OCT (Optical Coherense Tomography) : Teknik och tillämpning

Before year 1895, the doctors could only make a probable diagnosis based on what the patient could tell and it was hurt and there was no discernable change to the outside of the body. With X-ray, it was possible to see inside the patient without first cutting it, you can say that the X-ray was the starting point for diagnostic imaging.The further development of X-ray gave CT (Computed Tomography), where X-ray tubes and detectors rotate around the patient while the patient table moves. Besides CT also developed MRI (Magnetic Resonance Imaging), PET (Positron Emission Tomography) and Ultrasound. Common to these methods is that the produced 3D images.In 1990 a completely new approach for diagnostic imaging, OCT (optical coherence tomography), by measuring the phase shift and the intensity of reflected light, it provides real-time and non-destructive measurements (in vivo) a resolution of 1 to 15 microns, much higher than all other standard imaging techniques. You could say that OCT machine can be compared to ultrasound, which uses the reflection of sound waves to interpretation.The first OCT machines were of type TD (Time Domain), these had low resolution and low scanning speed. In 2005 came the SD-OCT, they had higher resolution and scanning speed, SD stands for spectral domain, SD-OCT is sometimes called FD-OCT as Fourier transformed signals and operating in the frequency domain.The development of OCT machines are only in their infancy, resolution, scanning speed and accuracy will increase all the time, this allows new uses and ways to diagnose developed. OCT can be used in such Oncology, MSD (Musculoskeletal disorders), cardiovascular medicine, teeth, nerves, but the largest field is the eye and then the back of the eye called the retina (retina).This thesis is limited to the eye, the purpose is to provide input to those who are likely to purchase an OCT-machine, but also show the measurement data OCT-machines are performing and how to use the OCT-machine more than to see age-related macular degeneration. Another aim is to increase understanding of the physics behind an OCT-machine for ease of understanding the output given.The manufacture/model that have selected for evaluation are Zeiss Cirrus 4000, Topcon 3D OCT-2000 and Heidelberg Spectralis, the reason is that there are only these three on the Swedish market and all are SD-OCT. The way to evaluate OCT-machines is to scan performance and what the various analysis programs can handle. Furthermore, each OCT-machine scans the macula and optic disk on a experimental person/ reference eye, in order to get the output of the precision, or if you want to call it repeatability, which is very important if one wants to follow a solitary disease course.The conclusion of this thesis is to OCT machines are quite similar. When it comes to ease of use when doing scans is the Cirrus is lightened by the use of the extra screen where you always look eye (iris camera), which makes it easy to adjust the sharpness and position of the mouse buttons. Topcon and Heidelberg is not difficult to use but requires more experience of the person making the OCT scans. Most measurement functions in the analysis program is Topcon and Heidelberg and best accuracy/repeatability is Heidelberg, both the macula and RFNL.OCT machine is a good tool to use on the anterior segment, but in the case the precision allows the precision used to monitor RNFL thickness changes in those with glaucoma. / <p>Validerat; 20131029 (global_studentproject_submitter)</p>

Les pyréthrinoïdes et les organophosphorés sont des pesticides très utilisés, à l’origine d’une imprégnation forte de la population, exposée à ces contaminants principalement via l’alimentation. De plus en plus d’études scientifiques suggèrent des liens entre l’exposition à ces composés et des maladies chroniques ou des troubles du développement de l’enfant. Paradoxalement, leur devenir biologique chez l’homme est mal connu. Certaines études suggèrent que ces insecticides sont susceptibles d’intéragir avec les transporteurs membranaires ABC et SLC, protéines localisées au niveau d’interfaces hémato-tissulaires qui prennent en charge de nombreux substrats endogènes, médicaments et contaminants de l’environnement. L’objectif de notre étude a été de