Postoj a možnosti sester při péči o pacienty s intoxikací ethylenglycolem. (teoretická práce) / Attitudes and options of nurses in the care of ethylene glycol poisoned patients. (theretical thesis)

KMETKOVÁ, Monika January 2015 (has links)
Ethylene glycol intoxication, suicidal as well as accidental, belongs to serious, immediately life-threatening poisonings, which can however be cured even in the most severe cases (provided early recognition and correct treatment), including the restoration of renal function to the original state. Ethylene glycol is most notably present in anti-freeze cooling liquids for vehicles (such as Fridex). Consumption of 100-150 ml of such a solution is considered a lethal dose in humans. follows Oral ingestion is followed by rapid absorption. Metabolic decomposition takes place predominantly in the liver by way of alcohol dehydrogenase, to glycoaldehyde, which is further metabolised to glycolic acid and oxalic acid. The clinical aspect is dominated by acute renal failure which is caused by acute tubular necrosis. Toxicological examination including an assessment of ethylene glycol levels in both blood and urine, with prognostic relevance, is indispensable to diagnostics of the poisoning as well as from the standpoint of differential diagnostics. Therapeutic measures include gastric lavage within one hour of ingestion and administration of activated charcoal. Administration of ethanol and fomepizole, compounds that bind alcohol dehydrogenase and thus prevent the conversion of ethylene glycol to its toxic metabolites, is considered valid causal treatment with simultaneous early institution of hemodialysis (even in cases of hitherto intact renal function). As fomepizole is currently not available in the Czech republic, ethanol remains the only antidote. Therapy further includes correction of metabolic acidosis by way of bicarbonates, and intravenous application of thiamine and pyridoxine. Therapy of such a grave condition must entail infirmary care provided by professional nurses as a necessary complement. The nurse serves their function in securing the patient's airways, connecting the patient to mechanical ventilation, monitoring physiological functions, assistance with invasive entry, as well as conducting sample extractions, assuring proper hygiene, the prevention of decubitus ulcers etc. It has been the aim of an extensive effort during my analysis of this topic to collate an up-to-date summary of practical knowledge on ethylene glycol intoxication, as well as subsequent therapeutic options for such patients, based on collected information and available source material. This final thesis, entitled "Approach and Options for Nurses in Therapy of Ethylene Glycol-intoxicated Patients" has been created relying on a body of purely theoretical Czech and foreign sources. To attain awareness of the current state of medical sciences described on the following pages, many professional and scientific publications, books as well as journals and websites of registered professional societies had to be studied. The aim of a detailed inspection of such an extensive body of data has been to provide relevant findings based on the facts determined. The accumulated information is further specified in particular sections. The first deal with intoxication, elimination and first aid in the pre-hospitalization phase from a broader perspective. Without these elementary notes the thesis as a whole would lack in conclusiveness. From this section the thesis fluidly continues with ethylene glycol intoxication itself, its treatment and all the way to the role of the nurse including particular therapeutic measures. These sections contain specific recommendations issuing from news and cutting-edge research on ethylene glycol poisonings.

Postoje a možnosti sester při péči o pacienty s intoxikací metanolem / Attitudes and options of nurses in the care of methanol poisoned patients (theoretical thesis)

DVOULETÁ, Zuzana January 2016 (has links)
Methanol poisoning usually comes to the forefront of both professional and general public interest at times, when there is a larger outbreak, outside of these periods these cases appear sporadically. They occur in the whole world, in the developed countries as well as in the developing ones, independently of any restriction on the consumption of alcoholic beverages by the state. In Czech society this topic came to surface especially in 2012, when as a result of methanol intoxication the minimum of 50 people died and many more suffer permanent damage. The trickiness of methanol lies in the fact, that it is not possible to recognize it from ethanol in an alcoholic drink or in any other mixture. Most often it penetrates the organism by way of ingestion; it can also be absorbed through the skin or by inhalation. Methanol itself is not toxic; its metabolites (formaldehyde and formic acid) however present a problem. Typical symptoms include visual disturbances, deepening metabolic acidosis with an increased anion gap and damage to basal ganglia, particularly the putamen. The intoxication is further accompanied by a range of nonspecific symptoms such as stomach pain, nausea, vomiting, impairment of consciousness and cognitive functions. The gravity of the intoxication depends on the amount of ingested methanol and on the mixing ratio with ethanol. Therapy consists of administering specific antidotes ethanol or fomepizole, modifying of the internal environment, continuous or intermittent hemodialysis, if necessary and allowed by the patient's condition and in supportive treatment for maintaining the stability of vital functions. Nursing care has an unreplaceable role in treatment of these patients. Nurses come to contact with these patients in every sector of healthcare, they take part in admission, stabilization and monitoring of vital functions, they tend to the invasions as well as to maximal psychic and physical comfort of the patient. They take part in rehabilitation and also help patients returning into normal life. This thesis deals with the properties of methanol, its production and toxicity, it explores methanol intoxication, its symptoms and sequelae, diagnostics and therapy as well as nursing care in detail. It follows the role of nurses of all the sectors of health care in managing all the stages of the intoxication with emphasis on selected nursing activities in the intensive, standard or aftercare. The thesis concludes with 4 documented case studies. The aim of this thesis was to process the information collected on this topic into a complex and current overview of knowledge on methanol poisoning and the possibilities of dealing with it all of the phases of the poisoning including the aftercare of the patients who were affected. A summary of procedures and variations thereof was acquired on the theoretical basis of Czech and foreign sources such as books, renowned periodicals in paper or electronic form and by means of web sites of professional societies. The sum of knowledge this thesis provides can serve for teaching needsor continuing education of healthcare workers concerning diagnostics, therapy and management of the sequelae of this intoxication with special emphasis on the possibilities of nurse´s intervention in any of the phases of it, or to provide base for further research in this area.

