A framework for utilisation of health services for skilled birth attendant and postnatal care in Ethiopia

The Ministry of Health (MOH) and its partners are determined to prevent and manage preventable causes of morbidity and mortality in mothers, neonates and children. In the last decade, special emphasis has been given to increasing the number of health facilities that provide maternal and child health services (MNCH), huge production of skilled birth attendants (SBAs), and equipping the health facilities to improve the utilisation of quality services. This study investigated the community perspectives of health service utilisation and proposes a framework for improving the utilisation of the available SBA and postnatal care (PNC).The purpose of this study was to develop a framework for the utilisation of skilled care for delivery and postnatal care by women of childbearing age (15-45). The study used a Sequential explanatory Mixed Methods Approach to investigate the utilisation of SBA and PNC in a district in Ethiopia. The Delphi Technique helped to solicit input from maternal health care experts on the development of a Framework for utilisation of SBA and PNC. This study utilised the Anderson Health Utilisation Model. In the study, 79.8% and 248 (71.5%) of the women who delivered within 12 months prior to this study received ANC and skilled birth attendance respectively from SBA. Fifty five (15.8%) mothers and 131 (38%) babies received Postnatal care from SBA with in 45 days after delivery. The study further found that women who can read and write and were educated are more likely to utilise SBA and receive quality health care services. The study showed that certain factors such as disrespecting service users, abusing service users’ lack of trust on the SBA by service users, religion and superstition contribute negatively to the use of SBAs. None of the predisposing, enabling and need factors predicted the use of SBA for PNC by the mothers. Nevertheless, through the focus group discussions (FGDs) and individual interviews (IDI), it was revealed that there was a widespread knowledge and perception gaps in the community related to the importance of postnatal period and PNC. Religious sanctification also have imperative role in hampering PNC service use by the mothers. The Health care workers (HCWs) also lacked the keenness and orientation to provide the service. Babies born from families with monthly income equal to 500 or above USD were more likely to use PNC within 45 days. There was a widespread misconception in the community that Babies do not need PNC before 45 days of birth except for vaccination purpose. To examine their role in health service utilisation for SBA and PNC, researches can integrate social support and social network to the Andersen’s health- ervice utilisation model. A framework for utilisation of SBA and PNC is proposed. Kvinnors upplevelser av postnatal vård efter en komplicerad graviditet eller förlossning vid ett svenskt sjukhus : En kvantitativ och kvalitativ enkätstudie / Women’s description of postnatal care after a complicated pregnancy or childbirth at a Swedish hospital

