A gastroplastia a Collis associada a fundoplicatura parcial (LIND) ou total (NISSEN) : estudo experimental em cães

Das várias técnicas operatórias empregadas hoje para esses casos, a operação da gastroplastia a COLLIS associada à uma fundoplicatura parcial ou total,é considerada eficiente no tratamento do refluxo. Trata-se de um alongamento do esôfago com a pequena curvatura gástrica,confeccionando-se uma gastroplastia tubular de mesmo calibre do esôfago, permitindo que este segmento permaneça no abdome envolto pela fundoplicatura, onde sofre influência da pressão abdominal. Este trabalho experimental foi elaborado com a finalidade de testar a eficiência desta técnica em cães, divididos em dois grupos: Grupo A - sete cães operados com a técnica da gastroplastia a COLLIS associada à fundoplicatura parcial de 270º à maneira de LIND.Grupo B quatro cães operados com a técnica da gastroplastia a COLLIS associada à fundoplicatura total de 360º à maneira de NISSEN. A cirurgia foi realizada após a facilitação do refluxo em todos os cães, através de uma miotomia longitudinal extramucosa da TEG em uma extensão de quatro centímetros. Todos os cães tiveram refluxo livre após a miotomia. Com a realização da técnica operatória, não ocorreu refluxo espontâneo em nenhum dos cães, sendo que houve uma diferença significante entre a média dos volumes que precipitou o refluxo com a miotomia e a média dos volumes que não precipitou o refluxo com a gastroplastia e fundoplicatura. O refluxo ocorreu em dois cães do grupo A (28,5%), quando se aplicou pressão manual sobre o estômago, entretanto a variação da pressão intragástrica medida nos dois grupos não teve diferença estatística. Em seguida a peça sirúrgica era retirada do animal, e submetida a medida das pressões ao nível do estômago, e da gastroplastia tubular envolvida com a fundoplicadura tanto parcial como total. Utilizando-se de cateter acoplado a microtransdutores, foi registrado em fisiógrafo, as variações de pressão com infusão de soro fisiológico no estômago e após aplicar pressão manual sobre o estômago. Os resultados mostraram que não havia diferença nas pressões obtidas ao nível da gastroplastia nas duas situações entre os grupos A e B. Também a extensão da gastroplastia envolvida pela fundoplicatura obtida com a cirurgia foi semelhante nos dois grupos.A conclusão final desta pesquisa,é que a cirurgia da gastroplastia tubular a COLLIS associada à uma fundoplicatura parcial ou total, protege o esôfago contra o RGE espontâneo, permitindo refluxo provocado no grupo A em 28,5% dos cães, sendo esta a única diferença entre elas. Diferentes testes estatisticos utilizados, não mostraram diferenças entre as duas técnicas anti-refluxo. / Abstract: The reflux esophagitis complicated through contracting wall of esophagus with narrowing or stenosis and followed by organ shortening, is a disease of difficult surgical treatment and with high rate of reflux recurrence after the conventional operations. One of the operative techniques employed today for these cases is the COLLIS gastroplasty associated with partial or total fundoplication. This technique consisted of a lengthening of esophagus with the small gastric curvature making a tubular gastroplasty of the same calibre of the esophagus, permitting that this segment stays in the abdomen, wrapped for the fundoplication, and receiving influence of the abdominal pressure. This experimental research was developed to verify the efficiency this technique in eleven dogs, divided in two groups: Group A - Seven dogs operated with a technique of the COLLIS gastroplasty associated with partial fundoplication of the 270º a way of LIND. Group B - Four dogs operated with a technique of COLLIS gastroplasty associated with total fundoplication of the 360º a way of NISSEN. The surgery was performed after reflux facilitation in all dogs through a extramucosal longitudinal myotomy of the esophagogastric junction in a extension. After the realization of operative technique, was not recorded spontaneous reflux in any dogs, and there was a significant difference between the rate of the volumes which precipitate the reflux after the myotomy and the rate of the volumes which not