Contexte : L’amiante est maintenant une cause avérée de cancer du larynx, mais le rôle des expositions professionnelles dans la survenue des cancers des voies aéro-digestives supérieures (VADS) reste largement méconnu. Bien que plusieurs études aient rapporté des associations entre les expositions professionnelles et ces cancers, il est difficile de synthétiser les résultats et d’en tirer des conclusions définitives. Objectifs : (1) Le premier objectif est de synthétiser les données épidémiologiques disponibles sur les associations entre les cancers de la cavité buccale, du pharynx et du larynx (les cancers des VADS les plus fréquents) et les expositions professionnelles ; (2) le second objectif est, à partir des données d’une large étude cas-témoins, l'étude Icare, d'identifier les professions ou industries présentant des risques élevés de cancer des VADS, puis d'étudier le rôle de certaines expositions professionnelles suspectées (amiante, laines minérales (LM), poussières de ciment, silice). Matériel et méthodes : (1) Une recherche bibliographique et des séries de méta-analyses ont été réalisées pour certaines expositions professionnelles suspectées. (2) L'étude Icare est une étude cas-témoins en population générale française incluant 2415 cas de cancer des VADS et 3555 témoins. L'historique professionnel complet des sujets a été recueilli, avec une description détaillée de chaque emploi exercé. L'évaluation des expositions à l'amiante, aux LM, aux poussières de ciment et à la silice sur l’ensemble de la vie professionnelle a été réalisée à l'aide des matrices emplois-expositions développées dans le cadre du programme Matgéné (Institut de Veille Sanitaire). Résultats : Des méta-risques relatifs (méta-RR) significativement augmentés ont été observés pour le cancer du larynx et les expositions aux hydrocarbures aromatiques polycycliques (HAP), aux gaz d'échappement de moteurs, au travail dans l'industrie textile et dans l'industrie du caoutchouc, et pour le cancer de la cavité buccale et du pharynx et les expositions à l'amiante, aux HAP et aux gaz d'échappement de moteurs. Des augmentations de risque significatives ont été observées pour plusieurs professions et secteurs d'activité exposant notamment aux nuisances présentant des méta-RR augmentés. Les résultats des analyses par nuisance confirment l'association entre l'amiante et le cancer du larynx et suggèrent également une association avec les cancers de la cavité buccale et du pharynx. Une association avec les poussières de ciment est également suggérée. Les résultats ne sont pas en faveur d’un rôle de l’exposition aux LM et à la silice.Conclusion : Ce travail renforce l’hypothèse d’un rôle des expositions professionnelles dans la survenue des cancers des VADS. Dans l’ensemble, nos résultats suggèrent des associations entre le risque de cancer des VADS et les expositions à l'amiante, aux HAP, aux poussières de ciment, et le travail dans l'industrie du caoutchouc. / Background: There is sufficient evidence that asbestos causes laryngeal cancer, but overall the role of occupational exposures in the etiology of head and neck cancer (HNC) remains largely unknown. Although several studies have reported associations between occupational exposures and HNC, it is difficult to draw firm conclusions. Objectives: (1) To summarize available epidemiologic data on occupational exposures and cancers of the oral cavity (OC), pharynx and larynx (the most frequent HNC); (2) using data from a large case-control study, to identify occupations and industries with an increased risk of HNC, then to investigate the role of some suspected occupational exposures (asbestos, mineral wools (MW), cement dust, silica). Methods: (1) A literature research and a series of meta-analyses were performed. (2) The Icare study is a French population-based case-control study including 2415 HNC cases and 3555 controls. Complete and detailed occupational histories were collected. Analyses by job title were conducted. Job exposure matrices, developed by the Occupational Health Department of the French Institute for Public Health Surveillance (InVS), were used to assess lifetime occupational exposure to asbestos, MW, cement dust and silica. Results: Significantly increased meta-relative risks (meta-RR) were obtained considering laryngeal cancer and exposures to polycyclic aromatic hydrocarbons (PAH), engine exhausts, working in the textile industry and the rubber industry, and for cancer of the OC and pharynx and exposures to asbestos, PAH and engine exhausts. Significantly increased risks were found for several jobs and industries, some of them entailing exposures to agents for which meta-RR were increased. Analyses for specific occupational exposures confirmed the association between asbestos and laryngeal cancer and showed an association with the risk of oral cavity and pharyngeal cancer. An association with exposure to cement dust was also suggested. The results did not support an association between HNC risk and exposure to MW or silica. Conclusion: This work emphasizes the role of occupational exposures in HNC. Overall, our results suggest associations between HNC and exposure to asbestos, PAH, cement dust, and work in the rubber industry.