caractériser les effets d’insecticides des familles des pyréthrinoïdes et des organophosphorés sur l’activité de nombreux transporteurs ABC et SLC prenant en charge des médicaments (P-gp, BCRP, MRPs, OATP-1B1,-2B1,-1B3, OCT1-3, OAT1, OAT3, MATE1 et MATE2K) par une approche in vitro. Nous nous sommes également attachés à caractériser par des expérimentations in vitro et in silico, les mécanismes des interactions et les éléments structuraux des pesticides à l’origine de ces effets. Nous avons montré que de nombreux organophosphorés et pyréthrinoïdes étaient capables d’inhiber des transporteurs d’efflux (MRP, BCRP, P-gp) et d’influx (OATP1B1, OAT3, MATE1, OCT1-2) et de stimuler l’activité de certains OATPs. Les pesticides testés inhibaient très fortement l’activité des transporteurs de cations (OCT1 et OCT2) et ont pu bloquer le transport de catécholamines médiés par ces protéines. Une approche qSAR a permis de définir des paramètres physicochimiques associés aux effets modulateurs des pesticides et une approche d’amarrage moléculaire (docking) a mise en évidence les sites de liaisons de la P-gp impliquées dans ces interactions. Les conséquences des modulations de l’activité des transporteurs, en termes d’effets toxiques et d’interactions médicamenteuses, restent à définir pour les populations exposées à de fortes doses de pesticides. Toutefois, la contribution des interactions observées aux effets toxiques de ces insecticides est peu probable car nécessitant des concentrations nettement supérieures à celles atteintes dans le cadre d’une exposition environnementale de la population générale. / The general population is chronically exposed to pyrethroids and organophosphorus insecticides, mainly through alimentation. Several epidemiological studies have found an association between non-occupational exposure to these pesticides and chronic diseases and developmental disorders. Paradoxically, their biological fate in humans is poorly understood. Some studies suggest that these insecticides could interact with ABC and SLC membrane transporters. These membrane proteins, located at blood-tissue interfaces (liver, kidney, intestine ...), handle many endogenous substrates, drugs and pollutants. The objective of our study was to characterize, using an in vitro approach, the effects of pyrethroid and organophosphorus insecticides on the activity of numerous ABC and SLC human drug-transporters (P-gp, BCRP, MRPs, OATP-1B1, -2B1, -1B3, OCT1-3, OAT1, OAT3, MATE1 and MATE2K). We have also tried to analyze the mechanisms of interactions and the structural requirements for insecticides-mediated modulation of drug transporters activities using in vitro and in silico approach. We have shown that many organophosphorus and pyrethroids are able to inhibit ABC (MRP, BCRP, P-gp) and SLC (OATP1B1, OAT3, MATE1, OCT1-2) transporters and can stimulate the activity of some OATPs. Moreover, the tested pesticides inhibited very strongly the activity of OCT1 and OCT2 and blocked catecholamine transport mediated by these transporters. A qSAR approach allowed to define physicochemical parameters associated with the modulating effects of pesticides and a molecular docking approach revealed the P-gp binding sites involved in these interactions. The consequences of transporter activitie modulation, in terms of toxic effects and drug interactions, remain to be defined for populations exposed to high doses of pesticides, occurring notably in response to poisoning. However the alterations of these transporter activities by insecticides are unlikely to contribute to organophosphorus or pyrethroids toxicities of chronic low-dose exposure.

In this work is describe anatomy and physiology of the cornea. The following are the primary non-inflamatory degeneration of the cornea. Then describe the physical principles diagnostic devices for cornea – keratometer, pachymeter, Michelson interferometr and optical coherence tomography (OCT). At the end of the theoretical introduction is describes the principle of laser correction surgery – LASIK. The practical part is divided into two main objectives. The first task is propose an algorithm for automatic detection of corneal surface and then calculation of corneal thickness and size of the chamber angle in Matlab. The aim of the second task is image flap analysis for boundary detection.

