INTRODUÇÃO: A alergia à proteína do leite de vaca (APLV) é a alergia alimentar (AA) mais comum na infância e o desenvolvimento de tolerância a este alimento vem ocorrendo cada vez mais tardiamente. OBJETIVOS: 1. Avaliar as características epidemiológicas, clínicas e laboratoriais de pacientes tolerantes e daqueles com alergia persistente à APLV. 2. Estabelecer o diâmetro da pápula do Teste cutâneo de Hipersensibilidade Imediata (TCHI) com extrato do leite de vaca (LV) associado ao desenvolvimento de tolerância oral (TO). 3. Comparar o diâmetro da pápula do TCHI, realizado com extrato de LV, entre os pacientes dos grupos tolerante e persistente, em três tempos distintos, no diagnóstico de APLV, no tempo médio da evolução da doença e no momento da aquisição de tolerância ou na data do último TCHI no grupo que persistiu alérgico. MÉTODOS: Trata-se de um estudo de coorte retrospectivo de análise de banco de dados dos prontuários de todas as crianças com diagnóstico de APLV mediada pela imunoglobulina E (IgE), em seguimento na Unidade de Alergia e Imunologia no Instituto da Criança do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (ICr HCFMUSP), que realizaram TCHI de forma evolutiva, sendo alocados em dois grupos: APLV tolerantes e APLV persistentes, no período entre janeiro de 2000 a julho de 2015. O TCHI foi realizado por equipe treinada e com extrato padronizado de LV total, sendo considerado positivo quando o diâmetro médio da pápula foi >= 3 mm em relação ao controle negativo. As variáveis nominais: gênero, história familiar de atopia, uso de LV no berçário, amamentação por menos de 6 meses, presença de outras doenças alérgicas foram descritas através de frequências e comparadas entre tolerantes e persistentes. No grupo tolerante as mesmas variáveis foram comparadas entre aqueles com idade 5 anos pelo Teste de Fisher. As variáveis contínuas (idade do início sintomas e idade na coleta dos dados, bem como a dosagem de IgE específica para LV pelo ImmunoCap) nos mesmos grupos foram descritas através de medianas e comparadas através teste de Mann-Whitney e Wilcoxon. Os resultados do TCHI (em mm) foram descritos através de medianas, valores mínimos e máximos ao diagnóstico, no tempo médio e ao final, no grupo de tolerantes e de persistentes e comparados em cada tempo pelo teste de Mann-Whitney. A comparação das medianas nos 3 tempos em cada grupo foram realizadas através teste de Friedman. Em todas as análises foi adotado um nível de significância de 5%. RESULTADOS: Após a aplicação de critérios de inclusão e exclusão, obteve-se uma amostra de 44 pacientes, 29 no grupo que desenvolveu tolerância, e 15 no grupo que persistiu com APLV na época da conclusão deste estudo. Não houve diferença estatisticamente significante entre os diferentes achados epidemiológicos e clínicos entre os grupos tolerantes e persistentes. No grupo tolerante ao LV foram encontradas as seguintes medidas de TCHI: ao diagnóstico a mediana foi de 6 mm (3 mm - 15 mm), no tempo médio de seguimento de 3 mm (0 mm - 15 mm) e no desenvolvimento de tolerância de 2 mm (0 mm - 5 mm), valores que mostraram diferença estatisticamente significante nos três períodos avaliados (p < 0,0001; teste de Friedman). No grupo de pacientes persistentes ao LV foram encontradas as seguintes medidas de TCHI: ao diagnóstico mediana de 7 mm (3 mm - 13 mm); no tempo médio de 4 mm (2 mm - 20 mm) e no momento do último TCHI realizado até o término deste estudo a mediana foi de 5mm (0 mm - 19 mm), não havendo diferença estatisticamente significante nos três períodos em que os pacientes foram avaliados (p= 0,256; teste de Friedman). Avaliando o decaimento dos valores do TCHI no grupo de pacientes tolerantes, a mediana do TCHI ao diagnóstico foi de 6mm (3 - 15 mm) e no momento da tolerância de 2 mm (0 mm - 5 mm), com diferença estatisticamente significante entre os valores (p < 0,0001; Teste de Wilcoxon). No grupo persistente não houve diferença estatisticamente significante entre o primeiro e o último TCHI, com p = 0,173 (Teste de Wilcoxon). CONCLUSÃO: O estudo mostrou que a utilização do TCHI para o diagnóstico e detecção de tolerância na APLV é uma ferramenta útil, devendo ser realizado de maneira seriada ao longo da evolução na APLV e seu decaimento poderia indicar o momento para realização de testes de provocação oral (TPO) / BACKGROUND: Cow\'s milk allergy (CMA) is the most common food allergy in childhood and the development of tolerance to this food has been occurred increasingly later. OBJECTIVES: 1.To assess the epidemiological, clinical and laboratory characteristics of tolerant patients and those with persistent allergy to CMA. 2. To establish the wheal diameter of the Skin Prick Test (SPT) with cow\'s milk (CM) extract associated with the development of oral tolerance (OT). 