precipitate the reflux after a gastroplasty. The reflux occurred in two dogs of the group A (28,5%}, when was applied manual pressure on the stomach wall, however, the variation of the intragastric pressure measured in the two groups had not statistics differences. The surgical specimen was dissected, separated from the animal and submitted to the measurement of the pressures at the leveI of the stomach and at the tubular gastroplasty wrapped with fundoplication partial as much as total. Using microtransducer catethers were recorded in a phisiograph the pressure variations after fisiological serum infusion into the stomach and after applying manual pressure on the stomach wall. The results showed that there were no differences in the pressures measured at the level of the gastroplasty in the two situations in the both groups. The final conclusions was that the tubular COLLIS gastroplasty associated with partial or total fundoplication protects the esophagus against the reflux of the gastric contents, being that the different statistics tests used not showed significant differences between the both anti-reflux techniques. / Mestrado / Cirurgia / Mestre em Medicina

Aspectos da clínica cirúrgica destes pacientes foram analisados secundariamente, pois a ênfase deste trabalho foi o estudo anatomopatológico de peças operatórias. Foram realizados vários cortes a partir da borda tumoral com o objetivo de estudar o fenômeno do carcinoma multifocal do esôfago, as alterações celulares e a permeação vascular na base tumorale Os resultados obtidos mostraram nesta série, 8 (13,79%) casos de carcinoma multifocal. Estes dados contra-indicam ressecções segmentares que estariam propensas a recidivas locais ou a não remoção de foco tumoral distante do tumor principal. / Abstract: In this study fifty eigth patients with epidermoid carcinoma of the esophagus have been evaluated. Since the emphasis was on the pathological anatomy of the surgically removed specimens, the clinical aspects were put on a second plan. Several slices have been done from macroscopic tumoral edge aiming to detect the phenomenon of the multicentric carcinoma of the esophagus, celular alterations and the vaseular permeation of the tumoral base. The obtained results showed in this series 8 (13,79%) cases of multicentric carcinoma. These data do not recommend segmental ressections wich could allowed local recurrences or the non removal of a distant tumoral foci from the principal tumor. / Doutorado / Doutor em Cirurgia

Estudo eletromanometrico do segmento de esofago inferior apos a vagotomia gastrica proximal

Os estudos eletromanometricos foram realizados no segmento de esôfago inferior "desnervado" e esôfago medio não atingido pela cirurgia, antes e apôs estimulo do Carbacol na dose de 4ftg/kg de peso por via subcutânea, analisa dos quanto ao numero de contrações, alterações do tônus, tempo de duração e alterações das ondas de deglutição e comparados ao grupo controle de normais. Os resultados demonstraram, mediante cálculos estatisticos, que o numero de contrações do segmento de esôfago inferior nos grupos de pacientes operados de V.G.P. sob estimulo do Carbacol, foi maior que nos normais. A Atônus do segmento inferior do esôfago, sob estimulo do Carbacol, sofreu significante elevação no grupo de pacientes operados de V.G.P. e que apresentaram disfagia no p.o.. O tempo de duração das ondas de deglutição neste mesmo segmento esofágico, antes do estimulo do Carbacol no grupo de doentes operados de V.G.P. e que apresentaram disfagia foi mais elevado e significativo que nos demais. Alem disso, as alterações morfológicas verificadas nas ondas de deglutição no grupo de pacientes sob estimulo do Carbacol foi mais significativo que operados nos normais. Assim, pode-se concluir que o segmento de esôfago inferior e realmente desnervado na V.G.P. e que os pacientes que apresentaram disfagia no põs-operatório teriam si do mais desnervados que os demais. Portanto, deve-se admitir a importante participação do segmento de esôfago inferior desnervado na etiologia da disfagia após V.G.P / Abstract: Proximal Gastric Vagotomy (P.G.V.) is the most modern technique used in the surgical treatment for duodenal