Role sestry v edukaci pacienta s peritoneální dialýzou / The nurse's role in patient education peritoneal dialysis.

JANSOVÁ, Miroslava January 2016 (has links)
Peritoneal dialysis is one of the possibilities of the chronic renal failure treatment. Its advantage is that the patient is treated alone in his home environment. The main indication of this method is the inability to secure vascular access for hemodialysis. Contraindications include damage to the peritoneum and adhesions in the abdominal cavity. The principle of the method is the exchange of solutes between blood and dialysis solution filled into the peritoneal cavity. Timely and proper education by the nurse is the most important precondition for successful treatment with peritoneal dialysis. The thesis is focused on the role of nurses in the education of patients with peritoneal dialysis. It deals mainly with issues of patient education in connection with the occurrence of complications during their treatment. The first section describes the problems of peritoneal dialysis, including technical aspects, complications and their treatment. Furthermore, the attention is focused on the automated peritoneal dialysis, which takes place mostly at night and therefore the patient has more time to care about his normal daily activities. The second section of the theoretical part is devoted to education. The first target of the research was the examination of the effect of educational activities on the occurence of complications. Before starting treatment, patients have always been educated about the issues of this kond of treatment. The informative materials about the treatment and its related complications were also provided. Some of them did look up the information on the internet, and they discussed it further with the nurse. All of the respondents were able to name and describe the symptoms of complications occurring at the beginning and during the therapy. The second goal was to compare awareness among patients with frequent complications compared to those with the least complications. We did not find any benchmark to determine, whether the inconvenience was caused y the degree of unawareness of the patients, or their attitude to the treatment. The discussion will reflect on the question, which errors did subsequently led to the complications. The third objective was to compare the incidence of complications on patients with APD compared to patients on CAPD. With CAPD, the patient opens and closes the transfer set at least 4 times a day, thereby increasing the risk of infection through the peritoneal catheter. The research shows that among respondents undergoing the APD treatment method, the number of complications is lower. It is not possible to determine, whether the numer of complications on APD is lower due to minimal use of the peritoneal catheter, or because some patients on CAPD have already suffered the complications and have been instructed to approach the APD more responsibly. The empirical part of the thesis is presented by a qualitative research. The survey was carried out in the form of non-standardized interview based on prepared questions, which were extended during interviews. The research group consists of ten patients from dialysis clinics in Tabor, Ceske Budejovice and Jindrichuv Hradec. The patients on CAPD and APD were of different ages and with different duration of treatment. Peritoneal dialysis is the elimination method of replacing the kidneys function, and it is performed by the patient himself, in their own social environment. A prerequisite for successful treatment is high-quality patient education, which is mainly provided by the nurse. This introductory education continues with ongoing reeducation. The results of this thesis may be used as the basis for further research. They can also be used as study material for students of nursing fields. Created brochure can help the patients to decide, how to select the treatment method, or mastering the problems of this treatment.

Zajištění bezpečí pacienta v pooperační péči. / Securing the patient in postoperative care.

