Frameworks for Patient Safety in the Nursing Curriculum

Patient safety (i.e., the degree to which patients are free from accidental injury) has received a great deal of media coverage during the past few years. Professional and regulatory agencies have indicated that patient safety education should be provided to healthcare workers to improve health outcomes. The primary purpose of this exploratory study was to gain a better understanding of the current status of patient safety awareness among pre-licensure nursing students. To this end, six research questions guided the study: 1. Will interpretable item constructs be identified when responses to the Healthcare Professional Patient Safety Assessment Curriculum Survey (HPPSACS) are intercorrelated and factor analyzed using R-technique exploratory factor analysis? 2. Will responses to items on the HPPSACS yield scores that are intemally consistent as indicated by alpha reliability coefficients? 3. What are the perceptions of nursing students about their awareness, skills, and attitudes regarding patient safety? 4. (a) To what extent is there a relationship between the demographic variables of age and gender and nursing students' perceptions of their patient safety awareness, skills, and attitudes? (b) To what extent is there a relationship between the demographic variable of race/ethnicity and nursing students' perceptions of their patient safety awareness, skills, and attitudes? 5. To what extent is there a relationship between the type of collegiate nursing program and nursing students' perceptions of their patient safety awareness, skills, and attitudes? 6. To what extent are there discernable program curriculum and instructional methodologies that have been traditionally associated with more positive nursing student perceptions of awareness, skills, and attitudes regarding patient safety? Phase I was a pilot test for reliability and construct validity for the HPPSACS. Data were factor analyzed to determine factor constructs for the purpose of identifying the key themes accounting for the variation in response across 23 survey items. Three factors with themes that were found to relate to perceptions of patient safety among a scholarly professional group of nurses were identified as comfort, error reporting, and denial. Findings in Phase II of the study indicated that there were four identifiable constructs with the study data: the themes of comfort, error reporting, denial, and culture. Older male participants had higher comfort subscale scores and lower culture subscales scores than did younger female participants. The Asian American participants were clearly distinguished from the combined set of African American and Hispanic participants on the denial and culture scores. The "other" ethnic identity was clearly distinguished from the combined set of Caucasian and Hispanic participants on the comfort and error reporting scores. The associate nursing degree programs were clearly distinguished from the combined set of the accelerated and traditional nursing degree programs. Findings in Phase III of the study indicated that all seven of the participating nursing schools included at least three of the Institute of Medicine's six core competencies, with one school exhibiting all of the core competencies.

Enterprise Resource Planning in Higher Education: A Comparative Case Study

Case study research on enterprise systems in higher education organizations has shown that the challenges associated with implementing enterprise systems in higher education occur when unique organizational characteristics found in universities do not align with the standard characteristics built into the software programs. Based on such findings, the purpose of this study was to further explore the interaction between higher education organizations and enterprise systems during Enterprise Resource Planning (ERP) implementations in order to gain insight into the effects of ERP implementations in higher education. Through the theoretical lens of actor-network theory, the purposes of this comparative case study at three universities were to identify (a) how higher education organizations re-structured to the standards of the enterprise software, (b) how ERP software was customized in order to adapt to the characteristics of higher education organizations, and (c) how the enterprise software and higher education organizations interacted and translated into a unique identity as a result of ERP implementation. The data for the study were collected through semi-structured interviews and institutional artifacts at three universities which were commonly bound by similar institutional characteristics and the same enterprise software. Further, the study was limited to the examination of the interaction between individuals associated with the registrars' offices at the three institutions and the student module found in each instance of the software. The data revealed that, while the institutions did not organizationally restructure or make policy changes in order to adapt their institutions with the infrastructure of the software, the registrars' offices made many reactionary changes in their business processes, procedures, and nature of work as a result of the enterprise system implementation. The data also revealed that the software customizations, developed to account for unique statutory requirements, caused overwhelming implementation challenges during the enterprise software implementation and post-implementation phases.