Manifestações oftalmológicas e neurológicas em portadores pré-sintomáticos e sintomáticos de ataxia espinocerebelar tipo 7

Introdução: a ataxia espinocerebelar tipo 7 (SCA7) é um distúrbio neurodegenerativo autossômico dominante causado por uma repetição CAG expandida (CAGexp) no gene ATXN7, resultando na inserção de uma poliglutamina (poliQ) alongada na proteína ataxina-7. Em consequência, pacientes com SCA7 desenvolvem ataxia, espasticidade e outros sintomas neurológicos. A SCA 7 se destaca de outras SCAs por se associar à distrofia retiniana, causando deficiências visuais que podem levar à cegueira. Sendo uma das mais raras SCAs, pequenas séries de casos têm aparecido na literatura. Poucas delas buscaram correlacionar os achados neurológicos com os oftalmológicos; e a fase pré-clínica jamais foi sistematicamente investigada. Objetivo: descrever os achados neurológicos e oftalmológicos de uma coorte de casos de SCA 7, comparando as manifestações encontradas em sujeitos sintomáticos com as encontradas em portadores assintomáticos e em parentes não portadores, em uma abordagem exploratória que buscou levantar potenciais biomarcadores de progressão da doença. Métodos: trata-se de um estudo transversal onde pacientes com diagnóstico molecular de SCA7 realizado na nossa instituição foram identificados em nossos arquivos protegidos. Tanto eles como seus parentes foram convidados a participar da presente investigação. Sujeitos em risco de 50% foram incluídos se tivessem mais de 18 anos. Após o consentimento, dados clínicos e demográficos foram coletados entre junho de 2016 e setembro de 2017. A seguir, todos os participantes realizaram uma bateria de escalas clínicas voltadas à medida da ataxia (SARA, CCFS, PATA e 8 MW) e das manifestações neurológicas (NESSCA e INAS); um questionário de qualidade de vida relacionada à visão (NEI-VFQ 25); avaliação da acuidade visual melhor corrigida (AVMC), desvio médio em campimetria computadorizada (MD) e espessuras da mácula e da camada de células ganglionares na tomografia de coerência óptica (OCT). A escala SARA e a AVMC foram escolhidas como as variáveis de referência para a gravidade dos quadros. A análise molecular do ATXN7 foi feita, mas participantes do estudo e avaliadores foram mantidos cegos para seus resultados; os indivíduos em risco interessados em receber seus resultados foram enviados para o programa de testes pré-sintomáticos. Como não houve critérios a priori para estimar tamanhos de efeito e como a SCA7 é uma condição rara, não houve como decidir um tamanho de amostra. O estudo foi exploratório e por isso não foram feitas correções para múltiplas testagens. Um p de 0,05 foi eleito para definir significância, e testes estatísticos foram aplicados de acordo com as características das variáveis em estudo. Resultados: 12 portadores sintomáticos (grupo 2) e 8 indivíduos em risco (3 portadores - grupo 1 - e 5 não-portadores - grupo 0) foram incluídos neste estudo. Todas as variáveis contínuas à exceção da CAGexp tiveram distribuição 4 normal. A AVMC estava reduzida em todos os participantes sintomáticos e claramente diferente entre estes e os outros dois grupos (p <0,0001, ANOVA), enquanto os portadores assintomáticos e os não portadores tiveram resultados semelhantes. A AVMC média foi 20/143, 20/18 e 20/20 nos grupos 2, 1 e 0, respectivamente. Não surpreendentemente, o NEI-VFQ 25 também demonstrou uma diferença estatisticamente significativa, mas o que foi inesperado foi a forma progressivamente diferente entre os 3 grupos (grupo 0 = 92,76 ± 6,7; grupo 1 = 74,9 ± 55,5; grupo 2 = 58,0 ± 21,3) (p= 0,012, ANOVA com Tukey) O MD mostrou um padrão linear estatisticamente significativo para piorar do grupo controle (-1,34 ± 1,15dB) para o assintomático (-2,81 ± 1,66dB) e do grupo assintomático para sintomático (-10,54 ± 6,95dB) (p = 0,027, ANOVA com Tukey). Além disso, o MD correlacionou-se com a AVMC (p = 0,020; r = 0,660) e apresentou tendência de correlação com a SARA (p= 0,073; r= -0,535). As medidas de espessura macular distinguem completamente os 3 grupos (grupo 0 = 243,6 ± 22,2 μ; grupo 1 = 204,5 ± 14,1 μ; grupo 2 = 137,95 ± 34,6 μ) (p = 0,0001, ANOVA) e também se correlacionou significativamente com os dois critérios planejados de gravidade, SARA (p = 0,050; r = -0,577) e AVMC (p = 0,007; r = 0,730). Discussão: alterações oftalmológicas estavam presentes já nas fases pré-clínicas da doença, quando os escores obtidos das escalas neurológicas ainda não distinguem portadores assintomáticos de não portadores: a espessura macular medida por OCT e o MD medido pela campimetria computadorizada. Esses achados demonstram que o processo neurodegenerativo já se encontra em curso e é detectável por essas medidas anatômicas e funcionais da retina. Além disso, ambas as alterações detectadas em fases pré-clínicas, ao serem estudadas no grupo total de portadores sintomáticos e assintomáticos, se correlacionaram com os nossos padrões-ouro da gravidade da doença, SARA e AVMC. Os dois achados - início em fase pré-clínica e correlação com a progressão da doença medida por escores independentes - sugerem que a