Sendo assim, a avaliação do acesso e do processo da assistência pré-natal é de suma importância na detecção dos nós que persistem no serviço oferecido. Os objetivos deste estudo são os de avaliar a adequação do processo da assistência pré-natal segundo os parâmetros do Programa de Humanização do Pré-natal e Nascimento acrescido dos procedimentos previstos pela Rede Cegonha, bem como o acesso à assistência pré-natal, segundo as dimensões de disponibilidade, capacidade de pagamento e aceitabilidade, no SUS da microrregião de São Mateus - ES. Trata-se de um estudo seccional que selecionou 742 puérperas em sete maternidades da região escolhida para a pesquisa. Os dados foram coletados por meio de entrevistas e análise do cartão da gestante e prontuário do recém-nascido. Posteriormente, esses dados foram processados e submetidos ao teste Qui-quadrado, ao teste exato de Fisher e ao teste não paramétrico de Spearman com p-valor menor que 5%. Apesar de o SUS garantir constitucionalmente o acesso universal ao sistema de saúde, nota-se que ainda existem iniquidades entre as puérperas no acesso aos serviços de saúde. Apenas 7,4% e 0,4% das puérperas receberam pré-natal adequado segundo os parâmetros do PHPN e Rede Cegonha, respectivamente. Recomenda-se que a localização das unidades de saúde, os recursos de transporte e o financiamento dos serviços de saúde sejam reorganizados, do mesmo modo o atendimento pré-natal precisa ser sistematizado, mais humano e focalizado na atenção as mulheres mais vulneráveis / Prenatal care is one of the factors that contributes to the persistence of adverse perinatal outcomes and maternal death at high levels. Thus, the evaluation of access and the process of prenatal care is of high importance in the detection of the issues who remain in the offered service. The objectives of this study are to evaluate the adequacy of the process of prenatal care according to the parameters of the Program for the Humanization of Prenatal and Birth plus procedures provided by Stork Network , as well as access to prenatal care, according to the dimensions of availability, acceptability and ability to pay, on the SUS of the region of São Mateus - ES. It is a cross-sectional study that selected 742 postpartum women in seven hospitals in the region chosen for the research. Data were collected through interviews and analysis of records from prenatal care and newborn records. Subsequently, these data were processed and subjected to chi-square test, the Fisher exact test and the nonparametric Spearman with p-value less than 5%. Although the SUS constitutionally guarantee universal access to health care, it is noted that there are still inequalities between pregnant women in accessing health services. Only 7.4% and 0.4% of the mothers received adequate prenatal care according to the parameters PHPN and Stork Network , respectively. It is recommended that the location of health facilities, transport resources and financing of health services are reorganized, the same way the prenatal care needs to be systematized, more humane and focused on attention to the most vulnerable women

Práticas obstétricas e influência do tipo de parto em resultados neonatais e maternos em Sergipe / Obstetric practices and the influence of mode of delivery on neonatal and maternal outcomes in Sergipe