peptic ulcers. The mechanism of the dysphagia after P.G.V. i s not clearly understood, despite several studies already performed. This research was performed objective of verifying the participation of the "denervated" segment of the lower esophagus after a P.G.V. on the postoperative dysphagia. Manometric studies were carried out with the aid of a cholinergic agent (Carbachol), in three groups of patients: GROUP 1 - Ten individuals without dysphagia, without esophageal pathologies and not undergoing surgical procedure, were considered the normal control group. GROUP 2 - Ten patients submitted to a P.G.V. without postoperative dysphagia. GROUP 3 - Ten patients submitted to a P.G.V. with postoperative dysphagia. The patients submitted to a P.G.V. were studied in the early postoperative period, i.e., between the 7th and 21st days, the period when normally the dysphagia should occur. Manometric studies were performed on the "denervated" segment of the lower esophagus and in the esophagus where surgery was not performed, before and after stimulus of 4 flg per kilogram by subcutaneous injection Carbachol. The study was determined reviewed according to number of contractions, change of tonus, time of duration and changes of characteristics of deglutition waves, compared controls. The results after statistical analysis show that the number of contractions of the segment of the esophagus in the groups submitted to P.G.V., stimulated with Carbachol, was greater than the constrols. The tonus of lower with the segment of the lower esophagus after stimulus with Carbachol had significant elevation in group 3. The time of duration of the deglutition waves of the esophageal segment before stimulus significant with carbachol in group 3 was elevated and more than in other groups. Above all, the morphological changes ,of the deglutitidn waves on groups 2 and 3 after the stimulus with Carbachol were:more significant than the control group. Therefore, we conclude that the segment of the lower esophàgus is indeed denervated after a P.G.V. and the patients with postoperative dysphagia were more denervated than the others. We must emphasize the important participation the lower esophageal segment denervation in the etiology postoperative dysphagia after a P.G.V. / Doutorado / Doutor em Medicina

Avaliação do efeito inibidor da vitamina C na carcinogenese esofagica experimental induzida pela dietilnitrosamina

Embora tenham sido obtidos alguns avanços no seu tratamento, o prognóstico da doença ainda está longe de ser satisfatório. Apesar do câncer de esôfago não estar entre os mais freqüentes, sendo a quarta neoplasia que mais acomete o aparelho digestivo do homem, seu estudo é muito importante, principalmente por duas razões: atinge as pessoas numa das funções mais importantes para a manutenção da vida, a nutrição; e as taxas de incidência são muito similares às de mortalidade, demonstrando a ineficiência do tratamento. A melhora desta realidade pode ser conseguida de duas maneiras: diagnóstico e tratamento precoce e prevenção. Assim, a procura de substâncias que possam interferir na carcinogênese, impedindo o aparecimento de tumores, parece ser uma idéia atrativa. O objetivo deste estudo foi avaliar o efeito da vitamina C na inibição da carcinogênese esofágica experimental induzida pela dietilnitrosamina (DEN). Foram estudados 240 ratos da raça Wistar todos machos, com peso médio de 155g e três meses de idade. Os animais foram acondicionados em gaiolas com cinco ratos cada, recebendo água e alimentação ad libitum. As drogas eram colocadas na água, sendo a DEN usada na dosagem de l0mg/kg/dia e a vitamina C, na de 1290mg/kg/dia. Para melhor estudo dos efeitos destas substâncias, os animais foram divididos em grupos segundo a droga ou drogas que receberiam: grupo I, controle, somente água nos sete dias da semana; grupo II, vitamina C nos sete dias da semana; grupo III, DEN três dias por semana e água nos outros quatro dias; grupo IV, DEN três dias por semana e vitamina C nos outros quatro dias; grupo V, DEN e vitamina C juntos no mesmo frasco, por três dias, e água nos outros quatro dias; e grupo VI, DEN e vitamina C juntos no mesmo frasco, por três dias, e