Bakgrund: Under graviditeten och efter förlossningen genomgår kvinnan en period som innebär en stor förändring i hennes liv och det är vanligt att uppleva ett brett spektrum av olika känslor. Den första tiden efter förlossningen kan symtom på allvarliga psykiska hälsoproblem utlösas, såsom en depression. Depression hos nyblivna mödrar, en så kallad postpartumdepression, har sedan 1990-talet ökat i intresse inom hälso- och sjukvården, inom omvårdnadsforskningen samt bland föräldrar.   Syfte: Syftet med denna studie var att belysa BVC-sköterskors upplevelser av att möta mödrar med symtom på postpartumdepression.   Metod: En induktiv ansats användes där datamaterialet analyserades med kvalitativ innehållsanalys. Datamaterialet utgjordes av intervjuer och åtta BVC-sköterskor intervjuades.   Resultat: Ur analysen av datamaterialet framkom fyra kategorier: etablera ett gott samarbete, ge råd och vägledning, att använda instrument som underlättar och kommunikativa hinder i arbetat samt tillhörande underkategorier.   Konklusion: För att kunna möta mödrar med symtom på postpartumdepression bör BVC-sköterskorna etablera ett bra samarbeta. Det första hembesöket var också betydelsefullt för att kunna etablera ett bra samarbete mellan BVC-sköterskorna och mödrarna. Att använda instrument såsom EPDS-formuläret, underlättade samtalet och var ett viktigt verktyg för BVC-sköterskorna. / Background:  During pregnancy and after childbirth the woman go through a period that represents a major change in her life and it´s common to experience a wide range of different emotions. The first time after birth, symptoms of serious mental health problems can be triggered, such as depression. Depression in new mothers, known as postpartum depression, has increased in interest since the 1990s in health care, nursing research and among parents.   Aim: The aim of the study was to illustrate Child health care nurses’ experiences of encounting mothers’ with symptoms of postpartum depression.   Method: Qualitative content analysis with inductive approach was used and data were collected through interviews. Eight Child health care nurses’, CHCN, were interviewed.   Results: Four categories emerged from the analysis: establish a good collaboration, provide advice and guidance, to use instruments that facilitate and communicative barriers at work and associated subcategorys.     Conclusion: In order to meet mothers with symptoms of postpartum depression CHCN need to establish a good collaboration. The first home visit was also important to establish a good collaboration between CHCN and mothers. The use of instruments such as EPDS, facilitate the conversation and was an important tool for the CHCN.