Background: Brazil has high frequency of inappropriate obstetric practices and cesarean sections. This procedure may be associated with increased maternal and perinatal morbidity. Objective: to describe practices and interventions used during labor and childbirth and factors associated with such practices and to evaluate the impact of mode of delivery in breastfeeding incentive practices and in neonatal and maternal complications. Methods: A Cohort study was conducted, between june 2015 and april 2017, at the 11 maternity hospitals in Sergipe with 768 puerperal women, interviewed in the first 24 hours after delivery, 45-60 days and 6-8 months after delivery. The associations between good practices and interventions used during labor and delivery with exposure variables were described by simple frequencies, percentages, crude and adjusted odds ratios (ORA) with the confidence interval and the association between breastfeeding incentive practices, neonatal and maternal, both short term and late complications and the exposure variables were evaluated by the relative risk (95% IC) and the Fisher exact test. Results: were fed 10.6% of women and 27.8% moved during labor; non-pharmacological measures for pain relief were performed in 26.1%; the partograph was filled in 39.4% of the charts and the companion was present in 40.6% of deliveries. Oxytocin, amniotomy and analgesia occurred in 59.1%, 49.3% and 4.2% of women, respectively. The delivery occurred in the lithotomy position in 95.2% of the cases, there was episiotomy in 43.9% and Kristeller's maneuver in 31.7%. The factors most associated with cesarean section were the private health sector (ORA = 4.27,95% CI: 2.44-7.47), had higher education (ORA = 4.54,95%CI 2.56 -8.3) and high obstetric risk (ORA = 1.9,95%CI: 1.31-2.74). Private-sector users had a greater presence of the companion (ORA = 2.12,95% CI:1.18-3.79) and analgesia (ORA = 4.96,95% CI:1.7-14.5). The C-section delivery resulted in less skin-to-skin contact immediately after delivery (intrapartum c-section: RR=0.18;95%CI:0.1-0.31 and elective c-section: RR=0.36;95%CI:0.27-0.47) and less breastfeeding within one hour of birth (intrapartum C-section: RR=0.43;95%CI:0.29-0.63 and elective C-section: RR=0.44;95%CI:0.33-0.59). Newborns from elective c-section were less frequently breastfed in the delivery room (RR=0.42;95%CI:0.2-0.88) and stayed less in rooming- in (RR=0.85;95%CI:0.77-0.95). Women who were submitted to intrapartum c-section had greater risk of early complications (RR=1.3;95%CI:1.04-1.64; p=0.037) and sexual dysfunction (RR=1.68;95%CI:1.14-2.48; p=0.027). There was no difference in the frequency of neonatal complications, urinary incontinence and depression according to the mode of delivery. Conclusions: good obstetric practices are poorly performed and unnecessary interventions are frequent, and the factors most associated with cesarean delivery have been the private health sector, higher schooling and high obstetric risk. C-section was negatively associated to breastfeeding incentive practices. C-section after labor increased the risk of early maternal complications and sexual dysfunction, six to eight months after delivery. / Introdução: No Brasil, verifica-se elevada frequência de práticas obstétricas inadequadas e de cesáreas. Este procedimento pode associar-se a aumento de risco de morbidade materna e neonatal. Objetivo: descrever as práticas utilizadas durante o trabalho de parto e parto e fatores associados e avaliar práticas de incentivo à amamentação, complicações neonatais e maternas precoces e tardias segundo tipo de parto. Pacientes e Métodos: estudo tipo coorte, no período de junho de 2015 a abril e 2016, nas 11 maternidades de Sergipe, com 768 puérperas entrevistadas após 6h do parto, 45 a 60 dias e 6 a 8 meses após o parto e análise de dados do prontuário das puérperas e dos recém-nascidos. As associações entre as boas práticas e intervenções utilizadas durante o trabalho de parto e parto com as variáveis de exposição foram descritas em frequências simples, percentuais, razões de chances brutas (OR) e ajustadas (ORA) com o intervalo de confiança e as associações entre as práticas de incentivo à amamentação, as complicações neonatais e maternas precoces e tardias e as variáveis de exposição foram descrias por risco relativo (IC=95%) e pelo teste exato de Fisher. Resultados: alimentaram-se 10,6% das mulheres e 27,8% movimentaram-se durante o trabalho de parto; medidas não farmacológicas para alívio da dor foram realizadas em 26,1%; o partograma estava preenchido em 39,4% dos prontuários e o acompanhante esteve presente em 40,6% dos partos. Ocitocina, amniotomia e analgesia ocorreram em 59,1%, 49,3% e 4,2% das mulheres, respectivamente. O parto ocorreu na posição de litotomia em 95,2% dos casos, houve episiotomia em 43,9% e manobra de Kristeller em 31,7%. Os fatores mais associados à cesárea foram ser do setor privado de saúde (ORA=4,27;95%CI:2,44-7,47), ter maior escolaridade (ORA=4,54;95%CI:2,56-8,3) e alto risco obstétrico (ORA=1,9;95%CI:1,31-2,74). Usuárias do setor privado tiveram maior presença do acompanhante (ORA=2,12;95%CI:1,18-3,79) e analgesia (ORA=4,96;95%CI: 1,7-14,5). Os recém-nascidos de puérperas que se submeteram a cesárea tiveram menor frequência de contato pele a pele com suas mães imediatamente após o parto (cesárea intraparto: RR=0,18;95%CI:0,1-0,31 e cesárea eletiva: RR=0,36;95%CI:0,27-0,47) e mamaram menos na primeira hora de vida (cesárea intraparto: RR=0,43;95%CI:0,29-0,63 e cesárea eletiva: RR=0,44; 95%CI:0,33-0,59). Recém-nascidos de cesárea eletiva foram menos frequentemente colocados para mamar na sala de parto (RR=0,42;95%CI:0,2-0,88) e ficaram em menor frequência em alojamento conjunto (RR=0,85;95%CI:0,77-0,95). As mulheres submetidas a cesárea intraparto tiveram maior risco de complicações precoces (RR=1,3;95%CI:1,04-1,64; p=0,037) e de disfunção sexual (RR=1,68;95%CI:1,14-2,48; p=0,027). Não houve diferença nas frequências de complicações neonatais, incontinência urinária e de depressão segundo tipo de parto. Conclusões: boas práticas obstétricas são pouco utilizadas e intervenções desnecessárias são frequentes e os fatores mais associados à operação cesariana foram ser do setor privado de saúde, ter maior escolaridade e alto risco obstétrico. A cesárea associou-se negativamente às práticas de incentivo à amamentação. A cesárea após trabalho de parto associou-se a maior risco de complicações maternas precoces e a disfunção sexual seis a oito meses pós-parto. / São Cristóvão, SE