3. To Compare the wheal diameter of the SPT, carried out with CM extract, among the patients of the tolerant and persistent groups in three distinct periods: during the diagnosis of CMA, during the average time of disease progression, and at the moment of tolerance acquisition or on the date of the last SPT in the group that persisted allergic. METHODS: This was a retrospective cohort study of database analysis of medical records of all children diagnosed with CMA mediated by immunoglobulin E (IgE), following the Allergy and Immunology Unit at the Children\'s Institute of the HCFMUSP (between jan/2000 to jul/2015) who underwent SPT in an evolutionary way. They were classified into two groups: CMA tolerant and CMA persistent. The SPT was carried out by trained personnel and with a standardized extract of whole CM, being considered positive when the average wheal diameter was >= 3 mm compared to the negative control. The nominal variables of gender, family history of atopy, CM use in the nursery, breastfeeding for less than six months, and the presence of other allergic diseases were described by frequencies and compared between tolerant and persistent groups. In the tolerant group, the same variables were compared among those aged 5 years by the Fisher Test. Continuous variables (age of onset symptoms and age in the data collection as well as specific IgE levels to CM by ImmunoCap) in the same groups were described by medians and compared using the Mann-Whitney and Wilcoxon test. The results of the SPT (in mm) were described by median, minimum and maximum values for the diagnosis, at the middle and at the end point in the tolerant and persistent groups, and at each time point were compared by way of the Mann-Whitney test. Comparison of the medians in the 3 periods in each group were made using the Friedman test. In all analyses, a significance level of 5% was adopted. RESULTS: After applying inclusion and exclusion criteria, we obtained a sample of 44 patients, 29 in the group that developed tolerance, and 15 in the group that persisted with CMA at the time of completion of this study. There was no statistically significant difference between the different epidemiological and clinical findings between the tolerant and persistent groups. In the CM tolerant group the following SPT measures were found: during diagnosis the median average was 6 mm (3 mm - 15 mm), during the average follow-up time it was 3 mm (0 mm - 15 mm), and for the development of tolerance it was 2 mm (0 mm - 5 mm); values that showed statistically significant differences in the three evaluation periods (p < 0.0001; Friedman test). In the group of patients with persistent allergy to CM, a SPT found the following measures: during diagnosis the median average was 7 mm (3 mm - 13 mm); during the average follow-up time it was 4 mm (2 mm - 20 mm), and at the time of the last SPT performed until the end of the study the median was 5 mm (0 mm - 19 mm), with no statistically significant difference in the three periods (p = 0.256, Friedman test). Evaluating the decay of SPT values in the group of tolerant patients, the median SPT at diagnosis was 6 mm (3-15 mm) and, at the time of tolerance, 2 mm (0 mm - 5 mm), with a statistically significant difference between the values (p < 0.0001; Wilcoxon test). In the persistent group there was no statistically significant difference between the first and the last SPT, p = 0.173 (Wilcoxon test). CONCLUSION: The study showed that the use of the SPT for the diagnosis and detection of tolerance to the CMA is a useful tool and should be performed sequentially throughout the evolution of the CMA and that its decay could indicate the time for performing oral food challenge (OFC)

A reabsorção óssea alveolar associada as perdas dentais constitui uma condição inerente ao processo de cicatrização fisiológico natural. Técnicas como a instalação de implantes imediatos e técnicas de preservação alveolar têm sido sugeridas com o objetivo de limitar as alterações adversas sofridas pelo processo alveolar. Contudo, frequentemente as perdas dentárias estão associadas a infecções crônicas que tradicionalmente contraindicariam os procedimentos de enxerto ou implantes imediatos, a menos que meticulosos debridamento e irrigação alveolar associados a adequado protocolo antibiótico pré e pós-operatório sejam empregados. Alternativas precisam ser testadas para a substituição do uso indiscriminado de antibioticoterapia sistêmica. O objetivo do presente estudo foi avaliar o efeito da TFDa na descontaminação de alvéolos infectados previamente a instalação de implantes imediatos e na cicatrização de alvéolos pós-extração associados ou não ao uso de enxertos xenógeno através de análises microtomográficas e histomorfométricas. Para tanto, foram selecionados 8 cães, os quais foram submetidos a uma fase de indução de doença periodontal por ligadura, seguida por um período de estabelecimento da doença. Após a exodontia dos prémolares bilaterais, aleatoriamente os alvéolos de um lado da mandíbula foram descontaminados por debridamento mecânico associado a irrigação com solução salina (grupo controle) e do outro lado por debridamento mecânico e irrigação com solução salina associados a terapia fotodinâmica antimicrobiana (grupo teste), e subsequentemente submetidos a instalação de implantes imediatos, dando origem aos grupos GT-I (Grupo Teste - Implante) e GC-I (Grupo Controle - Implante). Os demais alvéolos, foram utilizados para o estudo da dinâmica de cicatrização alveolar. Os sítios foram aleatoriamente alocados em: GT-C (Grupo teste - Coágulo), GT-BO (Grupo Teste - Bio-Oss®), GT-BOC (Grupo Teste - Bio-Oss® Collagen), GC-C (Grupo Controle - Coágulo), GC-BO (Grupo Controle - Bio-Oss®) e GC-BOC (Grupo Controle - Bio-Oss® Collagen). Após 12 semanas, os cães foram sacrificados