Le larynx artificiel : de l'in vitro à la première implantation clinique / Laryngeal replacement after total laryngectomy

Objectifs : Ce travail a pour but de développer un larynx artificiel composé de 2 structures : 1) une structure inamovible en titane poreux prolongeant la trachée; 2) une double valve remplissant la fonction de sphincter. Matériels et Méthodes : L’intégration de la prothèse en remplacement trachéal a été testée in vitro et in vivo sur les modèles animaux rats, lapins et brebis. Suite à l’ensemble de ces résultats, nous avons réalisé la première application clinique du larynx artificiel. Résultats : 1) L’ajout d’un tube de silicone endoprothétique améliore la survie chez le gros animal; 2) l’intégration tissulaire de la prothèse de trachée associée à un matériel polymérique biodégradable était supérieure à celle des prothèses en titane poreux nu; 3) la vitesse de colonisation des prothèses en titane poreux était accélérée lorsque l’on diminuait la taille des billes. Un premier patient a été implanté avec une prothèse de larynx artificiel avec des résultats satisfaisants à 9 mois. Conclusions : Nos études concernant l’intégration de prothèses de trachées in vitro et in vivo ont permis de contribuer à l’aboutissement de la première application clinique de larynx artificiel. / Background: The aim of this work is the design of an artificial larynx made of 2 elements: 1) a non-removable bio-integrable structure designed to provide a connection with the remaining trachea; 2) a double valve that fulfills the sphincter function. Materials and Methods: In vitro and in vivo tests (rat, rabbit and sheep model) were performed to achieve the tracheal prosthesis integration. Following all these results, we carried out the first clinical application of an artificial larynx. Results: i) The long-term survival of the animals was improved when an endoprosthetic silicon calibration tube was used; ii) tissue integration of the prostheses in porous titanium filled with biodegradable polymer was better than the bare porous titanium; iii) prosthesis integration was improved when pore size was decreased. A first patient was implanted with an artificial functional larynx with satisfactory results after 9 months of implantation. Conclusion: The whole in vivo and in vitro studies concerning the integration of the tracheal prosthesis has contributed to the success of the first clinical application of the artificial larynx.

Lower Vocal Tract Morphologic Adjustments Are Relevant for Voice Timbre in Singing

The vocal tract shape is crucial to voice production. Its lower part seems particularly relevant for voice timbre. This study analyzes the detailed morphology of parts of the epilaryngeal tube and the hypopharynx for the sustained German vowels /a/, /e/, /i/, /o/, and /u/ by thirteen male singer subjects who were at the beginning of their academic singing studies. Analysis was based on two different phonatory conditions: a natural, speech-like phonation and a singing phonation, like in classical singing. 3D models of the vocal tract were derived from magnetic resonance imaging and compared with long-term average spectrum analysis of audio recordings from the same subjects. Comparison of singing to the speech-like phonation, which served as reference, showed significant adjustments of the lower vocal tract: an average lowering of the larynx by 8 mm and an increase of the hypopharyngeal cross-sectional area (+ 21.9%) and volume (+ 16.8%). Changes in the analyzed epilaryngeal portion of the vocal tract were not significant. Consequently, lower larynx-to-hypopharynx area and volume ratios were found in singing compared to the speech-like phonation. All evaluated measures of the lower vocal tract varied significantly with vowel quality. Acoustically, an increase of high frequency energy in singing correlated with a wider hypopharyngeal area. The findings offer an explanation how classical male singers might succeed in producing a voice timbre with increased high frequency energy, creating a singer‘s formant cluster.

