Intervenção educativa para promoção do consumo de frutas e hortaliças sob a perspectiva de gestores de Unidades de Alimentação e Nutrição / Educational intervention to promote the intake of fruits and vegetables under the perspective of managers in Food and Nutrition Units

Introdução: O incentivo ao consumo de frutas e hortaliças ocupa importante espaço na atual agenda de promoção de saúde, já que sua ingestão insuficiente representa risco para a obesidade e doenças crônicas. O ambiente de trabalho é reconhecido como um lugar importante para facilitar este acesso e consumo. Objetivo: Avaliar o efeito de uma intervenção educativa para promoção do consumo de frutas e hortaliças nos gestores de Unidades de Alimentação e Nutrição. Materiais e métodos: Trata-se de um ensaio comunitário controlado aleatorizado desenvolvido com 29 gestores de Unidades de Alimentação e Nutrição (UANs) vinculadas ao Programa de Alimentação do Trabalhador, divididos em dois grupos (15 do grupo intervenção e 14 do grupo controle). Foi realizada uma intervenção multicomponente que abordou aspectos referentes ao planejamento de cardápio, oficina culinária e estratégias de motivação para o consumo de frutas e hortaliças, em quatro etapas, com intervalo médio de um mês e meio e duração média de seis meses, e em todas elas ocorreu a participação dos gestores da Unidade de Alimentação e Nutrição (UAN). A coleta de dados foi realizada antes e após a intervenção e para avaliar o seu efeito sobre os gestores, observou-se a alteração nos estágios de mudança, no planejamento do cardápio e a adesão aos materiais e recursos educativos utilizados. As associações de interesse foram estudadas por meio do teste exato de Fisher e do qui-quadrado. Resultados: Todos os gestores eram mulheres e a maioria nutricionistas. A maior parte das empresas apresentava gestão terceirizada da UAN. A intervenção não alterou os estágios de mudança dos gestores, mas observou-se que a maioria dos gestores do grupo intervenção referiu mudar o planejamento das refeições, sendo o principal motivo dessa mudança aumentar a oferta de frutas e hortaliças. Diante dos recursos educativos utilizados, os que tiveram mais adesão foram o display de mesa e a oficina culinária, assim como também, na percepção dos gestores, estes foram os recursos educativos mais efetivos. Observou-se que os materiais educativos que dependiam das empresas e dos gestores para sua implantação tiveram menor adesão que os materiais entregues pelos pesquisadores. Conclusão: Embora a intervenção não tenha alterado o estágio de mudanças dos gestores, proporcionou modificação do planejamento dos cardápios, indicando que mais ações devem ser direcionadas a esses atores para promoção da oferta e consumo de frutas e hortaliças / Introduction: The promotion of the consumption of fruits and vegetables occupies an important place on the health promotion agenda. Low intake of these foods increases the risk of developing chronic diseases and obesity. The workplace is recognized as an important place to increase access and consumption of fruits and vegetables. Objective: Evaluate the effect of an educational intervention to promote the intake of fruit and vegetables with managers of Food and Nutrition Units. Methods: A randomized intervention involving a sample of 29 managers of workplace cafeteria, participating in the Worker Food Program, divided into two groups (15 in the intervention group and 14 in the control group), was employed. A multicomponent intervention based on the ecological model of health promotion, with an average duration of six months, with four different stages was used. The cafeterias managers participated in all steps of the intervention strategies that involved aspects like menus planning, culinary workshops and motivational strategies for the intake of fruit and vegetables. Data collection occurred in two stages, before the intervention and after intervention. The effect of the intervention in the managers was investigated using the following indicators: alteration in the state of change, changes on menus planning and the adhesion to the educational resources. It was used Fishers exact test and Chi-Square test to evaluate the change. Results: All interviewed managers were women, most were dietitian and the majority of the surveyed Units were outsourced managed. The intervention did not affect the stages of change of cafeterias managers, however most of the managers in the intervention group, reported changing meal planning. The main change mentioned was the increase in the offer of fruit and vegetables. In the evaluation of educational resources used in intervention, the managers indicated the table display and culinary workshop as the most effective. It was observed that the educational materials that depended on the companies and managers for its implementation were considered less effective, in the evaluation of the managers. Conclusion: Although the intervention hasnt altered the stage of change in the managers, it provided modifications on the menus planning, designating that more actions should be directed to these players in order to promote the offer and consumption of fruit and vegetables