The main objective of the research was to adjust the cutoff scales for screening for depression, Edinburgh Postnatal Depression Scale (EPDS) and Beck Depression Inventory (BDI), for pregnant teenagers. It is a cross-sectional, population-based study carried out with pregnant adolescents (10-19 years old), which consulted the service prenatal Unified Health System (SUS) of Pelotas. Was carried out from October 2009 to March 2011. The sample was composed based on the SIS-Prenatal program records along the municipal health department and the Basic Health Units, and specialized clinics in the urban area of Pelotas / RS. The women answered a questionnaire with socio demographic and obstetric data, besides the two scales for screening for depression during pregnancy. Also participated in a structured clinical interview Mini International Neuropsychiatric Interview 5.0 (MINI) for the clinical diagnosis of depression. Demographic, obstetric and psychiatric social variables were assessed according to the WHO questionnaire. The accuracy of the scales was calculated by their Area under the curve (AUC) of ROC, as well as their respective sensitivity, specificity and predictive values. The best cutoff (PC) found for this sample was ≥10 EPDS scale, with sensitivity 81.1% and specificity 82.7% and AUC of 0.899. BDI for the best cutoff point was ≥11, sensitivity 81.1%, sensitivity 76.8% and an AUC of 0.869. Since the difference between the two AUC were statistically significant (p = 0, 0215). Based on the results, it is concluded that the EPDS scale has become more predictive and sensitive in screening for depression in pregnant adolescents, with respect to BDI. / A presente dissertação refere-se a um estudo epidemiológico sobre escalas de rastreamento para depressão em gestantes adolescentes. O objetivo principal da pesquisa foi verificar os pontos de corte das escalas para rastreamento da depressão Edinburgh Postnatal Depression Scale (EPDS) e Beck Depression Inventory (BDI), em gestantes adolescentes. Trata-se de um estudo transversal, de base populacional, realizado com gestantes adolescentes (10 a 19 anos de idade), as quais consultaram o serviço de pré-natal do Sistema Único de Saúde (SUS) de Pelotas-RS. Desenvolvido no período de outubro de 2009 a março de 2011. A amostra foi composta com base nos registros do programa SIS-Pré-Natal, junto a secretaria municipal de saúde e nas Unidades Básicas de Saúde, além de ambulatórios especializados na zona urbana de Pelotas/RS. As gestantes responderam a um questionário com dados sócio demográfico e obstétricos, além das duas escalas para o rastreamento da depressão durante a gestação. Também participaram de uma entrevista clínica estruturada Mini Internacional Neuropsychiatric Interview 5.0 (MINI) para o diagnóstico clínico da depressão. As variáveis sócio demográficas, obstétricas e psiquiátricas foram avaliadas nos domínios do questionário da OMS. A precisão das escalas foi calculada através de suas Áreas sob a curva (AUC) de ROC, assim como suas respectivas sensibilidade, especificidade e valores preditivos. O melhor ponto de corte (PC) encontrado para esta amostra na escala EPDS foi ≥10, com sensibilidade 81,1% e especificidade 82,7% e AUC de 0,899. Para a escala BDI o melhor ponto de corte foi ≥11, sensibilidade 81,1%, sensibilidade 76,8% e uma AUC de 0,869. Sendo que a diferença entre as duas AUC mostraram-se estatisticamente significativas (p=0,0215). Com base nos resultados, conclui-se que a escala EPDS apresenta-se mais preditiva e sensível no rastreamento para depressão em gestantes adolescentes, quando contrastada com a escala BDI.

Assessment of common perinatal mental disorders in a selected district hospital of the Eastern Province in Rwanda

Magister Public Health - MPH / Common perinatal mental disorders (CPMDs) are increasingly being recognised as an important public health issue including depression and anxiety. In low and middle income countries such as Rwanda, CPMDs are prevalent among women in perinatal period. In Africa, the estimated prevalence rates of depression are 11.3% and 18.3% during ante-postnatal respectively, while ante-postnatal anxiety rates are 14.8% and 14% respectively. However, in Rwanda there is limited literature on CPMDs. This study was aimed at determining the occurrence of CPMDs in a selected district hospital of the Eastern Province in Rwanda as well as the factors associated with CPMDs in the selected study area. A descriptive quantitative cross-sectional survey was conducted with a sample of one hundred and sixty five mothers in perinatal period, who were selected systematically. Demographic data and factors associated with CPMDs were determined using structured questionnaire and combined screening tools such as Zungu Self-rating anxiety scale (SAS) and Edinburgh Postnatal Depression Scale (EPDS). The Cronbach alpha values were 0.87 and 0.89 for SAS and EPDS respectively. SPSS Version 21 was utilized to analyse data. Univariate, bivariate correlational and multivariate analyses were performed. Most of the respondents (38.2%) were aged 25-29 years; Protestants (77.6%); married (44.8%); unemployed (77%) and had a primary school level of education (60.6%). With respect to participants in antenatal period (51.5%); 14.5% had a clinical level of anxiety and 19.4% had depression. In terms of participants in postnatal period (46.7%); 22.5% had a clinical level of anxiety and 29.7% had depression. However, participants in both periods (1.8%) all had a normal level of anxiety and 1.2% had depression.