e as amostras processadas para as análises de microtomografia computadorizada, histologia e histomorfometria. Na avaliação da cicatrização dos alvéolos pós-extração, embora os alvéolos descontaminados com TFDa (GT-C, GT-BO, GT-BOC) tenham apresentado melhores resultados numéricos, em relação a altura da crista óssea vestibular (ACOV) e a dimensão buco-lingual (DBL), não foram evidenciadas diferenças relevantes na análise histomorfométrica. Apenas a ACOV, mensurada na avaliação microtomográfica bidimensional, demonstrou-se significativamente inferior nos alvéolos do grupo teste que não receberam material de enxerto (GT-C) quando comparados aos respectivos alvéolos do grupo controle (GC-C). Na avaliação dos implantes imediatos, a análise histomorfométrica dos parâmetros: reabsorção da crista óssea vestibular (RCOV) e contato osso implante (BIC) demonstrou resultados significantemente superiores no GT-I em relação ao GC-I, e do parâmetro densidade óssea (DO) demonstrou apenas resultado numericamente superior no GT-I em relação ao GC-I. Todas as análises microtomográficas bidimensionais (RCOV) e tridimensionais (volume ósseo BV, porcentagem óssea BV/TV, densidade de superfície óssea BS/TV, espessura trabecular Tb.TH, número de trabéculas Tb.N e separação trabecular Tb.SP) demonstraram resultados significantemente melhores nos implantes do grupo teste (GT-I) em relação aos implantes do grupo controle (GC-I). A TFDa demonstrou potencial como agente de descontaminação de alvéolos pósextração periodontalmente infectados, previamente a instalação de implantes imediatos e na cicatrização de alvéolos pós-extração associados ou não a materiais de enxerto xenógenos, sem o uso de antibioticoterapia sistêmica associada. / Alveolar bone resorption following tooth loss is an inherent condition of the natural healing process. Therefore, several techniques, such as immediate implants placement and post extraction socket preservation, have been suggested in order to limit the adverse changes suffered by the alveolar process. However, extraction sockets commonly results from the removal of teeth affected by chronic infection, which conventionally contraindicates immediate bone graft and implant placement unless meticulous wound debridement and alveolar irrigation associated to a suitable pre- and post-operatory antibiotic protocol are employed. Alternatives ought to be tested in order to substitute the indiscriminate use of systemic antibiotic therapy. The aim of the present study was to evaluate the effect of the antimicrobial photodynamic therapy (aPDT) in the decontamination of infected post extraction sockets, previously to immediate implant placement and in the healing of post extraction sockets associated or not to xenografts. In the first phase, periodontitis was induced with ligatures in the mandibular premolars of eight beagle dogs. After 2 months, in the second phase of the study the dogs had their mandibular bicuspids bilaterally extracted, and randomly one hemi-mandible was decontaminated by mechanic debridement associated to saline solution irrigation (Control Group - CG), and the other hemi-mandible was decontaminated with mechanic debridement and saline solution irrigation associated to antimicrobial photodynamic therapy (Test Group - TG). Thereafter, 3 immediate implants in each side of the mandible were placed and the following groups were devised: TG-I (Test Group - Implant) and CG-I (Control Group - Implant). The remaining sockets were used for the study of the healing dynamic. The sockets were randomly assigned to the following groups: Test Group - Blood clot (TG-BC), Test Group Bio- Oss® (TG-BO), TG - BOC (Test Group Bio-Oss® Collagen), Control Group - Blood Clot (CG-BC), Control Group - Bio-Oss® (CG-BO) and Control Group - Bio-Oss® Collagen (CG-BOC). After 12 weeks, the dogs were sacrificed and the specimens were processed for microtomographic, histological and histomorphometric analysis. When the post extraction healing process was evaluated, the aPDT decontaminated sockets (TG-BC, TG-BO and TG-BOC) presented better numerical results in comparison to both buccal bone crest height (BCL) and in the bucco-lingual dimension (BLD). However, there were no statistically differences among the groups for these parameters in the histomorphometric analysis. Only the BCL measured in the two-dimensional microtomographic analysis showed statistic better results in the TG-BC when compared to the CG-BC. In the evaluation of the immediate implant placement the histomorphometric analysis presented statistically better results for the TG-I in the bone-implant contact (BIC), as well as in the vertical buccal bone loss (VBBL). The bone density (BD) was numerically better in the TG-I than in the CG-I. Both two-dimensional (VBBL) and three-dimensional (bone volume BV, percentage of the total bone volume - BV/TV, bone surface density - BS/TV, trabecular thickness Tb.Th, trabecular separation Tb.Sp and trabecular number TB.N) microtomographic analysis showed statistically better results in the TG-I. The aPDT showed potential in the decontamination of infected post extraction sockets previously to immediate implant placement and in the healing of post extraction sockets associated or not to xenogeneic grafts, without the use of systemic antibiotics.

