A inclusão social de usuários dos CAPS AD de um município paulista / Social inclusion of drug users at a Service of Psychosocial Attention and Care to alcohol and drug addiction (CAPS AD) in a city in the State of São Paulo - Brazil

Estudo descritivo exploratório, realizado nos Centros de Atenção Psicossocial Álcool e Drogas - CAPS AD Independência e Reviver -, localizados na cidade de Campinas -SP, com o objetivo de investigar a inclusão social de usuários em atendimento nestes serviços, sob a perspectiva dos profissionais que neles atuam. Esta pesquisa tem, como pressuposto, que a visão dos profissionais sobre os modelos de atenção para álcool e drogas influencia suas ações para a inclusão social dos usuários. A pesquisa foi realizada com 17 profissionais dos CAPS AD, atuantes nos locais há pelo menos três meses, sendo todos participantes voluntários. Os dados foram coletados por meio de entrevistas semiestruturadas realizadas com os participantes e observação participante, com registros no diário de campo. Com base nas informações coletadas, os dados foram organizados e analisados segundo o método de análise de conteúdo, na especificidade análise temática. A discussão fundamentou-se nos conceitos de reinserção social, trazidos pelas políticas em saúde mental e álcool e drogas, bem como nas concepções de reabilitação psicossocial de Saraceno, entre outros autores. Ainda, como parte de conceitos adotados para análise e compreensão dos resultados, recorreu-se ao conceito-processo de Sawaia sobre a \"dialética da inclusão/exclusão\" que, como forma de categoria de sua análise, baseia-se no sofrimento ético-político, porém aqui aplicado aos usuários de álcool e drogas. Desta forma, o processo de análise permitiu desdobramentos em três categorias temáticas: a visão do profissional sobre inclusão social dos usuários de álcool e drogas; o cuidado à inclusão social e os limites e potencialidades para inclusão social dos usuários de álcool e drogas. Da análise das informações coletadas emergiram que as percepções dos profissionais sobre inclusão social são diversas, e que o uso de uma única percepção não é suficiente para abarcar ações que sejam efetivamente de inclusão social; já sobre o cuidado dos profissionais no que se refere à inclusão social, evidenciou-se que as equipes atuantes ainda não dominam a temática, apresentando, portanto, dificuldades para identificar suas ações como sendo de inclusão social. Emergiram também limites e potencialidades para a inclusão social dos usuários de álcool e drogas apontados pelos profissionais sob três âmbitos: o primeiro seriam os fatores que dependem dos profissionais; o segundo, os aspectos que dependem da instituição CAPS AD; e o terceiro, aqueles que extrapolam a esfera da instituição CAPS AD. A pesquisa revelou ainda a influência dos modelos de assistência para usuário de álcool e drogas no posicionamento político-ideológico dos profissionais e, consequentemente, em suas perspectivas e ações para inclusão social, sendo ainda uma tema que deve ser mais amplamente discutido nas equipes, para que o cuidado em saúde mental para estes usuários seja mais qualificado / This is a descriptive, exploratory work aiming at investigating the social inclusion of drug users undergoing treatment, from the perspective of the professional sat the Independence and Revival Service of Psychosocial Attention and Care to alcohol and drug users (CAPS AD) in the city of Campinas, State of São Paulo, Brazil. This work is based on the assumption that professionals\' view about the models of \"Attention and Care to alcohol and drugs\" influence their actions towards social inclusion of users. Seventeen professionals, who work at CAPS AD for at least three months, participated voluntarily in this study. The data was collected through semi-structured interviews and participative observation, recorded on field diaries. The analysis was carried out with Thematic Analysis, within the theory and methodology of content analysis. We based our discussion on the concepts of Social Reinsertion in by mental health policies about alcohol and drugs, as well as concepts of psychosocial rehabilitation by Saraceno, among other authors. Still, among the concepts adopted for the analysis and interpretation of the data, we used the concept-process, by Sawaia, about the \"dialectic of exclusion/ inclusion\" which, as a form of analysis categorization, is based on the concept of ethical-political suffering, applied here to alcohol and drug users, however. Therefore, the analysis was unfolded into three categories: the