espessura macular medida por OCT e o MD medido pela campimetria computadorizada são potenciais candidatos a biomarcadores de estado (de progressão da doença) desde fases pré-manifestas na SCA7. / Background: spinocerebellar ataxia type 7 (SCA7) is an autosomal dominant neurodegenerative disorder caused by an expanded CAG repeat (CAGexp) at ATXN7 gene, resulting in the insertion of an elongated polyglutamine (polyQ) into the ataxin-7 protein. As a consequence, patients with SCA7 develop ataxia, spasticity and other neurological symptoms. SCA7 stands out from other SCAs by associating it with retinal dystrophy, causing visual deficiencies that can lead to blindness. Being one of the rarest SCAs, small series of cases appear in the literature. Few of them sought to correlate neurological findings with ophthalmologic findings; and the preclinical stage has never been systematically investigated. Objective: to describe the neurological and ophthalmological findings of a cohort of cases of SCA7, comparing the manifestations found in symptomatic subjects with those found in asymptomatic carriers and in non-carrier relatives in an approach exploratory study that sought to raise potential biomarkers of disease progression. Methods: patients with a molecular diagnosis of SCA7 performed at our institution were identified in our protected files. Both they and their relatives were invited to participate in the present investigation. Subjects at risk of 50% were included if they were older than 18 years. After consent, clinical and demographic data were collected between June 2016 and September 2017. All participants then performed a battery of clinical scales aimed at the measurement of ataxia (SARA, CCFS, PATA and 8 MW) and neurological manifestations (NESSCA and INAS); a visual function questionnaire (NEI-VFQ 25); assessment of better corrected visual acuity (AVMC), mean deviation in computerized campimetry (MD), and thickness of the macula and ganglion cell layer on OCT. The SARA and AVMC scale were chosen as the reference variables for the severity of the frames. Molecular analysis of ATXN7 was done, but study participants and evaluators were kept blind to their results; the individuals at risk interested in receiving their results were sent to the presymptomatic testing program. As there were no a priori criteria for estimating effect sizes and because SCA7 is a rare condition, there was no way to decide on a sample size. The study was exploratory and therefore no corrections were made for multiple tests. A p of 0.05 was chosen to define significance, and statistical tests were applied according to the characteristics of the variables under study. Results: 12 symptomatic carriers (group 2) and 8 individuals at risk (5 carriers - group 1 - and 3 non-carriers - group 0) were included in this study between June 2016 and September 2017. All continuous variables with the exception of CAGexp had normal distribution. AVMC was reduced in all symptomatic participants and clearly different between these and the other two groups (p <0.0001, ANOVA), while asymptomatic and non-carriers had similar results. The mean BCVA was 20/143, 20/18 and 20/20 in groups 2,1 and 6 0, respectively. Not surprisingly, NEI-VFQ 25 also showed a statistically significant difference, but what was unexpected was the progressively different form between the 3 groups (group 0 = 92.76 ± 6.7, group 1 = 74.9 ± 55, 5, group 2 = 58.0 ± 21.3) (p = 0.012, ANOVA with Tukey). The MD showed a statistically significant linear pattern to worsen from the control group (-1.34 ± 1.15dB) to the asymptomatic (-2.81 ± 1.66dB) and from the asymptomatic to the symptomatic group (-10.54 ± 6, 95dB) (p = 0.027, ANOVA with Tukey). In addition, MD correlated with AVMC (p = 0.020; r = 0.660) and showed a correlation tendency with ARDS (p = 0.073; r = -0.535). The macular thickness scores completely distinguish the 3 groups (group 0 = 243.6 ± 22.2 μ, group 1 = 204.5 ± 14.1 μ, group 2 = 137.95 ± 34.6 μ) (p = 0.0001, ANOVA ...) and also correlated significantly with the two planned criteria of severity, SARA (p = 0.050, r = -0.577) and AVMC (p = 0.007, r = 0.730). Conclusion: ophthalmologic changes were present already in the preclinical stages of the disease, when the scores obtained from the neurological scales did not yet distinguish asymptomatic non-carrier patients: macular thickness measured by OCT and MD measured by computerized campimetry. These findings demonstrate that the neurodegenerative process is already underway and is detectable by these anatomical and functional measures of the retina. In addition, both changes detected in preclinical stages, when studied in the total group of symptomatic and asymptomatic carriers, correlated with our gold standard of disease severity, SARA and AVMC. The two findings - pre-clinical onset and correlation with disease progression measured by independent scores - suggest that the macular thickness measured by OCT and MD as measured by computerized campimetry are potential candidates for disease biomarkers (disease progression) from pre-manifest stages in SCA7.