Détection orosensorielle des lipides alimentaires chez la souris : mécanismes impliqués et altérations au cours de l'obésité / Orosensory detection of dietary lipids in the mouse : mechanisms involved and alterations during obesity

Des études chez le rongeur et l’Homme ont montré que la détection orosensorielle des lipides alimentaires implique une dimension gustative. Deux lipido récepteurs candidats sont présents dans les bourgeons du goût : CD36, dont l’implication dans le « goût du gras » a été démontrée par des études menées au laboratoire chez la souris, et GPR120. Les travaux de cette thèse ont montré que le GPR120 n’intervient pas directement dans cette détection, mais permettrait de moduler la sensibilité aux lipides. Ce système gustatif permet d’adapter les choix alimentaires aux besoins énergétiques tout en satisfaisant au plaisir de la consommation d’aliments palatables. Cependant, les personnes obèses surconsomment les aliments riches en énergie. S’il existe des perturbations centrales des mécanismes de récompense, nos résultats suggèrent que le système de détection périphérique des lipides est aussi altéré chez les souris obèses. Une diminution réversible de la sensibilité gustative pour les lipides est observée chez des souris rendues obèses par un régime riche en acides gras saturés, conséquence d’une dérégulation de la signalisation calcique CD36-dépendante dans les papilles gustatives. Pour déterminer l’origine de ces perturbations, le rôle de l’endotoxémie métabolique (due à une augmentation du LPS plasmatique provenant du microbiote intestinal) observée lors de l’obésité a été testé. Nous avons montré que, pris hors du contexte de l’obésité, l’endotoxémie bas-bruit est insuffisante pour altérer à elle-seule la détection orale des lipides. L’origine de cette altération est donc multifactorielle, impliquant probablement une combinaison de modifications hormonales et inflammatoires. / Dietary lipids are detected by the gustatory system in rodents and humans. Two candidate lipid-receptors are found in taste buds: CD36, which is involved in the fat taste as shown by studies conducted in our laboratory, and GPR120. Our results show that GPR120 is not directly involved in the gustatory detection of lipids in mice, but could rather be involved in the modulation of the sensitivity for fat. When this gustatory system works properly, food choices can meet the organism’s energy needs. Besides, the pleasure brought by the consumption of palatable foods is satisfied. However, obese people often overconsume energy-dense food. In the central nervous system, perturbations of the reward mechanisms have been observed, but our data show that the peripheral detection system is also altered in obese mice. A reversible decrease in the gustatory sensitivity for lipids has been found in diet-induced obese mice (diet rich in saturated fatty acids). This phenomenon is the consequence of a deficiency in the regulation of the CD36-dependant calcium signaling in gustatory papillae. To determine the origin of these perturbations, the role of obese-associated metabolic endotoxemia (characterized by an increase in plasma LPS coming from the intestinal microbiota) was investigated. We showed that low-grade endotoxemia, when studied outside of the context of obesity, is insufficient to trigger an alteration of the oral lipid detection. The origin of this alteration is therefore multifactorial, probably involving a combination of hormonal and inflammatory modifications.

This question is one that is not often considered by individuals as they go about their daily lives, but can have large implications on public health – for, there is a strong, physiological connection between food consumption and one’s health and wellbeing. Accordingly, when reflecting upon the health of a nation it is often important to consider its nutritional status. Ultimately, many determinants can contribute to how and why an individual eats certain foods, as can be seen in Ecuador. In this Latin American country, for instance, historical, socioeconomic, cultural, behavioral, socioeconomic, and environmental factors (among others) can be seen to influence the different diets – and by extension, the nutritional statuses – of different ethnic, regional, and geographic populations. Though common across Ecuador, discrepancies among these groups are particularly noticeable in the highland region, the Sierra. Overall, this paper examines the different forms of malnutrition, their implications on one’s health, and their prevalence across Ecuador. Additionally, it considers how the Ecuadorian diet was shaped, and how different subcuisines lend themselves to varying forms of malnutrition. Xéno-hormones et homéostasie buccale : impact sur les perceptions gustatives et les glandes salivaires / Xenohormones and oral homeostasis : impact on taste preferences and salivary glands

