Análise das notificações em tecnovigilância em instituições hospitalares: diagnóstico de situação / Analysis of notifications in technovigilance in hospital institutions: situation diagnosis

Na área da saúde, principalmente a partir da década de 1970, verifica-se um desenvolvimento científico e tecnológico acelerado, com um aumento do consumo de diferentes tecnologias em todos os níveis de atendimento e consequente aumento dos custos. Muitas inquietações passaram a fazer parte do cotidiano das organizações de saúde diante dessa nova realidade, uma vez que o impasse de incorporar novas tecnologias à prática clínica conflita com a administração de recursos necessários. Em razão da grande diversidade tecnológica e de itens necessários para uma instituição de saúde de grande porte, evidências indicam a premência de mudanças importantes para um efetivo gerenciamento, com controles mais efetivos, exigindo dos gestores um desempenho pautado em eficiência e eficácia. Na atualidade, tem se constituído um desafio para as instituições implementar ações de monitoramento capazes de identificar precocemente possíveis problemas, pois, independentemente do tipo de produto médico-hospitalar utilizado, não há como garantir a ausência de riscos em sua utilização. Diante desta problemática, é de extrema relevância a implantação e o uso de sistemas de registros sistemáticos de avaliações de materiais, enquanto subsídio para uma gestão eficiente no que concerne à maximização de recursos econômicos. Nessa perspectiva, aliada ao interesse em oferecer ao paciente uma assistência isenta de danos e segura na atuação dos profissionais de enfermagem e da equipe multiprofissional é que se justifica a presente pesquisa, que teve como objetivo analisar as não conformidades relativas aos materiais médico-hospitalares notificados ao Gerenciamento de Risco das instituições hospitalares de Ribeirão Preto. Esse foi um estudo descritivo, retrospectivo, documental, com abordagem quantitativa, realizado na cidade de Ribeirão Preto nos serviços de saúde constantes do Cadastro Nacional de Estabelecimentos de Saúde (CNES) categorizados como hospitais públicos, privados ou filantrópicos, gerais ou especializados, nos anos de 2014, 2015 e 2016 e que tivessem serviço de gerenciamento de risco. Os dados foram obtidos do Serviço de Gerenciamento de Risco das instituições participantes. Foi construído um instrumento contendo as seguintes variáveis: produto (nome técnico); grupo de material (médico-hospitalar; higiene pessoal; esterilização); queixa técnica; categoria do problema (embalagem, estrutura e aspecto alterado); setor (unidade que registrou o problema). Foi utilizada estatística descritiva para análise dos dados, utilizando o software SPSS versão 24. Das oito Instituições de Saúde elegíveis para o estudo, cinco (62,5%) autorizaram a realização da pesquisa. A instituição 1 apresentou o maior número de notificações 950 (94%), representando quase a totalidade. Em relação ao ano, foram identificadas 357 (35%) em 2014, 283 (28%) em 2015 e 369 (37%) em 2016. Quanto ao grupo de materiais notificados, identificamos um predomínio dos materiais médico-hospitalares, em todos os anos da pesquisa, com um total de 991 (98,2%) notificações. Em relação ao tipo de material, houve variação dos principais itens durante os anos de estudo, porém, no total geral, identificamos que a seringa foi o principal tipo de material, com 172 (17,1%) notificações. O segundo material foram as luvas de procedimento, com um total de 79 (7,83%) eventos, seguidas dos equipos macrogotas, com 70 (6,94) notificações. Quanto à categorização das queixas que envolviam esses materiais em todos os anos, a principal queixa foi referente à rachadura ou quebra do produto ou de parte dele, com um total de 591 (58,6%) eventos. Os resultados desse estudo demonstraram a importância do monitoramento dos produtos utilizados na assistência à saúde na etapa de pós-comercialização, por meio das queixas técnicas. Identificamos um total de 1.009 notificações, sendo a maioria referente a produtos médico-hospitalares, como seringas e equipos, sendo a principal queixa referente a rachadura ou quebra do produto ou de parte dele. Ressalta-se que estes materiais têm contato direto com o paciente, o que pode ocasionar riscos à sua segurança / In the health area, especially since the 1970s, there has been an accelerated scientific and technological development, with an increase in the consumption of different technologies at all levels of care and consequent increase in costs. Many concerns started to be part of the daily life of health organizations in front of this new reality, since the impasse of incorporating new technologies into clinical practice conflicts with the administration of necessary resources. In this perspective, allied to the interest in offering the patient a harmless and safe assistance in the work of the nursing professionals and the multiprofessional team is that this research is justified, whose objective was to analyze the nonconformities related to the notified medical-hospital materials to Risk Management of the hospital institutions