Bakgrund: Varje år dör flera 100 000 kvinnor i världen på grund av graviditet, förlossning och abort. Länder världen över arbetar ständigt med FN:s globala mål till att minska mödradödligheten. I Sverige får kvinnor som fött sina barn ofta kvarstanna allt från ett dygn till några dygn för postnatal vård relaterat till komplikationer med mamman eller barnet. Kvinnor med okomplicerade och komplicerade graviditeter och förlossningar är en patientgrupp som barnmorskan möter dagligen i sitt yrkesutövande. Postnatal vård är därför en central del i barnmorskeyrket. Syfte: Syftet med den här studien var att beskriva kvinnors upplevelser av postnatal vård efter en komplicerad graviditet eller förlossning vid ett svenskt sjukhus. Metod: Studiens design var en kvantitativ och kvalitativ enkätstudie där författarna fick ta del av tidigare insamlade data. Sammanställning av enkäterna gjordes i SPSS och analys av kvalitativa data skedde genom en textanalys. Resultat: Majoriteten av kvinnorna var nöjda med den postnatala vården. Kvinnorna upplevde att de fick bra stöd från personalen. Ingen skillnad i nöjdhet mellan de olika vårdenheterna hittades. Kvinnor som hade sin partner med sig postnatalt var nöjdare än de som inte haft möjlighet att ha sin partner med. Förbättringsområden inom postnatal vård hittades. Slutsats: Trots det begränsade urvalet med kvinnor som haft komplikationer under graviditet och förlossning, så visade resultatet att de flesta kvinnorna var nöjda med den vård och stöd från personalen som de fått. Vissa områden kan dock förbättras, så som information gällande rutiner, barnläkarundersökningen och olika delar inom amningsstödet. Klinisk tillämpbarhet: Det är viktigt även för kvinnor med en komplicerad graviditet och förlossning att få en individualiserad vård. Den postnatala vården är viktig för kvinnans helhetsupplevelse. Därför är det av vikt att barnmorskan ha kunskap kring detta och kan utveckla sin yrkeskunskap. / Background: More than 100 000 women worldwide dies every year due to pregnancy, childbirth or abortion. Countries worldwide are continuously working towards the global goals to prevent maternal mortality. In Sweden, many women who give birth get to stay at a postnatal care unit for a few days after childbirth due to complications to the mother or the baby. Every day, midwives meet women who have a normal or a complicated pregnancy and childbirth. Postnatal care is a central part in midwifery practice. Aim: The aim of this study was to describe women's experience of postnatal care after having a complicated pregnancy or labor in a Swedish hospital. Method: The study design was a quantitive and qualitive survey study where the authors used previously collected data. Questionnaires were compiled in SPSS and the qualitative data was analysed with a text analysis. Result: The majority of women were satisfied with the postnatal care they received. They experienced that they got great support from the medical staff during their stay at the hospital. No significance was found in satisfaction between the care units at the hospital. Women who had their partner with them at the postnatal care unit were more satisfied than women who didn´t. Areas for improvement in postnatal care were identified. Conclusion: Despite the limited selection with women who had complications during pregnancy or childbirth, the result showed that most women were satisfied with the postnatal care they received. Quality of care improvement in some areas can be done, such as, routine information on each unit, pediatric examination and breastfeeding support. Clinical applicability: It is essential for women with complications during pregnancy or labor to have an individualized care. Postnatal care is very important for the woman's holistic experience. Therefore, it is vital that the midwife has knowledge and can further develop her professional knowledge.

Indigenous practices of women during pregnancy, labour, and puerperium amongst cultural groupings at selected hospitals in Limpopo Province, South Africa

Most women believe in indigenous practices because of their cultures and social structure. In South Africa regardless of the availability and accessibility of maternal and child health services, 50% of women were found that they still consult traditional birth attendants as their first choice during pregnancy, labour, delivery, and postnatal care. The purpose of the study was to determine the indigenous practices of women during pregnancy, labour and puerperium amongst cultural groupings at selected hospitals in Limpopo Province, South Africa. A Convergent parallel mixed method design was used in the study to collect both qualitative and quantitative data at the same time. Non-probability purposive sampling was used to select 15 participants and Probability simple random sampling was used to select 125 women who were pregnant, in labour and puerperium using slovin’s formula. Data were collected through a semi-structured interview with a guide for qualitative strand and a self-administered structured questionnaire for quantitative srtand. Data were analysed qualitatively using tech’s open coding method and quantitatively using Statistical Package for the Social Sciences (SPSS) Version 25 with the assistance of the University of Limpopo’s Bio-statistician. The results of the study showed that most women use indigeneous practices for protection against witchcraft, fear of giving birth in caesarian section and many other reasons.THPs and church leaders are regarded as the most principled people in their community. Indigenous women are aware of the sign and symptoms during pregnancy, labour, and puerperium which may determine consultation to healthcare practitioners, but they choose THPs and church leaders. Most women still rely on their religious beliefs to assist during their labour. Pregnant women, those in labour and puerperium should be supported to practice their religious beliefs and practices. THPs and church leaders are obliged to teach their clients and ensure that they know the names and components of the traditional medicines and church rituals they use.The nursing education should include indigenous practices in the curriculum so, that healthcare practitioners know about the indigenous practices and can serve as assistance in the training and development of health practitioners who continuously care for women during pregnancy, labour, and puerperium and as a result, may reduce maternal and child morbidity and mortality in Limpopo Province, South Africa.