Measuring and evaluating quality of care in referral maternities in Mali and Senegal in the context of overlapping interventions

Dans cette thèse, nous décrivons les résultats d’un projet de recherche visant à mesurer et évaluer la qualité des soins obstétricaux des hôpitaux de référence au Mali et au Sénégal. Dans ces pays, la mortalité maternelle hospitalière est élevée et est liée en partie à la pratique médicale inadéquate. Cette recherche a été réalisée dans le cadre de l’étude QUARITE, un essai randomisé en grappe évaluant l’efficacité du programme GESTA International visant à réduire la mortalité maternelle hospitalière. GESTA a été mis en œuvre entre 2008 et 2010 et consistait en la formation des professionnels de santé et en la revue des cas de décès maternels. En parallèle de QUARITE, les programmes de prévention de la transmission du VIH de la mère à l’enfant (PTME) ont été mis à l’échelle à travers les pays. Ces derniers ayant également la capacité d’augmenter la qualité des soins obstétricaux, nous avons donc évalué les effets des deux programmes (GESTA et PTME) sur la qualité des soins. Dans un premier temps, à l’aide d’une recension des écrits nous avons évalué la capacité d’un audit clinique basé sur des critères à mesurer la qualité des soins obstétricaux. Cet audit vérifiait si l’offre des soins avait respecté les critères cliniques définissant la meilleure prise en charge selon l’évidence scientifique et l’avis des experts. Nous avons démontré que cet outil est largement utilisé dans les pays à faibles et moyens revenus, malgré le peu d’évidence sur sa validité (article 1). Dans un deuxième temps, nous avons développé un audit clinique basé sur des critères qui s’applique au contexte ouest-africain et qui a été approuvé par des experts-obstétriciens nationaux et internationaux. À partir des dossiers obstétricaux, les actes médicaux posés pendant le travail et l’accouchement ont été évalués à l‘aide de cet instrument. La qualité des soins a été estimée sous forme de pourcentage de critères atteints. Appliqué dans différents contextes et par différents auditeurs, nous avons démontré que notre instrument est fiable et valide (article 3). Néanmoins, l’expérience de l’audit nous a amenés à nous questionner sur le mauvais remplissage des dossiers médicaux et ses conséquences sur la qualité des soins (article 2). Dans un troisième temps, l’outil a été appliqué à large échelle pour évaluer les effets de l’intervention GESTA (article 4). Nous avons mené une révision de plus de 800 dossiers obstétricaux dans 32 hôpitaux de référence (16 bénéficiaires de l’intervention et 16 non-bénéficiaires). Grâce à cet audit clinique, nous avons démontré que le programme GESTA contribue à l’amélioration de la qualité des soins, spécifiquement l’examen clinique lors de l’admission et le suivi après l’accouchement. Dernièrement, nous avons utilisé cet instrument afin d’évaluer les effets des programmes de PTME sur la qualité des soins obstétricaux (article 5). Notre travail a documenté que seulement certaines composantes du programme de PTME améliorent la qualité des soins telles que la formation des professionnels et les services complémentaires en nutrition. En conclusion, cette recherche a identifié plusieurs pistes d’intervention pour améliorer la qualité des soins obstétricaux en Afrique de l’Ouest. / In this thesis, we describe the results of a research project that aimed to measure and evaluate quality of care in referral hospitals in Mali and Senegal. In these countries, hospital maternal mortality is high and linked, in part, to inadequate medical practice. This research was conducted as part of the QUARITE cluster randomized trial that assessed whether the program, ALARM International, could reduce facility maternal mortality. ALARM was implemented from 2008 to 2010 and consisted of the training of local health professionals and the use of maternal death reviews. At the same time as QUARITE was ongoing, programs for the prevention of maternal to child transmission of HIV (PMTCT) were scaled- up; these can also improve obstetrical quality of care. Thus, we evaluated the effects of both programs (ALARM and PMTCT) on quality of care. We began with a systematic review of the literature to evaluate the capacity of a criterion-based clinical audit to measure the quality of obstetrical care (article 1). This type of audit verifies if the care provided meets criteria indicative of best clinical practices, according to the literature and expert opinion. Our review demonstrates that this tool has been used in a variety of low- and middle-income settings, but the way it has previously been employed leaves doubts as to its validity (article 1). We thus developed a criterion based clinical audit specific to the West African context and approved by national and international expert obstetricians. Using patient medical records, with this instrument we evaluated obstetrical care provided during labour and delivery. Quality of care was calculated based on the percentage of care criteria met. Applied to different sites and by different auditors, our instrument demonstrated concordant results and provided a valid image of the quality of obstetrical care provided at hospitals in the region (article 3). Nonetheless, the audit experience raised concerns about the implications of poor medical recordkeeping and archiving on quality of care (article 2). We used the criterion-based clinical audit to review over 800 medical records at 32 QUARITE hospitals (16 intervention and 16 control hospitals) in order to evaluate the effects of the ALARM intervention. We demonstrated that the ALARM program contributes to better obstetrical quality of care, especially during the first clinical examination and postpartum monitoring of women treated at intervention hospitals (article 4). Finally, we used this instrument to evaluate the effects of PMTCT programs on obstetrical quality of care (article 5). Our work demonstrated that certain components of a PMTCT program, specifically training of healthcare professionals and supplementary nutritional services, are associated with better obstetrical care. In all, this research identified several mechanisms that can be targeted by quality improvement interventions in West Africa.

