Military housing privatization initiative lessons learned program : an analysis

Approved for public release; distribution in unlimited. / The Military Housing Privatization Initiative (MHPI) represents a dramatic revision in the construction and maintenance of military housing. Since its inception in 1996, the number of projects has grown exponentially and the learning process has been continuous and steep. This thesis researches the effectiveness of the methods in place at the Department of Defense and Service levels to document, share, and, above all, learn from past experiences. This analysis reviews the cumulative documented lessons from all Services and compares it to the lessons learned within the first jointly partnered privatization project at the Presidio of Monterey (POM) and Naval Postgraduate School (NPS) in Monterey, California. This research showed at least six lessons previously documented within the lessons learned system, which were relearned at the POM/NPS project. Given this data, the lessons learned program is a partial success but overall has a positive impact on the MHPI. The current lessons learned program improves each new privatization project ensuring it does not experience all of the same lessons from previous projects. Only a few lessons seem to slip through the cracks and are repeated even after documentation and distribution. / Captain, United States Army

Managing knowledge sharing of extreme weather induced impacts on land transport infrastructure : Case study of the Swedish Transport Administration

Extreme weather events and effects of climate change are threats to the transport sector’s functionality and safety. Risk management in this context implies a necessity to focus on the connection between near-term experiences and coping strategies on one hand, and long-term adaptation analyses on the other. How learning from past events and subsequent knowledge sharing can be adopted is a question that needs to be explored, discussed and tested. A systematic approach to lessons learned calls for measures of investigation, reporting, planning, implementation and evaluation. A qualitative case study approach was used in this thesis. In the first paper the practices of accident investigation in operation and maintenance were inventoried within the Swedish Transport Administration (STA). Three accident investigation methods were applied and tested on a cloudburst event, causing flooding in a railway tunnel in Sweden. In the second paper, semi-structured interviews, documents, and archival records were used as means for penetrating deeper into the attitudes and understanding of lessons learned concerning extreme weather events within a procured public-private partnership. The results of the two studies showed weak signals of feedback on lessons learned. Partly, these weak signals could be traced back to weak steering signals. Various obstacles impeded learning curves from lessons learned. The obstacles were of both hard and soft values, e.g. resources in time and equipment, systematic investigation methods, incentives for lessons learned, education and knowledge, values, norms and attitudes towards how and why identified problems should be solved. Successful knowledge sharing requires that close attention is paid to such obstacles and that an adaptive approach is adopted. / Den pågående och framtida klimatförändringen sätter press på aktörer att möta risker som associeras med klimatförändring. Syftet med denna avhandling är att bidra med kunskap om lärande och kunskapsöverföring inom offentlig förvaltning av landtransportinfrastruktur. Eftersom lärande och kunskapsöverföring är grundläggande för planering och beslutsfattande om strategier och åtgärder som främjar ett robust transportsystem. Målet för detta arbete är att belysa lärande och kunskapsöverföring inom och mellan olika organisationer i det svenska samhället. Det socio-tekniska systemperspektivet - som används som analysram i båda studierna ger en djupare förståelse för bakomliggande faktorer. En kvalitativ ansats, som omfattar intervjuer, deltagande observationer och dokumentanalys, har används i detta licentiatarbete. I den första studien belyses möjligheten att använda sig av industriella utredningsmetoder på naturolyckor i en svensk kontext av en översvämning i en järnvägstunnel. De industriella utredningsmetoderna visade sig vara användbara för utredning av konsekvenser efter skyfall, med olika metodologiska fördelar och nackdelar. Den andra studien utforskar hur Trafikverkets verksamhetsområde Underhåll arbetar med lärande kopplat till väderextremer; hur de fångar upp erfarenheter och kunskap som finns hos kontrakterade entreprenörer, samt hur entreprenörerna uppfattar att lärande och erfarenhetsåterföring sker. Resultatet