Etude de l'angioplastie guidée par tomographie en cohérence optique / Optical coherence tomography-guided angioplasty as a new tool to improve coronary evaluation and guide percutaneous coronary intervention procedures

L'imagerie par tomographie en cohérence optique (OCT) est prometteuse comme support de la prise de décision au cours des procédures d'interventions coronariennes percutanées (PCI), pou évaluer les lésions athéromateuses, juger de la bonne implantation du stent, et dépister les lésions vasculaires dues au stent. L'OCT représente donc bien une aide potentielle pour le cardiologue interventionnel tout au long de la procédure de stenting, avec un impact certain sur la stratégie interventionnelle initialement programmée. De plus, l'OCT se révèle comme un nouvel outil pour prédire la dissection des bords de stent chez les patient avec ACS sans élévation du segment ST, rendant possible une stratification des patients quant à ce risque. Des essais cliniques randomisés sont maintenant nécessaires pour savoir si l'assistance par l'OCT pendant la procédure améliore le pronostic à long terme des patients après PCI / Optical Coherence Tomography (OCT) imaging is promising in decision making during Percutaneus Coronary Interventions {PCI) procedures, including evaluating controversial plaque lesions, assessing stent implantation, and surveying stent-related vascular injury. Thus, OCT has potential to guide interventional cardiologists throughout the stent implantation procedure, impacting on planned interventional strategy. In addition, OCT is the most novel image technology to predict stent edge dissection for patients with non-ST-segment elevation ACS, enabling risk stratification of patients who are at a higher risk of this complication. Large-scale randomized trials are now warranted to assess whether OCT results and guidance during de procedure improve long-term clinical outcomes of PCis.

Contribution to the study of synchronized differential oscillators used to controm antenna arrays / Contribution à l'étude d'oscillateurs différentiels synchronisés appliqués à la commande d'un réseau d'antennes linéaire

Le travail présenté dans ce mémoire traite de l'étude d'oscillateurs et d'Oscillateurs Contrôlés en Tension (OCT) différentiels couplés appliqués à la commande d'un réseau d’antennes linéaire. Après avoir rappelé les concepts d'antennes réseaux et d'oscillateurs, une synthèse de la théorie élaborée par R. York et donnant les équations dynamiques modélisant deux oscillateurs de Van der Pol couplés par un circuit résonnant a été présentée. Après avoir montré la limitation de cette approche concernant la prédiction de l'amplitude des oscillateurs, une nouvelle formulation des équations non linéaires décrivant les états de synchronisation a été proposée. Néanmoins, compte tenu du caractère trigonométrique et fortement non linéaire de ces équations, une nouvelle écriture facilitant la résolution numérique a été proposée. Ceci a permis l'élaboration d'un outil de Conception Assistée par Ordinateur (CAO) fournissant une cartographie de la zone de synchronisation de deux oscillateurs de Van der Pol couplés. Celle-ci permet de déterminer rapidement les fréquences d'oscillation libres nécessaires à l'obtention du déphasage souhaité. Pour ce faire, une procédure de modélisation de deux oscillateurs et OCTs différentiels couplés, par deux oscillateurs de Van der Pol couplés par une résistance a été élaborée. Les résultats fournis par l'outil de CAO proposé ont ensuite été comparés avec les résultats de simulations de deux oscillateurs et OCTs différentiels couplés obtenus avec le logiciel ADS d'Agilent. Une très bonne concordance des résultats a alors été obtenue montrant ainsi l'utilité et la précision de l'outil présenté. / The work presented in this thesis deals with the study of coupled