Learning Strategies and Leadership Behaviors of Small Business Leaders in Northeast Florida

The intent of this study was to examine the leadership behaviors and learning strategies of Successful Small Business Leaders (SSBLs) in Northeast Florida. Information gathered during this study may help current and future small business owners develop, maintain, and make their organizations more productive through the use of the learning strategies and leadership behaviors employed by SSBLs. Through a two-prong approach, the study examined both leadership and learning. The study addressed the following two questions: 1) What learning strategies are used most by successful small business leaders? and 2) What leadership behaviors are exhibited most by successful small business leaders? The Delphi method was selected as the research approach for this study because it provides the most flexible approach to seeking the perspective of SSBLs. The Delphi method uses a group of experts who anonymously discuss and respond to the research issue. A two-phase Delphi study design utilized both participant surveys and interviews. An analysis of the data collected during Phase 1, or the survey phase, identified seven leadership behaviors and six learning strategies. Examination of the seven leadership behaviors and six learning strategies by the participants demonstrated agreement among the behaviors and strategies identified. However, participants did not reach an agreement in the ranking of the leadership behaviors or learning strategies. The study contributes to the research of leadership behaviors and learning strategies of leaders as the focus was solely on those in a small business setting. The best practices and other data discovered during this Delphi study on small business may be used to better define and understand the characteristics of SSBLs.

L'image du médecin dans la littérature russe du XIXe siècle / The image of the doctor in the 19th century russian litterature

Cette thèse se donne pour enjeu d’étudier la figure du médecin dans la littérature russe du XIXe siècle, à travers deux volets : d’abord la présentation du personnage au sein des différents courants artistiques et ensuite, la formation de son image à travers des données historiques qui mettront en évidence le développement de la pensée scientifique et des théories médicales dominant dans les milieux intellectuels. Dans l’analyse chronologique du personnage, nous nous sommes appuyée sur plus de trente-cinq auteurs et avons sélectionné deux cents œuvres. Ce principe nous a permis de dégager, d’une part son image récurrente à travers le prisme professionnel; d’autre part d’établir les étapes de son évolution dans le système esthétique de chaque auteur. Grâce à l’étude historique des sciences, nous avons établi le lien entre les deux domaines - médecine et littérature. A chaque période marquée par les mutations scientifiques, nous avons voulu démontrer que la littérature s’empare de ces nouvelles données et impose une image du médecin. Les auteurs romantiques, séduits par la vogue des sciences occultes, font naître un héros mystique. Dans la littérature des années 1860, sensibilisée par Les Réflexes du cerveau de Sečenov qui transforme la médecine en symbole des temps nouveaux, le personnage devient actif et vrai porteur des conceptions scientifiques : il figure désormais au centre du sujet narratif. Ainsi cette recherche a-t-elle cerné la spécificité de l’image du médecin qui se forme sous l’influence de la pensée scientifique que les auteurs transposent dans leurs œuvres. Se situant au croisement de la littérature et de la science, le personnage apparaît véritablement comme la métaphore de son époque. / The goal of this work is to study the figure of the doctor in XIXth century Russian literature, through two data: first of all the presentation of the character within the different artistic trends and then, the development of his image through the historic data which highlight the development of scientific thought and medical theories dominating intellectual circles. In the chronological analysis of the character, we based the work on more than thirty five authors and selected approximately two hundred works. This approach allowed us to establish, on one hand a recurrent image through the professional prism; and on the other hand to establish the steps of its evolution in the aesthetic system of each author. Due to the historical study of sciences, we drew up the link between the two fields – medicine and literature. At each period marked by scientific transformations, we wanted to demonstrate that literary output seizes this new data and enforces the image of the doctor. The romantic authors, attracted by the vogue of occult sciences, gave birth to a mystical character. In the literature of the 1860’s, made aware by Reflexes of the Brain by Sechenov who transforms medicine into the symbol of new times, the character becomes active and a real bearer of scientific notions: from now on he features as the center of the narrative subject. Therefore this research determined the specificity of the image of the doctor which is formed under the influence of scientific thinking that the authors adapt in their works. The character, situated at the crossroads of literature and science, truly appears as the metaphor of his era.

