Karlsson, Clara, Spännargård, Mira January 2019 (has links)

CHANGES IN LONGITUDINALLY ASSESSED BIOMECHANICAL PARAMETERS RELATED TO INCREASED RISK OF ANTERIOR CRUCIATE LIGAMENT (ACL) INJURIES IN ADOLESCENT FEMALE AND MALE ATHLETES

Ford, Kevin Ray 01 January 2009 (has links)
Females suffer anterior cruciate ligament (ACL) injuries at a 2 to 10-fold greater rate compared to male athletes participating in similar sports. Altered movement patterns and inadequate knee stiffness are two interrelated factors that may increase ACL injury risk. Onset of these neuromuscular risk factors may coincide with the rapid adolescent growth that results in the divergence of a multitude of neuromuscular parameters between sexes. The overall purpose of this dissertation was to determine if neuromuscular ACL injury risk factors in female athletes increase following rapid growth and development compared to males. Male and female athletes were tested with three-dimensional motion analysis techniques during a drop vertical jump over two consecutive years to determine if ACL injury risk factors increased. Pubertal females showed a significant longitudinal increase in knee abduction angle compared to post-pubertal females and both male groups. The increase in knee abduction angle appeared to remain consistent, as the post pubertal female cohort had greater overall knee abduction compared to post-pubertal males. Similar results were found with a greater magnitude of knee abduction moment in post-pubertal females compared to males. Males and females increased ankle, knee and hip active stiffness from the first to second year of testing. Ankle and hip stiffness were increased significantly more in the pubertal group compared to post-pubertal. Sex and maturational group differences were found in hip and ankle joint stiffness. Post-pubertal males had significantly greater hip stiffness than the other groups (even when normalized to body mass). This indicates that post-pubertal males utilized a different neuromuscular strategy during landing. Males had a significantly greater increase from year to year in vertical jump height compared to females. Vertical jump height is often related to a measure of whole body power and indicates that males had a significant neuromuscular spurt compared to females. Early puberty appears to be a critical phase related to the divergence of increased ACL injury risk factors. Injury prevention programs that focus on neuromuscular training may be beneficial to help address the development of ACL injury risk factors that occur in female athletes during maturation.

Protocolo de captação, processamento e transplante de enxertos osteocondrais homólogos a fresco na articulação do joelho / Protocol of procurement, processing and transplantation of fresh osteochondral allografts in the knee joint

Tirico, Luís Eduardo Passarelli 07 April 2015 (has links)
The techniques available nowadays in Brazil, as debridement, microfracture and autologous osteochondral transplantation are insufficient for the treatment of chondral and osteochondral lesions larger than 4 cm2. Fresh osteochondral allografts on the knee joint have been used in other countries with great success. Until the present days there are no reports of the utilization of this technique in Brazil, a fact that creates boundaries for the treatment of young active patients that need cartilage repair treatment. Eight patients with age ranging from 15 to 45 years old and osteochondral lesions larger than 4 cm2 were included in this study. Fresh grafts were obtained from organ donors and the knee joint was harvest without violation of synovium capsule and then was transported to the tissue bank for processing. Donor and receptor tissue matching occurred according to size, localization and priority on waiting list and tissue was preserved in Ham F-12 - GIBCO media with glutamax (Invitrogen, Life Technologies, United States) and antibiotics. Surgical procedure was achieved with osteochondral plugs or shell techniques, with additional fixation when needed. Patients were evaluated with IKDC objective, IKDC subjective, KOOS and modified Merle D\'Aubigne-Postel outcome scores. Eight fresh osteochondral allografts were performed from march to october, 2012. The average age was 30.1 years old (17-44) and mean graft surface area was 10.6 cm2 (4.6 - 22.4 cm2). Average time from harvest to transplantation was 15.3 days (14-16 days) and mean number of previous surgical procedures was 2 (0 - 4 procedures). Pre-operative objective IKDC score was B in 12%, C in 25% and D in 63% and was A in 12% and B in 88% in 18 months follow-up. Pre-operative IKDC subjective and KOOS score were 31.99 ± 13.4 and 46.8 ± 20.9; and in 18 months follow-up were 81.84 ± 9.25 and 89.58 ± 7.06, respectively, with a significant increase along time on variance analysis (p < 0.01). Pre-operative mean modified Merle D\'Aubigne-Postel scores were 8.75 ± 2.25 and 16.75 ± 2.19 in 18 months follow-up. Non-parametric Friedman test analysis showed a significant progression in follow-up (p < 0.01). Standards of procurement, processing and utilization of fresh osteochondral allografts in the knee joint relay on a high specialized tissue bank and promote excellent results on the treatment of large osteochondral lesions, being a good alternative of a biologic repair for young patients in Brazil