Exposition professionnelle aux solvants et risque de cancer des voies aéro-digestives supérieures / Occupational Exposure to Solvants and Risk of Head and Neck Cancer

Contexte : Le rôle de l’exposition professionnelle aux solvants dans la survenue de cancers des voies aéro-digestives supérieures (VADS), suggéré dans quelques études, n’a été que peu examiné, malgré leur utilisation très répandue en milieu de travail. Objectif : L’objectif de cette thèse état d’évaluer les associations entre les expositions professionnelles aux solvants et le risque de cancer des VADS. Méthodes : Ce travail est basé sur les données de l’étude Icare, une large étude cas-témoins en population générale conduite en France entre 2001-2007. L’analyse a été restreinte aux hommes et a porté sur 1857 cas de carcinome épidermoïde de la cavité buccale, du pharynx et du larynx, et 2780 témoins. L’histoire professionnelle détaillée ainsi que les consommations de tabac et d’alcool ont été recueillies par questionnaire. Les expositions aux solvants ont été évaluées à l’aide de matrices emplois-expositions et incluaient cinq solvants chlorés (perchloroéthylène, trichloroéthylène, chlorure de méthylène, chloroforme, tétrachlorure de carbone), cinq solvants pétroliers (benzène ; essences carburant ; gazole, fiouls et kérosène ; essences spéciales ; white spirits) et cinq solvants oxygénés (cétones et esters ; alcools ; éther éthylique ; éthylène glycol ; tétrahydrofurane). Les odds-ratios ajustés sur les consommations de tabac et d’alcool et d’autres facteurs de confusion potentiels, et les intervalles de confiances à 95% ont été estimés par régression logistique. Résultats : Aucune association significative n’a été mise en évidence entre exposition professionnelle aux solvants chlorés, pétroliers et oxygénés étudiés et le risque de cancer de l’ensemble des VADS. Dans l’analyse par localisation, le risque de cancer du larynx augmentait significativement avec l’exposition cumulée au perchloroéthylène. Des risques élevés de cancer de l’hypopharynx, bien que non significatifs, étaient observés pour les hommes exposés à des niveaux élevés de chlorure de méthylène, de white spirits et de tétrahydrofurane. Une association entre exposition au tétrahydrofurane et cancer de la cavité buccale était également suggérée. Aucune association claire n’était observée pour les autres solvants, quelle que soit la localisation de cancer. Conclusion : Des associations positives ont été observées avec plusieurs solvants spécifiques. Toutefois, dans l’ensemble, nos résultats ne sont pas en faveur d’un rôle majeur de l’exposition aux solvants dans la survenue de cancer des VADS. / Background : The role of occupational exposure to solvents in the risk of head and neck cancer has been suggested in some studies but has been few investigated, despite their widespread use in the workplace. Objective: The objective of this thesis was to examine the associations between occupational exposure to solvents and the risk of head and neck cancer. Methods: This work is based on data from the ICARE study, a large population-based case-control study conducted in France between 2001 and 2007. The analysis was restricted to men and included 1,857 cases of squamous cell carcinoma of the oral cavity, pharynx and larynx and 2,780 controls. Detailed occupational lifetime as well as alcohol and tobacco consumptions were collected by questionnaires. Exposure to solvents was assessed by job exposure matrices and included five chlorinated solvents (perchloroethylene, trichloroethylene, methylene chloride, chloroform, carbon tetrachloride), five petroleum-based solvents (benzene; gasoline; diesel, fuels and kerosene; special petroleum products; white spirits) and five oxygenated solvents (ketones and esters; alcohols; diethyl ether; ethylene glycol; tetrahydrofuran). Odds-ratios adjusted for smoking, alcohol drinking and other potential confounders and 95% confidence intervals were estimated with logistic models. Results: No significant association was found between occupational exposure to chlorinated, petroleum-based and oxygenated solvents and the risk of head and neck cancer overall. In subsite analysis, the risk of laryngeal cancer increased with cumulative exposure to perchloroethylene. Non-significantly elevated risks of hypopharyngeal cancer were found in men exposed to high cumulative levels of methylene chloride, white spirits and tetrahydrofuran. An association between exposure to tetrahydrofuran and oral cavity cancer was also suggested. No other clear association was found for the other solvents under study, for any cancer site. Conclusion: Although positive associations were observed for several solvents, overall the results do not suggest a substantial role of exposure to solvents in head and neck cancer risk.