Adverse Life Events and Perinatal Depression Among Young Pregnant and Postpartum Women

Background: Young childbearing women have an increased risk of experiencing perinatal depression when compared to adult childbearing women. Perinatal depression has been associated with adverse life events in the literature and conceptually, in frameworks such as the Lifecycle Approach to Risk Factors for Mental Disorders Model. Purpose: The purpose of this manuscript-based thesis was to (1) determine the prevalence of: (i) adverse life events that have been associated with depression and (ii) depressive symptoms among the young pregnant and parenting women who access specialized services in an urban centre in Ontario, Canada; (2) determine which adverse life events are predictive of depression during the perinatal period, in this population; and (3) examine the psychometric properties of the Edinburgh Postnatal Depression Scale (EPDS) for use in a population of young childbearing women who access specialized services in an urban centre in Ontario, Canada. Methods: A survey was conducted with 102 young women from two agencies that provide specialized services to young parents. The interviewer-administered questionnaire included demographic questions, the Edinburgh Postnatal Depression Scale, the Antenatal Psychosocial Health Assessment, the Centers for Epidemologic Studies Depression Scale, Brown’s Support Behaviour Inventory. Results: 31.4% of the sample screened positive for perinatal depression. The only adverse life events found to predict perinatal depression were satisfaction with support from ‘others’ and intimate partner violence. Another predictor was very young maternal age (14 – 17 years). The EPDS was found to be psychometrically sound when used in this population of young childbearing women. Conclusion: In this study of young childbearing women in Ontario, Canada, we found a high prevalence rate of perinatal depression and adverse life events. Furthermore, we identified specific factors that predict the development of perinatal depression in this group. Nurses can use these findings to help prioritize perinatal screening efforts to identify this condition early on in order to lessen the adversities related to perinatal depression.

Qualidade de vida relacionada à saúde de mulheres grávidas com sintomas depressivos / Health related quality of life of pregnant women with depressive symptoms

Os objetivos do estudo foram: analisar a influência dos sintomas depressivos na qualidade de vida relacionada à saúde percebida por mulheres na gestação de baixo risco e pós-parto e verificar os fatores socioeconômicos e obstétricos associados à qualidade de vida e aos sintomas depressivos. Trata-se de um estudo longitudinal, com inclusão de 313 gestantes matriculadas em 11 Unidades Básicas de Saúde da zona sul do Município de São Paulo, SP. As participantes foram seguidas em quatro etapas: 20ª, 28ª, 36ª semanas de gestação e 45 dias após o parto, com ± 2 semanas em cada etapa. A coleta dos dados foi de julho de 2008 a março de 2010. A amostra final das quatro etapas constou de 132 mulheres. Os dados sociodemográficos e obstétricos foram obtidos por meio de entrevista na primeira etapa, com exceção dos do pós-parto, que foram coletados na quarta etapa. Para avaliação dos sintomas depressivos, foi utilizada a Edinburgh