O que acontece durante o período de espera?: contribuições para o estudo do autocontrole / What happens during the waiting period?: contributions to the study of self-control

This was made in order to answer the following question: which responses children emit while they were waiting for larger and delayed reinforcers? Does it occur chaining or stereotyped behaviors during the waiting period? Are there any relations between what is done and the waiting time so that it would facilitate the waiting period? Can a hand puppet work as an audience to the participants so that it could extend the chain of public events and diminish the chain of covert events? Those children that were waiting in pairs or using a puppet waited for longer periods in order to receive larger and delayed reinforcers? The experimental design was based on the Mischel&#8223;s, Ebbesen e Zeiss (1972) study. Fifteen children participated in the study divided in three experimental conditions: alone , hand puppet and peers . They should wait for 15 minutes so that they would obtain two chocolates. If they didn&#8223;t wait, they should ring a bell and the experimenter would return and the children/child would gain the chocolate that was close to her. The results showed that four of the 15 children didn&#8223;t wait for the whole time. The children emitted 14 categories of responses and the more frequent categories were similar for all of them. In the condition alone it was observed a smaller variation on the distribution of the responses between the categories and a smaller number of responses per minute. The condition hand puppet was the only one in which all the participants waited. However, only one participant emitted public oral verbal responses, which doesn&#8223;t allow the experimenter to say that the hand puppet enhancing the emission of these verbal responses. Otherwise, the report of one of the participants does not allow us to discard it entirely as an audience. The standard of the observed responses on the hand puppet condition was similar to the alone condition, but with a higher average of responses per minute. For the peers condition was possible to observe that an interlocutor facilitated the waiting and enhanced the number of responses in new categories. This condition presented the higher average of responses per minute. The results indicated that oral verbal responses were emitted almost exclusively in this condition. The categorized verbal operants most common were tacts and mands about the experimental condition and unrelated subjects . Stereotypy was observed in only one participant (P5). The low variability in the 'alone' condition and greater variability in the other conditions have revealed that more complex environments allowed the participants to respond into new categories. The children who had longer waiting times were the ones with the most responses per minute and/or responded in as many categories as possible / O objetivo do presente trabalho foi identificar variáveis que poderiam contribuir ou não para que crianças emitissem respostas que produziriam reforçadores maiores e atrasados. A partir de então, responder as seguintes perguntas: quais as respostas que crianças emitem enquanto esperam por reforçadores maiores e atrasados? Dentre as respostas observadas, existem encadeamentos ou estereotipias comportamentais? Existe algum tipo de relação ente o que se faz e o tempo de espera de maneira a facilitar a espera? Um fantoche poderia funcionar como audiência para o participante ampliando assim a cadeia de eventos públicos e diminuindo por sua vez a cadeia de eventos encobertos? Crianças esperando em duplas ou usando um fantoche teriam maiores tempos de espera por reforçadores maiores e atrasados? O arranjo experimental foi baseado nos estudos de Mischel, Ebbesen e Zeiss (1972). Participaram 15 crianças divididas em três condições experimentais: sozinha&#8223;, fantoche&#8223; e duplas&#8223;. Elas deveriam esperar 15 minutos para obter dois chocolates e caso não esperassem, tocariam um sino, o experimentador retornaria e a criança ganharia o chocolate que estava junto dela. Os resultados mostraram que quatro das 15 crianças, não esperaram o tempo total. As crianças emitiram 14 categorias de respostas e as mais frequentes foram semelhantes para todas. Na condição sozinho&#8223; houve menor variação na distribuição das respostas entre as categorias e menor número de respostas por minuto. A condição fantoche&#8223; foi a única em que todos os participantes esperaram. Entretanto apenas um participante emitiu respostas verbais orais públicas, e isto não permite afirmar que o fantoche foi capaz de aumentar a emissão destas respostas verbais. Todavia o relato de um dos participantes não nos permite descartá-lo completamente como audiência. O padrão de respostas observáveis da condição fantoche&#8223; foi semelhante ao da condição sozinho&#8223;, mas com uma média maior de respostas por minuto. Para a condição duplas&#8223; foi possível observar que ter um interlocutor facilitou a espera e aumentou o número de respostas em novas categorias. Esta condição apresentou as maiores médias de respostas por minuto. Os resultados indicaram que as respostas verbais orais foram quase que exclusivamente nesta condição e os operantes verbais classificados mais comuns foram tatos e mandos sobre a condição experimental&#8223; e assuntos não relacionados&#8223;. Foi observada estereotipia em apenas um participante (P5). A pouca variabilidade na condição sozinho&#8223; e a maior variabilidade nas outras condições permitem afirmar que ambientes mais complexos permitiram a emissão de respostas em novas categorias. As crianças que tiveram tempos de espera mais longos foram as que apresentaram mais respostas por minuto e/ou responderam em tantas categorias quantas possíveis

