman & Paper, 2010; Bonfanti, 2010; Leite, 2011; Kukar-Kinney, Ridgway e Monroe, 2012; Leite, Rangé, Junior & Fernandez, 2012), a investigação experimental proposta baseou-se na ideia de que indivíduos acometidos por tal patologia seriam menos propensos ao aluguel de produtos, hipótese confirmada em todos os testes, sem considerar dadas manipulações. O estudo mostrou também que o efeito da manipulação proposta, fundamentada na saliência da devolução de produtos, acabou por estimular compradores ditos compulsivos a alugarem produtos, contrariando expectativas iniciais, estas baseadas no suposto desconforto que tais indivíduos teriam ao abrir mão de seus objetos de compulsão. Pelo aparente ineditismo, tais resultados foram considerados importantes achados para a literatura e, portanto, foram explicados não somente à luz dos experimentos propostos, como também a partir da etapa qualitativa, de cunho exploratório e interpretativo. / Compulsive buying – henceforth named Oniomania – is part of a broader category of compulsive consumption behavior, which would be based on a repetitive and chronic purchasing, which becomes the first response to events or negative feelings (O'Guinn & Faber, 1989). Given the high prevalence in the general population - potential estimated up to 16% of the adult population - and severe negative consequences, due to such disorder, this thesis investigated the relationship between such shopaholics and their consumption goods, namely the declared preferences by such individuals about the possession, temporary, or definitive ownership of goods, tested through hypothetic and experimental situations of rental and purchase, respectively, considered an unexploited gap in the literature inherent in oniomania. Considering the same literature on compulsive buying, in which theoretical assumptions construct a narrative based on the acquisition, notably, purchase of products (Faber, O’Guinn & Krych, 1987; Glatt & Cook, 1987; Valence, d'Astous & Fortier, 1988; Krueger, 1988; O’Guinn & Faber, 1989; d'Astous, Maltais & Roberge, 1990; Christenson, Faber, de Zwaan, Raymond, Specker & Eckern, 1994; McElroy, Keck Jr, Pope Jr, Smith & Strakowski, 1994; Faber & Christenson, 1996; Black, 2001; 2007; Dittmar, 2005; Koran, Faber, Aboujaoude, Large & Serpe, 2006; Saraneva & Saaksjarvi, 2008; Tavares, Lobo, Fuentes & Black, 2008; Kukar-Kinney, Ridgway & Monroe, 2009; Trautmann-Attmann & Johnson, 2009; Workman & Paper, 2010; Bonfanti, 2010; Leite, 2011; Kukar-Kinney, Ridgway & Monroe, 2012; Leite, Rangé, Junior & Fernandez, 2012), the proposed experiment was based on the idea that individuals affected by this disorder would be less likely to rent products. This hypothesis was confirmed in all tests, without considering manipulations. The study also showed that the effect of manipulation proposal, based on product return information, eventually stimulate named shopaholics to renting more products, contrary to initial expectations, based on supposed discomfort that such individuals would feel at the time they would give up their compulsion objects. Due to the apparent novelty, these results were considered important findings for literature and thus were explained in the light of the proposed experiments and through complementary research of an explanatory and interpretative nature.

Continuing care recommendations associated with ongoing sobriety include going to another level of care (individual therapy, group therapy, outpatient treatment), attending twelve-step meetings and communicating with a sponsor. Continuing care recommendations are typically written and referred to as a continuing care plan.