JORDÁNOVÁ, Hana January 2016 (has links)
Current status: Safe process treatment should be provided to every patient in every hospital. Nurses should work to avoid security violation, especially in the surgical field, associated with irreversible changes in the treatment process. Perfect mastery of these measures assures patient safety. Risk prevention, safety of the patient and the medical personnel require the involvement of all members of the multidisciplinary team (Pokojová, 2011; Škrla and Škrlová, 2003). Objective of the research: to map the attributes of postoperative care in the safe treatment of patients. To determine whether nurses master preventive measures in safe patient care and whether they ensure compliance with the principles of safe care in practice. Methods: the research analysis was chosen to be based on qualitative research method. Data acquisition was performed in semi-structured interview with nurses, supplemented with hidden observation. The participation in the interviews was voluntary and anonymous. Audio data was recorded from the interviews and transcribed verbatim. The observation aimed to determine whether nurses respect the principles of safe care for patients in the postoperative care. Research cohort: consisted of general nurses who worked at the surgical wards and recovery room. The research was conducted in one of the hospitals of South Bohemia. Twelve nurses were interviewed. The interview and hidden observation were focused on patient safety in the postoperative period. The interviews were performed at nurse day-time rooms of individual surgical wards, some of them outside the hospital. All nurses met the criterion to be a provider of nursing care for post-operative patients at surgical wards and recovery rooms. The interviews were finished when reaching the theoretical saturation, when the answers and hidden observation of the respondents started to repeat the already acquired data, i.e. after twelve interviews and observations. Results: The nurses working in surgical wards and recovery rooms are exposed to a considerable mental and physical strain. This profession is very demanding and requires not only theoretical knowledge, but also manual dexterity in practice. Nurses are involved in the care process in a considerable extent. Therefore, it is important that the care was good and safe. In the treatment process, the nurses sometimes do not comply with relevant standards and the requirements for safe treatment. It is essential to follow the identification bracelets to avoid patient mismatch, thus endangering patients' health. Some nurses checked the identification bracelets thoroughly, but there were also nurses who ignored these rules too much and gamble with the patients' health. Medication errors are the most common cause of patient injury. The risk of medication errors increases also due to improper storage of hazardous drugs. In our investigation, the medication was repeatedly stored at a different place (not returned back to the original area), poorly stored or left freely available to anyone. If the physician records are illegible, the nurses solve the issue rather by guessing or asking an assistance from colleagues in the department. This area also includes replacement of medication by nurses if the prescribed product is not available at the ward, without consulting a physician first. The negligence in the use of protective equipment during dressing changes is alarming. Possible transmission of nosocomial infections. Poor or little use of disinfectants. Conclusions: Treatment of patients in the early and late postoperative care is one of the high risk parts of the hospitalization of patients at healthcare facilities. In this period, patient monitoring and risk prevention are essential. It includes pain control, the care for surgical wounds, safe patient identification, prevention of adverse events in both medication administration and patient falls, the compliance with hand hygiene ...

Dotyk v ošetřovatelství - terapeutická nebo riziková intervence / Touch in nursing-therapeutical or risk intervention

HYNKOVÁ, Miroslava January 2017 (has links)
A touch is an inseparable part of the mutual contact between a nurse and a patient. The more a patient depends on the assistance of the others in common daily activities, the more a nurse uses physical contact in the care, not only in relation to the professional ortherapeutic acts, but also to the expression of compassion, sympathy, encouragement or calming a patient.Despite the fact that a touch is so important in patient care, it only receives a marginal attention. The goal of the thesis was to examine the reasons for and the obstacles to the application of a touch in nursing care, and if a touch belongs to the present nursing care.Research questions were set to achieve the goals:How do nurses perceive a touch in the present nursing care? How do patients perceive a nurse's touch in the treatment?Three hypotheses were formulated in dependence on the above set goals:H1:Nurses perceive a touch more as a part of the direct (physical)care than a therapeutic intervention.H2:Nurses rather use a touch as an unintentional method than as an intentional therapeutic intervention.H3:Nurses use a touch rather intuitively, not realizing the risk that a touch can be perceived negatively. The theoretical part of the thesis was elaborated on the base of literature sources. The empiric part was implemented on the base of a qualitative and quantitative research. The data for the qualitative research was gained by means of semi-structured interviews with ten patients.A questionnaire research for general nurses was used for the data collection in the quantitative research. On the base of the results the hypotheses were evaluated as follows:H1 Nurses perceive a touch more as a part of the direct (physical) care than a therapeutic intervention unconfirmed.H2:Nurses rather use a touch as an unintentional method than as an intentional therapeutic intervention unconfirmed.H3:Nurses use a touch rather intuitively, not realizing the risk that a touch can be perceived negatively unconfirmed. It is obvious from the results we achieved in the research that a touch has been and will always be an inseparable part of the mutual relation between a nurse and a patient. The research has shown that nurses perceive a touch as a part of the direct care as well as a therapeutic intervention. And if nurses use a touch as a therapeutic intervention, they do it intentionally, mostly to build confidence, to calm a patient, to create the feeling of safety, certainty and understanding.More than a half of the nurses replied that they used a touch instinctively, not realizing that they are touching the patient, on the other hand, more than a half of the nurses replied that they cared about at what points a patient perceived a touch negatively.Although the nurses replied that a therapeutic touch was not time consuming, it is not applied frequently in the therapeutic care. The research has also shown that patients accept a touch within the care and they accept it resignedly. When nurses touch patients, the patients are glad if such a touch is accompanied by words. The interviews revealed that a nurse should not underestimate a handshake at the first contact, as patients perceive this act as a feeling of interest and creation of confidence. The research shows indisputably that a touch is a very important holistic nursing interventionin patient care. It has confirmed that although a touch and its perception is something so intimate, individual and indispensable in nursing care, it only receives a marginal attention. This thesis might serve as study material for numerous nurses, so that a touch is used as an effective intervention by a nurse that perceives a patient as a holistic being and has a sympathy for him/her in his/her uneasy life situation. This thesis might initialize a more extended research combining more research methods. An application of hidden observation for deeper understanding of the use of a touch in nursing care might be particularly interesting.