Organização dos trabalhadores do Sistema Único de Assistência Social no contexto das terceirizações, privatizações e precarização do trabalho

Connected to the stories of our time and the past, as a social product, and historically experienced by men, we ve traveled through the path of resistances and struggles needed to confront the material and objective conditions that make the daily life of these workers precarious. They ve experienced job and income fluctuation, flexibility in labor rights, temporary employment contracts of individuals or independent legal entities, subcontracting, outsourcing, lack of labor social protection, all contributing to the destruction of conquests and rights historically forged by the working class. Based on dialectical materialist Marxist method, this research values the dialogues with authors and works with conceptual basis grounded on the articulation of critical knowledge of the studied object, using the conception that reality will explain the ideas and not the opposite. The representation spaces of workers interests from the Social Assistance Single System have been immersed in a heatproof environment to class struggle and haven t even advocated immediate interests and claims, if they go against the ruling power. We use semi structured interviews with unions and opposition groups, as well as the formation of a focal group with workers in order to discuss the most important questions that the issue raises among municipal public servants in Campinas and those employed privately by the Social Assistance Single System. These data unveil their importance in the research investigative path, not only from a quantitative point of view, but in the sense that the individuals involved are products of a historical time / Esta dissertação pretende analisar as formas de organização dos trabalhadores do Sistema Único da Assistência Social (SUAS), no município de Campinas/SP, num contexto em que a reestruturação produtiva, as políticas privatizantes e a refuncionalização do Estado têm atuado para manutenção e sobrevivência do capital, impondo penúria e miséria para a classe trabalhadora. Conectadas com as histórias do nosso tempo e com o passado, enquanto produto social historicamente vivido pelos homens, perquirimos o caminho das resistências e lutas necessárias para o enfrentamento das condições materiais e objetivas presentes no cotidiano dos trabalhadores, que vivem a instabilidade de emprego e renda, flexibilização da legislação social do trabalho, contratos temporários de pessoas físicas ou jurídicas autônomas, subcontratados, terceirizados, ausência de proteção social do trabalho, enfim, tudo concorrendo para a destruição de conquistas e direitos historicamente forjados pela classe trabalhadora. Fundamentada no método marxista dialético-materialista, a pesquisa valoriza diálogos com autores e obras cuja base conceitual se sustenta na articulação do conhecimento crítico com o objeto estudado, na concepção de que a realidade vai explicando as ideias e não o seu contrário. Os espaços de representação dos interesses dos trabalhadores do SUAS estão imersos numa conjuntura refratária às lutas de classe e tampouco têm defendido interesses e reivindicações imediatas, se estas contrariam o poder vigente. Recorremos a entrevistas semiestruturadas com sindicatos e grupos de oposições, bem como à formação de um grupo focal com as trabalhadoras na perspectiva de problematizar as questões candentes que o tema suscita entre os trabalhadores concursados da Prefeitura Municipal de Campinas e os que estão contratados pela rede privada do SUAS. Esses dados desvelam sua importância no caminho investigativo da pesquisa, não apenas do ponto de vista quantitativo, mas no sentido de que os sujeitos envolvidos são produtos de um tempo histórico

Os traumatismos nas dentições decíduas e permanentes e suas estruturas de suporte ocorrem com grande freqüência em crianças e diversos estudos epidemiológicos identificaram a escola como um local com alta freqüência destes tipos de traumatismos. Desta forma, é comum que o atendimento imediato da criança seja realizado pelos professores, os quais podem ser responsáveis pelo insucesso do tratamento da lesão pelo cirurgião-dentista, caso haja falhas no manuseio dos dentes traumatizados no local do acidente. Assim, esta pesquisa teve como objetivo verificar os conhecimentos de professores do Ensino Fundamental sobre procedimentos de urgência nos traumatismos alvéolo-dentários com especial enfoque sobre as avulsões, seu reimplante e acondicionamento e avaliar a necessidade de informações adicionais sobre traumatismos destes profissionais. As escolas pertencentes ao bairro Butantã foram convidadas a participar deste estudo que consistiu na aplicação de um questionário com perguntas referentes a: dados pessoais como gênero, idade, experiência profissional, nível de instrução, disciplina lecionada, treinamento em primeiros socorros, assim como perguntas relativas a noções de atendimento ao aluno vítima de traumatismo alvéolo-dentário referente a simulações de um caso de fratura dental e um outro de avulsão dental, procedimentos de urgência em casos de avulsão e a medicação utilizada. Os resultados obtidos a partir de 177 questionários retornados pelos professores foram tabulados, apurados e analisados estatisticamente através do software Epi Info 6. Os resultados demonstraram que os professores apresentaram um número maior de respostas corretas nas questões que envolveram especificamente fratura dental, tempo e local de procura por atendimento profissional, sendo que para as questões relativas ao reimplante