L’homéostasie buccale conditionne fortement les perceptions gustatives ; elle repose sur un épithélium buccal sain et le bon fonctionnement des glandes salivaires, qui sont finement régulés par hormones sexuelles. Le but de cette thèse était de savoir si une exposition orale en bisphénol A, un migrant d’emballage alimentaire et de composites dentaires, et une alimentation riche en phyto-œstrogènes (soja) pouvaient modifier l’homéostasie buccale. Deux expérimentations ont été conduites : une étude dose-effet du BPA (5µg à 12,5 mg/kj/j) chez le rat adulte, et une étude d’interaction d’un régime riche en soja sur les effets du BPA. Sur la base de tests gustatifs et d’une approche histologique et moléculaire (qPCR-TR), la première étude identifie pour la première fois une action du BPA sur la sècheresse buccale. Nous avons constaté que le BPA était responsable d’une moindre consommation d’eau (p<0.01), d’une préférence augmenté au sel (p<0.05) et diminué au sucre (p<0.05), et d’une altération des sécrétions salivaires. Une réversibilité partielle des effets à l’arrêt du traitement. A contrario un régime riche en phyto-œstrogène augmente la prise d’eau (p<10-6) et diminue la préférence au sel (p<0.05) par rapport à un régime semi-synthétique, et peut s’opposer aux effets du BPA. Ces études montrent que BPA et phyto-œstrogènes exercent des effets œstrogéniques agonistes ou antagonistes en fonction de la cible biologique considérée, et qu’un régime à base de soja peut gommer la plupart des effets observés / Oral homeostasis strongly influences taste perceptions. It depends on a healthy oral epithelium and salivary gland secretions, which are both regulated by sex hormones. The aim of this thesis was to identify the effect of an oral exposure to Bisphenol A, a food packaging and dental sealer component, and of a soy-diet containing phytoestrogens on oral homeostasisTwo experiments were conducted in adult rats: a dose-response study of BPA (5μg 12.5 mg / kj / day), and a study about the impact of a soy-diet on the BPA disrupting effects. By using gustation choice tests, and histological and qPCR-TR molecular approach, we identify for the first time an action of BPA on dry mouth. We found that BPA reduced water consumption (p <0.01), increased salt intake (p <0.05) and decreased sugar intake (p <0.05), and also modulated salivary secretions. These effects were partially reversed by stopping oral exposure. In contrast, the soy-diet increased water intake (p <10-6) and decreased in salt preference (p <0.05) by comparing to the semi-synthetic diet did, and may correct the effects of BPA.These studies show that BPA and phytoestrogens exert agonist or antagonist estrogenic effects depending on the biological target, and that a soy-based diet can erase most of the observed BPA effects

