O clube dos saberes no hospital: análise institucional de uma intervenção / The Clube dos Saberes in hospital: institutional analysis of an intervention.

O objetivo desta pesquisa é analisar, a partir do referencial da Análise Institucional, a experiência do Clube dos Saberes (CS), um dispositivo que fez parte de uma intervenção institucional, num hospital psiquiátrico privado da cidade de São Paulo, entre os anos 2001 e 2005. O Hospital é uma instituição biforme, com uma organização hierárquica-piramidal e características manicomiais. O CS é uma rede de relacionamentos formada pela troca de saberes e de experiências entre as pessoas de um coletivo, que se organiza democraticamente e visa tanto o crescimento pessoal e individual quanto do próprio coletivo. Ele é proposto ao Hospital como possibilidade de superação da crise enfrentada através da inoculação de uma nova cultura de tratamento, de novas formas relacionamento, de novos modos de funcionamento e organização. A pesquisa envolveu, num primeiro momento, a reconstituição da história dessa experiência a partir da memória da pesquisadora-participante e de registros escritos e, num segundo momento, a seleção de trechos dessa história que possuíam a qualidade de analisador. A análise mostrou que apesar do CS representar uma esperança de mudança e de superação da crise do hospital, ele sofreu resistências por parte da equipe e enfrentou diversos obstáculos: participação pouco expressiva dos trabalhadores do hospital no desenvolvimento do CS; apreensão distorcida ou limitada do projeto; baixa efetividade das parcerias firmadas; persistência e recrudescimento das características manicomiais do hospital; predomínio da organização sobre a clínica e maior atenção à saúde financeira do hospital do que à saúde de seus agentes (pacientes e trabalhadores). Por fim, consideramos que o desenvolvimento do CS foi significativamente prejudicado pela falta de espaços de análise da instituição e de sua alienação. / The object of this research is to analyze, using institutional analysis as a reference, the experience of the Clube dos Saberes, ( Club of knowledge ) or CS, the instrument of an institutional intervention in a private psychiatric hospital in the city of São Paulo between the years of 2001 and 2005. The hospital is a two pronged institution, which has a hierarchicalpyramidal organization with characteristics of an asylum. The CS is a network of relationships formed by the exchange of knowledge and experiences between a collective, that organizes democratically and aims for a personal and individual growth as well as the growth of the collective. This instrument is offered to the hospital as a possibility of overcoming a crisis through the inoculation of a new culture of treatment, of new forms of relationship, of new ways of functioning and organization. The research involved, at first, rebuilding the history of this experience through the memory of the participant-researcher, and through written records, later there was the selection of passages of this history that had the quality of analyzer. The analysis showed that although of the CS represented a hope of change and a chance of overcoming the crisis in the hospital, it suffered resistance from the part of the team and it faced several obstacles: a participation of small significance from the staff in the development of the CS; limited or distorted apprehension of the project; low effectiveness of the established partnerships; the persistence and aggravation of the characteristics of the hospital as an asylum; The predominance of the organization in detriment of the clinic and a greater attention to the financial health of the hospital then the health of its agents ( staff and patients ). In conclusion we find that the development of the CS was significantly hindered by the lack of a space for an institutional analysis and its own alienation.

M.A. (Social Work) / South Africa is a society undergoing major and rapid social change. An understanding of psychiatric services in the psychiatric hospital should also be identified within this context. The needs of the mentally ill and other interested parties will determine the extent of the services that need to be rendered. The social worker as part of the interdisciplinary team in the hospital, must have a clear perception of her professional role in order to render a relevant service. Social work must become more responsive to the consumers' needs and allow itself to be shaped by the context in which it is practised. In this study the role of the social worker in Weskoppies Hospital was identified by means of a descriptive research design with qualitative and quantitative methods of data collection. The qualitative research was done by means of in depth interviews and focus groups with interested parties. The quantitative research was done by means of questionnaires completed by patients of the hospital. A model for the role description of the social worker was derived from a literature study; the view of Lister, as described by the authors Hepworth & Larsen; the view of other authors and the results of the qualitative and quantitative research. The results of the research confirmed a need for social work services in Weskoppies Hospital. The greatest need expressed, was a need for help with material problems, followed by a need for help to be linked with other resources. Although community involvement was emphasised, there still was a need for individual counseling and support. Other important roles were also identified. The model serves as an integrated description of the role of the social worker in the hospital. From the results of the research it was thus possible to convey a clear picture of the role of the social worker in Weskoppies Hospital. Hopefully this will enable the social workers in the hospital to render a better and more understanding service to the patients, hospital and community.