från båda studierna visar på både svaga styrsignaler och svaga återkopplingssignaler, vilket medför svaga lärandekurvor. Olika hinder sågs ligga bakom med svaga styr- och återkopplingssignaler bl.a. resurser i tid och processer, systematik i utredning av naturolyckor, incitament att lära av varandra, utbildning och kunskap, värderingar normer och attityder till hur och varför identifierade problem ska lösas. Ett adaptivt förhållningssätt innebär att man behöver ta hänsyn till dessa hinder på ett systematiskt sätt. / The agreement in Paris in 2015 was an historic manifestation that society has to work with both mitigation and adaptation to achieve a reduction of the adverse effects of climate change. One way to achieve adaptation is through the integration of present coping strategies. A first step is to study the existing processes and routines that support short-term coping. This licentiate thesis targets different aspects of learning as a strategy for coping and building adaptive capacity. Road infrastructure and maintenance in relation to extreme weather are used as the physical context and the Swedish Transport Administration as a case to study. Paper I shows the possibility to apply industrial accident investigation methods to an extreme weather event and get useful insights into underlying root causes. Paper II shows the intra- and interrelated patterns that exist in public-private partnerships (PPP) in Sweden. The paper describes a parallel of systems with infrequent overlaps regarding lessons learned.  In both papers the socio-technical perspective approach was used to highlight aspects of learning from and investigating damage due to extreme weather at different tiers in society. The socio-technical perspective provides an understanding of how decisions and legislation that affect our actions and behavior today may have been taken in different time and space settings. Strid i bebyggelse - Vilka faktorer möjliggör framgång

Världen globaliseras och urbaniseras i en allt högre takt vilket innebär utmaningar för framtida försvarsmakter. Försvarsmakterna kommer mer frekvent att få lösa militära operationer genom strid i bebyggelse (SIB). Denna undersöknings ansats är att pröva Alice Hills teori om framgångsfaktorer som analyserats fram ur krigshistoriens militära operationer i kontexten SIB. Hills argumenterar för en mängd framgångsfaktorer. Denna undersökning prövar faktorerna utbildningsståndpunkt/stridserfarenhet, tillgång till infanteriförband, nyttjande av indirekt bekämpning och tredje parts inverkan på operationer. Dessa faktorer prövas i en jämförande fallstudie av operationerna i Fallujah 2004 Operation Vigilant Resolve (misslyckad operation) och Operation Phantom Fury (framgångsrik operation) vilka båda ingick i Operation Iraqi Freedom. De källor som har nyttjats för att genomföra teoriprövningen är Vincent L. Foulks, ”The Battle for Fallujah: Occupation, Resistance and Stalemate in the War in Iraq” och Bing Wests ”No True Glory: A Frontline Account of the Battle for Fallujah”. Resultatet av undersökningen stärker Hills teori om de fyra prövade framgångsfaktorerna. Fallstudien påvisar att ingående förband i Operation Vigilant Resolve (misslyckad operation) endast hade en grundlagd utbildningsståndpunkt i SIB samt att stora delar av förbanden saknade stridserfarenhet. Vidare påvisar undersökningen att det var låg numerär av infanteriförband och att mixen mellan infanteri och pansarförband medförde att infanteriförbanden inte kunde lösa uppgiften att skydda pansarförbanden i tillräcklig omfattning. I denna operation finns inget som bevisar att någon form att markgrupperad, indirekt bekämpning nyttjats. Tredje parts påverkan var stor givet förutsättningen att endast en mindre del av befolkning flytt staden Fallujah. Operation Phantom Fury påvisar det motsatta gentemot Operation Vigilant Resolve. Förbanden var bättre utbildade genom att de fått dra lärdom under genomförande under Operation Vigilant Resolve samt att de fått stridserfarenhet genom samma operation. Stor numerär av infanteri och en bättre mix av infanteri och pansarförband där infanteriet understöddes av pansarförbanden istället för tvärtom som var fallet vid Operation Vigilant Resolve. Operation nyttjade indirekt bekämpning i form av granatkastare och artilleri i stor omfattning. Den fjärde och sist prövande framgångsfaktorn, tredje parts inverkan på operationen påvisar att stora delar av staden Fallujah var utrymd vilket indirekt påverkar framförallt faktorn nyttjande av indirekt bekämpning. Sammanfattningsvis stärker denna teoriprövande fallstudie Hills förklaringskraft avseende de fyra utvalda framgångsfaktorerna. Detta ger en vetenskaplig grund att bygga framtidens förband kring där SIB kommer vara frekvent återkommande.