professional\'s view about the social inclusion of alcohol and drug users; the Attention and Care to social inclusion; and the limits and potentiality to social inclusion of alcohol and drug users. The analysis shows that there are variations among professionals\' perceptions about social inclusion. Also, that the use of a single perception is not enough to comprehend effective actions of social inclusion. As for professionals\' procedures regarding social inclusion, it became clear that staff does not acknowledge the theme, showing difficulty to identify their own actions as of social inclusion procedures. The analysis also shows limits and potentiality of social inclusion of alcohol and drug users, pointed out by the professionals in three levels: firstly, factors depending on the professionals; secondly, aspects depending on the institution CAPS AD; thirdly, aspects which are beyond the realm the institution CAPS AD. This study also reveals the political-ideological influence of assistance models on the professionals at the Attention and Care of alcohol and drug users. Consequently, it exposes professionals\' perspectives and procedures towards social inclusion. That is still a theme to be widely discussed within the teams, so that mental health care for these users improves its quality

Foram avaliados 262 usuários de substâncias psicoativas. Foram realizadas avaliações de cárie dentária segundo as normas da OMS. Os exames foram feitos por uma única examinadora, ao ar livre, sob luz natural, sendo utilizada para o exame a sonda periodontal IPC, espelho clínico plano n°5 e espátula de madeira. As variáveis socioeconômicas, autopercepção de saúde oral, qualidade de vida foram captadas por meio dos seguintes instrumentos: OHIP-14 (Oral Health Impact Profile), WHOQOL (World Health Organization Qualityof Life), Meneghim et al., 2007. Para triagem do envolvimento com álcool, cigarro e outras substâncias, ASSIST-OMS. Inicialmente foram construídas tabelas de distribuição de frequências e o teste de Qui-quadrado foi utilizado, com nível de significância de 5%, para se avaliar as associações entre a qualidade de vida (WHOQOL) e o impacto da saúde bucal na qualidade de vida (OHIP) (variáveis dependentes) com as demais variáveis investigadas (variáveis independentes). As variáveis que apresentaram p<0,20 nas análises simples estimados para os indicadores que permaneceram no modelo de regressão múltipla, no nível de 5%. O programa estatístico utilizado para a realização de todas as análises foi o SAS (SAS, 12.3). Assim, indivíduos com CPOD > 13 (OR= 2,25; IC 95%= 1,30-3,89); baixa renda (OR= 2,41; IC 95%= 1,15-3,59 1,22-4,77) e usuário de cocaína /crack (OR= 2,02; IC 95%= 1,15-3,59) apresentaram mais chances ter baixa qualidade de vida. Voluntários com higiene bucal boa e regular (OR= 3,63; IC 95%= 1,96- 6,75); que não relataram dor (OR=2,69; IC 95%= 1,67-4,60) têm mais chance de apresentar menor impacto da saúde bucal na qualidade de vida. Conclui-se que a qualidade de vida pode ser afetada pela saúde bucal, portanto políticas públicas para a promoção de saúde nessa população são necessárias / Abstract: This cross-sectional study aimed to assess the impact of oral health on quality of life of drug users attended by CAPS-AD three cities the State of São Paulo: Limeira, Piracicaba and Rio Claro. Were analyzed 262 drug users. Reviews of dental caries were conducted according to WHO standards. The oral examination will be conducted by one trained and calibrated examiner, under natural light at CAPS-AD, using the CPI periodontal probe and dental mirror with previous drying teeth. The socioeconomic variables, self-perceived oral health, quality of life were identified through of the following instruments: OHIP - 14 (Oral Health Impact Profile), WHOQOL (World Health Organization quality of Life), Meneghim et al, 2007. For screening of involvement with alcohol, tobacco and other substances, WHO ¿ ASSIST. Initially the frequency distribution tables and the chi-square test were constructed was used with a significance level of 5 %, to assess the associations between quality of life (WHOQOL) and the impact of oral health on quality of life (OHIP) (dependent variables) and the other study variables (independent variables) . Variables with p < 0.20 in simple analyzes estimated for the indicators that remained in the multiple regression at the 5 % model. The statistical program used to perform all analyzes was SAS (SAS, 12.3). Thus, individuals with DMFT > 13 (OR = 2.25, 95% CI = 1.30 to 3.89), low income (OR = 2.41, 95% CI = 1.15 to 3.59 