Postnatal Depression Scale (EPDS) e para avaliação da qualidade de vida relacionada à saúde, o questionário Medical Outcomes Study 36-Item Short Form Health Survey (MOS-SF36), que foram autoaplicados nas quatro etapas. O estudo foi aprovado pelo Comitê de Ética em Pesquisa - Parecer nº 154/08-CEP/SMS. Os escores médios dos domínios do MOS-SF36 apresentaram declínio ao longo da gestação com recuperação no pós-parto, exceto Estado Geral de Saúde e Saúde Mental, com diferença significativa para Capacidade Funcional (p<0,000), Aspectos Físicos (p=0,001), Dor (p<0,001) e Vitalidade (p=0,002). A proporção de gestantes com sintomas depressivos variou nas quatro etapas, sendo de 29,5% na 20ª, 24,2% na 28ª, 24,5% na 36ª semanas de gestação e 33,6% com 45 dias de pós-parto. Houve correlação inversa significativa em todos os domínios do MOS-SF36 e a EPDS, com variação no coeficiente de Spearman de 0,234 a 0,785. Nas quatro etapas, os escores médios dos domínios mostraram diferenças significantes, na comparação entre gestantes sem e com sintomas depressivos, exceto Dor, na segunda etapa e Aspectos Físicos na quarta. Na regressão logística da qualidade de vida relacionada à saúde, os sintomas depressivos foram um fator de risco em todos os domínios, exceto Aspectos Físicos. The participants were followed up at their 20th, 28th, 36th weeks of gestation and 45 days postpartum, with ± 2 weeks in each periods. Data was collected from July 2008 to March 2010. The final sample of four periods consisted of 132 women. Sociodemographic and obstetric data was obtained by interviewing subjects in the first period and postpartum data in the fourth period. For the assessment of depressive symptoms, Edinburgh Postnatal Depression Scale (EPDS) was used and to assess the health related quality of life (HRQOL), the questionnaire Medical Outcomes Study 36-Item Short Form Health Survey (MOS-SF36), which were self-administered in four stages. The study was approved by the Ethics in Research Committee nº 154/08-CEP/SMS. The mean scores of the domains of MOS-SF36 presented decline over the gestation with postpartum recovery, except in General (Health) and Mental Health, with a significant difference in Physical Functioning (p<0,000), Role Limitation due to Physical Problems (p=0,001), Bodily Pain (p<0,001) and Vitality (p=0,002). The proportion of pregnant women with depressive symptoms was 29.5% at 20th, 24.2% at 28th, 24.5% at 36th weeks of gestation and 33.6% at 45 days postpartum. There was a significant inverse correlation among all domains of the MOS-SF36 and the EPDS, with Spearman coefficients range of 0.234 to 0.785. At the four periods, the mean scores of the domains showed significant differences between pregnant subjects with or without depressive symptoms except Bodily Pain in the second period and Role Limitation due to Physical Problems in the fourth one. At HRQOL logistic regression the depressive symptoms were a risk factor in all domains, except Role Limitation due to Physical Problems. To depressive symptoms, the associated factors were marital status, years of education and the number of prenatal consultations as protective factors and complaints as a risk factor. The presence of depressive symptoms at pregnancy and postpartum changes the subjective perception of Health related quality of life in low risk pregnant women.