of Ribeirão Preto. This was a descriptive, retrospective, documental study, with a quantitative approach, carried out in the city of Ribeirão Preto in the health services included in the National Register of Health Establishments (CNES) categorized as public, private or philanthropic hospitals, general or specialized, in the years 2014, 2015 and 2016 and that had a risk management service. Data were obtained from the Risk Management Service of the participating institutions. An instrument was constructed containing the following variables: product (technical name); group of material (medical-hospital, personal hygiene, sterilization); technical complaint; category of the problem (packaging, structure and altered appearance); sector (the unit that registered the problem). Descriptive statistic was used for data analysis, using SPSS software version 24. Of the eight Health Institutions eligible for the study, five (62.5%) authorized the research. Institution 1 had the highest number of notifications 950 (94%), representing almost all of them. In relation to the year, 357 (35%) were identified in 2014, 283 (28%) in 2015 and 369 (37%) in 2016. Regarding the group of materials reported, we identified a predominance of medical and hospital materials in all years of the survey, with a total of 991 (98.2%) notifications. Regarding the type of material, there were variations of the main items during the study years, however, in the overall total, we identified that the syringe was the main type of material, with 172 (17.1%) notifications. The second material was the procedure gloves, with a total of 79 (7.83%) events, followed by macrogotes, with 70 (6.94) notifications. Regarding the categorization of complaints involving these materials in all years, the main complaint was about the cracking or breaking of the product or part of it, with a total of 591 (58.6%) events. The results of this study demonstrated the importance of the monitoring of products used in health care in the post-marketing stage, through technical complaints. We identified a total of 1,009 notifications, the majority of which refer to medical and hospital products, such as syringes and equipment, the main complaint being the cracking or breaking of the product or part of it. Em organizações que operam segundo a lógica de serviço há uma mudança estratégica, com o deslocamento da produção de um produto para um valor. Seguindo esta dinâmica, surgiram na Saúde Pública propostas de modelos de atenção alternativos ao hegemônico, centrado em procedimentos e equipamentos. O presente estudo analisou o modelo da Estratégia de Saúde da Família, cuja proposta centra-se nas necessidades do usuário e no vínculo usuário-equipe multiprofissional, tendo como objetivo investigar como a organização e as condições do trabalho influenciam na utilização de recursos imateriais pelas equipes. Consistiu em um estudo de caso realizado junto a oito equipes de saúde da família do município de Caraguatatuba/SP. A metodologia compreendeu observação direta e realização de grupos focais com os profissionais das equipes. A análise abrangeu a categorização dos temas mais relevantes, em especial aqueles que se relacionavam ao uso e desenvolvimento dos recursos imateriais. Os resultados indicaram que, embora os profissionais valorizassem os aspectos relacionais, o processo de trabalho das equipes encontrava-se centrado na produção de procedimentos e informações quantitativos dos atendimentos, não incorporadas às práticas do cuidado. Os recursos imateriais, bem como seus resultados, não encontravam uma forma sistematizada de avaliação. E, dessa forma, enfrentavam desafios para serem apropriados e desenvolvidos como conhecimento pela organização. / Organizations operating according to the service logic imply a strategic change, the shifting of production from that of a product to that of a value. Following such dynamics, new models of attention emerged in the Public Health System, purporting alternatives to the hegemonic model.This study analyzes the model adopted by the Strategy of Family Health, which is focused on the users\' necessities and the bonds between those former and the multi-professional team. Its aim was to investigate how work organization and conditions influence the teams\' utilization of non-material resources. To such purpose a case study was carried out in the city of Caraguatatuba, São Paulo state, Brazil.The methodology comprehended direct observation and focal groups with the teams\' professionals. The analysis covered the categorization of the subjects considered more relevant, especially those concerning the utilization and development of non-material resources. The results pointed out that although relational aspects were valued by the professionals, the teams\' work process was mainly focused on the production of proceedings and information concerning the number of patients assisted, apart from the practices of care.There was no systematic evaluation of the nonmaterial resources and their results, which, therefore, could not be easily appropriated and developed as a knowledge by the organization.

