Profiling finger-hand function of rheumatoid arthritis patients using a telerehabilitation gaming system

The problem considered in this thesis is developing a set of digital features relevant in describing finger-hand function of early-onset rheumatoid arthritis (RA) patients. The premise is based on a novel telerehabilitation gaming system that operates on a store-and-forward design. The solution to this problem was to develop a full-scale gaming platform to examine client movement performance for precision aiming tasks based on a set of digital features. To complement the movement performance, still imagery in three unique poses are captured during a session to detect visual symptoms during disease activity and early warning signs of deformities that can arise from joint damage. Resulting data is gathered in a clinic or housed in a content management system where features are extracted and analyzed, providing reports/queries for care providers and allowing remote monitoring. The goal is to help automate monitoring patient finger-hand function between office visits from a remote location, on a smaller scale and with minimal supervision. The contributions presented in this work include development of a detailed set of digital features derived from a custom built gaming platform to highlight client movement performance and algorithms to extract hand structure to approximate goniometry measurements of joint angles monitoring for potential changes during progression of the disease. The significance of this contribution is that it provides a readily accessible, experimental platform for the provision of physical therapy tailored to the individual RA patient through the use of a telerehabilitation gaming platform.

Caracterização da postura e da mobilidade em adolescentes federados em basquete / Characterization of posture and mobility in adolescents federated to basketball

Adolescentes federados de basquete têm maior demanda de treino físico, técnico, tático e resistência muscular. Na puberdade, fatores ambientais e físicos, associados à imaturidade das estruturas musculoesqueléticas podem alterar a força muscular, equilíbrio, mobilidade e flexibilidade articular, além de influenciar o desenvolvimento postural. Na tentativa de conhecer e entender as consequências do treinamento esportivo, alguns estudos avaliaram a postura estática qualitativamente, entretanto os meios utilizados não foram confiáveis ou reprodutíveis. O propósito desse estudo foi de avaliar quantitativamente as alterações posturais, a mobilidade e a flexibilidade nos adolescentes federados no basquete. Participaram do estudo 74 adolescentes saudáveis, divididos em dois grupos, federados no basquete (GA) (n=36) e grupo controle (GC) (n=38). A análise postural foi realizada pela fotogrametria com o auxílio do software de avaliação postural (SAPO versão 0.63 ® e de marcadores colocados nas referências ósseas. As variáveis posturais analisadas foram cabeça, ombro, escápula, coluna torácica e lombar, pelve, tronco e inclinação lateral da coluna. Foram realizadas a goniometria para a avaliação da mobilidade dos membros superiores e o teste do 3° dedo ao solo para a avaliação da flexibilidade. Os dados foram submetidos à análise estatística descritiva (média, desvio padrão) e Ancova para verificar a influência das variáveis explicativas nas variáveis independentes, adotando-se igual a 5%. No plano frontal, os atletas apresentaram maior inclinação lateral da coluna (GA=5,0°±2,4 e GC=3,7°±1,6) e no plano sagital direito menores valores para: anteriorização da cabeça (GA=51,4°±5,3 e GC=48,6°±5,6), anteversão pélvica (GA=11,4°±5,9 e GC=14,5°± 5,1), alinhamento vertical do tronco (GA=-0,7°±2,8 e GC=0,7° ± 2,6) e cifose torácica (GA=27,9°±7,0 e GC= 32,9°±7,5). No plano sagital esquerdo, os atletas apresentaram menores valores para anteriorização da cabeça (GA=52,5°±5,7 e GC=46,4°±5,6), protusão do ombro (GA=5,9°±2,2 e GC=8,3°±2,1) e anteversão pélvica (GA=10,2°±6,1 e GC=14,6°±4,3). As variáveis posturais do plano frontal (alinhamento dos ombros e postura da escápula esquerda) apresentaram influência da idade. O índice da assimetria horizontal da escápula teve influência da altura e a lordose lombar esquerda sofreu efeito da intensidade da dominância. Quanto à mobilidade o grupo atleta apresentou maior amplitude com significância estatística para os movimentos do ombro (flexão E, extensão D, adução D/E), cotovelo hiperextensão D e E) e punho (supinação E). Sugere-se que a postura e a mobilidade articular sofrem influência do treinamento de basquete, deste modo devem ser consideradas durante o treinamento esportivo de basquete para acompanhar o desenvolvimento dos adolescentes atletas / Adolescents federated to basketball have an increasing demand for physical, technical, tactical and endurance training. At puberty, environmental and physical factors, associated with musculoskeletal structures immaturity may alter muscle strength, balance, mobility and joint flexibility, as well as influence the posture development. In an attempt to know and understand the consequences of sports training, studies have assessed the qualitative approach; however the means of assessing the posture statistics were not reliable or reproducible. The purpose of this study was to evaluate quantitatively the postural changes, the mobility and the flexibility presented in adolescents federated tobasketball. The study included 74 healthy adolescents, divided into two groups: athletes federated to basketball (AG) (n = 36) and A control group (CG) (n = 38). The quantitative postural analysis was performed by photogrammetry measured using the Postural Assesment Software (PAS/SAPO) and markers previously placed on bone references. The postural variables analyzed were head, shoulder, scapula, thoracic and lumbar spne, pelvis, trunk and lateral inclination of the spine. Goniometry was performed to evaluate the mobility of the upper limbs and the testing of the 3rd finger to the ground to assess flexibility. The data was analyzed by descriptive statistics (average, standard deviation) and Ancova to check the influence of the explanatory variables in the independent variables, by adopting the equal to 5%. In the frontal plane, athletes had greater lateral inclination of the spine (AG = 5.0° ± 2.4 and CG = 3.7° ± 1.6) and in the right sagittal plane they had lower values for the head (AG = 51,4° ± 5.3 and CG = 48.6° ± 5.62), pelvic anteversion (AG = 11.4° ± 5.9 and CG = 14.5° ± 5.1), vertical alignment of the trunk (AG = -0.76° ± 2.8 and CG = 0,7° ± 2.6) and thoracic kyphosis (AG = 27.9°± 7.0 and CG = 32.9° ± 7.5). In the left sagittal plane the athletes had lower values for the anterior head (AG = 52.5°± 5.7 and CG = 46.4° ± 5.6), protrusion of the shoulder (AG = 5.9 cm ± 2.2 and CG = 8.3 cm ± 2.1) and pelvic anteversion (AG = 10.2° ± 6.1 and CG = 14.6° ± 4.3). The variables in the postural frontal plane (shoulder alignment and posture of the left scapula) were influenced by age. The index of asymmetry of the horizontal scapula was influenced by the height and the left lumbar lordosis suffered the intensity of dominance. As for mobility, the athlete group showed higher amplitude with statistical significance for the movement of the shoulder (flexion L, extension R, adduction R / L), elbow (hyperextension R and L), and wrist (supination L). It is suggested that the posture and joint mobility are influenced by basketball training thus they should be considered during the basketball training in order to follow the development of young athletes