Nucleotide sequence variation and expression levels of TP53 in cancers of the upper gastro-intestinal tract

Thesis (MSc)--Stellenbosch University, 2004. / ENGLISH ABSTRACT: The work presented in this thesis deals with the association between cancers of the upper gastro-intestinal tract and the tumor suppressor gene, TP53, and can be divided into three parts: (i) the analysis of the mutational spectrum of TP53 with respect to laryngeal cancer, (ii) the analysis of the mutational spectrum of TP53 with respect to esophageal cancer and (iii) the analysis of TP53 transcriptional levels in esophageal cancer. Laryngeal cancer (LC) is the 6th most common cancer in the world and the 2nd most common respiratory cancer, with approximately 500 000 new cases per annum detected worldwide. Over the last few years, LC has become increasingly prevalent within the Coloured Community of the Western Cape. The mechanisms of tumorigenesis in LC remain unknown, although smoking and alcohol consumption are considered to be major risk factors. Mutations within the gene TP53 have been strongly implicated as playing a role in cancer development, as they are frequently found in several cancer types. We therefore screened exons 5 - 8 of TP53 for mutations in DNA from tumor biopsies (n=44) and blood samples (n=42) from Coloured LC patients, using polymerase chain reaction - single strand conformation polymorphism (PCR-SSCP) analysis and direct sequencing. Blood samples from a healthy, matched control group (n=40) were included in the study as controls. Significant correlations were found between the occurrence of LC and age and smoking, whereas daily meat consumption was a possible protective factor. In tumor-derived samples, mutations were found in 3 of the exons under investigation, representing 25% of the samples. The mutations were unique to the tumor biopsies, indicating a somatic origin for mutations. The data confirms that the region between codons 175 and 273 of TP53 is a mutational hotspot for cancers in general. This study reports 6 novel mutations within this same region. Esophageal cancer (EC) has a very high incidence in South Africa, relative to the rest of the world, and is particularly common amongst the Black Transkei population. The goal of this study was to determine whether there are differences in the TP53 mutational pattern observed in the Coloured Western Cape community as compared to that observed in the Black Transkei community. This required the analysis of the molecular structure of TP53, specifically exons 5 - 8, in a group of Coloured EC patients (n=44) treated at Tygerberg Hospital, Cape Town, South Africa. DNA obtained from tumor biopsies and blood (from patients) as well as from apparently healthy surrounding tissue was screened via PCR-SSCP and direct sequencing analysis. Only 4 nucleotide changes were observed from a total of 124 sequences obtained, of which two were novel to esophageal squamous cell carcinoma. These 4 nucleotide alterations were found only within the tumor biopsy sample set, representing 9% of the tumors investigated. This study revealed that the mutational spectrum of TP53 within the Coloured population of the Western Cape greatly differs from that of the Black community of the Transkei. This suggests that a different set of etiological factors are involved in the tumorigenic process for each of these distinct geographical communities, which is the subject of an epidemiological study undertaken by the MRC. The final part of this thesis deals with the quantification and comparison of TP53 transcription levels in esophageal