Social workers’ and physicians’ experiences with review panels in British Columbia

In British Columbia, individuals with a mental disorder can be hospitalised against their will under the Mental Health Act (1999), when a physician determines that "protection of the person or others" is an issue. Involuntary psychiatric hospitalisation involves a major infringement of an individual's civil liberty. When patients or their representatives disagree with the treatment teams about their involuntary hospitalisation, they can apply for an appeal hearing named the review panel under the Mental Health Act (1999). From a theoretical perspective, the traditional medical model and the social constructionist model, which offer different views concerning individuals with mental illness who are hospitalised against their will, are presented. This theoretical contrast underscores a major dilemma faced by mental health professionals in fostering client self-determination, while they are providing services on the principle of beneficent protection. An empowerment model of social work practice is then described to illustrate how services can be provided to help mental patients regain a sense of control over their lives. Concerns about the current legislation regarding involuntary hospitalisation are also discussed. To supplement the limited number of studies currently available on review panels, a quantitative descriptive study was conducted at Riverview Hospital, the only tertiary psychiatric hospital in British Columbia, surveying the experiences of 39 social workers and physicians with review panels. A questionnaire consisting of 22 Likerttype items was used. Four categories were identified: (a) patient-related issues, (b) effects of review panels on treatment teams, (c) role conflicts, and (d) operational issues. Despite the apparent lack of formal training, social workers and physicians generally reported having adequate knowledge of review panels. Social workers and physicians who were involved more frequently with review panels appeared to have more positive attitudes towards them. Their training pertaining to the Mental Health Act was also significantly related to their attitudes. Neither patients nor their families reportedly had adequate knowledge of the review panel process. Although some positive effects of review panels were acknowledged, staff generally had mixed attitudes about review panels. Guided by an empowerment model, these findings have important implications for social work practice. These include the need for further professional training, improved communication between health professionals and legal advocates, education for patients and families, and the support of patients' collective action, so that review panels can be a more empowering experience for mental patients. / Arts, Faculty of / Social Work, School of / Graduate

La salud mental en el Perú ha cobrado importancia en las últimas décadas en base al percate de la gravedad de los problemas y estadísticas que giran en torno a este. Ante estos problemas, antiguamente se hubieran preferido construir más hospitales psiquiátricos, sin embargo, estudios globales con más de dos décadas demuestran mayor efectividad en el tratamiento del paciente mediante un modelo de salud mental comunitario y una infraestructura con la capacidad de influir en su bienestar. En el Perú, el MINSA implementa dicha modalidad desde el 2015 pero falla en resolver el problema de la infraestructura. Es por esto que se propone el diseño de un Modelo de Red de Salud Mental Comunitaria en Chosica, que a partir del entendimiento de su infraestructura como medio terapéutico, se pueda influir en el bienestar anímico de un usuario cuya población crece conforme pasan los años. / Mental health in Peru has gained notoriety in recent decades based on the awareness of the seriousness of the problems and statistics that revolve around it. Given these problems, in the past they would have preferred to build more psychiatric hospitals, however, global studies with more than two decades proved greater effectiveness in the treatment of the patient through a model of communal mental health and an infrastructure with the ability to influence their well-being. In Peru, the MINSA(acronym in spanish) has implemented this model since 2015 but fails to solve the infrastructure problem. That is why the design of a Communal Mental Health Network Model in Chosica is proposed, which, from the understanding of it’s infraestructure as a therapeutic tool, can influence the well-being of an user whose population is growinf more and more through the years. / Tesis