L’Organisation mondiale de la Santé recommande aux individus de limiter leur consommation d’aliments sucrés dans le but de prévenir le développement des maladies chroniques. En santé publique, peu de recherches ont tenté d’identifier les facteurs individuels et contextuels qui peuvent influencer conjointement la consommation de ces aliments. Or, de telles connaissances seraient utiles pour guider les interventions nutritionnelles visant à en réduire la consommation. L’objectif de cette thèse est d'étudier les facteurs reliés au comportement et les contextes associés à la consommation quotidienne d’aliments sucrés chez des adultes vivant dans un milieu urbain occidental. Cette étude a été menée auprès d'une communauté moyen-orientale établie dans la Ville de Montréal. Les aliments sucrés ont été définis comme étant les glucides raffinés dont la teneur en sucres totaux dépasse 20 % de l’énergie totale. Lors de l’étape exploratoire (N = 42), un rappel de 24 heures a permis d’identifier les sources d’aliments sucrés et de déterminer l’apport quotidien en sucres totaux de cette communauté. Une étude qualitative descriptive a été privilégiée et un cadre écologique a guidé la réalisation d’entrevues semi-dirigées sur les contextes de consommation (N = 42). Une analyse de contenu employant des procédures de codage initial et focus a mené à l’élaboration d’un instrument de mesure quantitatif sur les contextes de consommation. Cet instrument a été soumis à un pré-test (N = 20), puis administré à l’échantillon principal (N = 192). Une analyse factorielle exploratoire a permis de préciser les contextes de consommation. Les facteurs individuels mesurés incluent les données sociodémographiques, les symptômes dépressifs, la maîtrise de soi, l’assoupissement de jour, les perceptions ainsi que l’hémoglobine glycosylée. La consommation quotidienne de sucres totaux a été mesurée par un questionnaire de fréquence alimentaire (N = 192). Une analyse de régression multivariée employant le modèle linéaire généralisé (distribution de type gamma et lien logarithmique) a été effectuée pour mesurer les relations entre les contextes de consommation, les facteurs individuels et la consommation de sucres totaux, en contrôlant l’âge et le sexe. L’apport quotidien en sucres totaux de l'échantillon est de 20,3 %, ce qui s’apparente aux apports des Canadiens et des Québécois. La consommation quotidienne moyenne est de 76 g/j. Les analyses qualitative et factorielle ont permis d’identifier un ensemble de 42 contextes de consommation regroupés en sept domaines (Actes et situations de grignotage, Stimuli visuels, Besoins énergétiques, Besoins émotionnels, Indulgence, Contraintes, Socialisation). The context of sweet food consumption was studied through a qualitative descriptive study. An ecological conceptual framework was used to frame the context of sweet food consumption and thus to guide the collection of semi-structured interviews (N=42). Content analysis using open and focused coding techniques was used to inform the development of a quantitative measure to investigate the context of sweet food consumption. This measure was pilot-tested on 20 individuals and administered to the main sample (N = 192). Contextual domains were clarified using exploratory factor analysis. Individual factors measured included depressive symptoms, mastery, sociodemographic factors, perception, daytime sleepiness and HbA1C. A food frequency questionnaire was used to measure daily intake of sweet food (N = 192). Sweet food consumption followed a gamma distribution. Multivariate regression analysis using the generalised linear model (gamma distribution and log-link function) was used to estimate the associations between contextual and individual factors and consumption of sweet food, accounting for covariates (gender and age). Total sugar consumption was 20,3 % of total energy, similar to the average intake of the population of Quebec and Canada. Daily consumption of total sugars averaged 76 g/day. Qualitative analyses and factor analysis led to the identification and refinement of a 42-item contextual measure with items grouped into seven domains (emotional needs, snacking, energy demands, socialisation, visual stimuli, indulgence and constraints). Consumption of total sugars was higher amongst males (B = 0,204, SE = 0,094, p = 0,03). Domains positively associated with total sugar consumption included eating dessert (B = 0,105, SE = 0,036, p = 0,001), snacking (B = 0,225, SE = 0,091, p = 0,01) and depressive symptoms (B = 0,017, SE = 0,094, p = 0,03). Indulgence (B = -0,103, SE = 0,052, p = 0,05), self-moderation (B = -0,121, SE = 0,042, p = 0,001), and age (B = -0,01, SE = 0,004, p = 0,02) were negatively associated with sugar consumption. This mixed-methods study developed a novel measure to study the factors associated with sugar consumption. Individual and contextual factors were jointly associated with consumption of sweet foods. Contextual as well as individual factors should be jointly targeted by public health efforts to support individuals in limiting their consumption of sweet foods, as necessary.

Domains positively associated with total sugar consumption included eating dessert (B = 0,105, SE = 0,036, p = 0,001), snacking (B = 0,225, SE = 0,091, p = 0,01) and depressive symptoms (B = 0,017, SE = 0,094, p = 0,03). Indulgence (B = -0,103, SE = 0,052, p = 0,05), self-moderation (B = -0,121, SE = 0,042, p = 0,001), and age (B = -0,01, SE = 0,004, p = 0,02) were negatively associated with sugar consumption. This mixed-methods study developed a novel measure to study the factors associated with sugar consumption. Individual and contextual factors were jointly associated with consumption of sweet foods. Contextual as well as individual factors should be jointly targeted by public health efforts to support individuals in limiting their consumption of sweet foods, as necessary.