vitan1ina C nos outros quatro dias. Cada grupo foi dividido em subgrupos de 10 animais, segundo o tempo de observação (T), sendo o sacrifício dos animais realizado em quatro diferentes T: 90, 120, 150 e 180 dias. A seguir, o esôfago era removido e avaliado macro e microscopicamente, para a identificação de tumores. A análise dos resultados mostrou que nos grupos I, II e nos T90 e T 120 dos grupos III, IV, V e VI não ocorreu a formação de nenhum tipo de tumor. No T150, na análise macroscópica, foram encontradas 10 le~ões no grupo III (1 lesão por animal) e sete lesões no grupo IV (0,7 lesão por animal), não ocorrendo nenhuma lesão nos grupos V e VI. Na análise microscópica foram identificadas seis lesões no grupo m (0,6 lesão por animal) e cinco no grupo IV (0,5 lesão por animal), não ocorrendo nenhuma lesão nos grupos V e VI. No T180, na análise macroscópica, foram encontradas 48 lesões no grupo m (4,8 lesões por animal);"31 lesões no grupo IV (3,1 lesões por animal); cinco lesões no grupo V (0,5 lesão por animal) e uma lesão no grupo VI (O, 1 lesão por animal). Na análise microscópica foram identificadas 23 lesões no grupo m (2,3 lesões por animal); 17 lesões no grupo IV (1,7 lesões por animal); três lesões no grupo V (0,3 lesão por animal) e uma lesão no grupo VI (O, 1 lesão por animal). Houve diferença significativa entre os grupos em relação à formação de tumores, verificando-se no grupo m o maior índice de neoplasias. Nos grupos que receberam vitamina C como inibidor (grupos IV, V e VI) ocorreu menor incidência de tumores, sendo o melhor efeito inibidor observado nos grupos que receberam a vitamina C e o DEN no mesmo frasco (grupo V e VI). Dessa forma, pode-se concluir que a vitamina C apresentou efeito inibidor do aparecimento de tumores de esôfago induzidos experimentalmente com a DEN, com melhores resultados quando as drogas eram dadas conjuntamente / Abstract: Cancer is today the second death cause in developed countries. In spite of the fact that certain advances have been obtained in the course of its treatment, the prognosis of this disease is still far from being satisfactory. Although esophageal cancer is not among the most frequent kinds of cancer actually it is the fourth neoplasia to affect man's digestive system - its studies are very important, basically for two reasons: first, this type of cancer affects people in their most natural life proceeding: nutrition; second, its occurrence rates are quite similar to those of mortality, clearly demonstrating the inefficiency of the treatment. However, two procedures may change this terrifying picture: early diagnosis and prevention. So, the search for substances that may interfere with the carcinogenesis keeping away the possibilities of new tumors looks like a very attractive idea. The aim of this report was to evaluate the inhibiting effect of vitamin C in the experimental esophageal carcinogenesis induced by diethylnitrosamine (NDEA). Were employed 240 male Wistar rats, all of them with approximately 155 gr. of weight and three months old. The animals were kept in cages with five rats each and fed with food and water ad libitum. Drugs were dissolved in water. Dosages were: NDEA: 10mg. / kg. / day and vitamin C: 1,290 mg. / kg. / day. The animals were divided into different groups according to the drug/drugs they would be given, for a better study of the drug effects Group I, control, only water, seven days a week; Group II, only vitamin C, seven days a week; Group III, NDEA, three days a week and water, the other four days; Group IV, NDEA, three days a week and vitamin C, the other four days; Group V, NDEA and vitamin C together in the same bottle, three days a week and only water, the other four days; Group VI, NDEA and vitamin C together in the same bottle, three days and vitamin C, the other from days. Every group was subdivided into other smaller groups of ten animals each, according to the observation time (T). The animals were sacrificed in four different times, 90, 120, 150 and 180 days. After the elapsed time, each animal was sacrificed and its esophagus was macro/microscopically evaluated to identify any tumors. As final results, in the groups I and 11 and in T90 and