This was possible by examining the perception of the subjects in relation to previous professional approaches, investigating the existence of conceptual metaphors that describes the expression of pain and then categorizing them and understanding the individual impacts to be bearer of TMD. It was a qualitative and exploratory study, carried out from June to August, 2007. Have participated 18 subjects of both sexes with age ranging from 14 to 64 years with muscle skeletal temporomandibular pain, assisted in the Center for Orofacial Pain Control of the School of Dentistry at the University of Pernambuco Brazil, selected from the Simplified Anamnestic Questionnaire DMF. For the constitution of the corpus were held semi-structured interviews that were recorded and later transcribed. The discussion of the results was based on analysis of the content and it was divided into three parts. The first referred to the representation that the subjects have of previous treatments and pointed to an attitude of health professionals as predominantly routing the patients to other services without explanation. In the second part were discussed data describing of the experience of pain and it was showed the existence of conceptual metaphors, which can be categorized into orientacional, ontological and personified. Finally, in the third part of the analysis/discussion of the results, were investigated the general and specific impacts caused by the pain in the lives of the subjects. It was found that the TMD causes general impacts predominantly in the leisure and work activities. And among specific impacts, causes relevant inconvenience to chew, to yawn, to make oral hygiene, smiling or laughing, and talking. It was concluded that there is a gap in the process of interaction between health professionals and the subjects carrying TMD, and that the Science of Language, through the study of metaphors, may become an important resource for understanding the experience of pain, assisting in the mitigation of impacts caused by pain in the lives of subjects / A presente dissertação teve o objetivo de estudar os enunciados dos sujeitos portadores de Disfunção Temporomandibular (DTM) acerca da experiência da dor na suas vidas. Isso foi possível analisando a percepção dos sujeitos em relação às abordagens profissionais anteriores, investigando a existência de metáforas conceituais que descreviam a manifestação da dor, e em seguida, categorizando-as e compreendendo os impactos particulares de ser portador de DTM. Tratou-se de um estudo qualitativo e exploratório, realizado no período de junho a agosto de 2007. Participaram 18 sujeitos de ambos os sexos, com faixa etária variando entre 14 e 64 anos, com dor músculo-esquelética temporomandibular, atendidos no Centro de Controle da Dor Orofacial da Faculdade de Odontologia da Universidade de Pernambuco Brasil, selecionados a partir do Questionário Anamnésico Simplificado DMF. Para a constituição do corpus foram realizadas entrevistas semi-estruturadas que foram gravadas e posteriormente transcritas. A discussão dos resultados foi baseada na análise de conteúdo e dividida em três partes. A primeira referiu-se à representação que os sujeitos fazem dos tratamentos anteriores e apontou uma atitude dos profissionais de saúde como sendo predominantemente de encaminhar os pacientes a outros serviços sem explicações. Na segunda parte foram discutidos os dados referentes à descrição da experiência da dor e constatouse a existência de metáforas conceituais, sendo possível categorizá-las em orientacionais, ontológicas e personificadas. Por fim, na terceira parte da análise/discussão dos resultados, foram investigados os impactos gerais e específicos causados pela dor nas vidas dos sujeitos. Verificou-se que a DTM causa impactos gerais sobressaindo no lazer e no trabalho e, entre os impactos específicos, causa transtornos relevantes para mastigar, bocejar, fazer higiene oral, sorrir ou gargalhar e falar. Concluiu-se que existe uma lacuna no processo de interação entre os profissionais de saúde e os sujeitos portadores de DTM, e que a Ciência da Linguagem, através do estudo das metáforas, pode tornar-se um importante recurso para a compreensão da experiência dolorosa, auxiliando na atenuação dos impactos causados pela dor na vida dos sujeitos

Culture-bound shifts in the first french and italian translations of Christopher Marlowe's Doctor Faustus / Les premières traductions françaises et italiennes du Docteur Faust de Christopher Marlowe : Variations selon le contexte socioculturel