Development of strategies to enhance quality kangaroo mother care at selected public hospitals, Limpopo Province

(Nursing)) -- University of Limpopo, 2021 / BACKGROUND The quality Kangaroo Mother Care (KMC) continues to be sub-standard as it is not managed properly, despite the maternal and neonatal services provided by the midwives during postpartum and neonatal period, as this can lead to increased neonatal mortality rate and maternal depression at selected hospitals of Limpopo Province, therefore the researcher is interested in developing strategies to enhance quality KMC at selected hospitals of Limpopo Province. KMC has been found to have physiological, behavioural, psychosocial and cognitive developmental benefits, and it enhances motherinfant bonding. The aim of the study was to develop strategies to enhance quality Kangaroo Mother Care in selected hospitals of Limpopo Province. RESEARCH METHOD A quantitative descriptive cross-sectional research method was used to collect numerical data with regard to the factors that hinders the quality Kangaroo Mother Care in selected hospitals of Limpopo Province. Population size was 77, Simple Random Probability Sampling was used in this study with the sample size of 65 midwives. Data were collected using self-administered questionnaires and analysed using Statistical Package for Social Sciences (SPSS) version 24. Quantitative analysis is the numerical representation and manipulation of observations for the purpose of describing and explaining the phenomenon reflected on observations (Babbie & Roberts, 2018). RESULTS The study revealed that the factors that hinders quality of Kangaroo Mother Care are lack of education and training to midwives regarding Kangaroo Mother Care, and other sources suggested that all categories of nurses should also be trained. The midwives are knowledgeable with regard to KMC, however, the problem remains the work overload when they have to monitor both the mother and neonate during feeding. It was further indicated that family members can also assist with regard to KMC. Strategies were developed, as optimal KMC environment, optimal KMC interventions, enhance optimal mother attitude to KMC crate gender sensitive environment. CONCLUSION The study concluded that the quality of KMC should be enhanced through both the promotion of education and training to all midwives, and involvement of families to assist in KMC at selected hospitals of Limpopo Province

Effekten av bäckenbottenträning hos kvinnor med postnatal urininkontinens : En litteraturstudie / The effect of pelvic floor muscle training in women with postnatal urinary incontinence : A review

Bakgrund: Urininkontinens är ett stort folkhälsoproblem i världen och det definieras som ofrivilligt urinläckage. Vid en graviditet är risken stor att drabbas av urininkontinens och många återhämtar sig inte utan får kvarstående problem. Bäckenbottenträning är den vanligaste behandlingsmetoden för urininkontinens.  Syfte: Denna litteraturstudie syftade till att utvärdera effekten av bäckenbottenträning som intervention hos kvinnor som lider av postnatal urininkontinens. Metod: En litteraturstudie med systematisk sökning i databasen PubMed. Studiernas kvalitet granskades med PEDros granskningsmall och evidensgradering genomfördes med GRADEstud. Resultat: Totalt inkluderades sju studier i studien, varav fyra var av god kvalitet och tre var av måttlig kvalitet. Fem av sju studier visade signifikanta skillnader i effekten av bäckenbottenträning på urininkontinens. Tre av fem studier visade signifikanta skillnader i effekten av bäckenbottenträning på styrka samt två av fem på uthållighet i bäckenbottenmuskulaturen. Evidensstyrkan bedömdes som låg och mycket låg, både gällande effekten på förekomsten av urininkontinens och muskelstyrkan och uthålligheten i bäckenbotten. Konklusion: Resultatet i denna litteraturstudie visar på att bäckenbottenträning postpartum har effekt på både förekomst av urininkontinens samt muskelstyrkan i bäckenbotten. Då tillförlitlighet var låg till mycket låg har dock inte resultatet någon klinisk relevans. Gällande uthållighet i bäckenbottenmuskulatur är resultatet ovisst och även här med låg tillförlitlighet. Den genomgående låga tillförlitligheten tyder på att det krävs fler fysioterapeutiska högkvalitativa studier för att kunna dra några större slutsatser. / Background: Urinary incontinence is a major public health problem in the world and is defined as involuntary leakage of urine. During pregnancy, there is a high risk of urinary incontinence, and many suffer from persistent problems. Pelvic floor training is the most common treatment for urinary incontinence. Objective: This review aimed to evaluate the effect of pelvic floor training for women suffering from postnatal urinary incontinence. Method: A systematic literature search was carried out in the database PubMed. The study quality was reviewed with the PEDro-scale and the certainty of evidence was evaluated with GRADEstud. Results: Seven studies were included in the study, of which four were of good quality and three were of moderate quality. Five of seven studies showed significant effect of pelvic floor training on urinary incontinence. Three out of five studies showed significant effect of pelvic floor training on strength and two out of five on endurance in the pelvic floor muscles. The certainty of evidence was assessed as low to very low. Conclusion: The review showed that pelvic floor training has a positive effect on both the occurrence of urinary incontinence and the muscle strength of the pelvic floor muscles. Regarding endurance, no significant difference could be established. However, as reliability was low to very low, the result has no clinical relevance, and thus, more high-quality physiotherapeutic studies are needed in order to come to further conclusions.