T120 of groups III, IV, V and VI there was no occurrence of any kind of tumor. The macroscopic studies in T 150 showed ten lesions in the group III (0.1 lesion per animal) and seven lesions in the group IV (0.7 lesion per animal), and no lesion the in the groups V and VI. The microscopic studies showed six lesions in the group III (O.6Iesion per animal) and five lesions in the group IV (OA lesion per animal), and no lesion in the groups V and VI. The macroscopic studies in T180 showed 48 lesions in the group III (4.8 lesions per animal), 31lesions in the group IV (3.1IesiolÍs per animal), 5 lesions in the group V (0.5 ,lesion per animal) and only one lesion in the group VI (0.1 lesion per animal). the microscopic studies identified 23 lesions in the group III (2.3 lesions per animal), 17 lesions in the group IV (1.7 lesions per animal), 3 lesions in the group V (0.3 lesion per animal) and only one lesion in the group VI (0.1 lesion per animal). The analysis of such results shows that there were significant differences among the groups towards the occurrence of tumors. On the other hand, group III was the one that showed the highest rate of neoplasias. Therefore, the analysis of the results demonstrated that vitamin C administered together with diethylnitrosamine (NDEA) showed an inhibiting effect over the occurrence of tumors in animals / Mestrado / Mestre em Cirurgia

Estudo do refluxo gastro-esofagico na infancia por meio da ultra-sonografia

Participaram do estudo 20 pacientes que foram submetidos a exames simultâneos de ultra-sonografia e registro do pH esofágico no período de 15 minutos após ingestão de suco de laranja. Foi observada a presença de episódios de RGE e efetuada a medida do esôfago intra abdominal. Os episódios de RGE visualizados à ultra-sonografia foram comparados com os episódios registrados à pHmetria quanto à presença e número por paciente, além de coincidência temporal. para essas comparações foram empregados os testes de Wilcoxon e de McNemar. As medidas do esôfago intra-abdominal foram comparadas com o índice de refluxo da pHmetria prolongada e com os resultados da avaliação endoscópica e histopatológica da mucosa esofágica. Para essas variáveis foi utilizado o teste de Fisher. episódios de RGE foram visualizados ao ultra-som e 24 episódios registrados pela pHmetria. Um paciente apresentou 23 episódios de RGE visualizados pelo ultra-som e apenas 4 na pHmetria e foi excluído da análise estatística global e estudado em separado. Foi encontrada coincidência temporal dos episódios de refluxo .em 100% do tempo de observação do ultra-som com o registro da pHmetria em 15 pacientes, em 75% do tempo em 2 pacientes, em 50% do tempo em 1 paciente e nenhuma coincidência em 2 pacientes. A coincidência temporal da pHmetria em relação ao ultra-som foi de 100% em 16 pacientes, 75% em-1 paciente, 50% em 1 paciente e nenhuma coincidência em 2 pacientes. A USG observou RGE em 10 pacientes, desses, 9 apresentaram RGE nos 15 minutos à pHmetria. A pHmetria (15 min) apresentou episódios de RGE em 10 pacientes, desses 9 apresentaram RGE ao ultra-som.- As medidas do esôfago intraabdominal estavam diminuídas em 7 crianças e nessas o índice de refluxo foi maior em comparação com o grupo de pacientes que apresentaram medidas normais do esôfago intra-abdominal. A endoscopia digestiva alta foi realizada em 15 pacientes, com o diagnóstico de esofagite por macroscopia em 7 e por microscopia em 10. Todos os pacientes com endoscopia digestiva e biópsia esofágica normais apresentavam esôfago intra-abdominal de tamanho normal e todos os pacientes com esôfago intra-abdominal diminuído e que fizeram endoscopia apresentavam diagnóstico de esofagite na avaliação macroscópica e microscópica. Conclusões: 1. Houve coincidência quanto à presença e número de episódios de RGE por paciente nos exames simultâneos de pHmetria e ultra-sonografia; 2. foi observada coincidência entre o período de observação do ultra-som e o registro da pHmetria e a mesma relação foi encontrada entre a pHmetria com o ultra som; 3. as crianças com medida do esôfago intra-abdominal diminuída para a idade apresentaram índice de refluxo maior do que as com esôfago normal; 4. o achado de