1.22 -4.77) and user of cocaine / crack (OR = 2.02, 95% CI = 1.15 to 3.59) were more likely to have lower quality of life. Volunteers with good oral hygiene and regular (OR = 3.63, 95% CI = 1.96 to 6.75), who reported no pain (OR = 2.69, 95% CI = 1.67 to 4.60) are more likely to have lower impact of oral health on quality of life. Diante desse quadro (visível), no limite dessas formas ou noções, o movimento de Redução de Danos opera como tensor, produzindo uma subtração criadora de diferenças de dinâmicas: linhas de cuidado que desafinam as relações constantes, a determinação atual de uma política pública de drogas um tanto quanto fantasmática e intimidadora, ou ainda, hipertrofiada e neurótica. Com isso, sabendo que nunca se está diante de uma tela em branco, a presente investigação pretende tornar possível outro fundo para se pensar uma política de drogas. Toma-se, nesse intuito, o procedimento de limpeza e traços do pintor Francis Bacon, que em si já sugere a mudança de um quadro, no caso, do afundamento da atual política de drogas à rearticulação de práticas de saúde junto às pessoas que fazem uso de drogas. Erratic Mothers and Wild Animals: A Critical Discourse Analysis of Online Newspapers' Representations of Female and Male Opioid Users

This thesis critically investigates how female and male opioid users are represented in local newspapers in Ohio, one of the states which has been most severely affected by the ongoing ‘opioid crisis’ in the United States. Through an analysis of 20 articles from the The Plain Dealer and The Columbus Dispatch, the study aims to highlight how women and men who use opioids are portrayed, and what ideologies are hidden in the texts. Guided by Fairclough’s framework for critical discourse analysis and van Dijk’s sociocognitive approach, the analysis was performed on three levels: text-level, whereby journalists’ word choices, contextualisation and linguistic emphasis were studied; discursive level, which focused on processes involved in the production and consumption of the news pieces, and; sociocultural level, which entailed analysing historical and current developments of drug policy locally and nationally. The study finds that journalists downplay the seriousness of (white) male opioid use by calling men by their nicknames, by portraying them as ‘mischievous’ and by using jokey undertones when referring to their drug use. Female opioid use is constructed as abnormal by use of words such as ‘erratic’ and ‘unruly’ and women are discursively penalised for failing in their roles as caregivers to children. This thesis exemplifies how language use by local journalists’ in Ohio reinforces societal perceptions of male and female opioid users, which may influence counteractive measures by authorities.

Studiens syfte var att analysera hur svensk dags- och kvällspress framställer narkotikabrukare. Metoden som användes för att besvara syfte och frågeställningar var en kritisk diskursanalys. Studiens teoretiska grund var socialkonstruktionism och kritisk diskursanalys i relation till Goldbergs (2010) stämplingsteoretiska modell. Resultatet visade på beskrivningar av narkotikabrukare som kriminella, utsatta, offer och beroende. Vad som också framkom var en bild av narkotikabrukare som en helt vanlig person, en Svensson med familj, barn och fast anställning. De diskurser som uttyddes var en förövardiskurs, en offerdiskurs och en normaliseringsdiskurs. Utifrån den stämplingsteoretiska modellen kunde det tolkas som att både offerdiskursen och förövardiskursen är stigmatiserande och således med i skapandet av avvikare medan normaliseringsdiskursen kan sägas vara icke-stigmatiserande för narkotikabrukare. / The purpose of the study was to analyse how the Swedish daily and evening press portraysdrug users. The method used to answer the purpose and questions of the study was a critical discourse analysis. The theoretical basis of the study was social constructionism and critical discourse analysis in relation to Goldberg's (2010) stigma theory. The results showed descriptions of drug users as criminals, victims and addicts. What also emerged was an image of drug users as an ordinary person with a family, children and full-time employment. The interpreted discourses were an offender discourse, a victim discourse, and a normalization discourse. Based on the stigma theory, it could be interpreted that both the victim discourse and the perpetrator discourse are stigmatizing and thus involved in the creation of deviants while the normalization discourse can be said to be nonstigmatizing for drug users.