Antenatal Stressful Life Events and Postpartum Depression in the United States: the Role of Women’s Socioeconomic Status at the State Level

The purpose of this dissertation was to examine patterns of antenatal stressful life events (SLEs) experienced by women in the United States (U.S.) and their association with postpartum depression (PPD). It further explored the role of women's state-level socio-economic status (SES) on PPD; the racial/ethnic dispartites in SLE-PPD relationship; and the role of provider communication on perinatal depression. Data from 2009–11 Pregnancy Risk Assessment Monitoring System (PRAMS) and SES indicators published by the Institute of Women’s Policy Research (IWPR) were used. Latent class analysis (LCA) was performed to identify unobserved class membership based on antenatal SLEs. Multilevel generalized linear mixed models examined whether state-level SES moderated the antenatal SLE-PPD relationship. Of 116,595 respondents to the PRAMS 2009-11, the sample size for our analyses ranged from 78% to 99%. The majority (64%) of participants were in low-stress class. The illness/death related-stress class (13%) had a high prevalence of severe illness (77%) and death (63%) of a family member or someone very close to them, while those in the multiple-stress (22%) class endorsed most other SLEs. Eleven percent had PPD; women who experienced all types of stressors, had the highest odds (adjusted odds ratio [aOR]: 5.43; 95% confidence interval [CI]: 5.36, 5.51) of PPD. The odds of PPD decreased with increasing state-level social/economic autonomy index (aOR: 0.75; 95% CI: 0.64, 0.88), with significant cross-level interaction between stressors and state-level SES. Among non-Hispanic blacks and non-Hispanic whites, husband/partner not wanting the pregnancy (aOR: 1.47; 95% CI: 1.14, 1.90) and drug/drinking problems of someone close (aOR: 1.37; 95% CI: 1.21, 1.55) were respectively associated with PPD. Provider communication was protective. That 1 out of every 5 and 1 out of every 8 women were in the high- and emotional-stress classes suggests that SLEs are common among pregnant women. Our results suggest that screening for antenatal SLEs might help identify women at risk for PPD. The finding that the odds of PPD decrease with increasing social/economic autonomy, could have policy implications and motivate efforts to improve these indices. This study also indicates the benefits of antenatal health care provider communication on perinatal depression.