differential oscillators and Voltage Controlled Oscillators (VCO) used to control antenna arrays. After reminding the concept of antenna arrays and oscillators, an overview of R. York's theory giving the dynamics for two Van der Pol oscillators coupled through a resonant network was presented. Then, showing the limitation of this approach regarding the prediction of the oscillators' amplitudes, a new formulation of the nonlinear equations describing the oscillators' locked states was proposed. Nevertheless, due to the trigonometric and strongly non-linear aspect of these equations, mathematical manipulations were applied in order to obtain a new system easier to solve numerically. This has allowed to the elaboration of a Computer Aided Design (CAD) tool, which provides a cartography giving the frequency locking region of two coupled differential Van der Pol oscillators. This cartography can help the designer to rapidly find the free-running frequencies of the two outermost differential oscillators or VCOs of the array required to achieve the desired phase shift. To do so, a modeling procedure of two coupled differential oscillators and VCOs as two coupled differential Van der Pol oscillators, with a resistive coupling network was performed. Then, in order to validate the results provided by our CAD tool, we compared them to the simulation results of two coupled differential oscillators and VCOs obtained with Agilent’s ADS software. Good agreements between the simulations of the circuits, the models and the theoretical results from our CAD tool were found.

Tanto eles como seus parentes foram convidados a participar da presente investigação. Sujeitos em risco de 50% foram incluídos se tivessem mais de 18 anos. Após o consentimento, dados clínicos e demográficos foram coletados entre junho de 2016 e setembro de 2017. A seguir, todos os participantes realizaram uma bateria de escalas clínicas voltadas à medida da ataxia (SARA, CCFS, PATA e 8 MW) e das manifestações neurológicas (NESSCA e INAS); um questionário de qualidade de vida relacionada à visão (NEI-VFQ 25); avaliação da acuidade visual melhor corrigida (AVMC), desvio médio em campimetria computadorizada (MD) e espessuras da mácula e da camada de células ganglionares na tomografia de coerência óptica (OCT). A escala SARA e a AVMC foram escolhidas como as variáveis de referência para a gravidade dos quadros. A análise molecular do ATXN7 foi feita, mas participantes do estudo e avaliadores foram mantidos cegos para seus resultados; os indivíduos em risco interessados em receber seus resultados foram enviados para o programa de testes pré-sintomáticos. Como não houve critérios a priori para estimar tamanhos de efeito e como a SCA7 é uma condição rara, não houve como decidir um tamanho de amostra. O estudo foi exploratório e por isso não foram feitas correções para múltiplas testagens. Um p de 0,05 foi eleito para definir significância, e testes estatísticos foram aplicados de acordo com as características das variáveis em estudo. Resultados: 12 portadores sintomáticos (grupo 2) e 8 indivíduos em risco (3 portadores - grupo 1 - e 5 não-portadores - grupo 0) foram incluídos neste estudo. Todas as variáveis contínuas à exceção da CAGexp tiveram distribuição 4 normal. A AVMC estava reduzida em todos os participantes sintomáticos e claramente diferente entre estes e os outros dois grupos (p <0,0001, ANOVA), enquanto os portadores assintomáticos e os não portadores tiveram resultados semelhantes. A AVMC média foi 20/143, 20/18 e 20/20 nos grupos 2, 1 e 0, respectivamente. Não surpreendentemente, o NEI-VFQ 25 também demonstrou uma diferença estatisticamente significativa, mas o que foi inesperado foi a forma progressivamente diferente entre os 3 grupos (grupo 0 = 92,76 ± 6,7; grupo 1 = 74,9 ± 55,5; grupo 2 = 58,0 ± 21,3) (p= 0,012, ANOVA com Tukey) O MD mostrou