Research report (MMed) -- Interdisciplinary Health Sciences, Stellenbosch, 2010. / ENGLISH ABSTRACT: Background The referral system is an important component of the health care system. In public health facilities, a high number of patients’ attendance has lead to a huge burden on the secondary and tertiary level of the care system in terms of manpower, equipments and resources. Public health in South Africa consumes around 11% of the government's total budget. The state contributes about 40% of all expenditure on health; the public health sector is under pressure to deliver services to about 80% of the population. Despite the huge spending on health care in most developing countries, health outcomes and services remain poor. Few studies are available to give insights into reasons for this disparity. Therefore the findings of this may help to explain some of the reasons for this overburden of public health facilities and further to make recommendations on how health service delivery may be improve on. The results of this study can be useful in future planning; this may lead to a reduction in huge health expenditure incurred by most developing countries. Methods A cross sectional survey of three different groups of people which comprises of 273 patients, 28 referral centre participants and 19 referring centre participants was carried out. All patients referred from Motherwell community health centre to Dora Nginza hospital were eligible for the study. Questionnaires were interview administered to patients after they had finished consultations in Dora Nginza Hospital. Health professionals from both facilities were also interviewed with the use of self administered questionnaires. Results Three out of every four patients interviewed were of the opinion that their referral to hospital was appropriate which is consistent with the results from referring health professionals, eighteen of nineteen respondents. However, only one-quarter (7) of the referral centre health professionals felt the referrals from referring centre to hospital were appropriate p<0.01.The majority of the patients were satisfied with the level of service received at the referral centre. 77% (210) reported that the staff at the referral centre was friendly and 84% (230) were happy with the explanation given for their illness. However, a source of concern is that, in most of the referred patients 58% (215), there was no formal response back to their primary care. In the referring centre, participants identified transportation of patients to referral centre as the major problem encountered when referring patients 68 % (13), whereas 32 % (6) felt it is communication. In addition, 73 % (14) were of the opinion that transportation was inadequate and 89 % (17) reported the response rate of transport was unsatisfactory. In the referral centre, results showed participants were more concerned about the adequacy of information provided in the referral letters with 78% (22) reporting they were often not adequate information on the referral letters. However, half of the respondents agreed that they do not have clear referral guidelines. Conclusion Primary care health professionals and patients in this study view the referrals to higher levels of care as appropriate. However, the referral centres health professionals were of the opinion that most referrals were inappropriate. The opinion of the referral centre can be attributed to their negative attitudes towards referrals. The referral centres needs to provide more support to primary care for a more efficient referral system .They also need to improve on the continuity of care by providing feedback to referrals. On the other hand, the primary health care needs to be strengthened in terms of resource allocation in order to gain more confidence from both patients and referral centres. / AFRIKAANSE OPSOMMING: geen opsomming

Constatamos, em nossa análise, que muitas delas apresentam caráter subjetivo, manifestando simpatia pelas matérias da revista, o que inclui também um certo tom confessional. Outras delas manifestam formações discursivas ideológicas distintas, vinculadas ao senso comum ou a heranças culturas passadas. Há ainda outras que se caracterizam por serem cartas de autoridade, ou seja, emitidas por pessoas que têm conhecimento dos temas tratados e que querem fazer críticas positivas ou negativas à maneira como a revista tratou o assunto em questão. / This work has the aim of studying, from the standpoint of the discourse analysis and the cognitive linguistics, a gender of letters, more speciffically letters from readers sent to a scientific divulgation magazine called Superinteressante. We have verified, in our analysis, that some of them presented subjective character, expressing sympathy for the subjects of the magazine, which includes also a sort of confessional feature. Some of them presented distinct discursive ideological structures connected to the common wisdom or to ancient cultural heritages. There were others distinguished by being authority letters, that is wrote by people who have knowledge about the treated subjects, who wanted to criticize in a positive or negative way the manner how the magazine dealt with the subject in focus.