Asymmetry in Elite Snowboarders : A Study comparing Range of Motion in the Hip and Spine, Power in Lower Extremities and Circumference of Thigh

<p>Snowboarding is a relatively young sport and has grown since the birth in the 1960-70.</p><p>Today, snowboarding still is a lifestyle to many, but also an accepted mainstream sport and has been an Olympic sport since the Olympic Winter Games in Nagano, Japan 1998 (18,35,36). The movement pattern and body position is asymmetric, since you stand sideways with the front foot ahead of the rear foot in the line of direction (14,18,28). Several studies that have investigated the biomechanics of snowboarding have showed that the loading of the lower extremities are different in the front leg compared to the rear leg during riding (14,18, 22,23,28).</p><p>The purpose of this study was to investigate if the asymmetric body position in snowboarding causes differences between front and rear leg considering; circumference of thigh, range of motion (ROM) in the hip joints, power in lower extremities, or causes asymmetrical ROM in the spine in the test group compared to the control group.</p><p>Two groups were used, one test group consisting of ten elite snowboarders (n=10) with an average age of 18 years, and one control group consisting of eight high level skiers (n=8), average age 17,25 years. All subjects were students at Malung/Sälen Alpine Elite Gymnasium. Measurements of ROM in hip and spine were made with a myrin incline goniometer and universal plastic goniometer. A one leg countermovement jump (CMJ) was made as a test of power in the lower extremities using Ivar ump & speed analyzer. Measurements of circumference of thighs were made using a soft tape measure.</p><p>The results show significant differences in four of the ten measurements in test group and in two of the ten measurements in the control group. There are significant differences in hip passive flexion (P<0,05) and adduction(P<0,05) in both groups (Tables 2,3) suggesting that in these movements there are individual differences. The one leg CMJ and circumference of thigh shows significant differences, (P>0,05) and (P<0,001), between front and rear leg in the test group (Table 2), but no such differences can be seen in the control group (Table 3) suggesting that these differences may be caused by the asymmetrical body position during snowboarding.</p>