The effects on staff of working in an eating disorders unit

1 online resource (vii, 138 leaves : ill.) / Following an awareness of an increased turn over of staff in the eating disorder unit as compared with other specialised units, in a psychiatric hospital in Johannesburg, South Africa, Tara Hospital, the researcher was motivated to investigate the issue. An exploratory, descriptive based research was chosen to explore and gain information about the topic and its implications. A qualitative research approach was used to gain insight into the perceptions and needs of the team working on the unit. The case study method was used. A pilot study was undertaken to test the validity of the interview schedules. The sampling category was a non- probability one. Individuals were selected from the population of staff working on the unit. Participants were selected from two groups, those who were presently working on the unit and those who had previously worked on the unit and now working in other units. Interview procedure involved personal semi-structured interviews conducted by the researcher and analysed qualitatively and a structured interview questionnaire analysed quantitatively. The researchers assumption that many staff members move from working in an eating disorders unit was confirmed and is due to the following: Staff turnover is due to constant exposure to occupational stress and burnout. Feelings of helplessness, a sense of being unappreciated and excessive exposure to conflict from the patients. In addition, staff experience minimum rewards leading to lowered job satisfaction due to the patients slow recovery rates and a poor prognosis of the illness. Staff also experience a change in their eating patterns and an increased awareness around food and food issues. Recommendations to the staff include: * Psycho-education on eating disorders. * Implementation of strategies to provide supportive care for all staff members. * Education on stress management and strategies to prevent staff burnout and lowered job satisfaction. * A multidisciplinary teamwork approach by the staff, when working in the unit. / Social Work / M. Las condenadas : an ethnography of sexuality and violence in Bolivia

This is an ethnographic study of discourses and experiences concerning sexual exchanges among kin “who are too closely related to marry each other” (OED), or what in lay language is called “incest”. I investigate the ways in which a certain kind of incest, that between older men and younger women, primarily from different generations, is experienced by women of predominantly rural origin, who have been hospitalized in the major public psychiatric hospital in Bolivia, in Sucre. In this sense, this research is as much a study of incest as it is of psychiatric institutionalization. These experiences will be considered in the context of a wider field of ethnic, class and gender discourses that are produced by medical staff, community organizations, as well as national judicial institutions. The category of 'incest' is problematized in terms of how kinship is constructed, not only as a series of dynamic discourses (as practices whose effect is the production of events) but also as mobile experiences, however socially regulated. With this in mind, I present an account of Andean concepts and treatment of incest, as well as of legal and medical categories. Specifically, I focus on the play between discourses in the context of the psychiatric hospital, the judicial court and the communities of selected inmates. I show how the inmates' experiences of intergenerational incest and sexual violence in general are related to the dominant ethnic, class and gender narratives produced by medical staff, community organizations, and judicial institutions.

The same can not be said if these same people come into conflict with the law. Understood as dangerous, they are throw out of the legal system aligned with human rights due to a supposed intellectual and volitional disability in the face of criminal act. The periculosity attested by forensic experts authorizes the justice to consider them in an inimputability situation, what, besides to not allow them to be called to answer for their acts, motivates the restriction of a number of constitutional rights. Considering the reality of rape and extreme violence own this context, this research aimed to discuss the processes of institutionalization and deinstitutionalization of so-called "insane offenders" in State of Pará, using the cartography method as intervention-research. We mapped some of the lines that establish the device "security measure" and the processes of institutionalization promoted by it, considering the production of fear as governmentality strategy in the constitution of punitive subjectivities. At the same time, we follow the flows triggered by the research towards the deinstitutionalization processes of this population. From the Custodial Psychiatric Hospital Santa Isabel of Pará, we have described the practices of power-knowledgesubjectivation intended for institutionalized people and profiled those submitted to security measure, these lives which have never been counted. In addition, it have been built devices - such as art workshops to psychiatric patients, itinerant exhibition of the resulting works, conversation circles with workers in the mental health network and a documentary film - favoring the connection of actors and various elements to function as machines that make see and talk about security measure dispositive, composing new sensibilities and forging aesthetic vectors of deinstitutionalization. The co-organization of a Meeting among various actors of justice and the rule of executive managers still allowed us following the configuration of a program of deinstitutionalization and approached the legal-political dimension of the Psychiatric Reform when we have discussed the person's responsabilization model with disorder mental in conflict with the law / No Brasil, as pessoas com diagnóstico de transtorno mental passaram a ser consideradas sujeitos de direitos a partir da Lei da Reforma Psiquiátrica. O mesmo não se pode afirmar se estas mesmas pessoas entram em conflito com a lei. Entendidas como perigosas, são lançadas para fora do ordenamento jurídico alinhado aos direitos humanos devido a uma suposta incapacidade intelectiva e volitiva diante do ato delituoso. A periculosidade atestada por peritos forenses autoriza a justiça a lhes considerar inimputáveis, o que, além de não permitir que sejam chamadas a responder por seus atos, motiva a restrição de uma série de direitos constitucionais. Considerando a realidade de extrema violação e violência própria desse contexto, esta pesquisa teve como objetivo problematizar os processos de institucionalização e desinstitucionalização dos chamados loucos infratores no Estado do Pará, utilizando o método da cartografia como pesquisa-intervenção. Mapeamos algumas linhas que constituem o dispositivo medida de segurança e os processos de institucionalização por ela promovidos, considerando a produção do medo como estratégia de governamentalidade na constituição de subjetividades punitivas. Ao mesmo tempo, seguimos os fluxos acionados pela pesquisa em direção aos processos de desinstitucionalização dessa população. A partir do HCTP de Santa Isabel do Pará, descrevemos as práticas de saber-poder-subjetivação destinadas às pessoas institucionalizadas e traçamos o perfil daquelas que aí cumpriam medida de segurança, estas vidas que não se contam. Além disso, foram construídos dispositivos como oficinas de arte aos internos, exposição itinerante das obras resultantes, rodas de conversa com trabalhadores da rede de saúde mental e um filme-documentário que favorecessem a conexão de atores e elementos diversos para funcionar como máquinas de fazer ver e falar o dispositivo medida de segurança, engendrando novas sensibilidades e forjando vetores estéticos de desinstitucionalização. A co-organização de um Encontro entre diversos atores da justiça e gestores do executivo do Estado nos permitiu ainda acompanhar a configuração de um Programa de desinstitucionalização e nos aproximou da dimensão jurídica-política da Reforma Psiquiátrica quando problematizamos o modelo de responsabilização da pessoa com transtorno mental em conflito com a lei