Etude cellulaire de la genèse et de l'apprentissage d'un comportement motivé chez l'aplysie / Cellular study of the genesis and learning of a motivated behavior in Aplysia

Les comportements motivés tels que les comportements alimentaires ou sexuels sont émis de façon irrégulière sous l’impulsion du système nerveux central. Ils sont régulés par des informations sensorielles et des apprentissages. Dans un apprentissage associatif, le conditionnement opérant appétitif, l’animal apprend les conséquences de son action parl’association d’une action à l’obtention d’une récompense (un stimulus à forte valeur appétitive). Il est établi que cet apprentissage induit la transition d’une motricité initialement peu fréquente et irrégulière en une motricité rythmique, fréquente et régulière. Cependant, les mécanismes cellulaires du système nerveux central qui sont responsables de cettetransition, restent largement méconnus. Notre étude chez le mollusque aplysie nous a permis d’identifier ces mécanismes dans un réseau neuronal identifié et générateur des patterns moteurs du comportement alimentaire. Sur des préparations du système nerveux isolé, nous avons sélectivement contrôlé l’expression fréquente d’une part et régulièred’autre part de la motricité apprise grâce à la manipulation expérimentale de la plasticité fonctionnelle de neurones pacemakers identifiés. Ainsi, nous avons nouvellement établi un lien de causalité entre (1) des modifications membranaires et l’accélération motrice et (2) le renforcement de synapses électriques et la régularité motrice. Nous avons mis en évidence le rôle du transmetteur dopamine dans l’induction de ces plasticités fonctionnelles et l’expression de la motricité fréquente et régulière. Enfin, nous avons analysé les propriétés intrinsèques du neurone responsable de l’impulsion spontanée et irrégulière de la motricité des animaux naïfs. Pour conclure, l’ensemble de ces travaux de thèse offre une vue étendue des mécanismes cellulaires qui déterminent la variabilité d’un comportement motivé et sarégulation par apprentissage. / Motivated behaviors such as feeding or sexual behavior are irregularly expressed by impulsive drives from the central nervous system. However, such goal-directed acts are regulated by sensory inputs and learning. In a form of associative learning, appetitive operant conditioning, an animal learns the consequences of its own actions by making the contingentassociation between an emitted act and delivery of a rewarding (highly appetitive) stimulus. It is now established that this learning procedure induces the transition from an initially infrequent and irregular motor activity to a frequent and regular behavior. However the cellular and central network mechanisms that mediate this behavioral plasticity remain poorlyunderstood. Our study on the marine sea slug Aplysia has allowed us to analyze these mechanisms in an identified neuronal network that is responsible for generating the motor patterns of the animal's feeding behavior. Using in vitro neuronal preparations, we selectively controlled the frequency and regularity of the motor activity induced by operant learning with experimental manipulations of the functional plasticity in identified pacemaker neurons. We found for the first time a causal relationship between the learning-induced plasticity and (1) changes in pacemaker neuron membrane properties and the increased frequency of feeding motor activity, and (2), in the strength of their interconnecting electrical synapses and the regularized phenotype of this motor activity. We then addressed the role of the transmitterdopamine in the induction of this functional plasticity and specifically the expression of a frequent and stereotyped rhythmic feeding motor pattern. Finally, we analyzed the intrinsic membrane properties of the essential pacemaker neuron for generating the irregular motor drive in naïve animals. In conclusion, the data from this thesis work have provided novelinsights into the cellular and synaptic mechanisms underlying the intrinsic variability of a motivated behavior and its regulation by learning.

Stability and change: addressing the symptom of substance dependency

The aim of this study is to demonstrate how the complementary concepts of stability and change could manifest during the therapeutic process, specifically with clients showing the symptom of dependency. The study is guided by a literature study on systems/cybernetic theory with a focus on the cybernetic complementarity of stability and change. A brief description is provided of the symptom of dependency from a more traditional lineal perspective as well as a non-lineal (systemic) perspective. A single case study is utilized to describe how both stability and change could manifest in the therapeutic process. The Perpetual Motion Machine is a collection of creative nonfiction essays about the author and her brother as they have experienced growing up both together and then apart throughout the years of their lives. The essays deal with the pair’s childhood, adolescence and adulthood as well as the issues of depression, anxiety and drug addiction. Some pieces are flash-style and others are longer works of lyric essay or general narrative.
