No capítulo 2 intitulado: Periodontal and microbiologic evaluation of two methods of arckwire ligation: ligatures wires and elastomeric ríngs, estes dois métodos em questão foram comparadas, considerando aspectos periodontais e microbiológicos. Few studies have investigated orthodontic devices, such as elastomeric rings and steel ligatures. Therefore, the aim of this study was to evaluate the periodontal and microbiological alterations in patients fixed orthodontic appliances. Literature review (chapter 1) showed significant results concerning accumulation of biofilm and gingival inflammation in patients under fixed orthodontic treatment. The present study revealed a strong correlation between A. actinomycetemcomitans and subgingival biofilm of orthodontic bands. The second chapter {Periodontal and microbiologic evaluation of two methods of arckwire ligation: ligatures wires and elastomeric rings) shows a comparison between these two techniques, considering periodontal and microbiological aspects. This study evaluated 14 patients having no clinical signs of gingival inflammation. Biofilm samples of teeth 15, 25, 32 and 42 were collected before fixed appliance placement and again six months after treatment. Each orthodontic arc was fixed with elastomeric rings on one of the sides of the arches, while steel ligatures had been used on the opposite side. Plaque index, gingival bleeding and probing depth were used as periodontal parameter. A technique of Molecular Biology called polymerase in chain reaction (PCR) was used to detect P. gingivalis, T. forsythia, A. actinomycetemcomitans, P. intermedia, and P. nigrescens. Analysis of variance (p < 5%) revealed statistically significant differences for the elastomeric rings, which showed high scores for bleeding and plaque index, while McNemar test showed that T. forsythia and P. nigrescens demonstrafed a larger number of positive sites for the elastomeric rings group, when compared to steel ligatures group (p < 5%). In conclusion, elastomeric rings favored the presence of these 2 pathogens, and due the periodontal findings, should not be recommended in patients with deficient oral hygiene / Mestrado / Ortodontia / Mestre em Radiologia Odontológica

Autism appears in the first stages of development. Its features include social deficit, language deficits and behaviour alterations and, quite often, there is retardation of neuropsychomotor development. The oral health may be precarious within this population, as their hygiene habits are inefficient, with a negative effect mainly on periodontal health. The prevention of mouth disorders through bacterial plaque control seems to be the best alternative for promotion and maintenance of good oral health, helping to reduce costs in the public sector and also to improve the quality of life of these patients. The main objective of this study was to assess participation in, and effectiveness of, a programme for bacterial plaque control and prevention among autistic patients, and to assess the conditions of oral health for autistic patients. The patients were evaluated at five times, until a period of 180 days was reached. The following assessment instruments were used: OHI-S (Simplified Oral Hygiene Index), DMF-T index (decayed, missed and filled teeth), the Fonnes brushing technique and a list for informing the diet. DMF-T index showed a mean of 2.2. The patients were divided into two groups, according to the co-operation with the programme: Group A (co-operative) and Group B (non-cooperative). Oral hygiene showed a statistically significant improvement (p<0.001), with 84.2% showing fair or satisfactory hygiene at the end of the study. Group A represented 57.9% of the patients and had a mean age significantly higher than Group B (p=0.02). Groups A and B both showed improvements in hygiene (p<0.001 and p=0.004), but this was significantly higher among the co-operative patients (p=0.009, p=0.013 and p<0.001). Conclusion: There was a significant improvement in hygiene among all patients, but this was more significant among the co-operative patients, who also had a higher average age.