Logistical support is an essential part of the activities of all components of the IRS. Currently, the emphasis is on current need to improve and streamline the intervention of the IRS. It can be clearly seen that the success of interventions of IRS components is in their own logistic support. Without the logistic support would not be any possible way to remove the effects of incidents in the required quality and speed. Concerned parts of IRS should be provided with the necessary amenities for a quality performance of professional activities, usually conducted in very difficult circumstances intervention. It can be clearly admission that the whole area of logistics support under the IRS should be in the spotlight of all competent authorities, even in times of relative peace in any case would not be essential logistic support and other components of IRS underestimated. The main object and purpose of this thesis is to map the basic Logistics Support of the Integrated Rescue System of South Bohemia. A partial object is the SWOT analysis, which specifies internal and external factors that are used when creating a project application, its proposal is part of this work, which can be used to purchase special equipment. Mapping is done in two levels. The first level represent standard security technical and substantive resources, which are collected in each financial year of the basic components of the budget. The second level is an advanced security. For the premium, necessary for aims of this work, is considered the acquisition of property by the European Union. Property acquired from the resources of the European Union has a clear positive impact on improving the logistics of such an intervention. The results of this work is the data that was obtained by secondary analysis and building on it in a standardized interview. Data will be used as background material for the processing of sub-project applications.

Det låter som att någon trillar ner för trappan : En studie om polyrytmik ur ett designteoretiskt perspektiv / It sounds like someone is falling down the stairs : A design research study about polyrhythm

Denna studie undersöker olika semiotiska resurser, teckensystem och tecken på lärande som visat sig i mitt övande på polyrytmik med stöd av det designteoretiska perspektivet. Övningen har haft sin utgångspunkt i två olika metodböcker; Applying Karnatic Rhythmical Techniques to Western Music av Rafael Reina (2015) och The Reference Rhythm Method av Mulle Holmqvist (2010). I Rafael Reinas bok lär författaren ut grunderna inom det indiska tala-systemet, som är en del av den sydindiska klassiska musiken. Mulle Holmqvists metod går ut på att lära sig polyrytmik genom att först lära sig en enklare rytm, som påminner om den önskade polyrytmen, för att sedan jämna ut slagen tills det uppstår en polyrytm. Genom att följa min egen lärprocess med hjälp av loggbok och videoinspelningar vid användandet av dessa metodböcker, redogörs i resultatet för hur olika semiotiska resurser kommit att visa sig och hur de i sin tur synliggjort olika tecken på lärande. Exponerandet av dessa tecken på lärande och användandet av resurser har resulterat i en ökad förståelse för hur olika metoder för polyrytmik kan användas och vilka metoder som fungerat bäst för mig. / This study examines various semiotic resources and signs of learning that have manifested themselves in my practice of polyrhythms, with the support of the design theoretical perspective. Two different method books have been used in this essay; Applying Karnatic Rhythmical Techniques to Western Music by Rafael Reina (2015) and The Reference Rhythm Method by Mulle Holmqvist (2010). Rafael Reina's book teaches the basics of the Indian tala-system, which is part of South Indian classical music. Mulle Holmqvist's method involves learning polyrhythms by first learning a simpler rhythm, which resembles the desired polyrhythm, and then level out the beats until a polyrhythm occurs. By following my own learning process with the help of a notebook and video recordings, whilst using these method books, it is shown in the results how different semiotic resources is discovered and how they affect different signs of learning. Exposing these signs of learning and the use of resources has resulted in a greater understanding of how different methods of polyrhythms can be used and which methods have worked best for me.