Bipolar Disorder in the Perinatal Period: Understanding Gaps in Care to Improve Access and Patient Outcomes

Background: Bipolar disorder (BD) is a significant cause of perinatal morbidity and mortality. Because BD is hard to detect and treat, these individuals often go without care. This dissertation was designed to: (1) identify the prevalence rates of BD and bipolar-spectrum mood episodes in perinatal individuals, (2) understand pertinent barriers to mental healthcare, and (3) elucidate how to bridge healthcare gaps. Methods: Data sources included: primary qualitative and quantitative data from obstetric clinicians, encounter data from Massachusetts Child Psychiatry Access Program (MCPAP) for Moms, a program aimed at helping clinicians to provide mental healthcare to perinatal patients. Analyses included: descriptive statistics, systematic review and meta-analysis, qualitative data analyses, longitudinal regression analyses, and group-based trajectory modeling. Results: The prevalence of BD in perinatal individuals was 2.6% (95% CI: 1.2 to 4.5%). Twenty to 54.9% were found to have a bipolar-spectrum mood episode. Barriers to mental healthcare for perinatal patients with BD included the paucity of psychiatric resources, difficulties in assessing BD, and stigma towards pharmacotherapy. Obstetric clinicians reported that MCPAP for Moms has helped them feel more comfortable in treating patients with BD. Longitudinal analyses of encounter data corroborated these findings - utilization of the program predicted increased clinician capacity to treat BD. Conclusion: Clinicians for perinatal individuals are being called upon and stepping up to care for complex illnesses like BD. Programs like MCPAP for Moms can help them feel more confident in this role, helping to bridge gaps in perinatal mental healthcare and ensuring that individuals with BD are able to receive appropriate care.

The Effectiveness Of Interventions And Bundles For Central Line-Associated Bloodstream Infections In The Neonatal Intensive Care Unit

Introduction: Central Line-Associated Bloodstream Infections (CLABSIs) are a major cause of increased mortality, morbidity and healthcare costs in neonatal intensive care units (NICUs) patients. Despite CDC's efforts to reduce infection rates, patients often suffer consequences. The objective of this study is to perform a systematic review of strategies utilized in the neonatal population and evaluate them with the current CDC's guidelines to assess the effectiveness of bundles in preventing CLABSI in NICUs. Methods: A systematic literature search was conducted using CINAHL Plus with Text, Cochrane Database of Systematic Reviews and MEDLINE from January 2008 up to 2018. There were multiple search terms used and these included "neonate OR newborn OR infant", "CLABSI OR central line-associated bloodstream infection", "intervention OR prevention" and "bundle". The search solely focused on the outcome of infant patients. Therefore studies were excluded for the following criteria: being non-peer reviewed, being published before 2008, and being a case in which CLABSI was assessed in patients outside the NICU. See Table 4 and 5 for further information. Results: Eight articles were eligible for inclusion all of which CDC's guidelines were implemented in their strategy of intervention. The systematic review showed that adherence to care bundles decreases infection rates drastically. All eight articles reported a significant decrease in CLABSI rates following the implementation of the bundle set by CDC with two studies achieving a CLABSI rate of zero. Author's Conclusion: Implementation of care bundles showed a success in reducing CLABSI rates in the NICUs; however none of the studies endorsed a specific bundle application utilized to achieve its intended goal. Some practices adopted CDC's guidelines more than others and those showed a greater decrease in infection rate. In addition, it is evident that nurses deliver the best care when preventing an infection. Further research is needed to assess the effectiveness of a specific bundle element.

