The Factors influencing job satisfaction of nurses working in a Provincial Psychiatric hospital in the Western Cape.

<p>Much evidence exists that nurses are leaving the public health sector for the private sector, or leaving the country to seek better working conditions and higher salaries. Studies conducted on the job satisfaction of nurses are proof that there is a need to know more about the factors that influence their sense of job satisfaction. Most of these studies focus on the general nursing context. Due to its unique circumstances, many studies abroad have identified the field of psychiatric mental health nursing to investigate job satisfaction of nurses. The minithesis is an attempt to fill the gap that exists in job satisfaction studies in South Africa of nurses in a provincial psychiatric hospital. The study was a cross sectional, correlational, survey design study. The instrument was a self-administered questionnaire, combining a quantitative questionnaire with one qualitative open-ended question. The study was conducted on nurses of all categories in a provincial psychiatric hospital in the Western Cape. Sixty- eight nurses participated in the study. The data was analyzed statistically using the SAS v9 statistical software and Statistical Package for Social Sciences (SPSS). The open-ended question was analyzed qualitatively. The results revealed that the participating nurses were dissatisfied with remuneration, recognition and appreciation, training and development, as well as benefits and incentives. Nurses were most satisfied with supervision and support, interpersonal relationships, and rendering patient care. The study identified the factors influencing job satisfaction and job dissatisfaction of nurses in a provincial psychiatric hospital. Recommendations were made based on the results of the research.</p>

Magister Curationis - Mcur / Research studies allude to the escalation of work related stress, which leads to burnout among mental health nurses, internationally. Mental health nursing is stressful because it is a helping profession that involves close interpersonal working relationships and the burden of providing nursing care to mental health care users (MHCUs) with complex emotional demands. These stressful working conditions render mental health nurses susceptible to burnout. Globally, health care services share a concern about the effects of burnout on nurses. Although burnout is a personal experience for the nurses, the results impact negatively on the quality of care delivered to MHCUs, while increasing the cost of care of the hospitals, which employ these mental health nurses.

Da maquinaria mortífera do manicômio judiciário à invenção da vida : saídas possíveis

A resposta penal para a junção entre crime e loucura é a medida de segurança contra o perigo. Forjado na costura entre os saberes médico e jurídico instituídos, a montagem ficcional do louco perigoso foi sendo criada a partir do século XIX e o seu destino acabou selado na maquinaria do manicômio judiciário como resposta normativa aos dispositivos de segregação na vigência da estratégia complexa de controle dos corpos na gestão biopolítica da nossa sociedade. Essa pesquisa parte da aposta e esperança na produção de uma outra saída e, para tanto, buscamos explicitar o que fez essa maquinaria se armar da forma com que se armou e quais as perspectivas de desarmá-la, fazendo uma genealogia sócio-histórica das bases epistêmicas que sustentam, ainda nos dias de hoje, a existência do manicômio judiciário. E, ao fazer esse caminho procuramos responder recorrendo a cenas-imagensmemórias, apresentando os efeitos no real da experiência, como as palavras afetam os corpos. Isto é, a forma pela qual essa ficção, que envolve os conceitos de crime e loucura juntos, incide sobre o sujeito. No desfazer a forma do manicômio judiciário trouxemos os movimentos da reforma psiquiátrica e a contribuição da psicanálise, quando o que se quer não é apontar a periculosidade, mas oferecer ao sujeito uma possibilidade de saída, colocando ele e suas respostas na centralidade da amarra discursiva que estão em jogo nessa engrenagem. / The criminal law response to the junction between crime and insanity is the measure of security against danger. Forged on the seam between the instituted medical and legal knowledge, the fictional mounting of the dangerous madman started being created as from the nineteenth century and its fate ended up sealed in the machinery of the judiciary psychiatric hospital as normative response to the devices of segregation in the validity of complex strategy to control the bodies in the bio-political management of our society. This research emerges from the betting I make on the hope of a different way out and, to this end, I seek the answer to what made this machinery arm itself the way it did and what are the prospects for disarming it, doing a social-historical genealogy of epistemic bases that support, to this very day , the existence of the judiciary psychiatric hospital. And in walking this path we seek to answer, recurring to scenes/images/memories, and showing the effects of real experience how words affect bodies. That is, the way in which this fiction, that involves the concepts of crime and madness together, acts on the subject. On the undoing of the judiciary psychiatric hospital we brought the psychiatric reform movements and the contribution of psychoanalysis, when what we want is not to point out dangerousness but to give the subject the possibility of an exit, putting him and his answers on the core of the discursive ties that are at stake in this mechanism.