Strategies to enhance accessibility to health care in rural areas of Zimbabwe

Background: Accessibility to health care in rural areas is globally impeded by physical, material, human, financial and managerial resources and societal barriers in the health care system. The Systems Model formed the backbone to developing a strategic action plan to address the challenges experienced by all stakeholders involved. Purpose: The purpose of this study was to describe accessibility to health care in rural areas to develop a strategic action plan to enhance accessibility to health care in these areas of Zimbabwe. Methods: A multiple methods approach combining qualitative and quantitative components during 4 phases. Phase 1 collected quantitative data with questionnaires from professional nurses and health care users who were conveniently sampled. Phase 2 collected qualitative data with a nominal group from national health directors who were conveniently sampled. Phase 3 was based on the findings from Phases 1 and 2 with a literature control to develop a draft strategic action plan. During phase 4 the strategic plan was amended and validated with a validation tool by members of the parliamentary portfolio committee on health in Zimbabwe with all-inclusive sampling. Framework: The Systems Model Framework was adopted for this study as outcomes and impact on people’s health is determined by inputs, processes and outputs. This model was relevant and applicable to accessibility to health care. Research findings: Accessibility to health care in rural areas of Zimbabwe is affected by inadequate distribution of physical resources, shortage of material and human resources, and a lack of financial resources. The strategies identified contributed to the strategic action plan which was amended and validated. The strategic action plan includes improving the health infrastructure, providing appropriate medical drugs, training and retention of health workers, providing medical equipment at the rural health facilities, reviewing the health worker workload and addressing staff shortages, providing free health care services in rural areas, and improving the capacity of the health care system. Conclusion: The strategic action plan was developed based on the inputs of the relevant stakeholders and the System Model. The inclusion of the parliamentary portfolio committee on health (members of parliament of Zimbabwe) might enhance the possibility for implementation which can enhance the accessibility to health care in rural areas of Zimbabwe. / Health Studies / D. The author designed the dispatching system structure of metalcutting FMS, at that it was placed high emphasis on hardware recourses, in the course of study. The author analyzed present information transmission and encoding methods in FMS: uniform codes, codes with control at par, optimum code, Chudakov code and Huffman - Liberman codes. The last one is the best for given problem. It was created the original Huffman - Liberman codes synthesis algorithm, which is conformed to present hardware level of development. On the base of this algorithm it was coded the Huffman - Liberman codes synthesis program, which was taken out the Certificate of state program registration № 2013612951. Huffman - Liberman codes for Russian, Ukrainian and English alphabets and CNC machines executive program were synthesized with the help of this program. The results of the work were published in four scientific-technical journals. In the course of study the author designed automatic guided vehicle control system, which involves rate of movement, motion path, positioning and acceleration -braking control systems. The latest two systems are original, which are designed by the author. Designed positioning control system allows to program the break time of automatic guided vehicle and avoid missing of stop place. This system took out a patent of Russian Federation № 130540. Designed acceleration -braking control system allows to change automatic guided vehicle speed fluently (without stage), and to change rate of acceleration -braking and maximum speed depending on cargo tonnage. This system took out a patent of Russian Federation № 132278. Engineering solutions, introduced in the work, can be used at any metalcutting FMS. / В ходе научной работы была разработана структура системы диспетчирования ГАЦ механообработки, при этом особое внимание было уделено средствам технического обеспечения разработанной системы. Были рассмотрены и проанализированы существующие методы передачи и кодирования информации в ГПС: равномерные