cancer tumor tissue to the TP53 levels in healthy esophageal tissue obtained from patients from a unique geographical and ethnic background. The cohort used in this study consisted of Coloured patients (n=2) treated at Tygerberg Hospital. The LightCycler system was implemented in order to try to accurately quantify TP53 mRNA levels. Unfortunately, the desired results were unattainable due to unforeseen difficulties encountered during the study. These difficulties included the insufficient preservation of samples for RNA based studies. Several recommendations were made concerning future similar studies, including an improved planning strategy as well as the employment of an RNA stabilizing agent. Additionally, a few important contributions were made through this study, including the design and optimization of TP53 primers specifically intended for future RNA studies. These primers would enable the identification of the presence of TP53 RNA species as well as the absence of DNA contamination in a single PCR amplification step. Other contributions include the development of a well-optimized RNA extraction method for the extraction of RNA from tough tissues (such as the human esophageal tissue used in this study). This method makes the extraction of large quantities of RNA from small amounts of tough tissue types possible. In conclusion, this study has made a significant contribution to the field of cancer research, by shedding light on the TP53 mutational spectrum with regards to laryngeal as well as esophageal cancer in a population unique to the Western Cape. The study's purpose was to describe selected laryngeal activity of brass-wind players during the performance of selected musical exercises. Research problems included the observation and description of three internal areas of activity of ten trumpeters as they performed each exercise. Specific areas of observation were 1) movement of the epiglottis during the performance of each exercise, 2) movement of the vocal folds/arytenoid cartilage which includes changes in the size of the glottis during the performance of each prescribed exercise, and 3) movement of the thyroid cartilage during the performance of each prescribed exercise. Musical exercises performed by each of the subjects included a sound volume change, use of vibrato, single-tonguing, step-wise descending and ascending slurs, descending and ascending lip slurs, register change, and a descending chromatic scale. In addition, each subject performed an excerpt from the second movement of the Haydn Trumpet Concerto. Data were collected through direct observation of subject performances and then described using three different means. Data analyses revealed a prominent amount of highly individual, non-patterned laryngeal activity which played an integral role in the performance of each subject. Individuals including Law (1960), Cramer (1955), Jacobs (Stewart, 1987), and Noble (1964) have advocated an unrestricted airway during brass performance. Contrary to this advice, findings in the present study indicate that a great deal of varying, involuntary restriction is present in the laryngeal area during performance. Further, such activity appears necessary to brass performance. Others, including Farkas (1962), Schuller (1962), and Wick (1971) , have endorsed conscious use of the glottis during brass performance. While findings in the present study imply that there is a presence of voluntary or reflexive glottal activity during brass performance, evidence does not support any theory which suggests conscious use of the laryngeal mechanism.