um esôfago intra-abdominal diminuído ao ultra-som corresponde à presença de esofagite tanto à avaliação macroscópica quanto à biópsia da mucosa esofágica / Abstract: The aim of this study was to determine the accuracy of the ultrasound in order to identify the episodes of gastroesophageal reflux (GER) and the value of the ultrasound measurement of the esophageal abdominal segment (EAS) in children with gastroesophageal reflux disease (GERD). Twenty patients participated on this study who were submitted to simultaneous examination of ultrasound and esophagic pH records during a period of 15 minutes after the ingestion of orange juice. It was observed the presence of episodes of GER and it was also made the measurement of the EAS. The episodes of GER at the ultrasound were compared to the episodes recorded on the pH measurement about the presence and the number of patients and also the ,time coincidence. Wilcoxon and McNemar's tests were used to make these comparisons. The measurements of the EAS were compared to the reflux index of the prolonged pH measurement and with the results of the endoscopic and histopatologic evaluation of the esophageal mucosa. The Fisher's test was used to these changes. Fourty episodes of GER were seen at the ultrasound and 24 were recorded by the pH measurement. One patient has shown 23 episodes of reflux by the ultrasound and on1y 4 episodes by pH measurement and this patient was exc1uded from this study and was studied separately. A time coincidence was found on the episodes of the reflux of about 100% of the observation time of the ultrasound with the measurement record of pH in' 15 patients, 75% in 2 patients, 50% in one patient and no coincidence in 2 patients. The time coincidence of the pH measuremeÍ1t in relation to the ultrasound wa,s of about 100% in 16 patients, 75% in one patient, 50% in one patient ando no coincidence in 2 patients. The ultrasonography has observed GER in 10 patients; nine of these have shown GER during the period of 15 minutes in the .pH measurement. The pH measurement (15 minutes) has demonstrated episodes of GER in 10 pacients, nine of these haveshovm GER at the ultrasound. The measurement of the EAS were reduced in 7 children and in these, the reflux index was greater when compared -to the group of the patients who had presented normal measurements of the EAS. The upper digestive endoscopy was performed in 15 patients, giving the diagnosis of esophagitis by macroscopy in 7 patients and by microscopy in 10 patients. All the patients with normal digestive endoscopy and esophageal biopsy have shown EAS of normal size and all the patients with reduced EAS and that they had already urdergone endoscopy have shown a diagnosis of esophagitis at the macroscopy and microscopy evaluation. Conclusions: 1. There had been coincidence about the presence and number of episodes of GER by patients at the ultrasound ultrasonography, simultaneously examination of pH measurement and 2. A coincidence was observed between the observation period of the ultrasound and record of the pH measurement and also it was observed the same relationship between the pH measurement by the ultrasound. 3. The children which had the measurenient of the EAS reduced considering their ages have shown a greater reflux index than the others with normal esophagus. 4. O componente intratorácico pode ocasionar a compressão da traquéia e do esôfago e causar disfagia. Sendo esta um sintoma freqüente no bócio mergulhante e que desaparece após a remoção cirúrgica do mesmo, não encontramos relato de estudo dos efeitos da compressão extrínseca sobre a motilidade esofágica. Esta pesquisa foi realizada com o objetivo de avaliar as alterações da motilidade esofágica dos pacientes com bócio mergulhante utilizando como métodos a eletromanometria esofágica e a videofluoroscopia. Foram avaliados 40 pacientes com bócio mergulhante e divididos em dois grupos: Grupo 1: n = 24 pacientes com bócio mergulhante e queixas disfágicas. Grupo 2: n= 16 pacientes com bócio mergulhante e sem queixas disfágicas. No momento inicial (M1), os pacientes foram submetidos à eletromanometria esofágica e à videofluoroscopia. A eletromanometria permitiu a análise