Early Childhood Caries in children 12-24 months old in Mitchell's Plain, South AfricaAli, Mustafa January 2008 (has links)
Magister Scientiae Dentium - MSc(Dent) / The American Academy of Pediatric Dentistry (2005/06) defines Early Childhood Caries (ECC) as the presence of 1 or more decayed (noncavitated or cavitated lesions), missing (due to caries), or filled tooth surfaces in any primary tooth in a child 71 months of age or younger. ECC can cause significant problems in preschool children and is a source of considerable societal costs. The South African national oral health survey conducted between the year 1999 and 2002 reported on the caries prevalence in young children. The caries prevalence was 50% in 4-5 year old children with a mean dmft of 2.4 (van Wyk and van Wyk, 2004).The aim of the study is to assess early childhood caries in children 12-24 months in the Mitchell's Plain district of the Western Cape. Objectives are to determine: a) The prevalence and pattern of early childhood caries. b) The relation between early childhood caries and infant feeding practices. c) The relation between early childhood caries and oral hygiene practices of the child. This study is a cross sectional study of ECC of children 12-24 months of age. Parent/child pair attending the Well Baby Clinic at Eastridge/Mitchell's Plain were informed about the study and invited to participate on a voluntary basis. The data collected consisted of a dental examination of 120 children (stratified by age: 60 in 12-18 months age group and 60 in 19-24 months age group) and a questionnaire completed by the accompanying parent/guardian. The dental examination was conducted using the WHO guidelines (Geneva 1997). Child age, tooth status (sound, decayed, filled, extracted, unerupted), and visible dental plaque on maxillary incisors (Spitz et al, 2006) were recorded. The prevalence of ECC for the sample was 23.3% (dmft =0.88). The maxillary incisors had the highest prevalence of decay (14%) followed by the maxillary molars (4%). There was a significant association (p=.006) between duration (12 months and more) of bottle use and presence of caries (40% caries prevalence). There is no significant difference between the different feeding practices (breast, bottle or both) and the presence of caries. There is a high prevalence of Early Childhood Caries (23.3%) in the 12-24 month age group. Early Childhood Caries was related to prolonged (12 months) bottle feeding. The association between the presence of dental plaque and ECC was the most significant factor (p= .000). / South Africa

Oral Care Assistance at Geriatric Homes in the County of Västerbotten, Sweden : A Questionnaire InterviewHolm, Moa, Larsson, Pernilla January 2017 (has links)
The elderly population in Sweden is growing and as a result of that, society is facing different types of challenges. Oral hygiene is important for preventing oral disease but also for the general health (GH). A goal for the Swedish dental society is that every inhabitant should have an equal right to dental-care and oral health (OH) under the same conditions. This is not only a challenge for dental staff but also for medical and healthcare personnel. Our hypothesis is that nursing staff do not possess the education needed regarding oral hygiene, especially not in relation to GH. The opinions regarding support for nursing staff differ between nursing staff and managers. This study was conducted through questionnaire interviews with managers and nursing staff at geriatric homes in three municipalities in the county of Västerbotten. Questions were asked about education and routines, performance of oral care, and OH linked to GH. Response frequencies were compared between municipalities, professions, and response options. A significant lower part (p <0.001) of nursing staff found that non-existing oral care could cause suffering in the form of general disease than in the form of oral disease. The opinion differs between managers and staff in questions about established routines (p <0.05). This study indicates that knowledge about relationships between oral and general health might be inadequate and that education is both wanted and needed among nursing staff. More extensive studies, and studies about how dental professions could support nursing staff in a better way is desirable.

: En Systematisk Litteraturstudie / What scientific evidence is available for use of behavioral change methods in dental care?Halitaj, Erjona January 2017 (has links)

A associação estabelecida entre estas variáveis indica a importância de uma abordagem biopsicossocial de atenção em saúde, com atuação centrada no adolescente e em seu âmbito familiar / Abstract: Considering the interest to con in designing oral health beyond the biomedical paradigm, meeting biopsychosocial model¿s precepts and prevention and health promotion, this study aimed to assess oral health behavior and biopsychosocial variables that permeate oral health in adolescents. Thus, three analyzes were developed based on observational epidemiological survey with cross-sectional design, which presented the following specific objectives. 1. To assess the frequency of dental visits and oral health, investigating its association with psychosocial variables, and socioeconomic status; 2. To investigate dental anxiety, and test associations between oral health, psychosocial and socioeconomic factors; 3. To assess the association between toothache, socioeconomic factors, oral health status and psychosocial variables. For the study 1 and 2, the sample involved 532 15-year-old adolescents from public schools in Piracicaba, São Paulo. For the study 3, the sample consisted of 592 students. We collected clinical data on dental caries, assessed by DMFT index according to WHO recommendations. Information concerning dental anxiety was collected by a validated questionnaire. The students also answered questions regarding adolescent access to dental visits, oral hygiene, reason for last dental visit, dental anxiety, family cohesion and adaptability and adolescents¿ food intake. Parents answered the socioeconomic status questionnaire. Data analysis were performed using descriptive statistics, chi-square test, odds ratio and Poisson regression. Regarding the study 1, controlling variables, low oral hygiene, low family income, more than four persons living in the same house and high dental anxiety were associated with low frequency of dental visits. Decayed teeth were associated with a low frequency of dental visits. Filled teeth was associated with a high frequency of dental visits and the female. Caries experience was associated with low oral hygiene, high frequency of dental visits and father with more than 8 years of schooling. For the second study, it was found that low frequency of dental visits and low oral hygiene was statistically associated with dental anxiety. In study 3, toothache was associated with low family income, more than four persons living in the same house, low oral hygiene, low frequency of dental visits, high dental anxiety, consumption of cariogenic foods, caries experience and decayed teeth. We conclude that the frequency of dental visits and oral health associated the socioeconomic and psychosocial variables (Chapter 1). In addition, dental anxiety was associated with frequency of dental visits and oral hygiene behavior (Chapter 2). Toothache was associated with socioeconomic factors, psychosocial variables and oral health status (Chapter 3), demonstrating the importance of behavioral and psychosocial variables in aspects that relates to oral health in school. The association established between these variables indicates the importance of a biopsychosocial approach, with activities centered on the adolescent and their family environment / Doutorado / Saude Coletiva / Doutora em Odontologia