Background: Intravenous way of drug application is the most common between problem drug users at Prague. Due Mravčík et al. (2015), the estimate is, that there was a 15 700 intravenous drug users at Prague in 2014 (7 500 pervitin users, 2 500 heroin users and 5 700 buprenorphine users). And that they spend about 15,5 millions of syringes every year (Švůgerová, 2015). But the data in Annual report on drug situation 2014 says, there was only 2, 6 millions of syringes distributed (Mravčík et al., 2015). This means, that Prague intravenous drug users spend many more syringes, that are distributed by low-treshold services and pharmacies. That is a reason of charting of the behaviour of drug users in connection with application of addictive substance, especially how the consuption of injecting material is connected with single type of drug user. Aims: The main aim of the thesis is to find out the frequency of drug application in intravenous drug users and to estimate the total consumption of injecting materialial depended on single of users typology (used drugs, frequncy of contact with harm reduction services…) Research sample: The sample consists of 155 clients of harm reduction services at Prague (3 contact centers and 3 streetwork programs). It is proposed to analyze the criminal alternative used by Goiás Judiciary to replace the prison sentence for cases of individuals in conflict with the law, for unlawful conduct associated with problem drug use or under the influence of these, focusing on the Justice Programme Therapy adopted in Goiânia County. Through literature, with study of foreign and national literature, this last little, in addition to legislation, official documents and using qualitative and quantitative methodology, we proceeded to the analysis of the data, for the period 2010 to 2013. The study recovered the history of prohibition in the world and in Brazil, legislation influenced, and the essential concepts for understanding the action of the drug and its relationship to crime. The history of Therapeutic Jurisprudence were raised, of foreign origin, and national experience with theoretical support in Wexler and Winick (2002), Lima (2009) and Fensteseifer (2009). It appears that the criticism that the Therapeutic Justice homeland receives are unjustified because it differs from the American and Canadian model. The analysis of the experimental data in Goiânia-GO concluded that 43.60 % of the participants completed the program with an average duration of 12 months and 85.1 % of the participants had no involvement in criminal proceedings as defendants in the region, similar to the results International, showing that it is an effective alternative to criminal probation and reduction of recidivism. / As tendências atuais no Direito Penal propõem a substituição da pena privativa de liberdade por penas alternativas, com foco na educação e prevenção, principalmente em ilícitos de menos potencial ofensivo, face os elevados custos e danos oriundos do cárcere para o indivíduo e a comunidade. Propõe-se analisar a alternativa penal utilizada pelo Poder Judiciário de Goiás em substituição a pena de prisão para os casos de sujeitos em conflitos com a lei, por conduta ilícita associada ao consumo problemático de drogas ou sob a influência destas, focalizando o Programa Justiça Terapêutica adotado na Comarca de Goiânia. Por meio de pesquisa bibliográfica, com estudo da literatura estrangeira e nacional, esta última escassa, além da legislação, documentos oficiais e utilizando metodologia quali-quantitativa, procedeu- se a análise dos dados obtidos, referente ao período de 2010 a 2013. O estudo recuperou a história do proibicionismo no mundo e no Brasil, a legislação que influenciou, e os conceitos essenciais para compreensão da ação das drogas e sua relação com criminalidade. Foram levantados os antecedentes da Therapeutic Jurisprudence, de origem estrangeira, e sua experiência nacional, com aporte teórico em Wexler e Winick (2002), Lima (2009) e Fensterseifer (2009). Constata-se que as críticas que a Justiça Terapêutica pátria recebe não se justificam porque difere do modelo americano e canadense. A análise dos dados da experiência na cidade de Goiânia-GO concluiu que 43,60% dos participantes concluíram o programa com duração média de 12 meses e 85,1% dos concluintes não tiveram envolvimento em ações penais como acusados na comarca, semelhantes aos resultados internacionais, mostrando que é uma alternativa penal eficaz para reinserção social e redução da reincidência penal.

The main goal of the theoretical part of submitted theses is to describe the field of specialized treatment of imprisoned drug users. The author's emphasis is to put the specialized treatment to a broader frame of the Risk-needs theory as well as to the frame of physical diseases and mental disorders that are often associated with addiction or drug abuse. Different kinds of specialized interventions that are usually used for treatment of imprisoned drug users in different countries are described. There is although demonstrated in which way these interventions are implemented in Czech prison system. By these interventions are especially meant treatment on the entry at the prison, detoxification, counselling and other programs and interventions with low intensity. Especial emphasis is put on residential prison programmes such as prison therapeutic communities for drug users and on opioid substitution treatment. A great number of pages are dedicated to the research of effectivity of treatment programmes. In the practical part of submitted theses results of cross-sectional study that was implemented in all Czech prisons in three waves in 2010, 2012 and 2014 are presented. 3456 ABSTRACT: Background: VHC is a widespread serious chronic infectious disease. Most of the patients affected by the disease consist of IDUs. Treatment of VHC is now perceived as an important preventive element for the spread of the disease in IDUs population. Adherence to treatment is absolutely crucial for the successful completion of treatment. Adherence to treatment is often questioned by clinicians who treat VHC in patients using addictive substances. Objectives: To compare the efficacy of treatment of VHC in patients with a history of drug in patients without a history of drug, to compare treatment adherence among patients without a drug history and patients with drug history, to map out the factors affecting adherence to treatment and to find a place in the therapeutic team for expertise of addictologist.