Embora o rastreamento e acolhimento dos casos nos serviços sejam recomendados, no cenário brasileiro, nem a busca ativa das mulheres em situação de violência e, nem tão pouco aformação em saúde prepara os profissionais para lidarem com o problema. Partindo do questionamento sobre que propostas político-pedagógicas poderiam contribuir para uma qualificação profissional em saúde diferenciada e, baseando-se na experiência de um projeto de pesquisa-ação(Projeto VDG), desenvolvido em hospital público com vistas a formar profissionais de saúde para a abordagem da violência doméstica na gravidez, o estudo de doutorado objetivou analisar o fenômeno da violência doméstica na gravidez (VDG), com foco na violência entre parceiros íntimos(VPI) e o uso da pesquisa-ação(PA) como ferramenta estratégica na qualificação da atenção em saúde,desdobrando-se em quatro artigos: um ensaio que demarcou conceitualmente o problema da violência contra mulheres e demonstrou a magnitude e complexidade do problema, seus efeitos na saúde da mulher e sua invisibilidade nos serviços; uma revisão que aprofundou dados sobre os limites e oportunidades na formação em saúde; um artigo que descreveu e analisou as entrevistas em profundidade com profissionais de saúde envolvidos na PA; e , o relato e análise da experiência VDG no que se referiu ao uso de entrevistas semiestruturadas e de espaços de discussão coletiva para organização de uma rotina de acolhimento da VPI. Na discussão ampliada da tese, entre os resultados da PA, destaca-se que: a própria metodologia desenvolveu uma estratégia de conhecimento e de ação sobre a VPI, aproximando-se de uma proposta de educação permanente; o processo de visibilidade construída sobre os sinais e situações associados à VDG iniciado com as entrevistas e, a angústia compartilhada sobre o processo de trabalho em saúde nos espaços coletivos de discussão, tanto potencializaram saberes como humanizaram e acolheram os profissionais, o que colaborou para uma mobilização coletiva e a proposição de uma rotina institucional para identificar e acolher a violência; no limiar desta experiência, vislumbrouse o ideal de um processo de crescimento profissional-político baseado na reflexão-ação-reflexão coletiva e na articulação permanente da gestão da educação à gestão do processo de trabalho em saúde. Conclui-se que esta forma de experimentação em situação real, sem o pressuposto da neutralidade, colaborou no levantamento de informações de difícil acesso e incrementou o conhecimento sobre as percepções e práticas de profissionais e, sobre indicadores qualitativos que comporiam um acolhimento qualificado e demarcariam tanto modelos político-pedagógicos mais adequados à formação em saúde, como as ações necessárias à organização dos serviços frente ao tema da VPI. / Violence during pregnancy is related to a very serious pattern of violence and is possibly more frequent than other problems routinely controlled-for during prenatal care. Although the active identification and support of women living in violent situations have been recommended, in Brazil routines for identification of existing cases have not been established, nor does the training of health professionals prepare them for dealing with this problem. This doctoral study begins with the question of which political-pedagogical proposals could contribute to adequate preparation of professionals and analyses an action research project (Domestic Violence during Pregnancy – DVP) which was carried out in a public hospital with the objective of preparing health professional for dealing with domestic violence in pregnancy, with a focus on violence between intimate partners. The study of action research as a strategic tool for constructing quality in health care is organized in 4 papers: an essay that conceptually delimits the problem of violence against women and demonstrates the magnitude and complexity of the problem, its effects on women’s health, and its ‘invisibility’ in health services; a review of literature on education and training of health professionals which reveals the limits and opportunities of such proposals; an article which analyses in-depth interviews with health professionals who participated in the DVP project; and an examination of this experience, which used semistructured interviews and collective discussion groups for the organization of a routine for cases of violence between intimate partners. Among the results of the VDP project, emphasis is given to the fact that the action research methodology developed a strategy of knowledge-building for action as a form of continued education in the case of violence between intimate partners; the process of ‘constructed visibility’ of the signs and situations associated with VDP built on discussion of the interview data and the ‘shared anxiety’ regarding the work process in health which occurred in the collective discussion groups, and both strengthened their perceptions as well as ‘humanized’ and gave support to the professionals, all of which collaborated in a collective mobilization and a proposal to develop a new routine for identifying and supporting women living in violence. This experience exemplified the possibility of professional/political growth based on a process of collective reflection-action-reflection and on the permanent articulation of education within the process of managing health care services. As a conclusion, this form of ‘experiment’ in a real-life situation, with no supposition of neutrality, collaborated in producing strategic information and increased knowledge of professional perceptions and practices and qualitative indicators regarding ‘qualified attention’ which are necessary in political/pedagogical models more adequate for health education, as well as for actions needed for the organization of services which deal with violence between intimate partner.