La présente thèse compare le drame Le Docteur Faust de Christopher Marlowe (1604, 1616) avec la première traduction française faite par Jean-Pierre Antoine Bazy (1850) et la première traduction italienne faite par Eugenio Turiello (1898) en visant à identifier les changements textuels révélateurs du contexte culturelle et idéologique au moment où se produisent les deux textes cibles. Le Docteur Faust est un exemple emblématique de l’instabilité du texte dramatique source. Il nous est parvenu en deux versions (le texte A et le texte B) différentes du point de vue structurel, thématique et doctrinal. En revanche, aucune version ne permet pas une interprétation cohérente. Ce travail a pour propos d’examiner si les traductions de Bazy et de Turiello, qui proviennent de contextes géographiques, historiques et littéraires différents mais étroitement liés, multiplient les lectures plausibles ou bien si elles aboutissent à une vision plus constante. En outre, on s’interroge sur la cause des variations textuelles, généralement dénommées en traductologie les glissements. Tout d’abord, j’ai identifié une régularité des glissements qui se manifestent dans deux traductions en question. Puis, j’ai analysé les effets des glissements sur la structure et la signification générales des textes. Enfin, en adoptant une approche socioculturelle de l’analyse des traductions, j’ai exploré la manière dont les changements sont déterminés par l’idéologie des traducteurs et leur interprétation de l’original. Cela explique leur position au sein de l’espace politique et idéologique de chaque culture d’arrivée, ainsi que les normes traductrices et culturelles adoptées au cours de la traduction. / The aim of this research is to compare Christopher Marlowe’s play Doctor Faustus (1604, 1616) with the first French translation by Jean-Pierre Antoine Bazy (1850) and the first Italian translation by Eugenio Turiello (1898) in search of the changes that are symptomatic of the cultural and ideological context of translation production. The case of Doctor Faustus represents the epitome of the instability of a dramatic source text. Two main versions of the play (the A-text and the B-text) differ in structural, thematic and doctrinal terms. At the same time, neither version delivers a coherent vision. The research seeks to examine whether Bazy’s and Turiello’s translation, belonging to different yet related geographical, historical and literary traditions, further multiply the potential readings of the original or whether they display a more consistent framework. In addition, we will analyse the causes of textual variation, commonly labelled in Translation Studies as shifts. First, we identified a pattern of shifts manifested in the target texts in question. Then, we discussed the ways in which the identified patterns of shifts affect the general meaning and the structure of the texts. Finally, adopting a socio-cultural approach, we showed how certain shifts are conditioned by the translators’ ideology and their interpretation of the original. This in turn reveals the positions they occupy within the political and ideological space of each target culture and the main cultural and translation norms operating in the recipient systems.

This diploma thesis deals with bioethical and medical issues of assisted suicide. Compatibility of assisted suicide and medical ethos is one of the most urgent ethical questions of modern medicine. The centre of the diploma thesis will consist of an ethical reflection of tension between assisted suicide by a doctor on one side and medical ethos, as contained, for example in the Hippocratic Oath, the code of the World Medical Association and other important medical documents, on the other side. In the introduction the diploma thesis describes the distinction between assisted suicide and other forms of active euthanasia, basic information about the legal regulations and essential elements of assisted suicide in individual countries where assisted suicide is legalized. Then, the subject will be systematically reflected based on critical analysis of the medical codes and legal regulations of the countries. Ethical reflection will also be based on the experience and reflection of doctors themselves, as recorded in professional ethical publications from countries where assisted suicide is practiced. Zábranův Doktor Živago: Okolnosti vzniku a analýza vybraných překladatelských řešení. / Zabrana's Doctor Zhivago: The Making and Analysis of Selected Translation Solutions

(in English): This thesis deals with a version of Boris Pasternak's novel Doctor Zhivago, translated into Czech by Jan Zábrana and Jiří Kovtun. The aims of the thesis are: (1) to follow the journey of the original exile edition from Italy to communist Czechoslovakia, (2) to clarify how both translators contributed to the poetry part of the novel, (3) to outline Zábrana's translation method a comparison of chosen typewritten versions of the translation from various phases of its creation. The primary source of material for all the individual objectives is Zábrana's inheritance, which answers the question why Jiří Kovtun's name does not appear in the first Czech edition of Doctor Zhivago (1990), although he is listed along with Zábrana as the translator of the poetry part in the remaining three editions (2003, 2005, 2011). The original material from the inheritance as well as Marie Zábranová's testimony emphasize the sequence of events that influenced the creation of Zábrana's translation. The final analysis compares chosen typewritten versions of the novel's eighth chapter with the first Czech edition and describes how Zábrana worked. The thesis aims to be beneficial for the Czech history of translation and contribute to the current knowledge about the translation method of Jan Zábrana. Keywords Jan...

Professionals unter Druck - Analyse und Auswirkungen aktueller Herausforderungen für Ärzte

