Improved Rehabilitation by Improving Discharge Processes to Decrease Readmissions

Inadequate discharge planning for individuals with chronic illnesses or injuries is associated with increased readmissions to the hospital or rehabilitation facility where the original treatments were administered. To help ensure the recovery of discharged patients and avoid readmissions, discharge planners guide medication and care processes. The rate of readmissions was high in a stand-alone rehabilitation center due to ineffective discharge plans. Patients, family members, and caregivers lacked knowledge about medications, treatments, and self-care guidelines after the patient left the facility. The purpose of this project was to ascertain the impact of improved discharge processes using the (a) IDEAL Discharge Planning Overview, Process, and Checklist; (b) the teach-back Method training for discharge nurses; and (c) the Postdischarge Rehabilitation Services Follow-Up Tool incorporating telephone calls to all participants during Weeks 1, 2, and 4 postdischarge. Lewin's theory of planned change undergirded this project. According to Centers for Medicare and Medicaid Services data, the rate of readmissions among the 50 participants was 4.4%, compared with 6% (all-facility readmission rate) during the same quarter of the prior year. Findings from this project suggest that reductions in readmissions were associated with improvements in discharge planning, training of caregivers, and the use of national tools to standardize practices in reducing readmissions. The implication of this project for positive social change is that patient-centered inpatient rehabilitation care and patient-centered care following discharge may reduce readmissions, reduce costs, improve reimbursement, and reduce deterioration of patients' conditions postdischarge.

The performance management process at Wica Cold AB is not very well developed and they would know like to know how it can be developed in order to improve performance in the order-to-delivery process. Performance in this thesis covers the aspects of efficiency, quality, deliveries, time, flexibility and improvements. The PM-process is in this thesis seen as measures used, collection and analyzing of data and make use and take action based on the measures. How to make use of the measurement information, what should be measured and that lack of resources is an important aspect for the PM-process in SMEs like Wica are the main problems identified. Objectives: The objectives are to describe the PM-process at Wica Cold and the corresponding performance in the OTD-process. Furthermore the objectives are to find ways to develop the PM-process in order to improve performance in the OTD-process, also taking into consideration the characteristics of a SME regarding the PM-process. Methodology: This thesis is a case study and it is both descriptive and explanatory. The main scientific perspective in this thesis is the positivistic and it has a deductive approach. Data is mostly collected through semi-structured interviews. The scientific credibility is secured by interviewing employees at different positions, consider many views (theory, the benchmark study at Getinge Disinfection and the variety of data collection at Wica) and by gather all relevant information for the thesis on my hard drive. Results, conclusions: The current PM-process only has a few measures and it is not communicated to the employees very well. The performance in the order-to-delivery process is satisfying but can in many ways be improved. The PM-process can develop with some new measures and some small changes and additions in the current measures. One main aspect in this regard is to measure the amount of guarantee claims and percentage of orders delivered on time instead of only costs for backorders and guarantee claims. The measures should be published on a board on the shop floor as a way to make use and take action based on the measures. Meetings every other week should be in place to discuss ideas from employees and the value-added time. This improves performance since it highlights important aspects and common problems and a way to improve that performance can be found. The characteristics of a SME are taken into consideration for instance by limiting the amount of measures used and by making them simple and easy to collect.

Implement BPR and CPI to optimize the process of getting medicine in pharmacy : a comparison between Sweden and China

<p>This report generally focuses on the process of getting medicine from the pharmacy in the hospital. The processes are different in Sweden and in China due to the different concrete conditions. Compared with the Swedish process the Chinese flow lags fairly far behind. To optimize the Chinese process by taking advantage of the Swedish process is the intention of this report; it also gives the advice on how to improve the performance for the Swedish process. By comparing the multiple theories for process improvement, I used Business Process Reengineering (BPR) theory to reengineer the Chinese process and used Continues Process Improvement (CPI) theory to ameliorate the Swedish process. The differences between BPR and CPI are also revealed in this report.</p>

Syftet: Var att beskriva äldres upplevelser av åldersrelaterad yrsel fysiskt, psykiskt och socialt samt att belysa vårdåtgärder som kan bidra till att förbättra dagligt liv. Metod: Examensarbetet genomfördes som en litteraturstudie med artiklar hämtade från databaser inom vårdvetenskap/medicinsk vetenskap. Resultat: Yrsel kan upplevas som en svår funktionsnedsättning att leva med. En samsjuklighet till somatiska sjukdomar och korrelation till psykisk belastning är vanligt. Påverkan på sociala faktorer medför en begränsning i att klara sitt dagliga liv och sitt förhållande till omgivningen. Många saknade bekräftelse från hälso- och sjukvården. Vestibulär rehabilitering förbättrade symtom, balans och yrselrelaterade handikapp. Likaså har kognitiv beteendeterapi i kombination med vestibulär rehabilitering visat sig ha positiv effekt. Det fanns ett behov av individuellt utvecklade åtgärder och stöd med ett tvärprofessionellt perspektiv. Slutsats: Yrsel är ett svårt handikapp att leva med och ger konsekvenser såväl fysiskt, psykiskt samt socialt. Det är viktigt med en utvecklad och strukturerad sjuksköterskeroll där den äldre får stöd och rådgivning med möjlighet att diskutera strategier för att lära sig hantera yrseln. Rehabilitering samt undervisning om hur balanssystemet fungerar har visat sig vara värdefullt samt är dessutom kostnadseffektivt. Vidare fanns ett behov av äldres perspektiv i utvecklandet av vårdåtgärder i tvärprofessionell samverkan och det är viktigt att inte underskatta äldres upplevelser av yrsel.