A transversalidade (in)visível da temática do cuidado na formação da enfermeira / La transversalidad (in)visible de la temática del cuidado en la formación de la enfermera / The (in)visible transversality of care in nursing education

Biasi, Luciana Spinato de January 2008 (has links)
This study, which has a descriptive, exploratory and qualitative feature, aimed to know the learning process of care under the view of the students from the first and the final terms of a Nursing Graduation Course. The study was accomplished at Universidade Regional Integrada do Alto Uruguai e das Missões – URI – Campus de Erechim, from April/2007 to June/2007. For data collection, the technique of focal group was used. The data were analysed through the thematic analysis of the content and evidenced two themes and six thematic-categories, which are: First Impressions about the Education Process; The view of Nursing; Expectancies with the education: ways of caring; Expectancies regarding the profession; Reflection about the education; The Nurse's view: the Knowledge about the profession; The process of education: the learning of care and expectancies with the end of the course. The findings allowed to infer that the students, somehow, state that the theme of Care can be visualized during the process of education. Through the statements, it could be noticed that the transversality of the theme occurs implicitly during the educational process, which leads us to infer that in the Nursing course under study there is a concern in assuring linearity of the content regarding care, which was evidenced by the student's perception. / Salvador

A model for intergrating spiritual nursing care in nursing practice : a Christian perspective

Monareng, L.V. 11 1900 (has links)
A qualitative, grounded theory study was undertaken to explore and describe how nurses conceptualise spiritual nursing care, and how they integrate spiritual nursing care in practice. An in-depth literature review through concept analysis on the phenomenon was conducted to assist the researcher with theoretical sensitivity and theoretical saturation. In-depth Individual interviews and focus group interviews were conducted to generate data. Interviews were audio-taped and transcribed by the researcher verbatim. Symbolic Interactionism was the philosophical base for the study. Data analysis was done through the use of the NUD*Ist computer soft ware programme version 4.0. The direct quotes of participants were coded and arranged into meaning units for analysis. A constant comparison method of data analysis was applied by following a process of open, axial and selective coding. Tech’s (1990:142-145) eight steps of analysis to analyse textual qualitative data was used until themes, categories and subcategories were identified and developed. Data analysis revealed that nurses had difficulty to differentiate spiritual nursing care from emotional, psychological or religious care. Nurses still felt inadequately prepared educationally on how to integrate spiritual nursing care in nursing practice. A Humane Care Model and practice guidelines were developed to guide nurses in clinical practice on how to provide such care. Recommendations proposed that the matter be taken up by nurse managers, educators and nurse clinicians to guide nurses in this regard. / Health Studies / D. Litt. et Phil. (Health Studies)

The quality of professional practice by registered nurses and midwives in central hospitals in Malawi

Lengu, Edoly Shirley 06 1900 (has links)
The purpose of this study was to evaluate the quality of professional practice by the registered nurses midwives as reflected in clinical nursing care records for postlaparotomy patients in public central hospitals in Malawi. The set process standards by Nurses and Midwives Council of Malawi (NMCM) were addressed in relation to clinical care of post-laparotomy patients. The hypothesis for this study was that the quality of professional practice by the registered nurse midwives as reflected in clinical nursing care of post-laparotomy patients in public central hospitals in Malawi is inadequate and non-compliant with the process standards set by the NMCM. The researcher used quantitative, evaluative, descriptive, contextual survey and participatory observations to collect data. A three-point rating scale consisting of compliance (C) = 1, partial compliance (PC) = 0.5 and non-compliance (NC) = 0.0 was used to evaluate the state registered nurse midwives’ compliance with process standards. The results showed partial compliance with the set NMCM process standards by the state registered nurse midwives in public central hospitals in Malawi. / Health Studies / M.A. (Health Studies)