Sintomas depressivos em mulheres no pós-parto residentes em um município do sul de Minas Gerais / Depressive symptoms in postpartum women living in a municipality in southern Minas Gerais

Introdução: Os transtornos depressivos pós-parto podem afetar a mãe, o bebê e outros membros da família. A depressão pós-parto costuma iniciar na quarta semana e evolui lentamente por várias semanas. Objetivos: Identificar a ocorrência de sintomas depressivos em mulheres no pós-parto e verificar a associação entre sintomas depressivos e as variáveis sociodemográficas e obstétricas. Método: Estudo transversal realizado com 302 mulheres que se encontravam entre 42 e 56 dias após o parto, residentes no município de Lavras (MG). Os dados foram obtidos no período de agosto de 2011 a janeiro de 2012, por meio de entrevista para obtenção da caracterização da puérpera e aplicação da Edinburgh Postnatal Depression Scale (EPDS), para rastrear a depressão após a gestação. O ponto de corte 13 pontos na EPDS foi adotado para indicar a presença de episódio depressivo. A associação de sintomas depressivos com algumas variáveis maternas foi analisada pelo teste do Qui-Quadrado e regressão logística. O projeto foi aprovado pelos Comitês de Ética em Pesquisa da Escola de Enfermagem da Universidade de São Paulo Processo nº 1059/2011/CEP-EEUSP e do Centro Universitário de Lavras - Protocolo nº 1059/2011. Resultados: As mulheres tinham em média 27,04 (±6,12) anos de idade; 49,3% eram de cor branca; média de 10,44 (±2,12) anos de estudo, 85,8% com parceiro. O coeficiente Alpha de Cronbach verificado foi 0,85. A prevalência de sintomas depressivos foi de 11,9%. Houve diferença significativa no nível de etnia (p=0,032), escolaridade (p<0,001); situação conjugal (p<0,001); número de gestação (p=0,004); paridade (p<0,001); descanso (p=0,004); fumo (p<0,001); drogas (p=0,004); planejamento da gestação (p<0,001); aceitação da gestação (p<0,001); gestação desejada (p<0,001); experiência pré-natal (p=0,007); experiência no parto (p=0012); violência antes da gestação (p<0,001); violência na gestação (p<0,001); problema psicológico antes da gestação (p<0,001); problema psicológico na gestação (p<0,001); amamentação (p=0,006) e bebê com problema de saúde (p<0,001). As variáveis associadas ter parceiro, estar amamentado e ter mais filhos foram consideradas fatores de proteção para sintomas depressivos no pós-parto e as variáveis associadas ser negra/parda, fumantes e ter sofrido violência, fatores de risco. Conclusões: A prevalência de sintomas depressivos (11,9%) em mulheres no pós-parto no município de Lavras (MG) está compatível com as da literatura nacional e internacional que se referem como 10% e 20%. O conhecimento das variáveis maternas que podem influenciar a ocorrência de depressão pós-parto é de fundamental importância, para que a equipe de saúde possa intervir de forma precoce e adequada no tratamento das mulheres. / The study objectives were to identify the occurrence of depressive symptoms in postpartum women and the association between depressive symptoms and sociodemographic and obstetric variables. This is a cross-sectional study on 302 women who were between 42 to 56 days after delivery, residents in Lavras (MG). Data were collected from August 2011 to January 2012, through an interview and application of Edinburgh Postnatal Depression Scale (EPDS). The EPDS is a self-assessment to track depression after pregnancy, which contains 10 statements, each item has a score ranging from 0 to 3, reaching a maximum score of 30. We adopted a cutoff 13 points in EPDS, 59.5% sensitivity, 88.4% specificity in the Brazilian population, to detect depressive symptoms. The reliability was assessed by Cronbach\'s Alpha coefficient and the association of depressive symptoms with some maternal variables were analyzed by chi-square. The women had an average of 27.04 (± 6.12) years of age; 49.3% white, mean 10.44 (± 2.12) years of education, 85.8% with partner. The Cronbach\'s Alpha coefficient was 0.85 verified. The prevalence of depressive symptoms was 11.9%. There were significant differences in the level of ethnicity (p = 0.032), education (p <0.001), marital status (p <0.001), number of pregnancy (p = 0.004), parity (p <0.001) and resting (p = 0.004) , smoking (p <0.001), drug (p = 0.004); planning pregnancy (p <0.001); acceptance of gestation (p <0.001); desired pregnancy (p <0.001); experience prenatal care (p = 0.007); experience in childbirth (p = 0012); violence before pregnancy (p <0.001); violence during pregnancy (p <0.001); psychological problem before pregnancy (p <0.001); psychological problem during pregnancy (p <0.001); breastfeeding (p = 0.006) and baby with health problems (p <0.001). The prevalence of depressive symptoms (11,9%) in postpartum women in Lavras (MG) is compatible with the national and international literature that refer as 10% to 20%. Knowledge of maternal variables that may influence the occurrence of postpartum depression is critical for the health team can intervene early and appropriate treatment of women.

Sintomas depressivos em mulheres no pós-parto residentes em um município do sul de Minas Gerais / Depressive symptoms in postpartum women living in a municipality in southern Minas Gerais