Im Beitrag werden wesentliche Herausforderungen denen Ärzte als Berufsgruppe aktuell gegenüberstehen aufgezeigt und in Bezug auf ihre Auswirkungen hinsichtlich des traditionellen ärztlichen Berufsbildes diskutiert. Ärzte können dabei als Angehörige einer geradezu klassischen Profession betrachtet werden. So werden im Beitrag allgemeine professionsspezifische Merkmale, wie z.B. die Anwendung hochspezifischen Wissens, der hohe Autonomiegrad in Bezug auf die Durchführung der eigenen Tätigkeit oder die Orientierung an einem „Berufsethos“, auf Ärzte übertragen und spezifiziert. Anschließend werden Herausforderungen in Form 1) der Entwicklung zur Wissens- und Informationsgesellschaft, 2) des demographischen Wandels in Deutschland und insbesondere 3) die aktuellen Tendenzen einer neuen politischen Steuerungslogik von Gesundheitsleistungen erläutert. Dann wird aufgezeigt, dass diese Tendenzen in vielerlei Hinsicht eine Bedrohung für das traditionelle professionelle Berufsbild des Arztes darstellen, indem etwa eine Aushöhlung des ärztlichen Wissensbestandes wahrgenommen werden kann oder die bisherigen Standards des ärztlichen Arbeitsvollzuges hinterfragt werden. Diese Bedrohung äußert sich dabei u.a. in konkreten Handlungskonflikten, denen Ärzte in ihrer alltäglichen Berufspraxis, d.h. im täglichen Umgang mit Patienten, gegenüberstehen. Insofern behandelt der Beitrag zugleich mögliche Auswirkungen auf den ärztlichen Arbeitsvollzug und die Erbringung von Gesundheitsleistungen. Der Fokus richtet sich dabei insbesondere auf die ärztliche Praxis in Krankenhäusern. Letztlich wird hervorgehoben, dass Erkenntnisse über Professionen, professionelle Handlungsmuster und die damit zusammenhänge Steuerungslogik professioneller Leistungen in sogenannten „professionellen“ oder „bürokratisch-professionellen“ Organisationen von genereller Wichtigkeit sind, um die aktuell ablaufenden Veränderungsprozesse in medizinischen Einrichtungen analysierbar und in ihrer sozialen Dynamik als auch ihrer Ergebnisse interpretierbar zu machen. / The paper shows that German hospital clinicians are faced with crucial challenges to their professional self-understanding nowadays. For this, clinicians are introduced as professionals by explaining specific characteristics of their occupation, like the knowledge intense activity or the high grade of work autonomy. Further the papers explains that the present challenges express conflicts between the traditional occupational orientation of clinicians and new expectations referring to the manner of clinical work. Three main challenges are to identify 1) the development of a knowledge intensive society, with growing possibilities to get information about disease diagnosis and therapy 2) the demographic change and 3) the partly corresponding changes in the political steering of public health services. The paper points out, that these developments threat traditional professional orientations as they lead to a socially perceived hollowing out of medical knowledge and a change of professional standards of medical performance. It is to highlight that these conflicts cannot only get separated as subjective or psychological problem of clinicians. Moreover these conflicts influence the concrete treatment of patients in hospitals. In this way, the paper delivers insights to analyse, understand and judge present change contents and change dynamics in „professional organizations” and „bureauprofessional organizations”.

MediHome es un aplicativo Móvil que facilita el acercamiento de los profesionales de salud con el paciente en la comodidad de su hogar. El paciente al ingresar a la plataforma de manera gratuita a través de un aplicativo móvil, se registrará y completará un formulario con sus datos personales, posteriormente ingresará sus signos y/o síntomas que le permitirán acceder o no a contactar con el profesional adecuado ; podrá buscar un profesional específico según su especialidad, a su vez encontrará toda la información detallada del mismo: currículo, experiencia, comentarios de pacientes, localización, etc.; para garantizar confianza y seguridad en la prestación del servicio. La finalidad es acercar al profesional de salud con el paciente en su domicilio y poder brindarle el servicio médico en la comodidad de su hogar. Las oficinas administrativas se encuentran ubicadas en el distrito de San Juan de Lurigancho. La presencia y funcionamiento del aplicativo se implementará de manera inicial en todos los distritos de la capital, para luego empezar operaciones en provincia. La importancia de MediHome radica en su propuesta de servir como el nexo entre el paciente y el profesional de la salud, de manera que el paciente pueda localizar al profesional que se encuentre más cerca de su casa y poder recibir una consulta en la comodidad de su hogar; de esta manera ambos evitan acudir a un centro de salud concurrido con posibles pacientes COVID positivos. Así, se contribuye a la descongestión de los hospitales, se brinda atención oportuna en la comodidad del hogar de manera aislada y segura de cualquier contacto de riesgo y se mejoran los indicadores de salud en nuestro país. La inversión total del proyecto es de S/ 315,400.00 y está compuesta por un 87% (S/ 275.400) de deuda y el 13% (S/ 40.000) de capital de accionistas.