The effects of antenatal health education on postnatal care among HIV positive women in Francistown City, Botswana

The purpose of the study was to determine the effect of antenatal health education on postnatal care (PNC) among Human Immunodeficiency Virus (HIV) positive women in Francistown city, Botswana. This study followed a quantitative research paradigm. Data was collected with the aid of a questionnaire from eligible women who consented in writing to participate in the study. Forty-five percent (45%) (n=45) HIV positive women came with babies for 6 weeks PNC and 55% (n=55) brought 8 weeks old babies either for weighing or for the two months immunisation from 28 April to 10 June 2014. The responses regarding the source of health education received were as follows: 40% lay counselors, 31% midwives, 15% doctors, 5% nurses without midwifery, 5% cadre unknown, 2% both lay counselors and midwives, 1% by a health education assistant and 1% was not health educated at all. Conclusion: The study revealed that HIV positive pregnant women received health education from different cadres of health and mostly by lay counselors. Barriers to utilization of antenatal care services among pregnant women in Omaheke region, Namibia

The purpose of the study is to determine the barriers to utilisation of antenatal care services among pregnant women in Omaheke Region - Namibia. A quantitative, cross-sectional descriptive research was followed, and a structured questionnaire was used to obtain information from study participants. One hundred and ten (110) women of child-bearing age between 18 to 49 years who visited Gobabis District Hospital in Omaheke Region for deliveries (full-term) or kept for postnatal care purposes were selected. A questionnaire with open-ended and closed-ended questions was designed using a Likert scale to gather information. SPSS software was used to generate descriptive statistics. Results indicated that the majority of respondents (39%) who attended antenatal care services, fell within the age range of 18-21 years. Participants demonstrated higher knowledge of the recommended number of antenatal care visits and of the importance of antenatal care services. Transport money and cost, long distances to health facilities, desirability, unplanned or unwanted pregnancy, and limited transportation options are the barriers that contribute to low utilisation of antenatal care services among pregnant women in Omaheke Region. Recommendations for deployment of more qualified midwives at rural antenatal clinics, building more antenatal care clinics in the region and creation of community awareness campaigns on the importance of antenatal care may increase the utilisation of antenatal care services in the Region. It is anticipated that this research will contribute to the promotion and utilisation of antenatal care services among pregnant women in Omaheke Region. / Health Studies / M. P. H.

Knowledge, attitudes and practices of nurse-midwives related to obstetric care at Thaba-Tseka district in Lesotho

The purpose of the study was to describe the knowledge, attitudes and practices of nurse-midwives related to obstetric care at Thaba-Tseka, Lesotho. A quantitative, descriptive, cross-sectional research design was used. Data were collected using structured questionnaire from 45 nurse-midwives. The findings revealed that nurse-midwives had mean knowledge score of 10.5(80.7%) out of a possible 13(Standard Deviation (SD) 1.31) on obstetric care issues. However, the majority of nurse-midwives (n=28, 62.2%) did not have knowledge on the steps of the active management of third stage of labour according to the WHO. The mean scores on practice were 34.5(86.2%) against a possible 40(SD 5.43) for antenatal care, 39.2(89%) against a possible of 44(SD 4.66) and 22.4(93.3%) against a possible of 24(SD 2.18) for postnatal care. The study revealed that nurse-midwives had positive attitudes towards obstetric care practices with mean score for attitudes was 23.4(86.7%) against a possible score of 27(SD 3.02) / Health Studies / M.A. (Public Health)

A humanização do cuidado pré e pós-natal em uma organização não-governamental / The humanization of antenatal and postnatal care in a non-governmental organization