Assim, a quantidade de palha retida no rebolo durante o processo de colheita mecanizada, pode reduzir a quantidade de gemas inviáveis por danos mecânicos e proteger as gemas das condições climáticas adversas. Objetivou-se, avaliar a qualidade de operação de plantio de cana-de-açúcar utilizando mudas com e sem palhada, com utilização de plantadoras e distribuidoras de cana-de-açúcar picada comparados ao plantio semi-mecanizado. O experimento foi conduzido na Fazenda Santa Clara, localizada no município de Pradópolis - SP, com cinco tratamentos sendo, plantadora mecanizada sem palha na muda, plantadora mecanizada com palha na muda, distribuidora mecanizada sem palha na muda, distribuidora mecanizada com palha na muda e plantio semi-mecanizado. A coleta de dados foi realizada em 50 pontos amostrais para cada tratamento, de acordo com as premissas do Controle Estatístico de Processo. A quantidade de gemas inviáveis e as falhas no estande reduziram com a presença de palha no rebolo. A presença de palha no rebolo possibilita o aumento do potencial da área plantada. Dessa forma, a presença de palha na muda melhorou a qualidade do plantio de cana de açúcar. ABSTRACT - The process of sugarcane seedlings mechanized harvesting is decisive in the next operation, which is the mechanized crop planting, mainly in the months of October/November, when the plants experience higher water stress and elevated temperatures. Thus, the amount of straw retained in the stalk during the mechanized harvesting process can reduce the number of non-viable buds due to mechanical damage and protect the buds from adverse climatic conditions. The objective of this study was to evaluate the quality of the sugarcane planting operation using seedlings with and without straw, employing planters and distributors of chopped sugarcane compared to semi-mechanized planting. The experiment was carried out at Santa Clara Farm, located in the city of Pradópolis, SP, Brazil, with five treatments, being: mechanized planter without straw in the seedling, mechanized planter with straw in the seedling, mechanized distributor without straw in the seedling, mechanized distributor with straw in the seedling and semi-mechanized planting. The data collection was performed in 50 sample points for each treatment, according to the premises of the Statistical Process Control. The amount of unviable gems and faults in the stand reduced with the presence of straw on the stalk. The presence of straw in the stalk makes it possible to increase the potential of the planted area. Thus, the presence of straw in the seedling improved the quality of sugarcane cropping. Prolegomena to a critical edition of the letters of Pope Leo the Great : a study of the manuscriptsHoskin, Matthew James Joseph January 2015 (has links)
This dissertation explores the transmission of the letters of Pope Leo the Great (pope, 440-461). In Chapter 1, I set out the contours of Leo’s papacy from external sources and from the letters, showing the significance of these letters for understanding his papacy and its context: our vision of the mid-fifth century would be much scantier without them. After discussing the letters in context and as sources, I conclude this chapter by examining the varied editions of his letters from Giovanni Bussi in 1470, through the only full edition, that of the Ballerini brothers in the 1750s, to the partial editions of Eduard Schwartz and Carlos Silva-Tarouca in the 1930s, a tribute to Leo’s enduring importance. Chapter 2 deals in detail with the pre-Carolingian canonical collections of Leo’s letters, beginning with the earliest in the late 400s and early 500s. Through these collections, I trace the ongoing significance of Leo for canon law as well as noting the links between early Italian collections, e.g. Teatina, Sanblasiana, and Quesnelliana, and postulate that one Gallic collection, Corbeiensis, was the source of another, Pithouensis. I also question the concept of a ‘renaissance gélasienne’ while still admitting the importance of this period for canonical activity. Chapter 3 deals with the letter collections gathered in relation to the Council of Chalcedon (451) – the old Latin version, Rusticus’ version, and the later Latin text, assessing their relationships and importance for our knowledge of Leo as well. Chapter 4 is an exploration of Leo’s letters through the Carolingian and post-Carolingian Middle Ages. The Carolingian explosion of manuscripts is the most important assessed, and I deal with Leo’s various collections in the period, especially Pseudo-Isidore, and demonstrate their relationships and those between them and the earlier collections. To give the reader a sample of the editorial implications of my scholarship, I include as an appendix an edition of Ep. 167 with an apparatus detailing the most significant manuscripts and a translation of my edition as a second appendix. This popular letter exists in different recensions, so it serves an important key to Leo’s text criticism. The third appendix is a conspectus of the letters.