Estudos recentes e a literatura mostram que o compartilhamento e a transferência de conhecimento estão em estágio aquém dos resultados efetivos observados, mesmo com altos investimentos em projetos de gestão e em ferramentas de tecnologia de informação. Este trabalho estuda a gestão de Lições Aprendidas na organização sob uma perspectiva de como os mecanismos de compartilhamento e transferência de conhecimento podem promover uma gestão dinâmica de Lições Aprendidas, visando diminuir essa lacuna. Utiliza, como base conceitual, os ciclos de aprendizagem propostos por Stewhart e Kolb, por exemplo, empregados em processos de gestão de melhorias nas organizações. Também, foram estudados modelos de gestão de compartilhamento e transferência de conhecimento, de diversas fontes relevantes, tais como, Szulanski, Argyris e Dixon, a partir da análise dos seus fatores-chaves e do levantamento de mecanismos de comunicação, interpessoais e eletrônicos, aplicáveis ao contexto do estudo. O quadro teórico gerado serviu como protocolo para ser aplicado em pesquisa de campo, através do método de estudo de caso, em três empresas, indústrias brasileiras competitivas, tidas como sólidas, bem sucedidas, inovadoras e líderes em seus mercados de atuação, entre outros atributos que favorecem o escopo deste trabalho. Buscou-se identificar a replicação da teoria e o grau de suficiência e de eficiência dos mecanismos de “capilarização” das Lições Aprendidas – metáfora criada, no sentido de que, se a gestão das Lições Aprendidas for “capilar”, ou seja, conseguir chegar a todos os pontos da organização, aumenta-se o potencial de aproveitamento dos benefícios da exploração do conteúdo delas. Assim, se tem potencial para melhorar o desempenho da organização, objetivo chave da gestão do conhecimento. / Exploiting the organization's lessons learned is a relevant activity within knowledge management. Lessons learned are part of endemic knowledge instruments which originate from success or failure experience and are motivated during the deployment of tasks or phenomena observation in the activities or organizational processes, It can increase the individual's knowledge when shared or transformed into explicit knowledge which consequently contributes to enhance the organizational knowledge. Recent research and literature have shown that effective results from knowledge sharing and transferring are far beyond from what expected, especially in relation to returns on the high investments made in projects and information technology tools. The present work studies lessons learned in the organization through a perspective of knowledge transfer and sharing mechanisms in the context as how these can promote the dynamic management of lessons learned and thus bridging the result gap. The theoretical framework was developed based on learning cycles proposed by Stewhart and Kolb, for example, applied in continuous improvement processes in organizations. Furthermore, knowledge sharing and transfer models were studied, exploring a broad scope of relevant researchers, such as, Szulanski, Argyris and Dixon. The key success factors were analyzed and a list of relevant communication mechanisms was built, both for personal, face to face, and electronic ones, applicable to the study's scope. This theoretical framework served as a research protocol applied in a field case study in three Brazilian industrial enterprises, recognized as competitive, solid, successful, innovative and leading companies in their markets, among other attributes which contribute for the objectives of present study. Replication of the theoretical framework was sought, besides surveying the degree of "capillarity" of the lessons learned in the organization through observation of the sufficiency and efficiency of the sharing and transfer mechanisms deployed. The "capillarity" metaphor used in this context, refers to the extent of how the lessons learned reach every spot in the organization, increasing the potential use of the benefits carried by the knowledge held in them, creating possibilities for organizational improvement; a key role of knowledge management.