It gives an account of the Federal Government intervention performed by the Ministry of Health in that hospital that took place in 2005 and makes some reflections about the role of the mental institutions in the contemporary society. We argue that nowadays psychiatric hospitals do not operate based on a disciplinary logic, that supported the development of the asylums for many years, but under a new logic of abandoning, tuned with the new forms of deregulation that characterize the new standards of sociability of the present times. The theoretic framework has contributions from Franco Basaglia and collaborators, in the perspective of practical-theoretical deinstitutionalization, and from Michel Foucault s works on the psychiatric hospital genealogy and the relation between power and knowledge. Also there are, although fragmentary and punctual, references to many others authors and concepts within which we highlight the Edgar Morin s concept of Complexity, that warn us to not simplify neither the madness phenomenon, nor the responds given to it. The body of the work is compounded by a logbook, written by the time of the events, and reflections related to it. The stake here is that demonstrating this new logic of abandoning of the mental institutions, this account may contribute to the practice and theory of the current psychiatric reform in Brazil / O objeto desta dissertação é a experiência de transformação de um hospital psiquiátrico localizado numa cidade do interior nordestino. Seu objetivo é relatar a experiência da intervenção federal que lá se deu em 2005 e tecer reflexões relativas ao papel do manicômio na sociedade contemporânea. Partiu-se da hipótese de que o hospital psiquiátrico atual funciona nem tanto a partir de uma lógica disciplinar, que acompanhou durante anos o desenvolvimento do asilo, mas de uma nova lógica do abandono, em sintonia com as novas formas de desregulamentação que caracterizam os novos padrões de sociabilidade dos tempos atuais. Seu estofo teórico são as contribuições de Franco Basaglia e colaboradores, na perspectiva teórico-prática da desinstitucionalização, e a obra de Michael Foucault, no que se refere à genealogia do hospital psiquiátrico e às relações saber/poder. Estão presentes também, ainda que de forma fragmentada e pontual, vários outros autores e conceitos. Dentre esses conceitos, destaca-se o de complexidade, presente nas idéias de Edgar Morin, que alerta para a necessidade de não simplificar o fenômeno da loucura nem as respostas dadas a ele. O corpo do trabalho é composto por um diário de bordo , escrito à época dos acontecimentos, e de reflexões teóricas a ele associadas. A aposta é que, ao demonstrar na prática o significado dessa nova lógica do abandono, este relato possa também contribuir para a prática e a teoria da reforma psiquiátrica em andamento no Brasil