Os valores médios obtidos para os ângulos anatômicos femoral proximal lateral e distal lateral foram 95,43° e 92,75°, e para os ângulos mecânicos femoral proximal lateral e distal lateral foram 93,39° e 96,75°. Os ângulos de inclinação pelos métodos de Tomlinson, Hauptman A e Hauptman B foram 133,80°, 147,32° e 128,63°. Os ângulos de Norberg e de inclinação do platô tibial foram 107,57° e 18,77°. A proporção L:P foi de 1,99 (± 0,13). Escore da subluxação dorsolateral, Ângulo da margem do centro lateral, Ângulo da borda acetabular dorsal, Índice da distância do centro aferidos nas imagens tomográficas apresentaram respectivamente os valores médios de 60,79°, 98,25°, 13,47° e 0,16. Conclui-se que os dados obtidos são importantes na caracterização de valores de normalidade para os membros de cachorros-do-mato. / The aim of this study was to evaluate goniometric values of the forelimb and hind limb joints by means of a standard plastic goniometer, radiographic values or computed tomographic values of articular angles of the hind limbs, and ratio of patellar ligament length to patellar length (L:P) in crab-eating foxes. Fifteen clinically healthy crab-eating foxes above 1 year of age were used. The range of motion of the carpus, elbow and shoulder for evaluator 1 were 138°, 108° and 118°, and for the tarsus, stifle and hip were 96°, 118° and 84°. The anatomic lateral proximal femoral angle and anatomic lateral distal femoral angle had mean values of 95.43° and 92.75°. The mechanical lateral proximal femoral angle and mechanical lateral distal femoral angle had mean values of 93.39° and 96.75°. The angle of inclination by Tomlinson, Hauptman A and Hauptman B methods were 133.80°, 147.32°, and 128.63°. The Norberg angle and slope angle of the tibial plateau were 107.57° and 18.77°, respectively. The L:P value was 1.99 (± 0.13). The dorsolateral subluxation score, lateral center edge angle, dorsal acetabular rim angle, and center distance index measured on the CT images had mean values of were 60.79°, 98.25°, 13.47°, and 0.16. In conclusion, the data obtained are important for characterizing normal values of the limbs in crab-eating foxes.

Title: Evaluation of the range of motion of the upper limbs using the OpenPose program Objectives: The aim of the work was to determine whether it is possible to use the OpenPose program as a tool for measuring upper limb range of motion, highlight OpenPose limits and consider whether the program could be used for evaluation in clinical practise or as part of telerehabiliation in the future. Methods: 48 probands participated in the research. Participants were intentionally selected from healthy population due to the nature of the work. Each participant was measured in 6 different positions while lying on a lounger. Measurements were taken bilaterally. A total of 14 different angles of shoulder and elbow were measured. These were maximum flexion of the elbow joint, flexion of the arm with flexion of the elbow, flexion of the arm, internal and external rotation of the arm and extension of the arm. Data were collected using a standardised examination method using a goniometer and a video recording. Video recordings were later used in the OpenPose program for angle evaluation. Finally, the results of both methods were compared. Results: 8 out of total 14 measured angles of shoulder and elbow were excluded based on evaluation criteria. Analysis and characterization of environmental friendly trivalent chromium passivation of aluminum alloys