um padrão linear estatisticamente significativo para piorar do grupo controle (-1,34 ± 1,15dB) para o assintomático (-2,81 ± 1,66dB) e do grupo assintomático para sintomático (-10,54 ± 6,95dB) (p = 0,027, ANOVA com Tukey). Além disso, o MD correlacionou-se com a AVMC (p = 0,020; r = 0,660) e apresentou tendência de correlação com a SARA (p= 0,073; r= -0,535). As medidas de espessura macular distinguem completamente os 3 grupos (grupo 0 = 243,6 ± 22,2 μ; grupo 1 = 204,5 ± 14,1 μ; grupo 2 = 137,95 ± 34,6 μ) (p = 0,0001, ANOVA) e também se correlacionou significativamente com os dois critérios planejados de gravidade, SARA (p = 0,050; r = -0,577) e AVMC (p = 0,007; r = 0,730). Discussão: alterações oftalmológicas estavam presentes já nas fases pré-clínicas da doença, quando os escores obtidos das escalas neurológicas ainda não distinguem portadores assintomáticos de não portadores: a espessura macular medida por OCT e o MD medido pela campimetria computadorizada. Esses achados demonstram que o processo neurodegenerativo já se encontra em curso e é detectável por essas medidas anatômicas e funcionais da retina. Além disso, ambas as alterações detectadas em fases pré-clínicas, ao serem estudadas no grupo total de portadores sintomáticos e assintomáticos, se correlacionaram com os nossos padrões-ouro da gravidade da doença, SARA e AVMC. Manifestações oftalmológicas e neurológicas em portadores pré-sintomáticos e sintomáticos de ataxia espinocerebelar tipo 7

Introdução: a ataxia espinocerebelar tipo 7 (SCA7) é um distúrbio neurodegenerativo autossômico dominante causado por uma repetição CAG expandida (CAGexp) no gene ATXN7, resultando na inserção de uma poliglutamina (poliQ) alongada na proteína ataxina-7. Em consequência, pacientes com SCA7 desenvolvem ataxia, espasticidade e outros sintomas neurológicos. A SCA 7 se destaca de outras SCAs por se associar à distrofia retiniana, causando deficiências visuais que podem levar à cegueira. Sendo uma das mais raras SCAs, pequenas séries de casos têm aparecido na literatura. Poucas delas buscaram correlacionar os achados neurológicos com os oftalmológicos; e a fase pré-clínica jamais foi sistematicamente investigada. Objetivo: descrever os achados neurológicos e oftalmológicos de uma coorte de casos de SCA 7, comparando as manifestações encontradas em sujeitos sintomáticos com as encontradas em portadores assintomáticos e em parentes não portadores, em uma abordagem exploratória que buscou levantar potenciais biomarcadores de progressão da doença. Multimodální registrace retinálních snímků z fundus kamery a OCT / Multimodal Registration of Fundus Camera and OCT Retinal Images

Tato práce se zabývá multimodální registrací snímků sítnice z různých skenovacích zařízení. Multimodální registrace umožňuje zvýraznit prvky na snímcích sítnice, které jsou důležité pro detekci různých typů onemocnění oka (jako je glaukom, degradace nervových vláken, degradace cév, atd.). Teoretická část tvoří zhruba první půlku práce a je následována praktickou částí, která popisuje postupy při různých typech registrací snímků z fundus kamery, SLO a OCT. Registrace fundus a SLO snímků je provedena pomocí prostorové transformace. Tato práce popisuje tři různé metody registrace SLO snímků se snímky z fundus kamery. První a zároveň nejjednodušší je manuální registrace. Druhou je automatická registrace založená na metodě korelace. Výsledky, včetně porovnání obou metod, jsou uvedeny v závěru. Třetím typem je poloautomatická registrace, která využívá výhod obou předchozích metod a tím pádem je kompromisem mezi rychlostí a přesností registrace. Registrace fundus snímků a B-scanů z OCT je realizována dvěma různými metodami. První je opět založená na korelaci a druhá na prostorové transformaci. Všechny tyto registrační metody jsou realizovány také prakticky v programovém prostředí Matlab.