e acondicionamento de dentes avulsionados as respostas foram inadequadas. Verificou-se que a responsabilidade pelo atendimento urgencial de um aluno vítima de algum acidente, segundo a amostra consultada, deveria ficar a cargo de toda a equipe escolar e não somente ao professor. Finalmente, constatou-se que os professores da amostra possuem pouco conhecimento sobre o tema, o que, associado à falta de treinamento, pode torná-los despreparados para solucionar adequadamente eventuais acidentes que envolvam os escolares em traumas dento-alveolares. Esta limitação de conhecimentos quanto aos procedimentos adequados relacionados aos traumatismos alvéolo-dentários, principalmente as avulsões, reforça a necessidade de educação para o público leigo não apenas no que se refere ao atendimento inicial de um traumatismo alvéolo-dentário, mas também no que se concerne às medidas preventivas. / Several epidemiological studies have reported that schoolchildren are prone to suffer accidents in their school environment that could cause dental trauma to either primary or permanent teeth and the corresponding supporting structures. These children normally receive first aid care from teachers who are not adequately trained to handle traumatized teeth at the site of the accident and therefore could lessen the chances of success of the dentist in treating the injury. The aim of our study was to evaluate the degree of information that elementary school teachers have when dealing with emergency procedures concerning replant and storage of avulsed teeth and consider the importance of offering further education on dental trauma. Our method consisted in applying a questionnaire to elementary schoolteachers at the district of Butanta, São Paulo. Questions included information concerning personal data such as: gender, age, professional experience, level of education, discipline taught, and first aid training, mainly in relation to dental trauma. Also included were a case simulations of dental fractures, a case of dental avulsion and questions referring to emergency procedures on dental avulsion and medication. The results obtained from the 177 questionnaires answered by the teachers were tabulated, confirmed and statistically analyzed by the software Epi Info 6. Questions involving dental fracture and time and place for pursuing professional help received the most correct answers whereas the ones referring to replant and storage of avulsed teeth were unsatisfactory. According to investigated sample not only the teachers, but also all the school personnel should be responsible for adequate emergency management of students suffering accidents, including dental trauma. The teachers involved in our study demonstrated very little knowledge on the subject and therefore their lack of adequate practical training results in inadequate management of situations involving accidents leading to dento-alveolar traumas, mainly avulsions. We reinforce the need of education of lay people dealing with the initial management of a dental trauma as well as orientation on preventive measures.

Finite element analysis of short-term and long-term building response to tunnelling

Tunnelling in urban areas causes short-term and long-term ground movements under existing buildings. Finite element analysis provides a useful option for assessing the likely extent of damage induced in these buildings. Although finite element analysis is suggested to be used in the later phases of the building damage assessment procedures employed in practice, only the effect of short-term ground movements is typically considered and there are no detailed guidelines on the specification and complexity of the modelling. This thesis addresses the tunnel-soil-building interaction problem and the effect of long-term consolidation, as well as demonstrating the application of 3D finite element analysis with appropriate simplifications for practical assessment purposes. Finite element models are developed to quantify the effect of shallow tunnelling on an example masonry building founded on strip footings, considering both single- and twin-tunnel scenarios in a typical London soil profile. Total stress and effective stress analyses are adopted with specific modelling procedures to focus on the short-term and long-term response respectively. The analyses use a non-linear model for the masonry, and allow slippage and gapping at the soil-footing interface. Two advanced constitutive models for the soil (the extended Mohr-Coulomb model and the modified two-surface kinematic hardening model) are implemented with customized stress update schemes. The finite element results present the interaction between the soil and the building by comparing with the greenfield ground response. The horizontal coupling between the foundation and the ground is shown to be relatively weak. The dominant deformation mode of the building varies with the tunnel configuration (i.e. single or twin tunnels) and the tunnel eccentricity. Strain localization is found around the explicitly modelled window and door openings. The long-term consolidation is sensitive to the permeability of the tunnel lining. The building response to long-term ground movements is further affected by the tunnel-tunnel interaction in the case of twin-tunnel configuration. Performing 3D analysis of a single facade and foundation provides useful damage predictions, without the need to model a complete building. The proposed result processing methods such as characteristic strain and damage bar chart are practical tools for assessment. The study highlights some limitations of the elastic beam assessment method, which is often adopted in the early phase of the damage assessment process.