As of 21st September 2017, industrial use of hexavalent chromium (Cr(VI)) will, due to its environmental toxicity and carcinogenicity, be restricted by REACH regulations. Saab Aeronautics is therefore shifting anticorrosion surface treatment of aluminum alloys from hexavalent chromium conversion to trivalent chromium (Cr(III)) passivation. The purpose of this thesis is to investigate the characteristics of conversion coatings formed with the passivation chemical SurTec 650V, in order to facilitate transfer to the more environmental friendly alternative. Process parameters, such as pH and immersion time in SurTec 650V passivation baths, have been investigated for passivation of three different aluminum alloys; 2024, 6061 and 7075. The characteristics of the Cr(III) conversion coatings achieved at laboratory scale in the thesis work have been compared with SurTec 650V process in production scale and with Cr(VI) containing Alodine 1200 and Alodine 1500 processes. The impact of varying process parameters have been investigated with goniometry, x-ray photoelectron spectroscopy (XPS), auger electron spectroscopy (AES), scanning electron microscopy (SEM) and transmission electron microscopy (TEM) to analyse characteristics of the conversion coatings. Differences in chemical composition on the surface and in depth profile was detected with XPS and AES, respectively and topology of coatings was found to differ for different aluminium alloys and parameter combinations. With TEM, the thickness of the conversion coating was found to be approximately 30-50 nm, which is thinner than the coatings formed with Cr(VI) passivation. Characteristics of coatings formed with Cr(III) passivation is concluded to be very dependent on parameter variation, especially for alloy 2024. Differences also occur between passivation at laboratory and production scale. Further evaluation of the production scale SurTec 650V process and corrosion testing should be performed, and an elaboration of a process control is required before the shift to an environmental friendly passivation process can be completely successful at Saab.

Tiriamoji problema Grafinis rankos piršto sąnarių kampų tyrimas su dvejomis sulankstyto popieriaus juostomis galėtų būti greita alternatyva standartinei goniometrijai ir savityros priemonė. Popieriaus juostų metodo patikimumas anksčiau nebuvo tirtas. Metodai Šis tyrimas palygino kartotinių matavimų, gautų abiem instrumentais, patikimumo charakteristikas (1) 24 sveikų medicinos studentų bei (2) 61 paciento ir 1 profesionalaus matuotojo imtyse. Neklinikinės studijos dalyviai keitėsi tiriamojo ir matuotojo rolėmis. Sveiki tiriamieji naudojo specialius kampainius ketvirto piršto dauginėms pozicijoms nustatyti. Pacientai atliko penkto piršto aktyvaus tiesimo savityrą be žymaus išankstinio pasiruošimo, o patyręs matuotojas paraleliai tyrė abiem metodais. Sąnarių brėžiniai buvo nuskenuoti ir išmatuoti. Rezultatai Neklinikinėje studijoje abiejų instrumentų patikimumas buvo panašus. Matuotojų vidinis ir tarpusavio vidinis klasės koreliacijos koeficientas (ICC) svyravo nuo 0,69 iki 0,93, o atitinkamos standartinės matavimo paklaidos – nuo 2,4 iki 4,9. Klinikinėje studijoje proksimaliniams tarpfalanginiams sąnariams matuotojų vidiniai ICC viršijo 0,99, o paciento ir tyrėjo tarpusavio ICC buvo mažesni nei 0,033. Tačiau priimtinų absoliučių ≤5 skirtumų proporcijos buvo ekvivalentiškos visose tyrėjo matavimų porose, o paciento ir tyrėjo poriniams matavimas šios proporcijos skyrėsi tarpfalanginių sąnarių atvejais. Reliability of the paper strip technique has not been explored. Methods This research compared reliability characteristics of repeated measurements obtained with both tools by employing (1) 24 healthy medical students to act interchangeably as subjects and raters and (2) 61 patients and a professional rater. The healthy subjects used special guides to position their ring finger joints at varying angles. The patients, without considerable preparation, self-evaluated active extension of the filth finger with paper strips, and the practitioner performed parallel evaluations with both tools. The diagrams of joint angles were scanned and measured. Results In the non-clinical study, reliability was similar for both tools. Intrarater and interrater intraclass correlation coefficients (ICC) ranged from 0.69 to 0.93, and the standard errors of measurements ranged from 2.4º to 4.9º. In the clinical study, for proximal interphalangeal joints, intrarater ICCs were above 0.99, and Interrater (patient versus rater) ICCs were below 0.033.