da amplitude da pressão nos esfíncteres superior e inferior do esôfago, amplitude das contrações nos terços proximal e distal do esôfago e porcentagem das contrações esofágicas peristálticas, peristálticas normotensivas e hipotensivas após a deglutição de água. A videofluoroscopia permitiu analisar os efeitos da compressão do bócio sobre o esôfago. A seguir, os pacientes foram submetidos à cirurgia para remoção do tecido tireoideano que comprimia o esôfago. Em um período de 3 a 6 meses após a cirurgia, os exames foram repetidos (M2). A análise estatística dos parâmetros eletromanométrico não demonstrou diferenças entre momentos no grupo, nem entre os grupos nos 2 momentos. Foi observada no pós-operatório da tireoidectomia a normalização da videofluoroscopia em 81,3% dos pacientes do grupo 1 e em 90% daqueles do grupo 2. Não foram encontradas evidências... This extension can lead to compression of trachea and esophagus and cause dysphagia. This symptom is very common in substernal goiter and disappears after thyroidectomy. No report of the effects of extrinsic compression about the esophageal motility was found. This research was made with the objective to investigate esophageal motility alterations of patients with substernal goiter. The methods esophageal eletromanometry and videofluoroscopy were used. Forty patients with substernal goiter were studied and divided in two groups: Group 1: n = 24 patients with substernal goiter and dysphagia symptoms. Group 2: n= 16 patients with substernal goiters and without dysphagia. At the first moment (M1) the patients were submitted to an esophageal eletromanometry and videofluoroscopy. The eletromanometry permitted the analysis of the amplitude of the pressure in the upper and lower esophageal sphincter, the analysis of the amplitude of the upper and distal body pressure and the analysis of the percentage of the total esophageal wave contraction peristalsis, the normal wave contraction and the hypotensive wave contraction after water deglutition. The videofluoroscopy allowed the analysis of the effects of goiter compression on the esophagus. After these exams the patients were submitted to a surgery to remove the thyroid tissue, the cause of the compression of the esophagus. Comparação entre achados clínicos e videofluoroscópicos na identificação de penetração laríngea e aspiração laringotraqueal no acidente vascular encefálico

Assim, este estudo tem como objetivo verificar a acurácia da avaliação clínica da deglutição orofaríngea para identificar penetração laríngea e aspiração laringotraqueal no paciente pós-AVE isquêmico, utilizando as consistências: pastosa e líquida, comparando os achados clínicos com os resultados encontrados na Videofluorocopia Foram avaliados 50 pacientes pós-AVE, submetidos à avaliação fonoaudiológica clínica e ao exame Videofluoroscópico da deglutição realizados no mesmo dia, comparando as consistências: pastoso fino e líquido ralo para a detecção de penetração laríngea e aspiração laringotraqueal. Foram considerados como sinais sugestivos de penetração e aspiração: tosse, voz molhada e alteração na ausculta cervical. The tracheal aspiration is detected in approximately 40% of these patients in the acute phase, with a high incidence of silent aspiration. The alterations in the swallowing can be detected through a clinical assessment followed by objective evaluation such as the videofluoroscopy or the nasofibrolaringoscopy. Although highly used, the clinical assessment is still cited through literature as a highly variable method, with no consensus on which one is the most appropriated. There is also no consensus on the most appropriated food volume and consistence to be used for the dysphagia evaluation. Thus, it is the objective of this study to verify the accuracy of the clinical assessment for oropharyngeal swallowing to identify laryngeal penetration and tracheal aspiration using nectar and liquid consistencies comparing the clinical findings with the results found in videofluoroscopy. Fifty post stroke patients were evaluated by a speech therapist and through videofluoroscopy, both performed on the same day, comparing the following consistencies: nectar and liquid.