As técnicas “inside-out” e “all-inside” foram predominantemente relatadas. / PURPOSE: To perform a systematic review of existing literature in order to analyze outcomes after meniscal repair in the pediatric population. The Multi-Ligament Quality of Life Questionnaire (ML-QOL): Development and Preliminary Testing of Measurement Properties in Patients with Multi-Ligament Knee Injuries

Chahal, Jaskarndip 21 November 2013 (has links)
Despite the existence of numerous knee-joint specific patient-reported outcome measures, the content contained within such instruments does not completely capture the spectrum of injury and impairments experienced by multi-ligament knee injured patients. Based on this shortcoming, the overall objective of the current study was to develop, and to test the reliability and validity of a novel disease-specific quality of life instrument for patients with multi-ligament knee injuries. The World Health Organization’s International Classification of Functioning, Disability, and Health was used to guide content development. Using a mixed methods approach (surveys, patient focus groups, expert interviews), the final Multi-ligament Quality of Life (MLQOL) instrument was comprised of 52 items distributed over 4 domains. This instrument demonstrated excellent content validity, test-retest reliability, and construct validity. Future work will evaluate the responsiveness of the MLQOL and will implement its use in randomized trials.

Tidigt insatt hög-intensiv vadmuskelträning efter knäplastikoperation : Effekt på vadmuskelstyrka, självvald gånghastighet och självskattad symptom, smärta, ADL funktion samt livskvalité / High-intensive calf muscle training in the early post-operative phase following knee arthroplasty : Effect on calf muscle strength, self-selected walking speed and self-rated symptom, pain, ADL function as well as quality of life

Svantesson, Charlotte January 2014 (has links)
Aim The aim of this study was to determine the feasibility of a high-intensive exercise intervention following a knee arthroplasty, and also to evaluate possible effects on self-selected walking speed, calf muscle strength and self-rated symptoms, pain, ADL function and quality of life after high-intensive calf muscle training in an early post-operative phase. Method Fifteen participants were randomized into an exercise group (n=7), who performed a home-exercise program, consisting of high-intensive calf muscle exercise, and into a control group (n=8) who performed the same rehabilitation program but without the high-intensive calf muscle training. A test procedure was conducted after 3 and 12 weeks following the knee arthroplasty. Self-selected walking speed was measured by a 30 meter walking test, calf muscle endurance training was measured with a standardized one-leg heel-rise test, maximal isometric calf muscle strength was measured with an isometric dynamometer, and self-rated symptom, pain, ADL function and quality of life were evaluated with Knee injury Osteoarthritis Outcome Score (KOOS). Results There were problems with adherence to exercise regimes in both groups. The results of both groups demonstrated improved self-selected walking speed, calf muscle endurance and maximal isometric calf muscle strength over time. However, the exercise group increased self-selected walking speed more than the control group. The exercise group reported fewer disorders due to pain 12 weeks after surgery. The results of both groups showed improved self-rated symptoms, pain, ADL function and quality of life over time, however, the exercise group improved all these variables significantly, while the control group significantly improved the quality of life aspect. Conclusions Early initiated high-intensive calf-muscle training following a knee artroplasthy is feasible and may have positive effects on self-rated symptoms, pain, ADL function and quality of life. Also, self-selected walking speed increased more with early initiated high-intensive calf muscle training after nine weeks when compared to the same rehabilitation program without high-intensive calf muscle training.

A padronização da captação, processamento e transplante de enxertos osteocondrais a fresco na articulação do joelho depende de um banco de tecidos altamente especializado e leva a excelentes resultados no tratamento de lesões osteocondrais grandes, sendo atualmente uma boa alternativa de tratamento biológico para pacientes jovens no Brasil / Treatment of chondral and osteochondral lesions of the knee in young patients is still a challenge for Orthopedic Surgeons.