Introdução: Os transtornos depressivos pós-parto podem afetar a mãe, o bebê e outros membros da família. A depressão pós-parto costuma iniciar na quarta semana e evolui lentamente por várias semanas. Objetivos: Identificar a ocorrência de sintomas depressivos em mulheres no pós-parto e verificar a associação entre sintomas depressivos e as variáveis sociodemográficas e obstétricas. Método: Estudo transversal realizado com 302 mulheres que se encontravam entre 42 e 56 dias após o parto, residentes no município de Lavras (MG). Os dados foram obtidos no período de agosto de 2011 a janeiro de 2012, por meio de entrevista para obtenção da caracterização da puérpera e aplicação da Edinburgh Postnatal Depression Scale (EPDS), para rastrear a depressão após a gestação. O ponto de corte 13 pontos na EPDS foi adotado para indicar a presença de episódio depressivo. A associação de sintomas depressivos com algumas variáveis maternas foi analisada pelo teste do Qui-Quadrado e regressão logística. O projeto foi aprovado pelos Comitês de Ética em Pesquisa da Escola de Enfermagem da Universidade de São Paulo Processo nº 1059/2011/CEP-EEUSP e do Centro Universitário de Lavras - Protocolo nº 1059/2011. Resultados: As mulheres tinham em média 27,04 (±6,12) anos de idade; 49,3% eram de cor branca; média de 10,44 (±2,12) anos de estudo, 85,8% com parceiro. O coeficiente Alpha de Cronbach verificado foi 0,85. A prevalência de sintomas depressivos foi de 11,9%. Houve diferença significativa no nível de etnia (p=0,032), escolaridade (p<0,001); situação conjugal (p<0,001); número de gestação (p=0,004); paridade (p<0,001); descanso (p=0,004); fumo (p<0,001); drogas (p=0,004); planejamento da gestação (p<0,001); aceitação da gestação (p<0,001); gestação desejada (p<0,001); experiência pré-natal (p=0,007); experiência no parto (p=0012); violência antes da gestação (p<0,001); violência na gestação (p<0,001); problema psicológico antes da gestação (p<0,001); problema psicológico na gestação (p<0,001); amamentação (p=0,006) e bebê com problema de saúde (p<0,001). As variáveis associadas ter parceiro, estar amamentado e ter mais filhos foram consideradas fatores de proteção para sintomas depressivos no pós-parto e as variáveis associadas ser negra/parda, fumantes e ter sofrido violência, fatores de risco. Conclusões: A prevalência de sintomas depressivos (11,9%) em mulheres no pós-parto no município de Lavras (MG) está compatível com as da literatura nacional e internacional que se referem como 10% e 20%. O conhecimento das variáveis maternas que podem influenciar a ocorrência de depressão pós-parto é de fundamental importância, para que a equipe de saúde possa intervir de forma precoce e adequada no tratamento das mulheres. / The study objectives were to identify the occurrence of depressive symptoms in postpartum women and the association between depressive symptoms and sociodemographic and obstetric variables. This is a cross-sectional study on 302 women who were between 42 to 56 days after delivery, residents in Lavras (MG). Validation of the Edinburgh Gotland Depression Scale for Swedish fathers

Paternal postnatal depression has begun to receive attention during the last decade. Studies have shown that the consequences of paternal and maternal postnatal depression are equally serious. There are currently no validated instrument for screening of paternal postnatal depression. In this cross-sectional study a self-report questionnaire, the Edinburgh Gotland Depression Scale (EGDS) is validated against the clinical interview SCID-CV as gold standard, and is further developed. A convenience sample of Swedish fathers (N = 95) who had children in the past year, answered an online questionnaire and a subsample (n = 52) of them were later interviewed with the SCID-CV. The revised EGDS showed improved criterion-related validity, sensitivity and specificity. The scale has problems disciminating between mildly and non-depressed fathers. A cut-off score of ≥8 on the revised EGDS results in sensitivity of 91.7 per cent and specificity of 85.0 per cent. This study should be replicated and cross-validated to provide further evidence of validity. / Postnatal depression hos fäder har börjat uppmärksammas under det senaste decenniet. Studier har visat att konsekvenserna av postnatal depression hos fäder och mödrar är lika allvarliga. Det finns för närvarande inga validerade instrument för screening av postnatal depression hos fäder. I denna tvärsnittsstudie har självskattningsformuläret, Edinburgh Gotland Depression Scale (EGDS) valideras mot den kliniska intervjun SCID-CV som gold standard, och vidareutvecklas. Ett bekvämlighetsurval av svenska fäder (N = 95) som fått barn under det senaste året, besvarade en webbenkät och en undergrupp (n = 52) av dem blev senare intervjuade med SCID-CV. Det reviderade EGDS visade förbättrad kriteriumrelaterad validitet, sensitivitet och specificitet. Skalan har problem med att diskriminera mellan milt och icke-deprimerade pappor. En cut-off poäng ≥8 för den reviderade EGDS resulterar i sensitivitet på 91,7 procent och specificitet på 85,0 procent. Denna studie bör replikeras och korsvalideras för att ge ytterligare belägg för validiteten.