In the conclusion I try to evaluate the effective legal regulation of legal acts at the termination of employment and to propose some possible changes of this legal regulation. INTRODUÇÃO: A alergia à proteína do leite de vaca (APLV) é a alergia alimentar (AA) mais comum na infância e o desenvolvimento de tolerância a este alimento vem ocorrendo cada vez mais tardiamente. OBJETIVOS: 1. Avaliar as características epidemiológicas, clínicas e laboratoriais de pacientes tolerantes e daqueles com alergia persistente à APLV. 2. Estabelecer o diâmetro da pápula do Teste cutâneo de Hipersensibilidade Imediata (TCHI) com extrato do leite de vaca (LV) associado ao desenvolvimento de tolerância oral (TO). 3. Comparar o diâmetro da pápula do TCHI, realizado com extrato de LV, entre os pacientes dos grupos tolerante e persistente, em três tempos distintos, no diagnóstico de APLV, no tempo médio da evolução da doença e no momento da aquisição de tolerância ou na data do último TCHI no grupo que persistiu alérgico. MÉTODOS: Trata-se de um estudo de coorte retrospectivo de análise de banco de dados dos prontuários de todas as crianças com diagnóstico de APLV mediada pela imunoglobulina E (IgE), em seguimento na Unidade de Alergia e Imunologia no Instituto da Criança do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (ICr HCFMUSP), que realizaram TCHI de forma evolutiva, sendo alocados em dois grupos: APLV tolerantes e APLV persistentes, no período entre janeiro de 2000 a julho de 2015. O TCHI foi realizado por equipe treinada e com extrato padronizado de LV total, sendo considerado positivo quando o diâmetro médio da pápula foi >= 3 mm em relação ao controle negativo. As variáveis nominais: gênero, história familiar de atopia, uso de LV no berçário, amamentação por menos de 6 meses, presença de outras doenças alérgicas foram descritas através de frequências e comparadas entre tolerantes e persistentes. No grupo tolerante as mesmas variáveis foram comparadas entre aqueles com idade 5 anos pelo Teste de Fisher. As variáveis contínuas (idade do início sintomas e idade na coleta dos dados, bem como a dosagem de IgE específica para LV pelo ImmunoCap) nos mesmos grupos foram descritas através de medianas e comparadas através teste de Mann-Whitney e Wilcoxon. Os resultados do TCHI (em mm) foram descritos através de medianas, valores mínimos e máximos ao diagnóstico, no tempo médio e ao final, no grupo de tolerantes e de persistentes e comparados em cada tempo pelo teste de Mann-Whitney. A comparação das medianas nos 3 tempos em cada grupo foram realizadas através teste de Friedman. Em todas as análises foi adotado um nível de significância de 5%. RESULTADOS: Após a aplicação de critérios de inclusão e exclusão, obteve-se uma amostra de 44 pacientes, 29 no grupo que desenvolveu tolerância, e 15 no grupo que persistiu com APLV na época da conclusão deste estudo. Não houve diferença estatisticamente significante entre os diferentes achados epidemiológicos e clínicos entre os grupos tolerantes e persistentes. No grupo tolerante ao LV foram encontradas as seguintes medidas de TCHI: ao diagnóstico a mediana foi de 6 mm (3 mm - 15 mm), no tempo médio de seguimento de 3 mm (0 mm - 15 mm) e no desenvolvimento de tolerância de 2 mm (0 mm - 5 mm), valores que mostraram diferença estatisticamente significante nos três períodos avaliados (p < 0,0001; teste de Friedman). No grupo de pacientes persistentes ao LV foram encontradas as seguintes medidas de TCHI: ao diagnóstico mediana de 7 mm (3 mm - 13 mm); no tempo médio de 4 mm (2 mm - 20 mm) e no momento do último TCHI realizado até o término deste estudo a mediana foi de 5mm (0 mm - 19 mm), não havendo diferença estatisticamente significante nos três períodos em que os pacientes foram avaliados (p= 0,256; teste de Friedman). Avaliando o decaimento dos valores do TCHI no grupo de pacientes tolerantes, a mediana do TCHI ao diagnóstico foi de 6mm (3 - 15 mm) e no momento da tolerância de 2 mm (0 mm - 5 mm), com diferença estatisticamente significante entre os valores (p < 0,0001; Teste de Wilcoxon). No grupo persistente não houve diferença estatisticamente significante entre o primeiro e o último TCHI, com p = 0,173 (Teste de Wilcoxon). CONCLUSÃO: O estudo mostrou que a utilização do TCHI para o diagnóstico e detecção de tolerância na APLV é uma ferramenta útil, devendo ser realizado de maneira seriada ao longo da evolução na APLV e seu decaimento poderia indicar o momento para realização de testes de provocação oral (TPO) / BACKGROUND: Cow\'s milk allergy (CMA) is the most common food allergy in childhood and the development of tolerance to this food has been occurred increasingly later. OBJECTIVES: 1.To assess the epidemiological, clinical and laboratory characteristics of tolerant patients and those with persistent allergy to CMA. 2. To establish the wheal diameter of the Skin Prick Test (SPT) with cow\'s milk (CM) extract associated with the development of oral tolerance (OT). 3. To Compare the wheal diameter of the SPT, carried out with CM extract, among the patients of the tolerant and persistent groups in three distinct periods: during the diagnosis of CMA, during the average time of disease progression, and at the moment of tolerance acquisition or on the date of the last SPT in the group that persisted allergic. METHODS: This was a retrospective cohort study of database analysis of medical records of all children diagnosed with CMA mediated by immunoglobulin E (IgE), following the Allergy and Immunology Unit at the Children\'s Institute of the HCFMUSP (between jan/2000 to jul/2015) who underwent SPT in an evolutionary way. They were classified into two groups: CMA tolerant and CMA persistent. The SPT was carried out by trained personnel and with a standardized extract of whole CM, being considered positive when the average wheal diameter was >= 3 mm compared to the negative control. The nominal variables of gender, family history of atopy, CM use in the nursery, breastfeeding for less than six months, and