A caracterização das patologias da laringe até o presente momento é predominantemente realizada de forma qualitativa. Alguns trabalhos recentes têm quantificados parâmetros acústicos no intuito de discriminar as diversas patologias. Este trabalho verifica os valores médios das diversas medidas acústicas da voz e as relaciona com as diferentes patologias. Desta forma, futuramente será possível utilizar estes valores para quantificar e acompanhar diferentes estágios do tratamento da patologia. O conjunto de valores médios compõem o que chamamos de perfil acústico. / Until now the characterization of larynx pathologies has been done in a subjective manner. Recently some researches have been conducted in order to quantify acoustic parameters mainly to discriminate the larynx pathologies. In this work several acoustic parameters of voice was evaluated and their correlation with the pathologies were established. In the future, would be possible to use these parameters to quantify and to follow the therapy procedures. These values constitute the acoustic profile of the pathologies.

"Riscos ocupacionais para o câncer de laringe: um estudo caso-controle" / Occupational risk for laryngeal cancer

No município de São Paulo, em 1970 foram relatados 169 casos novos de câncer de laringe, com uma taxa de incidência ajustada por idade pela população mundial de 25,5/100.000 (30-74 anos) e 299 casos em 1985, elevando a taxa para 37,6 (ambos nosexo masculino). O Tabagismo e o consumo de álcool são os fatores de risco mais bem estabelecidos para o câncer de laringe. Com relação aos fatores ocupacionais, o único carcinógeno estabelecido é a exposição à névoa de ácidos inorgânicosfortes. É proposto um estudo caso-controle de base hospitalar, para investigar e quantificar o papel da exposição ocupacional no desenvolvimento do câncer de laringe e é discutida uma nova proposta de avaliação de exposição que visa melhorar avalidade interna desse tipo de estudo / Laryngeal cancer comprises 3,1% of the new cases of cancer in men in the world, representing the tenth most common malign neoplasm for males. In São Paulo City, 169 new cases of laryngeal cancer were reported in 1970, representing 25,5/100.000 and 299 cases in 1985, increasing the rate to 37,6/100.000 (both for males, age standardized truncated rate, 30-74 years). The best established risk factors for laryngeal cancer are tobacco and alcohol. In respect to occupational factors, the only established carcinogen is exposure to strong inorganic acid mists. However, asbestos, pesticides, paints, gasoline and diesel engine emissions, dusts, among other factors are reported in the literature as occupational agents that increase the risk of laryngeal cancer. A hospital-based case-control study was conducted, in seven hospitals in São Paulo, to investigate occupational risk factors for laryngeal cancer. The study included 122 laryngeal cancers and 187 controls, selected by frequency matching on sex and age. Detailed information on smoking, alcohol consumption, and occupational history was collected. Occupational hygienists assessed the exposure to 49 agents. Odds Ratios (OR) and 95% confidence intervals (95% CI) were estimated by unconditional logistic regression, and were adjusted for sex, age, smoking and alcohol. Laryngeal cancer was associated with exposure to respirable free crystalline silica (OR 1,83, 95% CI 1,00 - 3,36), soot (from coal, coke, fuel oil, wood) (OR 1,78, 95% CI 1,03 - 3,03), fumes (OR 2,55, CI 95% 1,14 - 5,67) and to live animals (OR 1,80, 95% CI 1,02 - 3,19). This study showed that occupational exposures to: respirable free crystalline silica, soot (from coal, coke, fuel oil, wood), fumes and live animals are risk factors for laryngeal cancer.

Estudo histológico dos efeitos da lesão com laser diodo 980 nm e 1470 nm em pregas vocais de coelhos / Histological study on the effect of diode laser 980 nm and 1470 nm in vocal folds in a rabbit model