Regarding organizations, these tools can facilitate the sharing of lessons learned and knowledge among employees of an organization and the external public. These facilities can be seen through the real-time communication, access to information by multiple people at the same time and control of information exercised by management or organization. Moreover, the lessons learned are one of the central points of this research, which contributed directly to the organization, management and the security project teams. This study aims to evaluate to what extent the use of an artifact produced by a lessons learned management model can contribute to the management of security projects. As research question of this study narrowed to the following, "To what extent the lessons learned Target 2.0 management model can contribute to the management of security projects?" To this end, this study uses the Design Science Research as a research paradigm and the Technical Action Research as a method, which will reduce the gap between research and practice. This work is characterized as qualitative, using interviews and direct observations techniques. We used a Wiki as a central tool of this artifact, noting the facilities of inputs, consultations and control of the lessons learned from the management and design teams. The device was applied to the organization through a Web 2.0 tool, ie a Wiki, which served as support and support to project teams in the collection of lessons learned. The study found that even the use of the artifact has contributed significantly to the practice of security projects, as there was an improvement in communication between staff and management. Another point seen in the study was the ease of use of the tool users in the process. One of the limitations is the availability of Internet, which is still a problem, because without Internet, the virtual tools or social media are compromised, being a hindrance to the team's work in the field. As a contribution to the theory the artifact created can improve Lessons Learned storage and communication between property security project teams and their managers in the researched organization. / Os softwares sociais, e.g. Wikis, Blogues e Microblogues, têm ganho atenção nos últimos anos no cotidiano das organizações, podendo estes serem incorporados à gestão de projetos como ferramentas colaborativas. No que tange às organizações, essas ferramentas podem facilitar o compartilhamento das lições aprendidas e o conhecimento entre colaboradores de uma organização e o público externo. Estas facilidades podem ser vistas através da comunicação em tempo real, do acesso às informações por várias pessoas ao mesmo tempo e do controle das informações exercido pela gestão ou pela organização. Além disso, as lições aprendidas são um dos pontos centrais desta pesquisa, a qual contribuiu diretamente na organização, na gestão e nas equipes de projetos de segurança. Este estudo tem como objetivo avaliar em que medida o uso de um artefato produzido por meio de um modelo de gestão de lições aprendidas pode contribuir para o gerenciamento de projetos de segurança. Como questão de pesquisa deste estudo delimitou-se a seguinte, "Em que medida o modelo de gestão de lições aprendidas Target 2.0 pode contribuir com a gestão de projetos de segurança?" Para tal, este estudo utiliza o Design Science Research como paradigma de pesquisa e o Technical Action Research como método, que permitem reduzir a lacuna existente entre a pesquisa e a prática. Este trabalho se caracteriza como qualitativo, utilizando as técnicas de entrevista e observação direta. Utilizou-se uma Wiki como ferramenta central deste artefato, observando as facilidades dos inputs, as consultas e controle das lições aprendidas entre a gestão e as equipes de projeto. O artefato foi aplicado na organização por meio de uma ferramenta da Web 2.0, ou seja, uma Wiki, a qual serviu como suporte e apoio às equipes de projeto na coleta das lições aprendidas. O estudo apontou ainda que o uso do artefato contribuiu significativamente para a prática de projetos de segurança, pois houve uma melhora na comunicação entre a equipe e com a gestão. Outro ponto percebido no estudo foi à facilidade de uso da ferramenta pelos usuários nos processos apresentados pelo modelo Target 2.0. Como limitações percebeu-se que a Internet ainda é um problema, pois sem ela as ferramentas virtuais ou as mídias sociais são comprometidas, sendo um empecilho para o trabalho da equipe em campo. Como contribuição para a teoria o artefato criado pode melhorar o armazenamento de Lições Aprendidas e a comunicação entre as equipes de projeto de segurança patrimonial e seus gestores na organização pesquisada.