the presence of other allergic diseases were described by frequencies and compared between tolerant and persistent groups. In the tolerant group, the same variables were compared among those aged 5 years by the Fisher Test. Continuous variables (age of onset symptoms and age in the data collection as well as specific IgE levels to CM by ImmunoCap) in the same groups were described by medians and compared using the Mann-Whitney and Wilcoxon test. The results of the SPT (in mm) were described by median, minimum and maximum values for the diagnosis, at the middle and at the end point in the tolerant and persistent groups, and at each time point were compared by way of the Mann-Whitney test. Comparison of the medians in the 3 periods in each group were made using the Friedman test. In all analyses, a significance level of 5% was adopted. RESULTS: After applying inclusion and exclusion criteria, we obtained a sample of 44 patients, 29 in the group that developed tolerance, and 15 in the group that persisted with CMA at the time of completion of this study. There was no statistically significant difference between the different epidemiological and clinical findings between the tolerant and persistent groups. In the CM tolerant group the following SPT measures were found: during diagnosis the median average was 6 mm (3 mm - 15 mm), during the average follow-up time it was 3 mm (0 mm - 15 mm), and for the development of tolerance it was 2 mm (0 mm - 5 mm); values that showed statistically significant differences in the three evaluation periods (p < 0.0001; Friedman test). In the group of patients with persistent allergy to CM, a SPT found the following measures: during diagnosis the median average was 7 mm (3 mm - 13 mm); during the average follow-up time it was 4 mm (2 mm - 20 mm), and at the time of the last SPT performed until the end of the study the median was 5 mm (0 mm - 19 mm), with no statistically significant difference in the three periods (p = 0.256, Friedman test). Evaluating the decay of SPT values in the group of tolerant patients, the median SPT at diagnosis was 6 mm (3-15 mm) and, at the time of tolerance, 2 mm (0 mm - 5 mm), with a statistically significant difference between the values (p < 0.0001; Wilcoxon test). In the persistent group there was no statistically significant difference between the first and the last SPT, p = 0.173 (Wilcoxon test). CONCLUSION: The study showed that the use of the SPT for the diagnosis and detection of tolerance to the CMA is a useful tool and should be performed sequentially throughout the evolution of the CMA and that its decay could indicate the time for performing oral food challenge (OFC)

The aim of this thesis is to map the work of a psychologist in facilities for children requiring immediate help (FCRIH). The partial aim of the thesis is to compare the work of a psychologist in FCRIH with the work of a psychologist in school facilities of institutional and protective care. In the theoretical part the thesis deals with the system of alternative care for children and describes the various forms of care. After that the theoretical part focuses on FCRIH itself, especially the legislative framework of these facilities. The following chapter is focused on children in FCRIH and thus defines the work of a psychologist in these facilities in terms of the content of his work. The last chapter of the theoretical part focuses on psychological care in FCRIH and school facilities of institutional and protective care in order to compare them. The practical part was processed qualitatively through online questionnaires for directors and psychologist of FCRIH and subsequently semi-structured interviews with FCRIH psychologists were conducted. La thèse propose d’étudier l’émergence et la circulation du syntagme printemps arabe(s) comme dénomination événementielle dans la presse écrite. Elle a pour objectif principal d’examiner le processus de dénomination et de qualification des événements historiques dans et par les médias représentés ici par la presse écrite, en retraçant le parcours d’un syntagme-clé de l’Histoire immédiate. L’étude s’inscrit dans le domaine de l’analyse linguistique des discours médiatiques. Elle fait appel à plusieurs champs théoriques croisant sciences du langage et sciences de l’information et de la communication. La démarche relève de l’analyse du discours comme domaine de recherche - sur la langue, le discours et le sens - ancré dans les sciences du langage, où le traitement automatique se présente comme un outil d’investigation. Le corpus d’étude est formé d’un ensemble de textes de la presse écrite internationale d’expression française, arabe et dans une moindre mesure anglaise. Au-delà de cette étude de cas, l’étude sert d’exemple d’analyse lexico-discursive de la nomination des événements dans la presse écrite, comme de l’émergence et de la circulation d’un syntagme dénominatif lié à un moment historique et dont l’impact dépasse les frontières du monde arabe. / The thesis proposes to study the emergence and the circulation of Arab Spring phrase as event denomination in the press. Its main objective is to examine the process of naming and qualifying historical events in the media represented here by the written press. Going back over the course of a key phrase in the immediate history is the central object of the study.The research is a case of media discourse analysis. It uses several theoretical fields crossing language sciences and communication sciences. The approach is based on the analysis of discourse as a field of research - on language, discourse and meaning - rooted in the language sciences, where automatic processing presents a tool for study. The corpus of study is composed of the texts of the international written press.Beyond this specific case, the study serves as an example of lexical and discursive analysis of naming events in the written press, as well as the emergence and circulation of a denominational phrase associated with a historical moment.