Conselho Nacional de Desenvolvimento Científico e Tecnológico / Esta dissertação se dirigiu ao estudo sobre as internações psiquiátricas de pacientes em longa permanência e as ações voltadas aos processos de desinstitucionalização desta clientela na cidade do Rio de Janeiro. Ao se considerar que as práticas na saúde mental, bem como na saúde geral, estiveram relacionadas aos contextos sociais, econômicos e políticos do país, compreende-se que a existência da situação das internações de longa permanência dentro de hospitais psiquiátricos públicos e clínicas conveniadas têm relação direta com as políticas de saúde que vigoraram ao longo do tempo no país e com o trabalho específico sobre este campo na própria cidade. Assim também há a consideração de que a Reforma Psiquiátrica e suas inovações conceituais também estiveram ligadas a um novo momento das políticas de saúde no país, onde se direcionava com o SUS pela saúde de acesso universal, descentralizada e hierarquizada, o que possibilitou grandes mudanças no cenário da assistência. Estudou-se nesta pesquisa, portanto, as ações de desinstitucionalização na cidade, tomando?se como uma das primeiras iniciativas de gestão a reorganização da assistência psiquiátrica pela implantação dos Pólos de Internação, numa estratégia de co-gestão do Ministério da Saúde e Inamps ainda na década de 80. Seguido a esta reorganização que culminou em maior regulação da porta de entrada das internações psiquiátricas e diminuição das internações nas clínicas conveniadas, o município teve sua entrada como gestor na saúde mental na cidade, como novas ações implementadas. Para pesquisa destas ações utilizou-se como metodologia entrevistas semi-estruturadas com pessoas que participaram das iniciativas de gestão no município do Rio de Janeiro e análise documental de Relatórios de Gestão publicados pela Coordenação Municipal de Saúde Mental. No estudo, destacou-se a potência do serviço público enquanto direcionador de ações na saúde e ainda, a importância do trabalho de desconstrução do manicômio feito pelas internações psiquiátricas quando promovem restrições ao pensamento deste dispositivo como solução para problemas psiquiátricos e sociais de diversas ordens, e na orientação de descentralização como desmontagem do aparato manicomial pela lógica da atenção psicossocial. / This research aimed to study about the hospitalization of people for a long time in psychiatric hospitals and actions planned to finish this condition in the city of Rio de Janeiro. Considering that the practices in mental health and general health were related to social, economic and political context, it is understood that the existence of the long-stay hospitalization in public psychiatric hospitals and private clinics - in association with public investments - have direct relation to the health policies that prevailed over time in the country and the specific work on this in the town. So, there is also the consideration that the Psychiatric Reform and its conceptual innovations have also been linked to a new era of health policies in the country after the entrance of SUS, a very different kind of policy which changes all practices in health. This system points to the directions of a universal health access, decentralized and hierarchical, allowing large changes in scenario assistance. Therefore, in this research we study the actions of deinstitutionalization in the city, taking as one of the first initiatives of the management reorganization of psychiatric care for the deployment of Poles of Emergency, a strategy of co-management of the Ministry of Health and Inamps yet in the 80s. Following this reorganization that resulted in greater regulation of the entrance door of psychiatric hospitalizations and fewer hospitalizations in private clinics, the municipality had its entrance as a manager in mental health in the city, as new actions taken. In studying these actions and strategies planned it was used semi-structured interviews with people who participated in the management initiatives in health in the city of Rio de Janeiro, and also, document analysis of Reports published by the Management Coordination Municipal Mental Health. In this study, we emphasized the power of public services to give directions and formulate public health policies. Also we highlight the importance of the work done to by the deconstruction of the asylum psychiatric when there are restrictions to answer to a social problem with psychiatric hospitalization or reduce psychiatric problems of many orders to this procedure.