INTRODUÇÃO: Os lasers têm sido utilizados em microcirurgia de laringe, permitindo corte e coagulação simultâneos que, em alguns casos, pode favorecer uma cirurgia exangue e mais precisa. Do ponto de vista vocal, o resultado cirúrgico ideal é aquele em que a lesão é removida com menor dano possível ao tecido adjacente, já que uma cicatrização inadequada da prega vocal pode resultar em disfonia de difícil reparação. O dano colateral dos lasers nos tecidos depende, entre outros fatores, do tipo de laser e de seu comprimento de onda. O laser diodo, quando comparado aos lasers mais utilizados em microcirurgias de laringe, tem como vantagens ser portátil, silencioso, transmite radiação através de uma fibra óptica fina e flexível o que facilita a manipulação do instrumento pelo cirurgião. Entretanto, as reações cicatriciais causadas pelo uso dos diferentes comprimentos de onda do laser diodo (980 nm e 1470 nm) nas pregas vocais ainda não foram estudadas. OBJETIVO: O objetivo desta pesquisa é estudar os efeitos histológicos da variação do comprimento de onda do laser de diodo de 980 nm e 1470 nm, no processo de cicatrização de pregas vocais de coelhos, em 7 e 30 dias após a lesão. MÉTODO: Vinte coelhos machos da raça New Zealand foram distribuídos aleatoriamente em quatro grupos, com cinco animais em cada grupo. Em cada prega vocal foi realizada uma única lesão por 7 segundos, através do contato da ponta da fibra do laser com a superfície do tecido. De um lado, o laser foi ajustado ao comprimento de onda de 980 nm e na prega vocal contralateral 1470nm. Nos grupos I e III, o modo de entrega de energia foi pulsado, enquanto que, nos grupos II e IV, foi contínuo. Após 30 dias (grupos I e II) e 7 dias (grupos III e IV) as laringes foram excisadas e submetidas a coloração com hematoxilina-eosina, além de coloração para colágeno e elastina. A análise histológica realizada foi quantitativa e subjetiva. RESULTADOS: Com o uso do laser diodo foi observado que o tecido apresentava sinais de manipulação (presença de reepitelização e presença de tecido de granulação com fibroblastos). Não foram observadas formações de úlceras, bolhas ou necrose. A extensão linear da lesão fibrótica apresentou diferença significativa, entre os dois comprimentos de onda, sendo maior no laser 980nm, que receberam feixes de laser no modo pulsado, em coelhos sacrificados 7 dias após a lesão. A densidade das fibras colágenas apresentou-se elevada ao uso do laser 1470 nm no mesmo grupo de animais. CONCLUSÃO: O uso de laser diodo demonstrou maior extensão da lesão fibrótica (laser 980 nm, modo pulsado) e maior densidade de colágeno (laser 1470 nm, modo pulsado), em animais na fase inicial (7 dias) da cicatrização. No entanto, ao analisarmos a evolução da cicatrização, tal padrão não fora observado ao longo dos 30 dias. Desta forma, a variação do comprimento de onda do laser diodo, não causou diferenças histológicas significativas na cicatrização inicial / INTRODUCTION: Lasers have been used in endolaryngeal microsurgery, allowing simultaneous cutting and coagulation which, in some cases, may favor a bloodless and more accurate surgery. From a vocal point of view, the ideal surgical outcome is one in which the lesion is removed with the lowest possible damage to the adjacent tissue, since inadequate healing of the vocal fold may result in dysphonia that may be difficult to repair. The collateral damage of the lasers in the tissues depends, among other factors, on the type of laser and its wavelength. The diode laser, when compared to the lasers most used in laryngeal microsurgery, has the advantages of being portable, silent and to the fact it transmits radiation through a thin and flexible optical fiber, which facilitates the manipulation of the instrument by the surgeon. However, the cicatricial reactions caused by the use of different diode laser wavelengths (980 nm and 1470 nm) in the vocal folds have not been studied. OBJECTIVE: The aim of this study is to study the histological effects of diode laser wavelength variation of 980nm and 1470 nm in the healing process of vocal folds of rabbits, 7 and 30 days after the injury. METHOD: Twenty male New Zealand white rabbits were randomized into four groups, with five animals in each group. In each vocal fold, a single spot injury was performed during 7 seconds, through the contact of the tip of the laser fiber with the surface of the tissue. On one side, the laser was adjusted to the wavelength of 980 nm and on the contralateral vocal fold, 1470 nm. In groups I and III, the mode of energy delivery was pulsed, whereas in groups II and IV, it was continuous. After 30 days (groups I and II) and 7 days (groups III and IV) the larynx were excised and stained with hematoxylineosin, in addition to staining for collagen and elastin. The histological analysis was quantitative and subjective. RESULTS: With the use of diode laser, the tissue showed signs of manipulation (presence of reepithelialization and presence of granulation tissue with fibroblasts). No ulcer, blister or necrosis formations were observed. The linear extension of the fibrotic lesion showed a significant difference between the two wavelengths, being higher in the 980 nm laser, which received laser beams in the pulsed mode, in rabbits sacrificed 7 days after the lesion. The density of the collagen fibers was elevated to the use of the 1470 nm laser in the same group of animals. CONCLUSION: The use of diode laser showed a greater extension of the fibrotic lesion (980 nm laser, pulsed mode) and a higher density of collagen (laser 1470 nm, pulsed mode) in animals in the initial phase (7 days) of wound healing. However, when we analyzed the evolution of healing, such pattern had not been observed during the 30 days. Thus, the variation of wavelength of the diode laser did not cause significant histological differences in the initial healing of the vocal folds