коды, коды с контролем по паритету, оптимальные коды, коды Чудакова и коды Хаффмена - Либермана. Наиболее оптимальными для решения поставленных задач являются последние. Был создан оригинальный алгоритм синтеза кодов Хаффмена - Либермана, отвечающий современному уровню развития вычислительной техники, на основе которого была написана программ синтеза кодов Хаффмена - Либермана, получившая Свидетельство о государственной регистрации программы для ЭВМ № 2013612951. С помощью данной программы были получены коды Хаффмена - Либермана для управляющих программ станков ЧПУ, русского, украинского и английского алфавитов. Результаты работы были опубликованы в трех научно-технических журналах. В ходе работы также была разработана система управлением робокаром, включающая в себя системы управления скоростью движения, траекторией движения, позиционирования и разгоном - торможением робокара, при этом две последние системы являются оригинальными, разработанными автором диссертации. Разработанная система управления позиционированием робокара, в отличии от существующих аналогов, позволяет программировать время выстоя робокара и избежать "проскока" требуемой позиции остановки. На данную систему был выдан Патент РФ № 130540. Разработанная система управления разгоном - торможением робокара, в отличие от существующих аналогов, позволяет осуществлять регулирование скорости привода плавно, без ступеней, а также способна изменять темп разгона - торможения и максимальную скорость автоматически в зависимости от массы перевозимого груза. На данную систему был выдан Патент РФ № 132278. Технические решения, предложенные в диссертации могут рассматриваться как типовые для ГАП механообработки.

Структура магистерской диссертации включает в себя введение, три главы, заключение, список использованных источников и приложения. В первой главе дано определение термину «ресурс». Рассмотрены различные группы ресурсов. Рассмотрены существующие ресурсные концепции. Определены основные виды ресурсов, используемых муниципальными образованиями Свердловской области. Сформулирован вывод об эффективности процессов выявления и использования материальных и нематериальных ресурсов муниципальными образованиями региона. Во второй главе рассматривается группа нематериальных ресурсов. Уделено внимание роли нематериальных ресурсов в развитии муниципального образования на примере ГО Верхняя Пышма. Проанализирована стратегия социально-экономического развития ГО Верхняя Пышма с учетом использования нематериальных ресурсов. Разработана ресурсно-ориентированная методика оценки деятельности муниципальных образований Свердловской области на примере ГО Верхняя Пышма. В третьей главе проведена оценка социально-экономической эффективности планируемого к реализации инвестиционно-строительного проекта по строительству физкультурно-оздоровительного центра в ГО Верхняя Пышма. На основе проведённого анализа был выбран один из потенциальных векторов развития ГО, направленный на решение задачи сохранения и укрепления здоровья населения, на базе улучшения физкультурно-оздоровительной инфраструктуры ГО. / The structure of the master's thesis includes an introduction, three chapters, a conclusion, a list of sources used and applications. The first chapter defines the term “resource”. Various resource groups are considered. Existing resource concepts are considered. The main types of resources used by municipalities of the Sverdlovsk region are identified. The conclusion is formulated on the effectiveness of the processes of identifying and using material and intangible resources by the municipalities of the region. The second chapter discusses a group of intangible resources. Attention is paid to the role of intangible resources in the development of a municipality using the example of Verkhnyaya Pyshma civil defense. The author analyzes the strategy of socio-economic development of Verkhnyaya Pyshma GO taking into account the use of intangible resources. A resource-oriented methodology has been developed for assessing the activities of municipalities in the Sverdlovsk region using the example of Verkhnyaya Pyshma municipal unit. The third chapter assesses the socio-economic efficiency of the investment and construction project planned for the implementation of the construction of a fitness center in Verkhnyaya Pyshma. Based on the analysis, one of the potential vectors for the development of civil defense was selected, aimed at solving the problem of maintaining and strengthening the health of the population, based on improving the physical culture and health infrastructure of civil defense.