A humanização do cuidado ao ciclo grávido-puerperal no Brasil é marcada por distintos sentidos e experiências. Diante de tal diversidade, um conceito de humanização foi elaborado no presente trabalho, a partir de uma análise de documentos do Programa de Humanização no Pré-Natal e Nascimento (PHPN), lançado pelo Ministério da Saúde em junho de 2000. Tal conceito norteou o estudo, que se constituiu em uma pesquisa qualitativa de referencial fenomenológico-existencial realizada, à luz do PHPN, no ambulatório de uma organização não-governamental que presta uma atenção pré e pós-natal autorreferida como humanizada. Teve como objetivos: observar, descrever e compreender como se dá a humanização de tal atenção, tendo em vista as inter-relações de alguns dos sujeitos envolvidos em sua produção (usuárias, colaboradores e coordenadores) e o cenário no qual tais inter-relações se dão; compreender de que forma os processos de acolhimento e de produção de vínculo estabelecem relações com a humanização do cuidado, caso tais aspectos se fizessem presentes no serviço estudado; e refletir sobre elementos que favoreçam a superação de algumas das contradições que permeiam o campo da humanização. A coleta de dados foi realizada a partir de observação participante, levantamento de dados de cartões de gestantes e entrevistas com profissionais de saúde ligados ao serviço em pauta e com mulheres lá atendidas. A análise dos dados evidenciou, como eixos centrais do cuidado produzido: o acolhimento, explicitado nas dimensões de postura do profissional (discutida como dialógica/de figura de apego seguro, segundo o referencial gestáltico) e de (re)organização do serviço; a produção de vínculos; e a integralidade (ali marcada pelo olhar da Antroposofia). Tais processos afiguraram-se também como aqueles que marcam a forma com que a humanização é compreendida e mobilizada no serviço. Mostraram-se, nesse sentido, especialmente importantes à restituição e/ou reafirmação da condição de sujeitos integrais de usuárias e de profissionais e à experiência de ampliação e efetivação de potências de tais atores. Por fim, revelaram-se aspectos fundamentais à efetivação das mudanças necessárias à sedimentação de um cuidado humanizado: o reconhecimento, a valorização e a legitimação institucional da condição de pessoa do profissional; a fundamentação da produção do cuidado em saúde em um referencial filosófico humanista e holístico; e a superação das contradições identificadas não apenas no universo dos discursos mas também na existencialidade de cada um dos sujeitos que de tais discursos se fizer portador e multiplicador / In Brazil the humanization of maternity care is marked by different meanings and experiences. Facing such a diversity, a concept of humanization has been elaborated in this study, based on the analysis of documents from the Program for Humanization of Prenatal and Childbirth Care (PHPN), launched by the Ministry of Health in June 2000. The concept herein developed guided this work, which consisted of a qualitative research with an existentialphenomenological approach, held in the light of PHPN, in a non-governmental organization\'s ambulatory that provides a pre- and postnatal care which is self-defined as humanized. The aims of this study were (i) to observe, describe and understand the humanization of such a care, considering the interrelationships of some of the subjects involved in its production (users, collaborators and coordinators) and the environment in which the interrelationships take place, (ii) to understand how the processes of user embracement and promotion of bonding are related to the humanization of care, in the cases where the service studied included such aspects, (iii) and to reflect about elements that help to overcome some of the contradictions that are found in the humanization field. The data were collected through field participant observation, data collection from pregnants\' attendance card and interviews with health professionals related to the service and women there attended. The data analysis demonstrated the axes of the production of care were (i) the user embracement, which appeared in the dimension of professional attitude (herein exposed as a dialogical attitude and/or as a secure attachment figure attitude, according to the gestaltic approach) and in the service reorganization dimension, (ii) the promotion of bonding; and (iii) the comprehensiveness (as understood in Anthroposophy, mainly). The humanization was associated with such processes, which showed themselves to be especially important to the restitution and/or reaffirmation of the integrality condition of users and professionals and to the experience of ampliation and realization of potentialities of these subjects. Finally, the following aspects proved to be essential points to implement the necessary changes which solidify the humanized care: (i) the recognition, the valorization and the institutional legitimation of the health professionals\' personal condition, (ii) the grounding of the production of health care in a humanistic and holistic approach, (iii) and the overcoming of the contradictions identified not only at the level of discourses but also at the existential dimension of each subject that produces and propagates those discourses