Avaliação clínica e microbiológica periodontal em portadores de cardiopatia valvar na gestação / Clinical periodontal status in pregnant women with reumatic valvar diseaseLilia Timerman 01 August 2008 (has links)
Microorganisms of the oral cavity are known to cause systemic diseases, spread through sanguine current. Different factors, including the presence of periodontal disease, influencing the risk of oral bacteremia could cause infectious endocarditis for Streptococcus viridans. Nevertheless, the maintenance of the oral health is extremely important in pregnant women with rheumatic valvar disease, in which the risk of infectious endocarditis is eminent. The aim of this study was to investigate the clinical periodontal condition of pregnant women with valvar disease and to identify the presence of Porphyromonas gingivalis in saliva and subgingival samples. For these purposes, we studied 52 pregnant with valvar disease (GC) and 70 healthy pregnant women (GNC). The following periodontal parameters were evaluated: probing depth (PCS), clinical attachment level (NCI), gingival margin location (LEC/MG), bleeding on probing (IS) and plaque index (IP). The following mean periodontal parameters were obtained: PCS: 1.52 (GC) e 1.45 (GNC); NCI: 1.13 (GC) e 1.02 (GNC); LEC/MG: 0.41 (GC) e 0.40 (GNC); IS: 7.34 (GC) e 6.27 (GNC) e IP: 12.19 (GC) e 13.48 (GNC). There was no statistical difference for NCI among the groups. There was no difference between periodontal clinical conditions in pregnant women with valvar disease and healthy pregnant women. The presence of the Porphyromonas gingivalis in saliva samples of healthy pregnant women is statistically higher than in pregnant woman with valvar disease; however, there was no difference in periodontal samples

Oral behavior, dental, periodontal and microbiological findings in patients undergoing hemodialysis and after kidney transplantationSchmalz, Gerhard, Kauffels, Anne, Kollmar, Otto, Slotta, Jan E., Vasko, Radovan, Müller, Gerhard A., Haak, Rainer, Ziebolz, Dirk January 2016 (has links)
Background: Aim of this single center cross-sectional study was to investigate oral behavior, dental, periodontal and microbiological findings in patients undergoing hemodialysis (HD) and after kidney transplantation (KT). Methods: Patients undergoing HD for end-stage renal failure and after KT were investigated. Oral health behavior was recorded using a standardized questionnaire, e.g. dental behavior, tooth brushing, oral hygiene aids. Oral investigation included screening of oral mucosa, dental findings (DMF-T) and periodontal situation (Papilla bleeding index [PBI] periodontal probing depth [PPD] and clinical attachment loss [CAL]). Additionally, microbiological analysis of subgingival biofilm samples (PCR) was performed. Statistical analysis: Student’s t-test or Mann–Whitney-U-test, Fisher’s exact test (α = 5 %).
Results: A total of 70 patients (HD: n = 35, KT: n = 35) with a mean age of 56.4 ± 11.1 (HD) and 55.8 ± 10.9 (KT) years were included. Lack in use of additional oral hygiene (dental floss, inter-dental brush) was found. KT group presented significantly more gingivial overgrowth (p = 0.01). DMF-T was 19.47 ± 5.84 (HD) and 17.61 ± 5.81 (KT; p = 0. 21). Majority of patients had clinically moderate and severe periodontitis; showing a need for periodontal treatment of 57 % (HD) and 71 % (KT; p = 0.30). Significantly higher prevalence of Parvimonas micra and Capnocytophaga species in the HD group were found (p < 0.01). Conclusion: Periodontal treatment need and lack in oral behavior for both groups indicate the necessity of an improved early treatment and prevention of dental and periodontal disease, e.g. in form of special care programs. Regarding microbiological findings, no major differences between KT and HD patients were found.
